Description
- Die Anmeldung ist eine teilweise Fortsetzung der Anmeldung Nr. 839911, eingereicht am 21. Februar 1992, mit dem Titel "Intraluminal-Aufspannvorrichtung".
- Die vorliegende Erfindung betrifft allgemeine eine Gefäßprothese und insbesondere eine Intraluminal-Aufspannvorrichtung, welche einen flexiblen und elastischen, rohrförmigen Aufbau aufweist, mit ausreichender Ringstärke, um einen elastischen Rückprall gegen einen Ballon widerstandsfähiger Striktionsvorrichtungen zu verhindern, oder um eine verzögerte Dehnung derartiger Striktionsvorrichtungen zu erzeugen.
- Im Stand der Technik gibt es einen weiten Bereich von Intraluminal-Aufspannvorrichtungen und Transplantaten. Beispielsweise ist im U.S.-Patent Nr. 4 733 665 von Palmaz ein durch einen Ballon expandierbares Intraluminal-Transplantat beschrieben, einschließlich einer Ausführungsform mit einem Drahtnetzrohr. Einander überschneidende Drahtteile, die an ihren Schnittpunkten durch Schweißen, Löten oder Kleben verbunden sind, bilden das Drahtnetz und bilden ein diamantförmiges Muster aus. Dieser Aufbau stellt eine relativ hohe Widerstandsfähigkeit bezüglich eines Zusammenfalls in Radialrichtung zur Verfügung; dabei treten jedoch einige Schwierigkeiten auf. Zunächst einmal handelt es sich um einen starren Aufbau, der nicht einfach die Form eines gekrümmten Gefäßes annehmen kann, welches ihn aufnimmt. Zweitens muss man einen Ballonkatheder einsetzen, um den Aufbau zu expandieren und zu implantieren. Diese Anforderung begrenzt die Länge des Implantats, ebenso wie die Steifigkeit.
- Andere Aufspannvorrichtungen nach dem Stand der Technik weisen flexiblere Konstruktionen auf; jedoch treten bei ihnen andere Schwierigkeiten auf. In dem U.S.-Patent Nr. 4 886 062 von Wiktor beispielsweise ist eine Aufspannvorrichtung beschreiben, welche einen relativ flexiblen Aufbau aufweist. Dieser Aufbau umfasst einen verformbaren Draht, der zickzackartig gebogen und spiralförmig aufgerollt ist. Das sich ergebende Drahtrohr weist eine offene Form auf, mit einer verringerten Ringstärke. Jeder Ring ist praktisch von den benachbarten Ringen isoliert und wird durch diese nicht wesentlich abgestützt. Darüber hinaus erhöht die offene Form das Risiko, dass Blutplättchen-Elemente durch die Spule brechen können. Weiterhin muss man einen Ballonkatheter verwenden, um die Aufspannvorrichtung zu expandieren und zu implantieren. Daher kann die Länge der Aufspannvorrichtung nicht die Ballonlänge verfügbarer Ballonkatheter überschreiten.
- Das Dokument
US 5,104,404 beschreibt einen Stent, der ringförmige Stentsegmente aufweist, die im Abstand voneinander angeordnet sind. Jedes dieser Stentsegmente besteht aus länglichen Elementen, deren Enden über eine bestimmte Länge der Elemente hinweg, längs der Elemente miteinander verbunden sind. Die Stirnseiten bzw. die Spitzen der länglichen Elemente sind nicht miteinander verbunden. Jedes Stentsegment ist mit dem nächstfolgenden Stentsegment an jeweils nur einer Stelle über ein draht- oder spulenförmiges Gelenk gekoppelt, wobei das Gelenk dafür sorgt, dass zwischen den einzelnen Stentsegmenten ein Abstand erhalten bleibt. - Aus der
DE 39 18 736 A1 ist ein Metallgitterstent bekannt, bei welchem innen und auch außen eine PTFE-Schicht schlauchartig vorgesehen sein kann. Das Metallgitterstent ist maschenartig ausgebildet, wobei die Metalldrahtlaschen eines Umfangringes etwa hakenartig in die Maschen des nächsten Umfangringes greifen. - Die
DE 41 02 550 A1 offenbart einen Stent mit einem spiralförmigen Träger, welcher von einem elastischen Schlauch eng anliegend umhüllt ist. Der spiralförmige Träger ist aus an sich gerade verlaufendem Material ausgebildet. Seine Wendeln sind deutlich voneinander beabstandet. - Aus der
DE 42 22 380 A1 ist eine schlauchartige Endoprothese bekannt, deren Wandung aus einem Gestricke aus metallischem Fadenmaterial besteht, bei welchem locker ineinandergreifende Schlingen offene Maschen bilden. Die Prothese kann mit einer Umkleidung in Form eines schlauchartigen Überzugs versehen sein. - Der vorliegenden Erfindung liegt die Aufgabe zugrunde, eine Intraluminal-Aufspannvorrichtung zu schaffen, welche einen elastischen Aufbau mit guter Widerstandsfähigkeit in Radialrichtung hat und trotzdem gut an die Form gekrümmter Gefäße anpassbar ist.
