Description
Diese Erfindung ist auf eine einzigartige Vorrichtung und ein
einzigartiges Verfahren zum Durchdringen von Körpergewebe zu
medizinischen Zwecken gerichtet, wie zum Beispiel zur
Gewebeablation und zur Fluidsubstanzzufuhr. Die Vorrichtung
durchdringt Gewebe bis zu dem genauen Ziel, das ausgewählt
wurde, um dem Gewebe Energie zuzuführen und/oder Substanzen
zuzuführen. Sie beschränkt diese Behandlung auf den genauen
vorab ausgewählten Ort, wodurch ein Trauma von normalem
umgebendem Gewebe minimiert und ein größerer medizinischer
Nutzen erreicht wird. Die Vorrichtung ist eine katheterartige
Vorrichtung zum Positionieren einer Behandlungseinheit in dem
für eine medizinische Behandlung ausgewählten Bereich oder
Organ, mit einem oder mehr Mandrins ("stylets") im Katheter,
die zum Ausfahren aus einer Mandrinöffnung in der Seite des
Katheters durch umgebendes Gewebe in das für den
medizinischen Eingriff ins Auge gefaßte Gewebe angebracht
sind.
Eine Behandlung von Zellgewebe macht gewöhnlich eine
unmittelbare Berührung von Zielgewebe mit einem medizinischen
Instrument erforderlich, üblicherweise durch operative
Verfahren, die sowohl das Zielgewebe als auch
dazwischenliegendes Gewebe einem bedeutenden Trauma
aussetzen. Häufig ist eine genaue Plazierung einer
Behandlungssonde wegen der Lage der ins Auge gefaßten
Gewebebereiche im Körper oder der Nähe des Zielgewebes zu
leicht verletzbaren, kritischen Körperorganen, Nerven oder
anderen Körperteilen schwierig.
Zum Beispiel ist eine benigne Prostatahypertrophie oder
-hyperplasie (BPH) eines der häufigsten medizinischen
Probleme, denen über 50 Jahre alte Männer begegnen. Eine
Obstruktion des Harntraktes infolge einer Prostatahyperplasie
ist bereits seit den frühesten Tagen der Medizin erkannt
worden. Eine hyperplastische Vergrößerung der Prostatadrüse
führt häufig zu einem Druck auf die Harnröhre, der eine
Obstruktion des Harntraktes und die nachfolgende Entwicklung
von Symptomen zur Folge hat, einschließlich eines häufigen
Wasserlassens, einer Abschwächung des Harnstrahls, eines
nächtlichen Wasserlassens, von Schmerzen, Unwohlsein und
Harntröpfeln. Man hat gezeigt, daß die Verbindung von BPH mit
dem Altern bei über 50 Jahre alten Männern in der Häufigkeit
50% übersteigt und bei über 80 Jahre alten Männern auf über
75% ansteigt. Symptome einer Harnobstruktion treten am
häufigsten im Alter zwischen 65 und 70 auf, wenn ungefähr 65
% der Männer in dieser Altersgruppe eine Prostatavergrößerung
aufweisen.
Gegenwärtig gibt es kein nachgewiesenes wirkungsvolles
nichtoperatives Behandlungsverfahren für BPH. Zusätzlich sind
die verfügbaren operativen Verfahren nicht völlig
befriedigend. Gegenwärtig besitzen Patienten, die unter den
obstruktiven Symptomen dieser Krankheit leiden, wenig
Wahlmöglichkeiten: fortfahren mit den Symptomen fertig zu
werden (d. h. eine abwartende Behandlung), sich einer
Arzneimitteltherapie im Frühstadium zu unterziehen oder sich
einem operativen Eingriff zu unterziehen. In den Vereinigten
Staaten unterziehen sich pro Jahr mehr als 430 000 Patienten
einem Eingriff zur Entfernung von Prostatagewebe. Dies stellt
weniger als 5% der Männer dar, die signifikante klinische
Symptome zeigen.
Diejenigen, die unter BPH leiden, sind häufig ältere Männer,
viele mit zusätzlichen Gesundheitsproblemen, welche das
Risiko operativer Verfahren erhöhen. Operative Verfahren zur
Entfernung von Prostatagewebe sind mit einer Anzahl von
Risiken verbunden, einschließlich einer mit der Narkose im
Zusammenhang stehenden Morbidität, Blutungen, Koagulopathien,
Lungenembolie und ein gestörtes Elektrolytgleichgewicht.
Diese gegenwärtig durchgeführten Verfahren können auch zu
Herzkomplikationen, einer Blasenperforation, einer
Inkontinenz, einer Infektion, einer Harnröhren- oder
Blasenhalsverengung, einem Festsetzen von Prostatateilchen,
einem rückläufigen Samenerguß und zur Unfruchtbarkeit führen.
Infolge der umfassend invasiven Natur der gegenwärtigen
Wahlmöglichkeiten bei der Behandlung obstruktiver Harnleiden,
verzögert die Mehrzahl der Patienten eine endgültige
Behandlung ihres Leidens. Dieser Umstand kann als Folge der
obstruktiven Läsion in der Prostata zu einer ernsten
Schädigung von Strukturen führen (Blasenhypertrophie,
Hydronephrose, Erweiterung der Nierenbecken, chronische
Infektion, Erweiterung von Harnleitern usw.), was nicht ohne
bedeutende Konsequenzen ist. Zusätzlich sind eine
signifikante Anzahl von Patienten mit Symptomen, die
ausreichend schwer sind, um einen operativen Eingriff zu
rechtfertigen, daher schlechte Operationsrisiken und
schlechte Kandidaten für eine Prostatektomie. Zusätzlich sind
unter BPH leidende jüngere Männer, die keine Komplikationen,
wie beispielsweise Unfruchtbarkeit riskieren wollen, häufig
gezwungen, einen operativen Eingriff zu vermeiden. Somit ist
der Bedarf, die Wichtigkeit und der Wert von verbesserten
operativen und nichtoperativen Verfahren zur Behandlung von
BPH unstreitig.
