Description
Die Erfindung geht aus von
einem Verfahren zur Herstellung eines Bläschen
enthaltenden, biokompatiblen flüssigen Kontrastmittels für
Ultraschall-Bildgebungsverfahren.
Es existieren verschiedene Techniken, mit denen Teile des
tierischen oder menschlichen Körpers abgebildet werden, als
Diagnose- oder Therapiehilfe. Eine dieser Techniken ist die
Utraschall-Bilderzeugung. Bei der Abbildung des Herzens mit
Ultraschall spricht man von "Echokardiographie".
Ultraschallscanner für Ultraschall-Bildgebungsverfahren sind
im Stande der Technik bekannt. Ein Ultraschallscanner erzeugt
und empfängt Schallwellen. Er wird auf der Körperoberfläche
über dem abzubildenden Bereich aufgesetzt. Die durch den Scan
ner erzeugten Schallwellen werden auf den abzubildenden Be
reich gerichtet. Der Scanner fängt dann die von dem darunter
liegenden Bereich reflektierten Schallwellen auf und setzt
diese Daten in Bilder um. Wenn Ultraschallenergie durch eine
Substanz transmittiert wird, hängen die akustischen Eigen
schaften der Substanz von der Geschwindigkeit der Transmission
und der Dichte der Substanz ab. Änderungen in den akustischen
Substanzeigenschaften (oder dem akustischen Widerstand) sind
besonders an den Grenzschichten zwischen verschiedenen Sub
stanzen und Aggregatszuständen ausgeprägt. Durch die Änderung
der akustischen Eigenschaften beim Durchtritt von Ultraschall
energie durch verschiedene Medien ändern sich die Reflexions
charakteristiken, was ein durch den Ultraschallscanner empfan
genes intensiveres Schallreflexionssignal ergibt.
Bisherige Ultraschall-Bilderzeugungsverfahren, wie Echokardio
gramme litten unter ungenügender Schärfe. Aus diesem Grunde
wurden verstärkte Anstrengungen unternommen, um die Ultra
schallscanner und die zugehörigen Einrichtungen zu verbessern.
Zusätzlich wurden, beginnend 1968, Kontrastmittel in den Blut
strom injiziert, um klarere und "verstärkte" Ultraschallbilder
zu erhalten. Einige bekannte Kontrastmittel sind in dem Arti
kel von J. Ophir u. a. "Ultrasonic Backscatter from Contrast
Produced by Collagen Microspheres" in Ultrasonic Imaging by
Academic Press, Inc. 1980, beschrieben. Als Kontrastmittel
bekannt sind auch Mikro-Gasbläschen enthaltende Flüssigkeiten,
die manchmal mit Gelatine oder Saccharin eingekapselt und
manchmal durch mechanische Bewegung, d. h. durch Schütteln von
Hand und Mischen verschiedener Flüssigkeiten, hergestellt
werden.
Aus der US-PS 42 76 885 ist es bekannt, ein Mikro-Gasbläschen
enthaltendes Kontrastmittel zu erzeugen, indem Gas, z.B.
Stickstoff, durch eine Kapillare in eine Gelatinelösung gebla
sen wird. Die Größe der Gasbläschen kann beispielsweise durch
den Gelatine-Flüssigkeitsstrom geregelt werden und liegt unter
300 µm. Kennzeichnend für die Merkmale nach der US-PS 42 76 885
ist ferner, daß die Bläschen durch eine die Koagulation
verhindernde Oberflächenmembran eingekapselt sind, wie sie
z.B. aus einer Gelatineschicht gebildet wird. Nachteilig an
dem Verfahren ist, daß gerade die gewünschten besonders klei
nen Bläschen, die auch feine Blutgefäße passieren können und
eine bessere Bildauflösung ergeben, durch diese Kapillarströ
mungsmethode kaum zu erzeugen sind und daß auch die Größenver
teilung der erzeugten Mikrobläschen für eine gute Bildqualität
noch zu breit ist.
Aus der US-PS 42 47 406 sind als magnetisches Kontrastmittel
verwendbare, magnetisch lokalisierbare und biologisch abbaubare
Polymerpartikel bekannt. Dabei wird ferromagnetisches Material
in eine Aminosäure-Polymermatrix eingeführt. Die Mikropartikel
werden in Emulsionspolymerisation hergestellt, wobei die
Tröpfchengröße der wäßrigen Phase in Öl durch beschallen
vermindert werden kann. Auf Gasbläschen, die im Hinblick auf
den akustischen Kontrast besonders interessant sind, wird
nicht eingegangen.
