Abstract

A composition for transdermal administration resulting from an admixture includes: a therapeutically effective amount of a drug that includes a parent drug and a prodrug; and a pharmaceutically acceptable carrier, wherein the parent drug and prodrug are individually present in an amount sufficient for a pharmacological effect. In a preferred embodiment, the admixture includes: a therapeutically effective amount of a pharmaceutically active agent that includes a corresponding steroid and a steroid derivative; and a carrier for the pharmaceutically active agent. The steroid and the corresponding steroid derivative are present in a weight ratio of 10:1 to 1:10 steroid:
corresponding steroid derivative. In a preferred embodiment ratio is 6:1 to 1:6. In a preferred embodiment, the corresponding steroid derivative is a steroid ester. In another preferred embodiment, the carrier is a polymer that includes a pressure-sensitive adhesive. In another preferred embodiment, the parent drug is an ACE inhibitor such as ramipril and the prodrug is an ACE
inhibitor prodrug such as ramipril ethyl and/or methyl ester.

Claims (52)

1. A composition for the transdermal delivery of a drug resulting from an admixture comprising:
a therapeutically effective amount of a drug that includes a parent drug and a prodrug; and and a pharmaceutically acceptable carrier, wherein the parent drug and prodrug are individually present in an amount sufficient for a pharmacological effect.
2. A composition as claimed in claim 1, wherein the composition has an onset of the therapeutic effect that is longer or shorter than an identical composition having a pharmacologically equivalent amount of the parent drug or prodrug alone.
3. A composition as claimed in claim 1, wherein the composition has a blood level profile that is different from an identical composition having a pharmacologically equivalent amount of the parent drug or prodrug alone.
4. A composition as claimed in claim 1, wherein the composition has a permeation rate that is faster or slower than an identical composition having a pharmacologically equivalent amount of the parent drug or prodrug alone.
5. A composition as claimed in claim 1, wherein the composition has a duration of the therapeutic effect that is longer or shorter than a composition having a pharmacologically equivalent amount of the parent drug alone.
6. A composition as claimed in claim 1, wherein the prodrug is more lipophilic than the parent drug and the prodrug has a greater permeation rate through the skin.
7. A composition as claimed in claim 1, wherein the parent drug is a steroid and the prodrug is the steroid where the free hydroxy at the 3, 11 or 17 position has been reacted with an alcohol moiety.
8. A composition as claimed in claim 1, wherein the parent drug is a steroid and the prodrug is the steroid where the free hydroxy at the 17 position has been reacted with an alcohol moiety.
9. A composition as claimed in claim 1, wherein the melting point of the combined parent drug and prodrug is less than the melting point of either the parent drug or prodrug alone.
10. A composition as claimed in claim 9, wherein the parent drug is a steroid and the prodrug is the steroid where the free hydroxy at the 3, 11 or 17 position has been reacted with an alcohol moiety.
11. A composition as claimed in claim 9, wherein the parent drug is norethindrone and the prodrug is norethindrone acetate.
12. A composition as claimed in claim 11, wherein the weight ratio of the norethindrone acetate:norethindrone is >= 1.
13. A composition as claimed in claim 9, wherein the parent drug is estradiol and the prodrug is estradiol 17-enanthate.
14. A composition as claimed in claim 1, wherein the carrier comprises a pressure-sensitive adhesive that includes two or more polymers, and wherein the permeation of the drug is adjusted by changing the type and/or proportions of the two or more polymers.
15. A method of adjusting the blood level profile or permeation of a drug from a transdermal drug delivery composition, comprising:
(a) selecting a parent drug and a prodrug;
(b) combining the parent drug and prodrug in carrier that includes at least one pressure-sensitive adhesive;
(c) measuring the permeation and/or blood level profile of the drug;
(d) adjusting at least one of: (i) the selection of the prodrug; (ii) the relative concentration of the parent drug and prodrug; and (iii) the total concentration of the parent drug and prodrug.
16. A method according to claim 15, wherein the at least one pressure sensitive adhesive includes two pressure sensitive adhesives and the step of adjusting (d) further comprises: (iv) adjusting the type and/or proportions of the two pressure sensitive adhesives.
17. A composition for the transdermal delivery of a drug resulting from an admixture comprising:
a therapeutically effective amount of a drug that includes a steroid and a corresponding steroid derivative that provides a source of therapeutically active steroid; and and a pharmaceutically acceptable carrier, wherein the steroid and the corresponding steroid derivative are present in a weight/weight ratio of 10:1 to 1:10 steroid:
corresponding steroid derivative.
18. A composition according to claim 17, wherein the ratio is 6:1 to 1:6.
19. A composition according to claim 17, wherein the corresponding steroid derivative is a steroid ester.
20. A composition according to claim 17, wherein the carrier is a flexible, finite composition that includes at least one polymer.
21. A composition according to claim 20, wherein the flexible, finite composition comprises at least one of an acrylic-based polymer and a silicone-based polymer.
22. A carrier composition according to claims 20 or 21, wherein the flexible, finite composition is a pressure-sensitive adhesive.
23. A composition according to claim 17, wherein the composition further comprises an enhancer.
24. A composition according to claim 23, wherein the enhancer comprises dipropylene glycol and one or more of oleyl alcohol or oleic acid.
25. A composition according to claim 20, wherein the steroid and the corresponding steroid derivative are present in an amount of up to 20 wt % , based on the total composition.
26. A composition according to claim 20, wherein the steroid and the corresponding steroid derivative are present in an amount of up to 10 wt %, based on the total composition.
27. A composition according to claim 19, wherein the steroid comprises one or more progestins.
28. A composition according to claim 27, wherein the progestin comprises allylestrenol, anagestone, desogestrel, dimethisterone, dydrogesterone, ethisterone, ethynodiol, gestodene, haloprogesterone, 17-hydroxy-16-methylene-progesterone, l7.alpha.-hydroxyprogesterone, lynestrenol, medroxyprogesterone, melengestrol, norethindrone, norethynodrel, norgesterone, norgestimate, norgestrel, norgestrienone, norvinisterone, pentagestrone, and trimigestone.
29. A composition according to claim 27, wherein the progestin is norethindrone and the corresponding steroid ester is norethindrone acetate.
30. A composition according to claim 17, wherein the ratio is 1:1 to 3:1.
31. A composition according to claim 17, wherein the ratio is 3:2 to 5:2.
32. A composition according to claim 17, wherein the ratio is about 2:1.
33. A composition according to claim 19, wherein the steroid comprises one or more anabolic steroids and androgenic steroids.
34. A composition according to claim 33, wherein the anabolic steroid comprises androisoxazole, androstenediol, bolandiol, bolasterone, clostebol, ethylestrenol.

