- ALLGEMEINER STAND DER TECHNIK
- Die Erfindung betrifft verschiedenartige miteinander verbundene Stents oder Stent-Abschnitte, bei denen die Verbindungen aus Stücken eines Kunststoffwerkstoffs bestehen. Der Begriff „Kunststoff" wird hier verwendet, um Werkstoffe zu bezeichnen, die bleibend verformt werden können ohne zu brechen.
- Nach dem Stand der Technik sind Stents bereits bekannt zur Öffnung und Verstärkung der Innenwand von Blutgefäßen und anderen röhrenförmigen Gefäßen des Körpers.
- Stents sind im Allgemeinen röhrenförmig, im Durchmesser dehnbar und können sich entweder selbst ausdehnen oder durch einen von außen auf den Stent aufgebrachten Druck gedehnt werden, üblicherweise durch Ausdehnung eines Ballons, der im Inneren platziert ist. Stents werden aus verschiedenen Werkstoffen hergestellt wie z.B. Kunststoff oder Metall, wobei Metall für gewöhnlich bevorzugt wird.
- Da Stents eine gewisse Steifigkeit aufweisen müssen, um Verstärkung und Halt zu bieten und da sie gegebenenfalls eine erhebliche Länge haben müssen, um sich über einen langen Bereich zu erstrecken, ist es schwierig, die Forderung nach Steifigkeit und gleichzeitig die Forderung nach einem flexiblen Stent zu erfüllen, der problemlos implantiert werden kann, indem er über eine gelegentlich gewundene und gebogene Strecke eingeführt wird, wie es oft beim perkutanen Verfahren der Einbringung der Fall ist, das üblicherweise beim Implantieren von Stents verwendet wird. Dies wird zusätzlich dadurch erschwert, dass Stents sich gleich nach der Implantation ausdehnen können müssen, um Halt zu bieten.
- Es ist bekannt, dass mehrere Stent-Bestandteile durch Fäden oder dergleichen lose miteinander verbunden werden können, um ein längliches Stent-Gebilde bereit zu stellen. Derartige Anordnungen werden z.B. in den folgenden Patentschriften aufgezeigt:

- Stents sind auch in zahlreichen anderen Veröffentlichungen offenbart worden, einschließlich WO 9633671, WO 9603092, WO 9531945 und WO 98/20810. In WO 9633671 wird ein Verbindungselement offenbart, um benachbarte Bereiche eines gegliederten Stents zu verbinden. Das Verbindungselement umfasst mehrere flexible Verbindungen. Jede flexible Verbindung weist einen gebogenen Bereich auf. WO 9603092 offenbart einen Stent, der erste und zweite miteinander verflochtene, gewundene Muster aufweist, die sich in der ersten und zweiten Richtung erstrecken. WO 9531945 offenbart einen Stent aus mehreren Bestandteilen, der sich zuerst selbst ausdehnt und sich dann bis zu einer ausgedehnten Endgröße verformt. WO 98/20810 offenbart einen Stent, der röhrenförmige Rahmen und Verbindungsabschnitte aufweist, die sich zwischen benachbarten röhrenförmigen Rahmen erstrecken. Die Verbindungselemente sollen einen Abstand zwischen benachbarten Abschnitten beschreiben, der gleich bleibt, wenn sich der Stent-Rahmen bewegt.
- KURZDARSTELLUNG DER ERFINDUNG
- Eine Aufgabe der Erfindung ist es, einen Stent nach Anspruch 1 bereit zu stellen.
- Die Erfindung zielt ab auf die Verbesserung des allgemeinen Prinzips von verbundenen Stents oder Stent-Abschnitten (nachstehend zusammenfassend als „Stent-Abschnitte" bezeichnet), in denen sich ein „Kunststoff"-Werkstoff (der bleibend verformbar ist) zwischen Stents oder Stent-Abschnitten (nachstehend zusammenfassend als „Stent-Abschnitte" bezeichnet) erstreckt, um sie mit etwas eingeschränkter Bewegungsfreiheit zueinander, d.h. nicht lose, sondern beweglich verbunden, miteinander zu verbinden. Die Stent-Abschnitte weisen vorzugsweise einen geschlossenzelligen Aufbau auf und sind im besten Fall selbstausdehnend. Genauer gesagt, bestehen die Verbindungselemente aus einem anderen Werkstoff als der Stent-Werkstoff und sind plastisch verformbar.

