Zwei- oder einstielige Schneidschlinge für Resektoskope
Die Erfindung bezieht sich auf eine zweistielige oder in eine distale Gabel auslaufende einstielige, mit Hochfrequenzstrom
beschickbare Schneidschlinge für Resektoskope, deren halbkreisförmig gebogene Schneidschlinge in oder etwa in einer Ebene
senkrecht zur Längsrichtung liegt.
In der Resektoskopie kommen zum Abtragen des Prostata-Adenoms
bisher unipolare Schneidschlingen zur Anwendung, die in einem sogenannten Elektrotom, über das ein Außenschaft gesteckt wird,
mit HF-Strom beschickt wird,und zwar mit dem aktiven Pol eines ■—
HP-Generators verbunden wird, während der inaktive Pol des HF-Stromkreises großflächig mit dem Körper des Patienten verbunden
wird. Durch Längsbewegung der bügeiförmigen Schneidschlinge in proximale Richtung wird das Prostata-Adenom streifenförmig abgetragen.
Bei solchen unipolaren Schneidschlingen kann es durch Störungen zu unerwünschten und nicht kontrollierbaren, kapazitiven
Stromübergängen zum Außenschaft und damit zu Verletzungen der Harnröhrenschleimhaut kommen, die anschließend zu Harnröhrenstrikturen
führen. Beim Einführen des Resektoskops werden Gleitmittel ver-

wendet, die eine Isolierschicht zwischen Außenschaft und Harnröhrenwand
bilden, welche bei hohem kapazitivem HF-Stromübergang zerreißt, so daß es hier zu ungewollten Koagulationen kommt,
die nach einiger Zeit wieder zu Harnröhrenstrikturen führen.
Die Aufgabe der Erfindung besteht darin, bei der Prostata-Resektion
mittels mit HF-Strom betriebener Schneidschlingen die erwähnten unkontrollierbaren, kapazitiven HF-Stromübergänge zu vermeiden.
Dies wird nach der Erfindung dadurch erreicht,daß die Schneidschlinge
für Resektoskope aus zwei parallelen Drahtschlingen besteht, von denen die proximalseitige Drahtschlinge über den einen
Stiel oder Gabelarm an den aktiven Pol und die distalseitige Drahtschlinge über den anderen Stiel oder anderen Gabelarm an den inaktiven
Pol eines HF-Stromkreises ankuppelbar ist.
Damit ist es möglich, den Hochfrequenzstrom bipolar an das abzutragende
Gewebe der Prostata heranzubringen, ohne daß unkontrollierbare Stromübergänge auftreten können, wobei wesentlich ist, daß der
aktive Pol des HF-Stromkreises immer an die Drahtschlinge angelegt ist, die dem proximalen Ende des Resektoskops,also dem Auge des
operierenden Arztes zugekehrt ist, um das Prostata-Adenom in Schneidrichtung, d.h. von distal nach proximal unter Beobachtung
abtragen zu können. Der Abstand der beiden parallelen Drahtschlingen kann sich in Abhängigkeit von der HF-Spannung zwischen etwa

0,3 bis etwa 2 mm ändern. In jedem Fall wird der koagulierende
HF-Stromübergang zwischen der proximalseitxgen Drahtschlinge zur
distalseitigen Drahtschlinge eindeutig festgelegt und damit jegliche
ungewollte Koagulation von Gewebeteilen ausgeschlossen.
Die Erfindung wird nachstehend anhand der Zeichnung erläutert, in der zwei Ausführungsbexspiele dargestellt sind. Es zeigen:
Figur 1 eine zweistielige, bipolare Schneidschlinge
im unterbrochenen Längsschnitt mit vergrößerter Darstellung der distalen Stielteile,
Figur 2 eine Ausicht auf die Schneidschlinge nach
Figur 1 in natürlicher Größe,
Figur 3 eine unterbrochene einstielige bipolare
Schneidschlinge im vergrößerten Längsschnitt,
Figur 4 eine Aufsicht auf die Schneidschlinge nach Figur 3 in natürlicher Größe.
In den Figuren 1 und 3 sind die Drahtschlingen der Schneidelektrode
gegenüber den Figuren 2 und 4 zur besseren Darstellung etwas verdreht bzw. verkantet wiedergegeben.

Bei der zweistieligen Schneidelektrode nach Figur 1 und 2 schließt sich an das proximale Gewindeteil 1 als Anschlußkontakt
des einen Stieles 2 der aktive HF-Stromleiter 3, z.B. aus Kupferdraht, an, der in das eine Ende einer leitenden Rohrbrücke 4 eingelötet
ist,in deren anderes Ende der Schiingendraht 5, z.B. aus einem
speziellen Wolframdraht, eingelötet ist, der distal in einer Ebene senkrecht zur Längsrichtung zu einer halbkreisförmigen Drahtschlinge
6 gebogen ist und dessen freies Ende 7 distal in einer Metallhülse 8 des anderen Stieles 9 gelagert ist.
Der Aufbau des Stieles 9 für den Anschluß an den inaktiven Pol der
HF-Stromquelle ist der gleiche wie der des Stieles 2 mit dem aktiven Stromleiter 3, jedoch sir C hierzu die gleichen Teile mit den
gleichen Bezugszeichen aber mit dem zusätzlichen a versehen, und in jedem Fall liegt dabei die inaktive Drahtschlinge 6a distalseitig
der aktiven Drahtschlinge 6.
Die beiden parallelen, halbkreisförmigen und den gleichen Krümmungsradius
aufweisenden Drahtschlingen 6 und 6a besitzen in Abhängigkeit von derHF-spannung einen Abstand von etwa 0,3 bis etwa 2 mm und
können in ihrem Abstand durch isolierende Distanzstücke fixiert werden. Die Stromleiter 3, 3a, die geraden Schlingendrahtteile
5, 5a und die Rohrbrücke 4,4a sind je von einem Isolierschlauch 10,10a hoher elektrischer Durchschlagfestigkeit umhüllt, der
distal jeweils unmittelbar vor den Drahtschlingen 6,6a endet. Zusätzlich ist über den Schlauch 10,10a je nach Art des Resektoskops

