Abstract
Ein Embryo-Austauschkatheter weist einen biegsamen, extrudierten Schaft 1 aus einem transparenten Polyurethan mit einer Bohrung 10, die sich entlang seiner Länge erstreckt, auf. Gasblasen 12 mit einem Durchmesser im Bereich von 5 mu bis 10 mu werden durch Zugeben von Gas während der Extrusion in die Dicke der Wand des Schafts eingegliedert. Die Blasen 12 werden ausgewählt, um die Sichtbarkeit des Katheters unter Ultraschallabbildung zu steigern, während dennoch ermöglicht wird, daß eine Materialströmung entlang des Katheters zu sehen ist.
Description
- Diese Erfindung betrifft medizinisch-chirurgische Vorrichtungen aus Kunststoffmaterial.
- Die Erfindung betrifft insbesondere medizinisch-chirurgische Vorrichtungen wie z. B. Katheter, die unter Ultraschallbeobachtung sichtbar sind.
- Eine Ultraschallabbildungsausrüstung wird während Operationsvorgängen zunehmend verwendet, um den Ort einer Vorrichtung innerhalb des Körpers zu überwachen. Die Sichtbarkeit einer Vorrichtung unter Ultraschall hängt von verschiedenen Faktoren ab, einschließlich der Differenz zwischen der Schallimpedanz des Materials der Vorrichtung und jener des umgebenden Mediums wie z. B. des Patientengewebes oder von Körperflüssigkeit, in der sich die Vorrichtung befindet. Diese Differenz ist bei Kunststoffvorrichtungen wie z. B. Kathetern relativ gering und kann herkömmliche Katheter schwierig aufzufinden machen. Selbst Vorrichtungen aus Metall wie z. B. Nadeln werfen aufgrund der gerichteten Art der Reflexionen Probleme der Sichtbarkeit unter Ultraschallbeobachtung auf. In einigen Orientierungen kann eine Metallnadel deutlich sichtbar sein, aber in anderen Orientierungen kann sie beträchtlich weniger sichtbar sein.
- Versuche wurden unternommen, um die Sichtbarkeit von medizinisch-chirurgischen Vorrichtungen unter Ultraschallbeobachtung in verschiedenen Weisen zu steigern. Die Oberfläche der Vorrichtung kann modifiziert werden, wie z. B. durch Ausbilden von Nuten oder Einkerbungen in ihrer Oberfläche. Eine Reflexionsbeschichtung kann auf die Vorrichtung aufgebracht werden, wie z. B. Eingliedern von Blasen, wie in WO 98/19713 und EP 0624342 beschrieben. Alternativ kann eine Metallmarkierung an einem Kunststoffkatheter befestigt werden.
- Es ist eine Aufgabe der vorliegenden Erfindung, eine alternative medizinisch-chirurgische Vorrichtung bereitzustellen.
- Gemäß der vorliegenden Erfindung wird eine medizinisch-chirurgische Vorrichtung der vorstehend angegeben Art bereitgestellt, dadurch gekennzeichnet, daß das Material Gasblasen durch den Hauptteil der Dicke des Materials in zumindest einem Teil der Vorrichtung enthält, um die Sichtbarkeit der Vorrichtung unter Ultraschallabbildung zu steigern.
- Die Vorrichtung ist vorzugsweise röhrenförmig und die Gasblasen können um den gesamten Umfang der Vorrichtung vorgesehen sein oder können in einem Bereich der Vorrichtung, der nur einen Teil des Umfangs der Vorrichtung belegt, vorgesehen sein. Die Außenfläche der Vorrichtung kann glatt und nicht von Gasblasen unterbrochen sein, und die Vorrichtung kann eine Innenfläche aufweisen, die glatt und nicht von Gasblasen unterbrochen ist. Die Blasen können einen Durchmesser im Bereich von 1 µ bis 50 µ aufweisen und können im wesentlichen kugelförmig sein. Die Vorrichtung kann extrudiert werden, wobei die Gasblasen durch Hinzufügen von Gas während der Extrusion der Vorrichtung ausgebildet werden. Das Kunststoffmaterial ist vorzugsweise im wesentlichen transparent, wobei die Größe und die Dichte der Blasen derart ausgewählt wird, daß ermöglicht wird, daß eine Materialströmung entlang der Vorrichtung mit dem Auge betrachtet wird.
