Abstract
An artificial intervertebral disc includes housing members including spaced inner surfaces facing each other and oppositely facing outer surfaces for engaging spaced apart vertebral surfaces. Bearing surfaces extend from each of the inner surfaces for engaging each other while allowing for low friction and compression resistant movement of the housing members relative to each other while under compression. Load sharing pads are disposed between the inner surfaces and about at least a portion of the bearing surfaces for sharing absorption compressive loads with the bearing surfaces while limiting the relative movement of the housing members.
Claims (51)
1. An artificial intervertebral disc comprising:
housing members including spaced inner surfaces facing each other and oppositely facing outer surfaces for engaging spaced apart intervertebral surfaces;
bearing means extending from each of said inner surfaces for engaging each other while allowing for low friction and compression resistance relative to each other while under compression; and load sharing means disposed between said inner surfaces and about at least a portion of said bearing means for sharing absorption of compressive loads with said bearing means while limiting the relative movement of said housing members.

bearing means extending from each of said inner surfaces for engaging each other while allowing for low friction and compression resistance relative to each other while under compression; and load sharing means disposed between said inner surfaces and about at least a portion of said bearing means for sharing absorption of compressive loads with said bearing means while limiting the relative movement of said housing members.

A disc of claim 1, wherein said bearing means includes a concave surface portion extending from one of said inner surfaces and a convex surface portion extending from the other of said inner surfaces and seated against said concave surface portion for sliding movement relative thereto effectively resulting in relative pivoting motion of said housing members which can compress at least a portion of said load sharing means while distending at least a portion of said load bearing means.

A disc according to claim 2, wherein said load sharing means are elastic for allowing relative twisting movement between said housing members effecting relative three dimensional movement between said housing members while limiting said movement and preventing contact between said housing members except for said bearing means.
4. A disc according to claim 3, including a pair of disc members, each of said disc members including one of said bearing means, said inner surfaces of said housing members including disc seating means for seating one of said disc members therein in opposing relationship of the other of said disc members seated in said seating means of the other of said housing members.
5. A disc according to claim 4, wherein said seating means includes a pocket recessed into each of said inner surfaces of said housing members.
6. A disc according to claim 5, including locking means for selectively preventing relative movement of said disc member and said housing member once said disc member is seated in said pocket.
7. A disc according to claim 6, wherein said locking means includes a press fit taper in a wall of said pocket.
8. A disc according to claim 6, wherein said locking means includes a retaining ring operatively connected to said pocket.
9. A disc according to claim 6, wherein said locking means includes at least one undercut groove disposed about said pocket and said disc member includes at least one radially outwardly extending flange for locking engagement within said undercut groove.
10. A disc according to claim 9 wherein said flange includes at least one radially outwardly extending bayonet portion.
11. A disc according to clam 6 wherein said locking means includes at least one undercut groove disposed about a radial periphery of said disc member and said pocket of said housing member includes at least one radially inwardly extending flange for locking engagement within said undercut groove.
12. A disc according to claim 5, including a ramped surface extending from an edge of said inner surface of at least one of said housing members to said pocket allowing for lateral insertion of said disc members into said pockets when said housing members are disposed in spaced relationship to each other in an intervertebral space.
13. A disc according to claim 5 further defined as said housing members being an upper member and a lower member for relative positioning between vertebrae, said upper member including a relatively larger one of said pockets for seating a relatively larger one of said disc members having a concave surface and said lower member including a relatively smaller pocket for seating a relatively smaller one of said discs thereby providing an increased range of motion at the bearing surface interface.
14. A disc according to claim 4, wherein said disc members are made from a composition selected from the group including ceramics, plastics, and metal bearing materials.
15. A disc according to claim 1, wherein said load sharing means includes at least one pad member disposed and retained between said inner surfaces of said housing members and having cushioning and elastic properties for countering and thereby self centering against forces caused by relative movement of said housing member while under compressive forces are applied to said outer surfaces of said housing members.
16. A disc according to claim 15, wherein said housing members include seating means for seating said pad members between said inner surfaces.
17. A disc according to claim 16, wherein said seating means includes at least one pocket recessed into said inner surface of said housing member for seating a portion of said pad member therein.
18. A disc according to claim 17, including adhering means for fixedly adhering said pad member within said pocket.
19. A disc according to claim 17, wherein said pocket includes at least one undercut recess about at least a portion thereof and said pad members include at least one outwardly extending flange portion for being seated within said recess to lock said pad members in a desired position relative to said housing member.
20. A disc according to claim 17 wherein said pad member includes at least one peripherally located recess about an edge portion thereof and said pocket includes at least one inwardly extending flange portion including at least one inwardly extending flange portion for being seated within said recess to lock said pad member in a desired position relative to said housing member.
21. A disc according to claim 15, wherein said pad members are selected from a composition having a durometer in the range of 20 to 98 on the Shore OO