Hochfrequenzströme werden bei Elektrokaustikverfahren zum
Schneiden von menschlichem Gewebe eingesetzt, insbesondere
wenn ein blutloser Einschnitt gewünscht wird, oder wenn die
Operationsstelle mit einem normalen Skalpel nicht zugänglich
ist, jedoch durch natürliche Körperöffnungen, wie
beispielsweise die Speiseröhre, den Darm oder die Harnröhre
einen Zugang für ein dünnes Instrument aufweist. Beispiele
schließen die Entfernung von Prostataadenomen, Blasentumoren
oder Darmpolypen ein. In solchen Fällen wird der
Hochfrequenzstrom mittels einer Operationssonde in das zu
schneidende Gewebe zugeführt. Die sich ergebende abgeführte
Wärme bewirkt ein Kochen und Verdampfen der Zellflüssigkeit
an dieser Stelle, woraufhin die Zellwände reißen und das
Gewebe getrennt wird.
Eine Zerstörung von Zellgewebe in situ ist bei der Behandlung
vieler Krankheiten und medizinischer Leiden allein oder als
Hilfsmittel bei operativen Entfernungsverfahren eingesetzt
worden. Sie ist häufig weniger traumatisch als operative
Verfahren, und kann die einzige Alternative sein, wo andere
Verfahren unsicher sind. Ablative Behandlungsvorrichtungen
weisen den Vorteil eines Einsatzes von elektromagnetischer
Energie auf, die durch Ableitung und Konvektionskräfte von
zirkulierenden Fluiden und andere natürliche Körpervorgänge
schnell abgeführt und auf ein nichtzerstörerisches Niveau
reduziert wird.
Mikrowellen-, Radiofrequenz-, Schall(Ultraschall)- und
Lichtenergie (Laser) -Vorrichtungen und gewebezerstörende
Substanzen sind verwendet worden, um maligne, benigne und
andere Arten von Zellen und Gewebe aus einer breiten Vielzahl
von anatomischen Orten und Organen zu zerstören. Behandeltes
Gewebe umfaßt isolierte Karzinomansammlungen und insbesondere
Organe, wie beispielsweise die Prostata, und für die benigne
Prostatahyperplasie charakteristische Drüsen- und
stromaknötchen. Diese Vorrichtungen umfassen gewöhnlich einen
Katheter oder eine Kanüle, die verwendet wird, um eine
Radiofrequenzelektrode oder Mikrowellenantenne durch einen
Ductus zur Behandlungszone zu führen und Energie durch die
Ductuswand diffus in sämtlichen Richtungen in das umgebende
Gewebe zuzuführen. Während dieses Zellzerstörungsvorgangs
erleidet die Ductuswand häufig ein schweres Trauma, und
einige Vorrichtungen kombinieren Kühlsysteme mit
Mikrowellenantennen, um ein Trauma der Ductuswand zu
verringern. Um die Prostata mit diesen Vorrichtungen zu
behandeln, wird zum Beispiel Wärmeenergie durch die Wände der
Harnröhre in die umgebenden Prostatazellen abgegeben, im
Bemühen, das Gewebe wegzunehmen, welches die Verengung der
Harnröhre verursacht. Lichtenergie, gewöhnlich aus einem
Laser, wird unter "Hindurchbrennen" durch die Wand der
Harnröhre in Prostatagewebe-Zielorte abgegeben. Gesunde
Zellen der Ductuswand und gesundes Gewebe zwischen den
Knötchen und der Ductuswand werden bei dem Verfahren
ebenfalls unterschiedslos zerstört und können einen unnötigen
Verlust eines Teils der Prostatafunktion verursachen.
Außerdem kompliziert die hinzugefügte Kühlfunktion einiger
Mikrowellenvorrichtungen die Apparatur und macht es
erforderlich, daß die Vorrichtung ausreichend groß ist, um
dieses Kühlsystem unterzubringen.
Eine Applikation von Flüssigkeiten in bestimmte Gewebe zu
medizinischen Zwecken wird durch die Fähigkeit, eine Abgabe
ohne eine Traumatisierung von dazwischenliegendem Gewebe zu
erzielen, und eine auf das bestimmte Zielgewebe begrenzte
Abgabe zu bewirken, eingeschränkt. Eine örtliche
Chemotherapie, eine Infusion von Arzneimitteln,
Kollageninjektionen, oder Injektionen von Mitteln, die dann
durch Licht, Wärme oder Chemikalien aktiviert werden, würde
durch eine Vorrichtung stark erleichtert, die eine Öffnung
eines Fluid(Flüssigkeit oder Gas)-Zufuhrkatheters in dem
bestimmten Zielgewebe in geeigneter Weise und genau plazieren
könnte.
Es ist ein Ziel dieser Erfindung, eine Vorrichtung und ein
Verfahren bereitzustellen, um Gewebe durch dazwischen
liegendes Gewebe in das für eine medizinische Einwirkung, wie
beispielsweise eine Gewebeablation und/oder Abgabe einer
Substanz ausgewählte genaue Zielgewebe zu durchdringen, wobei
diese Tätigkeit auf den genauen vorab ausgewählten Ort
begrenzt ist, wodurch das Trauma minimiert und ein größerer
medizinischer Nutzen erreicht wird.
Es ist ein anderes Ziel dieser Erfindung, eine Vorrichtung
und ein Verfahren zur Gewebeablation von Körpergewebe
bereitzustellen, wobei die Vorrichtung die therapeutische
Energie unmittelbar in vorgesehenes Gewebe abgibt, während
Auswirkungen auf das umgebende Gewebe minimiert werden.
Es ist noch ein weiteres Ziel dieser Erfindung, eine
Vorrichtung und ein Verfahren bereitzustellen, um fluidische
Behandlungsmittel, wie beispielsweise fließfähige
Flüssigkeiten und Gase einer bestimmten Stelle im Körper mit
größerer Genauigkeit und Leichtigkeit zuzuführen.
Ein anderes Ziel dieser Erfindung ist es, eine Wärmezer
störungsvorrichtung bereitzustellen, die dem Operateur mehr
Informationen über die Temperatur und andere Bedingungen
liefert, die sowohl in dem vorgesehenen Gewebe als auch im
umgebenden Gewebe erzeugt werden. Zusätzlich liefert sie eine
bessere Kontrolle über die Plazierung des Mandrins im Körper
und über die Parameter des Gewebeablationsvorgangs.
