Description
Die vorliegende Erfindung betrifft ein Verfahren zur
Herstellung eines Körperimplantats nach dem Oberbegriff
das Patentanspruchs 1.
Körperimplantate, wie Knochenplatten, Nägel,
Stifte, Schrauben und Gelenkprothesen, werden seit
vielen Jahren in die Skelette von Menschen und Tieren
implantiert, um die Teile eines gebrochenen Knochens
miteinander zu verbinden oder um fehlende oder be
schädigte Skeletteile zu ersetzen. Es ist dabei oft
beabsichtigt, daß diese Teile zu einem permanenten
Teil des Körpers werden. In solchen Fällen ist es
wichtig, daß die Teile fest und permanent am Knochen
skelett fixiert werden.
Es ist seit mehr als 10 Jahren bekannt, daß eine feste
permanente Fixierung durch die Verwendung einer po
rösen Fläche am gesamten Implantat oder an einem Abschnitt
des Implantates erreicht werden kann, wenn die mittlere
Porengröße etwa 50 µm überschreitet. Dabei stellen
die älteren Verfahren der Knochenfixierung, in erster
Linie mit Hilfe von Reibungspassung oder mit Methyl
methacrylat, noch die am häufigsten verwendeten Ver
fahren zur Fixierung dar, und zwar trotz der Tatsache,
daß das Lösen von mittels Reibung und Zement fixierten
Prothesenteilen im Laufe der Zeit ein signifikantes
medizinisches Problem darstellt. Dies ist darauf zu
rückzuführen, daß zur Erzielung einer guten Fixierung
mit Hilfe des Einwachsens des Knochens der Patient
keine Kraft oder Last auf das Skelett im Implantat
aufbringen darf, bis das Knochenwachstum aufgetreten
ist, während bei einer Verbindung über Reibung oder
Zement nahezu sofort eine Belastung stattfinden kann.
In der nachfolgenden Beschreibung bedeuten die Worte
bioabsorbierbar und resorbierbar, daß die Substanz,
auf die sich diese Worte beziehen, abgebaut, ab
sorbiert oder in anderer Weise von der Chemie des
Wirtkörpers in Mengen entfernt wird, die ausreichen,
um den physikalischen Aufbau des Abschnittes des Implantates,
der bioabsorbierbar ist, innerhalb einer
Zeitspanne zu verändern, die geringer ist als die
Zeitspanne, die zur vollständigen Genesung von
Knochen erforderlich ist. Es versteht sich, daß sämt
liche Materialien, sogar Stahl, von der Chemie des
Körpers geringfügig absorbiert werden; derartige
geringfügige Absorptionen, die den physikalischen Auf
bau der bioabsorbierbaren Vorrichtung über die Zeit
spanne der Knochenheilung nicht verändern, sind je
doch nicht in den Begriffen bioabsorbierbar und
resorbierbar eingeschlossen. Es versteht sich fer
ner, daß, obwohl ein Material bioabsorbierbar ist,
eine geringfügige Menge desselben im Körper über
längere Zeitspannen zurückbleiben kann.
Ein Verfahren zum Fördern des Knochenein
wachsens in poröse Flächen von Implantaten besteht
im Beschichten von porösen Metallfasern mit
Hydroxyapatit durch Eintauchen der Faser in ei
nen Wasserschlamm von Hydroxyapatit und anschließendes Trocknen.
Hydroxyapatit wird nicht im allgemeinen als
resorbierbar im menschlichen Körper angesehen. Es wurde
festgestellt, daß Hydroxyapatit ein schnelleres Ein
wachsen des Knochens in die porösen Metallflächen über
eine Zeitspanne von bis zu vier Wochen nach der Im
plantation fördert, daß der erreichte Effekt jedoch nur
kurzzeitig war, da die Menge des Knochengewebes inner
halb der Poren nach dieser Periode von vier Wochen ab
fiel (P. Ducheyne et al "Effect of Hydroxyapatite
Impregnation on Skeletal Bonding of Porous Coated
Implants", Journal of Biomedical Materials Research,
Vol. 14, 225-237 (1980)). Zusätzlich zu der Tatsache,
daß das gesteigerte Einwachsen nicht permanent war,
wurde festgestellt, daß Hydroxyapatit mehr das Faser
wachstum als das Knochengewebewachstum an der
Fixierungsstelle über längere Zeiträume fördert, was
zu einem Lösen der Prothese führen kann. Darüber hinaus
wird der Hydroxyapatitüberzug in dem vorstehend ge
nannten Artikel als relativ zerbrechlich bezeichnet
und kann bei einer normalen Handhabung für kommerziell
erhältliche Produkte leicht brechen, so daß dies zu
keiner weitgestreuten kommerziellen Verwendung geführt
hat.
