Abstract
Description
The invention relates to an instrument for endoscopic Removal of gallstones and the like after the upper Concept of claim 1.
It is a medical dilator Endoscopes for inspection purposes or operations with a carrying probe are known, on the End of a ball is arranged on the meh in succession dilation tubes with increasing diameter are slidable. This is also for percutaneous removal of gallstones required to enter the puncture channel continue and insert the shaft of an endoscope over it ren (DE-PS 30 25 785). With this procedure, the Gal but avoid the bladder, which complicates the procedure.
It is also an instrument for puncturing hollow organs known with a hollow probe that connects to a suction line is closable, the hollow probe surrounding it Annulus has, which also to a suction line is closable, and wherein the hollow probe is longitudinally displaceable is. This is said to cause bile to leak into the body be prevented when puncturing the gallbladder because the annulus surrounds the puncture hole in the gallbladder and exiting bile is sucked into the annulus. (DE-OS 37 13 831). This results in the difficult one Disadvantage that the dotting without visual observation must follow and other operations, such as removal of gallstones, are not possible.
The invention has for its object the instrument to improve the type mentioned above so that under the gallbladder when removing gallstones can't dodge.
To solve this problem, the characteristic notes are male of claim 1 provided.
This allows through the cavity between the outer jacket and the inner wall the gallbladder wall at this gap sucked in and fixed in this way. The fixie The last stage of the dilator is then carried out Screw the spiral into the end near the patient Wall of the gallbladder in the sense of a corkscrew.
By the last one now anchored in the gallbladder Operations can now pass through part of the dilator optics with their relatively wide instrument channel introduced and the necessary manipulations for removal the gallstones are carried out, among other things, why Example an ultrasonic probe, a gripper and the can find the same use.
For this purpose, the features of claims 2 and 3 are appropriate.
Fig. 1 zeigt links den Außenmantel 1 des Endoskopschaftes. Fig. 1 shows the left outer casing 1 of the endoscope shaft. By dash-dotted lines is indicated here that the inner jacket 9 according to FIG. 3 is arranged according to the Invention. The annular space created between this outer jacket 1 and the inner sleeve 9 can be connected to a vacuum pump through the connection 2 . The outer jacket 1 and the inner sleeve 9 are locked by the bayonet locking ring 14 . Such interlocks are generally known and therefore do not need to be explained in detail.
Further to the right you can see a connection 15 , which is in communication with the further annular space between the guide tube 6 and the inner sleeve 9 , for suction, for example, a rinsing liquid or the bile during observation.
Further to the right you can see the bayonet locking ring 16 , through which the guide tube 6 with the instrument channel 7 according to FIG. 4 is locked with the inner sleeve 9 .
Finally, further to the right, a third bayonet locking ring 17 can be seen , through which the optical periscope 13 , which is not visible here, is screwed to the guide tube 6 . The periscope 13 has the eyepiece with the eye cup 18 in a known manner at the right end remote from the patient. The optics is still seen with a light guide connector 19 . This is well known to the person skilled in the art and therefore need not be explained in more detail.
Fig. 2 shows the outer jacket 1 of the endoscope shaft on its own, which is provided with the connection 2 for the vacuum and the bayonet locking ring 14 .
The inner sleeve 9 according to FIG. 3 is inserted into the outer sleeve 1 from the right and is then held at the end near the patient by the spacers 10, 11 and 12 at a distance from the outer jacket 1 , being offset from one another by approximately 120 °. As a result, the spacer 12 cannot be seen here. Connection 15 at the far end was also mentioned. It can also be a connection to a vacuum or a suction pump, as will be explained later.
The guide tube 6 can be inserted into the inner sleeve 9 and locked by means of the bayonet locking ring 16 according to FIG. 4. As can be seen the guide tube, is provided with an instrument channel 7 6, wherein the guide tube 6 to the right until the bayonet lock ring 17 extends. The instrument channel 7 , on the other hand, ends at the bayonet locking ring 16 , in the vicinity of which an opening 20 is provided for the instruments to be inserted. This is indicated by broken lines. The instrument channel 7 thus ends here, while the guide tube 6 extends to the bayonet locking ring 17 .
Fig. 5 shows the optical periscope 13 with the light guide connector 19 and the eyecup 18 in the patient's far right end. These periscopes are part of the standard equipment of every endoscope and therefore do not need to be described in more detail.
Fig. 6 shows the functional cannula 8 , which is particularly thin in its left pa near patient 22 , the right patient remote area is held a little stronger. This serves to improve the stability of the relatively long cannula. For the sake of clarity, this is shown in abbreviated form.