- Bezug nehmend auf
1 wird eine schematische Zeichnung eines erfindungsgemäßen flexiblen unterteilten Stents (10 ) gezeigt. Er besteht vorzugsweise aus mehreren geschlossenzelligen Stents oder Stent-Abschnitten (12 ), die durch Kunststoffverbindungselemente (14 ) miteinander verbunden sind. - Stents (
12 ) weisen am besten einen geschlossenzelligen Aufbau auf und sind selbstausdehnend, wie z.B. Nitinol-Stents, die aus röhrenförmigen Grundkörpern herausgeschnitten oder herausgeätzt werden oder aus geschnittenen oder geätzten flachen Blechen oder anderen Formgedächtnismetallen gerollt werden, die selbst keine bleibende Verformung aufweisen. - Im Allgemeinen neigt ein selbstausdehnender Stent dazu, in seinen entspannten oder ausgedehnten Zustand zurückzukehren. Außerdem weist diese Art Stent vorzugsweise einen geschlossenzelligen Aufbau auf. In Übereinstimmung mit der Erfindung wurde festgestellt, dass es besonders vorteilhaft ist, einen selbstausdehnenden elastischen Werkstoff für den Stent oder Stent-Abschnitt zu verwenden, d.h. einen Werkstoff, der nicht „plastisch" oder „verformbar" ist, und einen „plastischen", „verformbaren" Werkstoff für die Verbindungselemente zu verwenden. Derartige Werkstoffe wie Kunststoff, d.h. polymere Werkstoffe, die biologisch abbaubar sein können, Metalle wie Gold oder viskoelastische Polymere, wie Polyethylen, können verwendet werden. Durch diese Verbindungselemente ist eine eingeschränkte Bewegung möglich, aber auch eine gewisse Beweglichkeit der Stent-Abschnitte zueinander, und je nach Bedarf wird eine bleibende Ausdehnung der Verbindung ermöglicht.
- Wahlweise können die Stents von der Art sein, dass sie sich durch einen von außen aufgebrachten Radialdruck ausdehnen, wie im Fachgebiet bekannt, und können einen geschlossen- oder offenzelligen Aufbau aufweisen. Diese Stents können aus Metall wie z.B. Edelstahl, Titan, Nickel oder jedem anderen Metall sein, das körperverträglich ist. In dieser Art von Verbindung werden die Verbindungselemente jedoch erfindungsgemäß aus einem anderen Werkstoff sein als die Stents oder Stent-Abschnitte, aber die Verbindungselemente werden aus einem „Kunststoff", d.h. verformbaren Werkstoff sein, wie einem Polymer oder dergleichen, wie zuvor dargelegt.
- Im Einsatz werden diese Stent-Verbindungen verhältnismäßig lange Stents ermöglichen, die zum Zeitpunkt ihrer Verwendung auf jede gewünschte Länge gekürzt werden können.

4 ist ein Beispiel eines offenzelligen Aufbaus eines Stents (16 ).5 zeigt die offenen Zellen des Stents (16 ) im ausgedehnten Zustand. - In einer Ausführungsform der Erfindung betrifft sie selbstausdehnende Stents oder Stent-Abschnitte, die durch Verbindungselemente eines anderen Werkstoffs miteinander verbunden sind, der bleibende Verformung, d.h. „plastisches Verhalten" bei der Ausdehnung aufweist, wobei die Stents vorzugsweise einen geschlossenzelligen Aufbau aufweisen.
- In einer weiteren Ausführungsform der Erfindung betrifft sie ausdehnbare Stents mit Ballon oder ähnliche Stents oder Stent-Abschnitte, die durch strukturelle Verbindungselemente aus einem anderen „Kunststoff"-Werkstoff als die Stents oder Stent-Abschnitte starr miteinander verbunden sind, vorzugsweise aus einem polymeren Kunststoff, am besten biologisch abbaubar, obwohl das Verbindungselement im Falle eines Metall-Stents aus einem anderen Metall bestehen kann, das abweichende bleibende Verformungsmerkmale, d.h. plastisches Verhalten, aufweist.
- Die Verbindungselemente können aus jedem Werkstoff aus der Vielfalt implantierbarer Metalle oder polymerer Kunststoffe sein, wie z.B. Polytetrafluorethylen, Polyethylen, Polypropylen, Nylon, Polyester, Polyurethan und anderen, die bleibende Verformung aufweisen und aus einem anderen Werkstoff sind als dem der Stents oder Stent-Abschnitte an sich.
- Die Verbindungselemente können auch aus einem biologisch abbaubaren Werkstoff sein, wie z.B. Polycaprolacton, Polyglykolsäure, Polymilchsäure und dergleichen, solange der Werkstoff bleibende Verformung aufweist, und stellen einen strukturellen Bestandteil der Stent-Verbindung dar.
- Sind die Stents aus Metall, können sie mit einem biokompatiblen Werkstoff wie Polyurethan, Polyethylen, Polytetrafluorethylen, Silikon, Blockcopolymeren aus Polyurethan, Polyethylen und Silikon, biologisch abbaubaren Polymeren wie Polymilchsäure, Polyglykolsäure und/oder Hydroxybutyrat oder Valeratcopolymer beschichtet werden.
- In so einem Fall können die Verbindungselemente mit der Beschichtung auf jedem Stent-Abschnitt verschmolzen werden, um sie miteinander zu verbinden.