Der Erfindung liegt demgegenüber die Aufgabe zugrunde, ein
Verfahren zur Herstellung eines Bläschen enthaltenden, biokom
patiblen flüssigen Kontrastmittels für Ultraschall-Bildge
bungsverfahren zur Verfügung zu stellen, mit dem auf einfache
Weise hochstabile Mikrobläschen in enger Größenverteilung
erzeugt werden können, die klein genug sind, um enge Blutge
fäße passieren zu können.
Zur Lösung dieser Aufgabe sieht die Erfindung vor, daß eine
viskose, biokompatible Lösung bei einer Frequenz zwischen 5000
und 30 000 Hz unter Bildung von Mikrobläschen beschallt wird
und daß das Beschallen solange durchgeführt wird, bis eine
Dispersion von Mikrobläschen mit im wesentlichen einheitlichen
Durchmesssern, die 20 µm nicht überschreiten, entstanden ist.
Das Kontrastmittel nach der vorliegenden Erfindung ist (1)
echofähig (d. h. fähig, Schallwellen zu reflektieren) und (2)
durch geringe Bläschengröße in der Lage durch solche Kapilla
ren hindurchzufließen und solche Gewebe zu perfundieren, die
für bekannte in eine periphere Vene injizierte Bläschen ent
haltende Kontrastmittel undurchdringbar waren. Dadurch werden
verbesserte Bilder solcher Gewebe und Organe erzeugt und eine
Unterscheidung zwischen gut perfundierten und schlecht perfun
dierten Geweben ermöglicht. (3) sollen die neuen Kontrast
mittel quantifizierbar reproduzierbar sein.
Das Verfahren der vorliegenden Erfindung erlaubt (1) die Ab
bildung von Organsystemen, die unter Benutzung der bekannten
Ultraschallverfahren nicht abgebildet werden konnten, und (2)
die klarere und detailliertere bildliche Darstellung von ge
wissen Bereichen, die bereits mit bekannten Verfahren sichtbar
zu machen waren.
Bei der vorliegenden Erfindung wird eine viskose Lösung (z.B.
70% Dextrose, 50% Dextrose, 70% Sorbitol, Renogratin-76, Mi
schungen dieser Mittel und dergleichen) einer hochfrequenten
(5000 bis 30 000 Hz) Ultraschallenergie ausgesetzt. Dadurch
werden Mikrobläschen mit einem Durchmesser von angenähert 6
bis 20 µm erzeugt. Zur Erleichterung der Bezugnahme werden
diese Mikrobläschen im folgenden als "beschallte" Mikrobläs
chen bezeichnet.
Die Kontrastmittel der vorliegenden Erfindung werden durch
eine konventionelle Ultraschallscannereinrichtung erfaßt und
in der oben beschriebenen Weise in Bilder umgesetzt.
Neben der Überwindung vieler der mit dem Stand der Technik
verbundenen Probleme, macht die vorliegende Erfindung die
Erzeugung von neuartigen Bildern verschiedener Organsysteme
möglich. Obwohl das erfindungsgemäßeKontrastmittel auf verschiede
ne tierische und menschliche Körperorgansysteme anwendbar ist,
werden seine neuen Merkmale und Vorteile durch die folgende
Beschreibung beispielhaft in Anwendung auf die Erzeugung von
Bildern des myokardialen Gewebes und Perfusions- oder Blut
flußmustern erläutert.
Das Herz ist eine "Pumpe" die von vielen Blutgefäßen versorgt
wird. Im Laufe der Zeit können diese teilweise oder total
blockiert werden, was eine Schädigung des Herzgewebes verur
sacht. In der Vergangenheit wurden Informationen über das
Herzgewebe unter Verwendung der Radio-Nuklid-Bilderzeugung und
durch die Chirurgie gewonnen. Das Angiogramm erzeugt keine
direkten, das Gewebe betreffenden Daten, sondern erfordert die
Aufzeichnung von Inferenzen der erhaltenden Daten im Hinblick
auf die großen Blutgefäße und die Wandbewegungen des Herzens.
