Abstract

Description

带针头封垫的导管
本发明涉及静脉导管,特别是涉及可防止在这种导管的使用过程中导致无意中接触到血液的血液回流和淤血的导管。
美国专利4,762,516(Luther等人)和申请日为1989年4月10日的美国专利申请335,472描述了带有针头护套的静脉导管,这种针头护套被设计用来保护医务人员免受在使用导管针头之后被针头剌伤的意外伤害。这种针头的意外剌伤能够造成由于针头先前抽入的病人血管中的血液带有疾病而引起感染。该专利和专利申请所描述的导管通过在这针头从病人身上取下时在针尖上罩住一个从针头套筒伸出的针头护套来防止针尖的意外剌伤。
问题是不仅仅希望保护医务人员免遭受针头意外剌伤的危险,而且还希望提供避免与病人的血液有任何接触的保护。即使在使用前面提到的带有针头护套的导管时,由于不希望有的血液从导管泻漏,也有可能使医务人员与病人的血液接触。在将针头插入病人血管的过程中,操作导管的医务人员要将针头的针尖在病人的静、动脉血管中进行定位。当针头的针尖正确定位时,将有少量血流或血液泻放通过中空的针头进入位于针头近端的泻放腔内。医务人员将注意着血液在泻放腔中出现,以此作为针头正确置位的标志。然后,医务人员将导管送入血管中,并从病人身上取下针头,将导管插管留在血管中的适当位置上。
当针头的针尖移到靠近导管远端的一个位置时,血液将在静脉或动脉压力作用下流入导管和中空的针头。但是血液也可能进入到针头外壁与导管插管内壁之间的环形空间内。在该空间内流向导管套头的血液在此称为回流。通常情况下,血液回流是无妨的,因为一旦针头从导管中取出,导管套头一般会很快地接到管道装置上。但是,在前面提到的带有针头护套的导管中,针头护套的远端突出部在针头完全取出之前占据着导管套头。当针头护套沿针头长度方向朝针尖伸出时将带着导管套头同时将导管穿入病人的静脉或动脉。当护套到达全部伸出位置、上述运动终止时将把导管套头从护套的突出部顶出。因此,如果在导管套头从护套的突出部顶出之前,发生血液回流进入导管的话,针头护套将在脱离导管套头之前被病人血液所污染。要防止这种污染以便防止医务人员接触到针头护套突出部上的血液。
在申请日为1989年5月17日,申请号为353,276的美国专利申请中描述了一种阻止血液回流进入导管套头的技术。这一技术使用通过使导管插管近端变窄而形成一个阻流段的方法,该阻流段使得导管插管该部分紧密地包着插入针头,从而显著地阻止了血液流过围绕着针头的阻流段。只有很小一部分血液在针头从导管插管中取出之前能够通过阻流段流入导管套头。
然而,一旦针头抽出到达阻流段近端位置时,对于流入导管套头及位于套头内的针头护套的突出部的血液不存在限制。即使在针头护套从导管套头中取下并将套头与管道装置相连接之前的这种状态只是一种瞬时状态,仍希望阻止在此之前及此时的任何血液回流。这是因为,流入导管套头的血液仍然可以通过针头护套的突出部上的、针头穿过它伸出的开孔。通过针头护套上的开孔的血液回流能够进入针头护套后面并覆盖在针头上,在此处,血液就有可能与正在插入导管并取下针头装置的医务人员发生不经意的接触。因此,仍然希望防止通过针头护套的突出部上的、针头穿过的开孔的血液回流。
按照本发明的原理,提供一种阻止血液通过导管针头护套或套头的远端上的开孔的回流。这种限制是通过在针头位于其中的开孔中形成一个封垫来实现的,这里所说的这种封垫称为就地成形的封垫。封垫是由一种经过固化后与针头护套或套头牢固地粘接在一起但不与针头本身粘连的材料形成。因此,针头将能够平滑地滑动通过封垫,并且封垫将防止血液回流进入针头护套或套头,并在针头通过封垫时将擦去针头外面的血液。

