Abstract

A tooth implant comprises (A) a part or region which comes into contact with cortical bones (Substantia corticalis sive compacta) which has surface irregularities and (B) a part which comes into contact with the Substantia spongiosa adjoining part (A) having a surface of a material which exhibits biological activity, whereby part (A) contacts the cortical bones of the jawbone when the implant is implanted in the jawbone and part (B) is situated such that it is in contact with the Substantia spongiosa of the jawbone. USE/ADVANTAGE - The implants for artificial tooth roots penetrate both the hard outer part and the soft inner part of the jawbone. The configuration of part (A) ensures strong initial fixing of the implant in the cortical bone and the biologically activity of part (B) causes this part to become firmly anchored into the soft inner part, so that the implant becomes firmly and securely fixed into the bone and remains stable.

Description

Die Erfindung betrifft ein verbessertes Zahnimplantat. The invention relates to an improved dental implant.

In the past years, in the field of Dental implants developed suitable implants. Such one Dental implant, also known as an artificial tooth root, is used for direct implantation in the jawbone.

Mater. Res. Symp. Nr. 2 (Teil 1) S. 117-141 (1971) und den japanischen JP-OSen 50-21 015, 51-1 06 114, 53-1 45 394, 54-17, 54-1 35 496, 57-3739 und 58-1 18 746 bekannt sind. There were various implants made from biologically active ones Materials developed that chemically interact with the bone connect. Early examples of such materials are biologically active glasses and biologically active glass ceramics (crystalline glass), as described by J. Biomed. Mater. Res. Symp. No. 2 (part 1) pp. 117-141 (1971) and the Japanese JP-OSen 50-21 015, 51-1 06 114, 53-1 45 394, 54-17, 54-1 35 496, 57-3739 and 58-1 18 746 are known.

Also crystalline glass, for example A-W ceramics with a Apatite phase and wollastonite phase were developed by T. Kokubo developed. The application of these substances to implants is  in development, like from the Journal of Ceramic Society, 90, No. 3, pp. 151-153 (1982), although the biological activity is slightly worse than that of biological glass or biological glass ceramic that were mentioned above.

Außerdem wurde vorgeschlagen, künstlich hergestelltes Fluorapatit anstelle des Hydroxyapatits zu verwenden, wobei ein Implantat aus einem gesinterten Gemisch von Fluorapatit und biologisch aktivem Glas hergestellt wurde (JP-OS 62-52 163).Attempts have also been made from the beginning to use calcium phosphate for implants, especially hydroxyapatite, on the assumption that these substances are the same as the inorganic component of the bone material, although the biological activity of these substances is still less pronounced. Since hydroxy apatite is powdery in nature or an artificial product, it is necessary to solidify it by sintering in order to produce an implant. In order to improve the strength during solidification, a method was developed in which an inorganic oxide, for example CaO, MgO or Al 2 O 3, was used as a binder (see JP-OS 57-40 776, column 4, lines 37- 43). An implant has also been proposed which consists of sintered hydroxyapatite using glass as a binder (JP-OS 50-64 305). These binders, which are biologically inactive, reduce the biological activity of the entire implant. For this reason, an implant was developed in which a mixture of hydroxyapatite and biologically active glass was sintered as a binder (JP-OS 60-1 86 455). In addition, it has been proposed to use artificially produced fluorapatite instead of the hydroxyapatite, an implant being produced from a sintered mixture of fluorapatite and biologically active glass (JP-OS 62-52 163).

In the present context, the mixture of hydroxy apatite and fluoroapatite and biologically active glass (or a glass ceramic) are abbreviated as ABG.

Such sintered apatites have a mechanical one Strength that has a certain value, they are however not strong enough to withstand the bite. For this reason, an implant has been proposed, which was made by sintered apatite on the Surface or as an outer layer of a high metal Strength (for example titanium, titanium alloys or Ni-Cr alloys), monocrystalline or polycrystalline Alumina existing core element glued or was attached (JP-OS 64-80 366, 1-2 38 869 and 1-1 75 695).

An implant was also proposed in which a metallic core element by melt spraying with hydroxy apatite has been coated, which means better adhesion or ver  bind guaranteed (JP-OS 52-14 095 and 63-1 60 663).

The magnitude of biological activity biological active glass (or a glass ceramic) (I), from A-W- Ceramics (II), ABG (III) and hydroxyapatite or fluorine apatit (IV) behaves as follows:

I < II ≃ III < IV.I <II ≃ III <IV.

The biologically active implants were made in relative made simple shapes, they had, for example wise cylindrical shape, the shape of a bullet (tapering conically) or plate or leaf shape, because the relatively low mechanical strength of the biologically active materials, especially Ma materials of the glass family made this necessary.

The jawbone is made of a hard outer material (Substantia corticalis sive compakta), a soft Inner material, the substantia spongiosa, and also internal cortical bone material (sub stantia corticalis sive compakta). As a result a biologically active implant, more precisely "a Implant, in which at least the surface consists of one biologically active material ", a touch  area (possibly two areas of contact) that in contact with the substantia corticalis sive compakta arrives, and a contact area that with the Substan tia spongiosa comes into contact, i.e. the scaffolding fine trabeculae when the implant is in the jaw bone is inserted.