Die Erfindung wird im folgenden unter Bezugnahme auf die bei
gefügten Zeichnungen beschrieben. Es zeigen:
Fig. 1 in schematischer Darstellung die Anwendung eines
Ultraschallscanners in der Echokardiographie; und
Fig. 2 bis 5 Querschnittsbilder des Herzens, die durch die
Verwendung von das Herz durchfließendem Kontrastmit
tel verbessert sind.
Fig. 1 ist eine schematische Darstellung des Herzens und der
Lunge sowie einer einen Scanner 10 und eine Abbildungsvorrich
tung 12 aufweisende Ultraschallscannereinrichtung. Die Ein
richtung erzeugt sichtbare Bilder eines vorbestimmten Berei
ches, in diesem Fall der Herzregion eines menschlichen Kör
pers. Typischerweise ist der Scanner direkt auf der Haut 14
über dem abzubildenden Bereich 16 angeordnet. Der Scanner 10
enthält verschiedene elektronische Komponenten, einschließlich
Ultraschallwandlern. Der Scanner 10 erzeugt Ultraschallwellen
18, die eine Sektorabtastung der Herzregion 16 durchführen.
Die Ultraschallwellen 18 werden von den verschiedenen Teilen
der Herzregion 16 reflektiert, durch den erzeugenden Wandler
empfangen und mit bekannten Pulsechoverfahren verarbeitet.
Nach der Bearbeitung werden Signale zur Betrachtung zu der
Abbildungsvorrichtung 12 übertragen (ebenso im Stande der
Technik bekannt).
Nach Vorbereitung des
Patienten und Anbringung des Scanners 10 wird das beschallte Mikro
bläschen enthaltende Kontrastmittel z. B. durch eine Armvene,
allgemein bei 24 angezeigt, injiziert. Das Kontrastmittel
fließt durch die Vene 24 in Richtung des Pfeiles 26 durch die
rechte venöse Seite 28 des Herzens 30, durch die zu den Lungen
32 führende Lungenschlagader 29, durch die Lunge 32, durch die
Kapillaren 34, durch die Lungenvene 35 und schließlich in das
linke Atrium 36 und den linken Ventrikel 37 des Herzens 30.
Es wurde gefunden, daß die Verwendung von beschallten Mikro
bläschen Bilder erzeugt, die kräftige Kontrastbereiche aufwei
sen.
Im folgenden wird auf die Fig. 2 bis 5 Bezug genommen. Dort
kann man ein Kontrastechokardiogramm sehen, das sich bei der
Benutzung des beschallten Mikrobläschenkontrastmittels der
vorliegenden Erfindung ergibt. In den Figuren stellt ein huf
eisenförmiger Teil 50 den linken ventrikularen Wandmuskel
(oder Gewebe), der die linke ventrikulare Kammer 37 um
schließt. Die Mikrobläschen wurden in die Pulmonararterie
eines Hundes injiziert und kreuzten das Kapillargefüge der
Lunge, um in das linke Atrium 36 und die linke ventrikulare
Kammer 37, in die Aorta durch die Koronararterien und eventu
ell in das linke ventrikulare Gewebe 50 einzutreten, wodurch
das Bild verstärkt wird.
Fig. 2 zeigt ein zweidimensionales Echokardiogramm ("2-DE")
-Bild der linken ventrikularen Kammer 37 und des linken Atri
ums 36 vor der Einführung der Mikrobläschen. Fig. 3 zeigt die
Injektion von 10 ml einer beschallten Renografin/NaCl-Mischung
durch einen festgelegten Pulmonararterien-Katheter. Wie man
sehen kann, erscheint das Kontrastmittel im linken Atrium 36
und fließt in den linken Ventrikel 37. In Fig. 4 ist eine im
wesentlichen vollständige Abschattung der linken ventrikularen
Kammer 37 erreicht. In Fig. 5 ist die nachfolgende Abschat
tung des myokardialen Gewebes 50 zu sehen. Dieses ist der
Fall, weil das das Kontrastmittel transportierende Blut durch
die Aorta in die Koronarartieren geflossen ist, die das Blut
dem myokardialen Gewebe 50 zuführen. So können Beobachtungen
und Diagnosen gemacht werden mit Bezug auf die Durchlaufzeit
des Blutes durch die Lungen, auf Blutflußmuster, auf die Größe
des linken Atriums, auf den Zustand der Mitralklappe (die das
linke Atrium und den linken Ventrikel trennt), auf Kammerab
messungen in der linken ventrikularen Kammer und auf Abnorma
litäten in der Wandbewegung. Beim Ejizieren des Kontrastmit
tels aus dem linken Ventrikel kann der Zustand der Aortaklappe
ebenso analysiert werden, wie der Anteil oder der Prozentsatz
des aus dem linken Ventrikel ejizierten Volumens. Schließlich
zeigen die Kontrastmuster im Gewebe an, welche Bereiche -
sofern überhaupt - nicht geeignet perfundiert sind.
