Description
Die Erfindung betrifft Polstermaterial für chirurgische Nähte, das aus einer Matte aus kontinuierlich gesponnenen Mikrofäden
aus segmentierten Polyurethanen hergestellt ist, sowie die Kombination dieses Polstermaterials mit chirurgischem
Nahtmaterial.
Bei zahlreichen chirurgischen Eingriffen, insbesondere bei solchen, die Nähte in sehr zerreißlichem Körpergewebe erfordern,
wird im allgemeinen ein Polstermaterial verwendet. Dies soll verhindern, daß die Naht in das Gewebe einschneidet.
Die Verwendung von Polstern bei Herzklappenoperationen ist beispielsweise in der US-PS 4 164 046 (Figur 6, Ausschnitt
40) beschrieben. Derartige Polster müssen folgende Eigenschaften aufweisen: Sie sollen weich sein, ausreichende }
Reißfestigkeit besitzen und gewebeverträglich sein. Als chirurgisches Polstermaterial werden üblicherweise Polster
aus Polytetrafluoräthylen- oder Polyesterfilz verwendet, wie sie in den US-PSen 4 034 850 und 4 164 046 beschrieben
sind.
Obwohl das Polyester- oder Polytetrafluoräthylen-Filzmaterial, das bisher als Polster verwendet wurde, seinen
Zweck gut erfüllt hat, hat es noch einige Nachteile. Das Filzmaterial kann beispielsweise abfasern (d.h. Teilchen
verlieren), was unerwünscht ist. In manchen Fällen wächst das Körpergewebe in den Filz hinein oder sogar durch ihn
hindurch. Dies ist ein Nachteil, wenn das Polster entfernt werden muß. Bei manchen Herzklappenoperationen muß die
Klappe ersetzt werden. In solchen Fällen ist es sicher ein Nachteil, wenn das Gewebe in das Polster hineingewachsen
ist. Dies erschwert seine Entfernung und die Herstellung einer ringförmigen Öffnung für eine neue Klappe ist weitaus

traumatischer. Ein weiterer Nachteil von Polytetrafluoräthylen-Polstern
ist die Tatsache, daß sie durch Gammastrahlung so stark abgebaut werden, daß sie anderweitig
sterilisiert werden müssen.
Die vorliegende Erfindung baut auf der Erkenntnis auf, daß bestimmte Arten segmentierter Polyurethane ausgezeichnete
chirurgische Polster ergeben, die sich zur Kombination mit Nahtmaterial bzw. für eine chirurgische Naht eignen. Derartige
Polster fusseln nicht, sind gewebeverträglich, weich, haben eine ausreichende Reißfestigkeit, und lassen überraschenderweise
nur in geringem Maße ein Einwachsen des Kör— pergewebes zu. Eine spätere Entfernung des Polsters ist daher
weniger traumatisch als eine Entfernung der bekannten Filzpolster. Außerdem können diese Polster mit Gammastrahlen
sterilisiert werden.
Der Erfindung liegt somit die Aufgabe zugrunde, Polstermaterial aus segmentierten Polyurethanen für chirurgische
Nähte zu schaffen, das nicht fusseit, gewebeverträglich,
weich und ausreichend reißfest ist und nur in geringem Maße ein Einwachsen des Körpergewebes zuläßt und mit Gammastrahlen
sterilisierbar ist. Diese Aufgabe wird durch die Erfindung gelöst. Die Erfindung betrifft somit den in den Patentansprüchen
gekennzeichneten Gegenstand.
Die Herstellung von Fasermatten aus kontinuierlichen Fäden, die elektrostatisch aus fadenbildenden Polymeren, u.a. aus
Polyurethanen gesponnen sind, ist in den US-PSen 4 043 331 und 4 044 404 beschrieben. Als Verwendungsmöglichkeiten dieses
Materials sind u.a. Wundverbände, Gefäßprothesen und Filter genannt.
In der US-PS 4 127 124 sind Membranen aus hydrophilen PoIyurethanen
beschrieben, die als Bioimplantatmaterial verwendet werden können. Die Herstellung von Polstern aus diesen

