Abstract
A collapsible medical device and associated methods of occluding an abnormal opening in, for example, a body organ, wherein the medical device is shaped from plural layers of a heat-treatable metal fabric. Each of the fabric layers is formed from a plurality of metal strands and the assembly is heat-treated within a mold in order to substantially set a desired shape of the device. By incorporating plural layers in the thus-formed medical device, the ability of the device to rapidly occlude an abnormal opening in a body organ is significantly improved.
Claims (83)
1. A collapsible medical device comprising an outer metal fabric surrounding an inner metal fabric, said outer and inner metal fabrics each having a plurality of braided metal strands with an expanded preset configuration and proximal and distal ends, said proximal and distal ends having means for securing the plurality of braided strands comprising the inner and outer metal fabrics together, the medical device being shaped to create an occlusion of an abnormal opening in a vascular organ, said expanded preset configuration being deformable to a lesser cross-sectional dimension for delivery through a channel in a patient's body the outer and inner metal fabrics having a memory property such that the medical device tends to return to said expanded preset configuration when unconstrained.
2. The medical device of claim 1 wherein the pitch of the braided metal strands comprising the outer and inner metal fabrics is generally equal.
3. The medical device of claim 1 wherein the pitch of the braided metal strands comprising the outer and inner metal fabrics are not equal and the inner &

8. The medical device as in claim 1 wherein the number of braided metal strands comprising the outer metal fabric is 72 and the number of braided metal strands comprising the inner metal fabric is 144.

12. The medical device as in claim 1 wherein the diameter of the braided metal strands comprising the outer metal fabric is in a range from 0.001 to 0.003 inches and the diameter of the braided metal strands comprising the inner metal fabric is in a range of from 0.001 to 0.003 inches.

The medical device as in claim 1 and further including a third metal fabric disposed within the confines of the inner and outer metal fabrics, said third metal fabric comprising a plurality of braided metal strands with an expanded preset configuration corresponding to the expanded present configuration of the outer and inner metal fabrics, the plurality of braided metal strands of the third metal fabric having proximal and distal ends that are secured to the respective proximal and distal ends of the plurality of braided strands comprising the outer and inner metal fabrics.
14. The medical device as in claim 13 wherein the pitch of the braided metal strands comprising the third metal fabric is equal to the pitch of the braided metal strands comprising the outer and inner metal fabrics.
15. The medical device as in claim 13 wherein the diameter of the braided metal strands comprising the third metal fabric is equal to the diameter of the braided metal strands comprising the inner metal fabric.
16. The medical device as in claim 13 wherein the diameter of the braided metal strands comprising the third metal fabric is equal to the diameter of the braided metal strands comprising the outer metal fabric.
17. The medical device as in claim 13 wherein the number of braided metal strands comprising the third metal fabric is equal to the number of braided metal strands comprising the inner metal fabric.
18. The medical device as in claim 13 wherein the number of braided metal strands comprising the third metal fabric is equal to the number of braided metal strands comprising the outer metal fabric.

22. The medical device as in claim 1 wherein the means for securing the braided metal strands at the proximal and distal ends of the outer metal fabric is independent of the means for securing the braided metal strands at the proximal and distal ends of the inner metal fabric.
23. The medical device as in either claim 1 or 13 wherein the means for securing the braided metal strands at the proximal and distal ends of any one fabric are independent of the means for securing the braided metal strands of another fabric.

The medical device as in claim 1 wherein the means for securing the plurality of braided metal strands comprising the inner and outer fabrics together include first and second clamps joined individually to all of the strands of the outer and inner metal fabric at the proximal and distal ends thereof.

The medical device as in claim 13 wherein the means for securing the plurality of braided metal strands comprising the inner, middle and outer fabrics together include first and second clamps joined individually to all of the strands of the outer, middle and inner metal fabric at the proximal and distal ends thereof.
26. The medical device of claim 1 wherein the expanded preset configuration of the outer metal fabric is of a different geometrical shape than the expanded preset configuration of the inner metal fabric.
27. The medical device of claim 13 wherein the expanded preset configuration of the outer metal fabric is of a different geometrical shape than the expanded preset configuration of one of the middle and inner metal fabrics.
28. ~A collapsible medical device comprising an outer metal fabric surrounding an inner metal fabric, said outer and inner metal fabrics each having a plurality of braided metal strands with an expanded preset configuration and proximal and distal ends, said proximal and distal ends having means for securing together the plurality of braided strands comprising the inner and outer metal fabrics, the medical device being pre-shaped with a central cylindrical portion with end walls, said central cylindrical portion enclosed at each end by a coaxial disk member formed of back to back inner and outer walls, said disk members having an inner diameter portion perpendicular to the axis of the central cylindrical portion and a disk outer diameter portion which is angled toward the central cylindrical portion and being larger in diameter than the central cylindrical portion, said central cylindrical portion and disk members meeting at a reduced diameter that is small in comparison to the diameter of the cylindrical portion, to create an occlusion of an abnormal opening in a vascular organ, said expanded preset configuration being deformable to a lesser cross-sectional dimension for delivery through a channel in a patient's body the outer and inner metal fabrics having a memory property such that the medical device tends to return to said expanded preset configuration when unconstrained.

