Description
Die Erfindung betrifft ein Hohlorgan-Implantat gemäß
dem Oberbegriff des Anspruches 1.
Ein bekanntes Hohlorgan-Implantat dieser Art (DE-OS 26
54 658) weist einen durch schraubenlinienförmiges
Aufwickeln eines Geflechtes hergestellten Flechtschlauch
auf, dessen Lagen durch eine Naht verbunden
sind.
Ein anderes bekanntes künstliches Blutgefäß (DE-OS 20
17 330) weist, um die Möglichkeit seiner Punktierung
ohne Gefahr des Weiterreißens zu schaffen, einen
Schlauch auf, der zumindest in seinem Mittelbereich
aus punktierbarem Material besteht.
Solchen bekannten Hohlorgan-Implantaten ist gemeinsam,
daß sie relativ große "kritische Biegeradien" haben.
Mit kritischem Biegeradius ist derjenige Radius
verstanden, bei dessen Unterschreiten das Hohlorgan-
Implantat knickt. Das Knicken führt zu starker
Reduzierung des freien Lumens oder gar zum Verschluß
des Hohlorgan-Implantats.
Es ist deshalb Aufgabe der Erfindung, ein Hohlorgan-
Implantat der eingangs genannten Art zu schaffen, das
zumindest in einem Abschnitt seiner Länge kleineren
kritischen Krümmungsradius hat, wobei dies ohne
wesentliche Veränderung der Wanddicke des Hohlorgan-
Implantats gelingen soll.
Diese Aufgabe wird erfindungsgemäß bei einem
Hohlorgan-Implantat der eingangs genannten Art dadurch
gelöst, daß das Geflecht aus mindestens einem als
Hohlgeflecht hergestellten Flechtschlauch aus
biegsamen, elastischen Fäden besteht.
Der als Hohlgeflecht hergestellte Flechtschlauch gibt
in seinem Bereich dem Hohlorgan-Implantat erhöhten
Widerstand gegen Knicken und Zusammendrücken und
verkleinert in seinem Bereich den kritischen Krümmungsradius
des Hohlorgan-Implantats beträchtlich. Es
können sehr kleine kritische Krümmungsradien erreicht
werden. Auch erhöht der nahtlose Flechtschlauch den
Widerstand des Hohlorgan-Implantats gegen sonstige
Querschnittsverengungen ebenfalls beträchtlich. Dies
gelingt, ohne die Wanddicke des Hohlorgan-Implantats
nennenswert vergrößern zu müssen. Auch ist es
weiterhin recht biegsam.
Es ist zwar ein künstliches Blutgefäß bekannt (DE-OS
20 17 330), das aus einem als Hohlgeflecht hergestellten,
jedoch nach seiner Herstellung noch
gewellten Flechtschlauch besteht, wobei durch die
Wellung die Knickgefahr behoben werden soll. Jedoch
wird dies mit beträchtlicher Vergrößerung des
Außendurchmessers, schwieriger und zeitraubender
Herstellung und oft unerwünscht hoher Turbulenz des
ihn durchströmenden Blutes erkauft. Das
erfindungsgemäße Hohlorgan-Implantat bedarf dagegen
keiner Wellung und kann auch außer als
Blutgefäßprothese auch für andere Implantierungen
vorgesehen sein.
So kann das erfindungsgemäße Hohlorgan-Implantat in
menschlichen und tierischen Körpern zahlreiche
Einsatzgebiete haben. Bevorzugte Einsatzgebiete sind
als Blutgefäßprothese, als Speise- oder Luftröhrenprothese,
als Harnleiter- oder Harnröhrenprothese oder
als Gallengangprothese. Es ist auch denkbar, es als
Darmprothese verwenden zu können. Auch andere
Verwendungen sind möglich.
Bei einer bevorzugten Ausführungsform besteht der
Flechtschlauch aus Metallfäden. Auch ist es in vielen
Fällen zweckmäßig, ihn aus Fäden aus polymerem
Kunststoff zu flechten und/oder aus einer Mischung von
Metallfäden und polymeren Kunststoffäden. Die
Metallfäden ermöglichen es, den Flechtschlauch
besonders dünnwandig herzustellen. Sie können
vorzugsweise Durchmesser von 0,04 bis 0,15 mm,
vorzugsweise von ungefähr 0,05 bis 0,07 mm aufweisen.
