Description
Die Erfindung bezieht sich auf eine implantierbare Prothesenverankerung, die aus bioaktiven, resorbierbaren keramischen Werkstoffen auf der Basis von
Calciumphosphaten sowie aus polymeren Kunststoffen, die im Körpermilieu mechanisch und chemisch stabil
sind, besteht. Außerdem bezieht sich die Erfindung auf die Herstellung des entsprechenden Werkstoffverbundes. Es handelt sich dabei um ein System oder einen
Werkstoffverbund zur zementlosen Verankerung von hochbelasteten Endoprothesen vor allem von Gelenkendoprothesen.
Bisher wurden die vorwiegend metallischen Prothesen im allgemeinen mit Hilfe eines in situ erhärtenden
Knochenzements befestigt, der den Prothesenschaft in
einer entsprechend herausgearbeiteten Höhlung im Knochen, dem sogenannten »Implantatlager«, mechanisch verankert Diese Methode gilt heute jedoch als
problematisch, weil die polymerisierenden Zementbestandteil monomere Anteile an das umliegende
Gewebe abgeben und im Organismus schädliche Nebenreaktionen auslösen können. Außerdem kann es
durch die beim Polymerisieren entstehende Reaktionswärme zur Gewebezerstörung kommen. Diese und
andere Vorgänge haben eine Lockerung der Prothese zur Folge. Die genannten Nachteile haben vielfältige
Forschungsaktivität zur Entwicklung zementloser Verankerungssysteme hervorgerufen.
Die verbesserte Gewebeverträglichkeit von hochstabilen oxidkeramischen Prothesenwerkstoffen im Vergleich zu Metallen hatte die Hoffnung hervorgerufen,
daß mit Hilfe solcher Werkstoffe eine direkte Verwachsung zwischen Knochen und Keramik stattfin
den und so eine zementlose Verankerung erreicht
werden könne. Besondere, z. B. wellenartige Oberflächengestaltungen des Prothesenschaftes, Löcher oder
dergleichen, sollten dabei den Vorgang einer mechanischen Verankerung unterstützen. Verschiedene Caici-
umphosphatverbindungen und Mischungen aus solchen Verbindungen zur Herstellung künstlicher Knochen und
Zähne sind in der US-PS 37 37 900 beschrieben. Leider hat sich inzwischen gezeigt, daß diese Verankerungsmethoden bei Prothesen mit hohen mechanischen BeIa-
J0 stungen, wie Hüftgelenkprothesen, Zähnen usw, in der
Praxis nicht funktionieren, da durch Ausbildung einer bindegewebigen Zwischenschicht und Zurückbildung
des Knochens schließlich auch hier eine Lockerung der Prothese eintritt Diese Erkenntnisse haben zur
J5 Einbeziehung sogenannter bioaktiver Werkstoffe geführt, ■ die eine bindegewebsfreie Verwachsung des
Knochens mit der Werkstoffoberfläche bewirken sollen. Bei solchen ,Werkstoffen handelt es sich z. B. um
Calciumphosphate bestimmter Zusammensetzung, bei denen eine direkte, bindegewebsfreie Verwachsung des
Knochens mit dem Werkstoff eintritt (»Resorbierbare keramische Werkstoffe für den Knochenersatz«, Biomedizinische Technik, Band 20, Mai 1975, S. 115 ff;
»Neuere Werkstoffe in der medizinischen Technik«,
Chemie-Ingenieur-Technik, Heft 8, 1975, S. 327-333).
In bestimmter Gefüge- und Phasenausbildung sind diese Calciumphosphate im biologischen Milieu abbaubar,
d. h., sie werden von den an der Knochenneubildung beteiligten Zellen absorbiert Als alleinigen Werkstoff
für eine auf Dauer implantierbare Prothese kommen sie daher nicht in Frage.
Implantate aus porösen keramischen Materialien sind ebenfalls bereits bekannt (DE-PS 22 42 867). Durch
Ausbildung einer sehr gleichmäßigen, offenen Porösität mit Poren idealer Größe wollte man ein optimales
Einwachsen des neu gebildeten Knochengewebes in das Implantat erreichen. Für viele Anwendungsfälle läßt die
mechanische Stabilität solcher Implantate zu wünschen übrig.
