技术领域
本发明总体涉及一种可植入的装置和植入操作,尤其是涉及一种用于控制一个解剖孔或者腔室的内周边的可植入的装置和操作。 
背景技术
哺乳动物体内的许多解剖结构是中空的通道,其中该通道的组织的内壁限定一个中心腔室,该腔室作为血液,其它生理液体,营养物或者废物流经该结构的导管。在许多生理结构(setting)中,功能障碍是由于结构腔室太大或者太小所造成的。在绝大多数这样的情况下,功能障碍可以通过干预性地改变腔室的尺寸而减轻。 
因此在外科手术中,经常需要减小孔或者其它具有开口的解剖结构的内周边,从而减小孔或者开口的尺寸来获得所需的生理效果。通常,这样的外科手术需要阻断血液、其它生理液体,或者其它结构内容物通过孔或结构的正常生理流动。在流经孔或者结构的生理流体重新恢复流动之前,达到所需效果所需的精确减小量经常是很难估计。因此,具有一个可获得该缩窄效果的可调节的机构是有利的,使得在植入之后可以改变缩窄的程度,并且在正常流动恢复之后在原位置也可以改变缩窄的程度。

通常,大多数瓣膜病理是风湿性心脏病的继发病,作为链球菌感染的结果,通常会影响到二尖瓣,主动脉瓣,以及肺动脉瓣。这些感染性过程的结果是二尖瓣狭窄和主动脉瓣狭窄,导致二尖瓣关闭不全和主动脉瓣关闭不全。随着更好的抗生素疗法的出现,风湿性心脏病的发病率降低,在世界发展的今天占据了心脏瓣膜疾病的一小部分。风湿性二尖瓣狭窄切开术成为先天性心脏病领域外的一般性二尖瓣修复的一个较早的例子。然而,由于潜在的瓣膜病理以及疾病的发展,对风湿性瓣膜关闭不全的修复没有获得较好的效果。 
除了风湿病之外的许多二尖瓣疾病导致了瓣膜关闭不全,这种通常可以修复。腱索破裂是导致二尖瓣关闭不全的一个普遍原因,导致在病灶区域心脏血液返流。传统地,第一个成功的和可接受的外科修复之一是用于修复后部二尖瓣小叶破裂的腱索。该修复的技术可行性,良好的再生性,以及长时间的耐用性使得二尖瓣膜修复领域的外科创始人试图修复其它的瓣膜病症。 
二尖瓣脱垂是导致长时间的瓣膜关闭不全的相当常见的情况。对于该疾病,前部和后部小叶的连接面相对于正常的瓣膜是“心房化”的。该疾病可以通过把连接面修补到心室中而容易地修复。 
左心室中的乳头肌支撑二尖瓣膜并有利于二尖瓣的功能。无论是由于冠状动脉疾病导致的梗塞还是由于局部缺血导致的乳头肌的功能障碍,常常会导致二尖瓣关闭不全(通常称之为缺血性二尖瓣关闭不全)。在二尖瓣膜疾病的范围中,这是瓣膜修复中发展最快的领域。传统地,仅仅只有具有严重二尖瓣关闭不全的病人可以修复或者替代,但是在外科学文献中具有足够的证据支持具有较轻的关闭不全的病人的瓣膜修复,该关闭不全是由于缺血性二尖瓣膜功能不全。在这种病人中的较早的瓣膜修复显示出能够提高存活率 以及改善长期的心室功能。 
此外,在心肌扩大的病人中,二尖瓣膜关闭不全的病因在于由于扩大的心室瓣膜小叶缺乏扩张。由于缺乏小叶的扩张,导致血液回流。修复这些瓣膜的趋势逐渐增长,从而修复关闭不全和修补心室形状,因此改善整个心室功能。 
二尖瓣修复的两个主要的特征在于固定主要的二尖瓣假体(如果存在)以及支撑环或者使用假体减小环的尺寸,该假体通常为一种环或者带的型式。在二尖瓣修复中所存在的问题是外科医生不能完全地估计修复的效果,直到心脏完全关闭,并且病人切断心肺旁路。一旦这些已经完成,可以在手术室通过经由食管的超声波心动描记法(TEE)对瓣膜功能进行估计。如果仪器记录了显著的残留的瓣膜关闭不全,外科医生必须重新停止心脏的跳动,重新打开心脏,然后再次修复或者更换瓣膜。这增大了整个手术、麻醉和旁路的时间,因此增大了整个手术的风险。 
如果用于减小环的假体比理想的尺寸大,二尖瓣关闭不全会持续。如果假体太小,会导致二尖瓣狭窄。因此需要一个可调节的假体,该假体允许外科医生在一个跳动的心脏中原地调节环的尺寸,在TEE的指导或者其它的诊断型式下获得最佳的瓣膜关闭和功能。 
心脏手术仅仅是所述调节中的一个例子,其中解剖孔的环尺寸的原地调节是所需的。另一个例子是在肠胃手术的领域,尼森胃底折叠术长期用于使胃食管连接处变窄,从而解除胃中内容物回流到食管。在该调节中,外科医生通常面临这样的压力:即使得连接处足够狭窄从而获得对回流的控制,但是又要避免过度的狭窄,过度狭窄会干扰来自食道的营养物送入胃中的通道。再次地,需要一种方法和装置,通过该装置可以对胃食管连接处进行原地调节,以达到所需的狭窄程度,从而在使得连接处足够狭窄和避免过度狭窄这两方面获得最佳的平衡。

因此,需要一种输送系统和方法,可以避免在这种典型情况下进行开刀手术,并且允许输送,放置,和调节假体植入物从而在经皮的或者其它微创手术中减小这样的二尖瓣环的直径,同时还获得临床的和生理的效果,这些效果至少与用于解决这些相同问题的最好的外科手术等效。

发明内容

现在参照附图,其中附图中相似的附图标记用于表示相似的部件,图1显示了一个示例的具有植入体15的植入物10。植入体的形状和尺寸可以由哺乳类治疗对象预定的原始接受组织部位的解剖需要决定。例如,这样的一个原始接受组织部位可以是,通过示例的方式并且不仅限于,心瓣膜,靠近胃-食管连接处的食道,肛门,或者哺乳动物体内的其它的发生功能障碍的解剖部位,该障碍可以通过植入物得到解除,该植入物可以改变部位的尺寸和形状,并且在手术后保持所需的尺寸和形状。 