Wenn Kunststoffäden für den Flechtschlauch verwendet
werden,
können diese zweckmäßig Durchmesser von 0,05 bis 1 mm,
vorzugsweise von ungefähr 0,4 mm aufweisen.
Der Flechtschlauch kann am Schlauch unterschiedliche
Anordnung haben. Bevorzugt kann er im Inneren der
Wandung des Schlauches angeordnet sein, vorzugsweise
in ihn eingebettet sein. Jedoch ist es in vielen
Fällen auch zweckmäßig, den Flechtschlauch an der Innenoberfläche
der Wandung des Schlauches anzuordnen,
insbesondere dann, wenn es erwünscht ist, durch
ihn auch eine gewisse Rauhigkeit der Innenoberfläche des Hohlorgan-
Implantats herbeizuführen. Wenn man, was ebenfalls
in vielen Fällen zweckmäßig ist, den Flechtschlauch
an der Außenoberfläche des Schlauches anordnet,
dann kann in ihn nach der Implantation Körpergewebe einwachsen
und er so die Verankerung des Hohlorgan-Implantats
im Körper verbessern und sichern. Auch besteht die Möglichkeit,
vorhandene, herkömmliche Schläuche
nachträglich erfindungsgemäß mit einem außenseitig
aufgezogenen Flechtschlauch auszurüsten, um so insbesondere
den kritischen Biegeradius zu verringern.
Es ist ohne weiteres möglich, den Flechtschlauch über die
volle Länge des Schlauches zu erstrecken. In
vielen Fällen ist es jedoch ausreichend oder sogar zweckmäßig,
nur eine oder mehrere vorbestimmte Längsabschnitte
des Schlauches mit einem Flechtschlauch zu versehen,
um beispielsweise bei einem Blutgefäßprothesenschlauch,
der durch ein Gelenk, beispielsweise einen Ellenbogen
oder ein Knie hindurch verlegt werden soll, vorzusehen,
daß der Flechtschlauch sich nach der Implantation
nur im Bereich des Gelenkes befindet.
Es ist auch möglich, mehrere Flechtschläuche in Längsabständen
voneinander entlang dem Schlauch anzuordnen.
In manchen Fällen kann es auch zweckmäßig sein,
daß ein Flechtschlauch von mindestens einem weiteren
Flechtschlauch ohne oder im Abstand umfaßt ist, um beispielsweise
besondere Biegecharakteristiken zu erzielen.
So kann es in manchen Fällen zweckmäßig sein, wenn ein
Flechtschlauch nur auf einem Abschnitt seiner Länge,
beispielsweise auf einem Mittelabschnitt von einem wesentlich
kürzeren zweiten Flechtschlauch umfaßt ist, um so
einen Bereich, in dem die höchste Knickgefahr besteht
oder der besonders hohem Druck von außen ausgesetzt
ist, zusätzlich gegen Knicken und stärkere Querschnittsverengung
zu sichern.
Besonders vorteilhaft ist es, wenn zumindest eine Teilanzahl
der Fäden des Flechtschlauches, vorzugsweise alle
seine Fäden monofile Fäden sind, da hierdurch der Flechtschlauch
besonders hohe Biegeelastizität erhält und sich
so auch bei extremen Verformungen wieder in seine Ursprungsgestalt
zurückformt, sobald die Verformungskraft
aufhört. Unter einem monofilen Faden ist ein Faden verstanden,
der aus einem einzigen Elementarfaden besteht,
also im Falle eines polymeren monofilen Kunststoffadens aus
einem Loch einer Düse ersponnen ist oder im Falle eines
monofilen Metallfadens als dünner Draht aus einem Ziehloch
eines Ziehwerkzeuges gezogen ist.
Das außer dem oder den Flechtschläuchen vorhandene übrige
Material des Hohlorgan-Implantats kann von an sich bekannter
Art sein. So kann es mindestens
ein schlauchförmiges Gewebe, Gewirke, Gestrick
oder Vlies aus textilen Fasern aufweisen.
Es ist jedoch auch möglich, das Hohlorgan-Implantat so auszubilden,
daß es aus einem Kunststoffschlauch - vorzugsweise
aus expandiertem Polytetrafluoräthylen oder Polyätherurethan
oder Silikonelastomer - und dem Flechtschlauch besteht.
