技术领域

背景技术
据估计,仅在美国和欧洲就有6-7百万人患有充血性心力衰竭(CHF),其最常见的病因是缺血性和自发性心肌病。在20-50%的CHF患者中,呈现延长的PR间隔和宽的QRS复合波。这些患者中大约29%有左束支传导阻滞(LBBB)。
心脏中的正常电传导起始于窦房结,病经由房室结、His束以及右和左束支。在CHF和LBBB患者中,呈现长的机械性延迟,引起延迟的心室除极化,这就导致延迟的左心室喷射。在有LBBB时,收缩是不对称的。隔片先缩短,接着侧壁伸展。随后,侧壁缩短,而隔片伸展,引起左心室的无效收缩。心脏的重新同步治疗,亦即使右心室(RV)和左心室(LV)二者同时起搏,已显示出在患有CHF和LBBB患者中能有效地改进功能。
最近已显示,在有正常QRS或患有右束支阻滞的患者中也可看到两心室不同步,而在这些患者中两心室起搏可能有帮助。两心室起搏对有心房颤动的患者也已显示出有益的效果。
执行两心室起搏的一种已知方法包括:放置RV和右房引线,接着在冠状窦(CS)中放置套管。然后进行血管造影,以描绘出用于LV引线放置的合适分支。将用于LV起搏的引线放置到CS的后支或后侧支,然后用RV和LV引线使RV和LV同时起搏,从而达到与心房收缩的同步。
在两心室起搏另有帮助的20%以上的患者中,在CS中的引线放置可能是不成功或是非常长的操作过程,或者引线可能会从CS中移位。其它可能发生的会抑制该操作有用性的困难包括:没有可用的合适CS分支、由于左房(LA)和LV扩张引起的CS的显著旋转以及存在特贝西乌斯氏瓣。在大多数病例中,这些问题只有在介入操作时才能查明,并且该操作或被放弃,或将患者带回手术室作第二次介入操作,此时使用手术切口将LV引线从外面放到心外膜上。
在使用至少一种已知心外膜引线放置方法中所涉及的一些缺陷包括:使用胸廓微创术时LV的后外侧区或外侧区的视野有限。而且,可作的放置局限于提供合理的起搏和感测参数的部位。在某些情况下,可能无法确定LV距胸壁有多远,和/或无法识别在心动周期期间持续收缩的左心室的后外侧区或外侧区。该方法还会冒着损坏冠状动脉和静脉系统的风险。在一些操作中,由于心包外脂肪的存在或由于可见度有限看不清正常组织还是疤痕组织而增加困难程度。由于所有上述限制,在标识用于起搏的理想位置方面也会有困难。
此外,虽然许多技术例如组织的多普勒研究和超声波心动描记法已用来标识左右心室的不同步,但不可能标识可从重新同步中获得最大益处的位置。
发明内容

本发明的配置在计划时能有利地用于:观看从胸壁到心脏表面的距离;标识切口位置;并通过在手术前观看患者的解剖结构及它们的定向而便于作手术计划。本发明的配置还可用于观看CS和CS定向,以及到最佳或至少有利部位的最近分支的位置,从而提供信息来帮助确定如何使用经静脉的途径导入引线的策略,其中将LV引线放置到CS的分支中。

具体实施方式
本发明在某些方面提供了用于两心室起搏计划的心脏CT系统和方法。本文公开的系统和方法的技术效果包括提供用于计划介入操作的信息和图像,从而使电生理学家、心脏病专家和/或外科医生能够事先计划对该操作所采用的途径。其它技术效果包括提供LV及其与胸壁关系的详细3D几何表示,以使电生理学家、心脏病专家和/或外科医生能观看到脂肪的存在、主要血管及其分支的位置和定向以及可用来放置LV引线的活组织。某些配置的技术效果包括提供可视化,以标识放置LV心外膜起搏引线的最佳位置。同样,如果使用经静脉的途径,则某些配置包括标识和显现用于引线放置的最佳或至少有利的CS分支。
在一些已知的CT成像系统配置中,X射线源投射扇形射束,该射束被校准,以位于笛卡尔坐标系的X-Y平面内,通常称为“成像平面”。X射线束穿过被成像对象,例如患者。射束被对象衰减后打到辐射检测器阵列上。在检测器阵列上接收的衰减辐射束的强度取决于对象对X射线束的衰减。阵列中的每个检测器单元产生单独的电信号,其是在该检测器位置上射束强度的量度。分别从所有检测器获取强度量度,以产生传输分布图。
在第三代CT系统中,X射线源和检测器阵列随着成像平面中的机架并绕要成像的对象旋转,以使X射线束与对象相交的角度不断改变。在一个机架角度来自检测器阵列的一组X射线衰减量度,即投影数据,称为“视图”。对象的“扫描”包括在X射线源和检测器的一次旋转期间在不同的机架角度或视角所作的一组视图。
