本发明涉及一种用于既可以用于人类也可以用于兽类的外科手术用的夹子,本发明尤其涉及用于堵塞冠状动脉和其它动脉的夹子,用于对冠状动脉进行分流移植,也适用于具有大致和人类或动物中的类似尺寸的其它脉管或管状结构,以便压缩该结构以防止血液或其它流体通过。
发明背景
在进行“停止泵送(offpump)”的冠状动脉旁路外科手术(CAGS)时,其目的是为了在不使心脏停止跳动的情况下进行分流术。因此,有必要采用夹子在形成于该动脉中的小切口的任意一侧将冠状动脉堵塞住,该该分流动脉管将被缝在该小口上。因此,临时防止血液流向心脏部分,同时进行移植(grafting)。随后,去掉夹子,冠状动脉随后通过该移植物接收补充的血流。
现有的用于该目的的夹子的一种形式为一种传统金属“多爪线夹”(”bulldog clamp”)的塑料衍化形式,但是要小一些,轻一些。其通过手指按压而开启并通过该材料的固有弹性而闭合,从而进行工作。通常,所提供的夹紧压力不足以充分控制血流。而且,其比较长,很难方便地装配在一种相关的牵引器装置的稳定臂中。其可以垂直于心脏进行移动,使得其可以阻挡外科医生的视线以及在移植过程中挂住缝合线。金属夹子由于其重量而存在一些缺陷,这些缺陷在其安装到跳动的心脏上时由于其惯性而导致存在反作用力。因此,需要频繁地移动。其较大的尺寸使其在甚至非常困难的情况下也要放置在有限的空间中。

发明内容
根据本发明,提供了一种外科手术用的夹子,该夹子包括彼此相对的夹紧部件,该夹紧部件可枢轴转动地在打开和关闭状态之间彼此相对运动,以及一棘爪装置,该棘爪装置在关闭运动期间可以工作以便将该夹紧部件保持在一所选定的关闭夹紧状态。
优选的是,该棘爪装置包括一系列呈弓形排列并以该夹紧部件的枢轴运动的轴线为中心的棘齿,并且在所述夹紧部件相对另一夹紧部件作枢轴运动时,一棘齿可以随着其中一个所述夹紧部件而运动,从而使得该排棘齿运动,由此将其保持在一选定的夹紧位置。

具体实施方式
本发明的优选实施例的动脉夹子以一种适当的塑料(例如聚丙烯均聚物或乙缩醛共聚物)形成一单件式模制件,并且包括一对相对臂2、4,该对臂可通过一铰链6在一端处枢轴连接,该铰链6由一柔性连接板形成,该连接板由塑料材料制成,从而该对臂可以从基本呈V形的打开结构运动到一关闭夹紧结构,在该关闭夹紧结构下,该对臂2、4几乎平行。该臂2的外端形成有一凸出的翼部8,该翼部在其内表面上有一组棘齿10,该棘齿沿着一弯曲路径延伸,该组棘齿以连接该对臂2、4的铰链6的枢轴线为中心。该臂4的外端形成有一棘齿12,该棘齿与棘齿10相互配合。该对臂2、4中的每一个都形成有一夹紧柱14,该夹紧柱基本上垂直于该对臂2、4的平面凸出,该夹紧柱14形成该夹子的实际夹紧部件。在使用时,这两个夹紧柱位于动脉的相对两侧并夹紧该动脉。随着在臂4的末端处的棘齿12沿着在凸出翼部8上的棘齿10运动,直到实现所需要的夹紧状态,两个臂2、4通过绕铰链6的枢轴运动而关闭,其中该对臂2、4通过该棘齿的单向运动而肯定地保持在那种状态。
可以通过镊子或类似方式使得两个臂2、4的外侧啮合而使得该夹子得以定位、操纵和关闭,为此,最好在该侧面上沿着其长度方向设置一系列较小的凸起16以有利于该镊子的夹紧力。该对臂2、4每一个上也都设置有一些孔18,用于接收该镊子的顶端,以便使其进一步具有多功能性。
采用上述夹子,外科医生可以施加任何认为适于阻止流体流的夹紧压力,一旦施加,该压力将由于通过相互啮合的齿10、12而产生的棘爪作用而在两个臂2、4上产生的锁定作用而得以保持。为了松开该夹子,必要的是将凸出翼部8横向移出第二臂4的平面,由此将齿10和齿12之间的啮合松开。这可以通过该塑料材料的固有的柔性而方便地实现。

