Description
概括来说,本发明涉及到脊柱移植物系统的领域,这种类型的脊柱移植物系统使用脊柱杆,其利用包括脊柱螺钉在内的各种固定件沿脊柱在不同的位置上连接于脊柱上,更具体的说,本发明涉及一种具有角度可调性的脊柱螺钉固定件。
已经开发了几种用于校正和稳定脊柱曲线以及促进脊骨并合的系统。在一种系统中,一根可弯曲的杆靠近脊椎纵向设置,并被用若干个固定件沿着脊柱的长度固定到不同的脊骨上。可以使用各种各样的固定件,比如,卡子或者骨螺钉,它们被制成脊骨的特定位置相连接的形状。

Gotzen等人的美国专利US-4,662,365描述了一种外置的骨头固定件,该固定件使用径向刻有槽的交叉梳状构件来改变骨螺钉的角度取向。然而,对于内部固定到脊柱上来说,这种结构显得太过笨重和复杂。
Krag等人的美国专利US-4,987,892描述了一种脊柱固定系统,该系统使用具有径向刻槽的端头的梗节螺钉,所说的端头与整体地设置在铰接的夹持件上的径向槽或者齿相互交叉,而铰接的夹持件则夹持住稳定杆。铰接夹持件的排布要求径向槽相对于稳定杆是偏置的。假如可能的话这种偏置会产生对中问题,对中问题将使螺钉在垂直于稳定杆的轴线的平面内角位移发生困难,此外,Karg等人的系统需要一个用于梗节螺钉的专门制成的铰接的夹持件。如果把卡子、交联件或者其它的固定件连接到稳定杆上,则需要不同构造的杆子附助件。这样就复杂化了整个结构和装配方法。Karg等人的这个系统的另一个困难是,它不是一种形式设计,换句话说,铰接的夹持件必须在装配之前就接到杆上。相反,更有利的设计方案应是允许脊柱杆落入钉边接件。
Kluger的德国专利DE-3,219,575C2公开了一种脊柱移植物系统,该系统借助于交叉梳状盘接触表面在角件上的相对移动而实现骨螺钉的角度调整,每一个骨螺钉安装在所述的角件之内。该系统的结构复杂,因为需要用分离的调节装置来调整骨螺钉的角度取向,以及固定骨螺钉相对于角件的位置和骨螺钉互相之间的位置。骨销子在垂直于脊柱杆的平面内的任何旋转调整都将改变销子之间的距离,从而需要进一步调整套筒螺母。此外,该系统只限于在同一根杆上仅需求两个销子或者其它固定件的情况。
本发明的一个方面就是提供一种用于脊柱移植物系统的,可变角度的骨质螺钉系统,以便用其将脊骨固定件连接到靠近病人的脊柱延伸的一根脊柱杆上。在一个最佳实施例中,所说的系统特征是,有眼螺栓具有一个头部,该头部具有一个可穿过脊柱杆的通道。有眼螺栓还包括一个从头部延伸出的螺杆部分,该部分与脊柱杆轴线垂直相交。还提供一个具有头部和螺纹杆部的骨螺钉,其适合作为插入脊骨部分的螺钉。此外,还有一个垫圈,该垫圈带有安装到有眼螺栓上的安装装置。所说的安装装置起阻止垫圈相对于有眼螺栓螺杆的轴线转动的作用。骨螺钉的头部和垫圈包括交叉梳状锁定装置,用以在角度改变取向闭锁时阻止螺钉围绕有眼螺栓螺杆的轴线转动。该系统还具有一个同有眼螺栓的螺纹杆部相配合的螺母,因此,当螺母旋扭到有眼螺栓的螺纹杆上时,可将骨螺钉和垫圈夹紧在有眼螺栓的头部和螺母之间。
因此,本发明的目的是提供一种改进的脊柱移植系统。