- Die Aufgabe wird erfindungsgemäß gelöst mit einer Intraluminal-Aufspannvorrichtung (Intraluminal-Aufspannvorrichtungs- und -Transplantat-Vorrichtung) mit den Merkmalen des Anspruchs 1.
- Die Intraluminal-Aufspannvorrichtung gemäß der vorliegenden Erfindung vermeidet die Schwierigkeiten der Aufspannvorrichtungen und Transplantate nach dem Stand der Technik. Sie weist eine ausreichende Ringstärke auf, um einen elastischen Rückprall von gegenüber Ballons beständigen Striktionsvorrichtungen zu verhindem. Die Aufspannvorrichtung gemäß der vorliegenden Erfindung weist einen flexiblen Aufbau auf, so dass sie der Krümmung des Gefäßes folgen kann, welches sie aufnimmt. Sie weist einen elastischen Aufbau auf, der eine Implantation ohne einen Ballonkatheter ermöglicht. Diese Elastizität gestattet weiterhin ein Zusammendrücken der Anordnung und einen Rückprall nach der Implantation, um eine verzögerte Dehnung des aufnehmenden Gefäßes hervorzurufen.
- Gemäß einer Ausführungsform der vorliegenden Erfindung kann eine Intraluminal-Aufspannvorrichtung eine vorbestimmte Länge eines Drahtes aufweisen, welcher sinusförmig oder zickzackartig angeordnet ist, und eine kontinuierliche Wendel mit mehreren, verbundenen Spiralen oder Ringen ausbildet. Mehrere Schleifenteile verbinden benachbarte Spitzen benachbarter Wendelringe. Die Aufspannvorrichtung ist kompressibel und kann sich von selbst wieder ausdehnen, im Wesentlichen auf die Form vor dem Zusammendrücken.
- Gemäß einer alternativen Ausführungsform der vorliegenden Erfindung kann eine Intraluminal-Aufspannvorrichtung die durchgehende Wendel und die mehreren Schleifenteile aufweisen, die voranstehend beschrieben wurden. Weiterhin kann sie ein prothetisches Implantat aufweisen, welches in Längsrichtung der Drahtwendel innerhalb von deren zentraler Öffnung (oder um die Drahtwendel herum) vorgesehen ist. Eines oder mehrere der Schleifenteile befestigt das Transplantat an der Drahtwendel. Dieses Implantat ist eine flexible, rohrförmige Schale, welche es zulässt, dass sich die Drahtwendel zusammenziehen und wieder zurückprallen kann.
- Die Erfindung wird nachstehend anhand zeichnerisch dargestellter Ausführungsbeispiele näher erläutert, aus welchen weitere Vorteile und Merkmale hervorgehen. Es zeigt:

10 eine Perspektivansicht des Dorns, der zur Herstellung der Drahtwendel gemäß der vorliegenden Erfindung verwendet wird. - Zwar beschreibt der Anmelder die Erfindung im Zusammenhang mit bevorzugten und alternativen Ausführungsformen, jedoch wird darauf hingewiesen, dass die Erfindung nicht auf diese Ausführungsformen beschränkt ist. Weiterhin wird darauf hingewiesen, dass die Figuren nicht notwendigerweise maßstabsgerecht sind. In bestimmten Fällen kann der Anmelder Einzelheiten weggelassen haben, die zum Verständnis der vorliegenden Erfindung nicht erforderlich sind.