ein führendes Metallrohrchen 11,TJa gezogen, an das sich distalseitig
ein isolierendes Schlauchstück 12, 12a anschließt, welches die
Metallhülse 8,8a umhüllt.
Bei der einstieligen Ausführung nach Figur 3 und 4 läuft der Stiel
13 distal in die Gabelarme 14 und 14a aus. Der Gabelarm 14 nimmt /leder den geraden Teil 5 der aktiven Drahtschlinge 6 auf, deren
:reies Ende 7 im anderen Gabelarm 14a in gleicher Weise gelagert ist, wie zu Figur 1 und 2 beschrieben wurde, während der Gabelarm
14a den geraden Teil 5a der distalseitigen inaktiven Drahtschlinge
6a aufnimmt, deren freies Ende 7a im Gabelarm 14 gelagert ist. In diesem Fall sind die proximalen Enden der geraden Schlingendrahtteile
5 und 5a wieder in eine metallische, leitende Rohrbrücke 4 und 4a eingelötet, die gegeneinander isoliert sind.
Die Rohrbrücke 4 ist in eine Metallhülse 15 eingelötet, die bei 16 mit dem Leitungsdraht 3 über einen Exzenterrirg 17 durch Verlötung
mit dem Kontaktring 18 verbunden ist, der mit dem aktiven Pol der HF-Stromquelle verbunden wird. In die Rohrbrücke 4a wird
proximalseitig der inaktive, isolierte Leitungsdraht 3a eingelötet,
der proximal an den Kontakt 1a angeschlossen ist, welcher an den inaktiven Pol der HF-Stromquelle ankuppelbar ist.
Auch in diesem Fall muß die prahtschlinge 6 r die an den aktiven
Pol der HF-Stromquelle anzuschließen ist, proximal vor der Drahtschlinge
6a liegendie an den inaktiven Pol anzuschließen ist, und auch in diesem Fall besitzen die beiden parallelen, den gleichen

Krümmungsradius aufweisenden, halbkreisförmigen Drahtschlingen
einen bestimmten Abstand, wie zu Figur 1 und 2 beschrieben ist. Weiter ist auch bei der Ausführung nach Figur 3 und 4 proximalseitig
der Gabel 14, 14a ein in diesem Fall ovales, einseitig
abgeflachtes Profilrohr 11 aus Metall vorgesehen, welches als Führung und gegen Verdrehung im Resektoskop dient.
Es ist verständlich, daß bei der Ausführung nach Figur 3 und 4 die
proximalseitige Drahtschlinge 6 auch mit dem dann an den aktiven Pol der HF-Stromquelle anzuschließenden Kontakt la verbunden werden
kann, während dann der mit dem inaktiven Pol anzuschließende Kontaktring 18 mit der distalseitigen Drahtschlinge 6a verbunden wird.

Claims (5)

Juni i975Anmelder: Richard Wolf GmbH, 7134 Knittlingen, Pforzheimer Str.AnsprücheM J Zweistielige oder in eine distale Gabel auslaufende, einstielige, mit Hochfrequenzstrom beschickbare Schneidschlinge für Resektoskope, deren halbkreisförmig gebogene Schneidschlinge in oder etwa in einer Ebene senkrecht zur Längsrichtung liegt, dadurch gekennzeichnet, daß die Schneidschlinge aus zwei parallelen Drahtschlingen (6,6a) besteht, von denen die proximalseitige Drahtschlinge (6) über den einen Stiel (2) oder einen Gabelarm (14) an den aktiven Pol und die distalseitige Drahtschlinge (6a) über den anderen Stiel (9) oder anderen Gabelarm (14a) an den inaktiven Pol eines HF-Stromkreises ankuppelbar ist.
- 2. Schneidschlinge nach Anspruch 1, dadurch gekennzeichnet, daß das freie Ende (7) der einen über den einen Stiel (2) oder Gabelarm (14) an den aktiven Pol anschließbaren Drahtschlinge (6) und das freie Ende (7) der anderen über den anderen Stiel (9) oder Gabelarm (14a) an den inaktiven Pol anschließbaren Drahtschlinge (6a) jeweils im distalen Ende des anderen Stieles oder Gabelarmes isoliert gelagert ist.609851/0611
- 3. Schneidschlinge nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß der Abstand der beiden parallelen Drahtschlingen (6/6a) in Abhängigkeit von der HF-Spannung zwischen etwa 0,3 bis etwa .2 mm beträgt.
- 4. Schneidschlinge nach einem der Ansprüche 1 bis 3, dadurch gekainzeichnet, daß der Abstand der beiden parallelen Drahtschlingen (6,6a) durch isolierende Distanzstücke fixiert ist.
- 5. Schneidschlinge nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß der Anschluß der aus speziellem Wolframdraht bestehenden Drahtschlingen (6,6a) an die proximalen Kontakte der Stiele (2,9) oder des Stieles (13) je über eine leitende Rohrbrücke und einen zu den Kontakten (1,1a, 18) führenden Stromleiter (3,3a) erfolgt.60985 1/06 11