Der Ausdruck "Polster" ist hier anscheinend im weiteren Sinn gebraucht und bezeichnet eine Art von Kompresse
oder Tupfer, während es sich bei der vorliegenden Erfindung um Polstermaterial für chirurgische Nähte handelt. Die
in der US-PS 4 127 124 beschriebenen Membranen eignen sich nicht für die Zwecke der vorliegenden Erfindung.
Figur 1 zeigt in perspektivischer Ansicht ein erfindungsgemäßes
Polstermaterial, durch das ein Nahtmaterial hindurchgeführt ist.
Figur 2 und 3 sind 550-fach vergrößerte Abbildungen der zwei Außenseiten von erfindungsgemäßem Polstermaterial.
Das chirurgische Polstermaterial der Erfindung ist aus einer Matte kontinuierlicher Mikrofäden hergestellt. Derartige Matten
werden vorzugsweise durch elektrostatisches Verspinnen eines segmentierten Polyurethans hergestellt. Es· ist bekannt,
Polyurethanlösungen elektrostatisch zu verspinnen, um Matten aus kontinuierlichen Mikrofäden herzustellen. Dieses elektrostatische
Verspinnen bzw. Verspinnen im elektrischen Feld ist beispielsweise in den US-PSen 4 043 331 und 4 044 404
beschrieben. Bei diesem Verfahren wird ein in einem Lösungsmittel gelöstes Polymer in ein elektrostatisches Feld gebracht,
in welchem Tröpfchen dieser Lösung dadurch zu Fäden gestreckt werden, daß sie zur Oberfläche einer Elektrode gezogen
werden. Die Elektrode, z.B. ein rotierender Dorn, an der sich die Fäden sammeln, wird gewöhnlich relativ zur Austrittsöffnung
der Spinnlösung bewegt. Die Spinnbedingungen werden so gesteuert, daß das Lösungsmittel verdampft, während
sich die Fäden in dem elektrostatischen Feld bilden. Dadurch erfolgt kein Verkleben der an der Elektrode gesammelten
Fäden. Gelegentlich können die Fäden an Schnittpunkten infolge von Lösungsmittelrückständen verkleben. Dies ist
nicht notwendigerweise nachteilig. Lösungsmittelrückstände werden aus der Matte der angesaramelten Fäden während des
gesamten Spinnvorganges abgetrennt.

Die elektrostatisch gesponnenen Fäden sind sehr fein. Sie haben einen Durchmesser in der Größenordnung von 0,1 bis
25 Mikron, meist von 0,5 bis 10 Mikron. Der Durchmesser der
erfindungsgemäß verwendeten Fäden beträgt 1 bis 2 Mikron.
Das Spinnverfahren wird so gesteuert, daß eine Matte mit einer mikroporösen Struktur oder einer Beschaffenheit erhalten
wird, die ein Einwachsen von Gewebe nicht zuläßt. Die einzelnen Poren haben einen Durchmesser von weniger als etwa
5 Mikron.
Die erfindungsgemäß verwendeten Polyurethane sind segmentierte
Polyurethane, die abwechselnd weiche und harte Segmente aufweisen. Sie gehören zu den bekannten Polyurethanen,
die seit Jahren zur Herstellung von elastischen Fäden ("Spandex"-Fäden)verwendet werden. Geeignete Polyurethane
sind beispielsweise in den US-PSen 2 929 800, 3 428 711, 3 557 044 und 2 929 804 beschrieben. Die Polymerisationsreaktion
ist im wesentlichen eine Zweistufenreaktion. In der ersten Stufe wird ein Vorpolymer (Prepolymer) hergestellt.
Ein Polyester- oder Polyätherglykol mit einem Molekulargewicht von mindestens etwa 200 wird mit einem Überschuß an
organischem Diisocyanat umgesetzt. Dabei wird ein Vorpolymer mit endständigen Isocyanatgruppen erhalten. In einer zweiten
Stufe wird dieses Vorpolymer mit einem difunktionellen Kettenverlängerer
sowie gegebenenfalls mit einer sehr kleinen Menge einer monofunktionellen Verbindung als Molekulargewichtsregler
umgesetzt.
Die erfindungsgemäß verwendeten Glykole sind vorzugsweise
Poly(tetramethylenäther)glykole, die gewöhnlich ein Molekulargewicht
von über 600, vorzugsweise von etwa 800 bis etwa 5000 aufweisen. Diese Verbindungen sind bekannte Handelsprodukte, die gewöhnlich durch Kondensation von Tetrahydrofuran
unter geeigneten Bedingungen hergestellt werden. Das Glykol wird mit dem organischen Diisocyanat, das im
stöchiometrischen Überschuß verwendet wird, zur Reaktion ge-