Des weiteren ist es bereits bekannt, eine z. B. aus
Metall bestehende implantierbare Prothese mit gemahlenen Partikeln zu beschichten, die das Wachstum des
natürlichen Knochengewebes stimulieren und dabei im Laufe der Zeit von dem neu gebildeten Knochen ersetzt
werden sollen (US-PS 39 18 100). Nach der Resorption der Beschichtung steht auch bei einem solchen
Implantat das knochenfremde Prothesenmaterial unmittelbar dem neu gebildeten Knochengewebe gegen-
über, was dann wiederum zu den bereits geschilderten,
bindegewebigen Zwischenschichten und damit zur Zurückbildung des Knochens und Lockerung der
Prothese führt Das Einbetten von partikulärem Calciumsulfat in die Außenfläche eines implantierbaren
Kunststoffkörpers ist ebenfalls schon bekannt (US-PS 39 19 773). Auch hierbei bleiben nach dem Auflösen der
Partikel Poren zurück, in deren Innerem dann wiederum Knochengewebe direkt dem Kunststoff des Implan'ates
gegenübersteht Gleiche Nachteile resultieren bei Massen aus Polymeren und anorganischen, silanisierten
Füllstoffen, die für Dentalzwecke vorgeschlagen wurden (DE-OS 24 03 211) sowie bei Direkt-Implantaten,
die ein polymerisierbares Harzbindemittel und anorganischen,
mit Silarikoppler behandelten Füllstoff enthalten (DE-OS 24 60196). Hinzu kommt, daß eine
genügende mechanische Stabilität sich mit diesen Werkstoffen ebenfalls nicht erzielen läßt
Der vorliegenden Erfindung liegt nun die Aufgabe zugrunde, die geschilderten Nachteile zu überwinden
und ein für hochbelastete Endoprothesen geeignetes Implantat zu entwickeln, das zu einer dauerhaften
Verbindung zwischen dem Knochengewebe bzw. der Endoprothese führt
Es hat sich nun gezeigt, daß diese Aufgabe durch eine
Prothesenverankerung der eingangs genannten Art gelöst werden kann, wenn diese als Prothesenscbaftbeschichtung
ausgebildet ist und wenn die keramischen Werkstoffe in partikulärer Form in Partikelgrößen von
O^ bis 1 mm Durchmesser in den Kunststoff derart eingelagert sind, daß bei der Resorption des keramischen
Anteils ein durchgängig poröses Gefüge aus Kunststoff entsteht, auf dessen inneren Porenoberflächen
bioaktivierende Reste der Keramik zurückbleiben. Bei der erfindungsgemäßen Prothesenverankerung
kann nach der Implantation neugebildetes Knochengewebe simultan zu der Porenbildung bzw. zu der
Resorption der Keramik in das poröse Kunststoffgefüge penetrieren.
Die feste chemische Bindung von Keramikresten im Grenzbereich zwischen der Keramik und dem Kunststoff
hat zur Folge, daß bei der Resorption der Keramikpartikel im Randbereich Calciumphosphat-Baugruppen
am Kunststoffgerüst zurückbleiben und als bioaktivierende Haftvermittler zwischen dem Kunststoff
und dem einwachsenden Gewebe dienen. Bei dem erfindungsgemäßen Implantat bleibt nämlich nach dem
Einwachsen des Knochengewebes in die Poren, die durch die resorbierbaren Calciumphosphatpartikel
simultan mit der Knochenneubildung freigegeben werden, auf den Innenflächen dieser Poren eine fest an
den Kunststoff gebundene Schicht aus Keramikresten zurück, die ihrerseits — im Gegensatz zu dem
Kunststoff — bioaktivierend ist und di.her die
Ausbildung einer bindegewebigen Zwischenschicht — 5^
der Hauptursache für die Lockerung von Implantaten — verhindert. Dadurch entsteht eine nahezu übergangslose
Verbindung zwischen dem nichtresorbierbaren Anteil des Prothesenverankerungswerkstoffes und dem
neugebildeten, in die Poren eingedrungenen Knochengewebe. Durch die erfindungsgemäßen Maßnahmen
gelingt es also, die für die Knochenneubildung und dauerhafte Verankerung so wichtigen Eigenschaften
der bioaktiven Calciumphosphate mit der Festigkeit, der Dauerhaftigkeit und Belastbarkeit bioinerter polymerer
Kunststoffe zu vereinen. Durch das Zusammenwirken mit den Calciumphosphaten erhalten die bioinerten
Polymere, die in ihrer Gewebefreundlichkeit gegenüber dem Knochen zwar »neutral«, aber nicht »bioaktiv«
sind, die notwendige Bioaktivität, die schließlich zu einer beschleunigten, bindegewebslosen direkten Verwachsung
zwischen Implantat und Knochen damit zu der erstrebten dauerhaften Verankerung führt
Nach einer vorteilhaften Ausführungsart der Erfindung bestehen die keramischen Werkstoffe aus
gesinterten Calciumphosphatpartikeln der chemischen Zusammensetzung CaO: P2O5 im Mclverhältnis von
etwa 3:1.