Zusammenfassend umfaßt die medizinische Sondenvorrichtung
dieser Erfindung einen Katheter mit einem Steuerende und
einem Sondenende. Das Sondenende umfaßt ein
Mandrinführungsgehäuse mit mindestens einer Mandrinöffnung
und Mandrinführungseinrichtungen, um einen flexiblen Mandrin
durch mindestens eine Mandrinöffnung nach außen und durch
dazwischenliegendes Gewebe in vorgesehene Gewebebereiche zu
lenken. Ein Mandrin ist in mindestens einer der besagten
Mandrinführungseinrichtungen positioniert, wobei der Mandrin
eine nichtleitende Hülle mit mindestens zwei und vorzugsweise
drei darin angeordneten Lumina umfaßt. Ein
RF-(Radiofrequenz)-Elektroden-Lumen endet an einer distalen
Öffnung im distalen Ende der nichtleitenden Hülle, und eine
Radiofrequenzelektrode ist für eine Längsbewegung durch die
distale Öffnung im RF-Elektroden-Lumen positioniert.
Bevorzugt sind mindestens ein Teil einer gegenüberliegenden
Oberfläche des Elektrodenlumens und der Elektrode im Abstand
voneinander angeordnet, um einen Flüssigkeitszufuhrdurchlaß
zur Abgabe von Arzneimittelflüssigkeit zu bilden. Ein zweites
wahlweise vorgesehenes Fluiddurchlaßlumen endet an einer
distalen Öffnung im distalen Ende der nichtleitenden Hülle
und umfaßt Einrichtungen zum Hindurchführen eines Fluids.
Ein drittes Temperatursensorlumen endet in einem
abgedichteten Verschluß angrenzend an das distale Ende der
nichtleitenden Hülle. Mindestens eine und vorzugsweise eine
Mehrzahl von Temperaturfühlern, wie beispielsweise
Thermoelemente, sind im dritten Lumen positioniert, wobei
sich die Zuleitungen durch das Lumen erstrecken. Eine
bevorzugte Ausführungsform weist zwei im dritten Lumen
positionierte Temperaturfühler auf, wobei ein
Temperaturfühler innerhalb von ungefähr 1 mm vom distalen
Ende der nichtleitenden Hülle positioniert ist, und wobei der
zweite Themperaturfühler mindestens 3 mm und vorzugsweise
zwischen 3 und 6 mm vom distalen Ende der nichtleitenden
Hülle entfernt positioniert ist.
Zusammenfassend umfaßt eine andere Ausführungsform dieser
Erfindung einen Katheter mit einem Steuerende und einem
Sondenende, wobei das Sondenende ein Mandrinführungsgehäuse
mit einer Mandrinöffnung und Mandrinführungseinrichtungen
umfaßt, um einen flexiblen Mandrin nach außen durch die
Mandrinöffnung und durch dazwischenliegendes Gewebe in
vorgesehene Gewebebereiche zu lenken. Ein Mandrin ist in
mindestens einer der besagten Mandrinführungseinrichtungen
positioniert, wobei der Mandrin einen innerhalb einer
nichtleitenden Hülle eingeschlossenen elektrischen Leiter
umfaßt. Die Elektrode weist ein distales Stück mit mindestens
einer stromfokussierenden Nuteinrichtung darauf auf, sowie
eine distale Spitzen die so geformt ist, daß sie auf ihrem
Stirnende Strom fokussiert, wodurch ein RF-Strom, der von
dort in umgebendes Gewebe eintritt, ein Läsion bildet, die
sich von der Nut und der Spitze nach außen erstreckt. Bei
einer bevorzugten Ausführungsform weist das distale Stück
eine Mehrzahl von ringförmigen Fokussiernuten oder eine
spiralförmige Fokussiernut darauf auf.
Vorzugsweise ist mindestens ein Teil der Elektrode innerhalb
eines Stützrohrs eingeschlossen, das eine ausreichende
Festigkeit aufweist, um eine Elektrodenlinearität
aufrechtzuerhalten, wenn die Elektrode durch die
Mandrinöffnung nach außen gelenkt wird.

RF-Ablationskatheter-Ausführungsform dieser Erfindung mit einem
Faseroptik-Betrachtungszubehör.

Die Vorrichtung dieser Erfindung sieht ein genau gesteuertes
Positionieren eines Behandlungsmandrins in einem für eine
Behandlung ins Auge gefaßten Gewebe, eine Zerstörung oder
eine Probenahme von einem in der Nähe des Zielgewebes
positionierten Katheter aus vor.
Der Begriff "Mandrin", wie er nachfolgend verwendet wird, ist
so definiert, daß er sowohl massive als auch hohle Sonden
einschließt, die angepaßt sind, um sie aus einer
Katheteröffnung durch normales Gewebe hindurch in das ins
Auge gefaßte Gewebe zu führen. Der Mandrin ist so geformt,
daß er ein müheloses Hindurchführen durch Gewebe erleichtert.
Er kann ein massiver Draht, ein dünner Stab oder ein anderes
massives Modell sein, oder er kann ein dünnes Hohlrohr oder
ein anderes Modell mit einem Längslumen zum Zuführen von
Fluiden an einen Ort oder zum Entfernen von Material von
demselben sein. Der Mandrin kann auch ein dünnen Hohlrohr
oder ein anderes hohles Modell sein, dessen hohles Lumen
einen verstärkenden oder funktionalen Stab oder ein Rohr
enthält, beispielsweise eine Laser-Faseroptik. Der Mandrin
weist bevorzugt ein geschärftes Ende auf, um Widerstand und
Trauma zu verringern, wenn er durch Gewebe an einen Zielort
gestoßen wird.
