技术领域

车螺纹16可特别适合于切割骨和移除骨而不损坏邻近组织。该螺纹可包括用于从孔中移除骨碎片和切屑的锋利切割凹槽36以及一个或多个直槽38。例如,两个相互以180°对齐的直槽38可以安置于顶端。本领域的普通技术人员将容易理解,切割凹槽36和直槽38的数量和结构可作广泛的改变,即除此以外或可选择地,也可以使用其它各种不同的结构和结合。

锁环20的外表面84还可使露出的丝锥长度28易于快速、可靠及准确调节。在图10和11的实施例中,外表面84的尺寸从末端到基端逐渐减小。锁环20可在末端88处更厚以容纳适于与限位环18上的连接元件66(比如外螺纹)搭合或配合的内部连接元件90(比如内螺纹)。锁环20还可在末端88处更厚以提供舒适的握紧部分94,从而可以可靠地握紧和操作锁环20。
锁环20的外表面84可进一步包括突起区域96以增强锁环20的握紧和触觉感。此外,突起区域96可提高设置和锁定丝锥组件的长度28的舒适度和可靠性。在图10和11中,这些突起区域96是围绕锁环的外表面84径向设置的纵向脊的形式。在外表面84上其它加强握紧的构造也可由本领域的普通技术人员预想到,例如,比如圆周的突起区域或脊、或者纵向和圆周脊的结合,或者其它表面结构。
锁环20的基端92可具有比末端88薄的壁部,当看轴22上的标记时以增强经过锁环20的可见度。薄壁部还可增强经过锁环20的这些区域的可见度。末端88处相对薄的壁部还可增强沿轴22和通过可在锁环20中形成的孔或窗口的视觉透明度。例如,锁环20的基端92(即指状夹顶端)可与为所期望的丝锥长度28而选择的长度指示器26对准。在另一示例中,所选择的丝锥长度指示器26和指示标记56可通过在不透明的锁环20中的孔或窗口而可见。
在图10和11中,锁环20的基端92可包括透明材料,其可以基本透明。此种材料提供的可见度可允许用户直接看透锁环20,并容易地观察长度指示器26、指示标记56和限位环18的基端92以在视觉上确定可调节长度的丝锥组件的有效长度28。透明材料可以具有色彩或不必具有色彩。可用于制造锁环的一种透明材料是塑料。特别地,锁环20可作为模制件而构造和确定尺寸。由塑料制成的锁环20可具有用于连接到限位环18的可由其它材料制成的连接元件90(例如螺纹)。在优选实施例中,模制锁环20可由医用聚碳酸酯制成。例如,锁环可由MAKROLONTM制成。通常,可用于形成锁环20的塑料材料应是在包装时能够承受γ灭菌的。锁环20还可由其它生物相容的材料制成,包括连同轴和限位环的上述同样的材料。
参照图12和13,在可调节长度丝锥组件98的另一实施例中,锁环99可以由例如316不锈钢的金属合金制成而特殊构造和确定尺寸。如图13中所示,锁环99可包括具有不同尺寸的孔100的圆柱体,该圆柱体从末端101延伸到基端102。如图12中所示,锁环99能够在限位环104的基端103上方自由滑动,从而当连接到限位环时可实现长度指示器的无障碍观察。因此,锁环99可以或不必具有允许用户读取可调节长度丝锥组件98的有效长度28的设置的窗口或狭缝。
图14示出了示例性的器具集合104,器具集合104可包括用于组成可在骨中钻孔和攻丝的可调节长度丝锥组件12的预打包的手术工具箱106中(图15中所示)。器具104优选地可包括限位环18、锁环20和多于一个的丝锥12。器具集合104优选地还可包括用于可拆卸地紧固在每个丝锥12上的把手(未示出),以使用户可以手工在骨中钻孔和攻丝。在图14中所示的示例性实施例中,三个丝锥12包括在器具集合104中。丝锥12优选地可轮换地与限位环18和锁环20一起使用。丝锥12优选地还可具有相同的轴22和凹槽结构24。丝锥12可进一步具有适于相似种类但直径不同的螺钉的车螺纹16。或者,丝锥可具有基本相同大小和形状的车螺纹16,或适于不同种类螺钉的车螺纹16。应当理解,工具箱106中可包括各种不同的器具。例如,第一工具箱可打包适于特定下颌骨疗法的器具集合,第二工具箱可打包适于特定骨盆疗法的器具集合,第三工具箱可打包适于特定矫形疗法的器具集合。图14和15说明了器具集合104和工具箱框架106的许多不同种可能的实施例中之一。
