Description
- Gebiet der Erfindung
- Die vorliegende Erfindung betrifft Vorrichtungen zur Erleichterung der Knochenfusion. Die offenbarten Vorrichtungen können für die Fusion von Gelenken verwendet werden. Die Erfindung ist besonders vorteilhaft für die Fusion von Intervertebralgelenken.
- Hintergrund der Erfindung
- Chronische Rückenprobleme verursachen Schmerzen und Behinderung bei einem großen Anteil der Bevölkerung. Häufig kann die Ursache der Rückenschmerzen auf erkranktes Bandscheibenmaterial zwischen gegenüberliegenden Wirbeln zurückverfolgt werden. Wenn das Bandscheibenmaterial erkrankt ist, können die gegenüberliegenden Wirbel nicht ausreichend unterstützt werden, was zu einem anhaltenden Schmerz führt.
- Operationstechniken wurden entwickelt, um das erkrankte Bandscheibenmaterial zu entfernen und das Gelenk zwischen den gegenüberliegenden Wirbelkörpern zu versteifen. Stabilisierung und/oder Arthrodese des Intervertebralgelenkes kann den Schmerz reduzieren, der mit der Bewegung eines Intervertebralgelenkes verbunden ist, das erkranktes Bandscheibenmaterial hat. Im allgemeinen umfassen Fusionstechniken das Entfernen der erkrankten Bandscheibe und das Bepacken der leeren Fläche mit einer geeigneten Matrix zur Ermöglichung einer knochigen Einheit zwischen den gegenüberliegenden Wirbelkörpern.
- Es sind auch Operationsvorrichtungen zur Erleichterung der Zwischenkörperfusion entwickelt worden. Diese Vorrichtungen gewährleisten üblicherweise die Aufrechterhaltung eines geeigneten Intervertebralabstandes und die Stabilisierung der Wirbel während des Fusionsprozesses. Beispiele für solche Vorrichtungen sind zum Beispiel in den U.S. Patenten Nr. 5,458,638, 5,489,307, 5,055,104, 5,026,373, 5,015,247, 4,961,740, 4,743,256 und 4,501,269 offenbart.
- Gegenwärtige Verfahren zur Implantierung einer Fusionsvorrichtung erfordern oftmals, dass die Wirbel auseinandergezogen werden, um vor dem Implantieren einer Fusionsvorrichtung einen erkrankten Bandscheibenzwischenraum auf seine normale Höhe wiederherzustellen.
- Zusätzlich wird der Bandscheibenzwischenraum üblicherweise für die Aufnahme eines Implantates hergerichtet, indem eine Bohrung von geeigneter Größe zur Aufnahme des Implantates gebohrt und eingedreht wird. Demzufolge erfordern die gegenwärtigen Verfahren, die zur Einführung der derzeitig erhältlichen Fusionsimplantate verwendet werden, mehrere Schritte und spezialisierte Instrumentierung, um die Implantatstelle herzurichten.
- Die Zeit, die erforderlich ist, um die Schritte zur Vorbereitung der Implantatstelle vorzubereiten, verlängert die Dauer des operativen Verfahrens und erhöht damit die Zeitdauer, die der Patient unter allgemeiner Anästhesie ist. Weiterhin kann die verwendete Instrumentierung das Herstellen eines substantiellen Hautschnittes erfordern. Zusätzlich erhöhen die Schritte, die verwendet werden, um die Implantatstelle auseinander zuhalten und herzurichten, die Chance für ein Trauma des neuronalen, vaskulären und anderen Gewebes in der Nachbarschaft der Implantatstelle.
- US-A-5,290,312 offenbart ein Intervertebralimplantat, wie in der Präambel des Anspruchs 1 definiert, das ein äußeres Element umfaßt, welches einen Boden und beabstandete erste und zweite äußere Wände hat. Ein inneres Element ist vorgesehen, das beabstandete erste und zweite innere Wände hat, die innerhalb der äußeren Wände hineinpassen. Stifte sind vorgesehen, die von den äußeren Wänden hervorstehen, welche in die Öffnungen hineinpassen können, die in den inneren Wänden zur Fixierung des inneren Elementes relativ zu dem äußeren Element in einer ausgedehnten Position positioniert sind. In diesem bekannten Intervertebralimplantat müssen die Stifte in die Öffnungen hineingeschraubt werden, nachdem das Implantat in die ausgedehnte Position gebracht wurde. Die Stifte sind alle auf der gleichen Seite des Implantats positioniert.
