Abstract
Description
The invention relates to resorbable anastomosis protection, especially in neurosurgery and vascular surgery can be applied.
An anastomosis is a surgical technique, at of the two separate organ ends to be put together again to form a functioning organ. The operating result nis is called anastomosis. Anastomoses are in the Usually applied when an organ segment has been removed and the two ends or stumps of the residual organ produced thereby to be connected again or if one through separate organ should be made functional again. she play for example in bowel surgery, in vascular surgery and a role in nerve surgery.
Because nerve or vascular ends, using an anastomosis with should be connected to each other, often a small size special skill is required for such an operation such. Care must also be taken to prevent injuries to the to avoid thin organ ends and a safe healing process to ensure.
It is an object of the invention to provide a way to creating anastomoses in nerve surgery and in the To facilitate vascular surgery.
This task is solved by a resorbable Anastomo protection with the features of claim 1 and a system with the features of claim 9, resulting from such an Anasto mosquito protection and an insertion device. Beneficial Refinements result from the subclaims.
The resorbable anastomosis protection according to the invention has tube which can be pushed over nerve or vessel ends, at the end of which At least one retaining wing is attached to the outside. Preferential the tube is wise closed-walled.
The tube pushed over an anastomosis site serves as Protection against external influences. If the anastomosis associated nerve or vascular ends not directly with each other can be connected, but initially there is a gap remains (for example, to avoid excessive tension), the tube located above this defect acts as Guardrail for the growth of organ ends inside the Tubes can grow together unhindered until they come together connect yourself with each other and again a functional one Organ arises. When the wall of the tube is closed, sprouting of surrounding tissue is reliably prevented different. Furthermore, the tube quickly stops rest after the operation bleeding at the anastomotic site.
The at least one holding wing facilitates handling the anastomosis protection, which is safely attached to the holding wing can be gripped and placed with tweezers. In addition, the holding wing can increase the stability of the tube hen, especially if it is thin-walled and flexible. The Hal teflügel also makes it easier to fix the anastomosis protection during the operation.
In a preferred embodiment, the anastomosis protection made from a cuttable material. In this case the tube slightly to the required during the operation Length to be cut. It can also be advantageous if the retaining wing (s) change into one during the operation cut special shape or have it cut if necessary.
Preferably, the at least one retaining wing is sewable, i.e. H. he can be pierced with a surgical needle to remove the Holding wings for fixing the anastomosis protection, for example to be sewn to the surrounding tissue with single button sutures. If the wing is sewable in this way, the thread should Tear-out strength of the material used is sufficiently high his. In the case of harder material, sewability can be achieved by opening Achieve openings in the wing through which a thread is passed can be.
The inside diameter of the tube should be chosen so that the tube lies loosely around the vessel or nerve. With Ana ostomoses, in which the nerve or vessel ends are together should also be sewn inside the tube there is sufficient space to accommodate the sutures or it should be flexible enough to fit its shape to adapt the projecting suture material.
In a preferred embodiment of the invention, the tube chen formed from a piece of film, in which two opposite put end areas with the inner sides against each other Formation of a longitudinal seam of the tube and a retaining wing are interconnected. In another embodiment the tube from two pieces of film opposite each other formed, in which on two opposite sides of each lige end portion of the inside of a piece of film on the opposite end of the inside of the other slides pieces to form a longitudinal seam of the tube and one Holding wing rests and is connected. With the latter Design so there are two retaining wings. The two Pieces of film can initially be separate and at the manufacturer anastomosis protection can be arranged one above the other, or they can be formed by folding over a larger sheet are so that the correspond with the finished anastomosis protection fold line on the outer edge of one of the retaining wings. In both configurations, end regions lie against one another the piece of film or pieces of film preferably together glued or welded. These designs are inexpensive producible and ensure a secure attachment of the Hal teflügels or the holding wing on the tube, because the or Retaining wings are quasi integrally formed with the tube.
The anastomosis protection consists of resorbable material, so no foreign body during or after the healing process of the material remains and could lead to malfunctions. Before the anastomosis protection also has poly-p-dioxanone. Ande Re-known absorbable materials are also used bar, for example copolymers of glycolide and lactide.
To the onset of anastomosis protection during surgery An insertion device with an auxiliary tube can facilitate this Chen can be used. The auxiliary tube is longer than the tube protection of the anastomosis and through the tube of the anasto slide protection. Through the interior of the auxiliary tube is a thread that runs on both ends of the auxiliary tube protrudes and at one of its ends into a noose is shaped. The auxiliary tube is preferably relatively stiff or rigid and made of plastic. The introducer considerably facilitates the insertion of the anastomosis protection; how it is applied, goes from the following description of the Embodiments emerge.