图1中的植入物10包括一个环形的植入体15,该植入体具有一个可调节的褶皱部20,该褶皱部与套环连接机构25交替排列,该连接机构25具有窄的中间颈部。如图2和3所示,植入体15可以通过一个固定装置,例如固定在或者穿过连接机构25的缝合线35固定到心瓣膜30的环。当植入体15的周边变短或者变长时,褶皱部20折叠或者展开。植入物10的原地调节可以减小心瓣膜30的整体尺寸,增大瓣膜小叶40的接合,把图2所示的结构改变成图3所示的结构。 
本发明另一具体实施例100显示在图4和5中,图4中所示的心脏110具有一个开放的手术心脏切口105,图5中的心脏切口105关闭。如图4所示,根据本发明的示例的可调节植入物100包括一个带有连接装置120的植入体115,该连接装置固定在二尖瓣125的环上。示例的可调节植入物100还具有一个调节装置130,该调节装置通过一个粘附的或者连接的调节机构135控制。在图5所示的心肌切口105关闭之后,调节机构135保持粘附或者连接在调节装置130上,因此在经过心脏110的生理流体重新流通后,植入物100的尺寸和形状可以进一步被影响,但是胸部切口仍然保持打开。一旦获得所需形状和功能,调节机构135可以从调节装置130上拆除并且从心肌切口105中取出。在根据本发明的不同的实施例中,调节装置130可以构 造和设置成允许由调节机构135保持或者由调节机构135再次导入调节机构135中,用于调节下述的胸部切口的关闭。 
为了使用图4和5中所示的植入物100,内科医生以传统的方式在心脏110上切开一个开放的手术切口105,如图4所示。安装在调节机构135前端的植入物100,穿过切口105移动并且缝合到二尖瓣125的环上。然后操作调节机构135,例如,旋转,这取决于调节装置130的设计,导致调节装置减小植入体115的尺寸,因此缝合在其下的二尖瓣125获得一个合适的尺寸。如图5所示,心肌切口105现在可以被关闭,留下调节机构延伸穿过用于后期手术调节的切口。 
一旦病人离开“泵”并且经过心脏110的正常血液流动重新开始,但是在胸部切口关闭之前,通过操作调节机构135可以获得对二尖瓣125尺寸的进一步调节。 
图6-8示出了一个示例的调节装置200,用于调节环形植入物,例如前述的植入物100的周边。调节装置200包括一个齿条齿轮系统,其中一个具有啮合齿210的第一凸轮205和一个啮合连接器215在第一轮轴220上旋转。在该实施例中,第一凸轮205与一个啮合齿条225在一个或者多个第一带230的表面上相啮合。第一带230位于第一凸轮205和第二凸轮235之间,所述第二凸轮235在第二轮轴240上旋转,所述第二轮轴240连接在第二带245上。如图8所示,第一和第二轮轴220,240通过一个形成在第二带245端部的支架250保持适当的间隔关系。 
调节装置200优选地设置在前述类型的中空环形植入物100中,虽然在一个单独的结构中使用调节装置是可能的,其中第一和第二带230,245为相同的连续环形结构的对置端。在任一种情况下,用于调节植入物的长度的装置包括调节装置200,用诸如六角扳手这类的工具将第一凸轮205与啮合连接器215相啮合,并且在如图7所示的反时针方向旋转第一凸轮,如箭头255所示。第一凸轮205的旋转导致齿210驱动齿条225朝着右侧移动第一带230,如图7中的箭头260所示。第一带的移动拉紧了环形植入物的周边。如果内科医生疏忽地把植入物调节得太紧,啮合连接器215的反方向将使得植入物松动。 
根据本发明的不同的实施例,第一和第二带230,245可以为独立的结构,或者它们可以是相同的连续结构的对置端。在这样的一个实施例中,当移动传递给啮合连接器215时,第一凸轮205旋转,导致啮合齿210与啮合齿条225相啮合,并且导致第一带230相对于第二带245移动,从而调节植入物的周边。 
图9示出了根据本发明的一个示例的啮合装置300的稍微不同的结构,其中没有啮合连接器,并且支架350设置在凸轮的两侧从而使得第一凸轮315和第二凸轮320保持靠近。在一个设想的实施例中,支架具有紧公差,因此把第一带330紧压在第二带345上,从而通过摩擦以固定的相对位置保持带。在另一个设想的实施例中,支架350由一种弹性材料制造,因此凸轮315,320可以分离开从而在凸轮之间插入第一带330,因此凸轮通过足够的力被拉回在一起并通过摩擦使带330,345以固定的相对位置被保持。在本发明的又一个设想的实施例中,包括一个设置在凸轮315,320之间的弹性安装件,第一带330的下边缘和第二带345的上边缘具有啮合摩擦表面或者机加工表面,因此凸轮315,320可以分离,从而允许带之间相对的移动,或者松开以固定关系夹持所述带。 
图10示出了根据本发明的植入物的一个示例的连接装置400。连接装置400例如可以替代植入物10的连接装置25。连接装置400为包括一个壁415的套环410的型式,该壁限定了一个管腔420和一个连接表面425。这样的连接装置可以这样使用,即植入体延伸穿过管腔420以及通过固定装置,例如缝合线或者线,可以系紧在连接表面425上,或者连接穿过连接表面425。 
图11示出了根据本发明的用于植入物的连接装置500的另一个可选择的实施例。该连接装置500例如也可以替代植入物10的连接装置25。图11示出的连接装置500为一种中空管或者管段510的型式,该管包括一个限定了管腔520的壁515,一个外表面525,和一个连接片530。这样的连接装置可以这样使用,即植入体延伸穿过管腔520并且通过固定装置,例如缝合线或者线,系紧或者连接在或者连接穿过连接片530。这样的固定装置可以穿过设置在连接片530上的孔535。可选择地可以提供一个实心连接片530, 固定装置可以穿过实心固定片。这些连接装置的改进可以与一个没有缝合线的连接系统结合使用。 