参照图15,在示例性实施例中,工具箱106包括比如由冲切纸板、塑料片或热成形的塑料材料制成的内支架108。内支架108可包括隔开的短小突出部或类似物(未示出),其可在使用前灭菌和保存时将各种器具104置于牢固的位置。当作为无菌组件包装时,工具箱106可包括由加热等方法外围密封以包围内支架108使其不与外界接触的内包裹物110。内包裹物的一端可包括用于紧急使用时(特别是在灭菌环境中,如在操作间里)快速打开内支架108的传统可剥离封条(peal-away seal)112。当作为无菌组件包装时,工具箱106还可包括由加热等方法外围密封以包围内包裹物110的外包裹物114。外包裹物的一端还可包括用于打开内包裹物110及其中物品的传统可剥离封条116。外包裹物114可在预想的紧急使用中从内包裹物移除,而不破坏工具箱106中物品的无菌性。
每个内包裹物和外包裹物110和114可包括外围密封的顶片118和底片(未示出)。在示例性实施例中,顶片118优选地可由像聚乙烯或MYLARTM的透明塑料膜层材料制成,以使工具箱106中的物品视觉可识别。底片可由能透过ET0灭菌气体的材料制成,如TYVEKTM塑料材料。工具箱106还可包括在内支架108中为执行期望过程的工具箱106中物品的使用说明120。说明120所描述的示例性过程将在后面解释。当作为无菌组件包装时,说明120还可包括声明,如“只能一位病人使用”(或类似语),以针对工具箱106中物品的重复使用(使用后物品的性能特点和功效会降级)给出明确的警告。由于这些原因,可调节长度丝锥组件10可只用于一次手术,然后抛弃。说明120还可针对工具箱106中部分或所有物品的重复灭菌给出明确的指示,并且还可指示医生按照与可用生物废弃过程使用一致而抛弃至少工具箱106中这些物品。包装在无菌工具箱106中的器具集合104的呈现可向医生证实,其中物品是无菌的并且未被使用过。医生就可确信器具104能够达到确认的性能和无菌规格。
在使用中,锁环20在限位环18和丝锥12上滑动,并且当紧固在限位环18上时,锁定组件10。该锁定可通过将限位环18的指状夹68上的节子48推进轴22上的凹槽24中而实现。指状夹88的基端(即指状夹顶端)可与和节子48所搭合的凹槽24相关的丝锥轴12上的长度指示器26对准。一旦限位环18和锁环20连接到一起,节子48和锁定凹槽24之间的搭合程度可被控制。凹槽24和节子48之间的搭合程度可通过设计从紧搭合到松搭合变化。当松搭合时,限位环18和锁环20可以类似棘轮方式沿轴22一致移动。为增进限位环18、锁环20和轴22之间的可靠连接,限位环18的指状夹68在外表面76上具有突出区域80,使得内表面78上的节子48被稳固地推入与凹槽24搭合。在可调节长度丝锥组件锁定时,限位环18和锁环20优选地能够抵抗至少约为300N的轴向力而没有轴向移动。
可调节长度丝锥组件10针对的是在骨中钻孔和攻丝的方法。开始时,用户为布置适于特定螺钉构造的孔而选择丝锥12。该选择可基于,例如螺钉直径或螺钉零件号。限位环18而后可沿轴22围绕丝锥12安装,指状夹68的基端54指向轴22的基端31。锁环20可随后安置在丝锥12的轴22上,使得锁环20能够与限位环18连接。随后,限位环18和锁环20连接到一起。
