Abstract
Description
The invention relates to an HF surgical device with an HF generator, which is a setting element for the on represent the delivered RF power, and with a surgical hand connected to the HF generator grabbed the one active electrode and at least one Switch for switching the HF generator on and off HF current flowing to the active electrode.
Such an HF surgical device is, for example, from the Siemens publication "Radiotom 804 - Betriebsanlei tung ", order no. A91001-M3391-L040-01 this HF surgical device becomes a surgical handle used, which - in the direction of the longitudinal axis of the handle seen - two switches for switching the HF- flowing from the HF generator to the active electrode Has electricity. The two switches are as fingers trained switch. With one finger switch the operating mode "cutting" by finger pressure and with the other finger switch the operating mode "coagulation" selected. In other words, it's the surgeon possible, directly on the surgical handle depending on the type of Activity the HF current type "cutting" or "coagulation" to select. The characteristic curves in these two HF operations types differ among other things in the respective given RF power. In each of the two operating modes can be given during an operation RF power can be fine-tuned. This happens by pressing a button on the front panel of the actual HF surgical device, which is usually some distance away is placed in the operating field.
Must now the HF Lei during such an intervention in one of the two operating modes "Cutting" or "Coagulation" can be changed, so the surgeon is on instructed non-sterile auxiliary personnel who are at the genann front panel the setting desired by the operator makes. When instructed by the surgeon, it can hearing errors. Moreover, the setting is about an assistant awkward and time consuming and enables does not immediately respond to a surgeon changed situation in the operating field, for example if bleeding suddenly occurs.
The object of the invention is an HF surgical device type mentioned so that the Chi rurg when setting the RF power not on auxiliary staff is instructed.
This object is achieved in that a first and a second on the surgical handle Actuator are arranged, the one direction with the setting element of the HF generator such are connected that an actuation of the first actuator supply element an enlarged and an actuation of the second actuator a reduced delivery of the HF power causes.
In an HF surgical device designed in this way is the power control directly from the surgical handle possible. The surgeon can therefore use the HF Lei himself stung by actuating one of the two actuating elements set elements. He is therefore not in this regard dependent on auxiliary staff. That enables him Permissive and quick work in the operating field.
The two actuating elements are preferred as fingers trained switch. They are preferably - in rich direction of a parallel to the longitudinal axis of the handle seen the reference axis - on the surgical handle arranged one above the other.
Es zeigtFurther advantages and refinements of the invention he give themselves from the description of the attached Figu ren and from the subclaims. It shows
Fig. 1 einen erfindungsgemäß ausgestalteten Chirurgie- Handgriff für ein HF-Chirurgiegerät und Fig. 1 shows a surgical handle designed according to the invention for an electrosurgical unit and
Fig. 2 shows the surgical handle shown in Fig. 1 in cooperation with electronic components of the HF surgical device.
In Fig. 1, an embodiment of a surgical handle 2 is shown, which is elongated in the usual way and is provided with an active electrode 4 at the end. This electrode 4 is brought into contact with the tissue to be cut or coagulated during high-frequency surgical operations. When using the HF surgical device, the surgeon must be able to choose between the HF current types "cutting" and "coagulation" depending on the type of operative situation. This is possible because the surgical hand grip 2 is provided with two switching elements or switches 6 and 8 . These switches 6 , 8 are formed as a finger scarf ter and are actuated by pressing the index finger. The actuation of the first switch 6 causes the switching on of the HF current type "cutting", and the actuation of the second switch 8 causes the switching on of the HF current type "coagulation". The two switches 6 , 8 are arranged along a reference axis 10 which is parallel to the longitudinal axis 12 of the handle.
During an operation it is also necessary for the surgeon to change the delivered RF power according to the type of tissue, the severity of a bleeding, the surgical goal, etc. So that the surgeon is not dependent on auxiliary personnel who must make an operation of the actual HF surgical device on call, the surgical handle according to FIG. 1 enables the surgeon himself to change this HF power.