图12-18示出了根据本发明的经皮的瓣环成形术装置的另一个实施例,其中一个植入/输送系统阵列600包括一个腔室外壳605(图12中没有显示),一个安装在腔室外壳605中的可以同轴滑动的致动导管610,以及一个安装在致动导管610中的可以同轴滑动的中心导管615。中心导管具有一个中心腔空616(图13)。致动导管610和中心导管615为环形结构,或者如图13所示,致动导管和中心导管中的任一个或者两者分别具有一个或者多个带键的凸缘618,620,这些凸缘分别被腔室外壳605或者致动导管610的内部腔室中的互补的狭缝622,624接收。这样的带键的凸缘618,620限制内部元件在外部元件中的内旋转,这样的限制是为了保持对在使用过程中由于不希望的旋转运动所导致的内部内容物的反向移动的控制。 
植入/输送系统阵列600包括一个在中心导管615的前端的远端625。一个或者多个径向植入物支撑杆630具有可旋转地或者可弯曲地靠近远端625安装在中心导管615上的远端632。径向植入物支撑杆630的近端634通常沿着中心导管615延伸,但是可以设置成从中心导管向外。 
一个或者多个径向支撑支柱636具有可旋转地或者可弯曲地安装在致动导管610的远端上的近端638。每个径向支撑支柱636的远端640可旋转地或者可弯曲地连接在相应的径向植入物支撑杆630的中点上。当致动导管610相对于中心导管615移动时,径向支撑支柱636以伞架的型式压迫径向植入物支撑杆630向上和向外。因此致动导管610,中心导管615,径向支撑支柱636和径向支撑杆630共同形成一个展开的伞形物642。 
一个假体植入物645可拆卸地连接在径向植入物支撑杆630的近端634上。多个保持倒钩646围绕假体植入物645的周边并且靠近该周边延伸。此外,一个或者多个径向植入物支撑杆630包括接地(touchdown)传感器648,该传感器的近端靠近植入物645延伸。一个或者多个释放元件660在典型实施例600中延伸穿过中心导管615的中心腔室616(图13)并且从侧部650 
(图12)延伸出与远端625隔开,所述释放元件660用于从输送系统上释放植入物645,以及一个或者多个用于调节植入物的展开尺寸和效果的调节 元件665。因为释放元件660和调节元件665延伸穿过中心导管615的近端,如同图14-16中所示,这些元件可以直接或者间接由内科医生操作或者操纵。一个输送介面670(附图12,16)在该实施例中由展开伞形物642,释放元件660和植入物645的相互作用限定。在公开的实施例中,释放元件660可以为一种缝合线,纤维,或者环形的线,该释放元件经过植入物645和径向植入物支撑杆630上的激光钻孔,然后经过中心导管615的长度。在这样的一个实施例中,植入物645可以在需要的时间通过剪断在病人体外的释放元件660的近端从输送系统上释放,然后通过中心导管610收回释放元件660的自由端。 
图14-16示出了植入/输送系统阵列600的操作,其中假体植入物645的伞形展开通过腔室外壳605、致动导管610和中心导管615的滑动获得。首先参照图14,腔室外壳605延伸覆盖致动导管610和中心导管615的前端,用于植入/输送系统阵列600的血管插入。从该开始位置,腔室外壳605在以箭头662所示的方向上缩回。在图15中腔室外壳605缩回从而暴露致动导管610和折叠的展开伞形物642的前端。从该位置致动导管610在以箭头664所示的方向上移动。这将导致展开伞形物在以箭头666所示的方向上张开。图16示出了通过致动导管610相对于中心导管615的远端运动而使得伞形物642张开。在植入物645定位和调节到适当的尺寸之后,腔室外壳605在以箭头668所示的方向上移动,从而折叠和覆盖展开伞形物642,用于从病人身上取下装置。 
图17和18为示意图,示出了植入/输送系统阵列600的径向支撑支柱636和径向植入物支撑杆630。在图17中,一个径向支撑支柱636在它的近端638的一个第一枢转点可旋转地连接在致动导管610上。径向支撑支柱636的远端640连接在一个第二枢转点672的相应的径向植入物支撑杆630的中间点上。径向植入物支撑杆630的远端632通过一个第三枢转点674连接在中心导管620上。图17示出了折叠状态的组件。当致动导管610在中心导管615上移动,如箭头676所示,径向支撑支柱636和径向植入物支撑杆630通过在第一枢转点670,第二枢转点672和第三枢转点674上的运动延伸,如箭头678所示。该运动具有张开展开伞形物和折叠的植入物(图 17和18中没有显示)的效果,允许伞形物在前面所讨论的参照图12-16的啮合和植入之前获得最大的径向尺寸。 
