Description
Die Erfindung betrifft ein flexibles Implantat, insbesondere zur
Behandlung der Reflux-Oesophagitis oder/und der axialen Hiatus
hernie
In der EP 0 061 540 B1 ist eine auch unter dem Namen Angelchik-
Antireflux-Prothese bekannte Prothese beschrieben, die zur Be
handlung der Reflux-Oesophagitis eingesetzt wird. Dabei handelt
es sich im wesentlichen um ein C-förmiges Kissenelement, vor
zugsweise aus Silikon, an dessen beiden Enden jeweils ein flexi
bles Band angebracht ist. Diese Prothese wird in einem chirurgi
schen Eingriff um den distalen Bereich der Speiseröhre (Oesopha
gus) gelegt, und zwar oberhalb des Magenmundes (Kardia) und
unterhalb des Zwerchfells (Diaphragma). Wenn das C-förmige Teil
den Oesophagus umschließt, werden die beiden flexiblen Bänder
miteinander verknotet. Auf diese Weise entsteht ein Ring, der
bei einer axialen Hiatushernie ein Hochsteigen von Magenteilen
in den Thoraxbereich verhindert und gleichzeitig einen Reflux
des Mageninhalts in den distalen Bereich des Oesophagus redu
ziert.
Die vorbekannte Antireflux-Prothese kann nicht über eine Trokar
hülse in die Bauchhöhle eingeführt werden; dazu ist eine Laparo
tomie erforderlich. Lediglich die Positionierung sowie das Ver
knoten der beiden flexiblen Bänder sind endoskopisch möglich,
was jedoch vom Operateur viel Geschick erfordert.
Aus DE 36 19 197 A1 ist ein Implantat zur Behandlung der axialen
Hiatushernie bekannt, das im wesentlichen kissen- oder schlauch
förmig ist und als schlauchförmiges Polster mit einem Durchmes
ser von 12 mm kragenartig um den distalen Oesophagus gelegt
wird. Die sich überlappenden Enden werden vernäht.
In der US 5 006 106 ist eine Antireflux-Prothese nach Angel
chik beschrieben, die sich über eine Trokarhülse in die Bauch
höhle einführen läßt. Es handelt sich dabei um ein C-förmiges
Implantat, in das eine Feder eingearbeitet ist. Zum Durchführen
durch die Trokarhülse läßt sich das Implantat strecken, wobei
die Feder gespannt wird. Nach der Plazierung um den distalen
Oesophagus sorgt die Feder dafür, daß das Implantat wieder seine
C-förmige Gestalt annimmt und Halt hat. Andererseits erschwert
die von der Feder ausgeübte Rückstellkraft die Handhabung des
Implantats im Körperinnern, solange es noch nicht in seine end
gültige Lage gebracht ist.
Es ist Aufgabe der Erfindung, ein Implantat, insbesondere zur
Behandlung der Reflux-Oesophagitis und/oder der axialen Hiatus
hernie, zu schaffen, das auf vereinfachte Weise bei einer endo
skopischen Operation über eine Trokarhülse in die Leibeshöhle
eingesetzt werden kann.
Diese Aufgabe wird gelöst durch ein flexibles Implantat mit
einer zylinderähnlichen Grundform, wobei das Implantat in der
Nähe seines ersten Endes eine ösenartige Öffnung aufweist, deren
Achse quer zur Zylinderachse verläuft. Vorteilhafte Ausgestal
tungen ergeben sich aus den Unteransprüchen.
Die Grundform des erfindungsgemäßen Implantats ist nicht C-för
mig oder ringförmig, sondern zylinderähnlich. Damit das Implan
tat unterhalb des Diaphragmas um den Oesophagus gelegt werden
kann, ist es flexibel ausgestaltet. In der Nähe eines ihrer
Enden weist die zylinderähnliche Grundform eine ösenartige Öff
nung auf, deren Achse quer zur Zylinderachse verläuft. Nachdem
das Implantat zu einer Ringform gelegt ist, kann das andere Ende
durch diese ösenartige Öffnung geschoben werden, wodurch der
Ring dauerhaft geschlossen wird. Die Größe der ösenartigen Öff
nung ist so gewählt, daß Reibungskräfte ein Öffnen des einmal
zusammengefügten Rings wirkungsvoll verhindern.