在轴向扫描中,处理投影数据,以构建对应于通过对象所取的二维层面的图像。用于从一组投影数据重建图像的一种方法在业界称为滤回投影技术。该过程将来自扫描的衰减量度转换成称为“CT数”或“Hounsfield单位”(HU)的整数,它们用来控制阴极射线管显示器上相应像素的亮度。
为减少总扫描时间,可以进行“螺旋”扫描。为进行“螺旋”扫描,移动患者,同时获取规定数量层面的数据。这种系统从扇形射束螺旋扫描中生成单螺旋。由扇形射束绘制出的螺旋产生投影数据,从中就可重建每个规定层面中的图像。
用于螺旋扫描的重建算法通常使用螺旋加权算法,对采集的数据作为视角和检测器通道指数的函数进行加权。具体地说,在滤回投影过程之前,根据螺旋加权系数对数据进行加权,螺旋加权系数是机架角度和检测器角度二者的函数。然后对加权的数据进行处理,以生成CT数,并构建对应于通过对象所取的二维层面的图像。
为进一步减少总获取时间,已引入了多层面CT。在多层面CT中,多行投影数据在任一瞬间同时被获取。当与螺旋扫描模式相结合时,该系统生成单螺旋的锥形射束投影数据。与单层面螺旋加权方案类似,可以推导出在滤回投影算法之前对投影数据乘加权的方法。
在本文中使用时,以单数叙述的并用单词“一个”说明的单元或步骤应理解为不排除多个所述单元或步骤,除非对这种排除作了明确说明。而且,提到本发明的“一个实施例”不应解释为排除存在也结合了所述特征的附加实施例。
在本文中使用时,短语“重建图像”不应排除生成表示图像的数据但不生成可视图像的本发明实施例。但许多实施例生成(或配置为生成)至少一个可视图像。
参阅图1和2,图中示出了多层面扫描成像系统,例如计算机X射线断层扫描(CT)成像系统10,其包括机架12,表示“第三代”CT成像系统。机架12有X射线管14(本文中也称为X射线源14),它向机架12相对侧上的检测器阵列18投射X射线束16。检测器阵列18由多个检测器行(未示出)形成,包括多个检测器单元20,它们一起感测穿过阵列18和源14之间的对象例如患者22的所投射X射线。每个检测器单元20产生一个电信号,该电信号表示打入的X射线束的强度,因此可用来估算射束穿过对象或患者22时的衰减。在获取X射线投影数据的扫描期间,机架12和安装在其中的部件绕旋转中心24旋转。图2仅示出单行检测器单元20(即,一个检测器行)。但多层面检测器阵列18包括检测器单元20的多个平行的检测器行,以使对应于多个准平行或平行层面的投影数据可以在扫描期间同时被获取。

在112,利用心脏的LV壁运动分布图,来相对参考期检测在心脏周期的各期之间每个短轴层面的壁运动。然后在114,创建每个分段和/或层面的LV的位移和速度分布图,以定位指示收缩时间延迟的区域、最近达到最大位移的区域或最近开始收缩的区域(即,达到最大速度的最近区域)中的至少一项。在一些配置中,生成线网模型,以检测这些区域。例如,图6示出了线网格式的心脏CT图像,显示出收缩前期的心脏外壁(心外膜302)和内壁(心内膜304)。图7为同一心脏在心动周期收缩末期位置的图像。
然后,在116,生成2D和/或3D透视图。透视图上重叠有收缩时间延迟和/或相应的最大位移/速度的彩色编码图。
在118,在心脏半透明3D模型上显现主要血管,包括CS及其分支、冠状动脉和它们的分支以及脂肪,以避免将引线放置到主要血管附近的区域上。在120,在LV的3D模型上显现坏死组织,以避免将引线放置到无效区域上。在122,在所关注界标和胸壁处将显式几何标记插入到体中。在一些配置中,以半透明方式显现LV和CS,具有不透明的几何界标。
在124,根据需要保存特定的3D透视图和轴向图像(如DICOM图像、视频剪辑、胶片、多媒体格式等),用于介入计划的视觉参考。保存的视图可以用荧光系统上的投影图像或3D荧光系统或介入系统计算机工作站的X射线断层合成图像导出和记录。
在126,标识并显示在LV壁上放置心外膜起搏电极的最适当部位,在一些配置中还标识和显示放置该电极的次最佳区域。如果使用利用CS的经静脉的途径,则在126标识靠近最佳(或至少有利)部位的CS分支。
