Description
- Die Erfindung betrifft medizinische Vorrichtungen, insbesondere expandierbare Stents, mit einer Oberflächenbeschichtung, die auf einem Abschnitt der Oberfläche der Vorrichtung aufgebracht ist. Insbesondere betrifft die Erfindung eine im allgemeinen undurchlässige Oberflächenbeschichtung, die therapeutische Stoffe, die unter der Oberflächenbeschichtung angeordnet sind, vor der Umgebung so lange schützt, bis die Freigabe dieser Materialien erwünscht ist und durch Bruchbildung der Oberflächenbeschichtung erreicht wird.
- Angioplastik hat zwar eine hohe Akzeptanz gefunden, aber es sind abrupter Verschluß und Restenose als mögliche Folgeerscheinungen festgestellt worden. Abrupter Verschluß bedeutet eine akute Verstopfung eines Gefäßes unmittelbar nach oder innerhalb der ersten Stunden nach einem Dilatationseingriff. Ein abrupter Verschluß kann zu einem Myokardinfarkt führen, wenn der Blutstrom nicht rechtzeitig wiederhergestellt wird. Die primären Mechanismen abrupter Verschlüsse sind Arteriendissektion und/oder Thrombose. Restenose bedeutet erneute Verengung einer Arterie nach einer anfänglich erfolgreichen Angioplastik. Restenose tritt in erster Linie innerhalb der ersten sechs Monate nach einer Angioplastik auf und ist möglicherweise auf die Vermehrung und Wanderung von Zellkomponenten der Arterienwand zurückzuführen.
- Endovaskuläre Stents werden in dem dilatierten Segment eines Gefäßlumens angeordnet, um die Auswirkungen eines abrupten Verschlusses und einer Restenose mechanisch zu verhindern. In US-A-5 514 154 offenbaren Lau et al. einen expandierbaren Stent, der entlang seiner Längsachse relativ flexibel ist. Diese Flexibilität ermöglicht die Zuführung des Stents durch gewundene Körperlumen. Außerdem ist der Stent in einem expandierten Zustand radial ausreichend steif und stabil, um die Durchlässigkeit eines Körperlumens, z. B. einer Arterie, wenn dieser darin implantiert ist, zu erhalten. Solche Stents haben bisher jedoch weder einen abrupten Verschluß und noch eine Restenose ausgeschlossen.
- Jüngste Entwicklungen haben zu Stents geführt, die versuchen, thromboseverhindernde und andere Medikamente zu den Bereichen eines Blutgefäßes zu befördern, die durch Angioplastik oder andere Eingriffstechniken behandelt worden sind. In US-A-5 464 650 offenbaren Berg et al. ein Verfahren zur Herstellung eines intravaskulären Stents, wobei eine Lösung, die ein Lösemittel, ein Polymer, das im Lösemittel gelöst ist, und eine therapeutische Substanz, die im Lösemittel fein verteilt ist, auf den Stent und insbesondere auf seine Gewebekontaktfläche aufgebracht wird. Nachdem die Lösung auf den Stent aufgebracht ist, wird das Lösemittel dann verdampft, wobei die Oberflächen aus Polymer und therapeutischem Wirkstoff zurückbleibt. Berg et al. behaupten, daß diese Vorrichtungen in der Lage sind, sowohl kurzfristige Medizinabgabe über die ersten Stunden und Tage nach der Behandlung als auch langfristige Medikamentabgabe über die Wochen und Monate nach der Behandlung durchzuführen.
- US-A-5 324 261 betrifft einen Ballonkatheter mit einem Katheterschaft mit einem Markerband aus strahlenundurchlässigem Material, das die richtige Positionierung des Ballonabschnitts des Katheters im Körperlumen ermöglicht. Um den Schaft herum befindet sich ein Ballon mit einer Dichtung am Schaft an einem distalen Ende und mit einem Durchgang an einem proximalen Ende, was es ermöglicht, den Ballon aufzublasen. Um den Ballon herum ist ein viskoses Matrixmaterial, das ein Medikament enthält, und eine Hülle, die die viskose Materialmatrix auf den ringförmigen Raum zwischen dem Ballon und der Hülle begrenzt. Um zu verhindern, daß das viskose Matrixmaterial an den Enden der Hülle herausgequetscht wird, wenn der Ballon expandiert wird, sind Dichtungen am proximalen und distalen Ende vorgesehen.