Der Mandrin kann so gestaltet sein, daß er eine Vielzahl
medizinisch erwünschter Behandlungsformen eines ausgewählten
Gewebes vorsieht. Als Radiofrequenzelektrode oder
Mikrowellenantenne kann er eingesetzt werden, um vorgesehene
Gewebebereiche wegzunehmen oder zu zerstören. Als Hohlrohr
kann er eingesetzt werden, um ein Behandlungsfluid,
beispielsweise eine Flüssigkeit, in vorgesehene
Gewebebereiche abzugeben. Die Flüssigkeit kann eine einfache
Lösung oder eine Suspension von Feststoffen sein, zum
Beispiel von kolloidalen Teilchen, in einer Flüssigkeit. Da
der Mandrin sehr dünn ist, kann er vom Katheter aus durch
dazwischenliegendes normales Gewebe gelenkt werden, mit einem
Minimum an Trauma für das normale Gewebe.
Die Vorrichtung und das Verfahren dieser Erfindung sehen eine
genauere gesteuerte medizinische Behandlung vor, die geeignet
ist, Zellen von medizinisch ins Auge gefaßten Gewebebereichen
im gesamten Körper, sowohl innerhalb als auch außerhalb von
Körperorganen zu zerstören. Die Vorrichtung und das Verfahren
sind besonders zur Behandlung von benigner
Prostatahyperplasie (BPH) nützlich, und die Vorrichtung und
ihr Gebrauch werden zum Zweck einer Vereinfachung ihrer
Beschreibung nachfolgend unter Bezugnahme auf BPH
beschrieben. Für einen Fachmann ist es ohne weiteres
ersichtlich, daß die Vorrichtung und das Verfahren eingesetzt
werden können, um Körpergewebe in beliebigen Körperhöhlen
oder an beliebigen Gewebestellen zu zerstören, die mittels
perkutaner oder endoskopischer Katheter zugänglich sind, und
nicht auf die Prostata beschränkt sind. Eine Anwendung der
Vorrichtung und des Verfahrens in sämtlichen dieser Organe
und Gewebebereiche soll innerhalb des Umfangs dieser
Erfindung enthalten sein.
BPH ist ein Leiden, das von Reproduktion und Zellwachstum in
der Prostata und der Abnahme der Zellsterblichkeitsrate
herrührt, wobei Drüsen- und Stromaknötchen gebildet werden,
welche die Prostata erweitern und die Öffnung der
prostatischen Harnröhre verengen. Drüsenknötchen sind in
erster Linie innerhalb der Übergangszone konzentriert, und
Stromaknötchen innerhalb des periurethralen Bereichs.
Herkömmliche Behandlungsformen für dieses Leiden haben zur
Entfernung von Gewebe und zur Erweiterung des Durchlasses
eine operative Entfernung der gesamten Prostatadrüse, eine
diskrete Entfernung des Adenoms, sowie eine transurethrale
Resektion des Harnröhrenkanals und der Prostata umfaßt. Eine
mit diesen Verfahren verbundene bedeutende und ernste
Komplikation ist eine durch ärztliche Einwirkung entstandene
Unfruchtbarkeit. Vor kurzem ist eine Laserbehandlung
verwendet worden, um Gewebe zu entfernen, wobei Blutungen und
der Verlust von Körperfluiden begrenzt bleiben. Auch sind
Ballons innerhalb der Harnröhre erweitert worden, um deren
Durchmesser zu vergrößern, mit und ohne Wärme; man hat jedoch
herausgefunden, daß sie signifikanten Einschränkungen
unterworfen sind.
Eine Mikrowellentherapie ist mit einigem Erfolg eingesetzt
worden, indem eine Mikrowellenantenne innerhalb der
prostatischen Harnröhre positioniert und mit einem
elektromagnetischen Feld in dem die Harnröhre umgebenden
Gewebe Wärme erzeugt wurde. Kühlmittel werden manchmal
innerhalb des Katheterschafts zugeführt, um die Temperatur
der Harnröhrenwand zu verringern. Dies macht komplizierte
Mechanismen erforderlich, um gleichzeitig für eine Abkühlung
der unmittelbar angrenzenden Gewebebereiche zu sorgen,
während in dem weiter entfernten Prostatagewebe Wärme erzeugt
wird. Diese Technik ist der Mikrowellenhyperthermie ähnlich.
In ähnlicher Weise setzt eine Radiofrequenz-Gewebeablation
mit innerhalb der Harnröhre positionierten Elektroden die
Harnröhrenwand zerstörerischen Temperaturen aus. Um dies zu
vermeiden, müssen Temperatureinstellungen, die zum Schutz der
Harnröhre notwendig sind, so niedrig liegen, daß die
Behandlungszeit, die zum Erzeugen irgend einer nützlichen
Wirkung erforderlich ist, übermäßig verlängert wird, z. B. auf
eine Energiezufuhr von bis zu 3 Stunden Dauer.
Eine Ausführungsform der Vorrichtung dieser Erfindung
verwendet die Harnröhre, um Zugang zur Prostata zu finden,
und positioniert RF-Elektrodenmandrins unmittelbar in die zu
zerstörenden Gewebebereiche. Der Teil des Mandrinleiters, der
sich von der Harnröhre bis in die ins Auge gefaßten
Gewebebereiche erstreckt, ist innerhalb einer in
Längsrichtung verstellbaren Hüllabschirmung eingeschlossen,
die verhindert, daß das an die Hülle angrenzende Gewebe dem
RF-Strom ausgesetzt ist. Die Bewegung der Hülle wird auch
verwendet, um die abgegebene Energiemenge pro
Oberflächeneinheit zu steuern, indem man den Betrag der
freiliegenden Elektrode steuert. Somit ist die ablative
Zerstörung auf die zur Zerstörung vorgesehenen Gewebebereiche
begrenzt, nämlich auf diejenigen, welche die Verengung
verursachen. Andere Aspekte der Erfindung werden aus den
Zeichnungen und den begleitenden Beschreibungen der
Vorrichtung und des Verfahrens dieser Erfindung ersichtlich.
Für einen Fachmann ist es ohne weiteres ersichtlich, daß
dieses Verfahren in vielen Bereichen des Körpers für
perkutane Operationswege und Operationswege durch
Körperöffnungen eingesetzt werden kann.
