Abstract

Description

脊椎植入物
本申请案是1993年10月1日申请的NO.130,288美国申请案的部分后续申请。
本发明涉及一种脊椎植入物,以及使用这种脊椎植入物把一根脊柱的相邻的脊椎骨结合在一起的方法。
一种公知的脊椎植入物是矩形的,并且具有楔形端部。这种脊椎植入物有许多抓住邻近脊椎骨的结块。这些结块都有可用来使脊椎植入物向前滑动时嵌入邻近的脊椎骨上切出的槽内的斜面。这种公知的脊椎植入物公开在美国NO.4,834,757专利文献中。为了容纳脊椎植入物,要在脊椎骨上切出许多槽来,神经末稍有损伤的危险。
本发明提供了一种新的改进了的脊椎植入物,以及使用这种脊椎植入物把一根脊柱的相邻的脊椎骨结合在一起的方法。本发明的脊椎植入物包括基本上互相平行地延伸的第一和第二侧表面。用于和相邻的脊椎骨连接的上表面和下表面延伸在第一和第二侧表面之间,并且从脊椎植入物的第一端部延伸到第二端部。本脊椎植入物包括可与一种工具连接的手段,当把脊椎植入物放在相邻的两块脊锥骨之间时,上述工具可用于转动该脊椎植入物。
使用上述脊椎植入物把一根脊柱的相邻的脊椎骨结合在一起的方法包括至少取掉两根相邻的脊椎骨之间的一部分椎间盘。然后把脊椎植入物插入两根相邻的脊椎骨之间,此时,基本上互相平行地延伸的第一和第二侧表面朝向相邻的脊椎骨。然后把脊椎植入物转动到这样的位置,即,互相平行的两个侧表面处在从一根相邻的脊椎骨延伸到另一根相邻的脊椎骨的位置,而上表面与下表面则与相邻的脊椎骨接触。在相邻的脊椎骨上没有切槽,因此,手术所需要的时间短,而且病人承担的风险小。