首先参看图1,示出了一个带有针头护套的导管和针头,其结构可以设计成如前面提到的专利和申请号为335,472的专利申请中所描述的。该装置包括一个导管管子或插管10,与导管套头12相连。在导管套头12的近端有一个路厄氏锁定装置14。如美国专利4,191,185(Lemieux)中所描述的,插管通过在套头12内的插管近端中的喇叭形金属套筒16的压配合装到套头12上。
中空的金属插入针头20具有尖锐的远端26,针头20的近端粘接在泻放腔22的远端开口上,该泻放腔固定在针头套管或外壳3 内。泻放腔与外壳的固定在图中看不到,它包括一个从外壳内壁到泻放腔外壁的纵向的类似轨条的延伸部分。泻放腔的近端用一个如1988年7月20日申请的美国专利申请221,579中所描述的多孔塞24塞住,多孔塞在泻放腔内充满血液时能排出腔内的空气,并且塞子上的孔的大小不足以让血液从那里通过。
针头护套34可滑动地装在针头外壳30内,在图1表示的是其缩入位置。针头护套的内部是空的,以便容纳在其中的泻放腔。在针头护套的一侧有一个纵向开口或槽,泻放腔的安装部分由此通过。针头护套的远端或突出部36成锥形,并且有一个当针头护套伸出时让针头通过护套的远端开孔38。导管套头12装在针头护套的突出部36上,并且当护套伸出时跨在突出部上移动,直到护套完全覆盖着针头、导管和套头被顶出为止。
图1还示出了当针头针尖在血管中正确定位时所要求的进入导管装置中的血液的流动。血液将在动脉或静脉压力下通过中空的针头,如图中40a所示,并流入泻放腔22中,如图中40b所示。
图2表示当针头针尖已经在血管中就位之后导管装置中各部件的相对位置。此时,针头护套34由医务人员通过向远端方向推压护套的推出突片32而伸出。该运动使针头针尖26相对于导管10的远端缩回到图2所示位置,这种情况称为针头针尖“罩”在导管远端内。当针头针尖26被罩住时,血液将如42a所示地流入并充满导管的远端。但除去所希望的如图1所示的通过中空针头的血流,血液还可能进入并通过针头外部与导管插管内部之间的小的环形空间,如42b所示。该血液回流能够到达导管套头12的内部,在此发生淤血,如42c所示。这种淤血将在针头护套突出部36的外面产生不希望有的污染,还可能通过开孔38流入针头护套的突出部,如图中42d所示。本发明的一个目的就在于防止血液回流进入针头护套的突出部。
图3示出根据本发明原理构成的导管,该导管装置包括一个在导管套头12附近的导管插管10上形成的阻流段50,这是前面提到的申请号为353,276的美国专利申请的发明主题。阻流段50是导管插管的内径减小至与针头20的外径紧密接合的尺寸的一段。如图3所示,血液仍流入导管插管10的远端并围绕着针头的被罩住的针尖26,甚至可以开始通过针头和插管之间的环形空间回流。但当血液回流到达阻流段50时,插管与针头之间的相对紧密的配合阻止了血液进一步回流进入导管套头。这基本上防止了由在导管套头内围绕着针头护套的突出部36的周围的淤血对针头护套的污染。
根据本发明的原理,任何通过阻流段50并进入导管套头12中的血液回流都被在开孔38中围绕着针头设置的封垫60挡住,不能进入针头护套34的突出部36内,防止了血液进入针头护套从而泄漏并接触到医务人员,而且封垫60在朝针头近端的方向抽出针头时将会擦去针头20外面的血液。
结合附图4a、4b、5a和5b来说明就地成形封垫而不使用如O形环、聚四氟乙烯(Teflon)环形封垫之类的预制封垫的客观需要。图4a示出形成在针头护套突出部36的远端的开孔38内的封垫60。针头20基本上与开孔38、特别是与开孔的窄径部分39同心,在图4b的针头护套突出部的端视图中示出了各部件同心对中。
然而,由于需要保证针头护套突出部自由地沿针头滑动,因此,要有意放宽部件间的间隙及开孔的对中。这样,针头可能不总是与开孔38、39对中。在图5a中说明了这种情况,图中示出针头20在靠近开孔顶部的部位通过开孔38、39。这种偏心对位清楚地表示在图5b的针头护套突出部的端视图中。由于使用流动材料来形成封垫,则无论针头相对于开孔38、39对中与否,都会围绕着针头形成封垫。因此,就地成形的封垫保证了封垫的完整性,同时允许针头护套沿针头20平滑地滑动,而不管导管装置各部件相互间的偏心度。
图6a-6e表示用于形成本发明的就地成形封垫的优选方法。在这些图中,使用一种由紫外(UV)光固化的粘结剂来形成封垫。一种适用的粘结剂是LV3201-69型粘结剂,可从Amicon公司购买。其它合适的由UV固化的化合物可从Loclite和Dymax公司购买。UV粘结剂比较好是由于它具有在大量生产过程中所需要的快速固化的能力,并可形成100%固体封垫。在针头20上形成封垫之前,针头外壳30、针头护套34及针头20装配好,针头用粘结剂固定在泻放腔22的远端开口上,针头护套34装在外壳30中沿针头滑动。针头20用一种如含0.1%Dow Cor-ning有机硅的己烷溶液的润滑剂进行润滑。使用时可将润滑剂喷在针头上或将针头浸入润滑剂中来醮附。
如图6a所示,用如EFD XL-100型粘结剂施料器的施料器70将粘结剂施加到围绕着针头20的开孔38内。在一个优选实施例中,由一个带有24号针头的套管72来施加粘结剂。约有0.5至3毫克的粘结剂在约0.8秒时间内在18磅/英寸2的压力下施加于针头护套的开孔中。如图所示,针头装置保持垂直,粘结剂将充满围绕着针头的开孔。然后,针头装置移动到第二工位,如图6b所示,在此工位上,粘结剂用来自UV光源74的光进行0.5至1.0秒的固化处理。一种适用的UV光源可从Dymax公司购得。然后,针头装置移动第三工位,如图6c所示,在此工位上,粘结剂用第二个UV光源76照射,均匀地固化粘结剂。针头装置在该工位上再照射0.5至1.0秒的时间,UV照射过程便告完成。
在UV照射之后,立即将针头装置移到图6d所示的第四工位。粘结剂在第三工位上已完成1秒钟的照射,粘结剂已充分固化并牢固地粘接在塑料针头护套的突出部上,但它还没有与带有润滑剂的金属针头20形成牢固的粘结。在粘结剂与针头能够牢固地粘结之前,用一个推板78将针头护套34套着针头如箭头79所示地向上推动,所述推板78推靠着针头护套的近端34′,由于针头外壳30可靠地夹持住,推板78向上的推力将破坏针头与粘结剂的任何结合,针头护套34带着其突出部36将向上朝着针头远端的方向滑动一小段距离,如图6d所示。然后,去掉向上的力,并让针头护套滑动或人为地使其向下滑,如图6e所示。这样,针头装配完毕,针头护套和封垫此后可沿着针头平滑地滑动。然后可以将导管和导管套头装在针头和针头护套的突出部36上以完成导管装配。
可以用其它粘结剂来形成封垫60。也可以用诸如H.B.Fuller公司生产的石腊或聚酯或聚酰胺类材料之类的热熔粘结剂。由于具有较小的热容量,热熔粘结剂也满足了快速冷却并固化形成100%固体封垫的要求。在这种热熔粘结剂能够牢固粘结在针头上之前,也要进行针头护套和封垫沿针头滑动的工序。还可以使用一种可溶的粘结剂材料,诸如在己烷和丙醇溶液载体中含30%的TPH、聚氯乙烯(PVC)、或聚氨基甲酸乙酯。由于用这些材料形成的封垫将随着溶剂的蒸发而收缩,因此必须使用较大量的可溶性粘结剂(例如:3-5毫克)。用这些材料进行试验表明,在蒸发过程中会在固化的封垫中产生孔隙。硅橡胶也是一种合适的封垫材料,但存在固化时间增长的问题,从而限制了高速度的生产操作。
总之,能够用上述任何材料形成一种可接受的就地成形的封垫。这种封垫将防止通过围绕着针头的开孔的血液回流进入针头护套,并且封垫在针头护套伸出罩住针头时将擦去针头外表面上的血液。然后,被罩住针头的针头装置可安全地处理,显著地降低了医务人员与病人血液接触的事故。