38. The medical device as in claim 28 wherein the diameter of the braided metal strands comprising the outer metal fabric is in a range from 0.003 to 0.008 inches and the diameter of the braided metal strands comprising the inner metal fabric is in a range of from 0.001 to 0.002 inches.
39. The medical device as in claim 28 wherein the diameter of the braided metal strands comprising the outer metal fabric is in a range from 0.001 to 0.003 inches and the diameter of the braided metal strands comprising the inner metal fabric is in a range of from 0.001 to 0.003 inches.
40. The medical device as in claim 28 and further including a third metal fabric disposed within the confines of the inner and outer metal fabrics, said third metal fabric comprising a plurality of braided metal strands with an expanded preset configuration corresponding to the expanded present configuration of the outer and inner metal fabrics, the plurality of braided metal strands of the third metal fabric having proximal and distal ends that are secured to the respective proximal and distal ends of the plurality of braided strands comprising the outer and inner metal fabrics.
41. The medical device as in claim 40 wherein the pitch of the braided metal strands comprising the third metal fabric is equal to the pitch of the braided metal strands comprising the outer and inner metal fabrics.
42. The medical device as in claim 40 wherein the diameter of the braided metal strands comprising the third metal fabric is equal to the diameter of the braided metal strands comprising the inner metal fabric.
43. The medical device as in claim 40 wherein the diameter of the braided metal strands comprising the third metal fabric is equal to the diameter of the braided metal strands comprising the outer metal fabric.
44. The medical device as in claim 40 wherein the number of braided metal strands comprising the third metal fabric is equal to the number of braided metal strands comprising the inner metal fabric.
45. The medical device as in claim 40 wherein the number of braided metal strands comprising the third metal fabric is equal to the number of braided metal strands comprising the outer metal fabric.
46. The medical device as in claim 40 wherein the number of braided metal strands comprising the third metal fabric is 144.
47. The medical device as in claim 40 wherein the number of braided metal strands comprising the third metal fabric is between 72 and 288.
48. The medical device as in claim 40 wherein the diameter of the braided metal strands comprising the third metal fabric is in a range of from about 0.001 and 0.002 inches.
49. The medical device as in claim 40 wherein the means for securing the braided metal strands at the proximal and distal ends of the outer metal fabric is independent of the means for securing the braided metal strands at the proximal and distal ends of the inner metal fabric.
50. The medical device as in claim 28 wherein the means for securing the braided metal strands at the proximal and distal ends of any one fabric are independent of the means for securing the braided metal strands of another fabric.
51. The medical device as in claim 40 wherein the means for securing the plurality of braided metal strands comprising the inner and outer fabrics together include first and second clamps joined individually to all of the strands of the outer and inner metal fabric at the proximal and distal ends thereof.
52. The medical device as in claim 40 wherein the means for securing the plurality of braided metal strands comprising the inner, middle and outer fabrics together include first and second clamps joined individually to all of the strands of the outer, middle and inner metal fabric at the proximal and distal ends thereof.
53. The medical device of claim 40 wherein the expanded preset configuration of the outer metal fabric is of a different geometrical shape than the expanded preset configuration of the inner metal fabric.
54. The medical device of claim 40 wherein the expanded preset configuration of the outer metal fabric is of a different geometrical shape than the expanded preset configuration of one of the middle and inner metal fabrics.
55. The medical device of claim 28 wherein the diameter of the disk members is at least 1.5 times greater than the diameter of the central cylindrical portion.
56. The medical device of claim 28 wherein the diameter of the central cylindrical portion is at least 2 to 6 times larger than the reduced diameter where the disk member and central cylindrical portion meet.
57. The medical device of claim 28 wherein the disk member outer diameter is at least 3 to 12 times larger than the reduced diameter where the disk member and central cylindrical portion meet.
58. The medical device of claim 28 wherein the reduced diameter where the disk member and central cylindrical portion meet is less than 2 mm.
59. The medical device of claim 28 wherein the disk outer diameter portion which is angled toward the central cylindrical portion has an angle of between 20-40 degrees with respect to a disk plane perpendicular to the axis.
