Abstract

III.发明概述

本发明还包括用于制备止血敷料的方法,包括提供第一层血纤蛋白原,将凝血酶层涂敷在所述第一层血纤蛋白原上,和将第二层血纤蛋白原涂敷在所述凝血酶层上,其中,所述凝血酶层不是与所述第一血纤蛋白原层同延的和/或不是与所述第二血纤蛋白原层同延的。

用于所述止血敷料中的所述凝血酶可以是人血浆蛋白的冷冻干燥的混合物,它业已进行过纯化和病毒灭活。本发明的敷料通常包括效力为大约1-160国际单位(IU)/cm2的凝血酶,例如,至少5,10,15,20,25,30,35,40,45,50,60,70,80,100,125,150IU/cm2。选择性的成分包括白蛋白(通常为大约5mg/mL)和甘氨酸(通常为大约0.3M±0.05M)。所述凝血酶的pH通常在6.5-7.1的范围内。

所述敷料可以制成多种尺寸和形状中的任意一种。通常,所述敷料具有可以由本领域技术人员方便地操纵的尺寸和形状,一般沿任何一条边的长度都小于12″,例如,1″×1″,1″×2″,4″×4″等。所述敷料的含水量通常低于8%(例如,低于7,6,5,4,3,2或1%)。

一般,可以通过本领域技术人员所公知并且可以获得的任何方式将所述止血敷料的各层彼此粘接在一起。通常,所述血纤蛋白原层和/或所述凝血酶层(s)是作为一系列快速冷冻水溶液层添加的,并且随后进行低温干燥或冷冻干燥,例如,在添加每一层之后,并且在组装整个敷料之后。所述各层可以通过多种技术中的任意一种添加,包括喷雾,吸液(例如,使用多通道吸液管),喷洒,使用掩模,静电沉积,使用微量注射阵列系统,或使用分配歧管分配,它包括用于产生高密度阵列的口。

合适的血纤蛋白原和凝血酶可以通过本领域技术人员所公知的并且可以获得的任何纯化方法从人或哺乳动物血浆中获得;从重组组织培养物,病毒,酵母,或细菌等的上清液或培养液中获得。上述物质包括能表达人或哺乳动物血浆蛋白的基因,该基因是通过标准的重组DNA技术导入的;或从转基因动物的流体(例如血液,乳液,淋巴液,或尿液等)中获得,所述动物包括能表达人血纤蛋白原和/或人凝血酶的基因,该基因是按照标准转基因技术导入的。

为了制备所述敷料,按常规方式制备血纤蛋白原:35mg/ml的总蛋白(TP)存在于缓冲液D中(100mM NaCl,1.1mM CaCl2·H2O,10mMTris HCl,10mM柠檬酸钠,2%蔗糖,2.8mg/ml白蛋白,0.52mg/mlTWEEN-80,pH7.2),它含有浓度为80mg/g TP的白蛋白和浓度为15mg/g TP的聚山梨酸酯。用四种缓冲液中的每一种制备具有4745IU/ml效力的凝血酶浓缩物,以便获得含有2000IU/ml的凝血酶溶液。在表1中示出了这四种缓冲液。所述敷料是手工生产的,并且生产了四种凝血酶配方和覆盖类型的每一种的18个敷料,以便产生总共144种敷料。对所述敷料进行冷冻干燥,用脱水剂包装,然后通过体外测定分析外观,含水量,γ二聚体化,和可凝固蛋白百分比,并且进行活体猪动脉切开术测定。
外观
在外观测定中测试所有敷料的分层。如果所述敷料的所述血纤蛋白原层沿所有四条边彼此连接在一起的话,就认为这种敷料合格。所述敷料层的分层使得这种敷料难以操纵,并且如果在所述层上还存在表面裂纹的话,使得这种敷料容易断裂。如表1所示,与其中的凝血酶层是以完全覆盖的形式添加的敷料相比(1,3,5和7组),所述凝血酶层以圆形覆盖的方式施加的敷料(2,4,6和8组)在分层实验(即外观测试)中具有更高的合格率。尽管用凝血酶完全覆盖制成的敷料倾向于不能通过外观测试,用以圆形覆盖形式施加的凝血酶制成的敷料几乎100%通过了所述外观实验。