Bei der Herstellung der erfindungsgemäßen Prothesenverankerung bzw. des entsprechenden Werkstoffverbundes
können die keramischen Partikel vor der Einlagerung in den Kunststoff durch chemische
Behandlung oberflächlich mit polymeraffinen Molekülen beladen werden, die eine feste chemische Bindung
von Calciumphosphaten an den Kunststoff im Bereich der Keramik/Kunststoff-Grenzflächen herbeiführen.
Hierfür können als polymeraffine Moleküle acrylat-, methacrylat- oder vinylsubstituierte Di- oder Tri-Halogen-,
Alkoxy- oder AcyloxysiJane verwendet werden, die mit dem Kunststoff copolymerisieren. Als polymeraffine
Moleküle sind Silane mit mindestens einem in dem Kunststoff gut löslichen Substituenten mit 6 bis 20
Kohlenstoffatomen verwendbar, wobei das polymeraffine Molekül mit dem Kunststoffanteil nicht kovalent,
sondern durch chemische Bindung zweiter Ordnung verbunden ist Der Kunststoffanteil des Prothesenverankerungswerkstoffs
besteht vorzugsweise aus Polyacrylaten, Polymethacrylaten, Polyacrylnitril oder deren
Copolymeren oder aus Polyester, Polyamiden, PoIyäthylenterephthalaten,
Polyäthylen hoher Dichte, Polypropylen, Polysulfonen oder dergleichen.
Bei der Herstellung des Werkstoffverbundes aus Keramik und Kunststoff ist darauf zu achten, daß die
Calciumphosphatpartikel jeweils von benachbarten Partikeln berührt werden und dennoch genügend freier
Raum für eine Auffüllung mit dem Kunststoff bleibt. Diese Verhältnisse sind optimal, wenn die Calciumphosphate
möglichst gleichartige Größe und Kugelform besitzen. Für den Kunststoffanteil bleiben dann die
Zwickelräume einer solchen Kugelschüttung.
Um zu erreichen, daß das poröse Kunststoffgerüst, welches bei der Resorption der Keramik entsteht,
bioaktiv ist und auch bleibt, damit eine übergangslose
Verbindung mit dem Knochen entsteht, muß die Porenoberfläche des Kunststoffes bioaktive, knochengewebefteundliche
Moleküle enthalten. Die hier infrage kommenden Kunststoffe selbst sind keineswegs bioaktiv.
Deshalb werden nach einem bevorzugten Ausführungsbeisipiel der Erfindung die Phosphatpartikel vor
ihrer Einlagerung in den Kunststoff in der bereits genannten, nachfolgend noch detaillierten beschriebenen
Weise einer Oberflächenbehandlung unterworfen, durch die sie eine Affinität zum Kunststoff erhalten.
Die Calciumphosphatpartikel werden z. B. mit Silanderivaten behandelt, welche sowohl zum Calciumphosphat
als auch untereinander chemische Verbindungen eingehen und eine weitere funktionell Gruppe
besitzen, die eine starke Bindung zum Kunststoffgerüst ergibt. Es sind eine Reihe von solchen Silanderivaten
der allgemeinen Formel

bekannt, wobei »n«den Wert 1 oder 2 haben kann, »X«
ein Halogen, Alkoxy oder Acyloxy bedeutet und »Y« eine mit dem polymeren Kunststoff vornehmlich
kovalent gebundene Gruppe symbolisiert. Bei den
Polymeren und Copolymeren aus Acrylaten, Methacrylaten,
Acrylonitril ist »Y« z. B. eine mit diesen Monomeren copolymerisierbare Vinylgruppe.
Durch dieses Verfahren werden die grenzflächennahen Calciumphosphat-Moleküle so stabil gebunden, daß
bei der Resorption der Hauptmenge des Calciumphosphatanteiles in situ eine Calciumphosphatgrenzschicht
verbleibt und so eine bioaktivierende, den Knochenverbund erzeugende Prothesenoberfläche entsteht.