In der US-PS 36 05 123 ist
das Plasmaspritzverfahren einer metallischen porösen
Fläche auf eine Prothese beschrieben. In den
US-PS 38 92 648 und 38 92 649 wird die galvanische
Abscheidung von Knochengewebe und Collagen auf Implantate
oder in ein Kunststoffsieb auf Implantaten, um hierdurch
die Befestigung des Knochengewebes am Implantat anzu
regen, erläutert.
Die US-PS 39 19 723 betrifft das Einbetten von Calzium-
und Phosphatatomen in die Oberfläche eines keramischen
Implantates durch Erhitzen des Implantates und Ein
betten desselben in eine Schmelze von Calziumphosphat.
Es ist dabei speziell darauf hingewiesen, daß Tempe
raturen über 1300°C nicht verwendet werden sollten,
da sich das Calziumphosphat bei höheren Temperaturen
zersetzt. Diese Zersetzung bei höheren Temperaturen
ist charakteristisch für Materialien auf der Basis
von Calziumphosphat (siehe hierzu E. Hayek und H. Newesely,
Inorganic Synthesis 7 (1963) 63-65).
In der US-PS 42 02 055 ist die Kombination eines
bioabsorbieren, bioaktiven Calziumphosphatmaterials
mit einem Polymerisat auf der Oberfläche einer Prothese,
um hierdurch ein Einwachsen des Knochens in das
Polymerisat zu bewirken, beschrieben. Die US-PS
43 65 357 und 43 73 217 erläutern die Kombination
eines absorbierbaren Tricalziumphosphatmaterials
mit Knochenzement, um ein Knochenwachstum in den Ze
ment zu erzielen. Jede dieser Veröffentlichungen bezieht sich
auf die Einarbeitung des absorbierbaren Materials
in das Material, aus dem die Fläche besteht, und
schlägt vor, daß die poröse Fläche durch Absorption
des absorbierbaren Materials erzeugt wird, wodurch
Poren in der Fläche an der Stelle des Leerraumes
verbleiben, der durch die Absorption des absorbier
baren Materiales erzeugt wird. Keines dieser Veröffentlichungen
betrifft jedoch die Beschichtung einer bereits porenhaltigen
Fläche mit einem absorbierbaren Material, um dadurch
das Knochenwachstum zu fördern.
In der US-PS 43 38 926 wird die Hinzufügung einer
0,1 bis 1 mm dicken bioabsorbierbaren Schicht auf
eine Implantatfläche beschrieben. Hierdurch will man
ein Lösen des Implantates erzeugen, wenn das Material
absorbiert wird. Die Prothese, auf der die absorbier
bare Schicht angeordnet wird, ist porenfrei, da man
ein Knocheneinwachsen verhindern will.
Des weiteren ist es aus der DE-OS 20 08 010 bekannt,
zum Herstellen von Dentalimplantaten auf einen
Kern, der aus einem Material wie Metall, Keramik
oder einem Kunststoff besteht, einen Überzug
aus einem Eutektoid aus Tricalziumphosphat und
Tetracalciumphosphat heiß aufzuspritzen, wobei
als Eutektoid Knochenasche verwendet wird.
Zur Herstellung des Implantates wird eine eutek
tische Mischung aus Tricalciumphosphat und Tetra
calciumphosphat bei etwa 1560°C geschmolzen.
Nach dem Abkühlen der Schmelze bildet sich ein
Glas. Dieses Glas wird zermahlen und mit Hilfe
einer Flamm- oder Plasmaspritzvorrichtung auf
eine metallische oder nicht metallische Unterlage,
die im wesentlichen die Form des zu ersetzenden
Zahnteils aufweist, aufgetragen. Anschließend
wird wenigstens die aus diesem Überzug bestehende
Oberfläche bei etwa 1000°C in einer wasser
dampfhaltigen Atmosphäre geglüht und damit in
Hydroxyapatit zurückgeführt.
Ein Verfahren mit den Merkmalen des Oberbe
griffs des Patentanspruchs 1 ist aus der DE-OS
27 55 751 bekannt. In dieser Veröffentlichung ist
die Herstellung eines Implantates für Knochen,
Gelenke oder Zahnwurzeln beschrieben, das aus
einem geformten Keramikkern und einer darauf
thermisch aufgedampften Schicht aus einem Gemisch
von einem keramischen Material und Hydroxyapatit
besteht. Die Schicht aus dem Gemisch aus keramischen
Material und Hydroxyapatit kann durch ein thermisches
Aufdampfverfahren aufgebracht werden.
