Vorrichtung zur Verformung der Hornhaut
Die Erfindung betrifft die Verformung bzw. Rückverformung der Hornhaut,und insbesondere eine Vorrichtung zur Verformung
der Hornhaut mit hochfrequenter elektrischer Energie.
Extreme Fälle von Brechungsfehlern, beispielsweise solche, die durch Hornhautkegel (Keratokonusse) hervorgerufen werden,
sind häufig mit einer zusätzlichen externen Brechung nicht zu korrigieren. In solchen Fällen wird üblicherweise
eine Hornhaut-Verpflanzung vorgenommen. In jüngster Zeit wurde vorgeschlagen, die Form der Hornhaut durch thermische
Verfahren zu verändern, die auf einer starken Schrumpfung der Hornhaut-Collagen in einem Temperaturbereich von 55 bis
650C beruhen. Der Anwendung dieser thermischen Verfahren
waren jedoch dadurch Grenzen gesetzt, daß das Epithel und die Bowman1sehe Membran bzw. die äußere Grenzmembran beschädigt
wurden, und daß nur zeitweilige Änderungen erreicht wurden. Diese Schwierigkeiten scheinen mit dem Verlauf
bzw. der Verteilung der thermischen Dosis in der Hornhaut zusammenzuhängen, wobei diese Verteilung oder der Verlauf
der Wärmedosis durch herkömmliche leitende. Aufheizeinrichtungen erzeugt wird. Die im Epithel auftretenden Temperaturen
sind dabei relativ hoch, wogegen die in den tieferen Stromal-Collagen erreichbaren Temperaturen unter der
erforderlichen kritischen Schrumpftemperatur liegen. Es
liegen bereits eine große Vielzahl von Behandlungsergebnissen vor, wobei diese Vielfalt der Behandlungsergebnisse
wahrscheinlich darauf zurückzuführen ist, daß ungewollte Schwankungen und Unterschiede bei der Handhabung und Anwendung
der Wärmebehandlungen bei den einzelnen Verfahren
auftreten, wobei insbesondere die Zeitdauer, während der die Hornhaut mit Wärme beaufschlagt wird, unterschiedlich
war.
In der US-Patentanmeldung No.100,664, die am 5.Dezember
1979 in. den USA eingereicht wurde und für die James D.Doss und Richard L.Hutson als Erfinder genannt wurden, sowie in
"A Technique for the Selective Heating df Corneal Stroma",
Contact and Intraoccular Lens Medical JrI., 6, No.1, S. 13-17
(Januar bis März 1980) ist eine Elektrode zur Hornhautverformung beschrieben. Die einpolige Elektrode wurde bei
verschiedenen Säugetieren mit zufriedenstellendem Erfolg angewandt.
Der Erfindung liegt die Aufgabe zugrunde, eine "Vorrichtung zur Verformung der Hornhaut durch Wärmebehandlung zu schaffen,
die die Nachteile herkömmlicher Vorrichtungen und Verfahren verbessert.
Diese Erfindung wird erfindungsgemäß mit der in Anspruch 1 angegebenen Vorrichtung gelöst.
Die in Anspruch 9 angegebene. Vorrichtung löst ebenfalls die gestellte Aufgabe.
Hit der in Anspruch 10 angegebenen Vorrichtung wird die gestellte Aufgabe ebenfalls gelöst.
Vorteilhafte Ausgestaltungen der Erfindung sind in den Unteransprüchen
angegeben.
Hit der vorliegenden Erfindung ist es möglich, komplexe
Hornhaut-Brechungsfehler, die ihrem Wesen nach nicht mit einer zusätzlichen äußeren Brechung korrigiert werden können,
zu korrigieren, ohne daß chirurgische Eingriffe er-
forderlich sind.
Darüberhinaus ist es mit der vorliegenden Erfindung möglich,
übliche Honrhautbrechungsfehler zu korrigieren und dadurch eine externe Korrektur zu vermeiden.