- Am besten wird die Verbindung zwischen Stents jedoch so erreicht, wie in
6 gezeigt. In einer solchen Anordnung wird auf dem Verbindungselement (20 ) ein erhabener Abschnitt (18 ) gebildet, und eine Öffnung (22 ) wird in Stent (24 ) gebildet, wobei die Öffnung (22 ) so geformt ist, dass sie den Abschnitt (18 ) aufnimmt und mit ihm zusammenpasst. Natürlich kann auch die umgekehrte Anordnung verwendet werden, in der der aufgenommene Abschnitt (18 ) auf dem Stent (22 ) liegt und die Öffnung (22 ) auf dem Verbindungselement (20 ). - Die Verbindungselemente sind vorzugsweise flach und gestreckt, können aber unterschiedliche Formen wie gerade, S-förmig, U-förmig usw. sowie unterschiedliche Querschnitte aufweisen.
- Die zuvor aufgeführten Beispiele und Offenbarungen dienen der Veranschaulichung und sind nicht auf diese beschränkt. Die Beispiele und die Beschreibung werden einem Durchschnittsfachmann viele Abwandlungen und Auswahlmöglichkeiten aufzeigen.
Claims (16)
- Unterteilter Stent, umfassend mehrere ausgerichtete, im Allgemeinen ringförmige und im Durchmesser dehnbare Stent-Abschnitte (
12 ), wobei die Abschnitte benachbart zueinander sind und durch mehrere einzelne Verbindungselemente (14 ) aus einem plastisch oder viskoelastisch verformbaren Werkstoff miteinander verbunden sind, der Verformungseigenschaften aufweist, die von denen der Abschnitte abweichen, um bewegliche aber eingeschränkte Relativbewegungen zwischen den Stent-Abschnitten zu ermöglichen, wobei die Verbindungselemente (14 ) aus einem anderen Werkstoff bestehen als die Stent-Abschnitte (12 ). - Unterteilter Stent nach Anspruch 1, wobei alle Verbindungselemente (
14 ) aus einem plastisch verformbaren Werkstoff sind. - Unterteilter Stent nach Anspruch 1, wobei alle Verbindungselemente (
14 ) aus einem viskoelastisch verformbaren Werkstoff sind. - Unterteilter Stent nach Anspruch 2 oder 3, wobei die Stent-Abschnitte (
12 ) einen geschlossenzelligen Aufbau aufweisen. - Unterteilter Stent nach Anspruch 2 oder 3, wobei die Stent-Abschnitte (
12 ) aus Metall sind. - Unterteilter Stent nach Anspruch 2 oder 3, wobei die Stent-Abschnitte (
12 ) aus einem Formgedächtniswerkstoff sind. - Unterteilter Stent nach Anspruch 2 oder 3, wobei die Stent-Abschnitte (
12 ) aus einem metallischen Formgedächtniswerkstoff sind. - Unterteilter Stent nach Anspruch 5 oder 7, wobei die Stent-Abschnitte (
12 ) mit einem polymeren Werkstoff beschichtet werden und die Verbindungselemente (14 ) mit der Polymerbeschichtung verschmolzen werden. - Unterteilter Stent nach Anspruch 8, wobei die Polymerbeschichtung und die Verbindungselemente unterschiedlicher Zusammensetzung sind.
- Unterteilter Stent nach Anspruch 8 oder 9, wobei die Stent-Abschnitte (
12 ) mindestens eine Befestigungsvorrichtung zur Aufnahme eines Stücks eines polymeren Werkstoffs und die Verbindungselemente passende Vorrichtungen zur Verbindung mit der Befestigungsvorrichtung umfassen. - Unterteilter Stent nach Anspruch 10, wobei die Befestigungsvorrichtung einen erhabenen Abschnitt bildet und die passenden Vorrichtungen an dem Stück des polymeren Werkstoffs eine Öffnung enthalten, die so bemessen ist, dass sie über die Befestigungsvorrichtung passt und beide ineinander greifen.
- Unterteilter Stent nach Anspruch 11, wobei das Stück des polymeren Werkstoffs an jedem Ende einen erhabenen Abschnitt umfasst und je Stent-Abschnitt mindestens eine Öffnung vorhanden ist, die so bemessen ist, dass sie einen erhabenen Abschnitt aufnimmt und beide ineinander greifen.
- Unterteilter Stent nach einem der Ansprüche 1 bis 12, wobei jeder Stent-Abschnitt (
12 ) als unabhängiger Stent-Körper entworfen und angeordnet ist, der in einem röhrenförmigen Gefäß des Körpers einzeln Unterstützung leisten kann. - Unterteilter Stent nach einem der Ansprüche 1 bis 13, wobei die Verbindungselemente (
14 ) biologisch abbaubar sind. - Stent nach Anspruch 2, wobei die Stent-Abschnitte (
12 ) und die Verbindungselemente (14 ) aus Metall sind, letztere aber andere bleibende Verformungseigenschaften aufweisen als die Stent-Abschnitte. - Unterteilter Stent nach Anspruch 15, wobei die Stent-Abschnitte (
12 ) mit einem Ballon ausdehnbar sind.