图19和20进一步示出了图12中所示的接地传感器648的细节。图19和20中的接地传感器648包括一个远端部680,一个中间部682和一个近端部684。远端部680为弹簧固定,因此可以在中间部682上可滑动的,可叠缩的移动,从而在一个最大移动量上获得与近端部684的无缝连接。当接地传感器648在它的正常状态时,弹簧张开近端部,因此传感器表现出图19所示的导向。当植入物645(图12)位于面对组织开口的周边的位置时,传感器648的近端684压紧在远端部680上,如图20所示。远端部680和近端部684都具有外壳,或者覆盖有一种不透射线材料。然而,中间部682没有构造成或者覆盖有这样的不透射线材料。因此,当远端部680不动时,远端部从近端部684完全地延伸,暴露的中间部682所呈现出的间隙在X射线照相的检测下是可见。然而,当远端部680与近端部684最大程度地靠近时,这样的不透射线间隙在X射线照相下是不可见的,并且接地传感器被“致动”。该实施例允许用X射线照相监视接地传感器648相对于远端导管部680的延伸程度的位置。在所示的根据本发明的实施例中,一个或者多个接地传感器648用于确定假体装置的输送系统在适当的位置上,从而把植入物放入到二尖瓣环中。当该组织结构不能通过透视检测或者标准的X射线照相过程直接识别时,那么这样精确的定位是困难的。同时,二尖瓣环的精确定位和啮合对于适当的植入功能和安全是关键的。 
根据本发明的实施例的接地传感器可以具有多种型式,包括如前面所述的伸缩式,弹簧式,以及通过可透射线元件连接的不透射线元件。在实施例中使用磁共振图象,根据本发明的接地传感器可以利用设置在非金属部之间的金属部,以植入类似的伸缩式,弹簧式的设置。其它的实施例包括一个具有伸缩式,弹簧式元件的可视标志系统,该标志系统具有用于操作中的颜色编码或者其它的可见特征,在该操作中,直接检查或者通过内窥镜的检查是可能的。根据本发明的接地传感器的其它的实施例包括在其顶端具有微型开关的接地传感器,因此足够压力的瞬间接触使得一个电路连通并且向操作者发送致动接地传感器的信号。根据本发明的其它的接地传感器具有用于 Rahmen激光光谱学或者其它光谱分析技术的光学纤维路径,该路径可以在植入物的所需位置检测组织的独一无二的组织特性。此外,根据本发明的其它实施例包括接地传感器,该传感器具有电极或者其它电子传感器,可以进行检测,并且当检测到用于正确植入的所需组织的所需电生理学,电阻抗,或者其它可测特性时向操作者发出信号。这种电生理学接地传感器可以包括电路,该电路向操作者产生视觉的,声音的,或者其它的信号,传感器被致动并且植入物在适当的部位被连接。 
然而在根据本发明的另一个实施例中,其它的心脏内的或者心脏外的成像技术包括,但不仅限于,血管内的超声成像,核磁共振,虚拟解剖定位系统,或者可以使用其它的成像技术,从而不需要前述的接地传感器就可以确定植入物的适当位置。 
图21-24示出了根据本发明的一个实施例的植入物700。在该实施例中,植入体705为带状和柔性的。在植入体705的大部分长度上具有多个导向的有利于放置、保持和从装置上拆除的保持倒钩710。植入体705还具有一个可调节部715,在该实施例中该可调节部具有多个可调节止挡720。可调节止挡720可以是槽、孔、爪、凹部、凸起、齿、突起的部分或者其它允许在使用中适当调节植入物700的机械特征。在图21-24所示的实施例中,调节止挡720与一个啮合连接器725相啮合。图21为一个端视图,示出了卷曲的植入体705,保持倒钩710向外,并且可调节部715经过它与啮合连接器725的啮合并且在植入体705中向内卷曲形成一个封闭的,环形结构。图23示出了一个示例的啮合连接器725的具体结构,其中套730连接在植入体705上。腔室730包括和支撑一个机械螺杆740,该螺杆具有与第二啮合头755相啮合的连接的第一啮合头750。第二啮合头755连接在一个调节杆760上,该调节杆760加工成接受一个螺丝刀状的调节元件。根据本发明的不同的实施例需要多种不同的调节元件。在本发明的实施例中,调节元件作为一个完整的成卷线提供,该线具有一个由调节杆760(没有示出)的接受狭缝接受的远端。调节元件的远端和调节杆760之间的关系类似于螺丝刀刀刃与螺丝头之间的关系,因此通过操作者传递给调节装置的扭矩将导致调节杆760的转动,并且第二啮合头755允许第一啮合头750和蜗杆740的运动, 这使得可调植入物部715运动并且蜗杆与调节止挡725啮合。调节部715的多余长度穿过一个带缝735(图23),允许带在封闭的植入体705的中心移动。该实施例中的调节元件可以设置成在展开的伞形物缩回和取出之后仍然保持在一定位置上。调节元件的远端和调节杆760之间的连接可以为一种简单的摩擦连接,机械键/槽配合,或者为磁力的或者电子的连接。 
如图21进一步所示,具体实施例使用了连接在植入体705的外周边上的单向的保持倒钩710。保持倒钩710以相对于植入体705一致的,正切的位置导向,因此在展开过程中植入体的旋转运动将啮合或者释放保持倒钩710与所需组织的接触。保持倒钩710的位置允许操作者通过在植入物的轴线上旋转植入物700植入,从而把保持倒钩710啮合到邻近的组织中。如图24所示,每个保持倒钩710可以进一步在其端部具有一个末端倒钩775,该末端倒钩775允许当通过旋转植入物700使保持倒钩710啮合时具有穿过组织的光滑路径,由于末端倒钩775抓住周围组织(特别类似于倒刺鱼钩)的作用,不允许植入物700在相反的方向上旋转。因此末端倒钩775确保把植入物700固定到周围的组织中。 
图25-27示出了根据本发明设想的植入物800的另一个实施例。植入物800包括一个带805(图27),但是由于前述例子中的在外部植入物外壳810上的保持倒钩已经被去除。组织外壳810可以缝合或者固定到一个所需部位的组织上。植入物800的周边通过一个啮合的连接器825调节,相似于图23中所示的带状植入物阵列的啮合连接器。尤其是,带上的调节止挡820通过一个机械蜗杆840与一个连接的第一啮合头850啮合。第一啮合头850与第二啮合头855相配合。第二啮合头855固定在调节支杆860上,该支柱加工成接受一个螺丝刀状调节件。 
