技术领域

背景技术

通常,在源或者在将源连接到伤口敷裹物的导管线中,通过简单地测量减压源提供的减小的压力的水平来表征组织位置处的压力水平是不可能的。与RPWT系统相关联的管的主腔内的流体流动防止在装置处使用压力水平测量,以免在组织位置本身处精确地指示压力的水平或稳定性。因此需要用于直接监控伤口压力水平的其他方法。

发明内容

减压适配器
公开了RPWT系统中的改进,即提供减压适配器,以尤其是结合低型敷裹物来改进操作的可靠性,并防止或减少液体意外进入减压传输管的测量腔中的情形。传统的适配器典型地包括感测腔和减压传输腔以及肘形外壳。这些减压适配器失败的普遍原因是由液体进入感测腔所致,这可引起治疗的控制变得不稳定,且在极端情况中,可促使装置最终关闭。通常,该问题部分是由于流体输送的位置和伤口流体的性质。伤口排泄物和流体的稠度(consistency)通常是假塑性的,且在减小的压力的影响下将在减压装置的肘中喷溅和起泡沫。因此,本发明某些示范性实施方式的一个目的是防止伤口液体和其它非气态流体进入感测腔。
改进的减压适配器的一个概念是腔被向下分离到伤口敷裹物的分配歧管,使得分配歧管变成感测腔和减压路线之间的障碍物。减压适配器的下侧设置有通道部件,该通道部件在高速流动期间或在大部分液体已经被推至管中后在减压适配器内吸引少量喷溅滴。液体和固体物质偏好进入较大的主腔,并离开较小的辅助测量腔,这有助于防止腔受到阻塞。该概念进一步包括在垫中设置偏移的外部测量腔端口,其设计成使得在大多数方向中,两者之一应在敷裹物分配歧管中的流动水平之上。减压适配器进一步用来结合下面更详细描述的动压力控制方法操作。减压适配器还可包括将减压适配器固定在伤口内的粘性消毒盖布或覆盖物。减压适配器的外形较低,以增加病人的舒适性,且减压适配器优选肘形,使得连接管按路线完全地离开组织位置。
为了概括地描述包括在结合了本发明示范性实施方式的改进元件的减压伤口处理(RPWT)系统中的部件,首先参考图1。RPWT系统10的三个主要部件包括伤口敷裹物12、减压传输管14和远程流体容器和装置(remote fluid containment and instrumentation)16。
伤口敷裹物12通常包括分配歧管24,例如多孔垫或粒状泡沫,以及在组织位置处固定分配歧管的覆盖物或消毒盖布26。敷裹物12还可包括改进的减压适配器22,如所示设置在分配歧管24上的,且所述减压适配器22通过设置在减压适配器22上的粘合剂粘到分配歧管24,伤口消毒盖布26或单独的粘性消毒盖布与减压适配器22相关联。
减压传输管14是多腔管,包括作为组装结构的一个或多个管部分28,在减压适配器22和位于流体容器18上的容器连接器34之间提供连续的导管。如将在下面更详细地描述的,且和本领域已知的一样,由RPWT系统10排出的液体和其他渗出物从此处的管移除,并保留在容器18内。装置管36a和36b形式的额外的管的部分也从容器连接器34延伸到装置部件20。在本发明的某些实施方式中,装置部件20包括减压源38和压力监控装置部件40a和40b。这三个装置部件20中的每个都与从减压适配器22延伸到远程流体容器和装置16中的三个独立的导管(管或腔)之一单独地相关联,这将在下面将详细地描述。
为了更详细地描述减压适配器22,现参考图2-9。图2提供了减压适配器22的下侧的透视图,显示了减压适配器22的开口内的各种结构元件,这些元件适合于接触伤口敷裹物的分配歧管24(未显示)。
减压适配器22通常包括可粘到分配歧管的底座50和连接到底座50的导管外壳62。导管外壳62包括主导管和一对辅助导管。底座50包括孔53,该孔53定位在分配歧管上,且液体和气体(共同地称为“流体”)通过该孔53从组织位置抽出。改进的减压适配器22的重要特征是设置在孔53附近且与孔53流体相通的通道元件(channel element)的存在,以及通道元件将液体引导入主导管以排出的有效方式。液体进入主导管的路径维持系统的辅助导管敞开,以用于压力测量目的。
