Description
Die Erfindung geht aus von einem Hochfrequenz-Chirurgiegerät ent
sprechend dem Oberbegriff des Patentanspruchs 1.
Definitionsgemäß sind Hochfrequenz-Chirurgiegeräte für monopolare
Operationstechniken jeweils mit einer aktiven und einer inaktiven,
sogenannten neutralen Elektrode, ausgestattet. Die neutrale Elektrode
wird während der gesamten Dauer der Operation bestimmungsgemäß elek
trisch gut leitend und großflächig auf der Haut des Patienten, bei
spielsweise am Oberschenkel oder am Oberarm appliziert.
Prinzipiell kann die neutrale Elektrode elektrisch leitfähig
möglichst nahe am in der Regel geerdeten Operationstisch geerdet
werden oder aber möglichst gut gegen Erdpotential isoliert sein.
Mit der elektrisch leitfähigen Erdung der neutralen Elektrode soll
verhindert werden, daß der Patient während der Aktivierung des Hoch
frequenz-Chirurgiegerätes hochfrequente elektrische Spannung gegen
Erdpotential führt, weil hierdurch hochfrequenter Strom unbeab
sichtigt an allen Körperstellen des Patienten entstehen kann, wo
dieser elektrisch leitfähigen Kontakt mit direkt oder kapazitiv ge
erdeten Gegenständen hat. Außerdem können elektronische Geräte, ins
besondere Meß- und Überwachungsgeräte, durch die hochfrequente
Wechselspannung, die der Patient andernfalls führen würde, gestört
werden.
Aus der elektrisch leitfähigen Erdung der neutralen Elektrode und
damit auch des Patienten resultiert jedoch die Gefahr, daß
elektrischer Strom aus anderen geerdeten Stromquellen, beispiels
weise defekten elektrischen Geräten, welche gleichzeitig den Pa
tienten elektrisch leitfähig berühren, durch den Patienten fließt und
ihn gefährden kann.
Als Kompromiß zwischen der elektrisch leitfähigen Erdung und der
möglichst guten Isolation der neutralen Elektrode gegen Erdpotential
sind Hochfrequenz-Chirurgiegeräte bekannt, bei welchen die neutrale
Elektrode über einen Kondensator geerdet ist. Hierdurch wird bei ge
eigneter Dimensionierung des Kondensators erreicht, daß die neutrale
Elektrode bezüglich der Hochfrequenzspannung praktisch ausreichend
auf Erdpotential liegt, niederfrequente elektrische Ströme durch den
Patienten und die neutrale Elektrode gegen Erdpotential aber prak
tisch ausreichend klein bleiben. In den Sicherheitsvorschriften für
Hochfrequenz-Chirurgiegeräte VDE 0750 Teil 202 ist die Kapazität
dieses Kondensators auf höchstens 50 nF begrenzt.
Bei einem bekannten Hochfrequenz-Chirurgiegerät der eingangs ge
nannten Art ist eine Überwachungsvorrichtung vorgesehen, um über
eine Kapazität gegen Erdpotential fließenden Strom zu erfassen
und eine Schädigung des Patienten durch hochfrequente elektri
sche Ströme zu vermeiden (DE 26 02 517). Bei einem anderen be
kannten Hochfrequenz-Chirurgiegerät dieser Art ist ebenfalls
eine Überwachungsschaltung für hochfrequenten Strom vorgesehen,
welche Schaltung deshalb einen Kondensator mit geringer Impedanz
für hochfrequente Signale und hoher Impedanz für niederfrequente
Signale aufweist (US 42 31 372).
Bezüglich der Sicherheit des Patienten vor der Gefährdung durch
niederfrequente elektrische Ströme werden elektromedizinische Geräte
in der Empfehlung der IEC (International Electrotechnical
Commission), Publikation 601-1, entsprechend der maximal zugelassenen
niederfrequenten Leckströme in drei verschiedene Typen unterteilt,
und zwar in die Typen B, BF und CF. Hochfrequenz-Chirurgiegeräte
müssen entsprechend IEC 601-202 (identisch VDE 0750 Teil 202) Typ BF
oder Typ CF erfüllen. Bei Geräten des Typs BF darf der Patientenleck
strom maximal 0,1 mA bei Normalfunktion und max. 0,5 mA im ersten
Fehlerfalle betragen. Bei Geräten des Typs CF darf dieser Leckstrom
nur maximal ein Zehntel der Grenzwerte des Typs BF erreichen. Außer
dem darf bei Geräten des Typs CF der Leckstrom, der durch den Pati
enten hindurch in das Hochfrequenz-Chirurgiegerät des Typs CF hinein
fließen kann, wenn der Patient mit geerdeter Netzspannung in Berüh
rung kommt, maximal nur 0,05 mA erreichen.