Der Erfindung liegt die Aufgabe zugrunde, ein
Verfahren der angegebenen Art zu schaffen, mit
dem sich ein Körperimplantat herstellen läßt,
das besonders gute Einwachs- und Fixierungs
eigenschaften besitzt.
Diese Aufgabe wird durch ein Verfahren der an
gegebenen Art mit den kennzeichnenden Merkmalen
des Patentanspruchs 1 gelöst.
Das erfindungsgemäße Verfahren unterscheidet
sich von dem vorstehend genannten bekannten
Verfahren im wesentlichen dadurch, daß erfindungs
gemäß das Überzugsmaterial beim Aufbringen auf
eine Temperatur über 1350°C erhitzt wird und
daß als Überzugsmaterial neben Hydroxyapatit
β-Tricalciumphosphat verwendet wird, so daß
sich das Material bei seiner Aufbringung haupt
sächlich in absorbierbares α-Tricalciumphosphat
umwandelt. Beim erfindungsgemäßen Verfahren
wird gezielt ein Basiselement mit einem Knochen
befestigungsbereich mit poröser Oberfläche herge
stellt. Hierdurch wird ein besonders schnelles
und starkes Knocheneinwachsen sichergestellt.
Das Einwachsen wird permanent angeregt, und es
wird ein Knochenmaterial anstelle eines faserigen,
schwachen Gewebes gebildet, das bei reinen Hydroxy
apatit-Überzügen auftritt.
Die Hochtemperaturbehandlung des Überzuges führt
zu einem solchen, der sowohl absorbierbar ist als
auch eine besonders große Adhäsionskraft besitzt.
Die Implantate können somit verpackt, sterilisiert
und allgemein für Implantationszwecke gehandhabt
werden, ohne daß sich dieser Überzug vom Basis
element löst.
Die Erfindung wird nachfolgend von Ausführungsbeispielen
in Verbindung mit der Zeichnung im einzelnen er
läutert. Es zeigt
Fig. 1 eine Seitenansicht eines nach dem erfindungsgemäßen Verfahren hergestellten Körperim
plantates;
Fig. 2 eine perspektivische Ansicht einer
anderen Ausführungsform des Implantates;
Fig. 3 die zeichnerische Wiedergabe einer
Fotografie einer beschichteten porö
sen Oberfläche des bei der Aus
führungsform der Fig. 1 verwendeten
Typs, etwa 20fach vergrößert;
Fig. 4 die zeichnerische Wiedergabe eines
metallografischen Bereiches des Mikro
bildes eines Rasterelektronenmikros
kopes, und zwar des Materiales der
Fig. 3 mit 63, 5facher Vergrößerung,
wobei sich die Oberfläche der Probe
unten rechts in der Figur befindet;
und
Fig. 5 die zeichnerische Wiedergabe einer
Fotografie eines Abschnittes wie in
Fig. 4, jedoch 948fach vergrößert,
wobei nur ein Abschnitt von einem der
Metalldrähte dargestellt ist, der die
poröse Fläche und den Überzug des
Drahtes bildet.
In Fig. 1 ist ein prothetisches Teil dargestellt, das
als Körperimplantat verwendet werden soll. Bei dem
speziellen gezeigten Teil handelt es sich um eine Hüft
prothese. Derartige Prothesen umfassen üblicherweise
einen glatten, kugelförmigen Kopf 10, der eine Ober
fläche eines Hüftgelenks bildet und der sich in einer
Hüftpfanne drehen kann, einen Hals 14 und ein Basis
element oder Schaft 15. Der Schaft 15 dient zur Ver
ankerung im Femur des Menschen oder Tieres. Um die
Fixierung des Schaftes 15 im Femur zu verbessern, ist
ein Bereich 16 der Oberfläche des Schaftes 15 porös.
Bei der Ausführungsform der Fig. 1 umfaßt der poröse
Bereich 16 des Schaftes eine Matte, die sich aus
komprimierten kurzen Metallfasern zusammensetzt.
Dieses Metallfasersieb 17 ist mit ei
nem absorbierbaren Material beschichtet, wie nach
folgend im einzelnen beschrieben wird.
