Abstract
The invention relates to an implant which consists of two end pieces abutting against the adjacent vertebrae and an intermediate middle piece joined to one of the end pieces by a thread, whereby the overall length of the implant can be adjusted by turning the middle piece. The end pieces form tubular sleeves with openings through their walls, and fit axially into each other, slide axially against each other along their sleeve walls, and are secured against rotating relative to each other around the sleeve axis. The thread is arranged between the middle piece and the end piece that is threaded to it. The middle piece which turns in the thread is designed as an axial stop for the other end piece.
Description
IMPLANT FOR FILLING A SPACE BETWEEN VERTEBRAE
The invention involves an implant for filling a space between vertebrae, consisting of two end pieces for abutting against the adjacent vertebrae and an intermediate piece joined to one of the end pieces by a screw thread.
The three pieces of the implant and the thread are arranged coaxially along the axis of the spinal column, and the overall length of the implant can be adjusted by turning the middle piece. The end pieces of the implant are in the shape of tubular sleeves perforated with holes through their walls.
Implants of this kind are known from German patent publication DE 44 23 257 A1, and are used to fill the space left by removing a vertebra or portions of a vertebra from the spinal column. These implants enable the distraction (drawing apart) of the area of the spinal column containing the implant, by turning the middle portion of the implant.
In implants of this kind, each of the end pieces is connected to the middle piece by its own screw thread, forming a tubular sleeve, arranged so that both threads run coaxially along the axis of the spinal column, and are threaded in opposite directions to each other. By means of the holes in the walls of the pieces it is possible, if desired, to introduce material, such as bone cement or bone fragments, into the interior space of the tubular sleeve, so that the implant can be bonded in place more quickly.
Such an implant is well suited to inducing bone formation and stimulating the bone, and after implantation it can grow-in easily and quickly. In such implants, however, the overall axial length is at least as great as the overall length of the two opposite-running screw threads, a factor that imposes a limit on how short the implant can be.
These known implants, then, are not suitable as vertebrae spacers to replace an intervertebral disk.
The purpose of the invention is to provide an implant of the kind described that can be adjusted to a minimum length so that, besides being usable as a full replacement for a vertebra, particularly in the area of the neck vertebrae, it can also be used to take the place of an intervertebral disk, especially in the lumbar region.
According to the invention, there is provided a spinal implant selectively insertable as a vertebral replacement and as an insert between vertebrae, said implant comprising: an inner implant part having a tubular perforated sleeve and formed with a screw thread; an outer implant part having a tubular perforated sleeve and receiving said sleeve of said inner implant part, said sleeve of said inner implant part being axially slidable in said sleeve of said outer implant part; a center implant part braced on an end of said outer implant part and threadedly engaged with the screw thread on said sleeve of said inner implant part for altering a length of said implant, said parts and said screw thread being coaxial;
and means for preventing relative rotation of said inner and outer implant parts about a common axis of said sleeves.
Because the implant according to the invention has only one screw thread, the minimum length to which the implant can be reduced is determined solely by the length of this one thread, since the two end pieces can be screwed into each other almost completely along the entire length of their tubular sleeves. This allows for a very low axial profile and therefore permits the implant to be used as a disk replacement. During an operation, full distraction of the implant is still possible, since the middle piece has only to be turned in the thread in order to adjust the length of the overall implant, as desired.
One embodiment of the implant according to the invention that is preferred for its especially simple manufacture, assembly and handling is characterized in that the sleeve of the end piece that is threaded to the middle piece has its thread on its outer circumferential surface, and is radially gripped both by the middle piece and by the sleeve of the other end piece. The middle piece of the implant is thereby readily accessible for turning, and the thread of the end piece that guides this middle piece grips the other end piece axially on the outside.
An arrangement is also recommended whereby an axially running guide slot is provided in the sleeve wall of one end piece of the implant, and on the sleeve wall of the other piece there is a projection that fits into the guide slot, to prevent the two pieces from turning relative to each other. Several guide slots and projections may be arranged around the circumference of the sleeve walls. In order to be able to fix the reciprocal positioning of the two end pieces independently of the stop function of the middle piece, another advantageous embodiment of the invention is characterized in that the sleeve of one of the end pieces has at least one radially bored, threaded opening in the area where the sleeves of the two end pieces overlap, into which opening a set screw can be inserted and tightened against the sleeve of the other end piece.