无论制备所述凝血酶的缓冲液如何,都观察到了改善了的对分层的抗性。因此,从某些缓冲液配方中除去氯化钙,不会影响凝块的形成,尽管氯化钙是凝血酶激活所必须的。以上结果表明,在所述血纤蛋白原制剂中存在足够的氯化钙(1.1mM),以便促进凝血酶活化。类似地,固体含量的增加(赖氨酸)不会产生显著影响。
测定了来自每一组的敷料的含水量。所述敷料的含水量平均为0.98%,并且在该组中非常一致,在0.80-1.16%范围内。这种含水量是这种敷料所常有的。
将来自每一组的一个敷料用于在常规测定方法中测定γ-γ二聚体化。正如在以下文献中所披露的:Okude等,Biol.Pharm.Bull.16:448-452,γ-γ双体测定方法测定了凝血酶使血纤蛋白原的γ链交联,以便形成凝块的能力。一般,为了进行该测定,在低湿度腔室中将每一个敷料研磨成均匀的,细的粉末,并且分成两个相等的部分。将500μL0.9%的盐水添加到一个部分中,并且将500μL的Okude溶解溶液(ODS)(10M尿素,1%SDS,0.1%β-巯基乙醇,0.01%溴酚蓝)添加到另一个部分中。对这两部分进行短时间的涡旋搅拌,并且在40℃下培养5分钟。这样使得可以在所述盐溶液样品中形成凝块,而ODS中的变性制剂能抑制凝块形成。将两种样品悬浮在5倍体积的ODS中,在40℃下培养1小时,溶解在含有β-巯基乙醇的Laemmli样品中,并且在SDS-PAGE凝胶上电泳。用考马斯蓝对所述凝胶进行染色,以便观察蛋白。如表1所示,在凝结之后,所有敷料都表现出高百分比的二聚体化,而在非凝结敷料中表现出低百分比的二聚体化,这表明了在生产所述敷料期间,所述凝血酶和血纤蛋白原之间没有发生相互作用。
可凝固蛋白
还对来自每一组的两种敷料进行了测试,以便测定敷料中可凝固蛋白的百分比。在本测定方法中,将敷料润湿,并且通过离心使凝固的蛋白沉淀。从所述凝血酶制剂中的某一些中消除CaCl2,不会明显影响凝块的形成。正如上面说明二聚体化时所披露的,以上结果表明,在所述血纤蛋白原制剂中存在足够的氯化钙(1.1mM),以便促进凝血酶活化。
离体猪动脉切开术敷料性能测试
在来自离体的猪动脉切开术测定中测试了来自某些组的十二种所述止血敷料,以便确定所述敷料是否能够在模拟的伤口中保持适当的压力。如表1所示,以圆形覆盖形式生产的敷料,其所述凝血酶层不是与所述血纤蛋白原层同延的,在本测定方法中,它能够保持适当的压力。
按以下方法进行标准的来自离体的猪动脉切开术测定。获得冷冻的猪主动脉,并且解冻。主动脉可以在4℃下解冻过夜,或者在37℃的水浴中分别包裹。从所述主动脉的前面大约11厘米处切除多余的结缔组织。通常,前5-5.5cm没有侧枝血管。随后的5-5.5cm应当具有不超过1-2个侧枝血管。这些侧枝血管可以方便地密封或用氰基丙烯酸酯胶修补。
将所述主动脉切成两个5.5cm的片段。使用止血钳或平端镊子翻转主动脉,暴露出它的内部。在37℃用1-5mL的PBS洗涤所述血管的内部和外部。在20cc注射器上拉伸O形环,在它的一侧钻出大约0.6cm(0.25英寸)的孔。使用手指或止血钳将所述血管拉到所述注射器上。将具有相同尺寸的另一个O形环结合在它的底部。
使用弯曲的止血钳,小心地将两个O形环固定在所述血管顶部。在两个O形环之间的距离应当为3.5cm。所述动脉应当贴切地配合,并且可靠地固定。对所述固定的血管进行定位,以便所述注射器上的孔位于O形环之间的距离的中点。
用37℃的PBS填充所述注射器,并且通过所述注射器的外侧放入螺杆,并且进入所述柱塞,以便所述柱塞保持在固定状态。用1-2ml的37℃的PBS洗涤所述注射器上的动脉。使用16号针头在所述注射器孔的中央形成孔(大约距离每一个O形环1.75cm)。每使用12次之后就应当更换所述16号针头。
打开装有所述敷料的密封袋,并且马上将所述敷料放置在所述切口(距离每一个O形环大约0.5cm,以便不会接触每一个O形环)上。在使用之前,所有敷料都应当独立包装。
使用P-1000Pipetman,用37℃的PBS湿润所述敷料。对于15mg/cm2的敷料来说使用800μL,而对于8mg/cm2的敷料来说使用500μL。马上将所述注射器护罩放置在所述敷料顶部,以便不会接触任意一个O形环。轻轻按压以便固定。