- Ein ständiges Problem bei gegenwärtigen Medikamentenfreigabebeschichtungen, die auf Vorrichtungen wie etwa auf Stents aufgebracht werden, besteht darin, eine therapeutische lokale Konzentration eines Medikaments an einem Zielort innerhalb des Körpers zu erreichen, ohne unerwünschte systemische Nebenwirkungen hervorzurufen. Die Implantation von vaskulären Stents ist ein hervorragendes Beispiel für eine Situation, wo eine lokale Therapie mit Medikamenten benötigt wird, die auch unerwünschte systemische Nebenwirkungen hervorrufen. Da der Stent in einem Blutstrom plaziert wird, können während der Plazierung und nach der Implantation potentiell unerwünschte systemische Wirkungen dadurch entstehen, daß unerwünschte Mengen der therapeutischen Substanz in den Blutstrom gelangen. Wenn ferner während der Positionierung des Stents Mengen einer therapeutischen Substanz in den Blutstrom freigegeben werden, dann ist weniger für die tatsächliche lokale Behandlung verfügbar, wenn der Stent expandiert ist, was zu einer potentiellen unangemessenen lokalen Dosierung führt. Die vielen Versuche, eine lokale Medikamentenabgabe mit endovaskulären Mitteln zu bewirken, haben bisher das Problem der systemischen Steuerung der Abgabe der therapeutischen Substanz während der Implantation und nach der Implantation nicht gelöst.
- Deshalb besteht in der Fachwelt Bedarf an einer Einrichtung und einem Verfahren zur Durchführung einer lokalen Therapie, die eine hohe lokale Konzentration von therapeutischen Medikamenten an einer vorbestimmten Stelle aufrechterhalten kann, ohne unerwünschte Nebenwirkungen hervorzurufen, z. B. unerwünschte Mengen von Medikamenten, die in den Blutstrom gelangen. Es ist insbesondere notwendig, eine angemessene Konzentration von therapeutischen Wirkstoffen direkt an der Grenzschicht des Blutstroms nahe der Gefäßwand an einer angestrebten Behandlungsfläche zu erreichen, die die systemischen Nebenwirkungen und die Menge des Medikaments, die benötigt wird, um ein therapeutisches Ergebnis zu erreichen, stark reduziert.
- Die Erfindung betrifft eine medizinische Vorrichtung nach Anspruch 1, die eine dünne Schicht aufweist, die auf einer freizugebenden Schicht aus einem Medikament oder einer therapeutischen Substanz angeordnet ist. Die dünne darüberliegende Schicht schützt die Medikamentenschicht während der Po sitionierung und Entfaltung der medizinischen Vorrichtung in einem Körperlumen. Die dünne darüberliegende Schicht ist so gewählt, daß sie aus einem Polymermaterial besteht und eine gewählte Dicke hat, die im wesentlichen das Herausspülen der therapeutischen Substanz verhindert, bevor die medizinische Vorrichtung an einer gewünschten Stelle im Körperlumen plaziert und expandiert ist. Die Expansion der medizinischen Vorrichtung erzeugt Risse in der gesamten darüberliegenden Schicht, durch die hindurch die therapeutische Substanz herausgespült wird. Bei dieser Konstruktion wird sichergestellt, daß die therapeutische Substanz an der gewählten örtlich begrenzten Abgabestelle mit wesentlich reduzierter systemischer Wirkung, die daraus resultiert, daß das Medikament in den Blutstrom eintritt und von der Entfaltungsstelle weiterwandert, aufgebracht wird.
- Die Erfindung ist geeignet in Verbindung mit einer lokalen Abgabe von Medikamenten oder therapeutischen Substanzen auf einem expandierbaren Stent innerhalb des Gefäßsystems. Die Erfindung kann auch verwendet werden in Verbindung mit einer Medikamentenabgabe von Ballonkathetern oder Stents zur Verwendung in anderen Körperlumen. Die Erfindung ist besonders geeignet, wenn ein wasserlösliches Medikament oder eine therapeutische Substanz verwendet wird, die dazu neigen, sich innerhalb einer Blut- oder anderen Körperfluidumgebung zu lösen und zu wandern. Es ist außerdem nützlich, die darüberliegende Schutzbeschichtung zu verwenden, wenn das Medikament auf der medizinischen Vorrichtung mit einem hydrophilen Polymerträger plaziert ist, der das Medikament oder die therapeutische Substanz über eine längere Zeitperiode lösen und abgeben soll, wenn die Vorrichtung innerhalb des Körperlumens an einer gewählten Stelle plaziert ist. Die zweite oder darüberliegende Schicht aus Schutzmaterial, das bei Expansion der medizinischen Vorrichtung bricht, ist vorzugsweise ein biologisch abbaubares Polymer, das vom Körper über eine längere Zeitperiode absorbiert wird, aber dennoch das Herausspülen des Medikaments oder der therapeutischen Substanz während der Zeit, die erforderlich ist, um die Vorrichtung innerhalb des Gefäßlumens zu plazieren, verhindert.