参考图2,减压适配器22的导管外壳62包括界定进入表面55的凹进区域54。主导管在主端口60处终止于进入表面55,而主端口60中心地定位在凹进区域54的顶点处。辅助导管在辅助端口56和58处终止于进入表面55。辅助端口定位在孔53的直接相对的边缘附近。
主导管的第二端在主腔接口64处终止。主腔接口64通常中心地定位在孔66内。辅助导管的辅助腔接口48、49(见图5)也定位在孔66内,且在下面将更详细地描述。
图3提供了减压适配器22的平面图(从上面看)。导管外壳62优选“肘”形;但是,导管外壳可以任意期望的角度构造,或可从底座50垂直地延伸。在如图3所示的肘构型中,所示的减压适配器22包括底座50和中心地定位的导管外壳62。导管外壳62包括肘区域68,且导管外壳62内部包括在端口56、58、60之间的导管和辅助及主腔接口48、49、64。
图4、图5及图6显示了减压适配器22的侧视图和端视图。如图4所示的侧视图中所示,减压适配器22是低型构型,且底座50界定其横向界限。如上面所示,底座50可直接粘到分配歧管,或可使用伤口敷裹物的消毒盖布定位及粘附。减压适配器22定位在分配歧管上,使得底座50的孔53(在此视图中不可见)与分配歧管直接接触。在图4所示的视图中,主腔接口64从导管外壳62中心向外延伸,且被孔66围绕。导管延伸通过管接口和凹进区域54之间的减压适配器22的材料,如前面所述。肘区域68以允许系统放置在伤口敷裹物上且保持伤口敷裹物表面闭合的方式,将流体流从定位在减压适配器22下面的伤口敷裹物重新引导到与接口64关联的角落。
图5是与图4所示相同结构的端视图,其中更清楚地显示了肘区域68的构型和导管外壳62的内部构型。在此视图中,公开了与将减压适配器22粘到伤口敷裹物相关联的相同部件。底座50和孔53如图4所示定位。导管外壳62被显示为其定位成接收用于连接的管部分,以平衡本发明系统。
导管外壳62的内部是主腔接口64和辅助腔接口48、49。辅助腔接口48和49通过在主腔接口64上的管中放置主腔而与传输管中的相应腔对齐。在下面将更详细地描述与本发明改进的减压适配器结构一起使用的多腔管的一种实施方式的结构。
图6实质上提供了与图4一样但从相对侧看的减压适配器22的相同视图。显示的元件结构上与图4显示和描述的元件相同,其显示了连接器的横向(以及一定程度上,径向)对称。除非以其他方式显示,用于构造本发明改进的连接器的材料可选自在本领域已知的大量材料,这样的材料为病人提供必要的柔性和舒适性,同时维持足够的刚度或弹力,以维持对于减压适配器功能是整体的敞开的腔。
图7提供了减压适配器22的平面视图(从下面看),且阐明了凹进区域54内各种部件和元件的结构和功能,这些部件和元件起到优先选择液体和其他气态流体离开辅助端口56、58的作用。在此视图中,底座50显示为环绕凹进区域54的边缘。辅助端口56和58显示为按所示定位,其中关联的导管从端口56和58向内延伸到辅助腔接口(在此视图中被隐藏,且未显示)。主端口60被显示为在孔53内中心地定位。主导管从主端口60延伸通过主腔接口64(在此视图中被隐藏,且未显示)。凹进区域54内特定的结构起到将液体引流到主导管中的作用,并由此允许辅助导管保持畅通无阻,这参照图9在下面更详细地描述。