Die Zeichnungen zeigen in In the following, the invention is illustrated by means of exemplary embodiments explained in more detail. The drawings show in

In Fig. 1, an anastomosis protection 1 is shown in perspective view. The anastomosis protector 1 has a tube 2 which is open at both ends and is cylindrical in the exemplary embodiment. On the outside 3 of the Röhr chens 2 , two retaining wings 4 and 6 are attached.
The tube 2 is dimensioned such that it can be pushed over nerve or vessel ends, whereby it should preferably lie loosely around the nerve or the vessel. For a specific application, the dimensions of the tube 2 depend on the size of the organ ends on which an anastomosis is to be performed. Typical lengths for the tube 2 are in the range from 1 mm to 100 mm, typical lumen diameters (ie inner diameters) in the range from 0.2 mm to 10 mm. Suitable wall thicknesses range, for example, from 10 µm to 50 µm. The tube 2 should be so stable that it largely retains its circular lumen in the exemplary embodiment if no or only slight external forces act on it.
Fig. 2 shows the anastomosis protection 1 of Fig. 1 in cross section. In the exemplary embodiment, the anastomosis protector 1 is produced from two pieces of film, namely a first piece of film 10 with the inside 11 and a second piece of film 12 with the inside 13 . The first film piece 10 and the second film piece 12 lie opposite one another. Two ten pages most opposite (namely, in the view of FIG. 2 left and right) of the respective end portion is the inner side 11 of the first sheet piece 10 at the opposite end region of the inner side 13 of the second sheet 12 and is thus ver connected (preferably glued or welded). In this way, the tube 2 is formed by the central regions of the first film piece 10 and the second film piece 12 , while two longitudinal seams on the tube 2 and the retaining wings 4 and 6 are formed by the end regions. It is also conceivable to use a larger film instead of two separate film pieces 10 and 12 , which is folded by 180 ° along a fold line, as a result of which a first film piece and a second film piece are defined. In this case, the fold line in the view according to FIG. 2 would be at the extreme left or right end.
FIG. 3 shows one possibility of how the anastomosis protection 1 shown in FIG. 2 can be produced. For this purpose, the first piece of film 10 and the second piece of film 12 are placed around a tall tube 16 and welded to one another in their end regions, ie in the region of the holding wings 4 and 6 . A heat treatment can ensure that the tube 2 retains its shape even after the metal tube 16 has been pulled out.
In Fig. 4, an anastomosis protection 1 'is shown in cross section as a second embodiment, which is a variant of the anastomosis protection 1 . The anastomosis protector 1 'has a tube 2 ' with the outside 3 'and has only one holding wing 4 '. As can be seen from FIG. 4, the anastomosis protector 1 'is formed from a piece of film 18 with the inside 19 , so that two opposite end regions of the piece of film 18 are connected to one another with the inside sides to form the longitudinal seam of the tube 2 ' and the holding wing 4 ' (preferably by gluing or welding). The anastomosis protection 1 'can be produced analogously to the manner illustrated in FIG. 3.
In these embodiments, the anastomosis protection consists of two pieces of film 10 and 12 or one piece of film 18 . However, configurations are also conceivable in which the tube is produced from tubular starting material and the holding wing or wings are then attached to the outside of the tube. If necessary, more than two holding wings can be used.
Many different shapes are conceivable for the holding wings. The Fig. 5, 6 and 7 show examples of this. According to FIG. 5, an anastomosis protection 20 with a tube 21 has two essentially triangular holding wings 22 with preferably rounded corners. In Fig. 6, an anastomosis protection 23 is shown with a tube 24 , which is seen with two wavy holding wings 25 ver. Fig. 7 shows a further embodiment of an ana stomosis protection 26 with a tube 27th The anastomosis protector 26 has two holding wings 28 , in which there are perforation openings 29 . The perforation openings 29 can, for example, be punched into the material of the holding wings 28 . To fix the anastomosis protection 26 in the course of an operation in the patient's body, the surgical suture material can be guided through the perforations 29 . An anastomosis protection is also conceivable, in which the individual holding wings are designed differently.
While the retaining wings 28 of the anastomosis protector 26 can be sewn due to the perforation openings 29 , it is also imaginable to use a material for the retaining wings that can be relatively easily pierced by a surgical needle in the course of an operation, but also so has high strength that the seam does not tear out of the retaining wing.