图28示出了一种使用一个植入/输送系统阵列600把植入物645定位在病人体内的方法的例子,病人患有缺血环形扩张和二尖瓣逆流。外周边动脉入口通过传统的缩减,动脉孔,或者其它标准的入口技术获得。在进入到动脉系统之后,放置导线并且经血管进入心脏900,通过使用荧光的,超声波,三维超声波,磁共振或者其它的实时成像技术。导线,展开装置和植入物以逆行的方式经过主动脉瓣膜到达左心室905,然后进入左心房910。在该点 上,操作者撤去腔室外壳605,因此使得折叠的展开伞形物642和植入物没有外壳。然后展开伞形物642通过致动导管的远端运动扩张,导致径向支撑杆和支撑支柱完全张开。在这时,接地传感器648不会与任何固体结构相接触,并且它们的可透射线的间隙完全展示在图象系统上。一旦展开伞形物张开,整个组件背向二尖瓣膜915的区域拖动。在根据本发明的一个优选的实施例中至少使用两个接地传感器648。所有的接地传感器示出了它们中间的,可透射线的中间部并且被致动,然后张开的伞形物一定与二尖瓣环形/心房组织的附近区域的固体组织接触,并且可以进行植入物的进一步的张开和调节。然而,如果任何一个接地传感器没有被致动,存在可透射线的间隙,则装置不会正确地定位,因此必须在进一步张开之前重新定位。因此,通过根据本发明的输送系统接地传感器系统参与假体装置的展开和调节。一旦正确定位,操作者以指定的顺时针或者反时针方向旋转使导管致动,从而使植入物上的保持倒钩啮合到二尖瓣环/心房组织区域的组织上。如果需要重新定位,一个反向的运动将使得保持倒钩与环/心房组织脱离啮合,并且进行重新定位,再次使用接地传感器用于准确定位。一旦牢固地固定,操作调节元件可以获得所需程度的减小的环。可以使用实时的经由食道的超声波心动描记术,血管内的超声波心动描记术,心脏内的超声波心动描记术,或者其它的用于估计二尖瓣功能的型式,用于评估修复的二尖瓣功能的生理效果,并且还可以进行其它的调节。一旦获得所需的结果,致动释放元件把展开伞形物从植入物上取下。然后操作者收回致动导管并伸出腔室外壳,折叠展开伞形物和覆盖元件,从而顺利地、无创地从心脏和血管系统撤回装置。 
如果需要,在导管元件取出之后调节元件可以留在原位用于进一步的生理调节。在本发明的另一个实施例中,一个导管型的调节元件可以经过皮肤的或者其它的路径再次插入。这样的一个调节元件可以通过操作者容易地操作,并且具有磁的,电子的,电磁的,或者激光导向系统,从而允许调节元件与植入物中的调节机构对接(dock)。在另一个实施例中,调节机构可以通过植入的电动机械的电机或者其它的系统驱动,该电机或者系统可以通过电通量遥控或者其它的经皮的或者透皮的方法遥控。 
在有关肺动脉瓣治疗的情况下,最初的导管进入是通过周边或者中心静 脉进入。肺动脉瓣的进入同样可以从下面的瓣膜获得,从中心静脉入口通过经过右心房,三尖瓣膜,右心室,到达肺动脉瓣。 
在本发明的另一个实施例中,导管进入左心房可以通过中心或者周边静脉的插管获得,从而进入右心房。然后可以使用标准房间隔横穿方法产生医原性房间隔缺损(ASD)从而进入左心房。在这种情形下,与例1中所述的逆向进入相反,二尖瓣可以从瓣膜上部进入。通过使用前述相同的修复技术,可以使用植入物定位部件将植入物和反向展开的伞形物放置在二尖瓣环的心房中。然后医原性的ASD可以通过标准的装置方法关闭。进入主动脉瓣膜同样可以以相似的逆行的方式通过上述的主动脉瓣膜经过动脉进入。 
根据本发明的可调植入物和方法的其它实施例包括肠胃的紊乱,例如胃食管回流症(GERD),其中胃食管(GE)连接处缺少足够的括约肌弹性阻止胃的内容物回流到胃食管中,导致典型的胃灼热或者酸性回流。这不仅仅导致不舒服,而且长时间会导致对食道下部的损伤,这会导致癌前病变的发展(Barrett’s食道)或者在GE连接处的食道的腺癌。GE连接处的外科修复已经通过通常具有较好的手术效果的尼森胃底折叠术获得。然而,尼森手术通常需要麻醉和住院。使用根据发明的装置和方法,一个可调植入物将不需要患者住院并且可以在诊所或者肠胃医生的办公室进行。现在参照附图29和30,一个具有植入物645的伞形物展开装置600在内窥镜1000的导向下通过病人的口,食道1005,进入到胃1010,在胃中伴随着植入物645和接地传感器648的扩张,展开装置600打开,该接地传感器具有颜色编码或者其它可视的间隙。然后接地传感器在内窥镜的直接控制下围绕胃食管连接处1015啮合到胃部,直到所有的接地传感器648可视地致动。然后植入物连接到胃壁1020上,伞形物642释放并撤回,仅留下植入物645和调节元件。然后调节可调植入物直到获得所需的效果,也就是说,通过病人的症状,食道的pH监测,图象研究,或者其它的诊断装置确定具有最少的酸性回流。如果病人感到气胀难受,这是胃食道连接处修复的常见并发症,是由于修复得过紧,病人不能打嗝而导致的,那么可以松开植入物直到获得一个更好的效果。 