Sin Vorteil der vorliegenden Erfindung besteht darin, daß
das Hornhautgewebe an der Oberfläche wirkungsvoll gekühlt und dadurch geschützt wird, während das tiefere Hornhautgewebe
ausreichend stark aufgeheizt werden kann, um die Brechungsfehler in der Hornhaut zu korrigieren.
Ein weiterer Vorteil der Erfindung besteht darin, daß die in der Hornhaut hervorgerufenen Temperaturerhöhungen weit
genug in die Tiefe der Hornhaut vordringen, so daß auch Collagen-Fasern erreicht werden, und dadurch permanente Änderungen
der Hornhautform erzielt werden.
Hit der vorliegenden Erfindung ist es weiterhin nicht erforderlich,
entfernt angeordnete bzw. Fernelektroden zu verwenden. Die Hornhaut-Rück- bzw. Umverformung gemäß der vorliegenden
Erfindung kann nämlich durchgeführt werden, ohne daß eine Fernelektrode erforderlich ist. Dies bedeutet, daß
das Haar und die Haut am Hinterkopf des Patienten nicht naß gemacht und gesäubert werden muß, weil keine Fernelektrode
angelegt zu v/erden braucht. Da am Hinterkopf Iceine Fernelektrode
angelegt werden muß, fließt durch das Hirn und die Augennerven des Patienten auch ein wesentlich geringerer
Strom. Hit der vorliegenden Erfindung wird weiterhin auch vermieden, daß an den Rändern des Behandlungsbereichs der
in der US-Patentanmeldung 100,664 beschriebenen Hornhautverformungselektrode
hohe elektrische Felder auftreten. Bei Anwendung der vorliegenden Erfindung treten eine hohe Stromdichte
und eine hohe Kühlgeschwindigkeit an derselben Stelle auf der Hornhautoberfläche auf, so daß dadurch die Ober-
flächenwärme äußerst wirkungsvoll abfließen kann.
Bei der einpoligen Sonde gemäß der US-Patentanmeldung 100,664 fließt der elektrische Strom von der Sonde in eine
zur Hornhautoberfläche im wesentlichen senkrechten Richtung weg. Bei der vorliegenden Erfindung fließt der Strom
im wesentlichen parallel zur Hornhautoberfläche zwischen wenigstens zwei Elektrodenenden, obgleich unmittelbar unterhalb
einer Metallelektrode eine relativ große senkrechte Komponente des elektrischen Stroms vorhanden ist.
Wie erwähnt, besteht ein wesentlicher Vorteil der vorliegenden Erfindung also darin, daß nur ein sehr kleiner
Strom durch das Gehirn und die Augennerven des Patienten fließen muß. Darüberhinaus weist die vorliegende Erfindung
den Vorteil auf, daß am Rand der Behandlungszone keine
starken elektrischen Felder auftreten. Auch ist es bei der vorliegenden Erfindung von Vorteil, daß die hohen Stromdichten
und die hohen Kühlmittelströmungsgeschwindigkeiten notwendigerweise im selben Bereich auf der Hornhautoberfläche
auftreten, so daß daher alle Oberflächenschichten des Hornhautgewebes gut und sicher geschützt werden.