- In bevorzugten Ausführungsformen der Erfindung ist die medizinische Vorrichtung eine Vorrichtung, die zur Behandlung einer Gefäßlumenwand, z. B. einer Arterienwand, in Verbindung mit der Behandlung einer Stenose an der gleichen Stelle konstruiert ist. Die medizinische Vorrichtung weist vorzugsweise einen Körperabschnitt mit einer darauf gebildeten Außenfläche auf, wobei der Körperabschnitt von einer ersten Position, wo der Körperabschnitt für eine Einführung in das Gefäßlumen bemessen ist, zu einer zweiten Position, wo zumindest ein Abschnitt der Außenfläche der medizinischen Vorrichtung in Kontakt mit der Lumenwand ist, expandierbar ist. Eine bevorzugte medizinische Vorrichtung weist eine erste Beschichtung auf, die auf zumindest einem Abschnitt der Außenfläche des Körperabschnitts der medizinischen Vorrichtung angeordnet ist, wobei die erste Beschichtung zumindest teilweise ein Medikament oder eine therapeutische Substanz zur Behandlung der Gefäßwand aufweist. In bevorzugten Ausführungsformen weist die erste Beschichtung das Medikament oder die therapeutische Substanz in Kombination mit einem Polymerträger auf, der die Abgaberate des Medikaments oder der therapeutischen Substanz über eine längere Zeitperiode vorzugsweise steuert. Die therapeutische Substanz oder das Medikament löst sich aus der ersten Beschichtung in bevorzugten Ausführungsformen als Antwort auf den Kontakt mit einem Fluid innerhalb des Gefäßlumens oder an der Gefäßwand.
- Bevorzugte Medikamente oder therapeutische Substanzen, die in die erste Beschichtung einbezogen sein können oder in der ersten Beschichtung mit einem Polymerträger enthalten sein können, sind u. a. Heparin, Antibiotika, strahlenundurchlässige Wirkstoffe, thromboseverhindernde Wirkstoffe, wucherungshemmende Wirkstoffe, gefäßbildungsverhindernde Wirkstoffe und Kombinationen daraus. Spezifische Medikamente können Taxol, Taxolderivate, Colchicin, Vinblastin oder Epothilone aufweisen, die als wucherungshemmende Wirkstoffe und gefäßbildungsverhindernde Wirkstoffe klassifiziert sind. Ein bevorzugter Polymerträger für das Medikament oder die therapeutische Substanz ist ein biologisch abbaubarer Wirkstoff, der Polymilchsäure, Polyglycolsäure, Polyethylenoxid, Polycaprolactone, Po lydioxanon, Poly(orthoester), Polyanhydride, Polyphosphazene und Gemische oder Copolymere dieser Polymermaterialien aufweisen kann. Das Polymermaterial könnte auch so gewählt sein, daß es biologisch nicht abbaubare Polymere, z. B. Polyurethan, Polysiloxan, Polyacrylat und Gemische und Copolymere dieser Polymermaterialen aufweist.
- Die Erfindung weist eine zweite Beschichtung auf, die vorzugsweise auf mindestens einem wesentlichen Abschnitt der ersten Beschichtung angeordnet ist. Die zweite Beschichtung weist mindestens teilweise ein Material auf, das im allgemeinen beständig gegen das Herausspülen des Medikaments oder der therapeutischen Substanz ist, wenn der Körperabschnitt der medizinischen Vorrichtung in der ersten Position ist, während er in das Lumen eingeführt und darin plaziert wird. Ferner ist das Material der zweiten Beschichtung vorzugsweise relativ unelastisch, so daß die zweite Beschichtung während der Expansion des Körperabschnitts der medizinischen Vorrichtung zu der zweiten Position zerstört wird, um ein Herausspülen des Medikaments oder der therapeutischen Substanz durch durchgehende Risse, die in der Oberfläche der zweiten Beschichtung entstehen, zu ermöglichen. Die zweite erfindungsgemäße Beschichtung wirkt in Kombination mit expandierbaren Stents oder Medikamentenabgabeballons besonders gut. Die zweite Beschichtung ist vorzugsweise ein Polymermaterial, das Polymilchsäure, Polyglycolsäure, Polyanhydride, Polyphosphazene, Poly(orthoester) und Gemische und Copolymere dieser Polymermaterialien aufweisen kann. In bevorzugten Ausführungsformen ist die Dicke der zweiten Beschichtung etwa 0,01 μm bis etwa 5 μm.