限位环18和锁环20之间的连接紧密度可以调节以在限位环—锁环组合和丝锥12之间提供期望的阻力。限位环18的位置可以调节到由对准限位环的基端74与期望的长度指示器26而设置的期望丝锥长度28。例如,数字5可以表示相对于长度为5mm的特定螺钉的适当长度指示器26。限位环18和锁环20之间的连接可以随后被紧固,以保证丝锥组件的选定长度28。或者,限位环的位置可以以提供期望的丝锥长度而定位。限位环18可被推进以设置期望位置,而锁环位于轴上,与限位环连接,并调节锁定限位环和锁定环组件到轴上合适位置。
自钻孔丝锥12的基端31可随后插入到钻孔机(未示出)中,比如电池驱动的可换向的钻孔机。自钻孔丝锥12的顶端13可承座在导向板中,并将其与骨接触。钻孔机可随后用于旋转可调节长度丝锥组件10以在骨中攻丝及钻出预定尺寸的孔。当限位环的凸缘58接触到导向板时,可达到孔的最大深度。可调节长度丝锥组件10而后可从孔及导向板撤出,螺钉进入并固定在所钻的孔中。
组件10的锁定机构允许限位环18和锁环20相对于丝锥12旋转,特别是当限位环18碰到钻孔导向板之后,而不影响先前调节好的长度28的设定。因为节子48稳固地承座在凹槽24中,所以丝锥长度28不会改变,然而限位环18和锁环20可围绕轴22自由旋转。这可使得具有预选择深度的孔易于准确推进。例如,如果在限位环18与轴之间的相对旋转会改变露出的车螺纹28的长度,则每当限位环碰到钻孔导向板时,在轴和限位环之间产生的运动会潜在地致使丝锥12改变长度28。如果车螺纹28露出的长度无意地延长,则钻孔将不期望地运行以穿过骨而进入邻近组织。相反,如果车螺纹28露出的长度减小,则进入未钻孔的骨的螺钉会损坏骨或使骨断裂,或者在可再吸入螺钉的情况下,螺钉就会损坏。
由于限位环18和锁环20沿轴22运动时节子48与凹槽24搭合和分离的棘轮效应,丝锥组件还可提供有用的触觉反馈给用户。当限位环沿轴从第一已知丝锥长度设置到期望的第二丝锥长度设置平移,用户可感觉和听到节子与凹槽搭合和分离时发出的咔嗒声的数量,因此,增加了在治疗过程中丝锥调节的速度。此外,因用户不需要依靠纯视觉系统来选择或检查丝锥组件的长度调节,因此,锁定机构的棘轮效应可提供附加的检查以保证选择的是合适的长度。
虽然上述可调节长度丝锥组件参考某些优选实施例进行了描述,但是应当知道本发明的范围不限于这些实施例。例如,可调节长度丝锥组件可作修改或延长以容纳特定成分的结构材料或可需要不同连接元件的加工方法。同样,轴上凹槽的数量和间距可改变以容纳不同的螺钉长度。同样,不同的材料和表面涂层,或不同材料的外层可应用于可调节长度丝锥组件。此外,上述实施例可以修改,使得一个实施例的一些特征与另一实施例的特征一起使用。本领域的技术人员可对这些优选实施例作改变,然而,都将在本发明的精神和由下面提出的权利要求确定的范围内。

Claims (24)

1.一种用于在骨中钻孔的可调节长度丝锥组件,包括:
具有纵轴、基端和末端的轴,至少该轴的一部分具有在骨中钻孔的车螺纹,
具有基端和末端的限位环,该限位环以沿轴的纵轴可平移而构造和确定尺寸,其还包括具有内表面和外表面的主体,至少内表面的一部分以与轴在至少一个预定的位置搭合而构造和确定尺寸;和
锁环,其包括以在限位环的至少一部分上被容纳而构造和确定尺寸的元件,
其中锁环以与限位环搭合从而设定车螺纹的至少一个有效长度而构造和确定尺寸,从而防止限位环沿轴的纵轴移动。
2.如权利要求1的组件,其中锁环包括具有孔的管状元件。
3.如权利要求1的组件,其中锁环的末端能够在限位环基端的上方平移。
4.如权利要求1的组件,其中锁环和限位环以至少两种构造相互搭合,沿轴将限位环和锁环连接在一起并允许限位环和锁环一起沿轴平移运动的第一构造,和防止限位环沿轴的纵轴平移运动的第二构造。
5.如权利要求4的组件,其中当限位环和锁环以第二构造搭合时,限位环和锁环围绕轴自由旋转。