For this purpose, a first and a second actuating element 16 and 18 are arranged on the surgical handle. These actuating elements 16 , 18 are preferably also designed as finger switches. These two switching or actuating elements 16 , 18 are provided for plus or minus adjustment of the HF power of the HF surgical device. The two actuating elements 16 , 18 are - see in the direction of a further, also parallel to the longitudinal axis 12 reference axis 20 ge - on the surgical handle 2 in a row angeord net. In particular, the arrangement is such that the two reference axes 10 , 20 are arranged offset by 90 ° with respect to the handle longitudinal axis 12 . The first actuating element 16 lies next to the first switch 6 , and the second actuating element 18 lies next to the second switch 8 . The control and power lines are summarized in a cable 21 .
As will become clear later with reference to FIG. 2, the actuating elements 16 , 18 attached to the surgical handle 2 are connected via a device with the setting organ of the HF generator in the actual HF surgery device in such a way that an actuation of the first one Actuator 16 has an enlarged and actuation of the second actuating element 18 has a reduced output of the RF power.
The operation when the RF power is increased or decreased is carried out by holding two buttons or switching elements 6 , 16 or 8 , 18 . This prevents unintentional adjustment of the set RF power. In this way, the surgeon can easily set the HF power in the two HF operating modes "cutting" and "coagulation". For example, the requirement that the co-gulation channel should give more RF power means that the coagulation key 8 and the plus key 16 have to be held for some time. For example, on the other hand means the requirement that the cutting channel should deliver less power, that the cutting button 6 and the minus button 18 are to be held for some time.
The two switches 6 , 8 and the two actuating elements 16 , 18 , which are each preferably designed as pushbuttons, are selected with a view to easy sterilization.
From Fig. 2, the interaction of the surgical handle 2 with other components of an HF surgical device is shown in a schematic diagram. The central component of this HF surgical device is an HF generator 22 , which has an input or an adjusting element 24 for adjusting the HF power output at its output 26 . This output 26 is connected directly to the surgical handle 2 .
As already mentioned, actuation of the first actuating element 16 causes an enlargement and actuation of the second actuating element 18 reduces the RF power output at the output 26 . This is effected by control signals s , which are removed from the actuating elements 16 , 18 and are supplied to an electrical device 28 . This device 28 includes a pulse generator 30 and a downstream up-down counter 32nd The control signals s can come about by switching on a voltage source (not shown) by means of the actuating elements 16 , 18 . The up-down counter 32 is followed by a digital / analog converter 34 , the output of which is connected to the setting element 24 . At the output of the up-down counter 32 , a device 36 for di gital performance display is still connected.
The counting direction of the forward-downward counter 32 can be selected via the control signals s . For example, an increase in the counter reading caused when the actuating element 16 is pressed corresponds to an increase in the desired HF power, and a decrease in the counter reading caused when the actuating element 18 is pressed corresponds to a decrease in the desired HF power. An analog output variable proportional to the counter reading is fed via the digital-analog converter 34 to the setting gate 24 on the HF generator 22 . In this way, an operative connection is established between the output of the up-down counter 32 and the HF generator 22 .
The counting input of the up-down counter 32 is struck by the pulse generator 30 with rectangular pulses. The pulse rate of the pulse generator 30 is changeable: When the first or second actuating elements 16 or 18 are actuated, pulses with a low pulse rate and then pulses with a comparatively high pulse rate are delivered to the counting input of the up-down counter 32 . In this way, it is sufficient that when actuating the first or second loading actuating elements 16 or 18 is first counted at a slow counting speed; after that comes a quick count for a large area change. In other words, when one of the buttons 16 , 18 is pressed, the RF power is initially changed slowly, but then at a higher speed. This change speed is maintained as long as the relevant key 16 or 18 is pressed, ie until the desired RF power value is reached.
In Fig. 2, a special embodiment of a device 28 is shown schematically. However, other solutions are also possible. For example, the device 28 can comprise a DC motor, the direction of which is determined by the polarity of the control signal s . More generally speaking, the device 28 can comprise a motor, the direction of which is selected by actuating the first or second actuating element 16 or 18 . This motor is in operative connection with an actuator 24 of the HF generator 22 . For example, it can be a potentiometer that is driven by the motor; A voltage for controlling the HF power of the HF generator 22 is tapped at this potentiometer.