图8显示了与本发明减压适配器关联的底座的可替换的示例性实施方式。在此视图中(与图7相同的透视图),显示了在可替换的底座52下侧表面的添加部件。模制到底座52结构中的这些部件包括底座锯齿状引导通道70、周界收集通道72和中间收集通道74。这些通道的目的是引导液体离开两个辅助测量端口56、58,并进入主端口60。底座锯齿状引导通道70在底座50上定位并定向,以直接地捕获并引导被引进减压适配器中的液体的至少一半,并间接地引导被引进的其余液体的主要部分。底座锯齿状引导通道70的间隔的径向定向布置使液体集中离开辅助端口,并进入主端口。此外,周界收集通道72和中间收集通道74将引进径向定向引导通道70之间的液体流重新引导到引导通道70中,并离开辅助端口。该重新引导的流动的示例在图8中用粗体流动指示箭头显示。
现参考图9,更详细地描述了包括在导管外壳62的凹进区域内的部件和元件。这些部件定位在凹进区域54的进入表面55上,且被构造成优选液体和其他非气态渗出物离开辅助端口56、58,并进入主端口60。在此视图中,主端口60显示为中心地定位在凹进区域54内,并从中心位置延伸到凹进区域54的一侧。辅助端口56和58也在此视图中公开,被定位到主端口60的中心位置的任一侧。在此视图中,辅助端口56和58是圆形开口(每一个具有上升的圆周边缘),其朝通向延伸到关联的辅助腔接口(未显示)的内部导管的引流点延伸。可看到导管开口在辅助端口56和58的边界内。
图9所示结构内的四个基本部件定位成优选液体进入减压适配器22的主端口60。当减压适配器22定位在分配歧管表面上时,第一此种结构将辅助端口56和58简单放置在接近分配歧管表面的平面处孔53的周界附近。换句话说,当减压适配器22定位在伤口敷裹物上时,辅助端口56和58与分配歧管表面接触,或接近接触。采用这种方式,直接引导进入这些端口的喷溅或搅动液体的可能性是最小的。
引导液体进入主端口的三种其他部件是形成在凹进区域54的进入表面55的各个部分上的结构锯齿状通道。第一线性锯齿状通道部分42定位成与凹进区域54的近似半圆部分关联,其与辅助端口58相关联。包括凹进区域54这部分的顶部的材料覆盖并包含在辅助端口58和其接口(未显示)之间延伸的导管。顶部或壁构造成具有引导液体的一列锯齿状通道或条纹,液体落在该表面上,朝向凹进区域54中心处的主端口。被引入开口并落在进入表面55此部分上的任何液体将被直接引到主端口60中,而不是被引入辅助端口58中。
相似的构型被构造在近似三分之一圆形的径向锯齿状通道部分44。在没有内部导管被包含在凹进区域54的此部分中的情况下,部分44中的锯齿状通道可更深地延伸,且更直接地延伸至主端口60。这些径向锯齿状通道被从孔53的周界朝向引入主端口60的凹进区域54的顶点引导。这些径向条纹或通道从邻近辅助端口58的半径延伸,径向围绕约三分一圆到达邻近辅助端口56附近的半径。落在凹进区域54此部分上的任何液体将被中心地引导到主端口60,而不是被引导到辅助端口的任一个。
最终,在辅助端口56逼近主端口60的位置处支撑辅助端口56的壁部分被构造有锯齿状或条纹状通道46,其从辅助端口56的开口向主端口60的开口向下(在连接器的法向定位中为向上)延伸。
如上所述,凹进区域54的各种内部部件和元件被构造成将液体从凹进区域54内的大多数位置向中心定位的主端口60引导。只有直接进入辅助端口56或58的液体将可能被引入辅助腔。在这些端口很少或没有吸入发生的情况下,该结构大大减少辅助腔中液体形式的阻塞或材料阻塞的可能性。
减压传输管
现参考图10和11,进一步详细地描述了可结合本发明示例性实施方式的系统操作的改进的减压传输管80的结构。减压传输管80优选包括中心主腔82和辅助腔84、86。辅助腔84和86通常用于进行压力测量。在图11中,由实体箭头指示的流体流动被显示为其被指引通过主腔82,而辅助腔84和86通常保持没有液体或任意非气态物质。图10和11所示的截面透视图公开了与辅助腔84和86对比的主腔82的相对截面直径。传输装置80具有椭圆形截面,其使柔化达到最优,而不允许描述的任意腔损坏。该截面形状也定向辅助腔84和86,使得腔与上述改进的减压适配器上的接口大致对齐。