A composition according to claim 33, wherein the androgenic steroid comprises boldenone, fluoxymesterone, mestanolone, mesterolone, methandrostenolone, 17-methyltestosterone, 17.alpha.-methyltestosterone 3-cyclopentyl enol ether, norethandrolone, normethandrone, oxandrolone, oxymesterone, oxymetholone, prasterone, stanlolone, stanozolol, testosterone, testosterone 17-chloral hemiacetal and tiomesterone.
36 36. A composition according to claim 19, wherein the composition further comprises an additional steroid.
37. A composition according to claim 36, wherein the additional steroid is an estrogen.
38. A composition according to claim 37, wherein the estrogen is selected from the group consisting of: conjugated estrogenic hormones, equilenin, equilin, estradiol, estradiol benzoate, estradiol 17.beta.-cypionate, estriol, estrone, ethinyl estradiol and mixtures thereof.
39. A composition according to claim 37 wherein the steroid is norethindrone, the corresponding steroid ester is norethindrone acetate and the estrogen is estradiol.
40. A composition according to claim 39, wherein the carrier is a pressure-sensitive adhesive, comprising at least one of an acrylic-based polymer and a silicone-based polymer, and the composition further comprises an enhancer and a crystal inhibiting agent.
41. A composition according to claim 40, wherein the enhancer includes dipropylene glycol and olylalcohol, and the crystal inhibiting agent includes PVP.
42. A method for enhancing the transdermal delivery of a drug, comprising administering the drug in a pharmaceutically acceptable carrier that comprises a combination of:

a therapeutically effective amount of at least one steroid; and a corresponding steroid derivative that provides a source of therapeutically active steroid, wherein the steroid and the corresponding steroid derivative are present in a weight/weight ratio of 10:1 to 1:10 steroid: corresponding steroid derivative.
43. A method of suppressing crystal formation of a steroid in a transdermal drug delivery system comprising the step of combining two or more corresponding derivatives of the steroid in a pressure-sensitive adhesive carrier composition.
44. A method of making a dermal composition according to claim 19, comprising forming a mixture of the steroid, steroid ester and carrier.
45. A composition for the transdermal delivery of a drug resulting from an admixture comprising:

a therapeutically effective amount of a drug that includes norethindrone and norethindrone acetate; and a pharmaceutically acceptable carrier, wherein the norethindrone and norethindrone acetate steroid are present in a weight/weight ratio of 1:6 to 6:1.
46. A composition for the transdermal delivery of a drug resulting from an admixture comprising:

a therapeutically effective amount of a drug that includes norethindrone and norethindrone acetate and estradiol; and and a pharmaceutically acceptable carrier, wherein the norethindrone and norethindrone acetate are present in a weight/weight ratio of 1:1 to 1:3.
47. A composition for the transdermal delivery of a drug formed by a process comprising mixing: a therapeutically effective amount of a drug that includes a steroid and a corresponding steroid derivative that provides a source of therapeutically active steroid;
and a pharmaceutically acceptable carrier, wherein the steroid and the corresponding steroid derivative are present in a weight/weight ratio of 10:1 to 1:10 steroid:
corresponding steroid derivative.
48. A composition for the transdermal delivery of a drug resulting from an admixture comprising:

a therapeutically effective amount of a drug that includes an ACE inhibitor and a corresponding ACE inhibitor prodrug; and and a pharmaceutically acceptable carrier.
49. A composition according to claim 48, wherein the ACE inhibitor is ramipril and the corresponding ACE inhibitor includes one or more of ramipril ethyl ester and ramipril methyl ester.
50. A composition according to claim 49, wherein the weight ratio of the ramipril:ramipril ethyl ester is 1:1 to 1:5.
51. A composition according to claim 49, wherein the weight ratio of the ramipril:ramipril methyl ester is 2:1 to 1:9.
52. A composition according to claim 48, wherein the ACE inhibitor and the ACE inhibitor prodrug are individually present in an amount sufficient for a pharmacological effect.