- Dementsprechend gibt es einen andauernden Bedarf für verbesserte intervertebralstabilisierende Vorrichtungen. Die vorliegende Erfindung ist darauf ausgerichtet, diese Bedürfnisse anzugehen.
- Zusammenfassung der Erfindung
- Die vorliegende Erfindung bezieht sich auf ein Implantat zur Erleichterung der Knochenfusion. Die Erfindung kann vorteilhaft für die Stabilisierung und Fusion eines Gelenkes, beson ders eines Intervertebralgelenkes, verwendet werden. Die Erfindung hilft, die Schritte, die notwendig für die Vorbereitung der Implantatstelle sind, und die Zeit, die für die Durchführung des gesamten Implantationsverfahrens erforderlich, zu reduzieren. Die Erfindung reduziert auch die Wahrscheinlichkeit einer Verletzung an Geweben nahe der Operationsstelle.
- Es wird klar sein, dass in der gesamten Beschreibung eine Anleitung durch Listen von Beispielen gegeben werden kann. Die jeweils vorgetragene Liste dient nur als eine repräsentative Gruppe. Es ist jedoch nicht gemeint, dass die Listen ausschließend sind.
- Eine intervertebrale Fusionsvorrichtung gemäß der Erfindung kann eine ausgedehnte und eine nicht-ausgedehnte Konfiguration aufweisen. Die Fusionsvorrichtungen haben eine innere Oberfläche, eine äußere Oberfläche, ein vorderes Ende und ein hinteres Ende. Ein Teil der äußeren Oberflächen beinhaltet eine Eingriffsanordnung, um die Wahrscheinlichkeit einer Bewegung oder eines Ausstoßens der Fusionsvorrichtung, sobald sie innerhalb des Intervertebralraumes implantiert wurde, zu reduzieren. Das Innere der Fusionsvorrichtungen kann mit einer Knochenverstärkungsmatrix bepackt werden, um die Fusion zwischen den gegenüberliegenden Knochenoberflächen zu erleichtern.
- Die Fusionsvorrichtungen beinhalten jeweils ein äußeres Element und ein inneres Element. Ein Teil sowohl des äußeren als auch inneren Elements kann eine U-förmige Konfiguration aufweisen. Das äußere Element beinhaltet einen äußeren Boden und eine erste und zweite äußere Seitenwand, die durch eine Breite des äußeren Bodens beabstandet ist. Das innere Element beinhaltet auch einen inneren Boden und eine erste und zweite innere Wand, die mit einer geeigneten Distanz beabstandet ist, um zu garantieren, dass die ersten und zweiten inneren Wände innerhalb der ersten und zweiten äußeren Wände hineinpassen.
- Die Fusionsvorrichtungen beinhalten auch eine Arretierungsanordnung. Im allgemeinen beinhaltet eine Arretierungsanordnung ein Paar von Stiften, die mit einem Paar von Öffnungen ineinandergreifen. In einer Ausführungsform kann ein erster Stift von der ersten inneren Wand herausragen, und ein zweiter Stift kann von der zweiten inneren Wand herausragen. Die ersten und zweiten Stifte rasten durch Ineinandergreifen mit einer ersten und zweiten Öffnung, die jeweils auf den ersten und zweiten äußeren Wänden lokalisiert sind, ein.
- In einer anderen Ausführungsform kann ein erster Stift von der ersten äußeren Wand herausragen, und der zweite Stift kann von der zweiten äußeren Wand herausragen. Entsprechend dieser Ausführungsform greift der erste Stift, wenn er in der eingesetzten Position arretiert ist, mit einer ersten Öffnung, die auf der ersten inneren Wand lokalisiert ist, ineinander, und der zweite Stift greift mit einer zweiten Öffnung, die auf der zweiten inneren Wand lokalisiert ist, ineinander. Die Erfindung beschreibt verschiedene Umsetzungen der relativen Positionierung der Stifte und der Öffnungen, um das Arretieren der Fusionsvorrichtung in verschiedenen ausgedehnten Konfigurationen zu erlauben. In einigen Ausführungsformen beinhalten die Wände, in denen die Öffnungen lokalisiert sind, auch eine Nut, um die Stifte in die Öffnung zu führen.