bracht, wobei ein Vorpolymer mit endständigen Isocyanatgruppen gebildet wird. Die verwendeten Isocyanate sind gewöhnlich
aromatische Isocyanate, wie 4,4'-Diphenylmethandiioscyanat
("MDI") oder andere Verbindungen, wie Toluylendiisocyanat. Diphenylmethandiisocyanat ist bevorzugt.
Das Diisocyanat wird im stöchiometrischen Überschuß zu dem Glykol verwendet. Das Mengenverhältnis beträgt üblicherweise
etwa 1,2 bis etwa 1,9 Mol Diisocyanat pro Mol Glykol.
Das erhaltene Vorpolymer wird sodann mit einem difunktionellen
Kettenverlängerer zur Reaktion gebracht. Difunktionelle Kettenverlängerer sind Verbindungen, die zwei mit Isocyanatgruppen
reagierende Gruppen besitzen. Beispiele für solche Verbindungen sind zweiwertige Alkohole, Diamine, wobei jede
Aminogruppe mindestens ein Wasserstoffatom an der Aminogruppe
trägt, und Wasser. Die difunktionelle Verbindung wird normalerweise in genau stöchiometrischer Menge verwendet,
so daß eine Reaktion mit allen Isocyanatgruppen im Vorpolymer erfolgt. Es ist auch möglich, die difunktionelle Verbindung
in etwas geringerer Menge als der stöchiometrischen Menge einzusetzen und als Molekulargewichtsregler eine monofunktionelle
Verbindung zuzusetzen, beispielsweise Methanol oder einen anderen niederen aliphatischen Alkohol, oder ein
Amin, wie Dibutylamin, Diäthylamin oder Diisopropylamin.
Geeignete difunktionelle Verbindungen für die segmentierten Polyurethane sind Wasser, 1,4-Butandiol, Diäthylenglykol,
Äthylenglykol und andere bekannte Diole, ferner Diamine, vorzugsweise Diamine mit zwei primären Aminogruppen, wie
Äthylendiamin, Diphenylmethandiamin und 1,3- oder 1,4-Cyclohexylendiamin.
In einer anderen Ausführungsform kann eine weitere Menge
an Diisocyanat während der zweiten Reaktionsstufe zugesetzt werden, in der das Vorpolymer mit der difunktionellen Ver-
bindung zur Reaktion gebracht wird. In diesem Fall wird der Anteil an difunktioneller Verbindung und der gegebenenfalls
verwendeten monofunktionellen Verbindung entsprechend angepaßt.
Figur 1 zeigt eine Kombination 10 aus Nahtmaterial und
Polster. An dem Faden 14 sind Nadeln 12a und 12b angebracht.
Der Faden läuft durch das Polster 16. Das Polster ist eine Matte aus kontinuierlichen Mikrofasern, die im wesentlichen
aus einem segmentierten Polyurethan bestehen. Wie aus der Figur ersichtlich, hat die Matte eine Öffnung, durch die
das Nahtmaterial gezogen ist. Die Kombination von Nahtmaterial und Polster kann in üblicher Weise, z.B. durch
Gammastrahlung oder mit Äthylenoxid, sterilisiert werden.
Es wird ein segmentiertes Polyätherurethan hergestellt. Dazu wird zunächst 4,4'-Diphenylmethandiisocyanat mit Poly
(tetramethylenäther)glykol in einem Molverhältnis von etwa
1,7 Mol Diisocyanat pro Mol Glykol zur Reaktion gebracht. Es wird ein Vorpolymer mit endständigen Isocyanatgruppen erhalten.
Das Vorpolymer wird in einem geeigneten Lösungsmittel, wie N,N-Dimethylacetamid, gelöst und hierauf mit einer
stöchiometrischen Menge eines Gemisches aus 80 Molprozent Äthylendiamin und 20 Molprozent 1,3-Cyclohexylendiamin zur
Reaktion gebracht. Zur Regulierung des Molekulargewichts wird bei Bedarf eine kleine Menge (z.B. 5 bis 10 Molprozent, bezogen
auf das gesamte Diamin) eines Monoamine, beispielsweise Diäthylamin, zugesetzt.
Aus dem erhaltenen segmentierten Polyätherurethan wird eine Spinnlösung zubereitet. Zu diesem Zweck wird eine Lösung
hergestellt, die 15 5 Polymerfeststoff in einem Gemisch aus
N,N-Dimethylacetamid und Methyläthy!keton enthält.

