Abstract

配有一次性使用端件的电手术器械
发明领域
本发明涉及手术器械,这种器械具有一可重复使用的构件以及一固定连接于该可重复使用构件的一次性使用构件。具体地说,本发明涉及具有可重复使用的电极延伸轴以及一次性使用的端件的电手术器械。本发明可广泛应用于打开式或最低限度侵入手术的电手术器械(既可以是单极的也可以是双极的)。
发明背景
随着手术知识和技术的进步,相应产生了要求减小手术切口和侵入器械尺寸的趋势,藉以减轻病人的损伤,有助于病人迅速地恢复。这已经导致产生利用小的医用电极来实施腹腔镜和其它外科手术。艾滋病和其它高度危险或致命的传染性疾病已经显示了采用一次性使用构件或使用一次以上但彻底消毒构件的重要性。
为了防止疾病的传播,在减少手术设备总成本的同时,迄今已经提出了各种各样的电手术技术和工具,如1933年7月4日授予Ende的美国专利1,916,722、授予Weissman等人的且于1974年9月19日公开的德国专利申请2,404,764、1996年7月2日授予Nettekoven等人的美国专利5,531,743、Nettekoven等人于1995年10月24日提交的美国专利申请08/547,571、W.S.Walker于1982年1月25日提交的PCT国际申请82/0084、以及E.Langberg于1991年8月2日提交的PCT国际申请91/05520。根据这些专利的建议,多构件工具已经包括了带有可去除并抛弃的端件或电极的可重复使用主体。如果需要,主体或支座可以消毒和重复使用,而端件、刀片或电极可以在一次使用之后丢弃或经消毒并重复使用。
然而,在已有技术中建立起来的各种多构件电手术器械通常有很严重的限制和缺陷。例如,已有技术的某些装置在一次性使用的端件和可重复使用的电极轴之间采用相对较为简单的压配或摩擦连接,这样就有一些缺陷。首先,这样一种连接可无意地导致端件在手术过程中从支承轴上脱落,于是对病人造成危险,甚至可能需要再进行侵入性手术以将脱落的端件取出。另一个限制是,这样一种结构会使端件在使用过程中相对于轴转动。由于很多电手术端件是非对称的,使端件的位置相对于支承轴固定有助于医生在外科手术过程中采取正确的取向,从而使医生可以在手术过程中更精确地将手术端件定位在病人体内所需的手术位置上。另外,已有技术的很多装置都遇到这样的问题,即,端件经常不能刚性地固定于支承轴的端部。应该理解,器械在手术医生手中的感觉是非常重要的,因此,一次性使用端件与支承轴之间的接头处的任何游隙都是不希望有的。另外还有,已有技术中的很多装置不能使支承轴与端件之间的连接充分绝缘而免于暴露导电表面,该导电表面可能在手术过程中与病人接触而形成一个非所愿的、对病人的导电通路,并且在手术部位造成意外的灼烧或切割。这特别是涉及适用于腹腔镜手术的电手术器械,在此情况下,非所愿的灼烧或切割可能发生在腹腔镜提供给医生的视野之外。因此,还是需要有一种改进的配有一次性使用端件(resposable)型电手术器械,它包括一可重复使用的电极轴、一一次性使用的端件和一位于两者之间的改进的联接件,这种器械可以克服上述的缺陷。
本发明的概要和目的
根据本发明的改进的、配有一次性使用端件的电手术器械包括一可复用的电极轴、一一次性使用的端件和一用于将所述一次性使用端件可拆除地连接于电极轴的新颖的绝缘联接件。本发明具有若干优点和特征。联接件可便于在手术室环境中将一次性使用端件连接于可复用电极轴。一旦将端件连接于轴,联接件就可以确保在端件和轴之间有刚性而可靠的连接,这样就使手术医生感觉到联接后的端件/轴组件像是一个单件的整体式器械。重要的是,本发明的联接件是设计成使手术医生在使用该器械时的动作不会导致端件从轴上意外地脱落。此外,联接件可确保在端件和轴之间有良好的电连接,防止端件不当心地或意外地从可复用电极轴上脱落,并且可以防止端件相对于轴或者是轴相对于端件的转动和纵向移动。另外,联接件还是自对准的,即,它不允许端件联接于轴,除非是这两个构件相互对准,并且不需要医生用肉眼检查和对准这两个构件。
联接件还包括一个护套,该护套与联接件的导电表面电气绝缘,从而防止在病人和器械之间除了手术端之外还有其它的不希望有的导电通路。此外,该护套还可以提供一个围绕联接件之机械构件的有效的流体密封,藉以防止污染物或其它生物有害物的积聚。
本发明的目的是改进电手术器械。本发明的其它目的是:提供一种具有一可复用主体部分或轴以及一一次性使用端件的、改进的、配有一次性使用端件的电手术器械;提供一种改进的联接机构,用于将一一次性使用电手术端件连接于一可复用电极轴;提供一种具有一可复用电极延伸轴和一一次性使用端件的、改进的、配有一次性使用端件的电手术器械,它可以防止一次性使用端件在正常使用过程中无意地或意外地从可复用电极延伸轴上脱落;提供一种具有一可复用电极延伸轴和一一次性使用端件的、改进的、配有一次性使用端件的电手术器械,它是自对准的,并且可以保持端件相对于轴的取向:提供一种具有一可复用电极延伸轴和一一次性使用端件的、改进的、配有一次性使用端件的电手术器械,它可以防止在器械和病人之间产生除了所需手术端之外的其它不希望有的导电通路:提供一种改进的、配有一次性使用端件的电手术器械,它有利于和可复用电极延伸轴一起使用的涂层电手术端件的使用,所述端件通常是用后即弃的。