The anastomosis protector is preferably made of a material that can be cut manufactured so that it can be used in the course of an operation the right size, d. H. especially to the right length can be brought. However, it is also conceivable that Holding wing in a to the respective anatomical conditions bring adapted shape.
The anastomosis protection is resorbable, so after some time no foreign material in the patient's body after the operation remains and could lead to complaints. A suitable material al. for example in the form of films, is poly-p-dioxanone, its Tensile strength about 30 to 50 days after implantation on the half of the initial value drops over the course of about 180 days is completely dismantled. Other absorbable materials are also conceivable, for example homopolymers or copolymers Lactide and glycolide. The tube of anastomosis protection is preferably closed-walled.
In Fig. 8, a Einführeinrich device 30 is shown in perspective view, which facilitates the insertion of an anastomosis protection, for example the anastomosis protection 1 . The insertion device 30 has a cylindrical auxiliary tube 32 , which is open at both ends and whose outside diameter is smaller than the inside diameter of the anastomosis protection matched to it, that is to say here as the inside diameter of the tube 2 . Through the interior of the auxiliary tube 32, a thread 34 is guided, which emerges at both ends of the auxiliary tube 32nd The thread 34 can consist of non-resorbable or resorbable material. At one of its ends, a loop 36 is formed on the thread 34 , which preferably pulls itself when it is placed around an object and is pulled on the thread 34 . The opposite end 38 of the thread 34 is shown in Fig. 8 as a simple piece of thread. However, it may be advantageous if the opposite end 38 is knotted into a loop. This prevents on the one hand that the opposite end 38 can inadvertently get into the interior of the auxiliary tube 32 , and on the other hand it makes it easier for the loop to pull on the thread 34 . The auxiliary tube 32 is preferably made of a substantially rigid (non-absorbable) plastic material.
In Fig. 9 various successive steps are illustrated in an operation which is intended to connect a he at Stes nerve end 40 with a second nerve end 42. The anastomosis protection 1 is used using the insertion device 30 . For the sake of clarity, the second nerve end 42 is not shown in parts (a) and (b).
As shown in FIG. 9 part (a), the insertion device 30 is first brought up to the first nerve end 40 with the anastomosis protection 1 arranged over the auxiliary tube 32 . The loop 36 is then placed around the first nerve end 40 . By pulling on the opposite end 38 of the thread 34 , the loop 36 contracts and the first nerve end 40 can be pulled to the auxiliary tube 32 , see Fig. 9 part (b). If the first nerve end 40 is not to be displaced in the longitudinal direction of the nerve, the insertion device 30 can alternatively be moved towards the first nerve end 40 with the anastomosis protection 1 . As soon as the state shown in FIG. 9 part (b) is reached, the anastomosis protector 1 can be pulled off the auxiliary tube 32 without major problems and pulled over the nerve section with the first nerve end 40 , see FIG. 9 part (c). The insertion device 30 can then be removed from the operating area. If the thread 34 can be resorbed, remnants (e.g. the loop 36 ) can remain in the region of the anastomotic site.
In Fig. 9, Part (c) is located, the second nerve end 42 will now be moved up to the first end 40 of nerves. The two nerve ends 40 and 42 should be brought into position without tension. When the two nerve ends 40 and 42 are in contact, they can be fixed, for example with the aid of single button sutures 44 , see FIG. 9 part (d). Finally, the anastomosis protector 1 is pushed over the interface until the final state shown in FIG. 9 part (e) is reached. The Ana stomosis protection 1 reliably protects the anastomosis site from external influences and quickly stops any bleeding. If necessary, the retaining wings 4 and 6, which were a great help even at the previous handling of the Anastomosenschutzes 1 can be fixed to the body tissue, for example, by solid-sewing.
In Fig. 10 a different surgical technique is illustrated, which comes into play when the gap between the first nerve end (here labeled 40 ') and the second nerve end (here labeled 42 ') is so large that the nerve ends 40 'and 42 ' cannot be brought into contact.
Fig. 10 part (a) shows the state of the operation as it corresponds to that shown in Fig. 9 part (c) and part (d). The two nerve ends 40 'and 42 ' which cannot be brought into direct contact are fixed with the aid of longer pieces of thread 46 , so that the nerve ends 40 'and 42 ' can no longer move apart. The anastomosis protector 1 can then be pushed from the region of the first nerve end 40 'via the second nerve end 42 ', the thread pieces 46 being an aid.
Alternatively, it is also conceivable to dispense with the thread pieces 46 and to insert the second nerve end 42 'directly into the tube 2 after moving the anastomosis protector 1 , as illustrated in FIG. 10 part (b) in longitudinal section.