测量伤口压力的动力方法
本发明的系统也包括一种测量伤口压力的改进和动力方法,其克服了目前的减压伤口处理控制系统存在的固有问题。本领域已经发展了各种方法来控制减压伤口处理产品和系统的操作,以便确保维持伤口压力,并确保通过指定方法的有效操作治疗是安全的。目前,用一起共通并连接到一个压力传感器的多腔管的外部腔或腔测量伤口压力。如果液体进入腔内或者它们被阻塞,该结构可能产生一定问题。如果发生这样的液体侵入或阻塞,该系统可能变得不稳定,并发出压力变得不可靠的相关警报或指示。已经尝试了这些问题的各种机械补救方法,且一些已经部分成功。但是,最终,例如在已有技术中描述的系统将遇到液体在控制腔中的问题,除非设置抵抗液体进入测量腔的物理屏障。本发明的一个目的是当遇到治疗困境时一种比目前单个传感器测量腔系统更可靠且更坚固的系统。
参考图12,其中公开了本发明示例性实施方式的系统和其部件的功能关系。该系统包括在系统装置中的两个伤口压力传感器40a和40b,这两个伤口压力传感器40a和40b单独地(通过不连续的腔或导管)从装置延伸到减压适配器,并且直到不连续的腔在减压适配器和分配歧管的接口处结合才共通。如上面所述,减压适配器包括两个单独的压力感测端口和通过流体室18到达系统装置中的减压泵38的流体线路。在系统装置内部,每个辅助测量腔导管装有螺旋形阀92和94,阀92和94将在治疗结束时、治疗间歇期间或如果需要清除阻塞物时,缓解伤口压力。这些阀以及与减压源38关联的相似的阀由微处理器/控制器90控制。微处理器/控制器90也控制减压泵38的操作,并从第一和第二压力测量装置40a和40b接收数据。微处理器/控制器90的程序设定成通过与两个辅助腔路线关联的两个读数监控伤口压力。在液体进入腔之一的情况下,液体将引起该腔与无阻腔的响应时间的压力变化的延迟。当阻塞变得更加剧烈,延迟也将变得更加剧烈。当探测到延迟时,该系统将根据敞开腔的伤口压力来控制伤口压力,并将尝试通过将适当的阀向大气敞开来将液体从阻塞腔中清除。优选程序将尽力以这种方法多次清除阻塞物。如果系统没有成功地清除阻塞物,该程序将从那点开始,忽略受影响的腔,并用剩下的无阻腔控制系统。如上述,本发明的减压适配器设计这样设计以便在任意给定时间具有至少一个无阻辅助测量腔的机会最大化。
具有旋转功能的减压适配器
目前,在RPWT系统中,减压适配器通常允许将减压伤口治疗有效地连接到伤口,但是不允许重新设置管连接点(例如,在病人皮肤容易破损的情况下),或者不允许使用者错误地定位减压适配器的情况(例如,在将减压适配器朝向错误方向的情况下)。在这些情况中,使用者可移除并丢弃减压适配器,而在某些情况下,可移除并丢弃引起不舒服且使病人和使用者讨厌以及引起额外花费的消毒盖布。对减压适配器提供旋转或回转功能使得重新定位管成为可能,而不需要移去和重新定位减压适配器。该能力有助于其中管需要重新定位以避免组织损伤的任意情况。本发明的一个目的是提供一种减压适配器结构,该减压适配器结构允许管的轻易重新定位,而不移除并重新定位减压适配器或伤口敷裹物。
参考图13和14,描述了减压适配器结构的可选择的优选实施方式的构型。图13中显示的减压适配器110使用了硬塑性内芯,该硬塑性内芯形成支撑表面,以使橡胶O-型圈能够贴着其密封,且也使得支撑表面以相对较低的摩擦力滑动通过。起到保护和缓冲覆盖物作用的软热塑性或弹性聚合物结合到硬塑性内芯。图13和14显示了各种圆形圈部件,其一起组成本发明的回转连接。顶部旋转PVC部件112覆盖顶部ABS插入圈114,插入圈114本身被橡胶O-型圈116围绕。底座ABS插入圈118显示为保持O-型圈116束缚在插入圈118和顶部ABS插入圈114之间。然后,这些圈中的每一个装配在底座PVC圈120内,底座PVC圈120与减压适配器的底座和/或与伤口敷裹物自身接触。