在本发明预期的各个实施例中,植入体可以是直的,弯的,圆形的,卵 形的,多边形的,或者它们的组合。在本发明预期的各个实施例中,植入物可以在植入体中具有一致的或者不一致的孔或者腔室调节。植入体还包括完全封闭原始接受组织部位,或者它可以以一种阻断的型式仅仅封闭原始接受组织部位的一部分。在本发明的其它的实施例中,植入体可以为一个实心的结构,而在其它的实施例中植入体可以形成一个管状的或者中空的结构。在本发明的一个实施例中,植入体还可以是具有一个外部件,一个内部件和任选的连接件的结构。在这种实施例中,植入体的外部件可以作为植入体的一个外壳,并设计为能够利于和促进组织与原始接受组织部位的向内生长和生物一体化。在这样的一个实施例中的外部件可以为一种生物相容性材料,例如聚酯纤维,聚四氟乙烯,可延展性金属,其它的生物相容性材料或者采用模制,编织,或非编织结构的生物相容性材料的组合。在这样的实施例中的外部件还用于容纳内部件。在该实施例中,内部件具有一个调节机构,当通过一个可调机构操作时,该调节机构可以通过一种限定的方式改变外部件的形状和/或尺寸。 
在根据本发明的可选择的实施例中,调节装置可以位于外部件的外部或者置入到外部件中。在本发明设想的另一个可选择的实施例中,植入体由一个调节装置组成,该调节装置不具有一个覆盖所述调节装置的独立的外部件。 
在根据本发明的各个实施例中,调节装置还包括一个机构,该机构是带螺纹的或者不带螺纹的,并且该机构可以通过螺丝或者蜗杆螺丝,一个摩擦机构,一个摩擦爪机构,一个带齿机构,一个棘轮机构,一个齿条齿轮机构,或者其它这样的装置的作用啮合,从而进行仔细的调节并且一旦获得所需的尺寸,保持所需尺寸和所需部位。 
在根据本发明的另一个实施例中,调节装置可以包括一个勒除器或者囊绳状机构,其中缝合线,带,线或者其它的纤维结构,编织的或者非编织的,单丝的或者多丝的,依靠外科医生或者其它操作者改变传递给所述线或纤维的张力或运动,能够影响可植入的装置在一个原始组织接受部位上的组织和/或生理效果。在各个实施例中可以提供这样的一个环形的或者非环形结构的调节装置,张力或者运动的变化可以改变植入物的尺寸和/或形状。 
在根据本发明的各个实施例中,调节装置可以为金属的,塑料的,合成的,天然的,生物的,或者任何其它生物相容性的材料,或者它们的组合。这样的调节装置还可以通过挤压或者其它的模制技术,机加工,或者编织进行制造。此外,在本发明各个实施例中,调节装置可以为光滑的或者可以包括狭缝,卷边,凸起,或者任何其它光滑的或者粗糙的表面。 
在本发明的各个实施例中,植入体可以具有一个或者多个连接件,例如套环或者开口,或者其它的有利于把植入物连接到原始接受组织部位的连接件。在可选择的实施例中,植入体连接或者置入到一个机械组织表面系统上,该系统可以以无缝合线的方式把植入体固定到原始接受部位上。在本发明的另一个可选择的实施例中,缝合线或者其它的连接装置可以围绕或者穿过植入体把植入体固定到原始接受部位上。在本发明的另一个实施例中,把植入体固定到原始接受部位上的机械装置可以增加,或者使用纤维素或者其它的生物相容性组织胶水或者相似的粘附剂代替。 
在根据本发明的各个实施例中,可调节植入物用于可调节地扩大或者维持外周边或者孔,口,腔室,或者吻合处的其它尺寸,其中疾病发展过程趋于减小或者收缩这样的外周边或者其它尺寸。 
在根据本发明的各个实施例中,提供一种调节机构用于与调节装置相互作用,获得所需的在调节装置的尺寸和/或位置的改变。这样的调节机构包括一个或者多个螺丝,蜗杆螺丝排列滚子,齿轮,摩擦止挡,摩擦爪系统,棘爪,齿条和齿轮阵列,微型电机械系统,其它的机械装置或者电机械装置或者它们的一些组合。 
在本发明设想的一些实施例中,一个调节工具可以可拆卸地或者永久地连接到调节机构上并且设置成传递运动给调节机构,并且顺序地传递给调节装置,从而增大或者减小植入在原始接受部位上的植入物的解剖学效应。 
在根据本发明的可选择的实施例,可以设置带有一个或者多个具有相关的电子控制电路的微型电机械电动系统的微型电机阵列作为调节装置,并且可以通过由电磁射线或者穿过电子导线管的直接电路传送的信号遥控致动,该导线管永久地或者可拆除的连接在所述微型电机阵列上。 
在根据本发明的其它实施例,调节机构可以具有一个锁定机构,该机构 保持调节装置在选定的位置上,在该位置上使原始接受部位以及组织器官获得最佳的所需解剖和/或生理效应。在另一个实施例中,由于使用的调节装置的性能,不需要特殊的锁定机构。 
在本发明的其它实施例中,调节装置和/或外部件机构可以为易弯曲的合成材料,该材料可以在选定波长的电磁射线的照射下硬化,例如紫外线。在这样的实施例中,所需的电磁射线的照射可以通过外科医生从外部向植入物输送这种射线的获得,或者通过在外植入部件内从内部输送这种射线,这通过使用放置在所述外部件中并且连接到一个适当的外部射线源上的光学纤维承载器完成。这样的光线纤维承载器可以设置成在适当的射线照射和硬化所述调节装置之后,从外植入物部件中整体拆除或者部分拆除。 
本发明还提供了一种方法,该方法使用一种可调节植入物可选择地改变组织的解剖结构和/或者生理效应,该植入物形成哺乳动物体内的用于血液,其它生理流体,营养流体,半固体的,或者固体的,或者废物的通道。使用的这样的可调节植入物的各个实施例包括,但是不仅限于,通过一个开口外科切口将所述可调植入物通过手术放置在原始接受部位,在通过荧光镜,超声波,磁共振图象,或者其它图象技术的可视控制下,将所述植入物经皮的或者由皮的放置,把所示植入物通过组织结构壁放置,例如冠状窦或者食道壁,或者使用上述技术的组合的方法。在本发明设想的各个实施例中,在有或无心跳的心脏外科手术中或者内窥镜的或者经皮的肠胃外科手术中,通过经心房的,经心室的,经动脉的,经静脉的(即,经肺静脉)或者其它的路径,将可调节植入物放置和固定在原始接收组织部位的位置上。 
