Abstract

A plaque tack device for treating atherosclerotic occlusive disease is formed as a thin, annular band (30, 80, 90, 100, 110) of durable, flexible material having a plurality of barbs or anchoring points (33, 34) on its outer periphery for preventing it from being dislodged. The plaque tack may be used with a balloon angioplasty procedure or as a de novo treatment for blood vessel blockage to reopen the vessel lumen for desired blood flow. It has a width that is small relative to its diameter, to minimize the amount of' foreign structure placed in the blood vessel. One or more tacks may be applied in positions along a plaque accumulation site as needed to stabilize the site and/or hold pieces of plaque out of the way of blood flow. The barbs of the tack may be pressed into the plaque and/or blood vessel walls by balloon expansion.
Related methods of deployment and delivery devices are provided for insertion of the plaque tack in a compressed state into the blood vessel and expanding it back to its annular shape for holding plaque against the blood vessel walls.

Claims (15)

1. A tack device for holding plaque against blood vessel walls in treating atherosclerotic occlusive disease, comprising:

a thin, annular band of durable, flexible material having a plurality of barbs on its outer annular periphery, said annular band being dimensioned and designed to be applied with an outward expansion force against the plaque to press and hold it against the blood vessel walls, wherein each of said barbs is formed with a head having a sharp point integrally formed on a support body, and is made of a sufficiently rigid material so that, when the outward expansion force is applied, they pierce the plaque and maintain a locking relationship with the blood vessel walls.
2. A tack device according to Claim 1, wherein the annular band has a width in the axial direction of the blood vessel walls that is less than its diameter in order to minimize the emplacement of foreign structure in the blood vessel.
3. A tack device according to Claim 1, wherein the annular band is designed to be compressed, folded, or plied to take up a smaller-diameter volume for insertion into the blood vessel, wherein it can be released to expand, unfold or unply to its full diametral size within the blood vessel walls.
4. A tack device according to Claim 1, formed as a thin ribbon having a row of cutout portions that form as barbs on an outward side along its longitudinal length.
5. A tack device according to Claim 1, formed as a folding ring tack having V-shaped segments alternating with inverted-V-shaped points, said V-shaped segments enabling said ring tack to be folded to a smaller-diameter volume for insertion into a blood vessel.

A tack device according to Claim 1, formed as a flexible ring of pliable material with an array of outer barbs on an outer side of the ring, and an array of inner radial fingers, wherein the array of inner radial fingers are used to displace the outer barbs to lie horizontally flat in one axial direction when the fingers are pushed in the opposite axial direction.

A tack device according to Claim 1, formed as an annular band in a coil shape with an array of outer barbs and with opposite ends thereof unjoined, wherein the opposite ends can be stretched in opposite directions axially to flatten the coil tack in a spiral to a smaller-diameter volume for insertion in the blood vessel.
8. A tack device according to Claim 1, wherein said barbs are formed with conduits for enabling plaque treatment material to be conducted to the surface of the barbs through capillary channels.
9. A tack delivery device for delivering a compressible annular tack into a blood vessel for emplacement against plaque to press and hold it against the blood vessel walls, comprising:

a hollow catheter delivery tube having delivery tube end, a fabric carrier slidable along an inner surface of catheter delivery tube on which the annular tack is loaded in a compressed form and is moved by the sliding of the fabric carrier to the delivery tube end, and is released from the tube when the fabric is pulled over an outside surface of the tube to deploy an annular tack into the blood vessel, wherein the compressed annular tack is released at the delivery tube end to expand back to its uncompressed size for emplacement to hold plaque against the blood vessel walls.
10. A tack delivery device for delivering a compressible annular tack into a blood vessel according to Claim 9, wherein the fabric carrier is slidable on an outer surface of the catheter delivery tube with the compressed annular tack loaded thereon, and a cover is sleeved over the catheter delivery tube to hold the annular tack in its compressed state on the fabric carrier until it reaches the delivery tube end.
11. A tack delivery device for delivering a compressible annular tack into a blood vessel according to Claim 9, wherein a plurality of annular tacks are loaded in a series with the catheter delivery tube, and each is delivered in turn at the delivery tube end.
12. A tack delivery device for delivering a compressible annular tack into a blood vessel for emplacement against plaque to press and hold it against the blood vessel walls, comprising:
a catheter delivery tube having a delivery tube end, wherein a plurality of annular tacks are loaded in a series in a compressed form on an outer surface of the catheter delivery tube, and a cover is sleeved over the catheter delivery tube to hold the annular tacks in their compressed state on the outer surface of the catheter delivery tube, and said cover is retracted along the length of the catheter delivery tube from the delivery tube end in order to deliver each tack serially in turn to expand into a desired position against the blood vessel walls.
13. A tack delivery device for delivering an annular tack into a blood vessel for emplacement against plaque to press and hold it against the blood vessel walls, comprising:
a catheter delivery tube having delivery head, wherein the annular tack is to be made of a material having shape-memory and formed as a linear strip ribbon in its rest state and is capable of restoring to an annular shape when exposed to internal body temperature, wherein the delivery head of the catheter delivery tube has a plurality of barrels of a rotatable, multi-barreled tack magazine for loading a plurality of tacks therein in an array around the outer surface of the catheter delivery tube, and an internal pressure chamber for generating pressure for eject a ribbon strip from its barrel as it is moved by rotation of the magazine to bring each ribbon strip in turn to an ejector position in the delivery head.
14. A tack delivery device for delivering an annular tack into a blood vessel according to Claim 13, wherein the delivery head has an ejector track with a curved outlet tunnel for bending the ribbon strip towards its annular shape as the delivery head rotates.
15. A tack delivery device for delivering an annular tack into a blood vessel according to Claim 13, wherein the curved outlet tunnel is adapted to capture the end of the ribbon strip pushed into the ejector track and cause a middle part of the ribbon strip to bulge outward toward the blood vessel wall with the rotation of the delivery head.
