Abstract
Description
Diclofenac was patented as a substance as early as 1965, 10 years later it became one alternative synthesis pending. It has since been excellent in the treatment of acute Arthritis including gout attack, chronic arthritis, especially rheumatoid arthritis (chronic polyarthritis), as well as ankylosing spondilitis (ankylosing spondylitis) and others inflammatory rheumatic spine disorders, irritation in degenerative joint and Spinal disorders (arthrosis and spondylarthrosis), soft tissue rheumatism, painful swelling or inflammation after injury or surgery as well other non-rheumatic pain conditions and dysmenorrhea. There as Side effects often in this substance class and so also with Diclofenac gastric and Intestinal ulcers, gastrointestinal complaints such as nausea, vomiting, heartburn, Stomach pain, feeling of fullness, constipation or diarrhea occur with oral administration and a large proportion of the indications can also be treated topically, especially in the case of irritation with degenerative joint and spine diseases (Arthrosis and spondylarthrosis), soft tissue rheumatism, painful swelling or Inflammation after injury and surgery, the substance is also topical Formulations such as creams, ointments, gels, sprays etc. are used. This penetrates out In the formulation, diclofenac released the skin barrier to become inflamed Enrich soft tissue due to the inflammatory acidic environment and there to a topical effect in the painful and inflamed region of the body part Act.
Gurny R, Teubner A; Dermal and transdermal drug delivery, Wiss. Verlagsges. Stuttgart
The penetration of drugs through the skin is largely determined by the physico-chemical properties of the substance. The octa-nol / water distribution coefficient and the molecular size play a role here (Potts RO, Guy RH in: Gurny R, Teubner A; Dermal and transdermal drug delivery, Wiss. Verlagsges. Stuttgart (1993)). Since these parameters cannot be influenced without molecular modifications, there are essentially only two ways to increase the penetration rate:
Facilitate diffusion by adding penetration accelerators or application of electrical voltage (iontophoresis)
Increase in the drug concentration in the base also beyond the solubility limit out (supersaturation).
DE 198 30 649, WO 96229988 etc.).As a penetration accelerator u. a. Fatty acids, fatty alcohols, simple and polyvalent Alcohols, laurocapram and surfactants are used. However, many of these substances are overactive a disturbance of the skin's barrier function and are therefore more or less irritating to the skin classified. Nevertheless, numerous such systems are described in patent specifications (cf. DE 198 30 649, WO 96229988 etc.).
The use of systems in which the active ingredient is in a supersaturated form is more compatible is present. Usually the maximum flux of a substance through the skin is through its Solubility in the cornea (stratum corneum), which is the main barrier to penetration represents limited. This saturation concentration will occur when the Active ingredient in the vehicle, e.g. B. in the matrix of the transdermal system, also in one Concentration is present that corresponds to the solubility in the vehicle. One way to do this. To further increase maximum thermodynamic activity is to inject the drug a concentration exceeding the solubility in the vehicle. This is e.g. B. possible by incorporating the diclofenac sodium into acrylate copolymers (DE 198 43 027). However, the setting of the supersaturation must be so sensitive that the supersaturation is so are as high as possible, but are also as stable as necessary, as is well known, as oversaturated systems are metastable and change to the saturated state by recrystallization Storage. This then has the disadvantage that these systems increase due to the crystallization Lead product complaints due to lack of aspect as well as lack of adhesive strength. Close contact between the dermal system and the skin is also necessary in effective amount of diclofenac in the target area of the inflamed soft tissue receive.
It has now been found that incorporating the diclofenac sodium into a whole special acrylate copolymer achieved such a stable supersaturation that one too an effective product without having to add penetration accelerators as also obtains optimal adhesion to the skin, which is the shape that in close contact between the dermal system and the outer skin barrier over several days up to a maximum a week there is still a removability at any time without it Pain sensation still comes to skin irritation. This achieves the adhesive power of the dermal system according to the invention a significantly longer wearing time than, for example, products that are on the market and contain water-like preparations of the type of a cataplasm or Envelopes contain and much longer contact than conventional topical ones Formulations such as creams, gels, emulgels or sprays that come into contact with water or Clothes are removed.
Wirkstofffreisetzung in vitro. Die nachfolgende Tabelle gibt die Copolymer-Zusammensetzung
wieder:
Several solvent-based acrylate copolymers, as provided by National Starch & Chemical, BV, Zutphen, Netherlands under the trade name Durotak, were tested for their adhesive properties and the active ingredient release in vitro. The following table shows the copolymer composition:
As can be seen, the wearing properties are only through the use of the Durotak 4098 reached. However, it was surprisingly found that diclofenac was extremely bad from this Polymer is released again. This is surprising because this adhesive is free of functional groups and thus does not actually interact with the Diclofenac should result. Durotak 4350 is the best in terms of drug release suitable, but it does not adhere sufficiently. Also contains this adhesive Crosslinker, the resulting active substance-containing adhesive can no longer be spread out, but already gels in the solvent-containing state. Modify it now well releasing adhesive by veneering with small portions of Durotak 4098, so you get the good release and saturation solubility, but at the same time increases the adhesive strength in vitro and in vivo. It is completely unknown why the levels of vinyl acetate are so high of 5-10% already significantly improve the adhesive strength, shares of more than 10-50%, however Prevent the release of the active substance. In other words, a share of Vinyl acetate in the acrylate copolymer, which has a ratio of vinyl acetate to 2- Ethylhexyl acrylate must not stand 1: 1, but 1: 5-1: 12 in order to achieve optimal conditions for one dermal system with diclofenac sodium.
Only the adhesive mixture mentioned above leads to the best result, which was surprising. It these are special, unknown interactions between the active ingredient and Acrylate copolymer obtained by radical polymerization.
In addition, the carriers of the matrix play an important role in the wearing properties. Since the dermal system has to be applied to joints, great flexibility is required. Various materials were tested covering nonwovens ("Nonwovens"), foams ("Foams"), foils and fabrics ("Wovens"). It was also important for the compatibility that the carrier used has good water vapor permeability. A longitudinally and transversely elastic fabric made of polyester, which is available in white or skin colors from Karl Otto Braun, Germany, proved to be optimal. A siliconized polyester film known to the person skilled in the art, e.g. B. Hostaphan RN 100 from Diafoil, Hoechst, Germany, easy / easy, which must not be too thin (at least 36 µm layer thickness, preferably 100 µm layer thickness), so such a large system of 70 to 200 cm 2 , preferably from 90 to 140 cm 2 is still well applicable by the patient.
The dermal therapeutic systems according to the invention are preferably such that that it consists of a top layer impermeable to the active ingredient, one on the top layer adhesive adhesive layer containing active ingredient and a removable protective layer.
Die aufgebrachte wirkstoffhaltige Kleberschicht hat eine Dicke von 20 bis 500 µm.This simplest form of TDS can be made in the manner well known to those skilled in the art that a solution of the adhesive in a low boiling solvent with the Active ingredient is mixed, the mixture evenly on a removable protective layer is applied, the solvent is removed by heating and the product obtained with a carrier is covered. The applied active substance-containing adhesive layer has a thickness from 20 to 500 µm.