Zusammengefaßt hilft ein solches Muster von Bildern, unübliche
Blutflußcharakteristiken innerhalb des Herzens, Klappenunvermö
gen, Kammergrößen und Wandbewegungen zu diagnostizieren, und
es liefert einen potentiellen Indikator für myokardiale Per
fundierungen.
Im obigen Beispiel sind die Mikrobläschen aus einer Mischung
von Renographin-76 (einem relativ ungiftigen, biologisch ver
träglichen bekannten Strahlenkontrastmittel, und einer Salzlö
sung im Verhältnis 1 : 1 hergestellt. Diese Mischung wurde be
schallt, d.h. durch einen 375-Watt-Heizsystem-Beschaller für
30 Sekunden einer Hochfrequenzenergie ausgesetzt. Derartige
Beschaller sind für andere Verwendungszwecke bekannt und emit
tieren gewöhnlich Ultraschallenergie bei 20 000 Hz, obwohl
Ultraschallfrequenzen von 5000 bis 30 000 Hz oder höher im
Rahmen der vorliegenden Erfindung möglich sind. Abhängig von
dem gewählten Kontrastmittel, z. B. der oben beschriebenen
Mischung, Zuckerlösungen oder dergleichen, werden unterschied
liche Bläschengrößen erzeugt, die gewöhnlich in dem gewünsch
ten Bereich von etwa 6 bis 20 µm im Durchmesser liegen.
Neben dem oben kurz beschriebenen Scanner 10 existieren noch
andere Ultraschallscanner, wie sie z.B. in den US-PS 41 43 554
und 43 15 435 beschrieben sind. Grundsätzlich betreffen
diese Patente verschiedene Techniken, einschließlich der dyna
mischen Querschnittsechographie (DCE) zur Erzeugung sequenti
eller zweidimensionaler Bilder von Querschnittsscheiben der
tierischen oder menschlichen Anatomie mit Ultraschallenergie
bei einer Bildwechselfrequenz, die ausreichend hoch ist, um
eine dynamische Sichtbarmachung von bewegten Organen zu ermög
lichen. Die Erfindung ist mit allen diesen Scannern anwendbar.
Claims (6)
1. Verfahren zur Herstellung eines Bläschen enthaltenden,
biokompatiblen flüssigen Kontrastmittels für Ultraschall-Bild
gebungsverfahren, dadurch gekennzeichnet, daß eine viskose,
biokompatible Lösung bei einer Frequenz zwischen 5000 und
30 000 Hz unter Bildung von Mikrobläschen beschallt wird und
daß das Beschallen solange durchgeführt wird, bis eine Dis
persion von Mikrobläschen mit im wesentlichen einheitlichen
Durchmessern, die 20 µm nicht überschreiten, entstanden ist.
2. Verfahren nach Anspruch 1, dadurch gekennzeichnet, daß
als dispergierende Lösung eine viskose wäßrige Lösung verwen
det wird.
3. Verfahren nach Anspruch 1 oder 2, dadurch gekennzeichnet,
daß Mikrobläschen gebildet werden, deren Durchmesser in der
Größenordnung zwischen 6 und 20 µm liegen.
4. Verfahren nach einem der Ansprüche 1 bis 3, dadurch ge
kennzeichnet, daß die Beschallungsfrequenz bei 20 000 Hz
liegt.
5. Verfahren nach einem der Ansprüche 1 bis 4, dadurch ge
kennzeichnet, daß die viskose, biokompatible Lösung aus der
aus Dextrose, Sorbitol, einem relativ ungiftigen Strahlenkon
trastmittel und deren Mischungen bestehenden Gruppe ausgewählt
wird.
6. Verfahren nach Anspruch 5, dadurch gekennzeichnet, daß
die viskose Lösung eine Mischung aus relativ ungiftigem Strah
lenkontrastmittel und Natriumchloridlösung ist.
