Description
Die vorliegende Erfindung bezieht sich auf ein chirurgi
sches Nähgerät, insbesondere zur Durchführung der laparosko
pischen Appendektomie. Laparoskopische Operationen wie
Cholezystomie, Gastrostromie, Sterilisation und Appendekto
mie haben in den letzten Jahren eine weite Verbreitung
gefunden. Bei den bekannten Operationstechniken finden je
doch keine Nähgeräte zum Legen von Klammernähten Anwendung.
Allgemein bekannt ist die Anwendung chirurgischer Klammer
nähgeräte in der kardiovaskulären und pulmonalen Chirurgie
und bei den Operationen am Magen-Darm-Kanal, mit denen
Klammernähte hoher Festigkeit gelegt werden können und die
Operationsdauer verkürzen. Jedoch sind alle bekannten
chirurgischen Nähgeräte für die endoskopische Verwendung
unbrauchbar.
Aus dem SU-Erfinderschein 1 25 867 ist ein chirurgisches
Nähgerät zum Anlegen von seitlichen Magen-Darm-Anastomosen
bekannt. Dieses hat eine Klammerbranche mit Nuten für die
Klammern und mit einem Handgriff am einen Ende und eine
an der Klammerbranche angelenkte Stützbranche mit Aus
fräsungen zum Zurückbiegen der Klammerfüßchen. Ein Aus
stoßwerk besorgt das Ausstoßen der Klammern aus ihren Nuten,
während eine Verriegelungsvorrichtung die Branchen in zu
sammengeschwenkter Stellung hält.
In diesem bekannten Nähgerät ist das die Branchen ver
bindende Gelenk an einem Ende des Handgriffs in der
Klammerbranche angeordnet. Dadurch stehen die Branchen
in der auseinandergeschwenkten Lage unter einem Winkel
gegeneinander, was eine Verwendung dieses chirurgischen
Nähgeräts bei endoskopischen Handhabungen unmöglich macht.
Die Erfindung geht aus von einem aus der US-PS 45 66 620 be
kannten ähnlichen Nähgerät, bei dem die Baueinheit aus
Klammernbranche und an dieser angelenkten Stützbranche
gelenkig, und zwar mittels eines Kugelgelenks, am Ende
eines Schafts des Handgriffs angeordnet ist. Bei dieser
Ausbildung ist zwar die Manövrierfähigkeit verbessert, eine
Eignung für endoskopische Anwendungen wird jedoch auch
hierdurch nicht erzielt. Die Klammerbranche ist schon
für sich sehr sperrig und steht immer unter einem Winkel
zum Schaft des Handgriffs.
Aufgabe der Erfindung ist die Schaffung eines chirurgischen
Nähgeräts, mit dem die Operationszone durch den Operations
tubus eines Wundsperrers erreicht werden kann, so daß
laparoskopische Operationen möglich werden. Es soll also
letzten Endes möglich werden sowohl ein Klammernähgerät
mit seinen Handhabungsvorteilen zu verwenden als auch eine
laparoskopische Operationstechnik mit ihrem verringerten
Traumatismus anzuwenden.
Ausgehend von einem chirurgischen Nähgerät der oberbegriff
lich vorausgesetzten Bauart wird diese Aufgabe erfindungs
gemäß durch die im Patentanspruch 1 angegebenen Merkmale
gelöst. Bei einer solchen erfindungsgemäßen Ausbildung
ist das Gerät in der auseinandergeschwenkten Strecklage
so schlank, daß es durch eine enge Öffnung eingeführt
und in Position gebracht werden kann, insbesondere durch
einen nur einen Einstich erfordernden Operationstubus.
Zweckmäßige Weiterbildungen der Erfindung sind in den
Unteransprüchen angegeben.
Das erfindungsgemäße chirurgische Nähgerät macht eine
Appendektomie nach dem laparoskopischen Verfahren mit zwei
Durchstichen ohne Eröffnung der Bauchhöhle möglich. Dadurch
wird der Traumatismus solcher Operation herabgesetzt.
Das Nähgerät ist im Gebrauch bequem und verschließt die
Hohlorgane mit hoher Zuverlässigkeit, z. B. den Stumpf
bei der Appendektomie.
