技术领域
本发明涉及一种独特的装置与方法,用于为了医疗目的,比如细胞组织烧蚀及流质输送,来透入人体细胞组织。该装置在所选精确目标处透入组织,以便给组织输送能量与/或输送物质。它把这个治疗限制在精确预选位置,因此对正常周围组织的外伤最小,并且实现更大的医疗利益。这个装置是一个像导管的装置,在导管中有一个或多个通管针,安装成沿导管的侧边从一个通管针出口通过周围组织伸到为医疗介入而确定的目标组织,用于在为医疗而选择的区域或器官中定位一个治疗组体。
背景技术
治疗组织通常要用一个医疗器械与目标组织直接接触,通常按外科手术把目标与介入组织两者都暴露在显著的外伤状态。经常,因为目标组织位于人体之中,或者接近目标组织有容易损害的危险的人体器官、神经,或其它种种部分,所以难以精确地放置一个治疗探测器。
例如,良性前列腺肥大或增生(BPH)是五十岁以上的人所经历的最常见的医疗问题之一。从医学最早年代起就认识到了由于前列腺增生所造成的泌尿道阻塞。前列腺的增生扩大往往导致尿道压缩,引起泌尿道的阻塞,并且后来发展种种症状,包括尿频、尿流减小、夜尿症、疼痛、不适,以及滴尿。已经表明,BPH与老龄化之间的联系在五十岁以上人中超过50%,以及在八十岁以上人中发病率增加到75%以上。泌尿阻塞的症状最常发生在六十五至七十岁之间,届时在这个年龄组中大约有65%的人有前列腺扩大。
目前,还没有证明有行之有效的治疗BPH的非外科方法。此外,现有的外科手术不完全令人满意。目前为遭受这个疾病的阻塞症状的患者提供了很少几种选择:继续克服症状(即保守处理),在早期忍受药物疗法,或忍受外科介入。在美国每年有430,000以上患者经受外科手术,切除前列腺组织。这些代表不到呈现临床重要症状的人的百分之五。
那些遭受BPH的人往往是上了年纪的人,许多有其它健康问题,这些问题增加了外科手术的危险性。切除前列腺组织的外科手术伴随若干危险产生,包括与麻醉法有关的病状、出血、凝块、肺栓塞以及电解质不平衡。目前实行的这些手术还能引起心脏病患者并发症、膀胱穿孔、失禁、感染、尿道或膀胱颈狭窄、前列腺碎片滞留、呕吐以及无生殖力。由于目前治疗阻塞尿路的选择方案的广泛发病性,大多数患者耽误了他们健康状况的决定性的治疗。这种情况能导致种种组织的严重损害。次之是前列腺中的阻塞损坏(膀胱肥大、肾积水、肾骨盆膨胀、慢性病感染、输尿管膨胀),这并不是没有重要后果。此外,相当数量具有应手术介入的症状的患者因此不足冒手术危险,并且不愿前列腺切除。此外,遭受BPH的较年轻人不希望冒并发症比如无生殖力的危险,往往被迫避免外科介入。因此,用于治疗BPH的改进型外科及非外科方法的必要性、重要性及价值是毫无疑问的。
在用于切除人体组织的电灸手术中,尤其是当希望一个不流血切口时,或者当手术位置用一个正常的解剖刀不易接近,但是通过自然人体孔道,比如食管、肠或尿道为一个细的器械提供了一个进入通道时,就使用高频电流。例子包括切除前列腺瘤、膀胱肿瘤或肠息肉。在这些情况下,由一个外科探测器把高频电流送入待切除的组织。由此产生的散失热引起这个点的细胞流质的沸腾与汽化,在那上面细胞壁破裂,并且组织被分割。
在许多疾病治疗及医疗条件中,已经单独地或作为外科切除手术的一个附属来使用就地破坏组织。它往往比外科手术外伤小,并且可能在其它手术不安全时是唯一的选择。烧蚀治疗装置具有使用电磁能的优点,电磁能被循环流体以及其它自然人体过程的传导及对流力迅速扩散并且减小到一个非破坏性水平。
