Abstract
Description
The invention relates to a parietal prosthesis element in the strength of a biocompatible, not resorbable open, openwork fabric with blocked stitches, with a Texture that this along two roughly perpendicular to each other gives tensile strength to extending directions, and wherein the thread or threads that make up this tissue, at least in part consist of a multitude of threads.
Such a parietal prosthesis element is from the FR-A-2 712 177.
Such a parietal prosthetic element or an ent speaking parietal prosthesis can be used in humans or animals can be implanted, and can be used especially in different surgical treatments are used, such as the treatment of inguinal hernias or crural hernias, and in a transparent way, or by celioscopy (or Laparoscopy), and can also help with defects in the abdominal wall Find application.
According to French document FR-A-2 712 177 On behalf of the applicant, the company COGENT provides various parietal prosthesis elements, especially among the trade designations PARIETEX PAC, PARIETEX PAL and PARIETEX PAT. These prosthesis elements generally have the strength or thickness a biologically compatible, non-absorbable through broken tissue and are through a tricotage process with blocked meshes that have the shape of a square or Rectangle produced. The texture of one Fabric gives it a relatively high tensile strength, namely in two roughly perpendicular directions exercises.
The thread or threads that make up this fabric at least have sometimes a large number of individual fine threads or threads that can be intertwined.
Bisher hat man sich nicht prinzipiell mit den Charakteri stika und mit den mechanischen Eigenschaften dieser parietalen Prothesen beschäftigt, und zwar dahingehend, um Prothesenelemente zu erhalten, die in ihrer Gesamtheit den folgenden Anforderungen entsprechen, nämlichSo far, one has not dealt with the characters in principle stika and with the mechanical properties of this parietal Prostheses employed, namely to prosthesis elements to obtain, in its entirety, the following requirements correspond, namely
- - daß diese ausreichend haltbar und widerstandsfähig sind, damit sie als wirksame Schutzwand wirken können, und insbesondere jegliche spätere Nachbehandlungen nach einer chirurgischen Behandlung von Hernien (Eingeweidenbrüchen) ausschließen;- that they are sufficiently durable and resistant, so that they can act as an effective protective wall, and especially any subsequent treatments after one surgical treatment of hernias (hernias) exclude;
- - daß sie eine gewisse Steifigkeit oder ein "Formengedächtnis" aufweisen, damit diese einfach gehandhabt und einfach an Ort und Stelle gebracht werden können, insbesondere durch eine Zölioskopie.- that they have a certain stiffness or a "shape memory" so that they are easy to handle and easy to use Can be brought in place, especially by a celioscopy.
In der Praxis werden solche guten mechanischen Eigenschaften durch unterschiedliche Standardtests gemessen und bewertet, bspw. durchIn practice, such good mechanical properties measured and evaluated by different standard tests, e.g. by
- - einen Dehnungstest;- a stretch test;
- - einen Einreißfestigkeitstest;- a tear resistance test;
- - einen Berstfestigkeitstest.- a burst strength test.
Solche ausreichenden mechanischen Eigenschaften können durch die Auswahl und die Bewerkstelligung von zumindest einigen der folgenden physiko-mechanischen Charakteristika erhalten werden, nämlichSuch sufficient mechanical properties can through the selection and accomplishment of at least some the following physico-mechanical characteristics become, namely
- - durch einen oder mehrere das Gewebe aufbauende Fäden, die durch Zusammenfügen von an sich schon widerstandsfähigen Einzelfäden oder -fädchen erhalten werden, bspw. aus Polyester, mit einer relativ großen längenbezogenen Masse;- By one or more threads that make up the fabric by joining together already resilient Individual threads or threads are obtained, for example Polyester, with a relatively large linear mass;
- - durch eine relativ dichte Textur, die zu einem relativ ausgeprägten spezifischen Flächengewicht führt;- by a relatively dense texture that leads to a relative pronounced specific weight per unit area;
For each chain and / or weft a relatively large number of these building threads, etc.
From a biological point of view, such a selection leads to that the amount of material or foreign matter contained in the Organism of the patient to be treated is increased becomes, and this leads to the occurrence of defense cracks tion, inflammation or seroma, following such ent ignitions, is increased.
It goes without saying that with a biological compatible and resorbable tissue of this previously mentioned problem does not arise. Therefore describes the document FR-A-2 635 966 a woven fabric known as parietal or visceral prosthesis element can be used. The prosthesis consists of a material that is resorbable by the body the problem of defense reactions can be excluded and causing inflammation caused by the presence of a tissue caused in the human body to be degraded can. The resorbable material that makes up the woven fabric is a mixture of filaments from at least two resorbable polymers that have different melting points have, and the stitches are by fusing the lot fibrous and woven structure blocked. In this document however, the aforementioned problems are for non-absorbable Prosthetic elements in no way considered.
It is an object of the present invention to address these problems to overcome. So it is especially the task of the present Invention to create a parietal prosthetic element that is not absorbable by the organism into which it is implanted is tolerated better, but the satisfactory has mechanical properties and maintains them.
According to the invention the object is achieved in that the Mesh the openwork fabric one area each in the area have from 1 mm² to 70 mm², and that the basis weight of the openwork fabric is less than 90 g / m².
It could be shown that the task was solved by a parietal prosthesis element that meets these two conditions fulfilled, solved.
The area of the mesh is preferably in the range of 10 mm² to 5 mm², most preferably around 10 mm². At a Mesh area of less than 1 mm², the prosthesis element relative opaque and it becomes impossible for the surgeon to visually its operational processes through the prosthetic element to watch through. With a mesh area larger than 70 mm² is the mechanical resistance of the prosthesis elements weakened, and the prosthetic element does not become easy and colonized rapidly, for example by fibroplastic cells.