如上述与减压适配器关联的内部部件和元件连同非旋转实施方式在这里被同等地应用,且可通过引导部件的模制或通过将模制的插入物定位在外壳内以用于旋转部件112而结合到顶部旋转PVC部件112的内部结构中。在任何情况下,上述围绕腔端口的液体优选结构的相同好处在于可获得描述的可选择实施方式的旋转功能。
图14公开了与上面描述相同的部件,他们被组装,且由此以更清楚的细节显示了部件彼此互锁和相互旋转的方式。在此视图中,束缚的O-型圈116也被显示为由减压适配器110形成的内部减压室提供合适的密封。在此视图中,也清楚地显示了在端口开口中的内部部件和元件如何被合适地定位在顶部PVC部件112的下侧上的,以起到如上述优选液体进入主导管的功能。
具有微生物指示的减压适配器
目前的减压处理系统通常不警告照顾者伤口敷裹物中微生物的存在。这些微生物中的许多可能是控制伤口位置感染的重要因素。本发明的一个目的是提供一种警告照顾者在有氧、无氧、革兰氏阳性及革兰氏阴性分类下关键微生物的重要水平的系统。响应或指示是离散地反映上述四种分类的颜色类型的形式。
在某些实施方式中使用安装在减压适配器或其关联的消毒盖布上的挥发性有机化合物(VOC)敏感带。当暴露到已知的目标VOC以在有某些微生物的情况下形成时,颜色类型变得明显,并由此确定存在于伤口流体中的微生物的类型。参考图15和16,减压适配器122包括与先前参考图2-9所描述的相似的底座124和导管外壳130。在导管外壳130的凹进区域126中的VOC敏感板128的定位被显示。在图16中,可以看出,减压适配器122由材料给定的半透明或透明特征构造,当减压适配器122处于适当的位置,而底座124位于贴着伤口敷裹物的分配歧管的位置时,VOC敏感板128甚至可从外部视觉检查。因此,该VOC敏感板128的优选位置如所示在凹进区域126周界附近的凹进区域126内。图15更清楚地显示了该VOC敏感板128在凹进区域126内的放置,此处它持续地暴露到从伤口引进的流体材料。只要存在伤口流体的充分暴露,可选择的板的放置是可能的。

Claims (18)

1.一种用于将减小的压力应用到组织的系统,所述系统包括:
多腔减压传输管,其具有近端、远端、从所述近端到所述远端延伸通过导管的主腔和从所述近端到所述远端延伸通过所述导管的辅助腔;
真空泵,其连接到所述主腔的所述近端;以及
减压适配器,其连接到所述减压传输管的所述远端,所述适配器具有通道,以引导液体离开所述辅助腔,并进入所述主腔。
2.如权利要求1所述的系统,其中所述减压适配器包括:
导管外壳,其具有界定进入表面的凹进区域;
主导管,其通过所述导管外壳,在所述进入表面上与所述主腔和主端口流体相通;
辅助导管,其通过所述导管外壳,在所述进入表面上与所述辅助腔和辅助端口流体相通;且
其中所述通道定位在所述进入表面上,以引导液体离开所述辅助端口,并进入所述主端口。
3.如权利要求2所述的系统,进一步包括连接到所述导管外壳的底座。
4.如权利要求3所述的系统,其中所述导管外壳可关于所述底座旋转。
5.如权利要求3所述的系统,其中:
所述导管外壳可关于所述底座旋转;且
所述辅助端口定位成基本上与所述底座齐平。
6.如权利要求2所述的系统,进一步包括:
底座,其连接到所述导管外壳;以及
径向引导通道,其定位在所述底座上,以从所述底座的外围引导液体离开所述辅助端口。
7.如权利要求2所述的系统,进一步包括:
压力传感器,其连接到所述辅助腔的所述近端;
阀,其连接到所述辅助腔;以及
控制器,其连接到所述压力传感器、所述阀和所述真空泵,所述控制器被设定程序以:
操作所述真空泵来减小所述主腔中的压力;
