Abstract

The breathing cycle of patients (I) in the form of the frequency, period and timing of pauses during breathing while asleep are monitored together with EEG measurements (II, III). Both sets of data are stored in memory and are sampled and processed using cluster analysis (V) to determine if the patients are in a high risk group.

Description

The invention relates to a method for evaluating disorders with the Respiratory regulation in premature and newborn related Measurement data. This data evaluation is intended to pose an individual risk due to a sudden unexpected infant death in immature and mature Newborns as well as young infants can be recognized. Such The patient is generally classified into one appropriate apnea risk group classified. Beyond that it is Procedure suitable in principle, sleep-related breathing regulation disorders other causes.

The sudden unexpected infant death (international name: Sudden Infant Death Syndrome) is the most common in the first year of life Cause of death in all industrial countries. The frequency is in countries with competent medical statistics between 1 to 3 deaths per 1000 Live births, so that for Germany with about 1000 infant deaths must be calculated per year.

A causal treatment is not known. So far the only way to Prevention is the therapeutic influence in practice such catastrophic events through timely detection of at-risk infants (risk diagnostics). These high-risk infants need careful long-term monitoring to identify life-threatening events before a fatal end, d. H. in a phase of resuscitation is possible to discover.

Sleep-related breathing disorders of other causes are caused by a variety of chronic childhood diseases and / or through Medication evoked. As a rule, their severity does not reach any life-threatening dimensions, however, sleep quality and thus Quality of life significantly reduced.

Risk detection is based on the current state of the art through polysomnography, for which various commercial devices are used To be available. The newborn, or the infant must be in one special bedroom are spent. 2-6 (8) EEG- Electrodes, three electrodes for cardiorespirography, a nasal flow sensor, EMG electrodes on the eyes and chin and an oxygen sensor on the foot attached and stably attached. A sleep duration of at least two hours, better in older infants by at least four hours Prerequisite for a meaningful examination.

Risk detection using polysomnography is based on a complex of parameters:

  • - Es werden Schlafphasen, Atemregulation, Häufigkeit, Typ und Dauer von Atemstillständen als entscheidende Größen gemessen.- There are sleep phases, breathing regulation, frequency, type and duration of Breathing stops were measured as decisive parameters.
To identify and evaluate these parameters are simultaneous Measurements of additional vegetative parameters such as heart rate, Oxygen saturation, electroencephalogram as well as eye, chin and Body movements required.

The measurement methods of polysomnography are complex, expensive and therefore tied to special sleep laboratories.

Die Mehrzahl der kommerziellen Geräte bietet keine Optionen für Messungen im Säuglingsalter an.It must also be noted that this methodology is primarily for the Adult polysomnography to diagnose sleep disorders, sleep apnea syndrome in particular. Your application in newborns is due to numerous age peculiarities  Limitations of measurement and diagnostic reliability connected. The majority of commercial devices do not offer options for Measurements in infancy.

Such restrictions result from age-specific physical and psychological peculiarities of newborns and infants:

The application of a large number of measuring electrodes is annoying for the little patient.
  • - Es werden Bewegungsartefakte, Störungen des Elektrodenkontaktes durch Schwitzen und Schlafstörungen wie Einschlafverzögerungen und Aufwachphasen provoziert.- Movement artifacts, faults in the electrode contact are caused by Sweating and sleep disorders such as delayed sleep and Wake up provokes.
A sufficiently artifact-free measurement and the implementation of a long enough sleep is therefore difficult.
Meßwiederholungen sind manchmal unumgänglich.- As a result, the evaluation is often based only on time segments of the Sleep. Repeat measurements are sometimes unavoidable.

There are also problems with a stable mother-child relationship during the offer a long measurement period.