Um Abwehrreaktionen des menschlichen oder tierischen
Körpers gegen das Hohlorgan-Implantat zu begegnen oder solche
Abwehrreaktionen zu vermindern oder leichter beherrschbar
zu machen, kann auch vorgesehen sein, daß mindestens
eine schlauchförmige Schicht des Hohlorgan-Implantats aus
tierischem oder menschlichem Gewebe, vorzugsweise aus
einer Nabelschnur besteht.
Als besonders zweckmäßig hat es sich erwiesen, das den
Flechtschlauch bildende Hohlgeflecht in Leinwandbindung
zu flechten. Hierdurch stützen sich die Fäden des Flechtschlauches
gegenseitig ab, so daß seine Eigenelastizität
und damit auch seine Rückformelastizität besonders hoch
sind.
Im allgemeinen ist es auch erwünscht, daß derartige Hohlorgan-
Implantate beträchtliche Torsionsmomente ohne Querschnittsveränderung
oder fast ohne Querschnittsveränderung
aushalten können, d. h. gute Torsionsstabilität
haben. Zu diesem Zweck kann vorgesehen
sein, den Flechtwinkel (durch Längsmittelebene des
Flechtschlauches halbierter Kreuzungswinkel der
Fadenscharen) des Flechtschlauches zu 70 bis 110° zu
treffen. Hierdurch setzt das Hohlorgan-Implantat
Torsionsmomenten besonders starken Widerstand entgegen.
Das Hohlorgan-Implantat kann noch mit weiteren Teilen
versehen werden. Beispielsweise kann es in für
Herzgefäßprothesen noch ein Klappenventil eingesetzt
sein. Oder an es können Mundstücke oder sonstige
zusätzliche Teile angefügt sein.
In der Zeichnung sind Ausführungsbeispiele der
Erfindung dargestellt. Die Fig. 1 bis 4 zeigen je
ein längsgeschnitten dargestelltes, ungewelltes
Hohlorgan-Implantat in gebrochener Darstellung.
Das Hohlorgan-Implantat 10 nach Fig. 1 kann runden
oder auch anderen Querschnitt haben, der hier über die
Länge des Hohlorgan-Implantats konstant ist. Es ist
elastisch biegsam und besteht aus einem aus zwei
zueinander koaxialen, rohrförmigen Schichten 12, 13
bestehenden Schlauch 9 und einem Flechtschlauch 11.
Die innere Schicht 12 des Schlauches 9 besteht aus
polymerem elastischem Kunststoff und bildet ein
biegsames Rohr, in welches der Flechtschlauch 11 sich
über die gesamte Länge dieses Schlauches 10
erstreckend eingebettet
ist. Dieser Flechschlauch 11 ist koaxial zur
ihn einbettenden Schicht 12 des Schlauches 9 und unterteilt diese Schicht
12 praktisch in zwei Unterschichten 12′, 12′′. Jedoch
ist der Flechtschlauch 11 so geflochten, daß seine
Fadenscharen rhombenförmige kleine Durchbrüche zwischen
sich freilassen, so daß die beiden Unterschichten 12′,
12′′ durch mit ihnen einstückige Kunststoffbrücken,
die durch die rhombenförmigen Durchbrechungen des
Flechtschlauches 11 hindurchführen, miteinander unlösbar
verbunden sind und so praktisch eine einzige rohrförmige
Schicht 12 mit eingebettetem Flechtschlauch 11 bilden.
Diese rohrförmige Schicht 12 ist umfaßt von der mit ihr
verklebten rohrförmigen Außenschicht 13 aus miteinander
verklebten, in einer oder mehreren Vorzugsrichtungen
orientierten und schräg zur Umfangsrichtung des Schlauches
verlaufenden textilen Fasern. Die Herstellung dieser
Außenschicht 13 kann wie in der DE-OS 28 06 030 beschrieben,
erfolgt sein.