从所述压力传感器接收压力数据;
响应所述真空泵操作的相关变化而监控压力数据的变化;
鉴别所述辅助腔是否具有延迟的压力响应;以及
操作连接到所鉴别的所述辅助腔的所述阀。
8.一种用于将减小的压力应用到组织位置的系统,所述系统包括:
多腔减压传输管,其具有椭圆形截面、近端、远端、从所述近端到所述远端延伸通过导管的主腔和从所述近端到所述远端延伸通过所述导管的多个辅助腔;
真空泵,其连接到所述主腔的所述近端;以及
减压适配器,其连接到所述远端,所述适配器包括:
导管外壳,其具有界定进入表面的凹进区域;
主导管,其定位在所述导管外壳内,且终止于中心地定位在所述进入表面上的主端口;
辅助导管,其邻近所述主导管定位,且终止于位于所述进入表面上的辅助端口;
线性锯齿状通道部分,其沿所述进入表面的约一半;
径向锯齿状通道部分,其沿所述进入表面的约三分之一;
底座,其连接到所述导管外壳;
径向引导通道,其定位在所述底座上,以从所述底座的外围引导液体离开所述辅助端口;
中间收集通道,其定位在所述底座上,以引导液体进入所述径向引导通道;以及
挥发性有机化合物敏感带,其安装在所述凹进区域内。
9.如权利要求8所述的系统,其中所述导管外壳可关于所述底座旋转。
10.如权利要求8所述的系统,其中:
所述导管外壳可关于所述底座旋转;且
所述辅助端口定位成基本上与所述底座齐平。
11.如权利要求8所述的系统,进一步包括:
压力传感器,其连接到所述辅助腔的至少一个的所述近端;
阀,其连接到所述辅助腔的所述至少一个;以及
控制器,其连接到所述压力传感器、所述阀和所述真空泵,所述控制器被设定程序以:
操作所述真空泵来减小所述主腔中的压力;
从所述压力传感器接收压力数据;
响应所述真空泵操作的相关变化而监控压力数据的变化;
鉴别所述辅助腔的所述至少一个是否具有延迟的压力响应;以及
操作所述阀。
12.一种用于将减小的压力应用到组织位置的系统,所述系统包括:
多腔导管,其具有近端、远端、从所述近端到所述远端延伸通过所述导管的主腔和从所述近端到所述远端延伸通过所述导管的辅助腔;
真空泵,其连接到所述主腔的所述近端;
减压适配器,其连接到所述远端,所述适配器具有通道,以引导液体进入所述主腔,以及
操作装置,其用于使用来自连接到所述辅助腔的至少两个压力传感器的反馈来操作所述真空泵。
13.如权利要求12所述的系统,其中所述减压适配器包括:
导管外壳,其具有界定进入表面的凹进区域;
主导管,其通过所述导管外壳,在所述进入表面上与所述主腔和主端口流体相通;
辅助导管,其通过所述导管外壳,每一个辅助导管在与每一个辅助导管相关连的所述进入表面上与所述辅助腔中的一个和辅助端口流体相通;且
其中所述通道定位在所述进入表面上,以引导液体离开所述辅助端口,并进入所述主端口。
14.如权利要求13所述的系统,进一步包括连接到所述导管外壳的底座。
15.如权利要求14所述的系统,其中所述导管外壳可关于所述底座旋转。
16.如权利要求14所述的系统,其中:
所述导管外壳可关于所述底座旋转;且
所述辅助端口定位成基本上与所述底座齐平。
17.如权利要求13所述的系统,进一步包括:
底座,其连接到所述导管外壳;以及
径向引导通道,其定位在所述底座上,以从所述底座的外围引导液体离开所述辅助端口。
18.如权利要求13所述的系统,进一步包括:
压力传感器,其连接到所述辅助腔的至少一个的所述近端;
阀,其连接到所述辅助腔的所述至少一个;以及
控制器,其连接到所述压力传感器、所述阀和所述真空泵,所述控制器被设定程序以:
操作所述真空泵来减小所述主腔中的压力;
从所述压力传感器接收压力数据;
响应所述真空泵操作的相关变化而监控压力数据的变化;
鉴别所述辅助腔的所述至少一个是否具有延迟的压力响应;以及
操作所述阀。
