Abstract

一种用来装入病人胸腔内的心脏助动器具有一个两层壁的外套(10)可以至少把心脏(H)的心室部分包住,外套的内壁(11)至少是可以弯曲的,能够同心脏的外表面相紧贴在一起并且与心脏壁一起活动,外套内外壁之间的空间(13)是封闭的并且充满流体,流体可以在这个空间内任意流动。

Description

本发明是关于一种用于体内的心脏助动器。更具体地说,本发明是一种心脏助动器用来装在病人的胸腔中,至少将病人心脏的心室包住。
如Lundback.s1986年在斯德哥尔摩发表的论文“心室中隔膜的心脏抽吸功能”中所宣称的,人体心脏的抽吸和调节方式是与通常流体的观点不相同的。
根据上述论文所讲的,健康的心脏在完成抽吸动作的时候,其外形和容积并没有实质性的变化。
更具体地说,当心脏收缩(心脏动作周期中的排出期或能动期)时,那个所谓的瓣膜,即包含心房室瓣的膜片将被拉向心尖部分,迫使心室中的血液进行肺循环(小循环)的体循环(大循环),并且与此同时,由于心房室瓣的运动,使血液进入心房中。
当心脏舒张的时候,即心脏动作周期处于心脏肌松弛的时候,心房室瓣膜片在动量的影响下返回到其原始位置上,当心脏收缩时由于瓣膜向下运动的结果而有动量作用到流入的血液上。
在公开发表的论文中还说明(根据心脏动作周期中心脏外形和容积基本上保持不变的情况),心脏改变右心室和左心室相对体积的能力仅仅是由于其共同的心室壁,心室中隔膜,也就是借助于其在心脏松弛状态下的弯曲性,当心脏收缩时,心室中隔膜和左心室肌的其余部分总是处于一种圆断面形状,并且处于一个截然不同的位置,而与心脏舒张时的形状和位置无关。这是因为当心脏收缩时,左心室中的 压力总是大于右心室中的压力。如果心室中隔膜在心脏舒张时的形状和位置(即心脏松弛时的形状和位置)不同于心脏收缩(即能动)时的形状和位置,则作用象薄膜泵的心室中隔膜必然会给予一个心室以较大的行程容积,而给另一个心室以相应的较小的行程容积。这样一来,心室中隔膜就完成着一种双向的调节作用,以保持血液循环的两个分枝(肺循环和体循环,或小循环和大循环)之间的平衡。
心脏的这种工作和调节方式具有很多优点,由于心脏的这种动作方式也就回答了有关心脏生理学的问题,这个问题是迄今尚未得到一致的答案。
由于心脏能够在外形和容积不变的情况下实现其抽吸和调节功能,因此心室中隔膜就必须能够做到不比左心室肌的其他部分有本质上的减小,就象在心室中隔膜出现梗塞时的那种情况一样。由于出现这种破坏,当心脏收缩时,心室中隔膜就不能承受左心室中的压力,同时也无法完成其适当的调节功能。因此,心室中隔膜将屈服于左心室中的压力,使心脏的右侧产生一个不正常的增大的行程容积,而心脏的左侧则行程容积相应减小。这样的结果是,打入肺循环(小循环)的血液多于进入体循环(大循环)的血液。这样,由于心室中隔膜失去或削弱了它的调节功能,又会使血液积存在肺部引起肺水肿,这种情况是有生命危险的。
不过,通过在动物胸腔打开时心脏工作的试验,也就是心脏在空气环境时所做试验的结果表明,心室中隔膜遭受到模拟梗塞的情况并没有引起上面所讲的那种致命的结果。
在做试验的那种条件下,心脏是以一种不同型式的活塞泵的方式进行工作,也就是说,这种泵是在外形和容积变化的情况下进行工作。 这也意味着,当心脏舒张时,心室可以改变其容积,不仅是由于心室中隔膜的位移,也是由于心室壁其他部分的位移的结果。当心脏收缩时,心室可以得到不同的行程容积而不会太多地相互影响。这种抽吸和调节方式与人体内心脏工作方式的普通看法是一致的。