本发明的另外一个目的和优点是提供一种改进的脊柱移植物系统,该系统即允许骨质螺钉固定件的三维调节,又不使系统的整体结构复杂化或者使脊柱杆的装配变得更困难。
本发明的再一个目的是提供一种改进的脊柱移植系统,其具有上述优点且具有另外一个优点,那就是用于螺钉、卡子和交联件的所有脊柱杆连接件均可以制成具有通用的有眼螺栓连接机构。
本发明还有一个目的使可变角度螺钉具有顶端放入能力。例如螺钉的形式结构允许把螺钉插进梗节,然后,用根据本发明制成的有眼螺栓连接到一根适当地仿形的脊柱杆上。

明的原理的进一步应用,对于本领域的熟练技术人员来说是常常发生的。
现在参照附图详细说明,图1示出了总体标注以11的、安装了脊柱移植物系统10的骨盆和脊柱的一部分。系统10包括一组四根脊柱杆它们中间的两根-即杆12和13,以其下端固定在骶骨上,而另外的两根-即杆14和16则延着脊柱向上延伸。沿着杆14和16,在隔开的几个位置上使用了若干个骨螺钉和/或卡子,其将杆连接到脊骨上,特别是梗节上。此外,将调整环、螺钉和/或卡子也连接到杆12和13上,所有的调整垫、螺钉和/或卡子均被有眼螺栓夹紧在杆上。例如,确定杆12和13之间的间隔的调整杆17就是分别用有眼螺栓18和19在铰点位置固定到杆12和13上的。杆14由固定件15连接到脊柱上。
根据因素的多样性,所使用的特殊类型的脊柱固定件可以是卡子类元件或者骨螺钉。如果用卡子作为固定件,则可以以多种形状尺寸制造卡子,比如,象田纳西州麦菲斯市的“Danek Medical”有限公司1990年2月1日出版的用于TSRHTM脊柱移植物系统的“Danek外科技术手册”第6~10页所表示的和描述的那样。通常,当与那份手册中所描述的有眼螺栓相结合时,所有的螺钉、卡子和调节物均具有一个“三点剪切”夹持特性。然而,应当知道,已有的TSRHTM脊柱移植物系统的骨螺钉(比如那份手册的第9-10页所描述的)不能相对于杆的轴线改变它的角度取向。
参看图2,其示出了本发明的可变角度的骨螺钉20的优选结构。该螺钉20具有一个头部21和一个刻有螺纹的杆22。在一个最佳实施例中,杆22上刻有螺纹,以便同脊骨的梗节相连。头部21呈一个U形,构成容纳有眼螺栓的螺纹杆的开口箍圈23。开口箍圈23为螺钉20提供了一个有利的顶端放入方式。头部21的一面制出一系列径向延伸的齿牙或者凹槽24。在该最佳实施例中,每一个槽24沿圆周成6度的弧度。螺钉20的直径最好是5.5和6.5毫米,其长度最好在25~50毫米之间。

图5示出了安装在脊柱杆16上的可变角度的螺钉20、垫圈25和有眼螺栓/锁紧螺母组件26。有眼螺栓/锁紧螺母组件26包括有眼螺栓26a和锁紧螺母26b。有眼螺栓/锁紧螺母组件是一种已知的脊柱杆固定件,而且最好是由Danek Medical有限公司提供的,用于Danek TSRHTM脊柱移植物系统的,零件号为808-029的1/4寸有眼螺栓/锁紧螺母组件。把有眼螺栓26a沿着杆16放置在一个所需要的位置上,并把垫圈25从螺纹杆部分31的上方安装到有眼螺栓26a上,这样,垫圈25套于有眼螺栓的头部32,且其凹槽28紧靠在所接纳的杆16上,从而提供一种箍圈夹持力,该夹持力可靠地将有眼螺栓26a固定在杆16的位置上。当处于这种状态时,槽27在头部32上可移动一个足够的距离,以致于槽24和槽27相互交错接触时骨螺钉20的头部21能够以任何所希望的角度取向定位而不防碍头部32。骨螺钉20的有开口箍圈的头部21允许有眼螺栓26a的杆部31从顶端放到螺钉20上,不必象现有装置-比如:上面提