Nachstehend wird die Erfindung durch die Beschreibung
von Ausführungs- und Anwendungsbeispielen anhand der
Zeichnungen weiter erläutert. Es zeigt:
Fig. 1 das chirurgische Nähgerät in einer ersten Aus
führungsform mit bis in die Strecklage auseinanderge
schwenkten Branchen in perspektivischer Darstellung;
Fig. 2 das Gerät gemäß Fig. 1 in zusammengeschwenkter
Stellung der Branchen;
Fig. 3 Seitenansicht des Klammerstößels;
Fig. 4, 5, 6, 7, 8 einzelne Phasen einer Appendektomie

Das in Fig 1, 2 gezeigte chirurgische Nähgerät hat eine
Klammerbranche 1, die die Fortsetzung eines Schafts 26
eines Handgriffs 2 darstellt. Am distalen Ende der Klammer
branche 1 ist mittels einer Gelenkachse 3 das eine Ende
einer Stützbranche 4 angelenkt. Diese ist relativ zur
Klammerbranche 1 um 180° verschwenkbar und stellt in ihrer
in Fig. 1 gezeigten gestreckten Endstellung eine Verlängerung
der Klammerbranche 1 dar. Dieses ist die auseinanderge
schwenkte Stellung des Geräts.
In ihrer anderen Endstellung gemäß Fig. 2 ist die Stütz
branche 4 an die Klammerbranche 1 parallel zu dieser heran
geschwenkt. Dieses ist die zusammengeschwenkte Stellung
des Geräts.
Die Schwenkung der Stützbranche 4 wird bewirkt von einem
Verschwenkwerk mit einem Handhebel 5, der um eine Schwenk
achse 6 am Handgriff 2 schwenkbar ist, und einer Zugstange
7, deren eines Ende an der Stützbranche 4 angreift und
deren anderes Ende einen Stift 8 trägt, der in einen Aus
schnitt 9 des Handhebels 5 eingehängt ist. Zur Erzielung
symmetrischer Beanspruchungen kann das Verschwenkwerk eine
weitere, auf der anderen Seite des Schafts 26 und der Klammer
branche 1 verlaufende Zugstange 7 aufweisen. Die Anordnung
der Achsen ist derart, daß beim Niederdrücken des Handhebels
5 die Zugstange 7 eingezogen wird und die Stützbranche 4
um die Gelenkachse 3 aus der gestreckten Lage gemäß Fig.
1 in die zusammengeschwenkte Lage gemäß Fig. 2 schwenkt.
In der Klammerbranche 1 sind ein Längsschlitz 10 und zu
beiden Seiten desselben Nuten 11 für n-förmige Klammern ausge
arbeitet. Die Klammern bestehen beispielsweise aus Tantal
draht mit den Abmessungen 0,2×3, 0×4,0 mm. Gegenüber
den Nuten 11 sind in der Stützbranche 4 entsprechende zwei
Reihen von Ausfräsungen 12 zum Einbiegen der Klammerfüßchen
beim Ausschieben der Klammern ausgearbeitet sowie eine dem
Längsschlitz 10 gegenüberliegende Längsnut 13.
Zum Ausschieben der Klammern aus ihren Nuten 11 dient ein
an seinem einen Ende zugespitzter Klammerstößel 14 (Fig. 1, 3),
der ein Messer 15 aufweist und verschieblich in dem Längs
schlitz 10 angeordnet ist. Das zugespitzte Ende des Klammer
stößels wirkt mit aus den Zeichnungen nicht ersichtlichen
Stoßplatten zusammen, die innerhalb des Schlitzes 10 und
unterhalb der Klammern untergebracht sind und beim Durch
schieben des rampenartig zugespitzten Endes des Klanmer
stößels 14 hochgeschoben werden und die Klammern aus ihren
Nuten 11 ausschieben. An seinem rückwärtigen Ende hat der
Klammerstößel 14 eine Drucktaste 16 sowie einen Stift 17,
der längs eines im Handgriff 2 vorgesehenen Führungsschlitzes
18 gleitet.
Eine Verriegelungsvorrichtung hält während des Ausschiebens
der Klammern die Branchen 1, 4 in ihrer zusammengeschwenkten
Stellung. Zu dieser gehört ein am freien Ende der Stütz
branche befestigtes Schloß 19 in Form einer Schelle mit nach
außen abgebogenen Riegelnasen sowie mit diesen zusanmen
wirkende Keilleisten 22, die mittels eines Stoßhebels 20
vorschiebbar sind, der eine Klinkensperre 21 aufweist. Beim
Niederdrücken des Stoßhebels 20 werden die Keilleisten 22
vorgeschoben und erfassen mit den schrägen Keilflächen ihres
freien Endes die Riegelnasen des Schlosses 19 der einge
schwenkten Stützbranche wie in Fig. 2 ersichtlich. Die Klinken
sperre 21 verhindert bis zu deren Freigabe ein Lösen der so
erreichten Verriegelung.