Die vorliegende Erfindung schafft also eine Vorrichtung zur Hornhautverformung mittels hochfrequenter Energie, mit einer
V/echselstromquelle, mehreren rohrförmigen Elektroden,
die Enden aufweisen und mit der Hochspannungsquelle verbunden sind, und mit einem Gehäuse, das mehrere Elektroden
und eine zwischen den Elektroden angeordnete elektrische Isolation umgibt. Die Spitzen oder Enden der Elektroden
sind nahe der der Behandlung zu unterziehenden Hornhaut, jedoch beabstandet von ihr positioniert. Ein flüssiges,
elektrisch leitendes Kühlmittel wird durch wenigstens eine der Elektroden oder nahe wenigstens einer der Elektroden
auf die Hornhaut .gelenkt und dann durch die andere Elektrode
oder in der Nähe der anderen Elektrode wieder von der Hornhaut abgenommen. Eine Dämmeinrichtung, beispielsweise eine
elastische Einfassung, hält das Kühlmittel auf der Hornhaut, so daß es nicht von der Hornhaut herunterfließen kann. Die
Elektroden können voneinander beabstandete, jedoch nahe beieinander
liegende Elektroden sein, wobei die Enden querschnittsmäßig vorzugsweise im wesentlichen oval oder rechteckig
sind. Bei einer solchen Ausführungsform kann die Isolation
als Trennelenent zwischen den Elektroden ausgebildet sein, wobei das Trennelement eine oder mehrere Aperturen oder
Löcher für das Kühlmittel aufweist. Das Loch kann eine Bogen- oder "Kamm"-Fora aufweisen. Bei Verwendung einer "Kamm"-förmigen
Öffnung bleibt der Abstand zwischen der Hornhaut und den Metallelektroden-Enden konstant. Es können zwei, drei
oder mehrere Elektroden vorgesehen sein, die in wesentlichen parallel zueinander und voneinander beabstandet angeordnet
sind. Die Elektroden können auch konzentrisch oder koaxial angeordnet sein, und es kann ein Abstandshalter, durch den
das Fluid.fließt, verwendet werden, um den Abstand zwischen der Elektrode und der Hornhaut konstant zu halten.
Die Erfindung wird nachstehend anhand der Zeichnungen beispielsweise
näher erläutert. Es zeigen:

In Fig. 9 ist eine Ausführungsform der Erfindung mit zwei
Elektroden schematisch dargestellt. Eine v/echselspannungsquelle
12, beispielsweise ein Hochfrequenzgenerator, der einen elektrischen Strom mit einer .Frequenz von 0,1 bis 20 mHz erzeugt,
ist mit Elektroden 14 und 16 verbunden. Es ist irgendein Hochfrequenzstrom mit einer Frequenz von etwa 100 kHz und
etwa 20 mHz geeignet, der während einer Dauer von etwa 1 bis
etwa 10 s zwischen etwa 20 und 200 V-., erzeugt. Weiterhin
können verschiedene Spannungs- und Strompegel und verschiedene Elektrodenabmessungen zur Aufheizung verschiedener Hornhautbereiche
und in verschiedenen Hornhauttiefen verwendet werden, wobei die Stromdichte normalerweise mit zunehmender
Hornhauttiefe abnimmt. Die Abnahme der Stromdichte ist im wesentlichen von der Elektrodenkonfiguration abhängig. Normalerweise
bewirken größere Elektroden eine tiefere Aufheizung. Ein elektrisch isolierendes Trennelement 18 trennt die Elektroden
14 und 16 voneinander. Die Elektroden werden mit dem isolierenden Trennelement über die Oberfläche einer Hornhaut
20 gebracht. Während des Betriebs kann ein elektrisch leitendes Kühlmittel mit einer Pumpe 90 von einem Behälter 96
durch eine Leitung 94 über die Oberfläche der Hornhaut 20 oder den Spitzen der Elektroden 14 und 16 und der isolierenden
Trennwand 18 hindurch über eine Leitung 92, die Pumpe und eine Leitung 98 zurück zum Behälter 96 gepumpt werden.
Die Strömungsrichtung des Kühlmittels ist durch einen Pfeil
100 angedeutet. Die gestrichelten Linien 22 geben das von den Elektroden 14 und 16 erzeugte hochfrequente elektrische Feld
wieder. Eine Einrichtung, mit der das Kühlmittel über die Hornhaut strömen gelassen wird, ist in Fig. 9 nicht dargestellt,
jedoch beispielsweise aus Fig. 1 zu ersehen, in der eine mit einer Elektrode 34 verbundene Einlaß-Schlauch- oder
Rohrleitung 94 und eine mit einer Elektrode 36 verbundene
Auslaß-Schlauch- oder Rohrleitung 92 dargestellt ist. Die Schlauch- oder Rohrleitungen 92,94 und 98 können beispielsweise
flexible, elektrisch isolierende Schläuche oder Röhrchen, beispielsweise Neopren-Schläuche oder -Röhrchen sein,
die üblicherweise für Flüssigkeiten verwendet werden.