本发明的主要观点是认识到处于人体内的心脏的工作方式不同于暴露的心脏,即不同于由空气环绕的心脏,另外,在前面讲过的心室中隔膜严重梗塞可能会导致突然性的血液循环失调,而暴露在空气中的心房,模拟的心室中隔膜梗塞并不会引起致命的血液循环失调。
按照本发明提供的心脏助动器,可以装在病人的胸腔内,构成一个外套至少把病人心脏的心室部分包住,这种心脏助动器的特征将在权利要求第一项中说明。
正如后面所要详细介绍的那样,按照本发明的助动器是用来在胸腔内将心脏的至少心室部分依从的支撑住。由于有了这种依从的支撑,封闭在病人胸腔中的心脏就可以随着心脏的搏动有节律性地改变其外形和容积。
根据本发明的心脏助动器主要是在病人心脏的心室中隔膜出现梗塞的情况下用来进行治疗的,因此它不能实现其正常的调节功能来保持肺循环和体循环(即小循环和大循环)之间的平衡。
按照本发明的装置,外套可以允许与外套内壁相接触的左心室的那一部分外壁向外移动,因而增加左心室中舒张期的充血压力。所以,左心室不仅可以通过心房中隔膜向右心室的移动来增加其行程容积,而且可以通过心室壁的其他部分的向外移动来增加行程容积。
左心室壁的向外移动以及所引起的充血和行程容积的增加都会使得外套的内壁产生向外壁的移动,而外套的外壁受到病人胸部相邻组 织的相当牢固的支撑,也就是受到病人心脏所在的那个空腔的限制。结果,外套中的流体就会发生流动,因此就会由于右心室部分外壁向里移动等原因而使右心室的充血和行程容积减小,右心室的外壁是同外套相接触的。这样一种相互作用就可以起到平衡心室排出量的功能。
换句话说,根据本发明的装置,心室中隔膜的调节功能转移给了左心室的一部分外壁,左心室外壁与外套的内壁相接触。
外套内装的流体或者至少其中一部分最好是可以压缩的。为了便于添加或抽出外套中的流体,挠性导管的一端与外套的中间空间相连通,而其另一端则装一个可以自行密封的终端相连,这样就可以用注射器将流体通过这个终端装置加进去或在从外套中抽出来,这个终端放在病人皮肤下面最为适宜。
在本发明范围内也可以在外套上安装一个膨胀器与外套的中间空间相连通,同时也可以包括一个用来控制外套中流体压力的装置。
只将心脏的心室部分包住的那种外套在实际运用中可能已经足够,它的优点是结构简单和易于装到心脏上。如果采用那种把心脏整个包起来的外套就可以得到功能方面的优点。比如,可以通过改变包住心脏心室部分的那一部分外套的体积,使心脏的心房体积在与心室相反的方向发生变化。
外套可以做成单个的或整块的结构,它可以只包住心室部分或在把心脏整个包起来。另外,也可将外套做成两部分或几部分,用它们包在心脏的周围。
如果外套是把心脏整个包住,那么外套的边缘就要在适当的位置上形成一些凹处,以便为血管留出引导通道,使血管能以与心脏相 通。
如果外套是由两部分或几部分组成的话,各部分外套所包围的密封空间可以相互连通让流体能够在外套各个部分之间流来流去。
当外套装入病人身体内的时候,最好是用一种适当的材料来加强一下心室中隔膜。同时这种加强最好是在朝右心室的那一面进行,这样就可以防止加强材料所产生的任何栓柱进入体循环(大循环)系统中。
附图所示是按照本发明的两个实施例以及用这种装置来支撑病人心脏的使用情况。
图1所示是一个实施例的断面图,在这个实施例中,心脏助动器的外套只把心脏的心室部分包住。
图2所示是将心脏整个包住的心脏助动器外套这一实施例的断面图,在这种外套上有一些开口或引导通道以便让血管心脏相互连通。
图3所示是图2中的实施例的顶视图,表明外套是由两半或两部分组成,他们把心脏包围在中间。
