Description

The invention relates to a device for the treatment of Skin changes in the human or animal body according to the preamble of claim 1.

Dazu werden u.a. Helium-Neon, Argon- oder CO2-Laser verwendet. It has been known for some years to have skin changes such as B. Treat tattoos, warts, fire stains or the like by a laser beam. The laser beam enables gentle and anemic treatment of pigment or structural changes in the skin. Helium-neon, argon or CO 2 lasers are used for this purpose.

Usually, the laser beam is applied to the hand acting skin area. Not just for large areas Application inevitably leads to considerable swan the effects of energy from the laser. This sometimes leads to increased scarring, sometimes not enough irradiated areas. An exact dosage of the energy amount that is introduced into a volume element of the skin, is therefore not possible in this way.

It is also possible to use a laser beam automatically a treatment area, e.g. B. at constant speed, to lead, whereby an improved dosing can be achieved is. However, there are considerable problems with treatment limits, because the laser beam should only area to be treated.

The invention has for its object a device for the treatment of skin changes of the human or animal body using a laser beam, which an exact dosing of the energy input into the Allows skin over the entire area to be treated without that adjacent healthy skin is attacked.

This object is achieved by the inven in claim 1 solved. Advantageous developments of the invention are specified in subclaims.

According to the invention is an image acquisition system for recording and storing the images of a skin area to be treated intended. From one or more saved images are about a discriminator with adjustable decision the spatial coordinates of the area to be treated headed. The spatial coordinates obtained in this way become one Command sequence derived to control the impact coordi the laser beam on the area to be treated.

The image acquisition of the skin area to be treated can e.g. B. done via a TV camera, preferably several Bil which are recorded in different spectral ranges. By comparing the pictures of the saved pictures selectable discriminator threshold the room coordinates derive nates of the area to be treated with the aim of to precisely narrow down the specific skin change and forth to work out.

Another way of capturing images is that a low power laser beam is used, its ener no influence on the skin area. This Laser beam is applied to the skin area intended for treatment  offers guided like a grid. The ge from the skin surface scattered light is detected by an optical receiver, and the intensity and / or spectral range of the scattered th laser light is then used to build up an image of the treating skin area. Here, too, the determination the treatment limits by covering the area in ver different spectral ranges and subsequent image comparison when the discriminator threshold is set.

A third option is to help the area to detect a laser, the laser beam over a Interferometer, especially Michelson interometer in two superimposed wave trains is split. With this out the skin area is then illuminated. Topo graphic bumps then appear limited by ge closed light-dark rings. The generated image is then recorded with a TV camera, saved and z. B. on displayed on a screen. By predetermined decision The skin area to be treated can also swell be delimited in this way, e.g. B. for the treatment of Warts, the pigmentation of which is usually not different the surrounding skin.

The invention enables an almost automatic treatment skin pigmentation or topographical skin changes changes. The exact image capture of the Ge to be treated offers a treatment that almost no damage adjacent healthy skin. Through a through determined the spatial coordinates of the area to be treated programmed control of the laser beam can be an exact Dosage of the impact energy can be achieved. In a be preferred further development of the invention can be energy density also controlled depending on spatial coordinates  that arise when another decision is made threshold for the acquisition of spatial coordinates is placed.

The invention enables an exact adaptation of the treatment the type and structure of an area to be treated. A desired amount of energy can be specified very precisely the. Contrary to manual operation, in which only the Laserlei Stung is precisely adjustable, also exists according to the invention the possibility of the treatment speed, the beam diameter and precisely define the line spacing.

The interaction of light with skin takes place via scattering and absorption. The decrease in intensity of one in the skin incoming light beam is almost exponential. Around Reducing damage to healthy tissue is essential for For the purposes of the invention, a laser wavelength is selected at which the absorption path in healthy skin is as large as possible. Before the laser wavelength used is preferably in the range  between 400 and 1200 nanometers. In particular come into question permanent laser with a high penetration depth into biological Material. Preferably 5 to 15 W lasers are used. To simplify the handling and to increase the For safety, lasers with visible light should be used.

The light absorbed by the tissue is converted into heat, among others is dissipated via diffusion. The spatial resolution of a dermatology laser is accordingly by a thermal diffusion length characterizing the skin borders. Successful treatment implies an increase in temperature in the skin of approx. 25 ° C ahead. At 1 cm / second treatment speed therefore requires a laser power of approximately 5 watts.