As a result of sleep analysis for childhood patients with the current state of the art frequency and duration of Breathing stops - separated after active and quiet sleep phase - determined and according to their causes in central and obstructive forms differentiated. These events are summed up in different indices, their quantitative expression empirically a degree of expression of the SIDS Risk is assigned. Apart from the basic clinical Problem of assigning quantitative sleep parameters to SIDS Due to the measurement technology, there are high scattering of the indices and misinterpretation is a major clinical disadvantage.

It is known that by computer analysis one in uniform Periods divided EEG recording into physiological states  can be concluded (e.g. US Pat. No. 5,154,180 to create a Sleep profile for adults). In doing so, the tape performance in certain frequency bands to a predefined category physiological condition classes.

Desweiteren ist bekannt, daß aus einer durch Fourier-Transformationen aufbereiteten Messung physiologischer Daten (EEG, EMG, EKG, Augenbewegung etc.) mit Hilfe eines neuronalen Netzwerks auf somatische Zustände eines Probanden geschlossen werden kann (US-PS 5 601 090).Furthermore, it is known that from a Fourier transform prepared measurement of physiological data (EEG, EMG, EKG, Eye movement etc.) with the help of a neural network to somatic States of a subject can be closed (US Pat. No. 5,601,090).

These methods have the disadvantage that physiological features are known must then be within the scope of a targeted pattern recognition can be used. However, it is not possible to Records of premature and newborn babies defined by a priori evaluate simple patterns. Rather, the EEG is a multitude individual, e.g. T. superimposed development-related signatures, so that it is not easy and at least very time consuming for that To read relevant features from the EEG. In practice must therefore be carried out for a medical assessment of apnea Risks a number of patients physically and mentally stressful EEG measurements carried out and their often hours-long recording evaluated visually by the doctor as well as with the recorded Breath regulation data and possibly other data are compared.

The invention is therefore based on the object, with the least possible Burden on the patient and with a minimum of necessary especially visual, monitoring and evaluation of data from the Polysomnography clear and objective clues for To get respiratory regulation disorders, which is a division of the patient into allow an apnea risk group and ultimately objective  Provide apnea-relevant evaluation criteria for the doctor.

B. K. Holthausen, Theory Bioscienc. (1998) 117, S. 18-31] abgebildet.According to the invention, the EEG record of the early or Newborns, in a manner known per se, in existing sleep laboratories is obtained, with the help of information from his as well Breathing regulation data usually recorded in the sleep laboratory characteristic apnea-relevant EEG feature vectors obtained and evaluated. The EEG- Characteristic vectors of the subject with the help of a self-referential and based on the EEG feature vectors of an existing patient pool trained neural network [e.g. B. K. Holthausen, Theory Bioscienc. (1998) 117, pp. 18-31].

With an EEG analysis that is comparatively low-impact for the patient only requires a lead channel from which EEG Recording apnea-relevant data obtained with the said Evaluation for the first time objective criteria for dividing the patient into allow apnea risk groups so that the attending physician does not necessarily - as before - purely subjective evaluations time-consuming visual recording evaluations with comparison u. U. a variety of data and / or data recording phases. The assessment and division into apnea risk groups is carried out exclusively on the basis of the apnea phase dependent EEG data, regardless of the frequency of breathing stops and other parameters, like maximum and mean apnea duration.

This procedure avoids a number of disadvantages of classic polysomnography:

Damit wird das Meßverfahren technisch einfacher und durch die geringere Belastung patientenfreundlicher.- Reducing the number of electrodes from 12 to 18 to 2 to 4 trades usual EEG electrodes. This makes the measuring process technical  easier and more patient-friendly due to the lower load.
The method allows the evaluation of data in conventional Sleep laboratories are recorded, but also allows Use outside of a specialized sleep laboratory. It is suitable for both general and pediatric practice.
This makes the invention particularly suitable for screening Risk children as well as for the follow-up of infants at risk predestined.
In addition, an examination in the home environment would be carried out the parents realizable.
For these reasons, the procedure is a useful addition to the classic sleep laboratory.