Es ist Aufgabe der Erfindung, ein Hochfrequenz-Chirurgiegerät
entsprechend dem Oberbegriff des Patentanspruchs 1 derart zu
verbessern, daß Mittel vorhanden sind, die es beim Auftreten
niederfrequenter Leckströme ermöglichen, umgehend Maßnahmen zu
treffen, um eine schädigende Gefährdung des Patienten durch
niederfrequente Leckströme möglichst weitgehend vermeiden zu
können.
Diese Aufgabe wird erfindungsgemäß durch den Gegenstand des
Patentanspruchs 1 gelöst. Vorteilhafte Weiterbildungen und
Ausgestaltungen der Erfindung sind Gegenstand der Unteran
sprüche.
Bei einem derartigen Chirurgiegerät, welches zur Reduzierung der
hochfrequenten elektrischen Spannung zwischen Patient und Erd
potential mit einem Kondensator mit möglichst großer Kapazität
von beispielsweise 50 nF zwischen der neutralen Elektrode und
Erde ausgestattet ist und damit den Typ BF erfüllt, wird deshalb
zusätzlich eine Schaltung vorgesehen, welche den niederfrequenten
elektrischen Leckstrom durch diesen Kondensator überwacht und
diesen Kondensator von Erdpotential
abtrennt, sobald der niederfrequente elektrische Leckstrom einen
definierten Grenzwert übersteigt.
Anhand eines in der Zeichnung dargestellten Ausführungsbeispiels
eines Hochfrequenz-Chirurgiegerätes soll die Erfindung beispiels
weise näher erläutert werden.
Der Hochfrequenzgenerator 1 ist in bekannter Weise mit einem Trenn
transformator 2 ausgestattet, welcher den Anwendungskreis 34 des Hoch
frequenz-Chirurgiegerätes, welcher mit dem Patienten elektrisch leit
fähig in Verbindung ist, von der niederfrequenten Betriebsspannung
des Hochfrequenzgenerators 1 isoliert. Der niederfrequente Leckstrom
I2, der über die konstruktiv bedingte Streukapazität 28 von der
Primärwicklung 29 in die Sekundärwicklung 30 fließen kann, ist ohne
Probleme so weit reduzierbar, daß er praktisch vernachlässigt werden
kann.
Der Kondensator 3 zwischen der Sekundärwicklung 30 des Transformators
2 und der aktiven Elektrode 7 dient bekanntlich der Unterdrückung
niederfrequenter elektrischer Ströme infolge der nichtlinearen Ab
hängigkeit des Stromes von der Spannung im elektrischen Lichtbogen,
welcher insbesondere beim Schneiden unvermeidlich ist.
Der Kondensator 4 soll einerseits eine sehr kleine Impedanz für den
hochfrequenten elektrischen Strom darstellen, so daß die Hochfre
quenzspannung UHF zwischen Patient 5 und Erdpotential 32 möglichst
klein bleibt und andererseits eine möglichst hohe Impedanz für
niederfrequenten elektrischen Strom darstellen, so daß der nieder
frequente Strom INF durch den Patienten 5 möglichst klein bleibt,
wenn beispielsweise eine geerdete niederfrequente Spannungsquelle 8
den Patienten 5 elektrisch leitfähig berührt, 9. Diese beiden konträren
Forderungen sind bei bekannten Hochfrequenz-Chirurgiegeräten
nicht befriedigend gelöst. In dem Hochfrequenz-
Chirurgiegerät ist der Kondensator 4 nicht, wie allgemein üblich,
direkt elektrisch leitfähig mit Erdpotential 32 verbunden, sondern
über einen automatischen Schaltkontakt 10, der sofort öffnet, wenn
der niederfrequente Leckstrom INF einen definierten Pegel übersteigt.
Hierfür ist ein Stromsensor vorgesehen, welcher beispielsweise aus
einem Widerstand 11 und einem Spannungskomparator 13 besteht. Der
niederfrequente Leckstrom INF erzeugt an dem Widerstand 11 eine
Spannung, welche der Intensität des Leckstromes INF proportional ist.