将所述注射器放入37℃的培养箱中,使用所述真空室,以便保持所述注射器以及它的所有部件静止。用塑料覆盖物覆盖,在顶部牢固地放置200g重量。确保所述重量的均匀分布。让它在37℃下培养5分钟。
从所述培养箱中取出注射器。小心去掉覆盖所述敷料的护罩。将所述注射器连接在与蠕动泵连接的管上。所述管应当是这样安装的,以便它通过所述泵,并且与相反一侧的Y形接头连接。所述Y形接头产生了两个出口,可以在另一侧产生反压时从一侧将所述PBS泵送到所述注射器中。用联机压力换能器-分析仪直接测定反压,并且使用DMSI-200/1软件记录。
将37℃的PBS泵入所述注射器,并且马上开始监测产生的压力。开始30秒钟的缓慢提升(对于E229中的泵来说,将1x速度设定为4,而对于E132中的泵来说,将1x速度设定为7.5),以便所述初始流速为大约0.3mL/分钟。
在前30秒钟之后,将所述流速逐渐加大到大约3mL/分钟(两种设置均为10x速度)。进行这种操作直到获得200mm Hg的压力。一旦达到了200mm Hg的压力,启动定时器2分钟时间。
一旦获得200mm Hg,停止所述泵。监测所产生的压力。如果压力开始下降,再次启动泵,直到获得适当的压力。在所述2分钟的间隔中这种操作可以根据需要经常进行(在正常条件下,压力应当保持在200-215mm Hg之间)。另外,注意任何泄露以及它的位置。如果发生泄露,注意最大时间以及泄露时的爆发压力。根据以下合格/失败指标判断敷料性能。
如果敷料能保持相当一致的200mm Hg的压力两分钟时间并且绝对没有泄露的话,就认为它是合格的。如果敷料保持相当一致的200mm Hg的压力两分钟时间,并且仅有少量泄露的话同样认为它是合格的(例如,它自身能重新密封的缓慢渗出或泄露)。
如果敷料由于严重泄露而不能保持适当压力的话,就认为它是不合格的。这包括由于较差的粘接所导致的泄露,以及由于生产缺陷所导致的泄露。尽管如此,在本测定中被认为不合格的敷料也可用于处理不太严重的伤口。
例2
在下面的表2中提供的数据证实了分层可能是由于所述凝血酶层(或缓冲液)完全覆盖在所述第一血纤蛋白原层上造成的。生产出涂敷了凝血酶的敷料,以便所述凝血酶层不是与所述第一血纤蛋白原层同延的,大体上通过了所述分层外观测试(表2中的9-11组和13-16组)。类似地,生产的敷料没有中间层,即仅具有两个血纤蛋白原层,大体上不会变得分层,并且所述血纤蛋白原层紧紧地彼此粘接在一起。
在本实施例中,按上述方法制备血纤蛋白原,并且按表2所述制备凝血酶。所述血纤蛋白原层(大约1.2mL)是使用可编程的吸液管添加的,并且所述敷料是按本文所披露的方法手工生产的。通过使用喷枪喷雾或通过吸液添加所述敷料的中间层(即凝血酶或血纤蛋白原)。对于每一种配方和涂敷方法来说,添加了四种不同体积的中间层,120μl的体积被认为是“标准”体积。在通过喷雾添加所述中间层时,延长喷雾时间,以便增加喷涂的体积。其中所述凝血酶是喷雾的所有的组获得完全覆盖所述第一血纤蛋白原层。在通过吸液添加所述中间层时,所述第一血纤蛋白原层的完全覆盖只有在添加1000μl的中间层时才能获得。将较小体积的中间层用于验证分层是通过添加所述凝血酶层,以便它不是与所述血纤蛋白原层同延而抑制的。测试了每一组中的六个敷料。将第一组用作对照,并且评估含水量,γ-γ二聚体化,可凝固蛋白的百分比,以及外观(即分层)。对照敷料表现出的含水量为1.3%,在凝固时具有良好的二聚体化(82%),所述非凝固样品的低二聚体化(1%),以及低的,但是可接受的可凝固蛋白的百分比(68%)。评估所有敷料的分层,并且在表2中示出了合格敷料的数量。
在表2中示出的数据表明,在以完全覆盖所述血纤蛋白原层的形式添加所述中间层时(1-8和12组),所有敷料都变得在它们的边缘分层。在获得部分覆盖时,所述敷料大体上具有合格的外观。增加通过吸液添加的所述凝血酶中间层的体积可以增强所述敷料的分层这一发现表明,分层在更大程度上是所述中间层的覆盖率和组成的作用,而不是在敷料生产期间中间层冷冻的速度的作用引起的。因此,所述凝血酶层不是与所述血纤蛋白原层同延的敷料能够抑制分层。