此外,使用可调植入物装置的可选择的方法可以提供对接受所述可植入的装置的组织结构的尺寸的进行周期性的,植入后的调节,以便满足幼年患者自身接受部位的生长或者满足接受植入体的病人的生理需要的其它变化。 
在此公开的可调植入物的调节以及它们使用的方法设想通过外科医生或操作者使用诊断工具进行,从而提供了一种对获得所需效果的所需调节的特性的评估。这样的诊断工具包括,但是不仅限于,经食道超声波心动描记术,超声波心动描记术,诊断超声波,血管内超声波,虚拟解剖定位系统,磁共振,计算机化的断层成象或者其它成像技术,内窥镜检查,纵隔镜检查, 腹腔镜检查,胸腔镜检查,X光照相术,荧光检查,磁共振成像,计算机化的断层成像,血管内流量传感器,热传感器或者成像,远程化学或者光谱分析,或者其它成像或定量或定性分析系统。 
一方面,本发明的植入/输送系统包括一个折叠的,可压缩的或者可扩张的假体植入物和一个用于该假体植入物的输送介面,该系统可以把假体植入物以一种折叠的,压缩的或者非展开的状态输送到一个所需的组织接受部位,然后允许可控的膨胀或者扩张并且通过使用者把这样的一个假体植入物物理连接到所需的组织接受部位。这种系统允许输送系统和假体植入物通过套管针、护套经皮引入体内,这通过使用Soldinger技术实现,或者通过使用内窥镜穿过人体的自然开口、体腔,或者区域而引入体内,并且由外科医生或者操作者调整到所需的组织接受部位,输送系统和假体植入物可操作地张开用于展开。如果需要,根据本发明的植入/输送系统还可以允许在它们连接到所需的组织接受部位上之后,用户进一步调节假体植入物的尺寸和形状。根据本发明的输送系统可以从它与假体植入物的连接界面上拆除并且由操作者从组织部位上拆除。输送系统和假体植入物的形状和尺寸可以由哺乳类治疗对象体内的原始接受组织部位的解剖需要决定。这样的一个原始接受组织部位可以为心脏瓣膜,靠近胃食管连接处的食道,肛门,或者哺乳动物体内的其它产生功能障碍的组织部位,该功能障碍可以通过植入物解除,该植入物可以改变该部位的尺寸和形状并且在外科手术后保持一个所需的尺寸和形状。 
在本发明设想的各个实施例中,输送系统可以是导管,线,丝,杆,管,内窥镜,或者其它的机构,这些机构通过一个切口,孔,套针,或者通过一个解剖通道,例如脉管,孔,或器官腔室,或者经腹部或经胸部地到达所需的接受组织部位。在根据本发明的各个实施例中,输送系统通过操作者进行操作。输送系统还具有一个输送界面,该输送界面可以保持和输送一个假体植入物到达所需的接受组织部位。这样的一个输送介面可以可操作地扩张,改变形状,或者允许这样的假体植入物在所需接受组织部位上独立地扩张或者膨胀。此外,这样的输送介面可以具有一个操作装置,用于在所述植入物固定到所需的接受组织部位上之后原地调节扩张或者膨胀的尺寸,形状,或 者假体植入物的生理效应。在根据本发明的各个实施例中,这样的调节可以在植入物被放置的过程中进行,或者在随后的时间进行。取决于特殊应用的特殊解剖需要,输送介面和相假体植入物可以是直的,弯曲的,环形的,螺旋形的,管状的,卵形的,多边形的,或者它们中的一些的组合。在本发明的其它的实施例中,假体植入物可以为一个实心的结构,然而在其它的实施例中假体植入物可以为管状的,组合的,或者中空的结构。在本发明的一个实施例中,假体植入物还可以为这样的结构,该结构包括一个外部件,内部件和任选的连接部件。在这样的实施例中,假体植入物的外部件作为植入物的外壳,并且设计成便于和促进组织的向内生长以及与原始接受组织部位成为一个生理整体。在这样的实施例中的外部件可以由一种生物相容性材料制成,例如聚酯纤维,聚四氟乙烯,可延展的金属,其它的生物相容性材料或者这样生物相容性材料的组合,其采用模制的,编织的,或者非编织的结构。在这样的实施例中的外部件还可以容纳内部件。在该实施例中,内部件具有一个调节装置,当该调节装置通过一个调节机构操作时,该调节装置可以以一种限定的方式改变外部件的形状和/或尺寸。 
在根据本发明的一些实施例中,可调节的内部件和外部件的至少一部分是弹性的,从而向瓣膜,孔或者腔室提供一个可变的,人造肌紧张性元件,这样的可变性对组织的功能是有用的,例如在对直肠失禁和阴道脱落的治疗中。 
在根据本发明的各个实施例中,输送界面具有一个连接装置,用于沿途保持和输送假体植入物到原始接受组织部位并且当它被放置后由操作者对假体植入物进行任何原地调节。这样的连接装置可以进行反向操作,允许假体植入物在获得所需的位置和调节之后,假体植入物从输送介面上拆除。 
最后,应该理解的是通过示例的方式公开了优选的实施例,对本领域的普通技术人员而言,可以在不脱离附属的权利要求的范围和精神下进行其它的修改。 

Claims (22)

1.一种用于控制解剖孔或者腔室的内周边的可植入的装置,包括:
环形圈;
可以把所述环形圈紧固到解剖孔或者腔室周围的组织上的装置;
调节装置,该调节装置用于允许把所述环形圈的周边从第一周边调节到第二周边,其中,所述第二周边比所述第一周边既可以变短又可以变长;以及
与所述环形圈相联系的装置,用于保持所述环形圈在所述的第一周边上并且,当所述环形圈调节到所述第二周边上时,用于保持所述环形圈在所述第二周边上;
因此通过把所述环形圈的周边从所述第一周边调节到所述第二周边,可以调节所述解剖孔或者腔室的内周边。