Übersteigt diese Spannung einen am Spannungsteiler 14, 15
eingestellten Pegel, so liefert der Spannungskomparator 13 einen
logischen High-Pegel, der eine bistabile Kippstufe 16, beispiels
weise ein JK-Flip-Flop, am Eingang K so setzt, daß dessen Ausgang Q
Low-Pegel annimmt, wodurch der Transistor 20 sperrt und das Relais 21
stromlos wird. Hierdurch öffnet der Schaltkontakt 10 und unterbricht
somit die elektrisch leitfähige Verbindung zwischen dem Kondensator 4
und Erdpotential 32. Gleichzeitig schließt das Relais 21 den Kontakt 24
und schaltet ein akustisches 22 und/oder optisches 33 Signal
ein, welches den Operateur warnt. Dieser Zustand bleibt so lange er
halten bis entweder die Betriebsspannung +UB aus- und wieder einge
schaltet oder aber die Taste 27 betätigt wird, wodurch das JK-Flip-
Flop 16 am Eingang J so gesetzt wird, daß der Ausgang Q wieder High-
Pegel annimmt und über das ODER-Gatter 19 den Transistor 20 wieder
leitfähig schaltet, so daß das Relais 21 den Kontakt 10 wieder
schließt und den Kontakt 24 wieder öffnet, vorausgesetzt, die Ursache
für den zu hohen niederfrequenten Leckstrom INF wurde vorher elimi
niert. Das definitive Setzen des JK-Flip-Flops 16 über die RC-Kombina
tion 17, 18 bei jedem erneuten Einschalten der Betriebsspannung +UB
stellt außerdem sicher, daß nach jedem Einschalten des Hochfrequenz-
Chirurgiegerätes der Kontakt 10 geschlossen und der Kontakt 24 ge
öffnet ist.
Um zu verhindern, daß der Spannungskomparator 13 durch hochfrequente
Spannungen, welche ebenfalls am Widerstand 11 entstehen können, in
seiner Funktion gestört wird, ist ein Tiefpaßfilter 12 vorgesehen,
welches sowohl die Betriebsfrequenz des Hochfrequenzgenerators 1 als
auch dessen harmonische Frequenzen sperrt.
Die Betriebsspannung +UB wird zweckmäßig in bekannter Weise aus einem
elektronischen Spannungsregler entnommen, so daß die Schwellenspan
nung des Spannungskomparators 13 konstant bleibt.
In einem weiteren Ausführungsbeispiel der Erfindung ist die
Stromsensoreinrichtung mit einem Kontakt 31 ausgestattet, welcher auto
matisch und zeitsynchron mit der Aktivierung des Hochfrequenz
generators 1 schließt und somit die Stromsensoreinrichtung während der
Aktivierung des Hochfrequenzgenerators 1 abschaltet.
Dies ist vorteilhaft, wenn die Grenze für den niederfrequenten Leck
strom INF sehr klein eingestellt ist und die Hochfrequenzspannung am
Widerstand 11 nicht ausreichend durch das Filter 12 unterdrückt wer
den kann.
Ein weiteres Ausführungsbeispiel der Erfindung besteht darin, daß die
Stromsensoreinrichtung über einen Schalter 23 abgeschaltet werden kann,
wobei gleichzeitig ein optisches Signal 26 anzeigt, daß die Sicher
heitsschaltung außer Funktion ist.
Ein Vorteil der Erfindung besteht
unter anderem darin, daß niederfrequente Leckströme, die durch den
Patienten 5 über die neutrale Elektrode 6 und den Kondensator 4
und/oder durch den Patienten 5 über die aktive Elektrode 7 und den
Kondensator 3 gegen Erdpotential fließen können, rechtzeitig erkannt
und durch automatisches Öffnen des Kontaktes 10 verhindert werden,
unabhängig davon, ob dieser niederfrequente Leckstrom durch eine
externe niederfrequente Spannungsquelle 8 oder durch einen internen
Fehler im Hochfrequenz-Chirurgiegerät, beispielsweise einer Unter
brechung des Schutzleiters des Hochfrequenz-Chirurgiegerätes, ver
ursacht wird. Außerdem überwacht die Schaltung des
Hochfrequenz-Chirurgiegerätes indirekt, ob der Patient
gefährliche niederfrequente Spannung gegen Erdpotential führt, unab
hängig davon, wodurch diese Spannung verursacht wird.