- Die vorliegende Erfindung ist auf ein Implantat zur Erleichterung der Stabilisierung oder Fusion von Knochen gerichtet. Die Erfindung kann vorteilhaft bei der Stabilisierung und Fusion eines Gelenkes, besonders eines Intervertebralgelenkes, verwendet werden. Somit wird die Erfindung mit Bezugnahme auf die Stabilisierung und Fusion von benachbarten Lendenwirbeln beschrieben. Es wird jedoch klar sein, dass die offenbarten Vorrichtungen zur Verwendung mit allen Arten von Gelenken (z.B. Knöchel, intervertebral, interdigital, usw.) und in allen Regionen (z.B. Arme, Beine, Rückgrat, usw.) eines menschlichen oder tierischen Körpers geeignet sind. In der Wirbelsäule können die offenbarten Vorrichtungen für alle intervertebralen Gelenke verwendet werden, einschließlich jener in der zervikalen, thorakalen und Lendenregion.
- Bei Verwendung kann eine Fusionsvorrichtung der Erfindung in einen Gelenkzwischenraum in einer engen gepackten Konfiguration inseriert werden und dann in eine ausgedehnte Konfiguration entfaltet werden und in der ausgedehnten Konfiguration arretiert werden, um den Gelenkzwischenraum zu stabilisieren. In bevorzugten Ausführungsformen können die Vorrichtungen Öffnungen für Knochenwachstum durch die Vorrichtung beinhalten, um das Gelenk zu versteifen („fuse").
- Entsprechend der Erfindung beinhaltet eine Fusionsvorrichtung (Implantat) einen Körper, der eine äußere Region und eine innere Region hat. Im allgemeinen greifen Teile der äußeren Region in die Knochen ein, die benachbart zu dem Gelenkzwischenraum liegen und versteift werden sollen. Sobald die Vorrichtung zwischen den Knochen inseriert ist und entfaltet ist, stabilisiert sie das Gelenk, indem sie eine nach außen gerichtete Kraft gegen die Knochen ausübt, die die kontrahierende (nach innen gerichtete) Kraft der Weichgewebestrukturen, die das Gelenk umgeben, ausgleicht. Die innere Region der Fusionsvorrichtung definiert einen inneren Zwischenraum oder eine Kammer, die mit einer Knochenwachstumsmatrix gefüllt werden kann, um das Knochenwachstum über die Fusionsstelle hinaus zu erleichtern.
- Der Körper einer Fusionsvorrichtung umfasst mindestens ein erstes und zweites Element, von denen eines innerhalb des anderen in einer engen gepackten Konfiguration ineinander gesteckt ist, wenn sich die Versteifungsvorrichtung in dem nicht-entfalteten Zustand befindet. Ein Teil von jedem Element beinhaltet eine im allgemeinen U-förmige Konfiguration im Querschnitt, unabhängig, ob das Element offen oder geschlossen (z.B. tubulär) ist. Die U-Form kann in der Form eines Rechteckes, zirkulär oder oval, usw. sein.
- Jedes Element beinhaltet gegenüberliegende Seitenwände, die durch einen Boden beabstandet sind. Die Breite der Böden jedes Elementes bestimmt die Gesamtbreite der Vorrichtung. Die Höhe der Wände und die Position der Arretierungsanordnung bestimmt die Gesamthöhe und die Winkelanordnung („angulation") der Vorrichtung in der entfalteten (ausgedehnten) Konfiguration.