- In den Zeichnungen zeigt
1 die Intraluminal-Aufspannvorrichtung gemäß der vorliegenden Erfindung, insgesamt durch die Bezugsziffer10 bezeichnet. Diese Aufspannvorrichtung weist einen Drahtkörper11 auf, der aus einer vorbestimmten Drahtlänge hergestellt ist, die sinusförmig oder zickzackartig angeordnet ist und eine durchgehende Wendel mit einer Reihe verbundener Spiralen oder Ringe aufweist. Weiterhin weist sie Schleifenteile12 auf, welche benachbarte Spitzen benachbarter Wendelringe verbinden, um die Ausbildung der rohrtörmigen Aufspannvorrichtung zu unterstützen. Die Schleifenteile12 können sämtliche oder einige der Paare benachbarter Spitzen verbinden. - Der Drahtkörper
11 ist eine elastische Legierung, welche eine Radialelastizität für die Aufspannvorrichtung zur Verfügung stellt. Vorzugsweise besteht er aus einer Nitinol-Legierung, welche eine bessere Elastizität und Ermüdungsbruchswiderstandsfähigkeit aufweist. Der Draht hat einen runden Querschnitt; jedoch kann der Querschnitt auch irgendeine andere Form aufweisen, beispielsweise dreieckig, rechteckig, usw. Alternativ hierzu kann jedes Material mit genügender Festigkeit und Elastizität und den anderen voranstehend geschilderten Eigenschaften den Drahtkörper ausbilden, einschließlich Edelstahl, Tantal, Titan, oder irgendein Kunststoff. - Die Schleifenteile
12 verbinden benachbarte Spitzen benachbarter Ringe des Drahtkörpers11 so, daß die benachbarten Spitzen aneinander anstoßen (vgl. die2 -4 ). Daher wird jeder Ring von benachbarten Ringen unterstützt, was die Ringstärke des Gesamtaufbaus der Aufspannvorrichtung vergrößert und das Risiko einer Plaque-Herniation minimalisiert. Die Schleifenteile12 sind Ligaturen aus Nahtmaterial, wobei die Enden zur Ausbildung einer Schleife zusammengebunden sind. Dieses Material ist ein Polypropylenmaterial oder jedes andere, biologisch verträgliche Material ausreichender Festigkeit. Obwohl Nähte, also Nahtmaterial, die bevorzugte Verbindungseinrichtung darstellen, können andere Verbindungseinrichtungen wie beispielsweise Krampen und Ringe aus Metall oder Kunststoff dieselbe Funktion zur Verfügung stellen. - Bei der Aufspannvorrichtungs-Anordnung gemäß der vorliegenden Erfindung ist vor der Implantierung in ein menschliches oder tierisches Blutgefäß eine Kompression möglich. Nach der Implantierung, beim Freigeben der Kompressionskraft, schlägt die Aufspannvorrichtungsanordnung zurück (oder dehnt sich von selbst aus), bis zu ihrer Ursprungsform. Daher stellt sie kontinuierlich eine Dehnungskraft im implantierten Zustand zur Verfügung. Die Anordnung sorgt für eine Flexibilität, wodurch die Aufspannvorrichtung der Krümmung des Gefäßes folgen kann, welches sie aufnimmt.
