Abstract

External to the patient, the high voltage generator and controller form a treatment application unit. This includes an interface (12) for a detachable catheter line (3). Data relevant to patient and therapy are entered into the controller. Preferred Features: Data is input into a computer unit (6) connected to the application unit. The control unit feeds, positions and adjusts the catheter head (4) using a step motor and spooling system. A measurement unit is connected, to determine position and/or displacement of the catheter line. An opto-electronic measurement unit is integrated. A coolant reservoir (10) is included in the applicator unit, for the X-ray source. A safety system inactivates the high voltage, should coolant flow cease. A further safety system against high voltage breakdown is included. Shutdown occurs should the voltage or current profile deviate from a norm. A high voltage plug at the HV generator is shrouded, to prevent the danger of inadvertent manual contact. The controller assists planning of treatment, comparing e.g. vessel diameter and stenosis length with values stored in memory, to calculate dose and duration. This facility is integrated into the applicator unit, which is mobile.

Description

The invention relates to a system for intracorporeal, esp special intraluminal X-ray therapy that takes you into the Includes a catheter insertable inside a patient, which is an x-ray source with miniaturized x-rays contains tube, which on an object to be treated place is placeable, and which continues to have a Zu cable can be connected to the X-ray source High voltage generator and a control unit for control tion of the high-voltage generator and possibly other existing facilities.

Percutaneous translumi has been used to treat stenoses Coronary angioplasty (PTCA). A disadvantage of However, this method is a degree of restenosis of 30 to 50%. This can be reduced to about 28% by using stents be adorned; However, a stenosis-free Zu is sought stood, at least over a longer time interval.

In order to achieve an effective reduction in restenosis one has so far been using high-energy radioactive radiation sources len (gamma or beta rays) treated. Alternative to egg ner such radiation with radioactive preparations is one Irradiation with X-rays possible.

The use of a miniaturized X-ray is problematic tube in that one dose is usually used for therapy  must be applied in the vicinity of a tissue-damaging Limit dose is. The dose must therefore be local as well who is also applied and documented extremely precisely in terms of time the. The X-ray tube can only do so be a few millimeters long through the narrow, curved Coronary vessels to reach a stenosis region. With stenosis lengths of typically 2 cm is a quick, precise Driving down the stenosis region is necessary to get a homogeneous dose to ensure distribution and tissue destruction avoid. This is all the more important since, as a rule radial x-ray emission no homogeneous intensity distribution of radiation over the length of the tube is to be expected.

The aim of the present invention is therefore to provide a Practice easy-to-use system to indicate to the doctor opens up the possibility of intracorporeal or intralu minimal X-ray therapy with sufficient security for pati ent and personnel to be carried out effectively.

The system according to the invention contains an applicator which can include a number of modules, e.g. B. Positionie tion of the catheter to control the high-voltage generator tors and for the supply of a cooling liquid for cooling the X-ray tube, or the area surrounding the X-ray tube. The tax unit takes on control and function control tasks. These include a. the exact positioning and displacement the catheter when traveling a longer stenosis route or repositioning if the therapy is under or must be canceled. The control unit also takes over the task of activating the high voltage generator so that At each position, a previously defined in a planning tool put dose is applied. The control unit takes over finally the control of a module for cooling the beam lenquelle or the surrounding area, if such Cooling is available.

The dose to be applied temporally and spatially is given partly taken over by a planning tool which the therapy based on the required dose and the pa customer-specific conditions, such as vessel diameter and Stenosis length plans. This planning tool is preferably in a computer integrated in the applicator. He follows planning in an external computer, e.g. B. in one of the Ap plicator spatially separated PC, is in the applicator a corresponding interface between the control unit and the external planning tool.

The position values of the catheter are stored in the control unit or the radiation source via appropriate measuring sensors and Comparison with target values recorded and checked. At motori shear delivery of the catheter is advantageously that of a Actuator transmitted thrust from the control unit supervised.

The positioning of the catheter can be done with the help of an automa table feed unit to which the catheter line is coupled directly above via an intermediate piece. In the Feed unit can advantageously be a Act stepper motor with sufficient accuracy (0.5 to 5 mm) the catheter line and thus the rays source introduced into the vessel and on the to be treated Object location can be placed. In the event that peri phere vessels are to be treated that a comparative wise long shift (up to 100 cm), it is Appropriately, a device for storage in the applicator the catheter line. Such a facility can e.g. B. a cable drum equipped with restoring torque be. For position detection and control of the exact current The catheter position can advantageously be in the applicator optoelectronic or mechanical measuring device be there. This can for example by on the Kathe Terleitung applied markings, which with egg ner light source and a corresponding sensor  (Light barrier) interact. It can also be used for län stenoses with multiple x-ray tube positioning or also after repositioning during therapy break the tube.