With a basis weight of the openwork fabric of less than 90 g / m² and preferably less than or equal to 60 g / m² could be shown that an inventive parietal prosthesis element has a very good biological acceptance shows.
In a further embodiment of the invention, this is Ratio between the full and visible total area of the or the threads forming the openwork fabric and the total empty area of the mesh of the fabric under 0.5, preferably less than 0.4, and most preferably about 0.32.
Defense values of 0.5 are set Organism versus implantation of the parietal prosthesis element fest.
The mass per unit length is preferably or of the building threads at least 50 dtex.
This measure has the advantage that the mechanical The strength of the whole of the openwork fabric is preserved remains.
Other advantageous configurations are the others See subclaims.
It is understood that the above and the Features to be explained below not only in the combinations specified in each case, but also in others Combinations or alone can be used without the To leave the scope of the present invention.
Es zeigen:The invention is described below in connection with the accompanying drawings described and explained. It demonstrate:
Fig. 1 eine perspektivische Draufsicht eines erfindungs gemäßen parietalen Prothesenelements, Fig. 1 is a perspective plan view of a modern fiction, parietal prosthetic element,
Fig. 2 in vergrößertem Maßstab einen Ausschnitt des parietalen Prothesenelementes von Fig. 1, und Fig. 2 on an enlarged scale a section of the parietal prosthesis element of Fig. 1, and
Fig. 3 is a thread building element shown in Fig. 1 and 2 prosthesis.
A parietal prosthesis element 1 according to the invention has at least one layer of a fabric 2 , which in the present case, as will be described below, is obtained by weaving, namely an openwork fabric 2 which has square and immovable meshes 3 .
The fabric 2 is constructed from individual filaments 7, the the case 16 is built from Einzelfädchen 6 in turn of a plurality of individual filaments 6, in the present, which are intertwined, as is evident from FIG. 3. Each individual thread 6 consists of a polymer fiber which is resistant as such and which is not resorbable by the body, but is biocompatible, which consists, for example, of a polyester. The number of the individual threads 6 and the diameter of each individual thread 6 of one of the threads 7 making up the fabric 2 is such that the ratio between the developed outer total surface of the threads 6 and the apparent outer surface of the individual thread 7 , which is formed by the threads 6 is at least six, preferably at least about seven, per unit length. The outer diameter of each thread 6 is preferably in the range of 11 μm. The aforementioned ratio offers a considerable cellular contact surface per unit area of tissue 2 , which favors the colonization of the parietal prosthesis element.
The individual threads 7 which make up the fabric 2 are connected to one another by weaving, in such a way that weft threads 4 and warp threads 5 are formed. Each warp thread 5 has a multiplicity of individual threads 7 , in the present case six such threads 7 , which are entangled with one another and which are braided around the weft threads 4 , which in turn have a multiplicity of individual threads 7 , in the present case three such threads, and who are not involved. The latticework of the warp threads 5 and the weft threads 4 defines the individual square stitches 3 , which are blocked due to the weave described above. As a result, a texture is obtained which gives the fabric a pronounced tensile strength, in two directions which are approximately perpendicular to one another and correspond to the longitudinal extension of the warp threads 5 and the weft threads 4 .
Die Parameterwahl der Webart führt zu den folgenden physikalischen Eigenschaften des durchbrochenen Gewebes:The choice of parameters of the weave leads to the following physical properties of the openwork fabric:
- - Die Maschen 3 des Gewebes 2 weisen jeweils eine Fläche von etwa 10 mm² auf;- The stitches 3 of the fabric 2 each have an area of about 10 mm²;
- - das Verhältnis zwischen der vollen und sichtbaren Gesamt fläche der Gesamtheit der das Gewebe 2 aufbauenden Fäden 7, aufgeteilt in die Kettfäden 5 und in die Schußfäden 4, und der gesamten Leerfläche der Maschen 3 des Gewebes 2 liegt bei etwa 0,32;- The ratio between the full and visible total area of the entirety of the threads 7 constituting the fabric 2 , divided into the warp threads 5 and the weft threads 4 , and the total empty area of the stitches 3 of the fabric 2 is about 0.32;
- - aufgrund dieser Auswahl liegt das Flächengewicht des Gewebes 2 im Bereich von 56 g/m²;- Based on this selection, the basis weight of the fabric 2 is in the range of 56 g / m²;
- - die erhaltene längenbezogene Masse der das Gewebe 2 aufbauenden Fäden 7 liegt im Bereich von 50 dtex.- The obtained length-related mass of the threads 7 building the fabric 2 is in the range of 50 dtex.
Because of these properties you get how that from the enclosed table is visible, mechanical properties, which correspond to the values for a parietal prosthesis element are necessary, and in particular close to the values common parietal prostheses, where one is the same achieved a lighter parietal prosthesis element in time, the also better biological compatibility with the Organism in which it is implanted.
Es können auch noch die nachfolgenden Varianten der vorliegenden Erfindung bewerkstelligt werden, nämlichThe following variants of the present invention are accomplished, namely
- - das durchbrochene Gewebe 2 kann gewirkt sein, und zwar mit blockierten Maschen, anstatt durch einen Webvorgang erhalten zu werden;- The openwork fabric 2 can be knitted, with blocked stitches instead of being obtained by a weaving process;
- - die einzelnen Fädchen können aus Polypropylen anstatt aus Polyester bestehen; - The individual threads can be made of polypropylene instead of Polyester exist;
The different individual threads 6 or the fibers 7 that make up the tissue, or else the perforated tissue 2, can be coated with a biologically compatible material that promotes cell colonization, for example they can be coated with collagen.