- Eine bevorzugte erfindungsgemäße medizinische Vorrichtung weist einen Stent auf, der eine im allgemeinen röhrenförmige Struktur mit einer Außenfläche hat, die durch eine Vielzahl von miteinander verbundene Streben mit Zwischenräumen zwischen diesen gebildet wird. Die im allgemeinen röhrenförmige Struktur ist von einer ersten Position, wo der Stent für ein intravaskuläres Einführen bemessen ist, zu einer zweiten Position, wo mindestens ein Abschnitt der Außenfläche des Stents die Gefäßwand berührt, expandierbar. Das Expandieren des Stents wird durch Biegen und Knicken der miteinander ver bundenen Streben in der gesamten im allgemeinen röhrenförmigen Struktur gefördert. Die zweite Beschichtung, die auf der Stentoberfläche angeordnet ist und die therapeutische Substanz oder das Medikament während der Plazierung schützt, bricht beim Biegen und Knicken der Streben des Stents während der Expansion.
- Die zweite erfindungsgemäße Beschichtung kann auch auf einer medizinischen Vorrichtung benutzt werden, z. B. auf einem Ballonkatheter, wobei die zweite Beschichtung auf mindestens einem Abschnitt des Ballons vorhanden ist, der expandierbar ist, und zwar von einer gefalteten oder nichtexpandierten ersten Position zu einer expandierten zweiten Position, die zur Behandlung der Gefäßlumenwand durch Kontakt mit mindestens einem Abschnitt der Außenfläche des Ballons geeignet ist. Die zweite Beschichtung, die auf dem Medikament oder der therapeutischen Substanz angeordnet ist, ist wiederum relativ unelastisch, so daß bei Expansion des Ballons die Beschichtung bricht, um ein Herausspülen des Medikaments oder der therapeutischen Substanz durch Risse, die in der Beschichtung entstehen, zu ermöglichen.
- Zusätzliche Merkmale der Erfindung und daraus abgeleitete Vorteile und die verschiedenen Aspekte der Erfindung werden nachstehend anhand der Zeichnungen, der Beschreibung der bevorzugten Ausführungsformen der Erfindung und der Ansprüche deutlich.

1 gezeigten Typs, die in WO 98/56312 offenbart sind, dienen den gewünschten Anwendungen der Dilatation und Aufrechterhaltung der Durchgängigkeit verschiedener Lumendurchgänge innerhalb des menschlichen Körpers. Die oben genannte Anmeldung ist auf den Rechtsnachfolger der vorliegenden Erfindung übertragen. - Wenn wir uns nunmehr den Zeichnungen zuwenden, in denen gleiche Bezugszeichen gleiche Elemente in allen verschiedenen Ansichten bezeichnen, so zeigt
1 eine perspektivische Ansicht eines erfindungsgemäßen Stents10 in einer nichtexpandierten Form. Der skelettartige Rahmen des Stents10 weist vorzugsweise drahtähnliche Teile12 auf, die ein besonderes, sich wiederholendes Serpentinen- bzw. Schlangenmuster bilden. Dieses sich wiederholende Serpentinenmuster besteht aus mehreren U-förmigen Kurven14 . Diese U-förmigen Kurven14 bilden Zwischenräume16 . Ohne erkennbaren Anfang oder Ende dieses serpentinenförmigen Musters bildet der Draht12 ein expandierbares serpentinenförmiges Element18 . Serpentinenförmige Elemente18 sind entlang der Längsachse des Stents10 so angeordnet, daß die U-förmigen Kurven14 von anliegenden serpentinenförmigen Elementen18 durch ein Verbindungselement20 verbunden werden können. Durch die Verbindungselemente20 entsteht ein Rahmen aus einem Endlosdraht12 zwischen mehreren serpentinenförmigen Elementen18 , die den Stent10 bilden. - Eine Oberflächenbehandlung
30 wird selektiv auf den skelettartigen Rahmen des Stents10 aufgebracht. Diese Oberflächenbehandlung30 weist mindestens zwei darüberliegende Beschichtungen auf, die selektiv auf mindestens einem Abschnitt der Außenfläche der drahtähnlichen Teile12 aufgebracht sind. Die Funktion der beiden darüberliegenden Beschichtungen ist am besten aus den Bildern in2 und3 ersichtlich, wie nachstehend beschrieben wird. - Wenn wir nunmehr