Das beschriebene chirurgische Nähgerät wird, beispielsweise
zur Durchführung einer laparoskopischen Appendektomie, wie
folgt gebraucht:
Vor Beginn der Operation werden die Klammern in ihre Nuten
11 in der Klammerbranche 1 eingelegt. Hiernach wird in die
Bauchhöhle des Patienten ein Laparoskop am McBurneyschen
Druckpunkt eingeführt und ihre Revision durchgeführt.
Das Nähgerät kann zur Benutzung kommen, wenn der Fortsatz
frei liegt oder sich zumindest in den lockeren Kommissuren
befindet.
Aufgrund des Ergebnisses der durchgeführten Laparoskopie
wird nun der Tubus eines Wundsperrers durch einen anderen
Durchstich hineingesteckt und durch diesen das chirurgische
Nähgerät in seiner auseinandergeschwenkten Stellung in die
Bauchhöhle eingeführt (Fig. 4, 5). Dann wird der Fortsatz
in der Gegend seiner Spitze mit einer durch den Operations
kanal des Laparoskopes eingeführten Faßzange 23 (Fig. 5) er
faßt und die Klammerbranche 1 des Nähgeräts unter die Basis
des Fortsatzes und Gekröses geschoben.
Jetzt wird die Stützbranche 4 durch Drücken am Handhebel 5
des Handgriffs 2 um die Gelenkachse 3 um 180° eingeschwenkt,
wobei der Fortsatz und das Gekröse, wie aus Fig. 6 er
sichtlich, in der Nähe seiner Basis zwischen den Branchen
erfaßt wird. Die zusammengeschwenkte Stellung des Näh
geräts wird dadurch verriegelt, daß der Stoßhebel 20 nieder
gedrückt wird. Dadurch werden die Keilleisten 22 vorwärts
geschoben und erfassen mit ihren zugespitzten Enden die ab
gebogenen Riegelnasen der Schelle des Schlosses 19, wodurch
die Branchen 1 und 4 aneinander gedrückt werden. Dadurch
werden die Basen des Fortsatzes und Gekröses zwischen den
Arbeitsflächen der Branchen 1 und 4 mit dosierbarer Kraft
zusammengedrückt.
Hiernach wird der Klammerstößel 14 mit dem Messer 15 durch
Vorschieben der Drucktaste 16 in dem Längsschlitz 10 vor
wärtsgeschoben. Dabei hebt sein rampenartig zugespitztes
Vorderende die Stoßplatten und diese schieben die Klammern
aus den Nuten 11 heraus. Die Klammerfüßchen durchstechen
die zusammengedrückten Wandungen des Fortsatzes und beide
Gekröseblätter und biegen sich beim Auftreffen in den
Ausfräsungen 12 um. Unmittelbar nach dem Legen der Klammer
nähte 24 durchschneidet die Schneidkante des Messers 15 den
Fortsatz und das Gekröse zwischen diesen.
Jetzt wird die Klinkensperre 21 entsperrt und so die Ver
riegelung durch die Keilleisten 22 aufgehoben. Das Nähgerät
wird in seine auseinandergeschwenkte Stellung zurückver
schwenkt, indem der Handhebel 5 zurückverschwenkt wird.
In der gestreckten Lage kann das Gerät durch den Operations
tubus des Wundsperrers wieder aus der Bauchhöhle herausge
nommen werden. Der abgeschnittene Fortsatz wird durch den
laparoskopischen Wundsperrer 25 (Fig. 8) mittels der Faß
zange 23 aus der Bauchhöhle herausgezogen.
Zur Erhöhung der Sicherheit, daß durch die Zugkraft der
Zugstange 7 die Stützbranche 4 um den vollen Schwenkwinkel
von 180° auf die Klammerbranche zurückgeschwenkt wird, ist
in der Ausbildung gemäß Fig. 9 bis 12 die Stützbranche 4
über ein Zwischenglied 30 mit der Klammerbranche 1 verbunden,
wobei das Zwischenglied 30 mittels einer Gelenkachse 3′ an
der Klammerbranche 1 und die Stützbranche 4 mittels einer
Gelenkachse 3′′ am Zwischenglied 30 angelenkt ist. Das Zwi
schenglied 30 hat an seinem zur Gelenkachse 3′ gelegenen
Ende eine Anschlagfläche 31, die sich beim Einschwenken auf
den Boden der Klammerbranche 1 legt und den Einschwenk
winkel des Zwischengliedes auf 90° begrenzt.
