Abstract

Description
技术领域
本发明涉及一种外科手术钉,特别是髓内钉。
背景技术
髓内钉的锁紧属于现有技术。在髓内钉的横向孔中引入锁紧螺钉或锁紧销钉(后面还会采用术语锁紧螺钉,其应该还包含术语锁紧销钉)是在造影法的帮助(X光检查)或者或多或少复杂的瞄准装置的帮助下实现的。在两种情况下都不用回避一些定标准确度(Zielungenauigkeit),这也就是说,螺钉尖端不必准确地与横向孔的中心线共轴定位,但是由此会产生一定的偏差。因此尽管具有目标误差,该锁紧螺钉通入到横向孔中并且可以由其穿过,但是相对于横向孔直径螺钉外径可以尺寸更小。保持定标准确度在更小尺寸的范围内,从而锁紧螺钉尽管具有目标误差仍能顺利穿过通孔。但是,由于尺寸不足,锁紧螺钉相对于横向孔具有一些间隙。
该间隙限定出,通过锁紧螺钉固定在相应的锁紧孔中的骨头主要碎片相对于钉以及由于钉的刚性相对于其它用相同钉固定的骨头主要碎片能够运动的量。为了确保该锁紧对于外科医生的实用性,该间隙实际上是不可避免的,然而在一些适应症方面(例如,在干骺端软骨碎片的情况下)又是临床上不希望的。
具有完整横截面的钉本身不是无间隙的,该钉在锁紧孔中可以具有内螺纹。该内螺纹仅仅可以防止该钉轴向在该锁紧螺钉上移动。
在US6296645Hover等中公开了一种由金属制成的中空髓内钉,该髓内钉在横向孔的被称作窗口的直径相对的外壳孔中具有一个或两个塑料嵌件,通过其可以插入锁紧螺钉。该公知的髓内钉的缺点在于,窗口状塑料嵌件可能很容易被压入,因此就失去了所希望的功能。但是即使通过非常小心的操作,该两个塑料嵌件可能会在穿过锁紧螺钉时从它的“窗口”中压出,这样也导致了功能的丧失。
发明内容
在这里本发明提供了补救,本发明实现该目的在于,提供一种外科手术钉,特别是一种髓内钉,利用它能够安全地消除存在于它和锁紧螺钉之间的间隙,并且能够获得改善的保持力以及锁紧螺钉和髓内钉之间的改善的导向效果。
本发明利用具有以下特征的外科手术钉解决了所提出的问题。
按本发明的外科手术钉,尤其是髓内钉,具有一中心线、一同轴于中心线延伸的具有直径D的纵向孔、一具有内表面的外壳以及一横向于中心线延伸的横向孔,该横向孔适合于容纳一锁紧螺钉,并且横向孔具有横截面F和孔轴线,
其特征在于:
A)所述纵向孔是贯穿的;
B)在所述纵向孔中在横向孔的区域内设置有一使横截面F缩小的构件,该构件仍允许锁紧螺钉通过;
C)所述构件具有一径向突入纵向孔内的、厚度P>0.01D的部分;以及
D)所述构件是一纵向构件,该纵向构件平行于中心线定位
由此可以获得下述优点:
a)在引入该锁紧螺钉的同时目标准确度(Zielgenauigkeit)不受影响;
b)它允许对于确定量的负荷在一定方向上角度稳定地固定骨碎片;
c)可以在皮层上钻孔后将构件置入髓内钉中,也就是说不会在骨髓腔中形成异物切屑;以及
d)根据构件的选择,能够得到锁紧螺钉的较强或较弱的角度稳定性。
在一特殊的实施例中,该构件由一销钉、一钢丝件(Drahtstück)或者一缆线件(Kabelstuck)构成,其基本与钉的纵轴线平行地定位。
在一优选的实施例中,该构件由与髓内钉相同的材料构成,也就是说,对于钛钉由钛或者钛合金构成,或者对于钢钉由钢构成。
该构件还可以由生物相容的塑料构成,例如聚乙烯,特别是由高分子聚乙烯(HMWPE)构成。该材料的优点在于,一方面其不是太硬,另一方面不会导致塑料分解产生未知的分解产物。
但该塑料还可以是可生物吸收的聚合物,优选是聚交酯。通过这种方式实现了最初髓内钉无间隙地横向锁定,于是其随着聚合物的增强的再吸收而又逐渐取消,因此横向锁紧螺钉相对于髓内钉和因此所提供的骨碎片又变得可移动。这因而导致了骨碎片的动态生长(Dynamisierung),在实现断裂合并之后。
可生物吸收材料的另一个优点在于,在锁紧螺钉拧过钉横向孔时可能形成的切屑能够从身体中分离出。
横向孔是一圆孔,横截面F沿中心线的方向具有一最大的长度a并且在与其垂直的方向上具有一最大的宽度b,其中a=b。
该钉还包含一可插到横向孔内的具有外螺纹的锁紧螺钉或者一具有光滑的杆的锁紧销钉,其中,外螺纹或者杆具有直径“d”,横截面F沿中心线的方向具有一最大的长度a并且在与其垂直的方向上具有一最大的宽度b,并且满足条件a>d<b
该钉的横向孔可设计成圆孔(a=b)或者长孔(Langloch)(a>b)。
可插入到横向孔中的锁紧螺钉(或者锁紧销钉)适合地具有直径为d的杆,其中a>d<b。在锁紧螺钉的情况下,直径d还包括它的外螺纹。此外,该直径d优选比横截面F的两个尺寸a、b中较小者至少小5%。
该钉具有两个贯穿的横向孔,并且构件定位在横向孔的角部区域中。