Die leitende Kühl-LÖsung besteht vorzugsweise aus einer isotonischen
Salzlösung, die ,jedoch auch hypotonisch oder hyper-
tonisch ssin kann, um den elektrischen Widerstand der Salzlösung
gewünschtenfalls zu ändern. Wichtig ist dabei, daß die Salzlösung eine elektrische Leitung für den hochfrequenten
Strom zur Hornhautoberfläche bildet und für die Hornhautschichten nahe der Oberfläche als Kühlmittel wirkt und
dadurch das Oberflächengewebe beim Aufheizen tiefer liegender Hornhautgewebe schützt, wobei diese tiefer liegenden
Hornhautgewebe aufgeheizt werden müssen, um relativ dauerhafte Änderungen der Hornhautform hervorzurufen.
Ein noch ungelöstes Problem bei herkömmlichen Thermokeratophoresen
besteht darin, daß die im Epithel auftretende Wärmeenergie größer als im tieferen Stroma ist. In der US-Patentanmeldung
100,664 ist beschrieben, daß Wärme aus dem Epithel abgeleitet werden muß, wenn dieser Teil der Hornhaut nicht
überhitzt werden soll. Diese Schwierigkeit ist gemäß der US-Patentanmeldung 100,664 und bei der vorliegenden Erfindung
durch Verwendung eines elektrisch leitenden Kühlmittels gelöst.
Fig. 1 zeigt ein Gehäuse 24, das beispielsweise aus einem nichtleitenden Kunststoff bestehen kann. Das Gehäuse weist
vorzugsweise eine im wesentlichen zylindrische Form auf, so daß es vom Benutzer mit der Hand bequem über einem Auge gehalten
werden kann. An der Gehäusebasis ist um das Gehäuse herum eine elastische Einfassung 26 angebracht, die als
Dämmelement für das Kühlmittel dient. Die Einfassung 26 ist vorzugsweise der Oberfläche einer Hornhaut 30 im wesentlichen
angepaßt. Wie aus Fig. 1 weiter zu ersehen ist, ist eine Nut oder Rille 32 im Gehäuse 24 als Befestigungsmöglichkeit
für die Einfassung 26 vorgesehen, die einen Steg oder eine Rippe aufweist, welche in die Nut 32 einsetzbar
ist. Die Einfassung kann daher zur leichten Sterilisierung, zur Änderung der Einfassungsgrößen oder -abmessungen usw.
leicht ausgetauscht oder abgenommen werden. Im Gehäuse 24 sind rohrförmige Elektroden 34 und 36 vorgesehen, die mit
einer Umwälzpumpe 90 und einem Behälter 96 (vergl.Fig. 9) über flexible Rohr- oder Schlauchleitungen 92 bzw. 94 verbunden
sind. Das Kühlmittel tritt über die rohrförmige Elektrode 34 in das Gerät ein, es strömt dann über die Oberfläche
der Hornhaut 30 und tritt über die rohrförmige Elektrode 36 wieder aus. Zwischen den Elektroden 34 und 36 ist
ein isolierendes Trennelement 33 vorgesehen, wobei die beiden Elektroden 34 und 36 bei dieser Ausführungsform im wesentlichen
parallel und voneinander beabstandet angeordnet sind. Die Spitze des Isolators 38 berührt dabei nicht die
Hornhautoberfläche, so daß die Salzlösung von einer Elektrode über die Hornhautoberfläche in die andere Elektrode
fließen kann. Es können jedoch auch die in Fig. 3 und 4 dargestellten
Trennelementen-Enden verwendet werden. Fig. 3 zeigt eine bogenförmige Öffnung mit einem Rand 40. In Fig. 4
ist eine "kamm"-förmige Öffnung mit einem Rand 42.dargesteilt.