已经使用微波、射频、声能(超声)及光能(激光)装置,以及组织破坏物质,从各种各样的解剖位置及器官上破坏恶性、良性以及其它类型的细胞及组织。所治疗的组织包括孤立的恶性肿瘤块以及,更具体地说,比如具有良性前列腺增生特征的前列腺、腺及基质蛋白瘤的器官。这些装置典型地包括一个导管或插管,用来把一个射频电极或微波天线通过一个导管传送到治疗区域,并且通过导管壁以各个方向把能量扩散作用入周围组织。在这个细胞破坏过程中,一般由导管壁承受住严重的外伤,并且有些装置把冷却系统与微波天线结合起来,以减小对导管壁的外伤。例如,为了用这些装置治疗前列腺,热能通过尿道壁送入周围前列腺细胞,以努力烧蚀引起尿道狭窄的组织。光能,典型地来自激光,通过“燃烧穿过”尿道壁传送到前列腺组织目标位置。导管壁的健康细胞以及在瘤与导管壁之间的健康组织在该过程中同样无区别地被破坏,并且能引起某些前列腺功能的不必要损失。此外,某些微波装置加上冷却功能使装置复杂化,并且要求装置足够大,以适应这个冷却系统。
为了医疗目的对特定组织应用液体受到了不使介入组织外伤而获得输送,以及对限定的特定目标组织实现输送的能力限制。如果用一个装置能方便地并且精确地把一个流体(液体或气体)供应导管的出口放置在特定目标组织处,局部化学疗法、药物注入、胶原蛋白注射,或者那些随后由光、热或化学物活化的试剂的注射就大为便利。
发明内容
本发明的一个目的是提供一种用于透入组织的装置与方法,通过介入的组织到达为了医疗作用,比如组织烧蚀与/或物质输送所选择的精确目标组织处,同时把这个活动限制在精确的预选位置,由此使外伤最小,并且实现更大的医疗利益。
本发明的另一个目的是提供一种用于人体组织的组织烧蚀的装置与方法,它把治疗能量直接输入目标组织,而对它周围的组织影响最小。
本发明的再一个目的是提供一种把流体治疗试剂,比如可流动的液体及气体,更为精确与容易地引导到人体中一个特定位置的装置与方法。
本发明的又一个目的是提供一种热破坏装置,它给操作员提供更多有关目标治疗细胞组织及周围组织两者中所建立的温度及其它状态的信息。此外,它将对通管针的物理放置以及组织烧蚀过程的参数提供更多的控制。
根据本发明的一种医疗探测装置,用于对通过由一个壁所限定的自然人体内腔和向外提供的自然人体孔道可达到的人体中的一个治疗位置上的组织进行医疗,包括一个导向槽,具有近端和末端,并且具有一个从近端延伸到末端的通路,一个通管针滑动地安装在该通路中,并且该通管针包括一个具有一个尖端的软导电电极和一个同轴安装在该电极上但是露出尖端的绝缘套管,导向槽的末端带有的并且与引导电极的通路以及导向槽的绝缘套管边侧相联系的导向装置,耦连在导向槽的近端上的手柄装置,用于把导向槽的末端引入自然人体孔道,到达邻近治疗位置的一个位置,手柄装置包括用于推进通管针以使电极的尖端透入壁并在治疗位置的组织中引起一个热效应的装置,同时绝缘套管保护该壁免于由电能引起的热效应,该绝缘套管具有一个从近端到末端的内腔。
总之,本发明的医疗探测装置包括一个具有一个控制端和一个探测端的导管。探测端包括一个通管针导向槽,至少有一个通管针出口,以及通管针导向装置,通过至少一个通管针出口以及通过介入的组织把一个软通管针引出到目标组织。一个通管针适当安置在至少一个所述通管针导向装置中,该通管针包括一个不导电套管,其中至少有两个并且最好有三个腔。一个RF(射频)电极腔在不导电套管的末端中一个末端出口处终接,以及一个射频电极安置在该RF电极腔中,通过末端出口在其中纵向移动。最好是,至少一部分电极腔的一个相对的表面与电极相隔开,以便为输送药物液体限定一个液体供应通路。一个第二可选择的流体通路腔在不导电套管的末端中一个末端出口处终接,并且包括通过那里传送流体的装置。