2 betrachten, so ist ein Abschnitt des Stents aus1 in einer vergrößerten Ansicht mit der ersten Beschichtung32 , die auf mindestens einem Abschnitt der Außenfläche der drahtähnlichen Teile12 angeordnet ist, dargestellt. Die erste Beschichtung32 weist mindestens teilweise eine therapeutische Substanz oder Medikament auf. In einer bevorzugten Ausführungsform besteht die therapeutische Substanz zumindest teilweise aus einem wucherungshemmenden oder gefäßbildungsverhindernden Medikament. Diese therapeutische Substanz ist normalerweise fein verteilt und in einer Polymerträgermatrix enthalten, die die erste Beschichtung32 bildet. Die Verwendung einer Matrix ermöglicht eine selektiv gesteuerte Ausspülrate für das einbegriffene Medikament oder die einbegriffene therapeutische Substanz. Durch Modifizierung des Anteils des Polymerträgers an der therapeutischen Substanz wird die Fähigkeit der therapeutischen Substanz, sich aus dem Träger herausspülen zu lassen, größer oder kleiner. In einer weiteren bevorzugten Ausführungsform kann die Ausspülrate der therapeutischen Substanz durch Verwendung eines biologisch abbaubaren Polymerträgers gesteuert werden. Polymilchsäure (PLA), Polyglycolsäure, Polyethylenoxid (PEO), Polycaprolacton sind Beispiele für solche Polymerträger. Entweder können die Homopolymer- oder die Copolymerformen dieser Materialien bei der Einbeziehung in die therapeutische Substanz benutzt werden. Weitere Polymerträger, die verwendet werden können, weisen Polyurethan, Polysiloxan, Polyacrylat und deren Gemische oder Copolymere auf. - Andere therapeutische Substanzen oder Medikamente können auch in die Beschichtung einbezogen sein, entweder allein oder in Kombination mit den wucherungshemmenden und gefäßbildungsverhindernden Wirkstoffen. Diese Medikamente können Heparin, Antibiotika, strahlenundurchlässige Wirkstoffe und thrombosehemmende Wirkstoffe aufweisen.
- Diese Komponenten können dem wucherungshemmenden oder gefäßbildungsverhindernden Medikament hinzugefügt werden, um die Funktion der therapeutischen Substanz zu ändern. In einer bevorzugten Ausführungsform kann, um die Aggregation von Blutplättchen, Fibrin, Gerinnungsfaktoren und zellularen Elementen des Blutes im Ergebnis der Implementierung des Stents
10 zu verhindern, das wucherungshemmende oder gefäßbildungsverhindernde Medikament auch ein zusätzliches thrombosehemmendes Medikament enthalten, das mit den anderen Medikamenten, die die erste Beschichtung32 bilden, einbezogen sein kann. In einer weiteren bevorzugten Ausführungsform ist das gefäßbildungsverhindernde Medikament Taxol, Taxolderivate, Colchicin, Vinblastin oder Epothilon. - Die zweite Beschichtung
34 , vorzugsweise die äußerste Beschichtung, ist zumindest auf einem wesentlichen Abschnitt der ersten oder vorherigen Beschichtung32 angeordnet. Diese zweite Beschichtung34 weist ein Material auf, das im allgemeinen gegen das Herausspülen der therapeutischen Substanz oder des Medikaments, das in die erste Beschichtung32 einbezogen ist, beständig ist. Die zweite Beschichtung34 wirkt als Schutzbeschichtung für das Medikament oder die therapeutische Substanz und verhindert, daß das Medikament oder die therapeutische Substanz vor der richtigen Plazierung innerhalb des Gefäßlumens an einer Behandlungsstelle ausgespült wird. Ferner verhindert diese Schutzschicht eine physische Beschädigung der Medikamentenbeschichtungsschicht während des Einführens. In bevorzugten Ausführungsformen wird die Abgabe der therapeutischen Substanz durch Kontakt mit Wasser oder anderen Flüssigkeiten, die im Blut oder anderen Körperlumen vorhanden sind, verbessert. Dies gilt besonders für Ausführungsformen, die ein Medikament oder eine therapeutische Substanz aufnehmen, die wasserlöslich