Der in Fig. 4 gezeigte Kamm hält die Hornhaut in einem festen Abstand von den Spitzen der Elektroden beabstandet. Dies ist
sehr vorteilhaft, da ein für den Kühlmittelfluß verwendeter Unterdruck bewirken kann, daß die Hornhaut in die Öffnung
gezogen wird. Fig. 2 zeigt die in Fig. 1 dargestellte Ausführungsform von unten mit der das Gehäuse 24 einfassenden
elastischen Einfassung 26 und dem Isolator 38 zwischen den
Elektroden 36 und 34. Wie aus Fig. 2 zu ersehen ist, weisen
die Spitzen der Elektroden einen im wesentlichen ovalen Querschnitt auf. Die bipolare Hornhäutelektrοde gemäß den Fig.. 1
und 2 kann auch als Doppel-Parallelröhren-Elektrode bezeichnet werden. Die Elektroden können aus rostfreiem Stahl
bestehen und mit Gold beschichtet werden, um eine Korrosion in der Salzlösungs-Umgebung zu verhindern. Das Kunststoffgehäuse
24 kann aus Acrylkunststoff oder aus Celluloseacetobutyrat-(C.A.B.-)Kunststoff bestehen. Die Einfassung besteht
üblicherweise aus einem Silicongummi, beispielsweise aus dem
Silikongummi der Firma Dow Corning mit der Bezeichnung 3110 RTV. Es sind jedoch auch andere geeignete Materialien
für die Elektroden, das Gehäuse, die Isolatoren usw. möglich.
Die in Fig. 3 und 4 dargestellten Spitzen der Isolierungs-Trennelemente
können auch bei den in den Fig. 5 und 6 dargestellten
Ausführungsformen verwendet werden. Die Fig.5 und 6 zeigen ein isolierendes Gehäuse 50, das im wesentlichen
zylindrisch ist, so daß es vom Benutzer mit der Hand bequem gehalten werden kann. Im Gehäuse 50 befinden sich drei Elektroden
52,54 und 56, die durch Isolations-Trennelemente 53 und 60 voneinander getrennt sind. Bei den Merhfachelektrodengeräten,
beispielsweise bei dem in den Fig. 5 und 6 dargestellten Quadropol-Beispiel, sind benachbarte Elektroden
grundsätzlich mit den jeweiligen entgegengesetzten Klemmen der Hochfrequenzquelle verbunden. Beispielsweise sind die
Elektroden 52 und 56 in Fig. 5 miteinander und mit einer
Klemme der Hochfrequenzquelle verbunden. Die Mittelelektrode 54 ist an die andere Klemme der Hochfrequenzquelle angeschlossen.
Die elastische Einfassung 62 ist um das untere Ende des Gehäuses 50 herum befestigt und kann entsprechend
dem in den Fig. 1 und 2 dargestellten Ausführungsbeispiel mit entsprechenden Stegen oder Rippen und Nuten oder auch
durch Reibwirkung oder auf andere Weise angebracht sein. Die Unterseite des Gehäuses kann auf der Oberfläche einer Hornhaut
64 aufliegen. Bei dieser Ausführungsform wird das Kühlmittel vorzugsweise über die Mittele.lelektrpde 54 eingeleitet,
strömt unter den Isolatorbrennelementen 58 und 60 hindurch und wird von den seitlichen Elektroden 52 und 56 wieder von
der Hornhautoberfläche abgesaugt. Das Kühlmittel kann jedoch alternativ auch in umgekehrter Richtung strömen. Es sind
elektrische Feldlinien 66 dargestellt. Wie aus Fig. 6 zu ersehen ist, kann das Gehäuse 50 bei dieser Ausführungsform
einen im wesentlichen ovalen Querschnitt wie die Spitzen der Elektroden 53,54 und 56 aufweisen. Der Vorteil der in
den Fig. 5 und 6 dargestellten Multipolar-Elektrode, die auch als Quadropol bezeichnet wird, weil die Mittelelektrode
als zwei Pole zählt, besteht darin, daß größere Hornhautbereiche aufgeheizt, werden können, ohne daß dabei eine
unerwünschte Aufheizung in größeren Tiefen erfolgt, was bei einer Anordnung mit nur zwei (größeren) Elektroden der
Fall sein könnte. Bei der in den Fig. 5 und 6 dargestellten Ausführungsform sowie bei der Ausführungsform gemäß
den Fig. 7 und 8 können dieselben Materialien verwendet werden wie bei der Ausführungsform gemäß den Fig. 1 und 2.