The structure of the invention is approximately as follows:

An argon laser 1 , which is arranged on an optical bench, is preferably used. The output beam 8 of the laser 1 is directed to a beam deflection unit 5 , which is arranged in such a way that the best possible solid angle is ensured for the treatment of the patient. An XYZ scanner with mirrors 6 and 7 with high positioning accuracy is preferably used as the beam deflection unit. To control the scanner, the start and end coordinates and the scanning speed of a vector to be traversed are transmitted to it. This control data is supplied by a process computer via a computer interface.

To detect the skin area 2 to be treated, the following devices can be used, depending on the structure to be treated.

With pigmentations, such as. B. fire painting, tattoos, or the like, an image acquisition system can be used, the z. B. enables a light-dark distinction. For this purpose, a video camera 4 can be used, which delivers a black and white image, which is then stored. Preferably, the area to be treated by several spectral limited filters such. B. red, yellow, green, blue irradiated. The TV camera takes a picture of every colored exposure. To determine the limits of the treatment area, the various recordings are then compared with one another by logical comparison of the stored pixels. The pixel comparison can e.g. B. additive or subtractive. The folding of an image with a folding function or digital filtering, which, for. B. can also be two-dimensional. This evaluation is preferably carried out in a process computer that works with an optimization algorithm in order to be able to differentiate the treatment area from the adjacent healthy area as best as possible. The acquisition parameters for certain types of skin changes 3 can also be stored by the process computer and can then form the basis of the evaluation for later treatments. The decision threshold which is set for the delimitation of the skin area to be treated from the area not to be treated can be selected to be adjustable, but it can also be determined by the process computer itself in an optimization process.

Another possibility of image acquisition ( Fig. 2) is that first a laser with low power is guided in a grid pattern over the surface of the skin area to be treated. The energy density of this laser is so low that no skin damage is caused in this way. The laser light scattered from the surface of the skin is detected by a concave mirror 9 , which has a passage opening 11 for the laser beam, and an optical receiver 10 . The intensity and / or the spectral range (e.g. in the case of lasers with a tunable wavelength) is stored under assignment to the corresponding incident coordinates of the laser light and is then available in a form comparable to that of a picture taken with a TV camera. This recording can be evaluated in the same way as stated above.

For certain skin changes that are not associated with a pigmentation, such as. B. warts, the use of a TV camera or the grid detection with the help of a laser beam is not possible. In this case, the laser beam is preferably split up into a superimposed Michelson interferometer 12 into two superimposed wave trains ( FIG. 3) which are directed onto the area of skin to be treated. Bumps then appear as closed lines that can be recorded with a TV camera. Spatial coordinates can be derived from such a recording, which enable a precise delimitation of a region to be treated. In addition, the degree of skin change increase in healthy skin can be determined in this way, since the number of lines generated represents a degree for the amount of skin change. The height information can then z. B. can be used to control the energy density of the treatment laser on the skin area to be treated. The aforementioned measures can also be combined so that an optimal detection can take place depending on the type of change.

The device according to the invention makes it possible to determine the scanning speed, the laser power, the spot diameter and the line spacing of the treatment laser guided in a grid pattern over the skin area to be treated. The spatial coordinates obtained via the image acquisition system 15 are converted according to FIG. 4 in a computer unit 14 into a command sequence which is passed on to the XYZ scanner ( 5 ), which precisely positions the light beam from the laser. The choice of scanning speed, laser power, spot diameter and line spacing can be determined automatically by the computer unit by specifying the type of skin change on the control unit 13 .

So that the space determined by the imaging system coordinate with the laser beam during treatment does not change, the skin area to be treated must be exactly in be positioned with respect to the treatment facility. With spatial changes in the position of the skin to be treated area is a new acquisition of the spatial coordinates conducive. To avoid this disadvantage can be provided be that the absolute coordinates of the Ge to be treated offers z. B. detected via two marked fixed points the fact that their spatial position is constant during treatment is checked and in case of changes a coordinate trans formation of the treatment coordinates causes, so that at a Movement of the patient during treatment after-tax generation of the laser beam can take place.