- Sobald es in einen Gelenkszwischenraum inseriert wurde, kann das Implantat entfaltet werden und die Elemente in einer ausgedehnten Konfiguration arretiert werden. Eine Arretierungsanordnung zum Arretieren der Vorrichtung beinhaltet vorzugsweise einen Arretierstift und ein Arretierstiftloch zur Aufnahme des Arretierstiftes. Die transversale Querschnittskonfiguration des Arretierstiftes kann jegliche Form annehmen, einschließlich zirkulärer, quadratischer, rechteckiger, polygonaler, usw. Die longitudinale Querschnittskonfiguration des Stiftes kann parallel, konisch, usw. sein. Die Querschnittskonfiguration des Arretierstiftloches ist so ausgewählt, um den Arretierstift kooperativ aufzunehmen.
- Bei Verwendung werden die Implantate üblicherweise in einen Bandscheibenzwischenraum in einer kompakten (nicht-ausgedehnten) Konfiguration zwischen benachbarten Wirbeln inseriert. Sobald das Implantat in den Wirbelzwischenraum inseriert worden ist, kann es entfaltet werden und in eine ausgedehnte Konfiguration arretiert werden, um eine erwünschte Höhe des Bandscheibenzwischenraumes zwischen den Wirbeln zu bilden. Wie hier verwendet, be zieht sich eine "erwünschte" Höhe des Bandscheibenzwischenraumes auf die Distanz zwischen den Wirbeln, die als geeignet für den individuellen Zustand des Patienten bestimmt worden ist. Abhängig von dem Zustand kann die erwünschte Höhe die von dem normalen Bandscheibenzwischenraum in einem nicht-erkrankten Zustand sein, oder der Bandscheibenzwischenraum kann größer sein als die normale Bandscheibenzwischenraumhöhe oder geringer als normal. Ein Beispiel für den Bereich der Gesamthöhendimension eines Implantates der Erfindung für die intervertebrale Verwendung, wenn in der kontraktierten Konfiguration, kann etwa 2 bis 10 mm sein. Ein Beispiel für den Bereich der Gesamthöhendimension des gleichen Implantates in der ausgedehnten Konfiguration ist etwa 4 bis 20 mm.
- Im allgemeinen kann die Gesamthöhe der Fusionsvorrichtungen variieren, indem die Höhe jedes Elementes des Körpers variiert wird. Zusätzlich kann in einigen Ausführungsformen eine Vorrichtung der Erfindung eine Verjüngung beinhalten, um einen erwünschten Grad von Abknickung zwischen den benachbarten Knochen zur Verfügung zu stellen. Im Fall eines Intervertebralgelenkes kann der vorgesehene Winkel ungefähr 0° bis 25° sein.
- Eine oder mehrere Fusionsvorrichtungen können in einen einzelnen Gelenkzwischenraum implantiert sein. In dem Falle eines Intervertebralgelenkes kann die Anzahl der Vorrichtungen und die Längen- und Breitendimensionen der Vorrichtungen vorzugsweise derart ausgewählt sein, dass die Vorrichtung oder die Vorrichtungen innerhalb des Spielraumes der benachbarten Vertebralkörper hineinpassen und funktionieren. Beispiele für die Längen- und Breitenbereiche der Vorrichtungen der Erfindung sind etwa 20 bis 50 mm Länge und 5 bis 50 mm Breite.
- Die Fusionsvorrichtungen haben eine "vorderes Ende" und ein "hinteres Ende". Die Begriffe "vorderes Ende" und "hinteres Ende" sind relative Begriffe, die anzeigen, dass in der üblichen Verwendung das vordere Ende des Implantates in den Gelenkzwischenraum inseriert wird, gefolgt von dem hinteren Ende. Auf die Vorrichtung kann auch mit Begriffen, wie einem "distalen Ende" und einem "proximalen Ende", basierend auf der Orientierung des Implantates relativ zu dem Chirurg, Bezug genommen werden. Die Fusionsvorrichtungen können in einen intervertebralen Bandscheibenzwischenraum in den meisten bekannten Weisen, einschließlich anterior, posterior, lateral, usw., eingeführt werden.