For quick interruption of therapy, for example in the acute emergency, it may be beneficial to use the applicator or several manually operated switching options watch. In addition to an emergency stop button with which by switching off therapy is stopped immediately can, there may also be a pause button, with taking the therapy program for a certain time can be broken. The current position and the one here In this case, the dose already applied is stored on the computer saves. The catheter with the tube then becomes a defi nated piece withdrawn from the site to one unimpeded blood flow in the vessel to be treated possible. If the therapy can be continued, it can can be initiated by pressing another key. The The tube is positioned at the previous position and the remaining therapy according to the one determined in the computer Residual dose carried out.

The required operating parameters, such as voltage, current, Radiation, and possibly therapy or residual therapy times can advantageously on a display either on Applicator or displayed on the computer connected to it become.

The applicator also contains various safety devices conditions that protect both the safety of the patient and the patient Ensure or improve personnel. One of the measures can advantageously be to ensure that the high voltage can only be switched on if the Plug connection to the catheter is closed. In addition, additional fuses are installed, which the personnel in the Protect operations. A simple, beneficial security measure  Assumption can be in the area of the connector To provide a protective tube that surrounds the connector and This ensures that accidental touching is avoided that will. The length of the protective tube is such that the plug connection over the entire adjustment range Positioning or repositioning within the pipe can move.

Other security measures may include chip arcing on the plug, in the supply cable, on the con cycle between the tube and catheter or in the tube to be detected even by sensors known per se. in the In the event of a fault, the high voltage generator is removed from the control unit deactivated immediately. The same applies to others, on Components to be coupled to the catheter, e.g. B. a pump for Flushing or cooling the radiation source or the X-ray tube. The X-ray tube temperature can also be determined by detection so influenced that they are on one for the patient can be kept sufficiently low.

Fig. 1 shows an overall view of the system according to the Invention for intraluminal X-ray therapy. The system comprises, in accordance with a particularly advantageous embodiment, a preferably mobile carrying device 1 with a housing 2 for receiving an applicator which will be explained in more detail later, a catheter line 3 connected to the applicator housing with a catheter 4 located at the free end with integrated X-ray source 5 and a computing unit 6 , which is designed here as a PC. The Applika gate housing 2 is supported by a stand 7 with adjustable interpretation gertragarm 8, are performed in the various, supplies leading to the applicator supply lines, in particular a communication line 9 and a stop Erten from the stand ge 7 liquid container 10 outgoing Flüssigkeitslei tung 11, via the coolant for cooling the X-ray source can be supplied to the catheter.

The applicator housing 2 contains on the front a connec tion 12 for the detachable connection of the catheter line 3 , a display 13 for displaying various, for therapy necessary input and / or status data and an emergency switch 14 for rapid interruption of therapy in the event of a dangerous situation .

Fig. 2 shows a schematic representation of the construction of the catheter 4 with integrated X-ray source 5 . The x-ray source 5 includes a miniature x-ray tube 15 , which outputs a power of about 1 to 5 watts and is supplied by a high voltage generator 16 arranged in the applicator housing 2 with high voltage in the range between 0 and 30 kV. The structure of the X-ray tube 15 is known per se and is described, for example, in WO-97/07 740 in various variants. At a suitable point of the X-ray source 5 , preferably on the outer surface of the radiation source, a temperature sensor 17 is arranged, which is connected via a measuring line 18 to a control unit 19 . The temperature sensor 17 detects the temperature at the radiation source or in the immediate vicinity of the radiation source and emits a corresponding signal to the control unit. If he reaches a critical temperature, which could possibly lead to tissue damage, there is an optical and / or acoustic display via indicators 24 on the ap plicator, or the control unit causes the high-voltage generator 16 and thus the X-ray tube 15 to be switched off .

Fig. 3 shows a schematic representation of those elements which are combined in the preferred embodiment to form an applicator. The applicator 20 accordingly comprises the already mentioned high-voltage generator 16 , the control unit 19 , also already mentioned, a drive unit 21 for the catheter line 3 , a Pumpeinrich device 22 for conveying the cooling liquid in the liquid container 10 , a positioning device 23 , which serves to to place the catheter exactly at the object to be treated, as well as various safety devices that serve to increase the safety of the patient and the operating personnel.

One of the safety devices is that the parts 25 and 26 of the plug connection 12 are guided in a protective tube 27 arranged on the door housing 2 , so that the plug connection is surrounded by the protective tube 27 over the entire displacement path. The plug connection 12 is thus always secured against unintentional touching. Another security measure consists in mechanically securing the plug connection 12 , for example by means of a bracket 29 , the holding function of which is additionally checked electronically, with a signal for switching off the high-voltage generator 16 being given to the control unit 19 in the event of a disconnection via a control line 28 . This measure ensures that high voltage can only be switched on if the plug connection to the catheter is securely established. The fact that the connector 12 always moves during positioning in the protective tube 27 , the staff is protected during operation from possible flashovers on the connector. The registration of a rollover on the connector 12 or in the catheter line or at the contact point between the catheter and the X-ray tube or in the tube itself can be by means of measuring sensors known per se, for. B. by means of so-called Rogowski coils, or by known current / voltage measurements using shunts.