对一较佳实施例的详细描述
在进行详细描述之前,对术语“配有一次性使用端件”加以定义是有帮助的。在文中,“配有一次性使用端件”意味着这样一种装置,即,其中的一个构件(如一手术端件或刀片)可以任选地抛弃,而其中一个或多个其它构件(如一用于有选择抛弃部件的保持装置)是可重复使用的。
现请参见附图,特别是图1,其是由标号10来总地表示的本发明的电手术组件的立体图。电手术组件10大致包括一个手握杆或笔12、一细长的延伸轴或电极14、一一次性使用的电手术端件16、以及一用于有选择地将端件16联接于轴14的装置,这个联接装置总的由标号18来表示。有时可以将轴14、端件16和联接装置18总地称作为“电手术器械”。
笔12最好是设有一启动开关20,用以通过端件16对一病人的所需治疗部位施加射频(RF)能量。RF能量是通过一连接器22和电线24从一适当的电手术装置(未图示)提供的。虽然在图中开关20位于笔12上,但也可以采用一脚动开关以及其它任何可以对电手术器械施加电能的装置。
现请参见图2并且更详细地如下所述,笔12在其末端具有一圆柱形凹槽26,该凹槽是形成为能以相配的关系接纳轴14的近端。熟悉本技术领域的可以理解,一导电材料围绕圆柱形凹槽26内壁的至少一部分,该导电材料可以电连接于启动开关20的输出脚或极。