Fig. 1 ist eine isometrische Ansicht einer
RF-Ablationskatheter-Ausführungsform dieser Erfindung mit einem
Faseroptik-Betrachtungszubehör. Der an einem Griff 4
befestigte flexible Katheter 2 weist eine stirnseitige
Mandrinführung 6 mit zwei Mandrins 8 auf. Der Griff weist
Mandrinhüllenschiebertasten 10 und 11 und
Elektrodenschiebertasten 12 und 13 auf, wie nachfolgend
ausführlicher beschrieben. Der Griff 4 ist auch mit einer
optischen Betrachtungseinheit 14 und einem RF-Energieanschluß
16, einem Transponderanschluß 18 und Thermoelementanschlüssen
20 verbunden. Die Teile des Katheters 2, die vom Griff zur
Mandrinführungsspitze 6 führen, können wahlweise eine
abgestufte Steifigkeit besitzen. Zum Beispiel kann der
Katheter so gestaltet sein, daß er nahe dem Griff steifer und
nahe der Spitze flexibler ist, oder er kann beliebige andere
Steifigkeitsprofile besitzen. Der Katheter kann aus einem
inneren geschlitzten Edelstahlrohr mit einer äußeren
flexiblen Hülle aufgebaut sein, wie in der am 11. August 1991
eingereichten mitanhängigen Anmeldung mit der Serial No.
790,648 (entsprechend der veröffentlichten Australischen
Patentanmeldung Serial No. 9210858) beschrieben, deren
gesamter Inhalt durch Bezugnahme hier aufgenommen wird. Er
kann auch aus gewundenem oder geflochtenem Draht bestehen,
mit dem eine äußere Hülle verbunden ist.
Die Faseroptik-Betrachtungseinheit umfaßt bei dieser
Ausführungsform eine Linsenfokussiereinheit 22 und eine
Halteanschlußeinheit 24 für eine Linsenbetrachtungseinheit,
die durch ein flexibles Rohrstück 28 an einem als Stecker
ausgebildeten Schnellöseanschluß 26 befestigt ist.
Fig. 2 ist eine Querschnittsansicht eines Katheters der Fig.
1, die Einzelheiten des Mandrinführungsgehäuses zeigt. Das
Mandrinführungsgehäuse 6 besitzt einen gekrümmten Durchlaß
28, durch den der Mandrin 8 in das zu behandelnde Gewebe
ausgefahren wird. Weitere Einzelheiten dieser Bauteile sind
in den mitanhängigen Anmeldungen mit der Serial No.
08/012,370, eingereicht am 2. Februar 1993, und der am 13.
Mai 1993 eingereichten Anmeldung mit der Anwalts-Registrier-
Nr. 5254-015-37 CIP mit dem Titel MEDIZINISCHE
SONDENVORRICHTUNG MIT OPTISCHEM SEHVERMÖGEN beschrieben.
Fig. 3 ist eine Seitenansicht der Mandrin- und Lumeneinheit
dieser Erfindung. Die Schlüsselbauteile des Mandrins dieser
Ausführungsform sind eine Isolierhülle 30 und eine sich durch
diese hindurch erstreckende Elektrode 32. Die Elektrode 32
besitzt eine geschärfte Spitze, bei dieser Ausführungsform
eine verbreiterte speerförmige Spitze. Die proximalen Enden
der Elektrode und der Hülle sind durch einen jeweiligen
Hüllenverbinder 334 und Elektrodenverbinder 336 mit einer
Griffhülle und Elektrodenschiebern verbunden, die nachfolgend
unter Bezugnahme auf die Fig. 10 und 13 ausführlicher
beschrieben werden. Ein Elektrodenstützrohr 36 erstreckt sich
vom Elektrodenverbinder 336 bis zu dem Bereich 38 des
Hüllenverbinders 334, um eine Druckkraft zu übertragen, ohne
die Elektrode 32 abzuknicken. Ein aus einem Stück
Schrumpfschlauch bestehendes Isolierhüllenstützrohr 40
erstreckt sich vom Hüllenverbinder 334 bis zum Beginn oder
proximalen Ende 42 des äußeren Rohrstücks 44. Das Rohrstück
44 verbindet das Stützrohrstück mit dem Steuerrohr 46. Das
Steuerrohr 46, das sowohl die Elektrode als auch die
Isolierhülle trägt, erstreckt sich bis zur Zusammenführung 48
(vgl. Fig. 4) des Elektrodenlumendurchlasses 50 und der
Elektrode 32. Auf diese Weise wird über die Länge des
Mandrins, die sich vom Griff bis zur Dreifachlumen-Spitze
erstreckt, für eine Stützung gesorgt, die ein Abknicken oder
einen Linearitätsverlust der hochflexiblen Elektrode
verhindert, wenn sie durch das Mandrinführungsgehäuse
gestoßen wird.
Fig. 4 ist eine Querschnitts-Seitenansicht der
Zusammenführung der Mandrin- und Steuerrohreinheit entlang
der Mittelachse des Rohrstücks, und Fig. 5 ist eine
Querschnittsansicht der Zusammenführung der Mandrin- und
steuerrohreinheit entlang der Linie B-B der Fig. 4. An der
Zusammenführung 48 erstreckt sich die Elektrode 32 durch die
Wand 62 des oberen Elektrodenlumens und tritt in das
Elektrodenlumen 50 ein. Das äußere Rohrstück 52 umschließt
und stützt sowohl das distale Ende des Steuerrohrstücks 46
als auch dasjenige des Dreifachlumen-Hüllrohrs 54.
Bezugnehmend auf Fig. 5 kann der Zwischenraum 56 zwischen dem
Steuerrohr 46 und dem Dreifachlumen 56 mit einem Kleber
gefüllt sein, um diese zusammenzuhalten. Das Dreifachlumen
umfaßt ein Elektrodenlumen 50, ein Temperatursensorlumen 58
und ein Fluidzufuhrlumen 60 für die Zufuhr von wahlweise
vorgesehenen Fluiden, wie beispielsweise Antibiotika oder
Anästhetika in den Behandlungsbereich.
Fig. 6 ist eine Querschnittsansicht eines Dreifachlumen-
Mandrins dieser Erfindung entlang der Linie A-A in Fig. 3.
Die Dreifachlumenhülle 30 ist eine Isolierhülle für die
Elektrode 32 und enthält das zusätzliche
Temperatursensorlumen 58 und das Flüssigkeitszufuhrlumen 60.