ist, oder einen Polymerträger, der hydrophil ist und dazu neigt, bei Kontakt mit einer Flüssigkeit, z. B. Wasser, mehr therapeutische Substanz freizugeben. Daher ist die zweite Beschichtung34 oder Deckschicht der Schutzbeschichtung vorzugsweise undurchlässig für Wasser oder Körperfluide, zumindest für die Zeitperiode, die für die Plazierung des Stents innerhalb des Gefäßlumens an der Behandlungsstelle erforderlich ist. Auf diese Weise wird das Herausspülen des Medikaments so lange verhindert, bis sich die medizinische Vorrichtung oder der Stent an der erforderlichen Behandlungsstelle befindet. - Bei Verwendung wird also der erfindungsgemäße Stent oder eine andere medizinische Vorrichtung zunächst mit einem Medikament oder einer therapeutischen Substanz beschichtet, gefolgt von einer darüber angeordneten Schicht einer zweiten Beschichtung oder Schutzbeschichtung. Die erste Beschichtung kann auf die medizinische Vorrichtung aufgebracht werden, während sie in einer nichtexpandierten Konfiguration ist, oder die Vorrichtung kann als Alternative zur Beschichtung expandiert werden und dann zur Planierung im Gefäß kontrahiert werden. Die zweite Beschichtung oder Schutzbeschichtung wird so hergestellt, daß sie auf der ersten Beschichtung angeordnet ist, während die medizinische Vorrichtung in der ersten Position zum Einführen in das Lumen ist. In bevorzugten Ausführungsformen ist das zweite Beschichtungsmaterial relativ unelastisch, so daß die zweite Beschichtung während der Expansion des Körperabschnitts der medizinischen Vorrichtung zu der zweiten Position bricht. Der Abschnitt des Stents, der in
2 dargestellt ist, ist in3 in einem expandierten Zustand dargestellt, was deutlich eine große Anzahl von Rissen oder Brüchen40 in der gesamten zweiten Beschichtung32 zeigt, wobei das Medikament oder die therapeutische Substanz30 durch die Risse40 ausgespült wird. - Wie bereits ausgeführt, ist die zweite Beschichtung vorzugsweise eine Polymerbeschichtung, die relativ unelastisch ist, so daß die Expansion des Stents oder einer anderen medizinischen Vorrichtung bewirkt, daß sich Risse in der Beschichtungsoberfläche bilden. Diese Risse ermöglichen das Herausspülen des Medikaments oder der therapeutischen Substanz durch diese. Ein bevorzugtes Polymermaterial weist Polymilchsäure, Polyglycolsäure, Polyanhydride, Polyphosphazen oder Poly(orthoester) auf. Die Dicke der zweiten Beschichtung ist vorzugsweise etwa 0,01 μm bis etwa 5,0 μm. Die zweite Beschichtung ist vorzugsweise biologisch abbaubar oder biologisch absorbierbar, so daß die zweite Beschichtung mit der Zeit vom Körper absorbiert wird.
- Die Erfindung ist in Verbindung mit einem bevorzugten Stent beschrieben worden. Man erkennt jedoch, daß die hier offenbarte Erfindung mit einem beliebigen Stentaufbau benutzt werden kann, der aus einer ersten Position zu einer zweiten Position mit resultierendem Bruchgefüge einer zweiten darauf aufgebrachten Schutzbeschichtung expandiert wird. Bevorzugte Stents sind u. a. solche, die im allgemeinen röhrenförmige Strukturen mit einer Außenfläche aufweisen, die durch eine Vielzahl von miteinander verbundenen Streben mit Zwischenräumen dazwischen miteinander verbunden sind. In der ersten Position ist der Stent so bemessen, daß er in das Lumen des intravaskulären Systems eingeführt werden kann. Wenn sich der Stent an einem Behandlungspunkt befindet, kann der Stent zu einer zweiten Position, wo mindestens ein Abschnitt der Außenfläche die Wand des Gefäßlumens berührt, expandiert werden.
- Die Verwendung der zweiten Schutzbeschichtung, wie oben für einen Stent beschrieben, kann auch in eine solche medizinische Vorrichtung wie etwa ein Medikamentenabgabe-Ballonkatheter oder einen Ballondilatationskatheter einbezogen werden.