- Ein Embryotransfer-Katheter und sein Herstellungsverfahren gemäß der vorliegenden Erfindung werden nun beispielhaft mit Bezug auf die zugehörigen Zeichnungen beschrieben, in denen gilt:
- Fig. 1 ist ein Seitenaufriß des Katheters;
- Fig. 2 ist ein Seitenschnittaufriß eines Teils des Katheters von Fig. 1 in einem größeren Maßstab;
- Fig. 3 stellt schematisch die Herstellung des Katheters dar; und
- Fig. 4 ist eine Querschnittsansicht durch einen alternativen Katheter.
- Mit Bezug zunächst auf die Fig. 1 und 2 umfaßt der Katheter einen biegsamen Schaft 1 und eine Nabe 2, die am hinteren Ende des Schafts angefügt ist. Der Schaft 1 weist einen kreisförmigen Querschnitt und eine Bohrung 10 auf, die sich entlang seiner Länge erstreckt. Der Schaft 1 öffnet sich an seinem vorderen, rechten Patientenende 11, das atraumatisch abgerundet ist. Der Schaft 1 wird aus einem klaren, transparenten Polyurethanmaterial extrudiert und enthält kleine, mit Gas gefüllte Blasen 12, deren Größe und Verteilung ausgewählt werden, um die Sichtbarkeit des Katheters unter Ultraschallbeobachtung zu steigern. Typischerweise weisen die Gasblasen einen Durchmesser im Bereich von etwa 0,1 µ bis 300 µ, vorzugsweise zwischen 1 µ und 50 µ auf, wobei der bevorzugteste Bereich 5 µ bis 10 µ ist. Die Blasen 12 erstrecken sich durch die gesamte Dicke der Wand des Schafts 1 und können kugelförmig sein oder eine beliebige andere regelmäßige oder unregelmäßige Form aufweisen. Die Außen- und die Innenfläche 13 und 14 des Schafts können glatt und nicht von Gasblasen unterbrochen sein oder die Blasen können die Oberfläche unterbrechen.
- Die Nabe 2 dient zur Herstellung einer Verbindung mit dem Schaft 1 und wird aus einem starren, transparenten Kunststoffmaterial geformt und anschließend an das hintere Ende des Schafts geklebt.
- Der Schaft 1 wird in der in Fig. 3 gezeigten Weise unter Verwendung einer Extrusionsmaschine 20 extrudiert. Polyurethanmaterial 21 wird erhitzt und dem Extrusionskopf 22 in der üblichen Weise zugeführt, aber auch ein Gas wie z. B. Stickstoff oder Kohlendioxid wird durch den Einlaß 23 unter Druck in die Schmelze eingeblasen. Wenn der Kunststoff aus dem Extrusionskopf 22 austritt, expandiert das Gas, um die Blasen 12 zu bilden. Die relativ gasdurchlässige Art des Kunststoffs bedeutet, daß nach der Herstellung das die Blasen bildende Bas schnell entweicht und gegen Luft ausgetauscht wird.
- Der Schaft 1 kann mit niedrigen Kosten kontinuierlich extrudiert werden ohne den Bedarf für irgendwelche anschließenden Vorgänge abgesehen vom Befestigen der Nabe 2 und vom Endformen der Patientenendspitze 11.
- Der Katheterschaft könnte durch andere Schmelzprozesse wie z. B. Spritzgießen oder Blasformen ausgebildet werden.