- In den
7 bis9 ist eine alternative Ausführungsform der vorliegenden Erfindung gezeigt, welche wie voranstehend beschrieben den Drahtkörper und die Nahtmaterialverbindungen aufweist. Weiterhin ist bei dieser Alternative ein prothetisches Transplantat13 vorgesehen, welches innerhalb der zentralen Öffnung des Drahtkörpers angeordnet ist. Das Transplantat13 ist ein rundes, offenes Rohr, welches aus Polytetrafluorethylen (PTFE), Dacron oder irgendeinem anderen geeigneten, biologisch verträglichen Material besteht. Ein oder mehrere Ringteile verbindet bzw. verbinden das Transplantat13 mit dem Drahtkörper11 , wie in9 gezeigt ist. Am Ort angeordnet schließt das Transplantat die diamantförmigen Öffnungen der Aufspannvorrichtungs-Anordnung, um eine Plaque-Herniation weiter zu minimalisieren, und den Fluß von Fluiden und Zellenelementen durch die Anordnung zu minimalisieren. - Alternativ hierzu kann das Transplantat
13 so angeordnet sein, daß es sich um die Außenseite der Drahtwendel herum erstreckt. Darüber hinaus kann sich das Transplantat13 in der gleichen Richtung erstrecken wie die Drahtwendel, oder kann kürzer als die Drahtwendel sein. Schließlich kann das Transplantat13 mehrere Segmente aufweisen, die innerhalb der Drahtwendel oder um die Außenseite der Drahtwendel herum angeordnet sind. - Das Transplantat
13 ist eine ebene, gewebte Gewebekonstruktion, die nahtlos und rohrförmig auf konventionellen Geräten hergestellt wird, entweder einer Schiffchen-Schmalgewebe-Webmaschine oder einer Nadel-Schmalgewebe-Webmaschine. Das Rohr besteht aus mehrfädigem Polyestergarn von 40 Denier oder weniger (vorzugsweise 20, 30 oder 40 Denier). Die Wandstärke des Rohrs beträgt 0,2 mm oder weniger (bevorzugt 0,1 mm); und weist eine Wasserdurchlässigkeit zwischen 50 und 500 ml/cm2/min bei 16 kPa auf (Milliliter Wasser pro Quadratzentimeter pro Minute bei einem Druck von 16 kPa). Das Gewebe kann mit einer Arzneimittelsubstanz beschichtet sein, um die Permeabilität zu verringern, lokal eine Antikoagulation hervorzurufen, oder eine zelluläre Infaltration zu verhindern. - Das Verfahren zur Herstellung der Aufspannvorrichtung gemäß der vorliegenden Erfindung umfaßt das Biegen einer vorbestimmten Drahtlänge in Zickzackform zwischen den Stiften
14 des Dorns15 und um den Dorn herum, wodurch eine Wendel gebildet wird (siehe10 ). Der nächste Schritt umfaßt das Entfernen der Wendel von dem Dorn durch Entfernen der Stifte, und das Herunterziehen der Wendel von dem Dorn. Dann werden bei diesem Verfahren benachbarte Spitzen benachbarter Wendelringe verbunden. Ein Hersteller stellt jede Verbindung dadurch her, daß er eine Ligatur aus Nahtmaterial (oder irgendeinem anderen geeigneten Material) um die Drahtsegmente herum anordnet, welche zwei benachbarte Spitzen ausbilden, und die Enden des Nahtmaterials zusammenbindet, um eine Schleife zu bilden. Bei Anwendungen, bei welchen der Drahtkörper ein Nitinol-Draht ist, umfaßt der Vorgang die Befestigung der Enden des Drahtes an dem Dorn, und das Anlassen des Drahts auf eine vorbestimmte Temperatur (und so die Einprägung eines thermischen Gedächtniseffektes für die angelassene Form), bevor die Wendel von dem Dorn abgenommen wird. - Bei dem Verfahren zum Implantieren der Aufspannvorrichtung und des Transplantats gemäß der vorliegenden Erfindung wird sie zusammengedrückt und in die zentrale Bohrung einer Einführungsvorrichtung