4 und5 zeigen die erfindungsgemäße Einbeziehung in eine Ballonkatheterkonstruktion. Die oben beschriebenen Materialien und therapeutischen Substanzen sind bei der vorliegenden Ballonkatheterausführungsform gleichermaßen anwendbar. - Wenn wir nunmehr auf
4 betrachten, so ist ein Gefäß50 innerhalb des Gefäßsystems schematisch mit einem darin gebildeten Lumen52 dargestellt, das teilweise durch eine Stenose54 verschlossen ist. Das Lumen wird durch Lumenwände56 gebildet, wie dargestellt. Ein Ballonkatheter58 ist in der Darstellung in das Gefäßlumen52 eingeführt. Der Ballonkatheter58 weist einen Schaft60 auf, auf dessen distalem Abschnitt ein aufblasbarer Ballon62 angeordnet ist. Der Ballon62 ist in einem gefalteten oder nichtaufgeblasenen Profil dargestellt, das zum Einführen in das Lumen52 des Blutgefäßes50 geeignet ist. - Der Ballon
62 weist ein Medikament oder eine therapeutische Substanz auf, die auf dessen Oberfläche aufgebracht ist. In bevorzugten Ausführungsformen wird die gesamte Fläche des Ballons mit dem Medikament oder der therapeutischen Substanz beschichtet, entweder allein oder in einem Polymerträger, während der Ballon in einer aufgeblasenen Position ist, so daß seine gesamte Fläche das Medikament oder die therapeu tische Substanz aufweist. Die zweite Beschichtung oder Schutzbeschichtung wird aufgebracht, während sich der Ballon in einem geleerten oder einem Zustand mit geringem Profil befindet. Wie bereits erwähnt, verhindert diese Schutzbeschichtung das Herausspülen des Medikaments aus der Oberfläche des Ballons, die während des Einführens des Katheters vor dem Aufblasen des Ballons an der Behandlungsstelle freiliegt. - Wenn wir nunmehr auf
5 betrachten, so ist der Ballonkatheter58 aus4 mit dem Ballon in einem aufgeblasenen Zustand über dem Stenosebereich54 dargestellt. Wie in5 gezeigt, weist der Ballon auf: eine erste Schicht30 , die das Medikament oder die therapeutische Substanz aufweist, und eine zweite Schicht, die die Schutzbeschichtung ist, die aufgebracht wurde, als der Ballon in einem entleerten Zustand war.5 zeigt schematisch die vielfältigen Risse40 , die in der Ballonoberflächenbeschichtung32 nach Expansion des Ballons entstehen. Dies führt zu einem Herausspülen des Medikaments oder der therapeutischen Substanz30 durch die Risse in der Behandlungsfläche. - Zahlreiche Merkmale und Vorteile der Erfindung, die in diesem Dokument dargestellt sind, sind in der vorstehenden Beschreibung ausgeführt. Es versteht sich jedoch, daß diese Offenbarung in vielerlei Hinsicht nur darstellenden Charakter hat. Änderungen im Detail, insbesondere in bezug auf Form, Größe und Anordnung von Teilen, sind möglich, ohne den Schutzbereich der Erfindung zu verlassen. Der Schutzbereich der Erfindung ist natürlich in den Formulierungen der beigefügten Ansprüche ausgedrückt.
Claims (18)
- Medizinische Vorrichtung (
10 ;58 ) zur Behandlung einer Lumenwand (56 ) mit: einem Körperabschnitt mit einer darauf gebildeten Außenfläche, wobei der Körperabschnitt von einer ersten Position, wo der Körperabschnitt zum Einführen in das Lumen (52 ) bemessen ist, zu einer zweiten Position, wo mindestens ein Abschnitt der Außenfläche in Kontakt mit der Lumenwand ist, expandierbar ist; einer ersten Beschichtung (32 ), die auf mindestens einen Abschnitt der Außenfläche des Körperabschnitts angeordnet ist, wobei die erste Beschichtung (32 ) mindestens teilweise eine therapeutische Substanz aufweist; und einer zweiten Beschichtung (34 ), die mindestens auf einem wesentlichen Abschnitt der ersten Beschichtung (32 ) angeordnet ist, wobei die zweite Beschichtung ein Material aufweist, das im allgemeinen gegen ein Herausspülen der therapeutischen Substanz durch sie hindurch beständig ist, wenn der Körperabschnitt in der ersten Position ist, wenn er in das Lumen (52 ) eingeführt wird, und wobei das zweite Beschichtungsmaterial (34 ) relativ unelastisch ist, so daß die zweite Beschichtung (34 ) während der Expansion des Körperabschnitts in die zweite Position bricht, um mehrere Risse in der zweiten Beschichtung zu bilden, um das Herausspülen der therapeutischen Substanz durch die Risse (40 ) zu ermöglichen. - Medizinische Vorrichtung nach Anspruch 1, wobei die Vorrichtung ein expandierbarer Stent ist.