In a further preferred embodiment either one or both of the end pieces can be equipped with a ring-shape face plate at the end further from the middle piece, and this face plate has a number of cutting blades or points for penetrating into the adjacent vertebra. With respect to the design and arrangement of the openings in the sleeve walls of the end pieces the invention allows for a certain freedom of choice. On one hand the total surface area represented by the openings should be as large as possible, on the other the openings should not unduly weaken the threaded area, so as to ensure that the stability of the threaded binding between the end piece and the middle piece is not compromised. In general, the holes are made as circular or elongated openings, and distributed evenly around the circumference of the sleeve. It is advantageous to make further openings distributed around the outer circumferential surface of the middle piece of the implant, designed as keyholes through which a key can be inserted to turn the middle piece.

The implant as shown in the drawings is used as an insert to fill a space between two vertebrae (not shown).
Depending on the length to which the implant is adjusted, it can serve as a disk replacement or to take the place of a vertebra or portion of a vertebra that has been removed from the spinal column. The implant consists of two end pieces 1, 2 that abut against the respectively adjacent vertebra, and a middle piece 3 located between them. The middle piece 3 is connected by a thread 4 to one of the end pieces (in the illustration, to the upper end piece 1).
The three pieces 1 to 3, and the thread 4, are coaxially arranged in the longitudinal direction of the spinal column. By turning the middle piece 3 in the thread 4, the overall length of the implant can be adjusted within the limits as shown in Figures 1 and 2.
The end pieces 1, 2 are in the shape of tubular sleeves 5', 5", with walls that are pierced by openings 6.
These openings 6 make it possible for the implant to grow quickly into the surrounding tissue. The two end pieces 1, 2 grip each other axially with their sleeves 5', 5". The end pieces can be slid axially against each other along their sleeve walls, and are secured against rotating relative to each other around the sleeve axis, in a manner to be described below. The thread 4 between the middle piece 3 and the end piece (in the illustration, the upper piece 1) that is threaded to it is arranged on the sleeve 5' of that end piece. The middle piece 3, which turns in the thread 4 of this sleeve 5', is designed as an axial stop for the sleeve 5" of the other end piece (in the illustration, the lower one 2). By turning the middle piece 3 in the thread 4 of the upper end piece 1, the axial position of the middle piece 3 against the upper piece 1 is changed, so that the axial positions of the two end pieces 1 and 2 change relative to each other, since the position of the end piece 2 (the lower one in the illustration) relative to the end piece 1 (the upper one in the illustration) is determined by the striking of its sleeve 5" against the middle piece 3. The sleeve of the end piece that is threaded to the middle piece, i.e. the upper end piece 1, carries the thread 4 on its outer circumferential surface. This thread 4 is engaged by the mother thread 4' of the middle piece 3, which surrounds the sleeve 5' radially. The sleeve 5' of the end piece (the upper piece 1 in the illustration) that is threaded to the middle piece 3, is radially grasped on the outside by the sleeve 5" of the other end piece 2, as well as by the middle piece 3.
To prevent the two end pieces l, 2 from turning relative to each other the sleeve wall of the upper end piece 1, as shown in the illustration, is provided with an axially running guide slot 7, while the sleeve wall of the other, lower end piece, as shown in the illustration, has a projection 8 that fits into the guide slot 7. The projection 8 is formed by a pin 8' set radially into the sleeve wall. The length of the guide slot 7 determines the degree to which the two end pieces 1, 2 can be slid axially against each other. The sleeve 5" of the lower end piece 2 has in addition at least one radially bored, threaded hole 9 in the area where it overlaps with the sleeve 5' of the upper end piece 1, through which a set screw 10 (shown only _ 7 in Figures 1 and 2) is inserted and can be tightened against the sleeve 5' of the upper end piece 1. When the set screw is tightened in this way, the end pieces 1, 2 cannot be moved axially away from each other. The upper S end piece 1 is also equipped, at the end further removed from the middle piece 3, with a ring-shaped face plate 11, projecting radially outwards across the thread diameter, and which is supplied on its outer face with cutting blades or points 12' for penetrating into the adjacent vertebra.
Corresponding blades or points 12" are provided on the axially opposite face of the sleeve rim of the lower end piece 2. The circumferential surface of the middle piece 3 is perforated by a number of openings 13, arranged around the circumference, that serve as keyholes through which a key (not shown) can be inserted to turn the middle piece 3.