参照这些附图,特别是图1和图2,病人心脏H所在的胸腔壁用T表示。图上也可以看到上腔静脉S和下腔静脉I,它们都与右心房RA相通,而右心房又通过右房室瓣RAV与右心室RV相连;肺动脉P通过肺动脉瓣PV从右心室RV得到血液;肺静脉V与左心房LA相通,而左心房通过左房室瓣LAV与左心室LA相连;主动脉A通过主动脉瓣AV从左心室LV得到血液;两个心室中间有心室中隔膜W相隔。
在胸腔壁T与心脏H之间装入本发明的心脏助动器的象膀胱一样的外套。在图1中这个外套用10表示,而在图2和图3中是用 10a来表示。
现在参看图1,外套10呈碗状,其里面的形状与心脏H的心室部分的形状相适应。外套的内壁11是用柔软的可与生物相配的薄膜来制做,最好是完全不透射线或部分不透射线的材料,这样的材料可以与心脏的心室部分外形很好地服贴,具有相同的形状。因此,外套的内壁11能够同心脏的心室部分的外壁一起移动。
外套的外壁12最好也是用柔软的可与生物相配的薄膜制做,这样,当外套10装到病人胸腔中以后,外套的外壁12就可以与胸腔壁T的形状相一致。
在外套的内壁11和外壁12之间的密封空间13中充以适当的流体,如图上打点的部分所示,这种流体可以是气体或液体,也可以一部分气体和一部分液体。流体的至少一部分最好是气体。
外套内装的流体使内壁11和外壁12分离开,至少在内壁11与心脏H相接触的那个范围内,大部分是分离开的,这样,心室RVLV的外壁就可以朝外套外壁12的方向移动,也可朝相反方向移动,胸腔壁T也是这样。
除非外套的内壁11和外壁12的材料本身具有摩擦小的性能,就需要在外套内装的流体中加入适当的润滑剂,这个润滑剂可以是液体也可以是液体悬浮物,或者在内壁11和外壁12的表面上涂以适当的润滑剂,这样,在外套的使用中,当内外壁靠在一起的时候,内壁11就能够容易地在外壁12上面滑动。
如果与密封、不透气的外套10相连的是挠性导管14,通过这个导管将中间空间13与一个密封不透气而又能刺穿的膨胀器15相连通。如图所示,这个膨胀器具有入口阀16和出口阀17,分别呈 止回阀的形式,并且随着预先设定的膨胀器和中间空间13之间的压力差进行开闭。不过,这两个阀也不是非要不可的。
如图1所示,膨胀器15是一个口袋或者气囊,也可以被压缩也可以膨胀起来,以改变其体积。
由于心脏的心室部分得到内壁11的依从支撑(如果至少有一部分流体是可压缩的话,这种支撑就具有弹性),心脏H的心室壁可以不受什么约束地移动,因此心脏便可以按照上面所讲的那种方式实现其抽吸作用,这就是说,心室RV,LV之间的相互作用不仅是通过心室中隔膜W的中间部分,而且是通过心室壁的其他部分,当心室中隔膜W出现破损的时候这是很必要的,因为这样才可以保持心脏的调节功能。膨胀器15的尺寸大小可以在中间空间13的容积发生变化时,不致引起不适当的过大的压力变化。
图2和图3中的外套10a与图1中的外套10不同之处在于它象一个囊体把整个心脏H都了起来。为了便于将这个外套装到心脏H上,外套由两部分10a1和10a2组成,这两部分合并或组合起来包在心脏的周围,并且用适当的紧固件18相互连接起来,如图3所示。在这两部分的边上形成凹处16a1,16a2,当这两部分被安装在心脏的周围以后,这些凹处部分便可以相互结合或者紧靠在一起。这些凹处的位置和形状是这样考虑的,它们可以为上腔静脉S,下腔静脉I(图上未示),肺动脉P,肺静脉V和主动脉A形成可以引导通过的通道。
不同部分的中间空间13a都是密封不透气的,但同时又能够通过适当的导管19互相交通,或在被做成可以相互交通的,使流体能够在这两部分之间流动。