一个温度传感器第三腔在邻近不导电套管的末端一个密闭处终接。至少有一个并且最好有多个温度传感装置,比如热电偶安置在第三腔中,引线通过腔延伸。一个优选实施例具有两个温度传感装置,适当安置在第三腔中,一个温度传感装置适当安置在离不导电套管的末端大约1mm之内,并且第二个温度传感装置安置在离不导电套管的末端至少3mm并且最好在3到6mm处。
总之,本发明的另一个实施例包括一个具有一个控制端和一个探测端的导管,探测端包括一个通管针导向槽,有一个通管针出口,以及通管针导向装置,通过通管针出口以及通过介入的组织把一个软通管针引出到目标组织。一个通管针安置在至少一个所述通管针导向装置中,该通管针包括一个密封在一个不导电套管之内的电导线。电极有一个末端段,其上至少有一个电流聚焦坡口装置,以及一个末尖端,成形为在它的终端上聚焦电流,由此通过那里进入周围组织的RF电流形成一个从坡口到尖端延伸的破坏。在一个优选实施例中,末端段上有多个环形聚焦坡口或者一个螺旋形聚焦坡口。
最好一部分电极密封在一个具有足够强度的支持管之内,以便当电极通过通管针出口引起时保持为直线。

具体实施方式
在一个用于治疗、破坏或用一个导管从目标组织位置附近抽样而确定的目标细胞组织中,本发明的装置对一个治疗通管针提供一个精确的位置控制。
如下文使用的术语“通管针”限定为包括实心与空心探测器两者,它们适应成从一个导管出口通过正常组织到达目标组织。通管针成形为能方便容易地通过组织。它可以是一个实心线、细棒,或其它实心形状,或者它可以是一个细空心管,或其它具有一个纵向腔的形状,以把流体引到或把物料引出一个位置。通管针也可以是一个细空心管或其它空心形状,其空心腔包含一个增强或功能棒或管,比如一个激光纤维镜片。通管针最好有一个削尖端,以在它通过组织被推到一个目标位置时减小阻力及外伤。
通管针能设计成给一个选择的组织提供各种各样的医疗上希望的治疗处理。作为一个射频电极或微波天线,它能用来烧蚀或破坏目标组织。作为一个空心管,它能用来给目标组织输送治疗流体,比如液体。液体可以是一种简单溶液,或是一种液体中的固体悬浮物,比如胶体微粒。由于通管针非常细,它能以最小的外伤通过介入的正常组织从导管引导到目标组织。
本发明的装置与方法提供一种更精确能控制的医疗处理,它适合于破坏遍及人体的医疗目标组织的细胞,包括人体器官内部及外部。该装置与方法特别对治疗良性前列腺增生(BPH)有用,并且为了简化其叙述,该装置以及其应用在下文中相对BPH作叙述。对本领域技术人员将显而易见,该装置与方法能用来破坏任何利用经皮的或内诊镜的导管可以接近的人体空腔或细胞位置中的人体组织,并且不局限于前列腺。在所有这些器官中应用该装置与方法打算纳入本发明的范围之内。
BPH是由前列腺中细胞的复制及成长,以及细胞死亡率的下降而产生的一种状况,形成扩张前列腺及阻塞前列腺尿道出口的腺及基质蛋白结核。腺结核主要在过渡区内集结,以及基质蛋白结核主要在邻近尿道区内集结。这种状况的传统治疗包括外科切除整个前列腺、断续切除腺瘤,以及横断切除尿道及前列腺以切除组织并扩张通路。与这些手术相联系的一个重要及严重并发症是医原性的不育症。
新近,已经使用激光治疗切除组织,以限制出血及人体流质的损失。在尿道内还使用了探测气球,用热以及不用热来扩张它的直径,但是已经发现有相当大的局限性。
