Die i-Klammern werden seriell hintereinander in einem Magazin liegend entlang eines Seitenkanales gefördert, der parallel zur Hohlnadel und dem darin axial gleitgelagerten Hauptplunger verläuft. Danach wird eine erste Halbhülse der Innenhülse axial aus dem Gewebe zurückgezogen, so dass der distal vorderste Nagel sich auf der Seite der zurückgezogenen Halbhülse aufspreizt und sich distal in das Gewebe verkrallt, sowie proximal an den Innenmantel der Außenhülse anlegt. Danach wird die zweite Halbhülse der Innenhülse axial aus dem Gewebe zurückgezogen, mit dem gleichen Effekt. Schließlich wird dann der gesamte Applikator vom Ort der Applizierung des Nagels weggezogen, so dass der Nagel proximal außer Eingriff mit der Außenhülse gelangt und sich seine proximalen Enden um die Fäden des Herniennetzes herum ebenfalls in das Herniengewebe einkrallen. Durch eine Vorschubautomatik des Applikators werden nun alle Nägel im Magazin um eine Position distal nach vorn vorgerückt und der nächste Nagel kann an einer anderen Stelle appliziert werden. Nachteil ist, dass als Einschnitt in das Gewebe ein etwa halbrundförmiger Lappen ausgeschnitten wird, der durch das dezentral arbeitende Messer etwa u-förmig aus dem Gewebe herausgeschnitten wird. Durch diesen halbrundförmig geschnittenen und relativ großflächigen Gewebelappen soll nun die Nadel hindurchdringen und ein Netz verankern. Nachteil dieser Art der Anbringung des Nagels ist demzufolge, dass durch den großflächigen Gewebelappen es schwierig ist, den durch diesen Gewebelappen hindurchgebrachten Nagel soweit aufzuspreizen, dass er sich an den umliegenden Flächen des ausgestanzten Geweberandes festlegt. Weil eben gerade ein relativ großflächiger Gewebelappen durch die dezentrale Anordnung des Messers im Halter gegeben ist, ergibt sich dementsprechend auch ein großflächiger Gewebelappen, so dass die Halterung eines Netzes mit den angegeben Nägeln relativ schwierig ist. Überdies wird der Heilungsprozess durch das Ausschneiden eines derartigen, großflächigen Gewebelappens stark beeinträchtigt. Zudem besteht noch der weitere Nachteil, dass das dezentral schneidende Messer Schwierigkeiten hat, wenn ein solches Messer auf einen Faden des Netzes vordringt, denn dann ist ein weiteres Eindringen nicht mehr möglich, obwohl die Nadel noch ins Gewebe vorgeschoben werden könnte. Bei der genannten Druckschrift besteht dem gemäß die Gefahr, dass Fäden des Netzes verletzt werden, weil eine dezentrale Schneidaktion stattfindet. Weiterer Nachteil ist, dass die Leitaktion mit dem hälftigen, dezentralen Messer erfolgt, welches sehr tief den vorbeschriebenen halbkreisförmigen Lappen aus dem Gewebe herausschneidet und dass dann der Nagel freistehend und ohne besonderen Druck in das Gewebe eingesetzt wird. Hieraus ergibt sich, dass es überhaupt keinen Gegendruck gibt, so dass sich der Nagel zunächst nicht formschlüssig an das Netz anlegt. Nur durch die Tatsache, dass das Material des Nagels aus einem selbstlernenden Metallmaterial ist, soll sich dieser dann mit bogenförmigen Haltearmen um die Netzfäden herumschlingen, was mit dem Nachteil verbunden ist, dass überhaupt kein Gegendruck an das Netz stattfindet. Vorrichtung nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass an der kreisförmigen Scheibe (20) des Gewebenagels (13, 13a) ein Schaft (24) mit geringerem Querschnitt ansetzt, an dessen Außenumfang eine Reihe in Richtung der kreisförmigen Scheibe (20) zurückweisende Widerhaken (25) angeordnet sind, und der Schaft (24) in die Spitze (26) übergeht, an der sich ein distaler stirnseitiger quer verlaufender Schlitz (27) befindet, in dessen distalem Bereich die Durchgangsausnehmung (21, 28, 50) eine axiale Rinne (50) ausbildet, welche leicht konisch zugespitzt in Richtung freies Ende der Spitze (26) des Gewebenagels (13) verläuft, bei dem sich auf der proximalen Rückseite der kreisförmigen Scheibe (20) zwei einander gegenüberstehende axiale Anschlagflächen (70) befinden, die an zwei einander in der Öffnung (21) gegenüberstehende Rippen (22) angeordnet sind, welche den Durchmesser der Öffnung (21) verringern, wobei distal im Gewebenagel (13, 13a) dazu korrespondierende axiale Anschlagflächen (71) am Grunde eines in der Spitze (26) befindlichen quer verlaufenden Schlitzes (27) zugeordnet sind, zur jeweiligen Anlage an einen anderen in Schubrichtung davor und/oder dahinter liegenden anderen Gewebenagel (13, 13a).Device according to one of claims 1 to 4, characterized in that on the circular disc ( 20 ) of the tissue nail ( 13 . 13a ) a shaft ( 24 ) with a smaller cross-section sets at the outer periphery of a row in the direction of the circular disc ( 20 ) rejecting barbs ( 25 ) are arranged, and the shaft ( 24 ) in the top ( 26 ), at which a distal front-side transverse slit (FIG. 27 ) is located in the distal region of the through-hole ( 21 . 28 . 50 ) an axial groove ( 50 ) which tapers slightly towards the free end of the tip ( 26 ) of the tissue nail ( 13 ), in which on the proximal back of the circular disc ( 20 ) two opposing axial abutment surfaces ( 70 ), which are connected to each other in the opening ( 21 ) opposed ribs ( 22 ) are arranged, which the diameter of the opening ( 21 ), distally in the tissue ( 13 . 13a ) corresponding axial abutment surfaces ( 71 ) at the bottom of one in the top ( 26 ) transverse slot ( 27 ) are assigned to the respective plant to another in the direction of thrust in front and / or behind it other tissue nail ( 13 . 13a ).