Weitere Merkmale, Vorteile und Anwendungsmöglichkeiten der Erfindung gehen aus der folgenden
genaueren Beschreibung spezieller Beispiele zur Herstellung der erfindungsgemäßen Prothesenverankerung,
einschließlich der Vorbehandlung der keramischen Partikel, anhand einiger Zeichnungen hervor.
Es zeigt
F i g. 1 bis 3 in schematischer Darstellung eine Hüftgelenk-Prothese während der Herstellung der
erfindungsgemäßen Prothesenschaftbeschichtung,
F i g. 4 in vergrößerter Teildarstellung und im Schnitt den Grenzbereich zwischen der Prothese und der
Beschichtung nach F i g. 3 und
F i g. 5 bis 7 in vergrößerter Teildarstellung und im Schnitt verschiedene Stadien während des Einwachsens
von neugebildeten Knochen als Beschichtung ausgeführte Prothesenverankerung nach der Erfindung.
Gemäß F i g. 1 und F i g. 2 wird eine aus Metall oder Oxidkeramik bestehende Hüftgelenkprothese 1 zunächst
in einer geeigneten, z. B. aus Silikonkautschuk gefertigten Form 2 derart fixiert, daß zwischen dem
Prothesenschaft 4 und der Formwandung ein gleichmäßig breiter Spalt 3 von ca. 2—3 mm freibleibt. Die Breite
dieses Spaltes bestimmt die spätere Dicke der Prothesenschaftbeschichtung.
Danach wird dieser Spalt 3 mit kugelförmigen Calciumphosphatpartikeln 5, deren Durchmesser etwa
zwischen 0,5 und 1 mm liegen, so ausgefüllt, daß eine dichtgepackte Kugelschüttung entsteht, wie dies F i g. 2
zeigt. Anschließend wird der mit Kugeln gefüllte Spalt 3 mit flüssigen Monomeren oder einer Lösung von
ίο Polymeren und Monomeren, die mit Initiatoren versetzt
sind, ausgefüllt und zur Polymerisation gebracht. Im vorliegenden Fall wurde eine 2O°/oige Lösung von
Polymethacrylat in monomerem Methacrylat unter Zusatz von 0,1% Benzolperoxid und 0,2% p-Toluidin
verwendet.
Danach wird die Prothese entformt — vgl. F i g. 3 — und besitzt nun eine mit Calciumphosphatpartikeln 5
durchsetzte Kunststoffbeschichtung 7. Um eine mechanische Verzahnung mit der Beschichtung zu erreichen,
weist der Prothesenschaft eine wellenförmig oder zahnartig aufgebaute Oberflächenstruktur auf, wie dies
in Fig.4 angedeutet ist; zusätzlich oder anstelle der
Aufrauhung läßt sich auch ein Haftvermittler zwischen Prothesenoberfläche und Kunststoffbeschichtung aufbringen.
In vielen Fällen ist es günstig, die Calciumphosphatpartikel wie folgt vorzubehandeln:
Eine innige Pulvermischung von zwei Teilen CaHPO4
mit einem Teil CaCO3 wird durch Pressen verdichtet und in einem Brennofen zwei Stunden lang bei 15000C
gesintert. Nach der Formel.
2CaHPO4 + CaCO, 3CaO P2Os+ H2O | + CO2 }
entsteht bei dieser Reaktion im wesentlichen die Verbindung Tricalciumphosphat. Das Sinterprodukt
besteht, wie eine Gefügeuntersuchung und eine Röntgengemengeanalyse zeigt, aus einer Mischung von
glasigem Calciumphosphat mit Anteilen der «-(Hochtemperatur-) und «-(Tieftemperaturform) des Tricalciumphosphates.
Das Sinterprodukt besitzt ein Raumgewicht von 2,47 p/cm3, d. h, es besitzt nur ca. 79% der
theoretischen Dichte. Die Restporosität und die genannte Phasenzusammensetzung sind optimal für die
Gewebeverträglichkeit und die ResorbierbarkeiL
Zur Herstellung kugeliger Aggregate werden diese Sinterprodukte vorgranuliert, in einer rotierenden
Trommelmühle rundgeschliffen und schließlich gesiebt.