到的Krag等人的专利US-4,987,897中所描述的装置那样处理螺钉或者脊柱杆,使螺纹杆部穿过一个密闭的螺钉,把一个锁紧螺母26b旋扭到螺纹杆31上,直到它与螺钉20的无刻槽的那面相接触。随着螺母26b的旋进,它把螺钉20和垫圈25推向脊柱杆16,因此,杆16将被以三点剪切夹持的方式夹紧在有眼螺栓26a和垫圈25之间。
由于垫圈25和螺钉20之间的交叉梳状连接,因此,脊柱杆的轴线A和螺钉的轴线B之间的角度取向就能够随意改变,而且,与正交不同,它已不再必须以弯曲脊柱杆来调整角度。此外,把可变角度的螺钉20安装到脊柱杆上,所用的有眼螺栓与将其它固定件如卡子和隔离器所用的有眼螺栓相同。这样,可减少不同零件的数量,并简化装配工艺,除了借助于相互交叉的槽的相对转动来改变螺钉20在一个平行于脊柱杆的平面内的角度取向之外,还能够通过把有眼螺栓的螺纹杆相对于脊柱杆转动来改变螺钉在一个垂直于脊柱的轴线的平面内的角度取向。
本发明的可变角度的骨螺钉除了直接安装到有眼螺栓/锁紧螺母组件26上之外,还可安装到一个象于1991年12月4日递交的美国专利申请号为07/803,325、发明名称为“用于脊柱移植物系统的横向偏置连接件”中所描述的那类横向偏置连接件上,这个系统本文引作参考。图6示出了一个用有眼螺栓/锁紧螺母组件26安装到一根脊柱杆16上的偏置连接件40。该连接件40包括一对起夹持有眼螺栓26并阻止该有眼螺栓沿着脊柱杆16移动的箍圈作用的臂45。把垫圈25和可变角度骨螺钉20安装到一个导座上,该导座具有一个用以接纳垫圈的距形段和从该段延伸出的螺纹杆42。另一种可选择方案是,省去垫圈25,不再作为一个独立件,而把它的径向槽整体的合并到连接件40的螺纹杆42的基座上。锁紧螺母43固定骨螺钉20的位置。螺钉20相对脊柱杆16的横向偏移量可通过选择脊柱杆16在一对对正的凹槽44内的位置来有选择的调节,借此,调节脊柱在径向平面内的畸形侧向弯曲。

Claims (17)
1、一种用于在脊柱移植物系统中将脊骨固定件连接到靠近病人的脊柱延伸的脊柱杆上的可变角度的骨螺钉系统,包括:
一个有眼螺栓,该螺栓包括一个其上具有一个用以容纳从中穿过的脊柱杆的通道的头部,还包括一个自该头部起沿着与所说的脊柱杆的轴线垂直并相交的轴线沿伸的螺纹杆;
一个骨螺钉,该骨螺钉具有一个头部和一个适宜于旋拧进理想的脊骨固定位置的螺纹杆;
一个垫圈,该垫圈具有一个用以安装到有眼螺栓上的装置,该安装装置阻止垫圈相对于上述有眼螺栓的螺纹杆的轴线转动,上述骨螺钉的头部和垫圈具有交叉梳状锁定装置,用以阻止螺钉围绕有眼螺栓的螺纹杆的轴线转动,但许在可变的角度定向互锁;以及
一个螺母,用以同上述有眼螺栓的螺纹杆配合,因此,当该螺母被旋拧到有眼螺栓的螺纹杆上时,可将所说的骨螺钉和垫圈夹紧在螺纹杆的头部和该螺母之间。