16 eingebracht. Der nächste Schritt betrifft das Ankuppeln der Vorrichtung16 an die Nabe17 einer Hülle18 , die sich zum Implantationsort erstreckt. Beim nächsten Schritt wird ein Katheter19 verwendet, um die zusammengedrückte Aufspannvorrichtung zum vorbestimmten Ort hin zu drücken, und die Aufspannvorrichtung an diesen Ort durch den Katheter zu halten, und dann wird die Hülle entfernt. Beim letzten Schritt wird der Katheter entfernt, damit die Aufspannvorrichtung einen Rückprallvorgang durchführen kann. - Bei Anwendungen, bei welchen der Drahtkörper ein Nitinol-Metall ist, verringert ein Benutzer den Durchmesser der Aufspannvorrichtung dadurch, daß er sie zuerst abkühlt, beispielsweise dadurch, daß er sie in Eiswasser eintaucht. Diese Abkühlung versetzt das Nitinol in eine martensitische Phase und erleichtert eine manuelle Verringerung des Durchmessers und die Einführung der Aufspannvorrichtung in die zentrale Bohrung der Vorrichtung
16 . Die Vorrichtung16 und die Hülle18 halten die Aufspannvorrichtung zurück, bis sie an einem vorbestimmten Ort eingesetzt wurde. An diesem Ort im Körper eines Menschen erwärmen Körperfluide das Nitinol und versetzen es in eine austenitische Phase, welche die stabile Phase dieses Metalls darstellt, und welche einer vollständig geöffneten oder expandierten Form der Aufspannvorrichtung entspricht (bis zu ihrem ursprünglichen, angelassenen Durchmesser). - Zwar erläutern die voranstehende Beschreibung und die Zeichnungen eine Ausführungsform und eine Alternative, jedoch wird darauf hingewiesen, daß selbstverständlich die Erfindung nicht auf diese Ausführungsformen beschränkt ist. Fachleute auf dem Gebiet der vorliegenden Erfindung können andere Modifikationen vornehmen oder andere Ausführungsformen einsetzen, welche die Grundlagen der vorliegenden Erfindung verwenden, insbesondere unter Berücksichtigung der voranstehend angegebenen Lehren. Beispielsweise kann man ein verformbares Material dazu verwenden, den Drahtkörper
11 der Aufspannvorrichtung auszubilden, und dann einen Ballonkatheter dazu verwenden, diese anzubringen. Der Anmelder möchte daher mit den beigefügten Patentansprüchen erreichen, daß sämtliche Abänderungen und andere Ausführungsformen abgedeckt sind, welche diese Merkmale verwenden, welche die wesentlichen Merkmale der vorliegenden Erfindung darstellen.
Claims (9)
- Intraluminal-Aufspannvorrichtung (Stent) mit einem rohrtörmigen Aufspannvorrichtungsteil und einem rohrförmigen nahtlos gewebten Transplantatteil, welches an dem Aufspannvorrichtungsteil entweder innen oder außen befestigt ist, wobei das Aufspannvorrichtungsteil zusammendrückbar ist und sich selbst im Wesentlichen zu einer Form vor dem Zusammendrücken ausdehnen kann, wobei das Aufspannvorrichtungsteil aus einer vorbestimmten Länge eines elastischen Drahtes in sinusförmiger oder zickzackförmiger Anordnung besteht, und eine durchgehende Wendel mit mehreren Ringen ausbildet und darüber hinaus mehrere Verbindungseinrichtungen aufweist, die benachbarte Spitzen benachbarter Wendelringe verbinden.
- Intraluminal-Aufspannvorrichtung nach Anspruch 1, wobei das rohrförmige Aufspannvorrichtungsteil aus Nitinol mit Form-Gedächtnis-Effekt besteht.

- Eine Intraluminal-Aufspannvorrichtung nach Anspruch 3, wobei das Nahtmaterial ein Polypropylen ist.
- Eine Intraluminal-Aufspannvorrichtung nach Anspruch 1, wobei alle Spitzen eines Ringes axial an entgegengesetzt ausgerichtete Spitzen eines benachbarten Ringes angrenzen und miteinander verbunden sind.
- Eine Intraluminal-Aufspannvorrichtung nach wenigstens einem der vorangegangenen Ansprüche, wobei die Struktur der Aufspannvorrichtung diamantförmige Öffnungen aufweist und das Transplantat diese Öffnungen abdeckt.
- Eine Intraluminal-Aufspannvorrichtung nach wenigstens einem der vorangegangenen Ansprüche, wobei das Transplantat eine Wasserdurchlässigkeit zwischen 50 und 500 ml/cm2 pro Minuten bei einem Druck von 16 Kilopascal aufweist.
- Eine Intraluminal-Aufspannvorrichtung nach wenigstens einem der vorangegangenen Ansprüche, wobei das Transplantat aus einem Gewebe besteht, welches mit einer Arzneimittelsubstanz beschichtet ist.
- Eine Intraluminal-Aufspannvorrichtung nach wenigstens einem der vorangegangenen Ansprüche, wobei die Wendelringe aus einem Draht mit vorbestimmter Länge gebildet worden sind.