Zur Verriegelungsvorrichtung gehört ein am freien Ende der
Stützbranche 4 ausgebildetes Schloß 19, welches in dieser
Ausführungsform einwärts abgebogene Riegelnasen aufweist,
die beim Einschwenken der Stützbranche in eine Öffnung
25 am Ende des Schaftes 26 greifen. Auf diese Weise wird
die Axiallage der Stützbranche 4 gegenüber der Klammer
branche 1 und damit die genaue Lage der Ausfräsungen 12
gegenüber den Klammernuten 11 unverrückbar fixiert. Zur
Verriegelung werden die einwärts gebogenen Riegelnasen
des Schlosses 19 innerhalb der Öffnung 25 von den schrägen
Keilflächen der Keilleisten 22 erfaßt.
In beiden Ausbildungen ist sicherzustellen, daß die Stütz
branche 4 durch die Zugstange 7 wenigstens soweit einge
schwenkt wird, bis die schrägen Keilflächen am Ende der
Keilleisten 22 die Riegelnasen des Schlosses 19 erfassen
können. Hierzu kann die Zugstange 7 derart am oder im
Schaft 26 geführt sein, daß gegen Ende der Einschwenkbe
wegung das Ende der Zugstange 7 außer auf Zug auch noch
auf Biegung in dem Sinne beansprucht wird, daß die Gelenk
achse 33 am vorderen Ende der Zugstange 7 nach unten ge
zogen wird und ein entsprechendes Einschwenkmoment auf
die Stützbranche 4 ausgeübt wird. Die entstehende Biegung
der Zugstange ist in den Zeichnungen angedeutet. Auf diese
Weise kann zuverlässig sichergestellt werden, daß die Zug
stange 7 eine Schwenkung um volle 180° bewirkt.
Zur experimentellen Erprobung sind Versuchsmuster des be
schriebenen chirurgischen Nähgeräts hergestellt und an
Leichen von Menschen und Hunden geprüft worden. Diese Unter
suchungen haben die Brauchbarkeit der Konstruktion be
stätigt.
Beispielsweise wurde das Nähgerät erprobt an der Leiche
eines Mannes K., 34 Jahre alt, der wegen mechanischer
Asphyxie umgekonmen war. Die Laparoskopie zeigte, daß der
Fortsatz 7,6×0,5 cm medial lag und von den Kommissuren
frei war. Mit Hilfe des beschriebenen Nähgeräts wurde
eine Appendektomie durchgeführt. Die Laparotomie ge
schah unter Unterbringung des Krummdarmes, während auf den
aufsteigenden Darm die Staubinde angelegt wurde. Hiernach
wurde der Druck im Blinddarm bis auf 90 mm QS gebracht und
die Luftdichtigkeit des Schnitts beobachtet. Bei den er
wähnten Druckgrößen blieb der Schnitt des Stumpfes luft
dicht. Damit kann angenommen werden, daß das vorgeschlagene
chirurgische Nähgerät in der Praxis das laparoskopische
Verfahren der Appendektomie gefahrlos ermöglicht.
Claims (13)
1. Chirurgisches Nähgerät mit an einem Handgriff (2)
befestigter Klammerbranche (1), die ein Ausstoßwerk
für in Nuten (11) untergebrachte Klammern aufweist,
und mit einer an der Klammerbranche schwenkantreibbar
angelenkten Stützbranche (4) mit Ausfräsungen (12) zum
Abbiegen der Klammerschenkel
sowie mit einer die Stützbranche (4) in ihrer an die
Klammerbranche (1) herangeschwenkten Stellung haltenden
Verriegelungsvorrichtung,
dadurch gekennzeichnet, daß das die Branchen (1, 4) verbindende Gelenk (3) am freien Ende der Klammerbranche (1) angeordnet ist und dieses Gelenk eine Auseinanderschwenkung der Branchen derart ermöglicht, daß die Stützbranche (4) eine Ver längerung der Klammerbranche (1) darstellt.
dadurch gekennzeichnet, daß das die Branchen (1, 4) verbindende Gelenk (3) am freien Ende der Klammerbranche (1) angeordnet ist und dieses Gelenk eine Auseinanderschwenkung der Branchen derart ermöglicht, daß die Stützbranche (4) eine Ver längerung der Klammerbranche (1) darstellt.