2.根据权利要求1所述的可植入的装置,其中所述的把环形圈紧固到解剖孔或者腔室周围的组织上的装置包括多个从所述环形圈上延伸的倒钩,用于与所述解剖孔或者腔室周围的组织啮合。
3.根据权利要求2所述的可植入的装置,其中所述每个倒钩在一个相对于环形圈不变的,正切的方向上导向,因此环形圈在第一方向上的旋转运动使得所述倒钩与所需组织啮合,环形圈在与所述第一方向相反的方向上的旋转运动使得所述倒钩与所述所需组织脱离啮合。
4.根据权利要求3所述的可植入的装置,其中所述每个倒钩还包括一个在其自由端部的钩。
5.根据权利要求1所述的可植入的装置,其中所述可以把所述环形圈紧固到解剖孔或者腔室周围的组织上的装置,包括一个覆盖所述环形圈的,可以放置缝合线的外部结构壳,从而把所述可植入的装置缝合到所述解剖孔或者腔室周围的所述组织上。
6.根据权利要求1所述的可植入的装置,其中所述的把所述环形圈紧固到解剖孔或者腔室周围的组织上的装置包括多个围绕所述环形圈周边的间隔开的套环。 
7.根据权利要求6所述的可植入的装置,其中所述套环具有狭窄的中间颈部,缝合线可以围绕该颈部放置并穿过其下的组织,把所述环形圈固定到所述解剖孔或者腔室周围的组织上。
8.根据权利要求6所述的可植入的装置,其中所述套环包括一种材料,该材料足够地柔软,因此缝合线可以穿过所述套环以及其下的组织,把所述环形圈固定到所述解剖孔或者腔室周围的组织上。
9.根据权利要求6所述的可植入的装置,其中所述套环具有舌片,该舌片包括一种材料,该材料足够地柔软,因此缝合线可以穿过所述舌片以及其下的组织,把所述环形圈固定到所述解剖孔或者腔室周围的组织上。
10.根据权利要求6所述的可植入的装置,其中所述套环具有舌片,并且其中所述舌片限定缝合线可以穿过的孔,缝合线穿过所述孔然后穿过其下的组织,把所述环形圈固定到所述解剖孔或者腔室周围的组织上。
11.根据权利要求6所述的可植入的装置,其中所述可植入的装置包括围绕所述环形圈的周边间隔开的交替的褶皱部和套环部。
12.根据权利要求1所述的可植入的装置,
其中所述环形圈包括一个成型为环状的中空管,所述管具有围绕它的周边设置的间隔开的光滑部和褶皱部,
其中所述可以把所述环形圈紧固到解剖孔或者腔室周围的组织上的装置,包括所述管,该管足够柔软,因此缝合线可以经过所述管然后穿过其下的组织,以及
其中与所述环形圈相联系的所述装置,该装置用于保持所述环形圈在所述第一周边上,并且当把所述环形圈调节到所述第二周边上时,用于保持所述环形圈在所述第二周边上,包括褶皱部,该褶皱部足够地坚硬,因此在所述环形圈以一个给定的形状被放置之后,当承受所述选定的解剖孔或者腔室的正常组织力的作用时,所述褶皱部趋于保持所述给定的形状。
13.根据权利要求1所述的可植入的装置,
其中所述环形圈包括一个成型为环状的中空管,所述管具有围绕它的周边设置的间隔开的光滑部和褶皱部,
其中所述可以把所述环形圈紧固到解剖孔或者腔室周围的组织上的装 置,包括所述具有狭窄周边部的管,因此缝合线可以在所述狭窄周边部上围绕所述管并穿过其下的组织;以及
其中与所述环形圈相联系的所述装置,该装置用于保持所述环形圈在所述第一周边上,并且当把所述环形圈调节到所述第二周边上时,用于保持所述环形圈在所述第二周边上,包括所述褶皱部,该褶皱部足够地坚硬,因此在所述环形圈以一个给定的形状被放置之后,当承受所述选定的解剖孔或者腔室的正常组织力的作用时,所述褶皱部趋于保持所述给定的形状。
14.根据权利要求1所述的可植入的装置,其中,所述调节装置允许把所述环形圈的周边从第一周边调节到第二周边,包括:
形成在所述环形圈的至少一部分上的齿;
与所述环形圈的所述齿相啮合的齿轮;以及
所述齿轮和所述齿这样进行设置,即所述齿轮的转动导致所述环形圈的第一端和所述环形圈的第二端之间的相对运动,从而把所述环形圈的周边从第一周边调节到第二周边。
15.根据权利要求14所述的可植入的装置,还包括一个正切于所述齿轮的轮,因此所述环形圈的与齿轮相啮合的部分,经过所述齿轮和所述轮之间。
16.根据权利要求14所述的可植入的装置,其中所述齿轮相对于所述环形圈的所述第二端固定安装,并且所述齿靠近所述环形圈的所述第一端形成。
17.根据权利要求14所述的可植入的装置,还包括从远离所述齿轮的位置转动所述齿轮的装置,因此在关闭手术切口以及生理流体重新流过所述孔或者腔室之后,可以调节所述植入物的周边。
18.根据权利要求17所述的可植入的装置,其中从远处位置转动所述齿轮的装置在完成调节后与所述齿轮脱离啮合。
19.根据权利要求1所述的可植入的装置,其中所述调节装置允许把所述环形圈的周边从第一周边调节到第二周边,包括:
蜗轮;以及
与所述环形圈的第一端相联系的用于啮合所述蜗轮的装置,因此所述蜗 轮的旋转导致所述环形圈的第一端和所述环形圈的第二端之间的相对运动,从而把所述环形圈的周边从第一周边调节到第二周边。
20.根据权利要求19所述的可植入的装置,其中所述蜗轮在它的驱动端具有一个第一斜齿轮头,还包括一个与所述蜗轮偏置的轴以及在该轴第一端上的第二斜齿轮头,所述第一和第二斜齿轮头驱动啮合,因此所述轴的旋转驱动所述蜗轮。
21.根据权利要求20所述的可植入的装置,还包括用于从远离所述蜗轮的位置旋转所述轴的装置,因此在关闭手术切口以及生理流体重新流过所述孔或者腔室之后,可以调节所述植入物的周边。
22.根据权利要求21所述的可植入的装置,其中在完成调节之后,所述轴和所述用于从远处位置旋转所述轴的装置从所述蜗轮上脱离啮合。