所述塑料是一种可生物再吸收的聚合物或者共聚物。
构件的径向突入纵向孔内的厚度P>0.05D。
该构件可以安装或固定在髓内钉内的不同位置。然而优选固定在髓内钉壁体内表面上。

具体实施方式
在图1和2中示出的外科手术钉1是一种用于管状骨的具有中心线2的髓内钉,其由金属或者合金,也就是由具有相对高强度的材料构成,以及该髓内钉包括一具有横截面F和横向轴线8的横向于中心线2延伸的横向孔5。该横向孔5安置在近端,并构成为圆孔。它也可以构成为具有最大尺寸a和b的长孔,其中较长的尺寸a要沿轴向设置。
该髓内钉具有一同轴于中心线2延伸的纵向孔3和由此具有一具有内表面6的外壳4。在纵向孔3中,一使横向孔5横截面F缩小的构件7在横向孔5区域内固定在外壳4的内表面6上。在所示实施例中,构件7安置在纵向孔3的后部区域内,并且具有一突出的厚度P。厚度P这样选择,即使得具有外螺纹11的锁紧螺钉10恰好还能够旋入到横向孔5中。
构件7由一销钉构成,其基本上与纵轴线2平行定位。代替销钉也可以使用其它的纵向构件,例如钢丝件、线缆件或者附壁型元件(Profilstück)。
在图3中示出了本发明髓内钉的变型,其中构件7构成为单独的、借助合适的器械20在手术期内可插入到髓内钉1的纵向孔3中的元件。
在图4和5中示出了本发明髓内钉的另一个变型,其中外壳4的内表面6具有一平行于中心线2延伸的槽9,构件7可从髓内钉1的尖端14插入该槽中。构件7的长度这样确定,即使得其在完全插入到髓内钉1中时到达横向孔5的区域中,从而通过构件7缩小了其横截面F。槽9具有一部分圆形(teilkreisformigen)的横截面,它具有200度的中心角(Zentriwinkel)。在此构件7的直径这样确定,即其径向突入纵向孔3的部分促使横截面F的这种收缩,从而还可以使得具有光滑的杆13的锁紧销钉12恰好通过。
Claims (15)
1.一种外科手术钉(1),具有一中心线(2)、一同轴于中心线(2)延伸的具有直径D的纵向孔(3)、一具有内表面(6)的外壳(4)以及一横向于中心线(2)延伸的横向孔(5),该横向孔适合于容纳一锁紧螺钉(10),并且横向孔具有横截面F和孔轴线(8),
其特征在于:
A)所述纵向孔(3)是贯穿的;
B)在所述纵向孔(3)中在横向孔(5)的区域内设置有一使横截面F缩小的构件(7),该构件仍允许锁紧螺钉(10)通过;
C)所述构件(7)具有一径向突入纵向孔(3)内的、厚度P>0.01D的部分;以及
D)所述构件(7)是一纵向构件,该纵向构件平行于中心线(2)定位。
2.根据权利要求1的钉(1),其特征在于,所述构件(7)固定在外壳(4)的内表面(6)上。
3.根据权利要求1或2的钉(1),其特征在于,所述构件(7)是一销钉、钢丝件、线缆件或者附壁型元件。
4.根据权利要求1或2的钉(1),其特征在于,所述构件(7)由与钉(1)相同的材料构成。
5.根据权利要求1或2的钉(1),其特征在于,所述构件(7)由一种可生物相容的塑料构成。
6.根据权利要求5的钉(1),其特征在于,所述塑料是一种可生物再吸收的聚合物或者共聚物。
7.根据权利要求1或2的钉(1),其特征在于,横截面F沿中心线(2)的方向具有一最大的长度a并且在与其垂直的方向上具有一最大的宽度b,其中a>b。
8.根据权利要求1或2的钉(1),其特征在于,横向孔(5)是一圆孔,横截面F沿中心线(2)的方向具有一最大的长度a并且在与其垂直的方向上具有一最大的宽度b,其中a=b。
9.根据权利要求1或2的钉(1),其特征在于,该钉还包含一历插到横向孔(5)内的具有外螺纹(11)的锁紧螺钉(10)或者一具有光滑的杆(13)的锁紧销钉(12),其中,外螺纹(11)或者杆(13)具有直径“d”,横截面F沿中心线(2)的方向具有一最大的长度a并且在与其垂直的方向上具有一最大的宽度b,并且满足条件a>d<b。
10.根据权利要求9的钉(1),其特征在于,所述直径“d”比两个尺寸a、b中的较小者小至少5%。
11.根据权利要求1或2的钉(1),其特征在于,该钉具有两个贯穿的横向孔(5),并且构件(7)定位在所述横向孔(5)的角部区域中。
12.根据权利要求1或2的钉(1),其特征在于,构件(7)构造成单独的、在手术期内可插入到纵向孔(3)中的元件。
13.根据权利要求12的钉(1),其特征在于,外壳(4)的内表面(6)具有一平行于中心线(2)延伸的槽(9),构件(7)可插入到该槽中。
14.根据权利要求13的钉(1),其特征在于,槽(9)具有一部分圆形的横截面。
15.根据权利要求1或2的钉(1),其特征在于,构件(7)的径向突入纵向孔(3)内的厚度P>0.05D。