通过图示,夹子的从铰链6到棘爪翼部8的长度大约为10mm,这使得该夹子明显小于前面所讨论的现有夹子的形状,这种尺寸的显著减少可以通过如图所示的包含有锁定棘爪机构的基本形状来获得。因此,该棘爪机构不但使得夹紧力可以得以控制和保持,而且其可以用来实现整个长度的减少,以更方便地布置在受限制的空间内。
夹子的形状使其更少倾向与垂直与该心脏的表面凸伸,并且在构成连接的过程中也较少地倾向于挂住缝合材料。于金属相比,该塑料结构减小了重量,并因此使得该夹子将能方便地随着心脏移动,并且既不会由于心脏的跳动而倾向于移位也不会在用于堵塞该管道时过度牵拉管道。
可以看到,该夹子将为一次性使用产品,并不是可以进行再次清洁和再次消毒。实际上,再次消毒将会很困难,因为存在凹部/孔,这些凹部和孔用于使得夹子能够通过镊子或其它装置夹持,使之能够定位和紧固,这些凹部和孔也用于防止其得到充分的清洗和去掉夹子上的血液。
可以考虑用两个这种夹子来堵塞一个冠状动脉和用于该管道的另一个。在每次手术之中,该夹子可用于多个冠状动脉。
所移植的冠状动脉的尺寸大小在1.0-2.5mm的直径之间,其中最好为1.5-2.0mm之间。该管道的尺寸通常为1.75-3.4mm之间。这些尺寸相对类似许多用于其它形式的管腔或整形外科中的动脉和静脉,其中动脉被连接在一起或被移植。在其它外科学科中,会频繁地碰到这种尺寸的动脉,并且有时需要临时堵塞以改变病理或连接起来。因此,即使在不进行任何改变的情况下,该夹子都可以具有除了“停止泵送”CAGS之外的其它潜在用途。对该设计进行尺寸或其它微小的改变可以使得该夹子对在人类或动物身上的其他外科过程具有更宽的使用范围。
该实施例仅仅是作为一个例子进行描述并且在不脱离本发明的范围的情况下进行改变。

Claims (6)

1.一种外科手术用的夹子,该夹子包括:一对臂,该对臂在相邻的末端枢轴地相互连接,以便在一平面内、在一打开位置和一关闭位置之间进行枢轴运动,在所述打开位置,该对臂构成一V形结构,在所述关闭位置,该对臂几乎平行;以及一棘爪装置,该棘爪装置在关闭运动期间可以工作,以便将所述臂保持在一选定的关闭状态,其特征在于,每一个臂具有一夹紧部件,该夹紧部件以柱的形式基本上垂直于枢轴运动的所述平面从该臂延伸,每一个夹紧柱具有一夹紧表面,因此,当所述臂运动到其关闭状态时,可以将一动脉夹持在两个夹紧柱的夹紧表面之间。
2.如权利要求1所述的夹子,其特征在于,所述夹紧柱的夹紧表面沿着向夹紧柱的远端的方向会合。
3.如权利要求2所述的夹子,其特征在于,每一个夹紧表面都相对于夹紧柱的轴线倾斜大约4-6度的角度,以便以一个大约8-12度的夹角会合。

6.如权利要求1-5中的任何一项所述的夹子,其特征在于,所述夹子形成一个单件式塑料模制件,其具有由一铰链形成的枢轴,所述铰链由塑料材料的柔性连接板构成并将所述两个臂的相邻末端相互连接起来,且所述棘爪装置由形成在相应臂上的可以相互啮合的棘齿形成,所述棘齿可由于形成夹子的塑料材料的固有柔性而产生的挠性运动而脱开啮合以便松开夹子。