6.如权利要求1的组件,其中轴由生物相容的材料制成。
7.如权利要求6的组件,其中轴可由非磁性材料制成。
8.如权利要求1的组件,其中锁环的至少一部分是透明的。
9.如权利要求8的组件,其中锁环的至少一部分由医用聚碳酸酯形成。
10.如权利要求1的组件,其中限位环包括至少一个指状夹,以及轴包括多个凹槽,该至少一个指状夹具有内、外表面和形成在内表面上的至少一个突起,该突起以与至少一个凹槽相互作用而防止限位环沿轴的纵轴平移运动而构造和确定尺寸。
11.如权利要求10的组件,其中凹槽围绕轴连续地延伸。
12.如权利要求10的组件,其中凹槽相互基本等距。
13.如权利要求10的组件,其中凹槽基本相同。
14.如权利要求10的组件,其中该至少一个指状夹由至少两个纵向的、沿限位环的一部分延伸的狭缝形成,至少该两个狭缝中的每个的一部分从限位环外表面向限位环内表面延伸。
15.如权利要求10的组件,其中限位环具有至少两个指状夹,所述指状夹大体相同并围绕限位环的中心轴以基本对称的结构布置。
16.如权利要求10的组件,其中锁环以支承于该至少一个指状夹的外表面上的结构上而构造和确定尺寸,以使该至少一个突起与轴上的凹槽可拆卸地搭合。
17.如权利要求10的组件,其中该至少一个突起具有中点,并且至少一个凹槽具有中点,从该至少一个突起的中点到限位环基端的第一距离与从该至少一个凹槽的中点到相应的长度指示器标记的第二距离相关。
18.如权利要求17的组件,其中第一距离与第二距离基本相等。
19.如权利要求1的组件,其中轴包括多个长度指示器标记,该长度指示器标记以允许至少一个有效长度的受控设定而构造和确定尺寸。
20.如权利要求19的组件,其中每个长度指示器标记以对应于一个有效长度而构造和确定尺寸。
21.如权利要求20的组件,其中每个长度指示器标记以当设定一个有效长度时在视觉上与限位环基端对齐而构造和确定尺寸。
22.一种用于在骨中钻孔的可调节长度丝锥组件,包括:
具有纵轴、基端和末端的轴,至少该轴的一部分具有在骨中钻孔的车螺纹,
具有基端和作用为挡块以设定车螺纹有效长度的末端的限位环,该限位环以沿轴的纵轴可平移而构造和确定尺寸,其包括具有内表面和外表面的主体,至少内表面的一部分以与轴在至少一个预定的位置搭合而构造和确定尺寸;和
锁环,其包括以在限位环的至少一部分上被容纳而构造和确定尺寸的元件,
其中锁环以与限位环搭合从而可调节地设定车螺纹的有效长度而构造和确定尺寸,从而防止限位环沿轴的纵轴移动。
23.一种用于钻孔和攻丝的手术工具箱,包括:
多个轴,每个轴具有纵轴、基端和末端,每个轴的至少一部分具有用于在骨中钻孔的车螺纹,
具有基端和末端的限位环,该限位环以可沿多于一个轴的纵轴平移而构造和确定尺寸,并包括具有内表面和外表面的主体,至少内表面的一部分以与多于一个轴在至少一个预定的位置搭合而构造和确定尺寸;和
锁环,其包括以在限位环的至少一部分上被容纳而构造和确定尺寸的元件,
其中,锁环以与限位环搭合从而设定车螺纹的至少一个有效长度而构造和确定尺寸,从而防止限位环沿至少该轴的纵轴移动。
24.一种用于在骨中钻孔和攻丝的方法,包括:
选择具有纵轴和车螺纹的自钻孔丝锥,该车螺纹位于自钻孔丝锥的末端,并适于在骨中产生和钻孔以容纳螺纹;
在丝锥上安装限位环;
在丝锥上安置锁环;
至少使锁环的一部分在限位环基端上方穿过;
将限位环与锁环连接;
将限位环安置在至少一个预定位置,以设定车螺纹的有效长度;
通过视觉观察限位环基端相对于自钻孔丝锥的位置而核定限位环的位置;
锁定自钻孔丝锥、限位环和锁环沿纵轴的相对的纵向运动;
在骨上放置车螺纹;
旋转自钻孔丝锥以使车螺纹进入骨中;
接触限位环的一表面以阻碍自钻孔丝锥的进入;和
从骨中移出自钻孔丝锥。