In dieses Hohlorgan-Implantat 10, das beispielsweise eine Blutgefäßprothese
sein kann, kann nach seiner Implantation
infolge der ein Faservlies bildenden porösen Außenschicht
13 Körpergewebe einwachsen. Nachfolgend sei ein Herstellungsbeispiel
eines Hohlorgan-Implantats nach Fig. 1 beschrieben:
Ein als Kern dienender Stab (kreisrunder Querschnitt, 6 mm ⌀)
aus Wachs wird in eine Polyurethanlösung (10 Gew.-%
Polyurethan gelöst in Dimethylformamid) eingetaucht. Dabei
bildet sich eine homogene Schicht aus Polyurethan. Nach
Trocknen dieser Schicht wird ein vorgefertigtes Hohlgeflecht
aus rostfreien elastischen Stahlfäden (V4A-
Draht 0,06 mm ⌀), kreisrunder Querschnitt) (Geflechtskonstruktion:
40 Klöppel, Leinwandbindung, 30 Flechten pro cm)
auf die trockene Schicht aufgezogen. Dann wird der Stab
erneut in die 10 Gew.-% Polyurethanlösung getaucht und
darauf dann gemäß DE-OS 28 06 030 eine Vliesschicht
aus Polyurethanfasern durch Versprühen einer
Polyurethanlösung (8 Gew.-% Polyurethan in Dimethylformamid)
mittels Preßluft aufgebracht. Nach Austrocknung
der Schicht wird der Wachsstab durch Ausschmelzen entfernt
und das Hohlorgan-Implantat ist damit fertig. Es ist
weichelastisch, biegsam und hat sehr geringen kritischen
Krümmungsradius.
Das Hohlorgan-Implantat 10 nach Fig. 2 besteht aus einem
elastischen, biegsamen, porösen Schlauch 9 aus
miteinander verklebten, in einer oder mehreren Richtungen
vorzugsorientierten und schräg zur Umfangsrichtung des
Schlauchs 9 verlaufenden textilen Fasern, in
dessen mittleren Bereich ein wesentlich kürzerer, aus
monofilen dünnen Fäden aus Metall oder Kunststoff geflochtener Flechtschlauch 11
eingebettet ist. Die Herstellung eines solchen Hohlorgan-
Implantats 10 sei an einem Ausführungsbeispiel beschrieben:
Ein 50 cm langer Stab (4 mm ⌀, kreisrunder Querschnitt)
aus Wachs wird, wie in der DE-OS 28 06 030 beschrieben, durch Versprühen einer Polyurethanlösung
(8 Gew.-% Polyurethan gelöst in einem Gemisch aus Dimethylformamid/
Aceton 4 : 1) mittels Preßluft mit einer sich
über seine Länge erstreckenden Polyurethanfaservliesschicht
versehen. Darauf wird als Flechtschlauch ein vorgefertigtes
Hohlgeflecht aus Polyesterterephtalatmonofilen
(0,1 mm ⌀, kreisrunder Querschnitt. Geflechtskonstruktion:
32 Klöppel, Leinwandbindung, 15 Flechten pro cm)
aufgezogen, dessen Länge 10 cm beträgt. Dieser Flechtschlauch
wird in der Längsmitte des Stabes angeordnet.
Darauf wird eine weitere Schicht aus Polyurethanfaservlies
in derselben Weise wie oben beschrieben aufgebracht.
Danach wird der Wachsstab durch Schmelzen aufgelöst
und die Schlauchprothese ist fertig hergestellt.
Die Wanddicken der beiden Vliesschichten des Prothesenschlauches
betragen jeweils 0,3 mm. Die Gesamtwanddicke
beträgt 0,8 mm. Der kritische Biegeradius würde ohne
den Flechtschlauch 20 mm betragen und wird durch den
Flechtschlauch auf nur 2 mm reduziert.
Das Hohlorgan-Implantat 10 nach Fig. 3 besteht aus einem als
vorgefertigte, übliche, gewirkte, schlauchförmige
Doppelveloursprothese ausgebildeten Schlauch 9, auf den ein als Hohlgeflecht
hergestellter Flechtschlauch 11 aufgezogen ist. Die Herstellung
dieses Hohlorgan-Implantats sei ebenfalls an
einem Ausführungsbeispiel erläutert:
Der Flechtschlauch 11 kann aus Polyesterterephtalatmonofilen
(0,2 mm ⌀, kreisrunder Querschnitt) (Geflechtskonstruktion:
48 Klöppel, Leinwandbindung, 15 Flechten
pro cm, Innendurchmesser 12 mm) bestehen. Dieser Flechtschlauch
11 wird unmittelbar vor dem Aufziehen auf den
Schlauch 9 auf eine Temperatur von 195°C erhitzt
und bei dieser Temperatur ca. 5 Min. lang thermofixiert.
