Abstract

发明领域
本发明涉及一种对治疗创伤很有用的创伤敷裹。
发明背景
就创伤的护理和治疗而言,“创伤”一词包括慢性的创伤如压迫溃疡(pressure ulcers)(I至IV期)和腿部溃疡;急性创伤如外科创伤(例如手术后伤口);外伤如小擦破和裂伤、烧伤(一度和二度)、剌伤、莫氏(Moh′s)外科、皮肤切除等等。创伤护理的关键是要考虑上皮的需要,亦即创伤面上新生细胞区域的需要,该新生细胞区域是在愈合过程中形成的,对此加以特殊的考虑就可促进愈合。在创伤护理中另一个要考虑的问题是周围未受创伤皮肤的需要。
一般认为,若上皮通常从周边向内生长迁移即是创伤愈合的征兆,因此,要特别注意不去不必要地损害或刺激这一新生区域或已存在的中间组织。在使用现有技术的敷裹中,当敷裹在创伤面上停留过久或在更换敷裹时就会发生问题。例如,敷裹可能粘结在上皮上,而肉芽组织和新生细胞可能缠结在敷裹的基质之中。在此情况下,若取下敷裹就有损害敏感组织和新生物的危险,造成创伤愈合进程的倒退。
因此,在创伤护理中,另一个关键性的考虑是更换敷裹的频率。当创伤面逸出大量渗出物时可能是需要频繁地更换敷裹的,而当创伤面逸出较少渗出物时,更换频率即可较小。但是,每当更换敷裹时,都有一种危险性使敏感组织和新生物受到不必要的损害。加之,创伤液中的有益生长因子也有可能被从创伤面上不必要地取走,从而阻碍它的有益效用。因此,重要的是要及时适当地更换敷裹,但又不太勤,以致不必要地损害敏感组织和新生物,或不必要地取走有益的生长因子。
令人遗憾的是,大多数现有技术的敷裹都依赖于护理人员或病人来决定是否需要更换。特别是在非专职护理人员或单独只有病人时,就很难知道什么时候应该更换敷裹。例如,现在使用的一种创伤治疗方法,特别是治疗腿部溃疡时,是在溃疡面上贴上纱布,并加压包裹以保持纱布贴在溃疡面上。护理人员基本上必须简单地猜想什么时候该更换敷裹了。如果敷裹换得太勤,则下面的组织可能受到损伤,而敷裹的一部分使用寿命却浪费掉了。但是,如果纱布在创伤面上停留过久,则创伤渗出物将在纱布上涂满水合物并浸软病人周围的皮肤。
已经制成了几种能监视下面的皮肤或创伤情况的敷裹。例如,美国专利5,181,905就公开了一种敷裹。这种敷裹优选地配备了一种能感知下面的皮肤或创伤面情况的电子-机械指示装置。该指示装置是作温度感应、颜色感应的封装液晶的系列装置。
还有一种带指示装置的绷带如美国专利3,675,654所公开的绷带。这种绷带包括放在半透明的用不透水材料制成的背托片上的吸收垫。在吸收垫和背托片之间放有湿气致动指示剂。指示剂是少量能溶于水的染料。当使用中吸收垫和指示剂变湿时,在吸收垫和背托片之间出现的溶液就能透过背托片被看见,以提示已经潮湿。但是,单是潮湿并不足以说明绷带应当更换。事实上,如果绷带在每次有点潮湿时就更换,那就可能是换得太勤了,并每次都有伤及敏感组织和新生细胞的危险。
也已经有人在绷带上做出标记,例如来指示从病人皮肤上取下绷带的适当方向,以使对下面正在愈合的创伤面的损害达到最小(见例如美国专利4,334,530),来监视创伤面的减小(见例如美国专利5,000,172),以及来标记止血的最佳压迫点(见例如美国专利5,310,402)。
虽然如此,但考虑到各种类型的创伤、现有的敷裹不可计数,以及各种各样的愈合阶段,仍然需要有一种敷裹,它能使过早更换敷裹、特别是被非护理专职人员更换的机会减到最少,从而使绷带的寿命最佳化,并工作到能防止由于频繁更换敷裹造成的对周围皮肤、组织和新生细胞的伤害。