- Medizinische Vorrichtung nach Anspruch 1, wobei die Vorrichtung ein Ballonkatheter ist.
- Medizinische Vorrichtung nach Anspruch 1, wobei die zweite Beschichtung eine Dicke von etwa 0,01 μm bis etwa 5,0 μm hat.
- Medizinische Vorrichtung nach Anspruch 1, wobei die medizinische Vorrichtung ein intravaskulärer Stent ist, der zur Behandlung einer Gefäßwand geeignet ist; und wobei die Lumenwand (
56 ) die Gefäßwand aufweist; und wobei der Körperabschnitt eine im allgemeinen röhrenförmige Struktur aufweist; und wobei die Außenfläche mehrere miteinander verbundene Streben mit Zwischenräumen (16 ) zwischen ihnen aufweist; und wobei das Expandieren der im allgemeinen röhrenförmigen Struktur durch Biegen und Knicken der miteinander verbundenen Streben gefördert wird; und wobei die zweite Beschichtung als Antwort auf das Biegen und Knicken der Streben bricht. - Vorrichtung nach Anspruch 1 oder 5, wobei die therapeutische Substanz als Antwort auf Fluidkontakt mit der ersten Beschichtung (

10 ;58 ) eine intravaskuläre medizinische Vorrichtung ist; und wobei die Lumenwand (52 ) eine Gefäßlumenwand ist. - Vorrichtung nach Anspruch 1, 5 oder 8, wobei die therapeutische Substanz aus der Gruppe gewählt ist, die aus Heparin, Antibiotika, strahlenundurchlässigen Wirkstoffen, thrombosehemmenden Wirkstoffen, wucherungshemmenden Wirkstoffe, gefäßbildungsverhindernden Wirkstoffen und Kombinationen daraus besteht.
- Vorrichtung nach Anspruch 9, wobei die wucherungshemmenden und gefäßbildungsverhindernden Wirkstoffe aus der Gruppe gewählt sind, die aus Taxol, Taxolderivaten, Colchicin, Vinblastin und Epothilon besteht.
- Vorrichtung nach Anspruch 1, 5 oder 8, wobei die erste Beschichtung (
32 ) ferner einen Polymerträger mit der in diesem fein verteilten therapeutischen Substanz aufweist. - Vorrichtung nach Anspruch 11, wobei der Polymerträger biologisch abbaubar ist.
- Vorrichtung nach Anspruch 12, wobei der Polymerträger aus der Gruppe gewählt ist, die aus Polymilchsäure, Polyglycolsäure, Polyethylenoxid, Polycaprolacton, Polydioxanonen, Polyanhydriden, Polyphosphazenen, Poly(orthoestern) und Gemischen und Copolymere daraus besteht.
- Medizinische Vorrichtung nach Anspruch 1, wobei die medizinische Vorrichtung ein Katheter (
58 ) mit einem sich in Längsrichtung erstreckenden Schaft (60 ) mit einem distalen Ende und einem proximalen Ende ist, wobei der Schaft (60 ) ein Aufblaslumen bildet; und wobei der Körperabschnitt ein Ballonteil (62 ) aufweist, das nahe dem distalen Ende des Schafts (60 ) in Fluidkommunikation mit dem Aufblaslumen angeordnet ist. - Vorrichtung nach Anspruch 1, 5, 8 oder 14, wobei die therapeutische Substanz sich als Antwort auf Fluidkontakt aus der Außenfläche herausspülen lassen würde, wenn die zweite Beschichtung (
34 ) örtlich begrenzt nicht vorhanden wäre. - Vorrichtung nach Anspruch 1 oder 14, wobei die zweite Beschichtung (
34 ) ein Polymermaterial aufweist, das aus der Gruppe gewählt ist, die aus Polymilchsäure, Polyglycolsäure, Polyanhydriden, Polyphosphazenen, Poly(orthoestern) und Gemischen und Copolymeren daraus besteht. - Vorrichtung nach Anspruch 14, wobei die therapeutische Substanz aus der Gruppe gewählt ist, die aus Taxol, Taxolderivaten, Colchicin, Vinblastin und Epothilone besteht.
- Vorrichtung nach Anspruch 1, 5, 8 oder 14, wobei die therapeutische Substanz in Kontakt mit wasserhaltigen Fluiden herausspülbar wäre, wenn die zweite Beschichtung (
34 ) örtlich begrenzt nicht vorhanden wäre.