请继续参见图2和4,从中可以看到,端件16由一在其近端具有一键槽52的端部50、一横截面积缩小的颈部54、以及一位于其末端的刀片或工作端部56。另外,颈部54的一部分和刀片部分56上覆盖有一层合适的电绝缘材料58。此外,在本实施例中,刀片部分56最好是涂覆有一层合适的非粘性涂层,详细地请参见授予Blanch的美国专利4,785,807和4,876,110所述,这两个专利的内容可援引在此以作参考。在该实施例中,图中所示的端件或刀片部分56具有相对较为钝的、扁平的和带角度的构造。然而,应该理解,本发明的电手术端件可以采用其它任何形状或构造,端件的特定形状或构造并不是本发明的关键,图中所示仅仅是为了说明,并不对本发明的范围有任何的限制。
在下文中将会更详细地描述,键槽46和52是这样构造的,即,当轴14和端件16相互联接时,这两个键槽相互配合。一旦以下述方式可靠地联接起来,键槽46和52就会相互协作而在轴14和端件16之间形成一个电连接。此外,键槽46和52相互协作,以防止端件16相对于轴14或是轴14相对于端件16转动。
本发明的配有一次性使用端件器械还包括一套筒60和一护套或套靴61。在该较佳实施例中,套筒60最好是用一种合适的刚性医用等级的塑料(如聚碳酸酯)制成,但也可以用任何其它合适的材料,如金属、陶瓷等制成。护套或套靴61最好是用合适的电绝缘、且为柔性的医用等级高弹体制成。最好如图2和4所示,套筒60的横截面大致为圆筒形,其具有一近端62和一远端64。套筒60的近端62包括内螺纹66,这些内螺纹是构造成与轴14的外螺纹螺旋啮合。套筒60的远端64包括一内径减小的颈圈68,其内径接近于端件16的颈部54的外径。最好如图2所示,套筒60还包括一纵向槽70,其延伸的长度约为套筒60长度的一半,即,从套筒60的中点开始,延伸过套筒60的末端64和颈圈68。还是如图2所示,槽70包括一靠近颈圈68的加宽部分72。
最好如图4所示,护套或套靴61的形状大致为管状,并具有一近端80和一远端82。套靴61的内径基本上等于或略小于套筒60的外径,以便以大致压配的关系插入并配合于套靴61的内表面。套靴61的末端82终止在一内颈圈84和一外颈圈86中。内颈圈84的结构和尺寸是,当把套筒60插入套靴61时,可以紧贴地围绕套筒60的末端64。外颈圈86的结构和尺寸是,当以下述方式将电手术器械完全装配起来时,可以紧贴地围绕端件16的绝缘材料58的外表面。(如图3所示)。
端件16、套筒60和护套61一起形成了一个一次性使用的电手术端组件。按照本发明的一个较佳的方面,端件16、套筒60和套靴61将被预先组装起来,并作为单个的一次性使用装置出售,而延伸轴或电极14则可以反复消毒,供多次手术反复使用,这样就可以减少设备成本,进而降低腹腔镜手术的总的成本。若要装配本发明的端组件,需将端件16的端部50穿过形成在套筒60上的槽70的加宽部分72,并使端件16的颈部52位于套筒60的颈圈68中。应该理解,虽然相对较硬的套筒60具有足够的柔性,可以临时变形而让端件16的端部50通过槽70的加宽部分72插入从而位于套筒60中。一旦端部50和颈部52以此方式位于套筒60中,就可以用柔性的护套或套靴61套住套筒60以及端件16的中间部分,这最好如图3所示。
本发明还提供了用于有选择地并且可靠地将端件16联接于轴14末端的装置。在该实施例中,联接装置包括用于对准和保持端件16与轴14之间的相对位置的装置、用于有选择地和可拆除地将端件16紧固于轴14的装置、以及用于使联接装置密封和电绝缘的装置。
在图示的实施例(它也是本发明实施例的一种简单表示)中,与对准和保持装置相应的结构是轴14的键槽46和与之相协配的端件16的键槽52。虽然所描述和图示的键槽46和52具有大致为半圆形的横截面,并带有平的相配表面,但熟悉本领域的人员应该可以明白,可以采用任何数量的机械联接件来实现相同或等同的功能。例如,轴14可以终止在一个花键处,该花键与形成在端件16近端的一个相应的花键槽相配。本领域熟知的任何适当的机械联接件都可以实现使轴14与端件16相互对准的功能,随后再维持轴14和端件16的相对对准的位置,这些均应落入术语“对准和保持装置”所涵盖的范围内。