The new procedure enables an improved SIDS risk assessment objective criteria derived from data analysis. An advantage grows from the shown lower susceptibility to interference and artefact load of the simpler measuring technology. What is more important, however, is the fact that a Differentiation into inconspicuous and conspicuous patients takes place. The in the Polysomnography common classification using quantitative measures requires expert knowledge and is therefore only accessible to specialists who are on specialized in these areas. This limitation is for one wide practical use canceled with the inventive method. This makes the application outside of specialized laboratories too in terms of interpretation and evaluation.

The invention will now be described with reference to one in the drawing Embodiment will be explained in more detail.

From polysomnography there were records of 37 premature and newborn babies of different gestational ages and different degrees of respiratory regulation disorder (phase I of the schematic procedure in Fig. 1).

The post-conceptual age was between the 36th and 50th week of pregnancy. For all patients, comprehensive measurement data from the polysomnography were available for several hours of recording the respiratory regulation and a multi-channel EEG (phase II. Of the schematic procedure in FIG. 1). The EEG measurement data were divided into time slots before, during and after apnea and into apno-free time slots of five seconds each. Accordingly, four EEG data records were assigned to each patient's apnea and saved as computer-readable individual data records. These individual data sets were subjected to a spectral analysis known per se in the following frequency ranges: subdelta waves (σδ), delta waves (δ), theta waves (ϑ), alpha waves (α) and beta waves (β), which are divided into two sections (12.5 to 19 , 5 Hz and 19.5 to 25 Hz) were subdivided (phase III. Of the schematic process flow in FIG. 1).

Fig. 2).Four quantitative parameters were selected for further analysis of these stored individual data sets of the EEG measurement data: absolute and relative tape output and the absolute and relative variance. These sizes were calculated individually for the three time windows before, during and after apnea compared to the apnea-free time windows and for the six frequency bands mentioned above, so that a total of 18 parameters could be assigned to each size. With a suitable selection of the total of 72 parameters determined in this way, a link had to be established between a patient's EEG recording and his risk classification with regard to respiratory regulation disorders. For this purpose, a cluster analysis was carried out with the help of a neural network. The cluster analysis aimed for a two-dimensional representation (ie projection) of selected EEG parameters from the point of view that the parameters which originated from the EEG recording of a patient of a specific apnea risk group in each case in a narrowly defined area of the two-dimensional topological Mapping (ie a cluster). A successful cluster display then leads to a separation (ie separation) of the various apnea risk groups in the two-dimensional topological mapping of the parameters of the apnea-relevant EEG measurement data reduced by linking them to two output variables of the neural network (see FIG. 2).

For the evaluation per patient, M = 72 should therefore be spectral analysis determined parameters as apnea feature criteria semantically using a So-called evolution algorithm on K = 24 to be used for the evaluation Parameters are reduced, each a patient-dependent Form input vector for the neural network.

For this purpose, a neural feedforward network was used, the 24 inputs (only eight inputs K1-K8 of which are shown in FIG. 2 for reasons of clarity) for the 24 parameters selected as input vector per patient as well as two outputs N1, N2 as output vector for each patient for topological representation of the EEG characteristics. For every parameter K; The minimum and maximum values of each input vector were determined within an existing patient pool, so that the input values of the neural network could be normalized to the interval between 0 and 1.

An advantageous embodiment of the method was that neural network with originally stochastically chosen link adapted to the EEG measurement data by means of a self-referential learning process was [see K. Holthausen, Theory Bioscienc. (1998) 117, pp. 18-31]. This Variant of the procedure enabled on average a clearer Separation of the clusters.