从图2可以看得很清楚,心室RV,LV的外壁向里移动将会增加外套10a与心室相接触的那一部分的体积,而使外套与心房RA,LA外壁相接触的那一部分的体积相应减小。这种体积减小和所引起的外套内流体的移动,将使心房的外壁向外移动,也就是使心房的体积有所增加。同样,心室外壁的向外移动将会引起心房的外壁向里移动,这种相反方向的移动可以改善心脏的抽吸作用。
在图2所示的实施例中没有配备膨胀器装置,而是在挠性导管14的外端上安装一个可植入的终端装置20,这个装置具有一个可以刺穿而又能自行密封的隔膜21,在必要的时候可以通过这个隔膜将中间空间13a中的流体抽出去或者注进来。即用一个注射器将注射针头插入隔膜进行这种操作。当然,也可以采用与图1中的膨胀器15相对应的膨胀器来代替这种终端装置20,或者在终端装置以外再装一个膨胀器。同样,在图1所示的实施例中也可以用对应于终端装置20的一个终端装置来代替膨胀器15或者在膨胀器以外再加一个终端装置。
在第三个实施例(图上未示)中,也是用外套将整个心脏包住。在这个实施例中,外套包括一个与图1所示外套10相同的下面部分(具有膨胀器15或者不带这种膨胀器)和一个上面部分用来把心脏的上部或心房部分包住。外套的上面部分又由两块组成以便于将其装到心脏上,从上面看去,这一部分就相似于图3中的外套10a。第三个实施例的断面图差不多和图2所示的一样。
如果愿意的话,第三个实施例的上下两部分可以安装中间导管,以便让两部分内的流体相互交通。不过,这种交通也可以省去,不要也行。
必须指出,根据本发明的所有的实施例都有一个不透气的密封系统,流体在其中间可以流动,在使用这种装置来保持心脏调节功能(正常情况下是由心室中隔膜来完成,但当心室中隔膜出现梗塞的时候就转给心室壁的其他部分来完成)的时候,没有从身体以外通过外套施加给心脏任何外力。这就是说,按照本发明的这种装置区别于那种通过外力来压迫心脏的装置,比如,为了减少心室的体积或模拟心脏肌而采取的办法。

Claims (8)

1、一种用来装入病人胸腔内的心脏助动器,它包括一个外套(10,10a)具有可以相对移动的内外壁,内壁采用柔性的薄材料,并且至少可以适应贴附在心脏心室部分的周围,内外壁之间有封闭的空间(13,13a)并有可流动的流体,其特征在于所述心脏助动器是被动型的,而所述封闭空间(13,13a)包括一个左心室部分和一个右心室部分,这些部分相互间有流体流动,以至其中一个部分中的流体在该部分受到压缩时可向另一个部分中流动。
2、根据权利要求1的助动器,其特征在于所述外套(10a)是用来将整个的心脏(H)都包住。
3、根据权利要求1或2的助动器,其特征在于所述外套(10a)由至少两部分(10a1,10a2)组成,这两部分连接起来形成一个囊体包在心脏(H)的周围。
4、根据权利要求3的助动器,其特征在于所述每一部分(10a1,10a2)的内外壁之间的空间(13a)是封闭的,同时又与另一部分或其他部分的每一部分的空间进行交通。
5、根据权利要求1的助动器,其特征在于所述外套(10,10a)的内外壁(11,12)之间的空间(13,13a)与一个膨胀器(15)可以进行流体的交通。
6、根据权利要求5的助动器,其特征在于所述膨胀器(15)的体积是可以变化的。
7、根据权利要求1的助动器,其特征在于所述流体中的至少一部分是可以压缩的。
8、根据权利要求1的助动器,其特征在于有一根挠性的导管(14),其一端与外套(10a)相连接,而另一端装有一个可以刺穿并且有自行密封装置(21)的终端装置(20),可以通过这个密封装置向外套注射流体或从外套中抽出流体。