图1至图12表示了本发明的一个脊椎植入物,和把这种脊椎植入物植入相邻的脊椎骨之间的方法的第一个实施例。
一块或者几块基本上呈刚性的脊椎植入物10(其中的一块示于图1)并排地放在脊柱16的两块相邻的脊椎骨12和14之间,以便把这相邻的两块脊椎骨融合在一起。这种脊椎植入物10最好用切断的碳素纤维加强的聚合物注塑模压而成。但是,这种脊椎植入物10也可以用其他适当的可植入材料,例如不锈钢或钛制造。还有,用于制造脊椎植入物10的材料的抗拉极限强度最好高于每平方英寸10,000磅,以便脊椎植入物能防止相邻的脊椎骨12和14之间的相对移动,并支承脊柱的压缩载荷。
每一块脊椎植入物10(图2)都有两个平行的侧表面20和22。用于连接相邻的两根脊椎骨12和14的上表面24和下表面26延伸在侧表面20和22之间。邻近脊椎植入物10的第一端部30的上表面24和下表面26之间隔开的距离是第一间距。邻近脊椎植入物10的第二端部32的上表面24和下表面26之间隔开的距离是第二间距。第二间距最好大于第一间距,以便使脊椎植入物具有楔形,可用在带有脊柱前凸弧线的那部分脊椎骨。虽然图中表示的脊椎植入物10具有可用在带有脊柱前凸弧线的那部分脊椎骨的楔形,但是,这种脊椎植入物也可以具有可用在带有脊柱后凸弧线的那部分脊椎骨的楔形,或者根本就不是楔形。
上表面24和下表面26上有许多从侧表面20延伸到侧表面22的三角形的齿36,用于与脊椎骨12和14结合。每个齿36(图3)上都有一个朝向端部30的表面40,和一个朝向脊椎植入物10的端部32的表面42。齿36的表面40和42相交形成棱边44。相邻两个齿36的表面40和42相交形成棱边46。棱边46互相平行并且位于一个平面48上。
齿36的表面40与平面48成锐角X。齿36的表面42与平面48成锐角Y。角X和角Y最好相等,并且都是45°,以便使表面40和42互相垂直。因此,齿36是齐平的。上述齿36可防止脊椎植入物10向脊柱16的前部移动,同样也能防止脊椎植入物向脊柱16的后部移动。不过,角X和Y也可以按照外科医生的要求具有不同的值。
许多孔洞56和58从侧表面20通到侧表面22,以便于体液能从植入物10的一侧向另一侧流动和骨头的生长。孔洞58处在靠近植入物的端部30处。孔洞56和58垂直于侧表面20和22。通常,在侧表面20和22之间有五个孔洞56和两个孔洞58。孔洞56的直径大于孔洞58的直径。孔洞56和58的尺寸、形状和位置可以根据外科医生的要求而不同。虽然图中的孔洞56和58是垂直于侧表面20和22的,但它们也可以与侧表面20和22成锐角。此外,也可以设想,脊椎植入物10在侧表面20和22上具有用于容纳自体移植骨或者异体移植骨的骨头、骨蛋白质、骨头代用物或者类似物质的凹槽,用以代替孔洞56和58,或者同时具有这两者。
侧表面20上有凹槽60,而侧表面22上有凹槽62。孔洞58位于凹槽60和62中。凹槽60和62各有一个平坦的底面64。凹槽60和62还有垂直于底面64的,从脊椎植入物10的端部30向端部32延伸的平行的侧面66和68。弧形侧面70连接这两个平行的侧面66和68。根据植入物10是打算从后面,还是从前面,还是从前侧面植入,凹槽60和62可以位于侧表面20和22的任何位置上。侧表面20和22上的凹槽60和62是用来容纳器械80(图4)的,此器械夹持脊椎植入物10,以便于在相邻的脊椎骨12和14之间植入脊椎植入物,并且,当脊椎植入物处于相邻的脊椎骨之间时,用来转动该脊椎植入物。
任何能牢固地夹持住植入物,并能使植入物转动到位的器械都可以使用。其中的一种器械80(图4)有一个中间部分82(图5),一个手把84(图6)和一个夹持部分86(图7和8)。
上述中间部分82(图5)通常是圆柱形的,有一个沿着该中间部分的轴线延伸的孔洞92。此孔洞92包括一个从中间部分82的一个端部96延伸到中央部分98的第一圆筒部分94,一个从第一圆筒部分94延伸到孔洞92的圆锥部分102的第二圆筒部分100。孔洞92的第二圆筒部分100的直径大于第一圆筒部分94的直径。上述孔洞92的圆锥部分102从紧靠第二圆筒部分100的小直径逐渐扩大到与中间部分82的端部104相接的较大的直径。
中间部分82有一个大外直径部分108,它从端部96延伸到中央部分98。该108部分从大外直径逐渐缩小到小外直径部分110,而该110部分则从108部分延伸到端部104。108部分的外表面最好是滚花的,以便于外科医生握住该中间部分82。上述端部96则具有比108部分和110部分小的直径。
手把84(图6)有一个大直径的滚花部分116。从滚花部分116延伸出一根小直径轴118。这根轴118有一个带螺纹的端部120,用来连接夹持部分86。
夹持部分86(图7和8)有柄部126,柄部上有用于容纳手把84的带螺纹的端部120的内螺孔128。从柄部126伸出一对隔开间距的两半夹持器130。这对两半夹持器130具有向外呈锥形的表面132。该表面132从柄部126逐渐扩大到槽134。
两半夹持器130有一对延伸部分136,这对延伸部分可容纳在脊椎植入物10的侧表面20和22的凹槽60和62内。延伸部分136有平坦的内表面138(图7),以便与凹槽60和62的平坦的底面64接触。当用器械80夹持住脊椎植入物10时,延伸部分136的平行的侧表面140和142(图8)与凹槽60和62的侧表面66和68接触。用于与侧表面70接触的弧形侧表面144连接侧表面140和142。
手把84的轴118伸入中间部分82的孔洞92中,并且用螺纹与夹持部分86(图4)的柄部126连接。当手把84拧入夹持部分86的孔洞128中时,夹持部分就被拉入中间部分82的孔洞92中。两半夹持器130的锥形表面132与孔洞92的锥形部分102接触。当夹持部分86被拉入孔洞92时,两半夹持器130就被孔洞92的锥形部分102所压迫而靠近,从而在延伸部分136之间夹住脊椎植入物10。
下面描述把脊椎植入物10放入相邻的脊椎骨12和14之间,使相邻的脊椎骨结合在一起的方法。在脊椎骨12和14之间的椎间盘大都被取掉。脊椎骨12和14相对的表面用圆盘剃刀和修骨钳清理干净。通常在脊椎骨12和14之间留下椎间盘的一块环形物。
上述器械80是用来夹持一块脊椎植入物10的。脊椎植入物10根据外科医生的要求可以从前方,从后方或者从侧前方植入脊椎骨12和14之间,脊椎植入物的平行的侧表面20和22朝向邻近的脊椎骨12和14。图9中表示的是从后方植入的脊椎植入物10。脊椎植入物是这样植入的,即,端部32靠近脊柱16的前侧,而端部30靠近脊柱的后侧。然后脊椎植入物10旋转90,转到图10所示的位置,以便使上表面24和下表面26上的齿36与脊椎骨12和14接触,而侧表面20和22则处在脊椎骨12和14之间。脊椎植入物10的楔形的形状,减少了必须使脊柱的后部张开很大的距离,然后再压紧后部达到所需要的脊柱前凸的需要。只需要把脊柱的后部张开到所需要的插入盘的高度就可以了。
然后,把第二块脊椎植入物10与第一块脊椎植入物并排地植入脊椎骨12和14之间。第二块脊椎植入物10的植入方式和第一块相同。上述器械80用来夹持脊椎植入物10。脊椎植入物10植入时平行的侧表面20和22朝向脊椎骨12和14(图11)。然后把第二块脊椎植入物10转90°,转到如图12所示的位置,于是上、下表面24和26上的齿36就和脊椎骨12和14结合。
在两块脊椎植入物10之间和相邻的脊椎骨12和14之间剩下的空间,用自体移植骨或者异体移植骨的骨头、骨蛋白质、骨头代用物或者类似物质填充。在脊椎骨12和14完全融合在一起之前,要用一种使脊椎骨12和14保持要求的空间关系的装置,例如美国专利文献NO.46,96,290中公开的装置,安装在脊柱16上。这种使脊椎骨12和14保持要求的空间关系的装置可防止脊椎植入物10移动位置,和防止骨质接合物从两块脊椎植入物之间以及脊椎骨12和14之间掉出来。
在图1—12所说明的本发明的实施例中,脊椎植入物10上的孔洞56和58相对较小。在图13和14所说明的本发明的实施例中,在侧表面之间延伸的孔洞较大。由于在图13和图14中所说明的本发明的实施例大体上和在图1到图12中所说明的本发明的实施例相同,所以标示相同的组成部分都采用相同的数字,图13和图14中加在数字后面的后缀字母“a”是为了避免混淆。