Vorzugsweise ist das Implantat im Bereich der ösenartigen Öff
nung flach ausgestaltet. Im Bereich seines zweiten Endes kann
das Implantat mit einer oder mehreren, längs der Zylinderachse
gegeneinander versetzten Einschnürungen versehen sein. Kommt
nach dem Zusammenlegen des Implantats in Ringform eine solche
Einschnürung in den Bereich der ösenartigen Öffnung zu liegen,
so ist die endgültige Form des Implantats besonders sicher fi
xiert. Wenn mehrere Einschnürungen vorgesehen sind, kann der
Durchmesser der Ringform variiert werden.
Da das erfindungsgemäße Implantat in seiner Grundform zylinder
ähnlich ist, also langgestreckt, kann es ohne Schwierigkeiten
durch eine herkömmliche Trokarhülse, wie sie bei endoskopischen
Operationen verwendet wird, ins Körperinnere eingeführt werden.
Wenn das Implantat als Antireflux-Prothese verwendet werden
soll, kann es ohne besondere Schwierigkeiten mit endoskopischen
Greifinstrumenten, die durch weitere Trokarhülsen in den Abdomi
nalbereich eingeführt sind, gegriffen und um den Oesophagus zu
einer Ringform gelegt werden. Anschließend wird mit Hilfe dieser
Greifinstrumente das zweite Ende des Implantats durch die ösen
artige Öffnung gezogen und der Ring so weit geschlossen, bis die
gewünschte Größe erreicht ist. Ein kompliziertes Verknoten der
beiden Enden des Implantats ist also nicht erforderlich. Das
Einsetzen erfolgt schnell und sicher, und alle Vorteile, die die
endoskopische Operationstechnik bietet, kommen zum Tragen.
In einer vorteilhaften Ausgestaltung ist das erfindungsgemäße
flexible Implantat teilweise oder vollständig aus einem resor
bierbaren Material hergestellt. Ein solches Implantat kann ge
zielt den Gewebeaufbau fördern, indem Zellen in das Material
hineinwachsen können und so einen neuen Gewebeverbund bilden,
bevor das Material resorbiert ist. Auf diese Weise übernimmt das
neue Gewebe die ursprüngliche Funktion des Implantats, nämlich
eine stützende mechanische Wirkung. Andererseits entfallen die
Nachteile, die körperfremde Substanzen, z. B. Silikon, auf Dauer
mit sich bringen können.
Im folgenden wird die Erfindung anhand von Ausführungsbeispielen
näher erläutert.
Die Zeichnungen zeigt
Fig. 1 eine erste Ausführungsform des erfindungsgemäßen Im
plantats in seiner zylinderähnlichen Grundform;
Fig. 2 eine zweite Ausführungsform des erfindungsgemäßen Im
plantats in seiner zylinderähnlichen Grundform;
Fig. 3 eine Draufsicht auf die zweite Ausführungsform des
erfindungsgemäßen Implantats, nachdem es zu einer Ring
form geschlossen ist; und
Fig. 4 eine Schemazeichnung des Einsatzes des erfindungsgemä
ßen Implantates bei einer Hiatushernie, wobei zur Ver
anschaulichung ein Teil der Ringform entfernt ist.