- Die Blasen könnten in anderen Weisen als durch Einblasen von Gas in die Schmelze ausgebildet werden. Chemische Schaumerzeuger könnten beispielsweise zum Kunststoffmaterial zugegeben werden, wie z. B.: Azocarbonomide, Dinitrosopentmethylentetramin, Benzenphonohydrazin, 4,4-Oxybis(benzenphonohydrazin), NN1-Dimethyl-NN1- dinitrosoterephthalamid, Azoisobutyronitril, Natriumbicarbonat, Terephthalazid oder Trihydrazinatrazin. Eine weitere Art und Weise zum Ausbilden der Gasblasen würde im Eingliedern einer Flüssigkeit in die Kunststoffschmelze, die während des Schmelzprozesses verdampft, bestehen. Alternativ könnte festes, pulverförmiges Trockeneis (Kohlendioxid) in die Schmelze eingegliedert werden, so daß die Teilchen des Trockeneises während des Ausbildungsprozesses zu Gasblasen werden. Es könnte möglich sein, andere Feststoffe zu verwenden, die in dieser Weise eine Sublimation durchmachen. Die Blasen könnten direkt infolge einer chemischen Reaktion während der Polymerisation und/oder alternativ während der Vernetzung ausgebildet werden. Die Blasen könnten mechanisch durch Schlagen des Kunststoffs in einer flüssigen Form ausgebildet werden, wie z. B. in der zum Ausbilden von Latexschaum verwendeten Art und Weise. Die Blasen könnten durch die Eingliederung von hohlen Mikrokugeln aus Harz oder Glas ausgebildet werden. Alternativ könnten kleine Teilchen aus einem löslichen Material zur Kunststoffschmelze zugegeben und anschließend weggelöst werden.
- Ein Schaft dieser Art kann unter Ultraschallabbildung eine gute Sichtbarkeit aufweisen, ohne mehrere Echos zu erzeugen, und kann ungeachtet der Orientierung des Schafts ein gutes Bild erzeugen. Der Schaft kann für Ultraschallenergie ausreichend transparent gemacht werden, um zu ermöglichen, daß eine Materialströmung entlang der Bohrung des Katheters auf dem Ultraschallbild beobachtet wird.
- Da der Katheter keine Beschichtung oder separate Markierung erfordert, besteht kein Bedarf für anschließende Montagevorgänge und es besteht kein Risiko einer Abtrennung. Der Katheter kann aus herkömmlichen medizinisch zugelassenen Materialien bestehen, so daß er kein neues Risiko für den Patienten darstellt. Da die Oberfläche des Katheters glatt sein kann, kann der Katheter mit geringer Reibung durch einen äußeren Tubus eingeführt oder geschoben werden. Die glatte Bohrung des Katheters stellt eine freie Strömung entlang der Bohrung sicher, die wichtig sein kann, wenn der Katheter zum Transfer von Embryos verwendet wird. Die glatten Oberflächen verringern auch die Ansammlung eines Mikroorganismenfilms auf dem Katheter. Der Katheter kann ohne den Bedarf für Metallkomponenten hergestellt werden, was ein Vorteil sein kann, wenn der Katheter verwendet wird, während der Patient durch magnetische Abbildungsverfahren betrachtet wird. Der Katheter kann für Röntgenstrahlen vollständig durchlässig sein oder der Kunststoff, aus dem er besteht, könnte einen für Röntgenstrahlen undurchlässigen Füllstoff wie z. B. Bariumsulfat enthalten.
- Die Blasengröße und -dichte kann so ausgewählt werden, daß die optische Transparenz des den Schaft bildenden Kunststoffs ausreichend bleibt, um zu ermöglichen, daß eine Materialströmung entlang des Schafts mit dem Auge betrachtet wird.
- Es gibt verschiedene Arten und Weisen, in denen der Katheter modifiziert werden könnte. Es könnte beispielsweise bevorzugt sein, daß die Blasen eine nicht-kugelförmige Form aufweisen und in einer speziellen Richtung wie z. B. der Länge nach orientiert sind. Dies könnte durch ein Hindernis in der Extrusionsdüse erreicht werden, das die Blasen einschränkt und langstreckt, während sie hindurchströmen. Eine solche Anordnung kann eine Steigerung der Ultraschallsichtbarkeit ergeben, während die Undurchsichtigkeit des Schafts für das Auge verringert wird.