Die Fig. 7 und 8 zeigen eine konzentrische dipolare Elektrodenanordnung
mit einem Gehäuse 70 und konzentrischen
rohrförmigen Elektroden 72 und 74, die durch ein rohrförmiges Isolationselement 76 voneinander getrennt sind, wobei
dieses Isolationselement 76 ebenfalls zu den beiden Elektroden konzentrisch angeordnet ist. Es wird eine elastische
Einfassung 82 verwendet, die der Einfassung gemäß den zuvor beschriebenen Ausführungsbeispielen entspricht. Das
durch die Mittelelektrode 72 eingeleitete Kühlmittel strömt durch die Öffnung 84 auf und dann über die Oberfläche der
Hornhaut 78 und wird zwischen dem Gehäuse 70 und der äußeren Fläche der äußeren Elektrode 74 wieder abgesaugt. Die
rohrförmige Öffnung zwischen der Elektrode 74 und dem Gehäuse 70 steht mit einer Leitung 80 in Verbindung, durch
die das Kühlmittel abgesaugt wird. Die elektrischen Feldlinien 102 stellen das elektrische Hochfrequenzfeld dar,
das durch die Elektroden dieser Ausführungsform in der Hornhaut erzeugt wird. Fig. 4 zeigt eine Darstellung der Anordnung
von unten, bei der die elastische Einfassung 82, das Gehäuse 70, die äußere Elektrode 74, das rohrförmige Isolierelement
76 und die innere Elektrode 72 sichtbar sind.

Ein Vorteil dieser besonderen Elektrodenanordnung besteht darin, daß ein Ring - oder eintorusförmiger Behandlungsbereich erzeugt werden kann, da der elektrische Strom im
wesentlichen in einem torusförmigen Raum unter und zwischen den Elektroden 73 und 74 fließt. Ein solcher Behandlungsraum
läßt eine unbehandelte und relativ geschützt Zone im Mittelbereich des Torus frei, so daß diese Zone in der
Praxis zum Schutz des Hornhautsbereichs dienen kann, der sich direkt über der Pupille des Auges befindet.
Nachfolgend wird Fig. 10 beschrieben. Wenn Unterdruck verwendet wird, um das Kühlmittel über die Hornhautoberfläche
fließen zu lassen, kann es oft wünschenswert sein, die in den Fig. 7 und 8 dargestellte Sonde mit einem in den Fig.
10 bis 13 dargestellten isolierenden Abstandshalter 86 zu versehen, der an der Spitze der inneren Elektroden 72 angebracht
ist. Der Abstandshalter dient dazu, die Hornhaut 78 in einem geeigneten Abstand von den in Fig. 7 dargestellten
Elektroden 72 und 74 zu halten, da durch den zum Umwälzen des Kühlmittels verwendeten Unterdruck die Hornhaut
zu den Elektroden hin gezogen werden kann. In den Fig. 10 bis 13 ist ein typischer Abstandshalter 86 im
einzelnen dargestellt. Fig. 10 zeigt den Abstandshalter 86, wie er auf der Hornhautoberfläche 78 aufliegt und an
der Innenfläche der Innenelektrode 72, die. auch in den Fig. 7 und 8 dargestellt ist, befestigt ist. In dem in den
Fig. 10 bis 13 dargestellten Ausführungsbeispiel besitzt der Abstandshalter 86 ein Loch 88, das mit mehreren radialen
Auslaßöffnungen 87 in Verbindung steht, so daß das Kühlmittel durch den Abstandshalter und über die Hornhaut
fließt.
Bei allen drei dargestellten Ausführungsbeispielen ist die Strömungsgeschwindigkeit des konvektiven Kühlmittels, d.h.
der Salzlösung in dem Bereich der Hornhautoberfläche am größten, an dem auch die Intensität des elektrischen Feldes
am höchsten ist. Dies ist äußerst vorteilhaft, weil dadurch die wichtigsten Oberflächengewebe der Hornhaut inherent
geschützt werden.