To achieve a chemical bond with these bones the biologically active implant must generally mean between eight and sixteen weeks after the Implan tion remain immobile as there is a possible movement inflammation of the implant during this period the bone evokes or to form a fibrous Tissue leads between the bone and the implant, whereby the production of such fibrous tissue the chemical Binding makes impossible.

The object of the invention is to provide a dental implant, which is characterized by improved fixability in the beginning stage of implantation.

This problem is solved by the in claim specified invention.

In the event that part of an artificial tooth root is provided with surface irregularities without Make use of a biologically active material decreases the proportion of such a biologically active material on the surface size of the artificial root. Hence the chemical binding force decreases to the same extent between the cortical bone and the artificial tooth root off. The loss of the binding power of the artificial However, tooth root is limited because of the cortical bone is generally less in quantity than the sub stantia spongiosa. The disadvantages arising from the existing be such surface irregularities are limited because of the natural cortical bone low binding power with the biologically active material has a smaller number of bone cells and because the mechanical binding force, achieved by such surface irregularities will compensate for the decrease in chemical binding power siert.

The present invention aims to avoid the Loosen the artificial tooth root in the absence of a safe one Bond between the biologically active material and the Bond resulting from a lack of initial Fixation results. It has the presence of surfaces  irregularities in that with the cortical bone touching area to create a mechanical binding force and to achieve a secure and firmer initial fixation a meaning tendency influence the success of the artificial tooth root.

On the other hand, the substantia spongiosa, i.e. the Area with the fine trabeculae, the quantitative prevails, a larger reaction area with the bio logically active material, namely with that bio logically active area in the area with the substantia cancellous touching area of the implant is provided. Therefore, after completing the chemical bond the implant securely and firmly in the Jawbone is fixed and stays for a long time stable.

In representative embodiments, the dental implant of the invention may have a tapered shape as shown in FIGS . 5A and 5B (hereinafter referred to as type A), the implant may have a cylindrical shape as shown in FIGS. 6A and 6B (hereinafter) referred to as Type B), or it may be sheet-like or box-shaped as shown in Figs. 7A and 7B (hereinafter referred to as Type C). This represented in FIGS. 5B, 6B or 7B Darge example has two cortical bone contact areas. However, the dental implant according to the invention is not limited to the shapes shown in the drawings.

Ceramics in Tables 1 to 3 mean biological inactive ceramics.

The examples for the metal according to Tables 1 to 3 include in addition to those already in connection with substances mentioned in the prior art, pure titanium, titanium alloys as well as Ni-Cr alloys, while examples of biologically inactive ceramics monocrystalline or sinus tertiary alumina, monocrystalline or sintered zircon earth as well as monocrystalline or sintered titanium dioxide include.

In addition, the examples contain the biologically active Material in addition to that already in connection with the State of the art materials mentioned, and partly with overlapping these, hydroxyapatite, fluoroapatite, β-TCP (Tricalcium phosphate) and other calcium salts from phosphorus acid, ABG, A-W ceramics and mixtures of various apatites or calcium phosphate and a binder such as aluminum oxide.

The biologically active material may only be in be present on the surface area of the implant, wherein whose interior can consist of a core element to the increase mechanical strength. Part of the core element can form the cortical-bone contact area, this Design is preferred in practice.

Coating the core element with the biological active material can be done by spraying, whereby in this case, hydroxyapatite as known per se biologically active material can be used.

However, hydroxyapatite can be substituted, for example are replaced by (1) fluoroapatite, (2) mixtures with ver various binders mentioned above, such as ABG, or (3) Mixtures of fluoroapatite with different, above mentioned binders (for example biologically active Glass or glass ceramic).

Those provided at the cortical bone contact area Irregularities are widely identified in this text Sense used, examples include fine irregularities, a variety of irregularities, protrusions, exceptions grooves, longitudinal, transverse, diagonal or cross grooves, screws grooves, spiral grooves and through holes (penetrating or non-penetrating holes). The irregularities ver improve the friction (mechanical resistance) between the cortical-bone contact area and the cortical bones in this way an initial fixation of the Ensure the implant.

On the other hand, the one with a biologically active surface provided contact area for the substantia spongiosa generally designed as a smooth surface, and because of the low mechanical strength of the biologically active material, especially the materials from the glass family. However, Apatite or ABG have a rough surface caused by sintering or spraying during the manufacture of the implant is caused.

As stated above, the invention provides a Initial fixation of the implant achieved in that a cortical-bone contact surface is provided which comes into contact with the cortical bone, which is harder is called the substantia spongiosa, this contact area surface irregularities (as mechanical Binder), while still a stable and rigid fixation of the implant in the jawbone is obtained that one with the substantia spongiosa, the is predominant in quantity, more in touch Area provided with a biologically active surface is, so that a large contact area, that is large area realized with a chemical compound becomes.