- Es ist nicht wesentlich, daß die Blasen um den gesamten Umfang des Schafts vorgesehen sind. Wie in Fig. 4 gezeigt, könnten die Blasen 12' nur in einem oder mehreren Streifen, die sich entlang des Schafts 1' erstrecken, wie z. B. im Streifen 40, ausgebildet sein. Diese Anordnung kann verwendet werden, wenn der Schaft eine erhöhte Klarheit aufweisen muß, so daß das Material innerhalb des Katheters mit dem Auge zu sehen ist. Der Blasenbereich muß entlang der Länge des Katheters nicht durchgehend sein. Statt dessen könnten diskrete, separate Bereiche mit Blasen entlang der Länge des Katheters durch Bereiche ohne Blasen voneinander getrennt sein. Ein Schaft für einen solchen Katheter könnte durch Unterbrechen der Gasströmung zum Extruder hergestellt werden. Wenn die Blasen innerhalb eines Streifens enthalten sind, könnte dieser unterbrochen werden, um ihn unstetig zu machen, indem der Streifen unter Verwendung von zwei Hilfsextrudern extrudiert wird, wobei einer ein Material mit einem Treibmittel aufweist und der andere ein Material ohne das Treibmittel aufweist. Abwechselnde Extruder werden ein- und ausgeschaltet, so daß der Streifen Abschnitte aufweisen kann, die Blasen enthalten und die durch Abschnitte ohne Blasen voneinander getrennt sind. Ein Katheter mit einem unterbrochenen Blasenbereich kann eine klarere Ultraschallangabe der Bewegung des Katheters entlang seiner Länge ergeben und kann auch eine klarere Beobachtung einer Materialströmung entlang des Katheters sowohl durch Ultraschall als auch durch das Auge ermöglichen.
Claims (10)
1. Medizinisch-chirurgische Vorrichtung (1, 1') aus einem Kunststoffmaterial,
dadurch gekennzeichnet, daß das Material durch den Hauptteil der Dicke des
Materials in zumindest einem Teil der Vorrichtung Gasblasen (12, 12') enthält, die
ausgewählt sind, um die Sichtbarkeit der Vorrichtung unter Ultraschallabbildung
zu steigern.
2. Vorrichtung nach Anspruch 1, dadurch gekennzeichnet, daß die Vorrichtung (1,
1') röhrenförmig ist.
3. Vorrichtung nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß die Gasblasen
(12) um den gesamten Umfang der Vorrichtung (1) vorgesehen sind.
4. Vorrichtung nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß die Gasblasen
(12') in einem Bereich (40) der Vorrichtung (1') vorgesehen sind, der nur einen
Teil des Umfangs der Vorrichtung belegt.
5. Vorrichtung nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet,
daß die Außenfläche (13) der Vorrichtung (1, 1') glatt und nicht von Gasblasen
(12, 12') unterbrochen ist.
6. Vorrichtung nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet,
daß die Vorrichtung eine Innenfläche (14) aufweist und die Innenfläche glatt und
nicht von Gasblasen (12, 12') unterbrochen ist.
7. Vorrichtung nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet,
daß die Blasen (12, 12') einen Durchmesser im Bereich von 1 µ bis 50 µ
aufweisen.
8. Vorrichtung nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet,
daß die Blasen (12, 12') im wesentlichen kugelförmig sind.
9. Vorrichtung nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet,
daß die Vorrichtung (1, 1') extrudiert wird und daß die Gasblasen (12, 12') durch
Zugeben eines Gases während der Extrusion der Vorrichtung ausgebildet werden.
10. Vorrichtung nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet,
daß die Vorrichtung (1, 1') röhrenförmig ist, daß das Kunststoffmaterial im
wesentlichen transparent ist und daß die Größe und die Dichte der Blasen (12, 12')
derart ausgewählt werden, daß ermöglicht wird, daß eine Materialströmung
entlang der Vorrichtung mit dem Auge betrachtet wird.