Die zweipolige Sonde, die dem in den.Fig. 1 und 2"dargestellten
Ausführungsbeispiel entspricht und eine Isolatorspitze entsprechend der in Fig. 3 dargestellten Isolatorspitze aufweist, wurde an mehreren Hornhäuten von Kaninchen
getestet. Die einzige wesentliche experimentelle Schwierigkeit hing mit der Tatsache zusammen, daß das durch Unterdruck
umgewälzte Kühlmittel das Eindringen der Netzhaut in die Öffnung bewirkte, wodurch der Kühlmittelfluß teilweise
unterbunden wurde. Diese Schwierigkeit wurde später durch die Verwendung einer Öffnung umgangen, die der in Fig. 4
dargestellten Öffnung entsprach. Die Experimente waren dennoch erfolgreich. Es wurden bemerkenswerte Einflüsse auf
das endothele Gewebe festgestellt, die hauptsächlich den zusätzlichen mechanischen Beanspruchungen und Belastungen
zugeschrieben wurden, die dadurch auftraten, daß der Unterdruck die Hornhaut teilweise in die Öffnung zwischen die
Elektroden zog. Es wurden weitere, den in den Fig. 1 und dargestellten Sonden entsprechende Sonden gebaut. Eine in
Fig. 4 dargestellte "Kamm"-Öffnung wurde hergestellt. Bei
weiteren Tests wurde festgestellt, daß die Funktion dieser AusfUhrungsform der vorliegenden Erfindung ausgezeichnet
war. Die ausgezeichnete Funktion und Wirksamkeit wurde offensichtlich durch die Verwendung der in Fig. 4 dargestellten
"Kamm"-Öffnung erreicht. Diese Öffnung hält die Hornhaut in einem geeigneten Abstand von den Elektroden entfernt und
ermöglicht einen ausreichend großen Kühlmittelfluß über der Hornhautoberfläche. Die Kaninchen-Hornhäute, die in einem
zweiten Experiment behandelt wurden, wiesen nicht die zwar

geringen Beschädigungen auf, die bei dem früheren Experiment durch mechanische Beanspruchung des Endothels hervorgerufen
wurden. Diese Tatsache ist wahrscheinlich durch die Wirkung der "Kamm"-Öffnung zu erkären, die verhindert,
daß die Hornhaut durch den für den Kühlmittelfluß benötigten Unterdruck zu den Elektroden hin gezogen wird.
Für die in den Zeichnungen dargestellten verschiedenen Ausführungsformen
können geeignete Konfigurationen des elektrischen Feldes berechnet werden, um zu ermitteln, welche
Ausführungsform für eine bestimmte Art der Hornhautbehandlung am besten geeignet ist.
Die Erfindung wurde anhand von Ausführungsbeispielen beschrieben.
Dem Fachmann sind jedoch zahlreiche Abwandlungen und Ausgestaltungen möglich, ohne daß dadurch der Erfindungsgedanke
verlassen wird.

Claims (10)
- Vorrichtung zur Verformung der HornhautPatentansprüche:Vorrichtung zur Verformung der Hornhaut mit hochfrequenter elektrischer Energie,
gekennzeichnet durch- eine Wechselspannungsquelle (12),_ mehrere Enden aufweisende Elektroden (14,16;34,36;52,54,56;72,74) die mit der Wechselspannungsquelle (12) verbunden sind,- ein Gehäuse (24;5Ö;70), das die Elektroden (14,15; 34,36;52,54,5b;72,74) umschließt,- Isolierelemente· (38;58,βΟ;76), die im Gehäuse (24;50; 70) zwischen den Elektroden (14 bzii/. 1β;34 bzw. 36; 52 bzw. 54 bzw. 56;72 bzw. 74) angeordnet sind,- eine Einrichtung (26;62;82), mit der dio Elektrodenenden nahe einer Hornhaut (30;64;78), jedoch von ihr beabstandet, positioniert werden,- Einrichtungen (80;92,94), mit denen ein elektrisch leitendes, flüssiges Kühlmittel in der Mähe einer der• ν ο » »Elektroden (14,16;34,36;52,54,56;72,74) auf die Hornhaut (30;64;78), über diese und nahe einer anderen Elektrode (14,16j;34,36;52,54,56;72,74) von ihr wegfließen gelassen wird, und ■- Dämm-Einrichtungen (26;62;82), die das Kühlmittel auf der Hornhaut (30;64;78) halten, so daß es nicht wegfließt. ' - 2. Vorrichtung nach Anspruch 1, dadurch gekennzeichnet, daß die Dämm-Einrichtungen (26;62;82) eine Einfassung am Gehäuse (24;50;70) sind.