在128,标识从胸壁到LV上适当部位的距离,并估算和显示相关的尺寸,以帮助选择适当的起搏引线部位。在130,还估算并显示引线在向下转入分支前在CS中必须经过的距离。在一些配置中,用于这种估算的分支是确定为对于起搏最佳的分支。
在一些配置中,采用自动方法来执行任一项上述功能,可使用一种或多种几个计算机辅助检测和显现方法,例如灌注缺损的量化分析、局部收缩状态分布图(LV壁运动),和/或用连续的相同强度级来标识血管。当操作和所关注器官已被指定是,这些方法可以自动运行,或可至少部分与用户的输入交互地运行。
上述公开的本发明配置可以实现为计算机X射线断层扫描(CT)成像系统10的一部分,其中图像重建器34(它可以是计算机36的一部分)执行高速图像重建,并在软件或固件的控制下进行其它计算和确定。图像显现在显示器42上,且计算机36和图像重建器34包括内部随机存取存储器(RAM)和/或其它存储器,例如ROM、光盘存储器、磁盘存储器、闪存ROM等。计算机系统中用于存储和检索数据和中间结果的各种存储器以及存储器的一般用途在业界已众所周知。
在一些配置中,本文所述的过程(除获得图像数据集外)是用已获得的图像数据集进行的。这种配置可使用计算机36、显示器42以及与计算机36关联的存储器来执行本文所述一种或多种方法配置,或使用配置有适当软件和/或固件的独立工作站。在一些配置中,提供一种或多种机器可读介质52,其上记录有指令,这些指令配置成指令处理器执行上述一种或多种方法配置的步骤。在本文中使用时,“其上记录有指令的机器可读介质”不应认为限于单一类型的介质或介质的单个实例。因此,“机器可读介质”应认为包括不止一种介质或介质类型。例如,包括计算机程序的一组指令可能很长,需要多个软盘或CD-ROM或这二者的组合。其上记录有计算机程序指令的介质组合被认为是“机器可读介质”,如该术语在本文中所用的。
应理解,本发明的一些配置有助于将起搏引线实时地导入能达到最协调心室收缩的位置。一些配置提供了类似的帮助,可用于非实时计划目的。而且,在提供LV解剖和功能方面,本发明的一些配置起着重要作用,并由此标识引线放置的最适合位置,从而减少整个操作时间和提高功效和安全性。
具体地说,在一些配置中,创建了标识起搏的最佳部位的路线图,可以标识出靠近最佳部位的CS分支的定位和位置。而且,本发明一些配置提供的引线放置最佳部位的标识改进了用于两心室起搏的技术功效,不论所用技术如何(例如将LV引线放入CS分支之一或从心外膜的途径)。此外,在标识要用的最佳技术之前创建路线图简化了操作,而且除了提高功效外也提高了安全性。
虽然已就各种具体实施例对本发明作了说明,但业界技术人员会认识到,实践本发明时可以有在权利要求书的精神和范围内的改动。
Claims (24)
1.一种辅助计划对患者心脏的介入式两心室起搏操作的系统,所述患者心脏具有左心室、左心室心肌、心肌壁、左心室壁,且心动周期具有多个期,所述系统包括成像装置,所述成像装置配置成获取患者心脏在舒张期以及在多个收缩期和舒张早期的大量数据,所述获取的大量数据由此成为图像数据集;且所述系统配置成:
通过分段所述图像数据集来提取所述左心室和所述左心室心肌的面;
利用分段的图像数据集将所述左心室分成心肌分段,或分成多个短轴层面;
相对于参考期,检测在所述心动周期的期之间每个心肌分段或短轴层面的壁运动;
定位指示收缩时间延迟的区域、最近达到最大位移的区域或最近达到最大速度的区域中的至少一项;以及
生成2D或3D透视图,包括指示收缩的时间延迟、最大位移或最大速度中至少一项的透视图;
为心外膜起搏电极在左心室壁上的至少一个放置位置确定适当部位。
2.如权利要求1所述的系统,其中所述患者具有胸壁,且所述心脏具有左心室壁、左心室血管和心外膜脂肪,并且其中所述系统配置成在后处理软件的控制下分段所述图像数据集,并生成所述3D透视图,所述系统还配置成利用3D面透视图或体透视图中的至少一项来显现所述胸壁、左心室壁、左心室血管和心外膜脂肪。
3.如权利要求1所述的系统,还配置成利用灌注研究的显现来标识所述左心室心肌的坏死组织,并且其中为生成所述3D透视图,所述装置还配置成显现所标识的坏死组织。
4.如权利要求1所述的系统,其中为利用所述分段的图像数据集将所述左心室分成多个短轴层面或分段的心肌区域,所述系统配置成将所述左心室分成基底段、中间段和顶尖段。