Claims (23)

1.一种用来把一根脊柱的相邻的两根脊椎骨融合在一起的脊椎植入物,其特征在于,它包括:
基本上互相并行延伸的第一和第二侧表面;
在上述第一和第二侧表面之间延伸的,用来和相邻的两根脊椎骨连接的上表面和下表面;
第一端部和第二端部;以及
用于与一种器械连接的手段,该器械使上述脊椎植入物从上述第一和第二侧表面朝向相邻的脊椎骨的位置,转动到上述上表面和下表面与相邻的脊椎骨连接的位置。
2.如权利要求1所述的脊椎植入物,其特征在于,上述上表面和下表面从第一端部延伸到上述第二端部,上述靠近第一端部的上表面和下表面之间隔开的距离是第一间距,而上述靠近第二端部的上表面和下表面之间隔开的距离是第二间距,并且第二间距大于第一间距。

14.如权利要求1所述的脊椎植入物,其特征在于,上述与用来转动上述脊椎植入物的器械连接的手段包含在上述第一和第二侧表面上的两个凹槽,以便容纳夹持和转动上述脊椎植入物的器械。
15.如权利要求2所述的脊椎植入物,其特征在于,上述用来与转动上述脊椎植入物的器械连接的手段具有位置靠近上述脊椎植入物的第一端部的两个凹槽。
16.一种把脊柱的两根相邻的脊椎骨融合在一起的方法,其特征在于,它包括下列步骤:
除掉至少一部分两根相邻的脊椎骨之间的椎间盘;
预备一块第一和第二侧表面基本上互相平行,并且在第一和第二侧表面之间有上表面和下表面延伸着的脊椎植入物;
把上述脊椎植入物植入相邻的两根脊椎骨之间,并且使该脊椎植入物的基本上平行的第一和第二侧表面朝向相邻的两根脊椎骨;
把脊椎植入物转动到这样的位置,在此位置上,脊椎植入物两个平行的侧表面从相邻的一根脊椎骨延伸到另一根脊椎骨,而上表面与下表面与相邻的两根脊椎骨接触。
17.如权利要求16所述的方法,其特征在于,它还包括提供具有第一端部和第二端部的脊椎植入物,其中,上表面和下表面在靠近第一端部处隔开一个第一间距,而在靠近第二端部处隔开一个第二间距,并且第二间距大于第一间距。
18.如权利要求17所述的方法,其特征在于,上述植入脊椎植入物的步骤包括把脊椎植入物从脊柱的后方植入到这样一个位置,在这个位置上,脊椎植入物的第一端部位于靠近脊柱的后方的位置上,而脊椎植入物的第二端部位于靠近脊柱的前方的位置上。
19.如权利要求16所述的方法,其特征在于,上述植入脊椎植入物的步骤包括并排地植入两块脊椎植入物。
20.如权利要求16所述的方法,其特征在于,它还包括,在转动脊椎植入物的步骤之后,把骨移植物填在脊椎植入物的周围的步骤。
21.如权利要求16所述的方法,其特征在于,它还包括把一种用于保持上述两根脊椎骨所要求的空间关系的装置安装在脊柱上的步骤。
22.如权利要求16所述的方法,其特征在于,上述除掉至少一部分椎间盘的步骤包括沿相邻的脊椎骨的周边上留下一圈环形的椎间盘。
23.如权利要求16所述的方法,其特征在于,它还包括把骨移植物的材料填入穿过脊椎植入物的一个孔洞的步骤。