On the right end remote from the patient you can see the handle 21 , the outer end 23 of which can be pulled out to the right and is connected to a thin wire, not shown, which extends through the entire cannula to the beveled end 24 near the patient.
Fig. 7 shows the dilator 4, projecting from its Dilatationsröhren 27-33 at the left end nearest the patient, the probe 26 with its spherical or conical end 25. This end 25 is somewhat stronger than the first dilatation tube 27 , so that all tubes 27-33 on the probe 26 can be pulled out to the right to the far end from the last stage 3 of the dilator. This is known to be achieved in that the distal ends of the dilatation tubes are provided with a closure plate which has a hole for the passage of the probe 26 . Only the last dilation tube 3 is therefore not provided with such a closure plate. At the end near the patient, this last stage 3 has a corkscrew-like spiral 5 , the purpose of which is described in more detail below. At the far end, the spiral 5 is closed by a collar 34 .
The interaction of all parts is explained below with reference to FIGS. 1-7.
First, only the abdominal wall of the patient is opened and expanded in a known manner in order to introduce the complete endoscope according to FIG. 1. Subsequently, the endoscope shaft 1 is brought up to the gallbladder wall and the connection 2 creates a vacuum in the gap between the outer jacket 1 and the inner sleeve 9 . In this way, the gallbladder wall is sucked into the annulus and fixed. Then the puncture cannula 8 is inserted through the opening 20 and puncturing the gallbladder is carried out under visual observation.
The puncture cannula 8 is then removed and the dilator 4 according to FIG. 4 is inserted to dilate the gallbladder. Finally, the last stage 5 is fixed in the gallbladder in such a way that the spiral 5 is screwed into the gallbladder wall. Then all other stages 25-33 are removed, leaving the last stage 5 by simply pulling them out together using probe 1 , leaving stage 5 . Now, through this last stage 5, the surgical optics with their relatively wide instrument channel 7 can be reintroduced and the necessary manipulations for removing the gallstones can be carried out, for example, using an ultrasound probe or a gripper.
The decisive advantage is that the gallbladder cannot escape by applying the vacuum via port 2 . In this case, flushing or removal of the liquid bile can also take place through the connection 15 .
Another great advantage is that the last stage 3 of the dilator cannot accidentally slip out of the gall bladder or the like.
The invention is not based on the illustrated embodiment limited form. Rather, the specialist has the opportunity create deviations from this within the scope of the claims and especially to perform other operations with it.
FIG. 8 shows a further embodiment which can be carried out not only with an instrument of the aforementioned type, but also, for example, with a somewhat differently constructed trocar sleeve known to the person skilled in the art. In the following, the person skilled in the art can also imagine a trocar sleeve of a known type instead of the inner sleeve 9 .
For example, the gallbladder gives way when piercing with Trocar out. There are also known trocar sleeves that end up have a spiral that slips out of the trocar sleeve should prevent. But this is not reliable enough and often leads to the gallbladder unilaterally slips.
This is prevented by the further embodiment according to FIG. 8, in that an outer sleeve 38 is arranged on the trocar sleeve 9 or the inner sleeve and can be rotated relative to the inner sleeve 9 by means of the handle 40 . At the end of this rotatable outer sleeve 38 near the patient, a plurality of sharp-ended spirals 35, 36, 37 are arranged at an angle to one another. It can be, for example, three such spirals, which are offset from one another at an angle of 120 °. It can also be a plurality of spirals or just two spirals that are soldered to the outer sleeve 38 or attached in some other way. The individual spirals are ground in a needle shape.
For the connection of the thread 39 with the inner sleeve 9, a pin 41 may be disposed on the inner sleeve 9, which engages in the inner thread of the outer sleeve 38th Since the inner sleeve does not need to have a thread. It is preferred that the pitch of the thread 39 is the same as the pitch of the spirals 35-37 at the end near the patient.
When turning by means of the handle 40 , the outer sleeve 38 thereby moves in the longitudinal direction, and the needle-shaped ends of the spirals 35, 36 and 37 drill into the wall of the gall bladder. The connection to the gallbladder is only released again when the handle 40 is rotated counterclockwise. The invention is also not limited to this illustrated embodiment with regard to the features mentioned. Rather, the expert has the possibility in this case, for example, to form the inner sleeve 9 as a trocar sleeve, as already mentioned, or to carry out something which is within the scope of the claims and the inventive concept of the same.