- 3. Vorrichtung nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß die Einfassung elastisch ist.
- 4. Vorrichtung' nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß die Einfassung am Gehäuse (24;50;70) abnehmbar befestigt ist.
- 5. Vorrichtung nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß das Gehäuse (24;50;70) aus einem isolierenden Material besteht.
- 6. Vorrichtung nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, daß die Enden der Elektroden (14,16; 34,36;52,54,56;72,74) einen im wesentlichen normalen Querschnitt aufweisen.
- 7. Vorrichtung nach einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, daß die Elektroden (14,16;34,36;52,54,56; 72,74) im wesentlichen parallel zueinander angeordnet sind.
- 8. Vorrichtung· nach einem der Ansprüche 1 bis 7, dadurchgekennzeichnet, daß die Fluidströmungseinrichtungen (80; 92,94) Einrichtungen -umfassen, mit denen das Kühlmittel durch die Elektroden (14,16;34,36;52,54,56;72,74) fließen gelassen wird.
- 9. Vorrichtung zur Verformung der Hornhaut mit hochfrequenter elektrischer Energie, gekennzeichnet durcheine Wechselspannungsquelle (12), eine erste, zweite und dritte rohrförmige Elektrode (52,54,56), die jeweils ein Ende aufweisen und mit der Viechselspannungsquelle (12) verbunden sind,- Isolierelemente (58,60), die zwischen den Elektroden (54,54,56) angeordnet sind und diese gegeneinander isolieren,- ein Gehäuse (50), das die Elektroden (52,54,56) und die Isolierelemente (58,60) umschließt,- Einrichtungen (62), mit denen die Elektrodenenden in einem gewählten Abstand von der Oberfläche der Hornhaut (64) positioniert werden,- Einrichtungen, mit denen ein elektrisch leitendes, flüssiges Kühlmittel durch wenigstens eine (54) der Elektroden (52,54,56) auf und über die Hornhaut (64) und dann von wenigstens einer (52,56) der Elektroden (52,54,56) weggeleitet wird, und- eine Einrichtung (62), die das Kühlmittel auf der Hornhaut (64) hält (Fig. 5 und 6).
- 10. Vorrichtung zur Verformung der Hornhaut mittels hochfrequenter elektrischer Energie, gekennzeichnet durch- eine Wechselspannungsquelle"(12),eine innere rohrförmige Elektrode (72), die ein Ende aufweist und mit der Wechselspannungsquelle (12) verbunden ist,
eine äußere rohrförmige Elektrode (74), die ein Endeaufweist, rait der Wechselspannungsquelle (12) verbunden ist und die erste rohrförmige Elektrode (72) in einem Abstand von ihr ringförmig umschließt, ein ringförmiges Isolierelement (7o), das die innere und äußere Elektrode (72,74) voneinander isoliert, ein Gehäuse (70), das die innere und äußere Elektrode (72,74) sowie das Isolierelement (76) umschließt, eine Einrichtung (82), mit der die Enden der inneren und äußeren Elektroden in einem gewählten Abstand von der Hornhaut (78) positioniert wird, Einrichtungen, mit denen ein flüssiges Kühlmittel auf die Hornhaut (78) und von ihr weggeleitet werden, und eine Einrichtung (82),die das Kühlmittel auf der Hornhaut (78) hält (Fig. 7 und 8).