Die innere Oberfläche der Elektrodenlumina kann in einem
Abstand "h" von der äußeren Oberfläche der Elektrode
angeordnet sein, der zum Beispiel von etwa 1 bis 3 mm
betragen kann, um einen zusätzlichen Flüssigkeitszufuhrkanal
mit einem ungefähr ringförmigen Querschnitt zu bilden.
Fig. 7 ist eine Querschnitts-Seitenansicht der in Fig. 6
dargestellten Dreifachlumen-Mandrinspitze entlang der Linie
C-C. Das Stirnende des Temperatursensorlumens 58 ist
abgedichtet, um die elektrischen Bauteile zu schützen. Ein
Thermoelement 64 ist am distalen Ende der Hülle 30 plaziert,
um die Temperatur des die Elektrode 32 umgebenden Gewebes zu
überwachen, und ist vorzugsweise weniger als 1 mm von der
freiliegenden Elektrode entfernt. Ein Thermoelement 66 ist
mindestens etwa 3 mm und vorzugsweise zwischen etwa 3 bis 6
mm von der Spitze der Hülle 30 entfernt angebracht, um die
Temperatur der Ductuswand (beispielsweise der Harnröhre) zu
überwachen, durch welche der Mandrin ausgefahren wird. Dies
ist vorgesehen, um sicherzustellen, daß die Temperatur der
Ductuswand keine zerstörerischen Höhen erreicht, wenn die
RF-Behandlung von die ausgefahrene Elektrode umgebendem Gewebe
im Gange ist.
Fig. 8 ist eine Draufsicht auf die Ringnut-Ausführungsform
der Stromdichtefokussierelektrode dieser Erfindung. Bei
dieser Ausführungsform ist die Elektrode zu einer einzigen
scharfen stromfokussierenden Spitze 68 ohne eine sekundäre
Ecke oder andere scharfe Kanten geschliffen, die Strom
ebenfalls fokussieren oder verdichten könnten. Ein
zusätzliches Stromfokussieren kann entlang der
Elektrodenoberfläche durch die ringförmigen Nuten 70 und 72
vorgesehen werden. Die Temperatur des die Elektrode
umgebenden Gewebes steigt zuerst in den Anfangszonen 74, 76
und 78 an. Die Zone mit erhöhter Temperatur dehnt sich dann
auf zwei Zwischenzonen 80 und 82 aus, während die Zonen aus
den Nuten verschmelzen. Danach verschmelzen sämtliche der
Zonen mit erhöhter Temperatur unter Bildung der einzelnen
ovalen Zonenläsion 84. Eine Verwendung dieser
stromfokussierenden Nuten 70 und 72 erzeugt eine
symmetrischere Läsion.
Fig. 9 ist eine Draufsicht auf die Spiralnut-Ausführungsform
der Stromdichtefokussierelektrode dieser Erfindung. Bei
dieser Ausführungsform ist die Elektrode ebenfalls zu einer
einzigen scharfen stromfokussierenden Spitze 86 ohne
sekundäre scharfe Ecken oder Kanten geschliffen, die Strom
ebenfalls fokussieren oder verdichten könnten. Ein
zusätzliches Stromfokussieren kann entlang der
Elektrodenoberfläche durch mindestens eine spiralförmige oder
schraubenförmige Nut 88 vorgesehen werden. Die Temperatur des
die Elektrode umgebenden Gewebes steigt zuerst in der
Anfangsspitzenzone 90 und in einer spiralförmigen Zone 92 an.
Die Zone mit erhöhter Temperatur dehnt sich dann auf zwei
Zwischenzonen 94 und 96 aus, während die spiralförmige Zone
92 unter Bildung einer einzigen Zone 96 verschmilzt. Danach
verschmelzen sämtliche der Zonen mit erhöhter Temperatur
unter Bildung der einzelnen ovalen Zonenläsion 98. Eine
Verwendung der spiralförmigen Fokussiernut 38 erzeugt eine
symmetrischere Läsion.
Fig. 10 ist eine auseinandergezogene Ansicht der in Fig. 1
dargestellten RF-Ablationskathetereinheit. Die obere
Griffplatte 276 weist zwei mittlere Schlitze 278 und 280 auf,
durch welche die Elektrodensteuerschieber 10 und 11 an einem
jeweiligen linken Elektrodenschieberblock 282 und rechten
Elektrodenschieberblock 284 befestigt sind. Die
Hüllensteuerschieber 12 und 13 sind durch äußere Schlitze 286
und 288 mit einem jeweiligen linken Hüllenschieberblock 290
und rechten Hüllenschieberblock 292 verbunden. Ein
Faseroptik-Aufnahmegehäuse 30 ist auf der proximalen
Oberfläche der oberen Griffplatte 276 angebracht. Die
Aufnahme 294 für die elektrischen Anschlüsse ist in
jeweiligen Ausnehmungen 296 und 298 in der oberen Griffplatte
276 und der daran befestigten unteren Griffplatte 300
eingesetzt. Die untere Griffplatte 300 weist eine mittige
Ausnehmung 302 auf, welche die Elektroden- und
Hüllenschieberblocks und zugehörige Teile aufnimmt.
Mikroschalteraktivierblocks 304 (nur am linken Hüllenblock
dargestellt) sind mit den Hüllenschieberblocks 290 und 292
verbunden. Sie sind so positioniert, daß die Mikroschalter
306 betätigt werden, wenn der jeweilige Hüllenblock (und die
daran befestigte Hülle) vorwärtsbewegt worden sind. Die
Mikroschalter 306 halten die jeweiligen RF-Energie-
Stromkreise offen, bis die jeweiligen Hüllen in eine Position
jenseits der Harnröhrenwand und in die Prostata hinein
vorwärtsbewegt worden sind, um zu verhindern, daß die
Harnröhre den erregten RF-Elektroden unmittelbar ausgesetzt
ist. Ein Ausfahren der Hülle um 5 mm über die Führung hinaus
ist gewöhnlich ausreichend, um die Harnröhre zu schützen.
Die Zug/Drehmoment-Rohreinheit 308 ist im distalen Ende des
Gehäuses in der Aufnahme 310 angebracht.