In Fig. 1 ist ein Implantat 1 dargestellt, bevor es zu einer
Ringform gelegt ist. Die Grundform des Implantats ist zylinder
ähnlich. In der Nähe seines ersten Endes 2 weist das Implantat
1 eine durchgehende Öffnung 3 auf, deren Achse 4 quer zu der
(nicht eingezeichneten) Zylinderachse verläuft. Dabei kann sich
die Achse 4 unter einem rechten Winkel oder auch unter einem
anderen Winkel, also schräg, zur Zylinderachse erstrecken. Vor
zugsweise ist das Implantat 1 im Bereich der Öffnung 3 flach
ausgestaltet, wie an der in Fig. 3 dargestellten Ansicht zu
erkennen ist. Das Implantat 1 kann sowohl an seinem ersten Ende
2 als auch an seinem zweiten Ende 6 abgerundet sein. Die Größe
der Öffnung 3 ist etwas kleiner als die Querschnittsfläche des
Implantats 1. Da das Implantat 1 aus einem flexiblen Material
besteht, gibt die Öffnung 3 beim Einführen des zweiten Endes 6
etwas nach, so daß das zweite Ende 6 hindurchgesteckt werden
kann und dann durch Reibungskräfte sicher gehalten wird.
Bei der in Fig. 2 gezeigten Ausgestaltung ist das Implantat 1
im Bereich seines zweiten Endes 6 mit zwei Einschnürungen 7 und
8 versehen, die längs der Zylinderachse gegeneinander versetzt
sind. Die Querschnittsfläche an den Einschnürungen 7 und 8 ist
ungefähr so groß wie die der Öffnung 3. Wenn das Implantat 1 zu
einer Ringform geschlossen wird, siehe Fig. 3, ist immer dann
eine besonders stabile Fixierung erreicht, wenn die ösenförmige
Öffnung 3 im Bereich einer Einschnürung 7 oder 8 zu liegen
kommt. Um zu dem in Fig. 3 dargestellten Zustand zu gelangen,
wurde der Bereich der Einschnürung 8 ganz durch die Öffnung 3
hindurchgezogen, so daß die Öffnung 3 nun im Bereich der Ein
schnürung 7 liegt, wodurch sich eine engere Ringform ergibt.
Das erfindungsgemäße Implantat besteht aus einem beliebigen
flexiblen Material, das gewebeverträglich ist und für den be
absichtigten medizinischen Zweck angewandt werden kann. Besteht
das Implantat 1 oder ein Teil davon aus einem resorbierbaren
Material, kann dafür beispielsweise Polydioxanon, Polyglactin
oder eine Mischung davon verwendet werden.
Fig. 4 veranschaulicht, wie der Oesophagus O durch das Zwerch
fell Z führt und an der Kardia K in den Magen M einmündet. Zur
Behandlung der Reflux-Oesophagitis und/oder der axialen Hiatus
hernie ist ein erfindungsgemäßes Implantat 1 unterhalb des
Zwerchfells Z und oberhalb der Kardia K als Ring um den Oesopha
gus O gelegt.
Das erfindungsgemäße Implantat ist nicht auf die Anwendung bei
endoskopischen Operationen beschränkt, sondern läßt sich auch in
der offenen Chirurgie vorteilhaft einsetzen.
Claims (4)
1. Flexibles Implantat, mit einer zylinderähnlichen Grundform,
dadurch gekennzeichnet, daß das Implantat (1) in der Nähe
seines ersten Endes (2) eine ösenartige Öffnung (3) auf
weist, deren Achse (4) quer zur Zylinderachse verläuft.
2. Flexibles Implantat nach Anspruch 1, dadurch gekennzeichnet,
daß das Implantat (1) im Bereich der ösenartigen Öffnung (3)
flach ausgestaltet ist.
3. Flexibles Implantat nach Anspruch 2, dadurch gekennzeichnet,
daß das Implantat (1) im Bereich seines zweiten Endes (6)
mit einer oder mehreren, längs der Zylinderachse gegenein
ander versetzten Einschnürungen (7, 8) versehen ist.
4. Flexibles Implantat nach einem der Ansprüche 1 bis 3, da
durch gekennzeichnet, daß das Implantat (1) teilweise oder
vollständig aus einem resorbierbaren Material hergestellt
ist.