Also the presence of mechanical binders that illuminating conceptually has the advantage that it both the patient and the treating physician convey the feeling that the implant is safe and tight.

If the surface irregularities in the surface in contact with the cortical bone have relatively large recesses, so generates these recesses closely observed bone area for a long time Visible tissue, which fills the recesses, thereby the resistance of the implant to twisting is increased and the implant sits even more firmly.

If the oral hygiene is not good, the gums can cause inflammation if the neck is absent. However, the neck portion is not an essential element of the present invention.

Below the neck part 0 is a cortical-bone contact part 1 , which is composed of a part of the metallic core element 4 , and which is provided in the surface with screw grooves as a special type of surface irregularities. The cortical bone contact part 1 has a height t 1 of 3 mm and comes into contact with the hard cortical bone when the implant is implanted. If the implant is screwed into a hole in the jawbone immediately before the implantation, the screw grooves offer a strong mechanical bond between the implant and the cortical bone, which is an initial fixation.

Below the cortical-bone contact part 1 there is a contact area 2 which comes into contact with the substantia spongiosa and extends to the end of the implant. The contact area 2 is composed of biologically active glass with a smooth surface. The layer made of biologically active glass has a thickness of approximately 300 μm, below which a glass layer with weaker activity is provided, which has a thickness of approximately 100 μm.

The term "top" here refers to the case that a tooth of the lower row of teeth (implantation in the Jawbone) is processed.

On the top of the implant there is a hexagonal opening (shown in simplified form as a circle in FIGS. 1A to 1C), from which a post hole 5 with a hexagonal cross section extends downward. This hole 5 can be used to use a hexagonal tool for rotating the implant, or it can be used to record a hexagonal cross-sectional post core to attach the upper structure to the implant gene.

The implant according to this embodiment is inserted adds to a third tooth up to the third tooth which the mental foramen as a passage for the nerve in the Jawbone is missing.

The implant has the structure shown in Fig. 2A and comprises, starting from the top, a cervical part 0 of length t 1 = 2 mm, a cortical bone contact part 1, a with the cancellous bone in contact entering contact part 2, and a cortical-bone contact part 3 .

The cervical part 0 has a mirrored surface that comes into contact with the gums, but can also be left out.

Die Oberfläche des biologisch aktiven Glases 6 bildet den mit der Substantia spongiosa in Berührung gelangenden Kontaktteil 2.Otherwise, the implant is composed of a metallic core element 4 and biologically active glass 6 (or another biologically active material), which covers the central part of the core element. The surface of the biologically active glass 6 forms the contact part 2 which comes into contact with the substantia spongiosa.

The uppermost part of the metallic core member 4 forms the cervical part 0 with a ver mirrored surface, and a part below it forms the cortical-bone contact part 1 with screw grooves.

A part further below serves as the core and is provided with the biologically active glass layer 6 on the surface. A lowermost part of the core element 4 forms the cortical-bone contact part 3 with screw grooves. The lower end forms a sharp tip with an angle of approximately 90 °, the screw grooves being provided slightly above the tip. With the help of the screw grooves, the implant grips itself.

The cortical bone contact part 3 is also provided with a passage opening (a through hole) 3 a and an internal hollow hole 3 b, which are connected to each other in connection. If the implant is self-cross screwed into the jaw bone, the resulting bone meal accumulated over the through hole 3 a b in the hollow hole. 3

Fig. 2B shows the manner how the implant is inserted into the jawbone. The cortical-bone contact parts 1 and 3 are on the screw grooves in a firm grip with the cortical bone 11 , which testifies to a firm initial fixation of the implant.

The implant according to this embodiment is essentially the same as the implant according to embodiment 1 in terms of shape and structure, with the exception that the screw grooves on the cortical-bone contact part 1 are replaced by fine surface irregularities.

These fine surface irregularities are caused by Spraying of titanium powder is achieved, which is a particle size of a few tens of micrometers.

Fig. 3 shows an embodiment of the implant, which is similar in shape and structure to embodiment 1, except that the screw grooves in the cortical-bone contact part 1 are replaced by a groove 13 , the depth of which is a few to a few hundred micrometers and the like Width is a few microns to a few millimeters. If the cortical bone is newly formed after the implantation of the implant, bone material enters the groove and thereby increases the holding force of the implant (resistance to rotation) and ensures a longer fixation. It also prevents the epithelium from growing down. In addition, there may be several grooves.

In the embodiment according to FIG. 4, the groove 13 provided in the cortical-bone contact part 1 of the embodiment 4 is replaced by a longitudinal, diagonal or grid pattern which is produced by processing by rollers or drums. Surface irregularities of a few to a few hundred micrometers, which are formed by this machining on the contact part 1 , improve the initial fixing force against twisting or loosening of the implant. In addition, the generation of cortical bone material that is produced in the direction of the irregularities ensures a longer fixation of the implant, and the irregularities prevent the epithelium from growing down.