使用微波疗法已经获得一定的成功,把微波天线安置在前列腺尿道内,并且用电磁场在尿道周围组织中产生热。有时在导管轴内应用冷却剂,以减小尿道壁的温度。这样需要复杂机构,以便提供在较远处前列腺组织中产生热的同时,又在冷却直接邻近的组织。这一技术与微波过热相似。类似地,射频组织烧蚀用安置在尿道内的电极,把尿道壁暴露在破坏性温度中。为了避免这一点,保护尿道所要求的低温定值必须如此小,以致产生任何有用效果所要求的治疗时间被过分延长,例如应用能量多达三小时。
本发明的装置的一个实施例使用尿道以接近前列腺,并且把RF电极通管针直接放入待破坏的组织。从尿道伸到目标组织的那部分通管针导线被密封在一个纵向可调整的套管防护罩中,它防止了把邻近套管的组织暴露在RF电流下。通过控制露出电极的大小,套管移动还用来控制单位面积所传送的能量。由此烧蚀破坏限定在所要破坏的目标组织,即那些引起狭窄的组织。本发明的其它方面将从本发明的装置与方法的附图及所附说明中显而易见。对本领域技术人员显而易见,这个手术能通过经皮接近以及通过人体腔孔接近用于人体的许多部位。
图1是本发明的一个具有一个纤维光视附件的RF烧蚀导管实施例的一个立方视图。软导管2连接在手柄4上,有一个具有两个通管针8的终端通管针导向装置6。手柄有通管针电极接头10与11以及套管接头12与13,它们将在下文更详细地叙述。手柄4还与一个光视组件14以及RF电源连接器16、发射应答连接器18以及热电偶连接器20连接。从手柄4引导到通管针导向尖端6的导管2的各各部分可以有选择地具有分等级的刚性。例如,导管能设计成靠近手柄更硬些,以及靠近尖端更软些,或者取其它刚性分布。导管能构成为一个具有外部软套管的内部开槽不锈钢管,就像美国专利No.5,322,064中叙述的那样,这些申请的全部内容在这里合并作为参考。它也能用结合有一个外部套管的盘绕或编织线制成。
在本实施例中纤维光视组件包括一个透镜聚焦组件22,一个透镜视看组件支持连接器24组件,用软导管28与一个插入式灵敏拆开连接器26连接。
图2是图1的一个导管的一个剖视图,表示通管针导向槽的详细结构。通管针导向槽6有一个弯曲的通路28,通过它通管针8伸入待治疗的组织。这些部件更详细结构在1993年2月2日提出的编号为08/012,370的共同待定申请,以及1993年5月13日提交的标题为“具有光视能力的医疗探测装置”,编号为5254-015-37CIP的代理人摘要申请中叙述。
图3是本发明的通管针与腔组件的一个侧视图。这个实施例的通管针的关键部件是一个绝缘套管30,以及一个通过那里延伸的电极32。电极32有一个削尖端,在本实施例中是一个放大了的矛形尖端。电极与套管的近端分别由套管连接器334及电极连接器338连接,以处理下文参照图10与图13更详细叙述的套管与电极的滑动。一个电极支持管36从电极连接器338伸到套管连接器334的区域38中,以便传送压缩力而不毁坏电极32。一个由收缩导管制成的绝缘套管支持管40从套管连接器334延伸到外部导管44的起始或近端42。导管44把支持导管连接到控制管46。支持电极与绝缘套管两者的控制管46延伸到电极腔通路50与电极32的接合处48(见图4)。照这样,对从手柄伸到三腔尖端的通管针的全段提供了支持,防止了当它通过通管针导向槽被推进时,破坏或损失高度软的电极的直线性。
图4是沿导管的中心轴线所取的通管针与控制管组件的接合处的一个侧剖视图,以及图5是沿图4直线5-5所取的通管针与控制管组件的接合处的一个剖视图。在接合处48,电极32通过上部电极腔壁62伸出并且进入电极腔50。外部导管52密封并且支持控制导管46及三腔套管54的两个末端。
参看图5,在控制管46与三腔套管54之间的空间56可以用粘性物质填塞,以把它们固定在一起。这三腔包括一个电极腔50,一个温度传感器腔58以及一个用于对治疗区域供应可选择的流体,比如抗生素或麻醉剂的流体供应腔60。