Fig. 2 zeigt eine zweite beispielhafte Ausführungs
form eines Körperimplantates. Das zu implantierende Teil 20
ist eine Acetabular-Hüftprothese, die üblicherweise
als Hüftbecher bezeichnet wird. Dieser Hüftbecher 20
besitzt eine glatte, sphärische Oberfläche 21, die
die andere Oberfläche eines Hüftgelenks bildet und
die sphärische Fläche 10 der Prothese der Fig. 1
aufnehmen soll. Die Außenfläche 23 des Hüftbechers
20 soll in das Acetabulum implantiert werden und ist
durch Plasmaspritzen mit einer porösen metallischen
Oberflächenschicht 24 bedeckt.
Die poröse Schicht umfaßt
ein absorbierbares Material auf mindestens einem
Abschnitt seiner Fläche, das das permanente Knochen
einwachsen in den porösen Bereich fördert.
Ein anderer häufig eingesetzter poröser Überzug
ist ein ge
sinterter Überzug aus Metallpartikeln.
Die vorstehend beschriebenen Typen von porösen Flächen
dienen nur als Beispiele. Es versteht sich, daß irgend
einer aus einer Vielzahl von Typen von porösen Flächen,
die für die Fixierung von in den Körper implantierten
Teilen verwendet werden, Anwendung
finden kann.
In Fig. 3 ist eine Gitterstruktur mit poröser Ober
fläche, wie beispielsweise die Matte 17, etwa 20fach
vergrößert dargestellt. Die Gitterstruktur 17 besteht
aus kurzen Metallfasern 18, die komprimiert sind, und
ist mit einem absorbierbaren Material 19 beschichtet,
bei dem es sich bei der dargestellten Ausführungsform
primär um α-Tricalciumphosphat handelt.
Ein Abschnitt der Gitterstruktur 17, der durchge
schnitten und mittels eines Rasterelektronenmikroskops
untersucht worden ist, ist in Fig. 4 dargestellt.
Die Oberfläche der Gitterstruktur (die aus dem Mikro
bild der Fig. 3 herausweisende Richtung) zeigt zur
unteren rechten Ecke der Fig. 4. Das Mikrobild des
entspechenden Abschnittes ist 63,5fach vergrößert.
Typische Schnitte durch die Drähte sind bei 31 und 32
dargestellt. Die Verlängerungen der Drahtabschnitte,
wie bei 32 gezeigt, sind auf die Tatsache zurückzu
führen, daß die Schnittebene mit dem Drahtdurchmesser
einen schiefen Winkel bildet. Partikel, die lose er
scheinen, wie bei 33 gezeigt, sind tatsächlich nicht
lose, sondern stellen nur Partikel dar, die
außerhalb der Schnittebene verbunden sind. Die Flächen
34, 35, 37 der Drähte sind mit dem α-Tricalcium
phosphat 33, 36 und 38 beschichtet. Bei der darge
stellten Ausführungsform beträgt die maximale Über
zugsdicke etwa 30 µm, mit einer durchschnittlichen
Dicke von 20 µm an den Außenflächen, wie bei 37, der
Außendrähte der Gitterstruktur. Der Überzug dringt etwa
500 µm in die Gitterstruktur ein, wobei die
Kontinuität des Überzuges von der Außenseite zur
Innenseite hin abnimmt. Fig. 5 zeigt eine andere An
sicht der Ausführungsform der Fig. 4, die 948fach
vergrößert ist.
Die in den Fig. 1, 3, 4 und 5 dargestellten be
schichteten Fasermetallteile werden in der folgenden
Weise hergestellt. Im Handel erhältlicher reiner
Draht wird zu Kissen geformt auf der Stelle des Implantats
gesintert. Die
Gitterstruktur wird dann mit einem Material plasma
bespritzt, das nach dem Spritzen und Abkühlen
absorbierbar ist. In entsprechender Weise können die
in Fig. 2 dargestellten beschichteten Teile 20 her
gestellt werden, indem zuerst die Metallfläche auf das
Basiselement (unter der Fläche 24) plasmagespritzt wird,
und dann auf die entstehende poröse Fläche ein Überzug
einer Substanz plasmagespritzt wird, die zu einem
bioabsorbierbaren Material abkühlt.