Der Flechtschlauch 11 wird anschließend noch mittels
Silikonelastomerkleber (medical grade) punktweise
(pro cm Länge vier Klebepunkte gleichmäßig am Umfang
verteilt) verklebt. Statt ein vorgefertigtes Hohlgeflecht
als Flechtschlauch 11 auf den Schlauch 9 aufzuziehen,
kann auch so vorgegangen werden, daß der Schlauch 9
zunächst auf einen kreiszylindrischen Stab formschlüssig
aufgeschoben wird und dann dieser Stab durch die Mittenbohrung
einer Drahtflechtmaschine mit gleichmäßiger
Geschwindigkeit geschoben wird, so daß der auf dem
Stab befindliche Schlauch 9 mit einem Flechtschlauch
umflochten wird. Bei einer im Versuch erfolgten Herstellung
eines solchen Hohlorgan-Implantats hatte die Flechtmaschine
32 Klöppel, die mit je einem monofilen Titanfaden
(0,1 mm ⌀, kreisrunder Querschnitt) bestückt waren.
Die Vorschubgeschwindigkeit des Stabes war so eingestellt,
daß 17 Flechten pro cm Stablänge gebildet wurden.
Auch dieses Hohlorgan-Implantat 10 ist elastisch biegsam und
hat sehr kleine kritische Krümmungsradien.
Das in Fig. 4 dargestellte Hohlorgan-Implantat 10 besteht
aus einem Schlauch 9 aus mikroporösem expandiertem Polytetrafluoräthylen,
auf das nur in seinem mittleren
Längsbereich ein wesentlich kürzerer Flechtschlauch 11
aufgeschoben und durch punktförmige Klebestellen auf
ihm unlösbar befestigt ist.
Die in den Ausführungsbeispielen dargestellten Hohlorgan-Implantate
haben über ihre Länge jeweils konstante Querschnitte
(Fig. 1-3) bzw. im wesentlichen konstanten Querschnitt (Fig. 4), vorzugsweise runde Querschnitte, doch ist es
auch möglich, ihre Querschnitte über ihre Länge ungleich
zu gestalten, und die Erfindung gegebenenfalls auch bei Hohlorgan-
Implantaten anzuwenden, die schlauchförmige Verzweigungen,
beispielsweise Aufspaltung des Schlauches in
zwei weiterführende kleinere Schläuche, aufweisen.
Claims (26)
1. Hohlorgan-Implantat, welches aus einem biegsamen
Schlauch und einem Geflecht besteht, dadurch
gekennzeichnet, daß das Geflecht aus mindestens
einem als Hohlgeflecht hergestellten Flechtschlauch
(11) aus biegsamen, elastischen Fäden
besteht.
2. Hohlorgan-Implantat nach Anspruch 1, dadurch
gekennzeichnet, daß der Flechtschlauch (11) aus
Metallfäden besteht.
3. Hohlorgan-Implantat nach Anspruch 1, dadurch
gekennzeichnet, daß der Flechtschlauch (11) aus
Fäden aus polymerem Kunststoff besteht.
4. Hohlorgan-Implantat nach Anspruch 1, dadurch
gekennzeichnet, daß der Flechtschlauch (11) aus
Metallfäden und Kunststoffäden besteht.
5. Hohlorgan-Implantat nach Anspruch 2 oder 4,
dadurch gekennzeichnet, daß die Metallfäden des
Flechtschlauches (11) aus rostfreiem Stahl oder
Titan bestehen.
6. Hohlorgan-Implantat nach einem der vorhergehenden
Ansprüche, dadurch gekennzeichnet, daß der Flechtschlauch
an der Innenoberfläche der Wandung des
Schlauches angeordnet ist.
7. Hohlorgan-Implantat nach einem der Ansprüche
1 bis 5, dadurch gekennzeichnet, daß der Flechtschlauch
(11) an der Außenoberfläche des
Schlauches (9) angeordnet ist (Fig. 3, Fig. 4).