5.如权利要求1所述的系统,其中为利用所述分段的图像数据集将所述左心室分成心肌分段或多个短轴层面,所述系统配置成将所述心肌壁的短轴层面分成多个带和箱。
6.如权利要求5所述的系统,其中所述多个带和箱包括总数至少为100个带和箱。
7.如权利要求6所述的系统,还配置成利用16到17个带创建至少6个区域中的每一个,所述六个区域为:前区、前外侧区、后外侧区、后区、后间隔区以及前间隔区。
8.如权利要求1所述的系统,其中所述心脏还包括具有分支的心窦和具有分支的冠状动脉以及脂肪,并且为生成所述3D透视图,所述系统还配置成将所述心窦及其分支、冠状动脉及它们的分支以及脂肪显现在所述心脏的半透明3D模型上,以便于将引线放置到远离主要血管的区域上。
9.如权利要求1所述的系统,还配置成标识所述左心室心肌的坏死组织,并且其中为生成所述3D透视图,所述系统还配置成将所标识的坏死组织显现在所述左心室的3D模型上,以便于避免将引线放置到无效区域上。
10.如权利要求1所述的系统,其中为生成所述3D透视图,所述系统还配置成在所关注界标处将几何标记插入到显现的体中。
11.如权利要求10所述的系统,其中为生成所述3D透视图,所述系统还配置成以半透明形式显现所述左心室,并以不透明形式显现所述几何标记。
12.如权利要求1所述的系统,其中为所述心外膜起搏电极在左心室壁上的至少一个放置位置确定适当部位,所述系统还配置成显现所述左心室壁上的所述适当部位。
13.如权利要求12所述的系统,其中所述患者具有胸壁,且所述系统还配置成确定和显示从所述胸壁到所述左心室壁上所述适当部位的距离。
14.如权利要求13所述的系统,其中所述心脏具有心窦,且所述系统还配置成估算和显示左心室起搏引线在向下转入适合起搏的分支之前在所述心窦中必须经过的距离。
15.一种用于辅助计划对患者心脏的介入式两心室起搏操作的计算机系统,所述患者心脏具有左心室、左心室心肌、心肌壁、左心室壁,且心动周期具有多个期,所述计算机系统具有处理器、存储器和显示器,并且所述计算机系统配置成:
通过分段所述患者心脏的图像数据集来提取所述患者心脏的所述左心室和所述左心室心肌的面;
利用分段的图像数据集将所述左心室分成心肌分段,或分成多个短轴层面;
相对于参考期,检测在所述患者心脏的心动周期的期之间每个心肌分段或短轴层面的壁运动;
定位最近达到最大位移的区域和最近达到最大速度的区域;以及
生成2D或3D透视图,包括指示收缩的时间延迟、最大位移或最大速度中至少一项的透视图;
为心外膜起搏电极在左心室壁上的至少一个放置位置确定适当部位。
16.如权利要求15所述的计算机系统,还配置成利用3D面透视图或体透视图中至少一项来显现所述患者的胸壁、左心室壁、左心室血管和心外膜脂肪。
17.如权利要求15所述的计算机系统,其中为利用所述分段的图像数据集将所述左心室分成分段或多个短轴层面,所述计算机系统配置成将所述左心室分成基底段、中间段和顶尖段。
18.如权利要求15所述的计算机系统,其中为利用所述分段的图像数据集将所述左心室分成心肌分段或多个短轴层面,所述计算机系统还配置成将所述心肌壁的短轴层面分成多个带和箱。
19.如权利要求15所述的计算机系统,其中为生成3D透视图,所述计算机系统还配置成将所述患者的心窦及其分支、冠状动脉及它们的分支以及脂肪显现在所述心脏的半透明3D模型上,以便于将引线放置到最接近主要血管的区域上。
20.如权利要求15所述的计算机系统,还配置成标识所述左心室心肌的坏死组织,并且其中为生成3D透视图,所述计算机系统配置成将所标识的坏死组织显现在所述左心室的3D模型上,以便于避免将引线放置到无效区域上。
21.如权利要求15所述的计算机系统,其中为生成3D透视图,所述计算机系统还配置成在所关注界标处将几何标记插入到显现的体中。
22.如权利要求15所述的计算机系统,其中为所述心外膜起搏电极在左心室壁上的至少一个放置位置确定适当部位,所述系统还配置成显现所述左心室壁上的适当部位。
23.如权利要求22所述的计算机系统,还配置成确定和显示从所述胸壁到所述左心室壁上所述适当部位的距离。
24.如权利要求22所述的计算机系统,还配置成估算和显示所述心外膜起搏电极的引线在向下转入分支之前在所述患者心脏的心窦中必须经过的距离。