发明概述
因此,按照本发明,就提供出了一种改进的创伤敷裹。本敷裹包括其上带有参照标志的背面层、水胶体层和泄放层或称传送系统。使用时,敷裹被放在创伤面上。创伤面所逸出的渗出物被水胶体层吸收,水胶体层随即膨胀。在一个优选实施方案中,随着水胶体层膨胀,它也变白。膨胀可透过背面层看到或感觉到。当膨胀扩展到或超过参照标志时,就应更换敷裹。因此,该种敷裹可以尽可能长地贴在创伤处,而不会由于频繁更换敷裹而损伤下面的皮肤,也不会由于时间过长而冒泄漏的危险。这样,本发明的创伤敷裹就提供了极其优越的创伤护理:更换敷裹的次数最少,由于频繁更换敷裹造成的损害创伤面周围正在愈合的嫩弱皮肤的机会也最少。此外,渗漏的危险也降到了最小,敷裹的总的管理、特别是对非护理专职人员来说也简化了,护理费用也降低了。

发明详述
请参看图1,图中示出本发明的创伤敷裹10的一种实施方案,它包括其上带有参照标志25的背面层20,背面层覆盖着水胶体层30。图中还示出了泄放层40。
水胶体层30典型地包括有本技术领域已知的能与流体相互反应的胶合剂,它的作用是治疗正在逸出渗出物的创伤面,该胶合剂特别应是散布在聚合物基质中(例如悬浮在弹性基质中)的水胶体。该类材料优选应能粘附在潮湿的表面上。
例如,Chen在美国专利3,339,546中公开了一种胶合剂,它包括一种或一种以上能溶于水或遇水膨胀的水胶体与一种粘性物质如聚异丁烯的混合物。用一层由不溶于水的材料制成的薄膜贴在胶合剂的一个表面上,在此就相当于背面层。
Doyle等人在美国专利4,551,490中还公开了一种适于用在本发明敷裹中作水胶体层的压敏胶合剂。这种胶合剂含有:5至30重量百分数的一种或多种聚异丁烯或一种或多种聚异丁烯与丁基橡胶的混合物;3至20重量百分数的苯乙烯径向型或嵌段型共聚物;8至40重量百分数的矿物油;15至65重量百分数的一种或多种能溶于水的水胶体树胶;最高达15重量百分数的一种或多种遇水膨胀的增粘剂,条件是水胶体树胶和增粘剂的总量为15至65重量百分数之间,和增粘剂为7.5至15重量百分数。
水胶体层30最好是用例如从ConvaTec公司可购得的产品中使用的胶合剂如:Durahesive、DuoDERM、DuoDERMCGF以及Stomahesive。但是,虽说这些水胶体层或胶合剂非常适用于本发明,但它们只是示例性的。任何与皮肤相容的水胶体都可使用。同样地,水胶体层的厚度也可以是任何方便的厚度,这在业内人士是很好理解的。例如,该层厚度可大约从1密耳至200密耳,优选为10~100密耳,更优选为20~50密耳,特别是35~45密耳。
在本发明的一个实施方案中,创伤敷裹的水胶体材料中还可包括抗菌剂、创伤愈合剂和/或气味控制剂等佐剂。此外,还可包括变色添加剂以增进水胶体的目视明显度。特别是,变色添加剂可加在水胶体层中与参照标志范围相合的地方(下面将进一步论述参照标志问题)。随着膨胀前沿推展到或超出参照标志处,变色剂就会在参照标志处使水胶体层变得更能明显看到。在创伤护理中常用的一些制剂也可包括在该层中。例如约2至20重量百分数、优选为10重量百分数的氧化锌可以包括在水胶体材料中。氧化锌不但能帮助护理创伤周围的皮肤,而且内含氧化锌的能与流体相互反应的胶合剂材料还会变得更加柔韧。