22. A disc according to claim 15, wherein said pad members are selected from a composition having a durometer of 10 to 100 on the Shore A Scale.
23. A disc according to claim 15, wherein said pad members are selected from a composition having a durometer of 22 to 75 on the Shore D Scale.
24. A disc according to claim 15, wherein the pads are made from a composition selected from the group including polymers and elastomers.
25. A disc according to claim 24, wherein the pads are made from a composition selected from the group including silicone, polyurethane, and urethane composites, plastics, polymers, and elastomers.
26. A disc according to claim 1, wherein said housing members are constructed from a composition selected from the group including metals, ceramics, and plastics.
27. A disc according to claim 1, wherein said outer surface of said housing member includes a surface texture for accepting bone growth therein.
28. A disc according to claim 27, wherein said surface texture is selected from the group including physically roughened, porous coated, and plasma coated surfaces.
29. A disc according to claim 27 further including bone growth inducing means disposed on said outer surface of said housing member for increasing bone growth on the said outer surface.
30. A disc according to claim 27, wherein said bone growth inducing means is a coating selected from the group including hyroxyapatite and bone morphogenic proteins.
31. A disc according to claim 1, wherein said load sharing means consists of a unitary pad member disposed about said bearing means and separating substantially all of the reminder of said inner surface.
32. A disc according to claim 1, wherein said load sharing means includes at least two pad members disposed peripherally relative to said bearing means and between said inner surfaces.
33. A disc according to claim 1, including at least one flange extending from said outer surface for fixing said housing members to a vertebral surface.
34. A disc according to claim 1, wherein at least one of said outer surfaces includes a recessed portion.
35. A disc according to claim 34 further including a porous coating disposed within said recess.
36. A method of assembling an artificial intervertebral disc in vivo by:
inserting upper and lower housing members into an intervertebral space;
disposing cushioning pads between inner surfaces of the housing members, placing the pads in compression; and inserting a pair of disc members between the inner surfaces of the plates, the disc members having abutting low friction surfaces therebetween the disc members being effectively surrounded by the pads whereby the disc members and pads share compression forces.

disposing cushioning pads between inner surfaces of the housing members, placing the pads in compression; and inserting a pair of disc members between the inner surfaces of the plates, the disc members having abutting low friction surfaces therebetween the disc members being effectively surrounded by the pads whereby the disc members and pads share compression forces.
37. A method according to claim 36 further including the steps of aligning the housing members relative to each other by adjusting the relative height of the cushioning pads to maintain spinal curvatures.
38. A method according to claim 36 further including the steps of creating a no load condition on the cushioning pads by adjusting the relative heights of the cushioning pads.
39. A method according to claim 36 further including the steps of adjusting the relative range of motion of the housing members by adjusting for relative elasticity of the cushioning pad members.
40. A method of separating opposing vertebrae at an intervertebral space by:
engaging artificial bearing surfaces between the intervertebral surfaces while allowing low friction and compression resistant movement of the bearing surfaces relative to each other;
sharing absorption of the compressive forces with at least one load bearing pad disposed about at least a portion of the bearing surfaces; and limiting the relative movement of the bearing surfaces.

41. A method of artificially spacing apart intervertebral surfaces by:
disposing housing members separated by bearing surfaces surrounded by pad members between the intervertebral spaces; and sharing compressive loads between the bearing surfaces and pad members.

42. The method as set forth in claim 41 further including the steps of limiting compression of the pad members by the bearing surfaces being noncompressible and limiting motion of the bearing surfaces by the pad members having limited elasticity.
43. The method as set forth in claim 42 further including steps of adjusting the elasticity of the pad members intraoperatively to adjust the extent movement of the bearing surfaces.
44. An artificial intervertebral disc comprising;
noncompressible bearing surface means for allowing motion between two bearing surfaces while limiting compression thereof; and compression absorbing means for sharing compressive forces with said bearing surface means while limiting motion of said bearing surface means.

45. A disc member including said bearing means as set forth in claim 1.
46. A pad member including said load sharing means as set forth in claim 1.
47. A method of assembling an artificial intervertebral disc in situ by:
inserting a pair of housing members connected together by a pair of load sharing pads into an intervertebral space and then inserting a pair of bearing surfaces between the housing members and between the load sharing pads whereby the disc members and pads share compression forces.
inserting a pair of housing members connected together by a pair of load sharing pads into an intervertebral space and then inserting a pair of bearing surfaces between the housing members and between the load sharing pads whereby the disc members and pads share compression forces.
48. A method of artificially spacing apart opposing intervertebral spaces by sharing compressive forces between bearing surfaces and load sharing pad members.
49. A load sharing pad member comprising a body portion and at least one peripheral flange portion for engaging recess in a portion of a vertebral disc housing member.
50. A load sharing pad member comprising a body portion and at least one peripheral recess portion for being engaged by a flange portion in a product of a vertebral disc housing member.
51. A vertebral disc housing member including one centrally located recess portion and at least one other recesses portion disposed peripherally relative to said centrally located recess portion.