2、根据权利要求1所述的可变角度骨螺钉系统,其特征是,垫圈具有一个当脊柱杆被夹紧在有眼螺栓的头部和螺母之间时用来接触该脊柱杆的弧形槽。
3、根据权利要求1所述的可变角度骨螺钉系统,其特征是,骨螺钉的头部包括一个制成开口状的箍圈部分,在此箍圈内容纳有眼螺栓的螺纹杆,并允许把螺纹杆从顶端放到骨螺钉上。
4、根据权利要求1所述的可变角度骨螺钉系统,其特征是,所说的垫圈固定装置包括一个该垫圈上的矩形通孔,该孔的大小应允许把有眼螺栓的头部精密地装配在其内。

至少一根适宜于靠近病人的脊柱延伸的脊柱杆;
若干个用以将上述脊柱上的若干块脊骨借助有眼螺栓/锁紧螺母组件连接到上述至少一根脊柱杆上的卡子和螺钉,每一个有眼螺栓/锁紧螺母组件均具有一个有眼螺栓和一个锁紧螺母,所说有眼螺栓包括一个其上具有一个用以容纳从中穿过的脊柱杆通道的头部,还包括一个自该头部起沿着与脊柱杆的轴线垂直并相交的轴线延伸的螺纹杆,所说的锁紧螺母与该螺纹杆相啮合;

8、根据权利要求7所述的脊柱移植物系统,其特征是,骨螺钉的头部包括一个制成开口状的箍圈部分,在此箍圈内接受有眼螺栓的螺纹杆,并允许把有眼螺栓自螺钉顶端放到骨螺钉上。
9、根据权利要求8所述的脊柱移植物系统,其特征是,垫圈包括一个当脊柱杆被夹紧在有眼螺栓的头部和上述螺母之间时用来接触脊柱杆的弧形槽。
10、根据权利要求6所述的脊柱移植物系统,其特征是,骨螺钉的螺纹杆部包括螺纹结构,以便拧入脊骨的梗节。
11、一种在脊柱移植物系统中用以将脊骨固定件连接到靠近病人的脊柱延伸的脊柱杆上的可变角度的骨螺钉系统,包括:
一个有眼螺栓,该螺栓包括一个其上具有一个用以容纳从中穿过的脊柱杆的通道的头部还包括一个自该头部沿着与所说的脊柱杆的轴线垂直并相交的轴线延伸的螺纹杆;
一个具一个有头部和一个适宜于拧进理想的脊骨固定位置的螺纹杆的骨螺钉;
一个用以将有眼螺栓固定到脊柱杆上的箍圈夹持装置;
一个与骨螺钉配用的交叉梳装锁定装置,用以阻止骨螺钉绕着有眼螺栓的螺纹杆的轴线转动,但允许在可变的角度定向上骨螺钉和箍圈夹持装置互锁;以及
一个同有眼螺栓的螺纹杆相啮合的螺母,借此,当该螺母旋拧到有眼螺栓的螺纹杆上时将骨螺钉和箍圈夹持装置夹紧在有眼螺栓的头部和螺母之间。
12、根据权利要求11所述的可变角度骨螺钉系统,其特征是,所说的箍圈夹持装置是一个具有对正的弧形槽的垫圈。
13、根据权利要求11所述的可变角度骨螺钉系统,其特征是,所说的箍圈夹持装置包括一个横向偏置连接件,该连接件起横向偏移骨螺钉相对于脊柱杆的位置的作用。
14、根据权利要求13所述的可变角度骨螺钉系统,其特征是,所说的横向偏置连接件包括一对横向延伸的臂。
15、根据权利要求11所述的可变角度骨螺钉系统,其特征是,所说的交叉梳状锁定装置包括多个设在垫圈和骨螺钉上的相互啮合的、径向的、彼此隔开的槽。
16、根据权利要求1所述的可变角度骨螺钉系统,其特征是,所说的交叉梳状锁定装置包括多个设在垫圈和骨螺钉上的相互啮合的、径向的、彼此隔开的槽。
17、根据权利要求6所述的脊柱移植物系统,其特征是,所说的交叉梳状锁定装置包括多个设在垫圈和骨螺钉上的相互啮合的,径向的、彼此隔开的槽。