In order to be able to place the catheter exactly at the object site to be treated, an adjusting and positioning device is provided which, on the one hand, consists of the drive unit 21 and the positioning device 23 . At the drive unit 21 includes a stepper motor 30 , which is activated by the control unit 19 and a drive roller with winding device 31st The catheter line 3 contains a marking 32 over a certain distance, which corresponds to the adjustment path of the catheter, which interacts with an optical sensor 33 . The optical sensor can e.g. B. be a (reflex) light barrier. The optical sensor 33 sends a corresponding position signal to the control unit 19 .

As already mentioned, the applicator 20 contains means for cooling the X-ray source or the adjacent surroundings of the radiation source, so as to avoid an inadmissibly high heating of the therapy area. The pump device 22 activated by the control unit 19 conveys the coolant into the catheter line 3 from the liquid container 10 , which may contain, for example, saline solution or another suitable coolant, via a feed line 34 . As shown in FIG. 3, the coolant can be supplied externally to the applicator housing. A supply within the applicator housing in the area of the drive roller 31 is also conceivable, where the high-voltage lines and the measuring line 18 from the temperature sensor 17 ( FIG. 2) are introduced into the catheter line.

By supplying coolant, the heat development in the area of the radiation source can be kept at a low level for the patient. It is also advantageous here to provide a corresponding coolant flow detecting sensor 35, the event of failure or reduced coolant supply effected via a control line 36 to shut down the high voltage generator and thus prevents an increase in temperature.

With reference to Fig. 4, a further important point of the OF INVENTION system will be explained to the invention. The applicator contains a planning tool 37 , which plans the therapeutic measures necessary for the special therapy case, such as dose and radiation duration and, if applicable, the advancement speed of the catheter on the basis of the existing vessel diameter and the length of the stenosis. This planning tool can preferably be integrated in the ap plicator 20 ; conceivable and within the scope of the inven tion it is also possible to integrate the planning tool, for example in an external computer, using the example of FIG. 1, in the computer 6 . The planning tool 37 contains a computer program with a number of patient-specific data stored there, which, for. B. different vessel diameters, under different long stenoses and the applicable radiation dose and radiation duration. The current patient-specific data are entered into the planning tool via data entry means 38 .

An advantageous variant can be that the pati duck individual stenosis course previously z. B. by IVUS (Intra-vascular ultrasound) or by X-rays is reconstructed and the data calculated from it in the Planning tool.

The existing computer automatically calculates the dose and radiation duration to be used in the special case, as well as the feed speed for the catheter from the data stored in the memory. These values are then fed to the control unit 19 , which specifies the radiation dose and the duty cycle to the high-voltage generator 16 . Furthermore, the control unit 19 gives the drive and positioning device 23 corresponding control signals for automatic advancement of the catheter during therapy.

Die Dateneingabe erfolgt dann über die Tastatur des Rechners 6.If planning is to be carried out externally on a computer, for example on the computer denoted by 6 in FIG. 1, an appropriate interface between the control unit and the external planning tool must be provided in the applicator. The data is then entered via the keyboard of the computer 6 .

As already mentioned, the control unit 19 contains control tasks, in particular via the position of the x-ray tube, and compares the position with predefinable setpoints. A control of the thrust force transmitted from the drive unit 21 onto the catheter line and a control of all existing safety devices enables the Immediately switch off the radiation source in the event of dangers (high-voltage flashovers, excessive temperature at the radiation source, insufficient or lack of cooling, etc.).

The above-described feed unit positions the catheter with sufficient accuracy, the length of the diff depending on the application, between 0.5 cm and 100 cm can.

The operating parameters, such as voltage, current, radiation intensity, are shown on the display 13 , moreover, it may be advantageous to also display the remaining therapy time calculated in the planning tool. The entire program worked out by the planning tool or the program sequence can be recorded in a suitable manner in a documentation 39 .

The manual input means 40 can include not only the emergency stop button 14 , but also include a pause switch option with which the therapy can be interrupted, the current position of the catheter and the dose already applied up to that point being stored. If the therapy can be continued, a further switching element (continue button) is actuated, with which the repositioning of the catheter and thus the radiation source to the previous object location is then initiated. The rest of the therapy can then be carried out. In the control device, a possibility can be provided for withdrawing the catheter a defined distance when the therapy is interrupted, so as to enable unimpeded blood flow in the vessel to be treated.

Other safety devices can consist in that the feed force with which the stepper motor applies the catheter drives, is measured by a force sensor and with pre given setpoints or by measuring the feed rate speed is compared.

In conclusion, it should be noted that the proposed System not only for intraluminal X-ray therapy, but also generally for intracorporeal x-ray therapy, for example for the therapy of joint clefts or the like, successfully can be used.