Fig. 11 ist eine isometrische Ansicht der Verstellblock- und
Zugrohreinheit 308 des in Fig. 10 dargestellten
RF-Ablationskatheters. Das Drehmomentrohr 312 erstreckt sich von
der Drehmomentkupplung 314 durch den Drehsteuerkopf 316 bis
zur Mandrinführung 6. Eine Verbiegung des Drehmomentrohrs 312
während des Gebrauchs verlängert den Weg vom Griff zur
Führungsspitze 6. Um ein daraus resultierendes Zurückziehen
der Mandrinhüllen- und Elektrodenbauteile zu verhindern, wenn
das Drehmomentrohr verbogen wird, ist ein Zugrohr 318
vorgesehen, das eine feste Länge und einem Durchmesser
aufweist, der kleiner als der Innendurchmesser des
Drehmomentrohrs 12 ist. Das distale Ende des Zugrohrs 318 ist
fest mit der Mandrinführung 6 verbunden, und das proximale
Ende 320 ist am Verstellblock 322 befestigt, beispielsweise
mittels eines Klebers. Die axiale Position des Verstellblocks
322 kann verstellt werden, um sicherzustellen, daß die
Mandrins anfänglich gerade innerhalb der Auslaßöffnungen in
der Mandrinführung 6 positioniert sind. Die
Drehmomentkupplung 314 ist auf dem Kupplungsblock 324
montiert. Ein Drehsteuerknopf-Anschlagbolzen 326 erstreckt
sich in eine Nut (nicht dargestellt) und begrenzt die Drehung
des Steuerknopfs 316.
Fig. 12 ist eine Detailansicht "A" der Zugrohrverbindungen am
distalen Ende des in Fig. 11 dargestellten Zugrohrs. Das
Zugrohr 318 ist fest mit dem proximalen Ende 328 der
Mandrinführung 6 verbunden, beispielsweise durch ein Stück
Schrumpfschlauch 330.
Fig. 13 ist eine auseinandergezogene Ansicht der Hüllen- und
Elektrodenschieberblockeinheit der in Fig. 10 dargestellten
Ausführungsform. Der rechte Hüllenschieberblock 292 weist
einen Vorsprung 332 auf, der sich unter dem rechten
Elektrodenschieberblock 284 nach innen zu erstreckt. Der
rechte Hüllenverbinder 334 ist am inneren Ende des Vorsprungs
332 montiert und fest mit dem Ende des proximalen Endes der
Hülle 336 verbunden. Der rechte Elektrodenverbinder 338 ist
an einer inneren Oberfläche des Elekrodenschieberblocks 284
befestigt und ist fest mit dem proximalen Ende der Elektrode
340 verbunden. Die rechten Hüllen- und
Elektrodenschieberblocks 292 und 284 sind mittels Schrauben
(nicht dargestellt) durch Schlitze 344 und 346 hindurch
verschiebbar an der rechten Reibeinstellschiene 342
befestigt, wobei die Schrauben verstellbar sind, um für eine
ausreichende Reibung zwischen den Blocks und der Schiene 342
sorgen, um eine sichere Steuerung über die Mandrinbewegung zu
schaffen. Der linke Hüllenschieberblock 290 und der linke
Elektrodenschieberblock 282 sind spiegelbildliche Abbilder
der rechten Blocks und sind in ähnlicher Weise an der linken
Reibschiene 344 angebracht. Die linke Hülle und die linken
Elektroden sind nicht dargestellt.
Obwohl bevorzugte Ausführungsformen des Gegenstands der
Erfindung in einiger Ausführlichkeit beschrieben worden sind,
ist klar, daß offensichtliche Abwandlungen vorgenommen werden
können, ohne vom Geist und vom Umfang der Erfindung
abzuweichen, wie durch die beigefügten Patentansprüche
definiert.
Claims (17)
1. Medizinische Sondenvorrichtung, umfassend einen Katheter
mit einem Steuerende und einem Sondenende, wobei das
Sondenende umfaßt: ein Mandrinführungsgehäuse mit mindestens
einer Mandrinöffnung und Mandrinführungseinrichtungen, um
einen flexiblen Mandrin nach außen durch jede Mandrinöffnung
und durch dazwischenliegendes Gewebe in vorgesehene
Gewebebereiche zu lenken, einen in mindestens einer der
besagten Mandrinführungseinrichtungen positionierten Mandrin,
wobei der Mandrin eine nichtleitende Hülle mit einem
Elektrodenlumen und einem zweiten Lumen darin umfaßt, wobei
das Elektrodenlumen an einer distalen Öffnung im distalen
Ende der nichtleitenden Hülle endet, und wobei eine
Radiofrequenzelektrode für eine Längsbewegung durch die
distale Öffnung in dem besagten Elektrodenlumen positioniert
ist.
2. Medizinische Sondenvorrichtung nach Anspruch 1, bei
welcher die Radiofrequenzelektrodeoberfläche und mindestens
ein Teil einer gegenüberliegenden Oberfläche des
Elektrodenlumens in einem Abstand voneinander angeordnet
sind, um einen Flüssigkeitszufuhrdurchlaß zur Abgabe von
Medikamentenflüssigkeit zu bilden.
3. Medizinische Sondenvorrichtung nach Anspruch 1, bei
welcher das zweite Lumen ein Fluidzufuhrlumen ist, das an
einer distalen Öffnung im distalen Ende der nichtleitenden
Hülle endet.
4. Medizinische Sondenvorrichtung nach Anspruch 1, bei
welcher das zweite Lumen ein Temperatursensorlumen ist, das
angrenzend an das distale Ende der nichtleitenden Hülle
endet, wobei mindestens ein Temperaturfühler im
Temperatursensorlumen positioniert ist.
5. Medizinische Sondenvorrichtung nach Anspruch 4, bei
welcher der Temperaturfühler ein in der Nähe des distalen
Endes der nichtleitenden Hülle positioniertes Thermoelement
ist, dessen elektrische Zuleitungen sich durch das
Temperatursensorlumen erstrecken.