图6是沿图3中直线6-6所取的本发明的一个三腔通管针的一个剖视图。三腔套管30是一个用于电极32的绝缘套管,并且包括附加温度传感器腔58以及液体供应腔60。电极腔50的内表面与电极的外表面之间可以按一个距离“h”,例如它可以大致在1到3mm之间,留取间隔,以便用一个大约环形的截面限定一个附加液体供应导管。
图7是图6所示沿直线7-7所取的三腔通管针尖端的一个侧剖视图。温度传感器腔58的终端被密封,以保护电元件。热电偶64放置在套管30的末端,以监视电极32周围的组织的温度,并且它最好离露出的电极不到约1mm。热电偶66至少放置在离套管30的尖端大约mm,并且最好在大约3到6mm处,以监视通管针伸过的导管壁(比如尿道)的温度。提供这个热电偶是为了保证当正在对伸出电极周围的组织进行RF治疗时,导管壁的温度不达到破坏性水平。
图8是本发明的电流密度聚焦电极的环形坡口实施例的一个平面视图。在这个实施例中,电极被磨成一个单独的电流聚焦锐利尖端68,而没有同样能聚焦或积聚电流的第二棱或其它锐利边。由环形坡口70与72可以沿电极表面提供附加电流聚焦。电极周围的组织的温度最初在起始区域74、76与78中增加。随着坡口周围区域吞没,升高温度区域随后延伸成两个中间区域80与82。此后,所有升高温度区域吞没,以形成单独的椭圆形区域损坏84。使用这些电流聚焦坡口70与72,产生一个更对称的损坏。
图9是本发明的电流密度聚焦电极的螺旋形坡口实施例的一个平面视图。在这个实施例中,电极也被磨成一个单独的电流聚焦锐利尖端86,而没有同样能聚焦或积聚电流的第二棱或其它锐利边。由至少一个螺旋形或螺线形坡口88可以沿电极表面提供附加电流聚焦。电极周围的组织的温度最初在起始尖端区域90与螺旋形区域92中增加。随着螺旋形区域92吞没,以形成一个单独区域96,升高温度区域随后延伸成两个中间区域94与96。此后,所有升高温度区域吞没,以形成单独的椭圆形区域损坏84。使用螺旋形聚焦坡口88,以提供一个更对称的损坏。
图10是图1所示的RF烧蚀导管组件的一个分解图。上部手柄板276有两个中央槽口278与280,通过它们电极控制滑动装置10与11分别连接在左边电极滑块282及右边电极滑块284上。套管控制滑动装置12与13通过外边槽口286与288分别连接在左边套管滑块290及右边套管滑块292上。纤维光接收器槽30安装在邻近上部手柄板276的近表面处。电接收器294安放在上部手柄板276与下部手柄板3O0中的各自空腔296与298中,在那里被连接。下部手柄板300有一个中央空腔302,它接纳电极与套管滑块,以及连带的部件。
微动开关启动块304(仅仅表示了左边套管块)与套管滑块290及292连接。当各个套管块(以及在那里连接的套管)已被推进时,它们处在致动微动开关306的位置。微动开关306保持各个RF电源电路开路状态,直列各个套管被推到尿道壁之外并且进入前列腺的一个位置为止,以防止把尿道直接暴露在带电RF电极下。把套管伸出导向装置5mm通常足够保护尿道。
张力扭转管组件308安装在接收器310中槽的末端中。
图11是图10所示的RF烧蚀导管的调整块与张力管组件308的一个立方视图。扭转管312从扭转联接器314通过扭转控制把手316伸到通管针导向装置6。在使用中扭转管312的弯曲折褶延长了从手柄到导向尖端6的路径。为了防止在扭转管312被弯曲时所引起的通管针套管及电极部件的缩回,提供一个具有固定长度并且直径比扭转管312的内径小的张力管318。张力管318的末端牢固地连接在通管针导向装置6上,并且近端320例如用一种粘性物质固定在调整块322上。调整块322的轴向位置可以调整,以保证通管针8最初刚好位于通管针导向装置6中出口之内。扭转联接器314安装在联接器块324上。扭转控制把手制动销钉326伸进一个凹口中(没有表示),并且限制了控制把手316的旋转。