Die Calciumphosphatpartikel werden vor ihrer Einlagerung in den Kunststoff mit im Handel erhältlichem,
methacrylatfunktionellem Silan behandelt. Dadurch wird auf der Ca^PO^-Oberfläche eine Methacrylatsubstitution
mit der Dichte von ungefähr 3 μ Mol Silansubstituenten pro g Calciumphosphat erreicht
Die einzelnen Vorgänge und Wirkungen bei der Anbringung der erfindungsgemäßen Prothesenverankerung
und dem Einwachsen des Knochengewebes lassen sich anhand der F i g. 5—7 wie folgt erläutern:
Das Monomer-Polymergemisch Methacrylat-Polymethacrylat,
aus dem hier das Kunststoffgerüst 7 gemäß F i g. 5 hergestellt wird, polymerisiert in den Zwischenräumen
der Kugelschüttung aus oberflächenbehandelten Calciumphosphatpartikeln 5 aus und copolymerisiert
im Grenzbereich 8 zwischen der Keramik und dem Kunststoff mit den methacrylatfunktionellen Silangruppen,
die in F i g. 5 durch + symbolisiert sind.
F i g. 6 veranschaulicht den Vorgang der Resorption der Calciumphosphatpartikel 5 und der gleichzeitigen
Penetration durch neugebildeten Knochen 9 in das Kunststoffgerüst 7; dabei dringt der Knochen 9 vom
äußeren Rand her der als Prothesenbeschichtung ausgebildeten Verankerung in die Porenräume vor. An
der Poreninnenwandung verbleiben dank der chemischen Bindung Reste von Calciumphosphatmolekülen
10 und fungieren als bioaktivierende Grenzzone. Diese Moleküle 10 sind in Fig.6 durch das Zeichen O
symbolisiert
In dem Stadium nach Fig.7 ist schließlich der
Vorgang der Verbundbildung zwischen dem zurückgebliebenen porösen Kunststoffgefüge 7 und dem
neugebildeten Knochen 9 angeschlossen. Alle Poren sind von Knochen erfüllt Es entsteht ein tragfähiger
Verbundbereich zwischen dem Schaft der Prothese 1 und dem Knochen 9.

Claims (6)
1. Implantierbare Prothesenverankerung, bestehend aus bioaktiven, resorbierbaren keramischen
Werkstoffen, auf der Basis von Calciumphosphaten sowie aus polymeren Kunststoffen, die im Körpermilieu mechanisch und chemisch stabil sind, wobei
die Prothesenverankerung als Prothesenschaftbeschichtung ausgebildet ist, dadurch gekennzeichnet, daß die keramischen Werkstoffe in
partikulärer Form in Partikelgrößen von 0,5 bis 1 mm Durchmesser in den Kunststoff derart eingelagert sind, daß bei der Resorption des keramischen
Anteils ein durchgängig poröses Gefüge aus Kunststoff entsteht, auf dessen inneren Porenoberflächen
bioaktivierende Reste der Keramik zurückbleiben.
2. Proihesenverankerung nach Ansprucn 1, dadurch gekennzeichnet, daß die keramischen Werkstoffe aus gesinterten Calciumphosphatpartikeln der
chemischen Zusammensetzung CaO : P2O5 im MoJ-verhältnis von etwa 3 :1 bestehen.
3. Prothesenverankerung nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß der Kunststoffanteil
aus Polyacrylaten, Polymethacrylate^ Polyacrylnitril oder deren Copolymeren oder aus Polyester,
Polyamiden, Polyäthylen besteht
4. Verfahren zur Herstellung einer Prothesenverankerung nach einem der Ansprüche 1 bis 3, dadurch
gekennzeichnet, daß die keramischen Partikel vor der Einlagerung in den Kunststoff in an sich
bekannter Weise durch chemische Behandlung oberflächlich mit polymeraffinen Molekülen beladen
werden, die eine feste chemische Bindung von Calciumphosphaten an den Kunststoff im Bereich
der Keramik/Kunststoff-Grenzfläche herbeiführen.
5. Verfahren nach Anspruch 4, dadurch gekennzeichnet, daß als polymeraffine Moleküle acrylat-,
methacrylat- oder vinylsubstituierte Di- oder Tri-Halogen-, Alkoxy- oder Acyloxysilane verwendet
werden, die mit dem Kunststoff «»polymerisiert werden.
6. Verfahren nach Anspruch 4, dadurch gekennzeichnet, daß als polymeraffine Moleküle Silane mit
mindestens einem in dem Kunststoff gut löslichen Substituenten mit 6—20 Kohlenstoffatomen verwendet werden, wobei das polymeraffine Molekül
mit dem Kunststoffanteil nicht kovalent, sondern durch chemische Bindung zweiter Ordnung verbunden ist.