创伤敷裹的背面层20的材料优选为适用的聚合材料。它可以是本领域已知的聚合物膜、无纺材料、编织物等中的任何一种或它们的组合物。它优选是用薄的、柔韧的、有弹性的、柔软的、易弯曲或软弱的材料制成,它能弯能曲以贴合不规则的表面或外形、如解剖学的身体部位外形。背面层20优选为透明或半透明的,或者也可以是不透明的。背面层20可以是透气、透湿的,以使氧气能透过敷裹,同时使来自皮肤表面的湿气能通过敷裹逸出。另外,背面层20可以是不能透过气体、不能透过液体或不能透过细菌的,就象本领域人员如需治疗的特种创伤或表面所选择的材料那样。聚氨酯或聚乙烯薄膜是特别优选用作本发明的背面层材料的。
背面层20上具有参照标志25。参照标志可以以任何方便的形式出现。例如,参照标志可以印刷或压印在背面层上,或者可做在另外的一层材料上但可通过背面层看到或感觉到。参照标志应放置得能指示出需要更换敷裹。亦即,当吸收的创伤面渗出物已扩展到或已超出参照标志时,就应更换敷裹。参照标志可以是任何方便的大小、形状或构造的,这取决于例如敷裹的大小和形状、水胶体层的组成和厚度以及所敷裹的创伤情况。举例说,参照标志可由实线、虚线、粗体线、影线、符号或任何其它外形组成。本领域的普通技术人员就能不费多大力气例如在背面层上把参照标志设置在适当的位置,例如设置在靠近敷裹的边上,以使敷裹的使用寿命达到最大,并使更换敷裹的需要达到最小。当然,在某些情况下,把参照标志设置在小于敷裹最大使用寿命的位置是有意义的。本发明包括把参照标志设置在任何适宜位置的敷裹。
泄放层40可以是本领域已知的任何方便的泄放层或泄放系统。例如,该层可结合有硅化处理过的聚酯泄放薄片和硅化处理过的泄放纸。
图2代表本发明将被更换的创伤敷裹一种实施方案的透视图。如同在图1中一样,创伤敷裹10上也示出了参照标志25。当创伤面逸出渗出物时,渗出物被水胶体层所吸收。水胶体层就膨胀,在一个优选实施方案中还同时变白。(在缺少某些外加着色剂或添加剂的情况下,水胶体变白是水合作用的结果)。这可透过背面层看得到。当这一膨胀区50扩展到或超出参照标志25时,该敷裹就应更换。在这样的方式下,敷裹的更换能使敷裹的最大寿命得到利用,同时又使周围皮肤、组织和新生细胞不致因不必要的频繁更换敷裹而受到干扰。
创伤敷裹10,以及上面已讨论过的参照标志,可以是任何方便的大小和形状,主要视被敷裹的创伤而定。图中把它画成同心矩形只是为简单明了而已。
本领域的技术人员会正确理解,所示出的实施方案可加以变动而不会脱离本发明的精神和范围。

Claims (10)

1.一种创伤敷裹,它包括其上带有参照标志的背面层、水胶体层和泄放层,其中参照标志的设置位置能使水胶体层的膨胀情况受到监视。
2.权利要求1的敷裹,其中所说的水胶体层中含有悬浮在弹性基质内的水胶体。
3.权利要求1的敷裹,其中所说的水胶体层中含有抗菌剂、创伤愈合剂或气味控制剂。
4.权利要求1的敷裹,其中所说的背面层是聚合物薄膜。
5.权利要求4的敷裹,其中所说的背面层是聚氨酯层。
6.权利要求1的敷裹,其中所说的泄放层中包括有硅化处理过的聚酯泄放薄片和硅化处理过的泄放纸。
7.权利要求1的敷裹,其中参照标志的设置位置能使水胶体层的膨胀扩展到或超出参照标志的周边时即指示出敷裹应当更换。
8.权利要求7的敷裹,这样可使敷裹的使用寿命达到最大,并使更换敷裹的需要达到最小。
9.权利要求1的敷裹,其中所说的参照标志是印刷在该背面层上的。
10.一种创伤敷裹,它包括其上带有参照标志的背面层、水胶体层和泄放层,其中背面层为聚氨酯层、水胶体层中含有悬浮在弹性基质内的水胶体,参照标志的设置位置能使水胶体层的膨胀扩展到或超出参照标志的周边时即指示出敷裹应当更换。