The patient pool was divided into three apnea risk groups, with a distinction being made between normal patients (group 1), patients with frequent apneas (group 2) and patients with conspicuously prolonged apneas (group 3). In the exemplary embodiment according to FIG. 3, groups 2 and 3 were combined to form a group (here: cluster 2) for risk identification. The pre-structured neural network was adapted to the clinical grouping using an evolution algorithm. For this purpose, the currently selected K = 24 parameters for each patient were defined as input vectors for the neural network, with the two respective output values x and y of the neural network being assigned a coordinate (x and y) of a two-dimensional surface. Each patient (and therefore each input vector) was identified as a risk patient (belonging to cluster 2 by definition) or as a normal patient (belonging to cluster 1 by definition here). In the exemplary embodiment, the existing patient pool was divided into 15 normal and 22 risk patients. The mean values of the 15 and 22 x and y coordinates defined the centers of gravity of clusters 1 and 2.

After calculating the distance between the two cluster focal points one of the 24 selected parameters by a random one from the Total of 72 parameters selected new parameters replaced. In order to a component of the input vector was changed, whereby this Modification made for each patient (for each input vector) and said topological mapping through the neural network were repeated. As a rule, this led to a shift of the two Cluster focus. When the distance between the two increases The change in the input vector was discarded (i.e. again the previously selected 24 parameters as a component of the Input vector), otherwise it was retained.

This procedure was carried out iteratively, using it for the selection gave a maximum of 24.72 possibilities. After a few thousand steps the process converged; H. through a further selection the Do not further increase the distance between the two cluster focal points.

The neural network thus created a 24-dimensional projection of the a total of 72-dimensional EEG parameters for each patient. Separate Parameters such as the relative band power in the δ band could be represented several times in the input vector.

Fig. 3). It can be seen that risk patients and normal patients are each shown in opposite positions in the two-dimensional topological image, so that the degree of risk can be visualized immediately. In addition, the numerical distance from the cluster focus provides a quantitative statement for assessing this hazard (see Fig. 3).

However, it must be emphasized that the EEG feature category is not always clearly linked to a pathological respiratory regulation disorder. In the patient clustering shown in the drawing, two patients identified with (F) are identified as risk patients. This is a functional EEG immaturity, the characteristics of which match the characteristic EEG parameters of the risk group. The detection of such immaturity thus provides an additional feature for risk assessment. However, an assessment of the pathological situation requires further follow-up. In this context, the method for displaying the EEG parameters can advantageously be integrated into an expanded expert system. In Fig. 1, the essential process steps of risk detection and assessment are summarized.

The trained neural network can now be used for risk detection of previously unrecognized patients. For this purpose, the links of the neural network are stored in a computer-readable manner, and the 24 EEG parameters selected in the method are obtained from the EEG recording of a new patient. These are combined into an input vector and mapped onto the two-dimensional topological output surface by the stored neural network. The coordinates of this output vector can then be compared with the previously stored centers of gravity of clusters 1 and 2. It is advantageous to simultaneously map the coordinates of the output vectors of the patients previously analyzed in the method in order to display the scatter within the clusters. In Fig. 3, the data of a new patient near are imaged on the center of the cluster of normal patient (white circle). The network can thus be used for risk assessment after only one training session. A further advantageous embodiment of the method consists in that the training data are supplemented by the EEG parameters of the new patients at regular time intervals and the above-described method of cluster separation is repeated.

The cluster separation shown above is based on data from a multi-channel EEG recording. However, with the EEG parameters from the Recording of only a single channel already unique cluster Achieve separations. For this reason, the apnea phase-relevant Individual data records exclusively from the recording of a single EEG Lead channel are generated, which is a minimum patient load great reliability in the recording, as in the event of failure an electrode (e.g. by demolition) of a two-channel recording.

In addition to risk detection based on a single investigation interested in the long-term observation of individual patients. For this, the Methods are designed so that the output vectors, for example two successive measurements are shown simultaneously and such a change of direction with regard to the two cluster focal points is determined. In this way it can be determined how the EEG characteristics are after taking pharmaceuticals or by endogenous or exogenous Change stimulations. Such a follow-up could be Support treatment sustainably.