60. The medical device of claim 28 wherein the angled disk outer diameter portion maximum diameter is at least twice a diameter of the inner disk maximum diameter portion.
61. The medical device of claim 28 wherein the disk members are mirror images of each other.
62. The medical device of claim 28 wherein the central cylindrical portion has a length that is from 2 to 8 times a distance between the disk members at their closest point.
63. The medical device of claim 28 wherein the metal fabric is comprised of strands of shape memory Nitinol wire.
64. The medical device of claim 28 wherein the fabric is a braided tubular member
65. The medical device of claim 28 wherein the device is pre-shaped by placing the outer and inner metal fabrics within a mold of a desired expanded preset configuration for the device so as to conform to inner walls of the mold and heating the mold to a predetermined temperature for a time sufficient to thermally set the braded metal strands to said expanded preset configuration.
66. The medical device as in claim 28 wherein the disk members at each end of the central cylindrical portion are generally parallel to each other and at a predetermined angle to a longitudinal axis of the central cylindrical portion.
67. The medical device as in claim 28 wherein the disk members at each end of the central cylindrical portion are non-parallel to one another and at a predetermined angle to a longitudinal axis of the central cylindrical portion.
68. The medical device of claim 28 wherein the coaxial disk member at one end of the central cylindrical portion is generally circular and the coaxial disk member at an end opposite the one end of the central cylindrical portion is of a first radius over a first predetermined arc and of a second radius over a second predetermined arc.
69. The medical device of claim 28 wherein the inner metal fabric is co-extensive with the outer metal fabric throughout the coaxial disk members and the cylindrical portion.
70. The medical device of claim 28 wherein the inner metal fabric and the outer metal fabric are coextensive only in the cylindrical portion.
71. The medical device of claim 70 wherein the means for securing together the plurality of braided metal strands comprising the inner and outer metal fabrics include separate clamps at the proximal and distal ends of each of the inner and outer metal fabrics.
72 The medical device of claim 70 wherein the means for securing together the plurality of braided metal strands comprising the inner and outer metal fabrics include a clamp common to one of the distal and proximal ends of the inner and outer fabrics and separate clamps at the outer of the distal and proximal ends of the inner and outer metal fabrics
73. The medical device of claim 71 and further including an elastic member joining the separate clamps at the proximal end of the inner metal fabric and the proximal end of the outer metal fabric.
74. The medical device as in claim 73 and further including an elastic member joining the separate clamps at the distal end of the inner metal fabric and the distal end of the outer metal fabric.
75. A collapsible medical device comprising a cylindrical central segment and distal and proximal end segments, each of said segments having a plurality of braided metal strands exhibiting an expanded, preset configuration where the distal and proximal end segments comprise disk members having diameters greater than that of the central segment, the disk members being integrally attached to opposed ends of the central segment by connecting segments of a diameter less than that of the central segment, the central segment surrounding an inner cylindrical layer of braided metal strands.
76. The medical device of claim 75 and further including means affixed to the distal and proximal end segments for preventing unraveling of the braided metal strands.
77. The medical device of claim 75 wherein the pitch of the braided metal strands comprising the cylindrical central segment and the inner cylindrical layer are of an unequal pitch.
78. The medical device of claim 75 and further including means for securing the cylindrical central segment to the inner cylindrical layer.
79. The medical device of claim 78 wherein the securing means comprises a suture tie.
80. The medical device of claim 76 and further including means separate from the means affixed to the distal and proximal end segments for preventing unraveling of the inner cylindrical layer of braided metal strands.
81. The medical device of claim 1 wherein the outer metal fabric surrounds a plurality of discrete structures of braided metal strand construction, the outer metal fabric having said expanded, preset configuration and said plurality of discrete structures substantially filling a volume defined by the expanded, present configuration.
82. The medical device of claim 1 wherein a peripheral surface of the outer metal fabric is in its expanded preset configuration conforms to anatomical structures with which the device is made to abut.
83. The medical device of claim 81 wherein the plurality of discrete structures of braided metal strand construction comprise a plurality of spherical members joined to one another in a chain-like configuration of a predetermined length.