Nach dem vorstehend beschriebenen Verfahren wurden Me
tallzylinder hergestellt. Bei dem verwendeten Draht
handelte es sich um im Handel erhältliches reines
Titan mit einem Durchmesser von etwa 0,254 mm, das in
Längen von etwa 25,4 mm zerschnitten wurde. Das Gitter
besaß eine Dicke von etwa 2 mm und wurde auf einen
mittleren Titanstab gesintert, der einen Gewindeab
schnitt besaß, um eine Befestigung an einer
mechanischen Testvorrichtung zu ermöglichen. Die Zy
linder besaßen einen Durchmesser von 9,5 mm und eine
Länge von 50 mm. Die Fasergitterzylinder wurden dann
mit einem Material, das etwa 50% Hydroxyapatit und
50% β-Tricalciumphosphat enthielt, plasmabespritzt.
Dabei fand ein übliches Plasmaspritzverfahren Anwendung.
Die
nachfolgende Analyse des plasmabespritzten Materials
durch Röntgenstrahlbeugung ergab, daß sich das
50/50 Hydroxyapatit - β-Tricalciumphosphat-Material
primär in α-Tricalciumphosphat umgewandelt hatte. Es
verblieb etwas β-Tricalciumphosphat und Hydroxy
apatit. Die Beugungsanalysendaten zeigten ferner
einige Linien, die noch nicht identifiziert wurden,
von denen jedoch angenommen wird, daß es sich um
Hochtemperatur-Calciumphosphat handelt. Die Beugungs
linienbreite zeigte an, daß die Kristallit-Größe
geringer war als die bei einer herkömmlichen Keramik.
Die Zylinder wurden zusammen mit ähnlichen Zylindern,
die mit einer Reihe von anderen Materialien behandelt
worden waren, nämlich in der Literatur als osteogenisch
beschriebenen Materialien, wie beispielsweise β-Tri
calciumphosphat, entmineralisiertem Knochenpulver und
autogenem Knochen und Mark, in die Oberschenkelknochen
von Hunden implantiert. Nach einer Zeitdauer von zwei
Wochen bis sechs Wochen wurden die Tiere getötet,
und die Proben wurden aus den Oberschenkelknochen her
ausgezogen. Die nach dem angegebenen Verfahren hergestellten Proben
besaßen die höchste Festigkeit gegenüber Herausziehen.
Dies zeigt offensichtlich die stärkste Fixierung an
und wird allgemein als verbessertes Knocheneinwachsen
interpretiert. Diese Proben
konnten hinsichtlich Verpackung und Implantation in
den Tieren ohne jegliche Qualitätsverschlechterung
normal gehandhabt werden.
Es wird angenommen, daß die Umwandlung des Hydroapatit/
β-Tricalciumphosphat-Materials in α-Tricalcium
phosphat auf die Tatsache zurückzuführen ist, daß das
Material durch das Plasmaspritzverfahren Umwandlungs
energie erhält. Aufgrund dieser Tatsache können auch
andere Verfahren zur Aufbringung des Materials, die
Energie für die Umwandlung zur Verfügung stellen,
eingesetzt werden, um den Überzug aufzubringen, wie
beispielsweise Bedampfungsverfahren, Elektrophorese,
elektrostatische Sprühverfahren etc.
Die nach dem angegebenen Verfahren beschichtete poröse Fläche kann auch in Kombination
mit vielen Arten von anderen implantierbaren Fixatoren
als beschrieben verwendet werden, beispielsweise mit
Knieprothesen, Knochenplatten, Intramedularstäben etc.
Es können auch andere Dicken als vor
stehend beschrieben Anwendung finden. Das Basiselement
des Implantats und/oder die poröse Schicht können auch
aus Metallen bestehen, beispielsweise Kobalt-Chrom-
Stahllegierungen, Edelstahl etc. und anderen Materialien,
die für die Implantation im Körper geeignet sind.
Claims (3)
1. Verfahren zur Herstellung eines Körperimplantats mit
folgenden Schritten:
- - Herstellen eines Basiselementes, das einen Knochen befestigungsbereich mit poröser Oberfläche aufweist;
- - Herstellen eines Überzugsmateriales; und
- - Aufbringen des Überzugsmateriales auf mindestens ei nen Abschnitt des Knochenbefestigungsbereiches unter Zuführung von thermischer Energie,
dadurch gekennzeich
net, daß als Überzugsmaterial Hydroxyapatit und Beta-
Tricalziumphosphat verwendet, beim Aufbringen auf ei
ne Temperatur über 1350°C erhitzt und dabei in ab
sorbierbares Alpha-Tricalziumphosphat umgewandelt
werden.
2. Verfahren nach Anspruch 1, dadurch gekennzeichnet,
daß das Überzugsmaterial durch Plasmaspritzen aufge
bracht wird.