Claims (10)
1. Hochfrequenz-Chirurgiegerät zum monopolaren Schneiden und
Koagulieren biologischer Gewebe mittels hochfrequentem
elektrischem Strom, mit einem an den Hochfrequenz-Generator
(1) über einen Trenntransformator (2) angeschlossenen An
wendungskreis (34), der eine aktive Elektrode (7) und eine
neutrale Elektrode (6) enthält, welche mit dem Gewebe
elektrisch leitfähig in Verbindung gebracht wird und
durch einen Kondensator (4) auf Erdpotential (32) bezogen
ist, dadurch gekennzeichnet, daß eine Stromsensorein
richtung (11, 12, 13) vorhanden ist, welche niederfrequente
Leckströme (INF) in der elektrisch leitfähigen Verbindung
zwischen dem Anwendungskreis (34) und Erdpotential (32)
erfaßt und ein elektrisches Signal liefert, wenn dieser
niederfrequente Leckstrom (INF) einen vorgegebenen Pegel
übersteigt, und daß
in der Verbindung zwischen
dem Anwendungskreis (34) und Erdpotential (32) ein Schalt
kontakt (10) eingefügt ist, der beim Auftreten des elektrischen
Signals geöffnet wird.
2. Hochfrequenz-Chirurgiegerät nach
Anspruch 1, dadurch gekennzeichnet, daß
durch das elektrische Signal ein Warnsignal
erzeugt wird.
3. Hochfrequenz-Chirurgiegerät nach Anspruch 1 oder 2, dadurch
gekennzeichnet, daß die Stromsensoreinrichtung (11, 12, 13)
ein Tiefpaßfilter (12) enthält, welches für die niederfrequenten
Komponenten des für den Patienten gefährlichen Frequenz
spektrums durchlässig ist und die hochfrequenten Komponenten
entsprechend der Grundfrequenz des Hochfrequenz-Generators
(1) und dessen harmonischen Frequenzen sperrt.
4. Hochfrequenz-Chirurgiegerät nach Anspruch 1, 2 oder 3, dadurch
gekennzeichnet, daß die Stromsensoreinrichtung (11, 12, 13)
einen Spannungskomparator (13) enthält, an dem ein oberer
Grenzwert für den niederfrequenten elektrischen Leckstrom
(INF) einstellbar ist.
5. Hochfrequenz-Chirurgiegerät nach Anspruch 4, dadurch gekenn
zeichnet, daß das Ausgangssignal des Spannungskomparators (13)
einer bistabilen elektronischen Kippstufe (16) zugeführt wird,
an deren Ausgang ein Relais (21) angeschlossen ist, das sofort
nach dem Überschreiten des Grenzwerts des niederfrequenten
Leckstroms den Schaltkontakt (10) öffnet.
6. Hochfrequenz-Chirurgiegerät nach einem der vorhergehenden
Ansprüche, dadurch gekennzeichnet, daß dem Relais (21)
ein weiterer Schaltkontakt (24) zugeordnet ist, bei dessen Schließen
eine optische und/oder akustische Signalgeneratoreinrichtung
(22, 23) aktivierbar ist.
7. Hochfrequenz-Chirurgiegerät nach Anspruch 1 oder 2, dadurch
gekennzeichnet, daß die zur Überwachung der niederfrequenten
Leckströme (INF) dienende Stromsensoreinrichtung während der
Zeit, in der der Hochfrequenz-Generator (1) aktiviert ist,
automatisch und zeitsynchron mit der Aktivierung des
Hochfrequenz-Generators (1) abgeschaltet wird und während
der Zeit, in der der Hochfrequenz-Generator (1) nicht
aktiviert ist, eingeschaltet wird.
8. Hochfrequenz-Chirurgiegerät nach einem der Ansprüche 5 bis 7,
dadurch gekennzeichnet, daß an die Kippstufe (16) eine
einen Schalter (27) enthaltende Einrichtung (17, 18) ange
schlossen ist, durch die die Kippstufe (16) wieder zurück
gesetzt werden kann, wenn der Pegel des niederfrequenten
Leckstroms (INF) wieder kleiner als der eingestellte
Grenzwert ist.
9. Hochfrequenz-Chirurgiegerät nach einem der Ansprüche 5 bis 7,
dadurch gekennzeichnet, daß die Kippstufe (16) bei jedem
Einschalten der Betriebsspannung (+UB) des Hochfrequenz-
Chirurgiegeräts in den Zustand gesetzt wird, bei welchem
der Schaltkontakt (10) geschlossen und der weitere Kontakt (24)
geöffnet sind.
10. Hochfrequenz-Chirurgiegerät nach Anspruch 1 oder 2, dadurch
gekennzeichnet, daß die zur Überwachung der niederfrequenten
Leckströme (INF) dienende Stromsensoreinrichtung durch einen
Schalter (23) abschaltbar ist und daß dieser Zustand durch
ein optisches Signal (26) anzeigbar ist.