The implant as shown in the drawings is used as an insert to fill a space between two vertebrae (not shown).
Depending on the length to which the implant is adjusted, it can serve as a disk replacement or to take the place of a vertebra or portion of a vertebra that has been removed from the spinal column. The implant consists of two end pieces 1, 2 that abut against the respectively adjacent vertebra, and a middle piece 3 located between them. The middle piece 3 is connected by a thread 4 to one of the end pieces (in the illustration, to the upper end piece 1).
The three pieces 1 to 3, and the thread 4, are coaxially arranged in the longitudinal direction of the spinal column. By turning the middle piece 3 in the thread 4, the overall length of the implant can be adjusted within the limits as shown in Figures 1 and 2.
The end pieces 1, 2 are in the shape of tubular sleeves 5', 5", with walls that are pierced by openings 6.
These openings 6 make it possible for the implant to grow quickly into the surrounding tissue. The two end pieces 1, 2 grip each other axially with their sleeves 5', 5". The end pieces can be slid axially against each other along their sleeve walls, and are secured against rotating relative to each other around the sleeve axis, in a manner to be described below. The thread 4 between the middle piece 3 and the end piece (in the illustration, the upper piece 1) that is threaded to it is arranged on the sleeve 5' of that end piece. The middle piece 3, which turns in the thread 4 of this sleeve 5', is designed as an axial stop for the sleeve 5" of the other end piece (in the illustration, the lower one 2). By turning the middle piece 3 in the thread 4 of the upper end piece 1, the axial position of the middle piece 3 against the upper piece 1 is changed, so that the axial positions of the two end pieces 1 and 2 change relative to each other, since the position of the end piece 2 (the lower one in the illustration) relative to the end piece 1 (the upper one in the illustration) is determined by the striking of its sleeve 5" against the middle piece 3. The sleeve of the end piece that is threaded to the middle piece, i.e. the upper end piece 1, carries the thread 4 on its outer circumferential surface. This thread 4 is engaged by the mother thread 4' of the middle piece 3, which surrounds the sleeve 5' radially. The sleeve 5' of the end piece (the upper piece 1 in the illustration) that is threaded to the middle piece 3, is radially grasped on the outside by the sleeve 5" of the other end piece 2, as well as by the middle piece 3.
To prevent the two end pieces l, 2 from turning relative to each other the sleeve wall of the upper end piece 1, as shown in the illustration, is provided with an axially running guide slot 7, while the sleeve wall of the other, lower end piece, as shown in the illustration, has a projection 8 that fits into the guide slot 7. The projection 8 is formed by a pin 8' set radially into the sleeve wall. The length of the guide slot 7 determines the degree to which the two end pieces 1, 2 can be slid axially against each other. The sleeve 5" of the lower end piece 2 has in addition at least one radially bored, threaded hole 9 in the area where it overlaps with the sleeve 5' of the upper end piece 1, through which a set screw 10 (shown only _ 7 in Figures 1 and 2) is inserted and can be tightened against the sleeve 5' of the upper end piece 1. When the set screw is tightened in this way, the end pieces 1, 2 cannot be moved axially away from each other. The upper S end piece 1 is also equipped, at the end further removed from the middle piece 3, with a ring-shaped face plate 11, projecting radially outwards across the thread diameter, and which is supplied on its outer face with cutting blades or points 12' for penetrating into the adjacent vertebra.

Claims (6)
1. ~A spinal implant selectively insertable as a vertebral replacement and as an insert between vertebrae, said implant comprising:
an inner implant part having a tubular perforated sleeve and formed with a screw thread;
an outer implant part having a tubular perforated sleeve and receiving said sleeve of said inner implant part, said sleeve of said inner implant part being axially slidable in said sleeve of said outer implant part;
a center implant part braced on an end of said outer implant part and threadedly engaged with the screw thread on said sleeve of said inner implant part for altering a length of said implant, said parts and said screw thread being coaxial; and means for preventing relative rotation of said inner and outer implant parts about a common axis of said sleeves.

2. ~The spinal implant defined in claim 1 wherein said sleeve of said inner implant part is formed with said screw thread along its external periphery and is surrounded by said center implant part and by the sleeve of said outer implant part.
3. ~The spinal implant defined in claim 1 wherein said means for preventing relative rotation of said inner and outer implant parts about said common axis of said sleeve includes an axially-extendable slot in one of said inner and outer implant parts, a guide projection on the other of said inner and outer implant parts engaged in said slot.
4. ~The spinal implant defined in claim 1 wherein one of said sleeves has a radial threaded bore receiving a screw braced against the other of said sleeves.
5. ~The spinal implant defined in claim 1 wherein one of said inner and outer implant parts is provided at an end turned away from the center implant part with an annular front plate provided with means for penetrating into an adjacent vertebrae.
6. ~The spinal implant defined in claim 1 wherein said~