Das Molekulargewicht des zur Herstellung des Vorpolymers verwendeten
Poly(tetramethylenäther)glykols beträgt etwa 1800. Vorzugsweise wird zu Beginn des Spinnverfahrens ein PoIyätherurethan
verwendet, bei dem ein Poly(tetramethylenäther) glykol mit höherem Molekulargewicht von beispielsweise etwa
3 000 verwendet wird. Dies erleichtert die Bildung der Matte beim elektrostatischen Spinnen der Spinnlösung.
Die Polyurethanlösung wird elektrostatisch zu Matten aus kontinuierlichen Mikrofäden versponnen. Als Spule dient ein
schnell drehender Dorn; vgl. z.B. Figur 3 der US-PSen 4 043 331 und 4 044 404. Der Dorn hat einen Durchmesser
von 5 cm und eine Länge von 51 cm, und er besitzt eine glatte Edelstahloberfläche. Die Polymerlösung wird durch
einen Verteiler, der eine Anzahl Düsen, z.B. 7) besitzt, geschickt. Die Düsen sind ähnlich wie Injektionsnadeln gebaut.
Der Verteiler oszilliert parallel zur Längsachse des Dorns. Bei einer elektrostatischen Spannung von etwa 20 KV
beträgt die Entfernung zwischen Düsenspitze und Oberfläche des rotierenden Dorns 5 bis 35 cm. Die Vorrichtung wird mit
einem warmen Luftstrom umgeben, damit die Lösungsmittel während des Verspinnens der Polymerlösung besser verdampfen
können. Auf diese Weise wird eine Matte mit einer Dicke von etwa 1 bis 1,5 mm hergestellt. Das Material hat eine Dichte
° von etwa 0,55 g/cm und eine Porosität von etwa 43 %. Der
Fadendurchmesser beträgt etwa 1 bis 2 Mikron. Das Mattenmaterial wird in Stücke mit einer Länge von 6 bis 10 mm und
einer Breite von 3 bis 5 mm geschnitten. Das Material ist im wesentlichen fusselfrei und ausreichend reißfest, so daß
es nicht von dem chirurgischen Nahtmaterial durchschnitten wird. Bei Einpflanzung in den Körper kann das Körpergewebe
nur etwa 40 Mikron (etwa 0,04 mm) weit einwachsen. Dadurch ergibt sich eine biegsame Fläche zwischen Körpergewebe und
Polster. Das Polster kann somit später einfacher entfernt
werden als Polsterfilz aus Polyester oder Polytetrafluoräthylen.

Figur 2 ist eine vergrößerte Abbildung einer Seite des erfindungsgemäßen
Polsters (die dem Dorn zugewandte Seite), während Figur 3 eine vergrößerte Abbildung der anderen Seite
des selben Polsters ist.
Die Orientierung der Mikrofäden in dem Mattenmaterial kann
isotrop oder anisotrop sein. Ist die Orientierung der Mikrofäden anisotrop, so wird das Nahtmaterial so durch das
Polster gezogen, daß die Orientierung des Nahtmaterials etwa senkrecht zur vorherrschenden Orientierung der Mikrofäden
ist. Die Orientierung der Mikrofäden im Mattenmaterial läßt sich mit konventionellen Methoden, wie Ermittlung der
Zugspannung in verschiedenen Richtungen der Mattenebene, feststellen. Die Richtung mit der höchsten Zugspannung ist
zugleich die vorherrschende Orientierungsrichtung der Mikrofäden.

Claims (10)
1. Chirurgisches Nahtmaterial mit Polster, dadurch
gekennzeichnet, daß das Polster aus einer Matte kontinuierlicher Mikrofäden besteht und die
Mikrofäden im wesentlichen aus einem segmentierten Polyurethan bestehen, wobei das segmentierte Polyurethan
das Reaktionsprodukt aus einem Polyäther- oder Polyesterglykol mit einem Molekulargewicht von mindestens etwa
200, einem organischen Diisocyanat und einem difunktionellen Kettenverlängerer ist.
2. Polster gemäß Anspruch 1, dadurch gekennzeichnet, daß das Glykol ein Poly(tetramethylenäther)glykol mit einem
Molekulargewicht von etwa 800 bis etwa 5000 ist.
3. Polster gemäß Anspruch 1, dadurch gekennzeichnet, daß das organische Diisocyanat ein aromatisches Diisocyanat
ist.
4. Polster gemäß Anspruch 3, dadurch gekennzeichnet, daß das aromatische Diisocyanat Diphenylmethandiisocyanat
ist.
5. Polster gemäß Anspruch 1, dadurch gekennzeichnet, daß der difunktionelle Kettenverlängerer ein zweiwertiger
Alkohol, ein Diamin mit zwei primären Aminogruppen oder Wasser ist.
6. Polster gemäß Anspruch 5, dadurch gekennzeichnet, daß der difunktionelle Kettenverlängerer 1,4-Butandiol ist.
7. Polster gemäß Anspruch 1, dadurch gekennzeichnet, daß das segmentierte Polyurethan einen monofunktionellen
Endblocker enthält.
8. Polster gemäß Anspruch 1, dadurch gekennzeichnet, daß die Orientierung der Mikrofäden in der Matte anisotrop
ist und das Nahtmaterial so durch das Polster geführt ist, daß die Orientierung des Nahtmaterials im wesentlichen
senkrecht zur Richtung der vorherrschenden ■φ Orientierung der Mikrofäden ist.
* 20 9. Polster für chirurgische Nähte, dadurch gekennzeichnet,
daß das Polster aus einer Matte kontinuierlicher Mikrofäden besteht und die Mikrofäden im wesentlichen aus
einem segmentierten Polyurethan bestehen, wobei das Polyurethan das Reaktionsprodukt aus einem Polyäther- oder
Polyesterglykol mit einem Molekulargewicht von mindestens 200, einem organischen Diisocyanat und einem difunktionellen
Kettenverlängerer ist.
10. Material gemäß Anspruch 1 bis 9 in sterilem Zustand. 30