Claims (8)

1.一种导管装置,它包含有:导管及导管固定在其上的导管套头;带有尖锐的远端针尖的中空针头及针头粘接到固定在其上的泻放腔上的针头外壳;可滑动地装在针头外壳内并可沿着针头滑动的针头护套;该针头护套具有一个呈锥形的远端,在该远端上具有一个针头从其中穿过的远端开孔,导管套头套装在该远端上;其特征在于在该针头护套的远端的远端开孔中成形有一个用来围绕着针头来阻止血液流过该远端开孔并进入该针头外壳内的封垫,所述封垫由一种粘结材料制成,所述粘结剂粘结到所述针头护套上,而不粘结到所述针头上。
2.按照权利要求1所述的导管装置,其特征在于所述封垫是在所述针头放置在所述开孔内时在该开孔内形成的。
3.按照权利要求1所述的导管装置,其特征在于所述封垫是由一种通过紫外光固化的粘结剂形成的。
4.按照权利要求1所述的导管装置,其特征在于所述封垫是由一种热熔粘结剂形成的。
5.按照权利要求1所述的导管装置,其特征在于所述封垫是由一种溶解于溶液中的粘结剂形成的。
6.一种包括一个具有尖锐的末端针尖的中空针头及一个连接在所述针头近端的泻放腔的导管装置,它包含有一个设置在所述泻放腔远端的可滑动的针头护套。这针头护套具有一个跟导管套头接合的远端凸块,这远端凸块带有一个在所述护套沿所述针头滑动以罩住该针头的远端针尖时该针头在其中穿过的开孔;其特征在于还含有一个设在该开孔内用来防止血液通过该开孔流入该针头护套内的封垫,所述封垫由一种粘结材料制成,所述粘结剂粘结到所述针头护套上,而不粘结到所述针头上。
7.按照权利要求6所述的导管装置,其特征在于所述封垫是在所述针头放置在所述开孔内时在该开孔内形成的。
8.按照权利要求6所述的导管装置,其特征在于所述封垫是由一种通过紫外光固化的粘结剂、一种热熔粘结剂或一种溶解于溶液中的粘结剂中之一形成的。