8. Hohlorgan-Implantat nach einem der Ansprüche
1 bis 5, dadurch gekennzeichnet, daß der Flechtschlauch
(11) im Inneren der Wandung des
Schlauches (9) eingebettet ist (Fig. 1, 2).
9. Hohlorgan-Implantat nach einem der vorhergehenden
Ansprüche, dadurch gekennzeichnet, daß an mindestens
einem Längenbereich mehrere zueinander
koaxiale Flechtschläuche vorgesehen sind.
10. Hohlorgan-Implantat nach einem der Ansprüche 2
oder 4 bis 9, dadurch gekennzeichnet, daß die
Metallfäden des Flechtschlauches (11) Durchmesser
von 0,04 bis 0,15 mm, vorzugsweise von ungefähr
0,05 bis 0,07 mm aufweisen.
11. Hohlorgan-Implantat nach einem der Ansprüche 3 bis
9, dadurch gekennzeichnet, daß die Kunststoffäden
des Flechtschlauches (11) Durchmesser von
0,05 bis 1 mm, vorzugsweise von 0,4 mm aufweisen.
12. Hohlorgan-Implantat nach einem der vorhergehenden
Ansprüche, dadurch gekennzeichnet, daß zumindest
eine Teilanzahl der Fäden des Flechtschlauches
(11) monofile Fäden sind.
13. Hohlorgan-Implantat nach einem der vorhergehenden
Ansprüche, dadurch gekennzeichnet, daß es
mindestens ein schlauchförmiges Gewebe, Gewirke
oder Gestrick aus textilen Fasern aufweist.
14. Hohlorgan-Implantat nach einem der vorhergehenden
Ansprüche, dadurch gekennzeichnet, daß es
mindestens ein schlauchförmiges Vlies aus
miteinander verklebten textilen Fasern aufweist.
15. Hohlorgan-Implantat nach einem der Ansprüche
1-12, dadurch gekennzeichnet, daß der Schlauch
(9) aus einem Kunststoffschlauch (16),
vorzugsweise aus Polyätherurethan, Silikonelastomer
oder expandiertem Polytetrafluoräthylen,
besteht.
16. Hohlorgan-Implantat nach einem der Ansprüche
1-14, dadurch gekennzeichnet, daß der Schlauch
(9) aus einer inneren Schicht (12) aus polymerem
elastischem Kunststoff und einer mit ihr
verklebten äußeren Schicht (13) aus miteinander
verklebten Fasern besteht.
17. Hohlorgan-Implantat nach einem der Ansprüche 1
bis 14, dadurch gekennzeichnet, daß es mindestens

18. Hohlorgan-Implantat nach einem der vorhergehenden
Ansprüche, dadurch gekennzeichnet, daß der
Flechtschlauch oder mindestens ein Flechtschlauch
sich nur über einen Teil der Länge des Schlauches
(9) erstreckt (Fig. 4).
19. Hohlorgan-Implantat nach einem der vorhergehenden
Ansprüche, dadurch gekennzeichnet, daß es mehrere
in Längsabständen voneinander angeordnete
Flechtschläuche aufweist.
20. Hohlorgan-Implantat nach einem der vorhergehenden
Ansprüche, dadurch gekennzeichnet, daß das den
Flechtschlauch bildende Hohlgeflecht in Leinwandbindung
geflochten ist.
21. Hohlorgan-Implantat nach Anspruch 20, dadurch
gekennzeichnet, daß der Flechtwinkel
(durch Längsmittelebene des Hohlgeflechtes
halbierter Kreuzungswinkel der Fadenscharen) des
Flechtschlauches 60 bis 160°C, vorzugsweise 70 bis
110° beträgt.
22. Verwendung eines Hohlorgan-Implantats nach einem
der vorhergehenden Ansprüche als Blutgefäßprothese.
23. Verwendung eines Hohlorgan-Implantats nach einem
der Ansprüche 1 bis 21 als Speise- oder
Luftröhrenprothese.
24. Verwendung eines Hohlorgan-Implantats nach einem
der Ansprüche 1 bis 21 als Harnleiter- oder
Harnröhrenprothese.
25. Verwendung eines Hohlorgan-Implantats nach einem
der Ansprüche 1 bis 21 als Gallengangprothese.
26. Verwendung eines Hohlorgan-Implantats nach einem
der Ansprüche 1 bis 21 als Darmprothese.