Claims (24)
1.一种止血敷料,它包括:
(i)第一血纤蛋白原层;
(ii)靠近所述第一血纤蛋白原层的凝血酶层;和
(iii)靠近所述凝血酶层的第二血纤蛋白原层,其中,所述凝血酶层不是与所述第一血纤蛋白原层和/或所述第二血纤蛋白原层同延的。
2.一种止血敷料,它包括:
(i)可再吸收的材料层;
(ii)靠近所述可再吸收的材料层的第一血纤蛋白原层;
(iii)靠近所述第一血纤蛋白原层的凝血酶层;和
(iv)靠近所述凝血酶层的第二血纤蛋白原层,其中,所述凝血酶层不是与所述第一血纤蛋白原层和/或所述第二血纤蛋白原层同延的。
3.一种止血敷料,它包括:
(i)第一血纤蛋白原层;
(ii)靠近所述第一血纤蛋白原层的可再吸收的材料层;
(iii)靠近所述可再吸收的材料层的凝血酶层;和
(iv)靠近所述凝血酶层的第二血纤蛋白原层,其中,所述凝血酶层不是与所述第一血纤蛋白原层和/或所述第二血纤蛋白原层同延的。
4.如权利要求1-3中任意一项的止血敷料,其中,所述凝血酶层是与5%-95%的所述第一血纤蛋白原层同延的。
5.如权利要求4的止血敷料,其中,所述凝血酶层是与20%-50%的所述第一血纤蛋白原层同延的。
6.如权利要求1-3中任意一项的止血敷料,其中,所述凝血酶层是与5%-95%的所述第二血纤蛋白原层同延的。
7.如权利要求6的止血敷料,其中,所述凝血酶层是与20%-50%的所述第一血纤蛋白原层同延的。
8.如权利要求1-3中任意一项的止血敷料,其中,所述凝血酶层被设计成一系列包含凝血酶的斑点。
9.如权利要求1-3中任意一项的止血敷料,其中,所述凝血酶层被设计成包含凝血酶的单个斑点。
10.如权利要求1-3中任意一项的止血敷料,其中,所述凝血酶层被设计成包含凝血酶的多条线。

19.一种制备止血敷料的方法,该方法包括:
提供第一层血纤蛋白原;
将凝血酶层涂敷在所述第一层血纤蛋白原上;和
将第二层血纤蛋白原涂敷在所述凝血酶层上,其中,所述凝血酶层不是与所述第一血纤蛋白原层和/或所述第二血纤蛋白原层同延的。
20.如权利要求19的方法,其中,所述凝血酶层是与5%-95%的所述第一血纤蛋白原层同延的。
21.如权利要求19的方法,其中,所述凝血酶层是与5%-95%的所述第二血纤蛋白原层同延的。
22.一种制备止血敷料的方法,该方法包括:
提供在它上面结合了第一层血纤蛋白原的可再吸收的或不可再吸收的背层;