6. Medizinische Sondenvorrichtung nach Anspruch 4,
enthaltend zwei im Temperatursensorlumen positionierte
Temperaturfühler, wobei ein Temperaturfühler innerhalb von
etwa 1 mm vom distalen Ende der nichtleitenden Hülle
positioniert ist, und wobei der zweite Temperaturfühler
mindestens etwa 3 mm vom distalen Ende der nichtleitenden
Hülle entfernt positioniert ist.
7. Medizinische Sondenvorrichtung nach Anspruch 6, bei
welcher die Temperaturfühler Thermoelemente sind, deren
elektrische Zuleitungen sich durch das zweite Lumen
erstrecken.
8. Medizinische Sondenvorrichtung nach Anspruch 1, bei
welcher die nichtleitende Hülle mindestens ein
Elektrodenlumen, ein Temperatursensorlumen und ein
Fluidabgabelumen enthält, wobei das Elektrodenlumen an einer
distalen Öffnung im distalen Ende der nichtleitenden Hülle
endet, wobei eine Radiofrequenzelektrode für eine
Längsbewegung durch die distale Öffnung in dem besagten
Elektrodenlumen positioniert ist, wobei das Fluidabgabelumen
an einer distalen Öffnung im distalen Ende der nichtleitenden
Hülle endet, und wobei das Temperatursensorlumen angrenzend
an das distale Ende der nichtleitenden Hülle endet, wobei
mindestens ein Temperaturfühler im Temperatursensorlumen
positioniert ist.
9. Mehrfachlumen-Mandrin zur Verwendung in einer
medizinischen Sondenvorrichtung, umfassend eine nichtleitende
Hülle, die ein Elektrodenlumen, ein Fluidabgabelumen und ein
Temperatursensorlumen enthält, wobei das Elektrodenlumen an
einer distalen Öffnung im distalen Ende der nichtleitenden
Hülle endet, wobei eine Radiofrequenzelektrode für eine
Längsbewegung durch die distale Öffnung in dem besagten
Elektrodenlumen positioniert ist, wobei das Fluidabgabelumen
an einer distalen Öffnung im distalen Ende der nichtleitenden
Hülle endet, und wobei das Temperatursensorlumen angrenzend
an das distale Ende der nichtleitenden Hülle endet, wobei
mindestens ein Temperaturfühler im Temperatursensorlumen
positioniert ist.
10. Mehrfachlumen-Mandrin nach Anspruch 9, bei welchem die
Radiofrequenzelektrodenoberfläche und mindestens ein Teil
einer gegenüberliegenden Oberfläche des Elektrodenlumens im
Abstand angeordnet sind, um einen Flüssigkeitszufuhrdurchlaß
zur Abgabe von Arzneimittelflüssigkeit zu bilden.
11. Mehrfachlumen-Mandrin nach Anspruch 9, bei welchem der
Temperaturfühler ein in der Nähe des distalen Endes der
nichtleitenden Hülle positioniertes Thermoelement ist, dessen
elektrische Zuleitungen sich durch das Temperatursensorlumen
erstrecken.
12. Mehrfachlumen-Mandrin nach Anspruch 11, enthaltend zwei
im zweiten Lumen positionierte Temperaturfühler, wobei ein
Temperaturfühler innerhalb von etwa 1 mm vom distalen Ende
der nichtleitenden Hülle positioniert ist, und wobei der
zweite Temperaturfühler mindestens etwa 3 mm vom distalen
Ende der nichtleitenden Hülle entfernt positioniert ist.
13. Mehrfachlumen-Mandrin nach Anspruch 12, bei welchem die
Temperaturfühler Thermoelemente sind, deren elektrische
Zuleitung sich durch das zweite Lumen erstrecken.
14. Medizinische Sondenvorrichtung, umfassend einen Katheter
mit einem Steuerende und einem Sondenende, wobei das
Sondenende umfaßt: ein Mandrinführungsgehäuse mit einer
Mandrinöffnung und Mandrinführungseinrichtungen, um einen
flexiblen Mandrin nach außen durch die Mandrinöffnung und
durch dazwischenliegendes Gewebe in vorgesehene
Gewebebereiche zu lenken, einen in mindestens einer der
besagten Mandrinführungseinrichtungen positionierten Mandrin,
wobei der Mandrin einen innerhalb einer nichtleitenden Hülle
eingeschlossenen elektrischen Leiter umfaßt, wobei die
Elektrode ein distales Stück mit mindestens einer
stromfokussierenden Nut darauf und eine distale Spitze
aufweist, die so gestaltet ist, daß sie auf ihrem Stirnende
Strom fokussiert, wodurch ein von dort in umgebendes Gewebe
eintretender RF-Strom eine Läsion bildet, die sich von der
Nut und der Spitze nach außen erstreckt.
15. Medizinische Sondenvorrichtung nach Anspruch 14, bei der
das distale Stück eine Mehrzahl von ringförmigen
Fokussiernuten darauf aufweist.
16. Medizinische Sondenvorrichtung nach Anspruch 14, bei der
das distale Stück eine spiralförmige Fokussiernut darauf
aufweist.
17. Medizinische Sondenvorrichtung umfassend einen Katheter
mit einem Steuerende und einem Sondenende, wobei das
Sondenende umfaßt: ein Mandrinführungsgehäuse mit mindestens
einer Mandrinöffnung und Mandrinführungseinrichtungen, um
einen flexiblen Mandrin nach außen durch jede Mandrinöffnung
und durch dazwischenliegendes Gewebe in vorgesehene
Gewebebereiche zu lenken, einen in mindestens einer der
besagten Mandrinführungseinrichtungen positionierten Mandrin,
wobei der Mandrin eine nichtleitende Hülle mit einem
Elektrodenlumen umfaßt, wobei das Elektrodenlumen an einer
distalen Öffnung im distalen Ende der nichtleitenden Hülle
endet, wobei eine hochflexible Radiofrequenzelektrode für
eine Längsbewegung durch die distale Öffnung in dem besagten
Elektrodenlumen positioniert ist, wobei mindestens ein Teil
der Elektrode innerhalb eines Stützrohrs eingeschlossen ist,
das eine ausreichende Festigkeit aufweist, um eine
Elektrodenlinearität aufrechtzuerhalten, wenn die Elektrode
durch die Mandrinöffnung nach außen gelenkt wird.