图12是图11所示的张力管的末端张力管连接处的一个详细视图“放大器”。张力管318例如用一段收缩导管330与通管针导向装置6的近端328牢固地连接。
图13是图10所示实施例的套管与电极滑块组件的一个分解视图。右边的套管滑块292有一个凸出部分332,它在右边电极滑块284下面向内伸长。右边套管连接器334安装在凸出部分332的内端面,固定在套管336的近端的端面。右边电极连接器338连接在电极滑块284的一个内表面上,并且固定在电极340的近端上。右边套管与电极滑块292及284通过槽口348与346用螺旋丝杆(没有表示)滑动地连接在右边摩擦调整轨342上,螺旋丝杆是可调整的,以在块与轨342之间提供足够的摩擦,以对通管针移动提供可靠的控制。左边套管滑块290与左边电极滑块282是右边块的镜式复制物,并且类似地安装在左边摩擦轨344上。左边套管与电极没有表示。
虽然本主题发明的优选实施例已经就某些细节作了叙述,但是不用说,在不违反附加权利要求所限定的本发明的精神和范围下,可以作出种种明显改变。
Claims (11)
1.一种医疗探测装置,用于对通过由一个壁所限定的自然人体内腔和向外提供的自然人体孔道可达到的人体中的一个治疗位置上的组织进行医疗,包括一个导向槽,具有近端和末端,并且具有一个从近端延伸到末端的通路,一个通管针滑动地安装在该通路中,并且该通管针包括一个具有一个尖端的软导电电极和一个同轴安装在该电极上但是露出尖端的绝缘套管,导向槽的末端带有的并且与引导电极的通路以及导向槽的绝缘套管边侧相联系的导向装置,耦连在导向槽的近端上的手柄装置,用于把导向槽的末端引入自然人体孔道,到达邻近治疗位置的一个位置,手柄装置包括用于推进通管针以使电极的尖端透入壁并用绝缘套管伸过壁来伸入治疗位置的细胞组织的装置,用于把电能供给电极以在治疗位置的组织中引起一个热效应的装置,同时绝缘套管保护该壁免于由电能引起的热效应,该绝缘套管具有一个从近端到末端的内腔。
2.如权利要求1的一种医疗探测装置,其中,绝缘套管有一个末端,温度传感装置安置在绝缘套管的末端处内腔中。
3.如权利要求2的一种医疗探测装置,其中,一个附加温度传感装置放置于绝缘套管的内腔中,该附加温度传感装置与第一指定的温度传感装置邻近隔开,由此当绝缘套管放置在壁中时,第一指定的以及附加的温度传感装置安排在壁的相对侧。
4.如权利要求1的一种医疗探测装置,其中,导向槽带有用于向绝缘套管的内腔提供液体的装置。
5.如权利要求4的一种医疗探测装置,其中,该内腔是一个环绕电极的环形空间,同导向槽耦连用于供给内腔一种药物的装置。

7.如权利要求6的一种医疗探测装置,其中,该绝缘套管具有一个附加内腔,从近端延伸到末端,与第一指定腔隔开,用于盐水溶液从治疗位置返回到导向槽的近端。
8.如权利要求1的一种医疗探测装置,其中,绝缘套管相对电极即由手柄装置携载的装置可移动,手柄装置用于在绝缘套管与电极之间引起相对移动。
9.如权利要求1的一种医疗探测装置,其中,电极是一种射频电极并且具有一个末端段,其上设置至少一个电流聚焦坡口,末端尖端成形为聚集它的终端上的电流,由此通过那里进入周围组织的射频电流形成一个从坡口及尖端向外延伸的损坏。