2. Chirurgisches Nähgerät nach Anspruch 7, gekennzeich
net durch einen Schwenkwinkel des Gelenks (3) von
180°.
3. Chirurgisches Nähgerät nach Ansprüchen 1 oder 2,
dadurch gekennzeichnet, daß der Schwenkantrieb aus
wenigstens einer an der Stützbranche (4) angreifenden
Zugstange (7) besteht.
4. Chirurgisches Nähgerät nach Anspruch 3, dadurch
gekennzeichnet, daß die Zugstange (7) mittels eines
Handhebels (5) betätigbar ist, der schwenkbar am Hand
griff (2) angelenkt ist.
5. Chirurgisches Nähgerät nach einem oder mehreren
der vorhergehenden Ansprüche, dadurch gekennzeichnet,
daß die Verriegelungsvorrichtung aus einem
Schloß (19) am freien Ende der Stützbranche (4)
und einer mit diesem zusammenwirkenden Keilleiste (22)
besteht, die gegenüber dem Handgriff (2) vorschiebbar
ist und deren Keilfläche das Schloß (19) erfaßt.
6. Chirurgisches Nähgerät nach Anspruch 5, gekennzeich
net durch einen mittels einer Zahnklinkensperre (21)
arretierbaren Hebel (20) zum Vorschieben der Keilleiste
(22).
7. Chirurgisches Nähgerät nach einem oder mehreren der
vorhergehenden Ansprüche, dadurch gekennzeichnet, daß
das Ausstoßwerk ein in der Klammerbranche (1) vor
schiebbarer Klammerstößel (14) ist und ein Messer (15)
zum Durchtrennen des zwischen den Branchen eingeklemmten
Gewebes aufweist.
8. Chirurgisches Nähgerät nach Anspruch 7, gekennzeichnet
durch zwei parallele Reihen von Nuten (11) und Ausfräsungen
(12) für die Klammern, wobei des Messer (15) in einem
Längsschlitz (10) zwischen den Nutenreihen geführt ist.
9. Chirurgisches Nähgerät nach Ansprch 7, gekennzeichnet
durch drei parallele Reihen von Nuten (11) und Aus
fräsungen (12) für die Klammern, wobei das Messer (15)
in einem Längsschlitz (10) geführt ist, auf dessen einer
Seite eine Reihe von Nuten und Ausfräsungen liegt und
auf dessen anderer Seite zwei Reihen von Nuten und Aus
fräsungen liegen.
10. Chirurgisches Nähgerät nach Anspruch 5, dadurch ge
kennzeichnet, daß das Schloß (19) eine Schelle mit aus
wärts abgebogenen Riegelnasen ist, die zusanmenwirken
mit den Schrägflächen der keilförmigen Enden von zwei
beidseits eines Schaftes (26) geführten Keilleisten
(22; Fig. 1, 2).
11. Chirurgisches Nähgerät nach Anspruch 5, dadurch ge
kennzeichnet, daß das Schloß (19) ausgebildet ist als am
freien Ende der Stützbranche (4) einwärts abgebogenen
Riegelnasen (Fig. 10), die im eingeschwenkten Zustand
der Stützbranche (4) in eine Öffnung (25) am Ende des
Schaftes (26) greifen und in dieser von den Schrägflächen
der keilförmigen Enden von Keilleisten erfaßt werden.
12. Chirurgisches Nähgerät nach einem oder mehreren
der vorhergehenden Ansprüche, dadurch gekennzeichnet, daß
die Stützbranche (4) mittels eines Zwischengliedes (30)
an der Klammerbranche (1) angelegt ist, dessen Schwenk
winkel gegenüber der Klammerbranche durch eine Anschlag
fläche (31) begrenzt ist.
13. Chirurgisches Nähgerät nach einem oder mehreren der
Ansprüche 3 bis 12, gekennzeichnet durch eine Führung
der Zugstange (7) derart, daß diese in der zusammenge
schwenkten Lage des Geräts derart auf Biegung beansprucht
ist, daß auf die die Zugstange (7) mit der Stützbranche
(4) verbindende Gelenkachse (33) eine Kraftkomponente
nach unten zur Klammerbranche (1) hin wirkt.