在图示的实施例(它是本发明实施例的一种简单表示)中,与密封和绝缘装置相应的结构是套靴61。虽然描述和图示的套靴61是一个大致为管状的柔性、弹性的高弹体,但熟悉本领域的人员应该理解,可以采用任何数量的变化型实施例和结构来实现相同或等同的功能。因此,本领域内熟知的、能对联接装置进行相同或等同的密封和电绝缘作用的适当材料和结构都应落入术语“密封和绝缘装置”的范畴内。
一旦把端件16、套筒60和套靴61以上述方式装配起来,就可以将端件16有选择地和可拆卸地联接于轴14,即,将轴14的末端36插入套靴61的近端,随后再插入套筒60的近端。如果需要,接着可以使轴14相对于端件16(或使端件16相对于轴14)转动,直到键槽46和52正确地对准而进一步地接合。一旦使键槽46和52以此方式对准,就可以将轴14进一步地插入套筒60和套靴61,直到轴14的外螺纹44开始与套筒60的内螺纹相啮合。最后,当螺纹44和66开始相互啮合时,套筒60和套靴61就相对于轴14和端件16转动,直到螺纹44和66完全啮合,并且键槽46和52牢固而可靠地相互抵触。
键槽46和52以及螺纹44和66的相对尺寸和间隔是,螺纹44不能啮合于螺纹66,除非并且直到键槽46和52正确地对准(至少要相互配合)。在此方式下,本发明的电手术器械是自对准的,端件16不能联接于轴14,除非并且直到轴14和端件16正确地对准。此外,将轴14和端件16对准的工作不需要医生用肉眼来检查器械,也不需要用肉眼来检查设置在装置上的任何标记。相反,手术医生可以简单地使端件16相对于轴14转动,直到键槽46和52正确的对准,这可以仅仅通过医生的感觉就可以进行检测,即,随着键槽46和52的前边缘的相互接触,可以使端件16沿纵向相对于轴14轻微地移动。然而,当螺纹44的前边缘抵靠于螺纹66的相对的前边缘时,这种纵向的移动是有限的。

Claims (17)

1.一种改进的电手术器械,包括:
一细长的导电轴,它具有一近端部分、一中间部分和一远端部分,其中中间部分覆盖有一层电绝缘材料;
一电手术端件,它具有一近端部分、一中间部分和一远端部分;以及
用于有选择地和可靠地将所述端件联接于所述轴的远端部分的装置,藉以防止所述电手术端件相对于所述轴纵向移动和转动。
2.如权利要求1所述的电手术器械,其特征在于,所述联接装置包括:
一第一装置,用于对准和保持所述轴和形成在所述轴之远端部分的所述端件的相对位置;以及

其中,所述外螺纹部分和内螺纹部分是构造成能以互补的配合关系相互配合。
13.如权利要求12所述的电手术器械,其特征在于,所述器械还包括一细长的、大致管状的、电绝缘的、柔性的弹性护套,该护套以紧容差、压配的方式套覆并围绕所述联接装置。
14.一种改进的电手术器械,包括:
一细长的导电轴,它具有一近端部分、一中间部分和一远端部分,其中中间部分覆盖有一层电绝缘材料;以及
一一次性使用的电手术端组件,其中端组件以这样一种方式有选择地和可拆卸地连接于所述轴的远端部分,即,所述端组件刚性地固定于所述轴,因而可以防止所述端件相对于所述轴纵向移动和转动。
15.如权利要求14所述的改进的电手术器械,其特征在于,所述端组件包括:
一导电的电手术端件,它具有一近端部分、一中间部分和一暴露在其远端的工作面,其中,所述中间部分覆盖有一层电绝缘材料;
一细长的、基本上刚性的、大致为圆筒形的套筒,该套筒具有一近端、一中间部分和一远端,其中,所述套筒的远端围绕并机械地联接于所述端件的近端部分;以及
一细长的、大致管状的、电绝缘的、柔性的弹性护套,该护套以基本上紧容差、压配的方式套覆并围绕所述套筒以及所述端件的近端部分。
16.如权利要求15所述的改进的电手术器械,其特征在于,所述轴还包括一在其远端的第一键槽,其中所述端件还包括一形成在其近端的第二键槽,其中所述第一和第二键槽是形成为能以互补的配合关系相互配合,从而将所述端件对准所述轴,并防止所述端件相对于所述轴转动。
17.如权利要求16所述的改进的电手术器械,其特征在于,所述轴还包括一围绕所述轴的远端部分形成的外螺纹部分,该部分靠近并向内地间隔于所述远端部分,其中,所述套筒还包括一形成在所述套筒的近端部分的内螺纹部分,以及其中所述外螺纹和内螺纹部分是构造成能以互补的配合关系相互配合,从而使所述第一和第二键槽能保持相互配合,并防止所述端件相对于所述轴纵向移动。