- Ein Teil der äußeren Region von jedem der Elemente kann eine biologische Oberfläche oder ein biokompatibles Material beinhalten, das Oberflächen hat, die sich mit den Oberflächen von Knochen oder Knochenendplatten, die den Gelenkzwischenraum umgeben, einrichten. Geeignete, sich einrichtende Oberflächen sind üblicherweise konturiert und beinhalten zum Beispiel eine poröse Knocheneinwuchsoberfläche, Spitzen, Knoten, Kanten, Gewinde, oder andere ähnliche Arrangements, um die Stabilisierung des Implantats in dem Gelenkzwischenraum zu erleichtern. In dem Fall, dass eine Vorrichtung einen zirkulären Querschnitt hat, können helikale Gewinde für die Insertion des Implantats in einen angebohrten oder nicht-angebohrten Bandscheibenzwischenraum zur Verfügung gestellt werden.
- Geeignete Materialien zur Herstellung einer Fusionsvorrichtung der Erfindung beinhalten Metalle, wie Titan, Edelstahl, Kobalt-Chrom, usw.; Legierungen, einschließlich Titanlegierungen und Nicht-Titanlegierungen; superelastische Materialien, wie zum Beispiel Nitinol; Kunststoffe und Kunststoffverbundstoffe; Kohlenstoffgraphit; Keramik; usw. In einigen Ausführungsformen kann das erste und/oder zweite Element gebeugt sein, wenn es in der engen gepackten Position ist, oder das Element kann gebeugt sein, wenn es in der ausgedehnten Konfiguration entfaltet ist. In diesen Ausführungsformen hat das gebeugte Element vorzugsweise elastische Eigenschaften, die die Beugung des Elementes innerhalb seiner Materialgrenzen ohne signifikante Beeinflussung der strukturellen Integrität des Elementes erlauben.
- Vor der Insertion kann eine Diskektomie (Bandscheibenentfernung) durchgeführt werden und vorzugsweise das Ringband (Ligamentum annulare) erhalten bleiben. Die Endplatten des Knochens können geschabt, ausgekratzt oder ein ähnliches Verfahren durchgeführt werden, um eine offene Endoberfläche zur Erleichterung des Knochenwachstums über die Fusionsstelle hinaus zu schaffen. Mit der Vorrichtung in der engen gepackten Konfiguration kann dann das vordere Ende in den Bandscheibenzwischenraum inseriert werden, gefolgt von dem hinteren Ende. In einigen Fällen kann es vorteilhaft sein, die benachbarten Wirbel vor der Insertion einer Fusionsvorrichtung auseinanderzuhalten. Ein solches Auseinanderhalten kann das leichtere Entfernen des Bandscheibenmaterials und/oder größeres Aussetzen gewährleisten, um die Herrichtung der Endplatten zu erleichtern. Das Auseinanderhalten kann auch eine größere Genauigkeit bei der Bestimmung der geeigneten Größe der Fusionsvorrichtung, die implantiert werden soll, gewährleisten.
- Sobald die Vorrichtung an der geeigneten Stelle inseriert wurde, kann sie entfaltet werden. Wenn mehr als ein Implantat an einem besonderen Gelenkzwischenraum verwendet wird, kann jedes Implantat innerhalb des Gelenkzwischenraumes vor der Entfaltung positioniert werden. Alternativ können individuelle Implantate inseriert werden und vor der Insertion von einem nachfolgenden Implantat in dem gleichen Gelenkzwischenraum entfaltet werden.
- Wenn eine zylindrische Fusionsvorrichtung verwendet wird, kann es vorteilhaft sein, erst eine Bohrung in den Bandscheibenzwischenraum von einer Größe, die ausreicht, um die Vorrichtung vor der Insertion aufzunehmen, zu bohren. In einigen Ausführungsformen kann die Bohrung auch für Führungsgewinde, die auf der Oberfläche der Vorrichtung anwesend sein können, eingewunden sein.
- Die Fusionsvorrichtungen können in der ausgedehnten Konfiguration entfaltet werden und in dieser Konfiguration arretiert werden, unter Verwendung zum Beispiel einer reversen Zange, einem Spreizer (Abstandshalter), Wundhaken, usw. Sobald entfaltet, kann die innere Kammer der Fusionsvorrichtung und/oder der freien Gebiete, die die Vorrichtung(en) umgeben, mit einer Knochenunterstützungsmatrix bepackt werden, um das Knochenwachstum in dem Gelenkzwischenraum zu erleichtern. Wie hier verwendet, ist eine "Knochenunterstützungsmatrix" ein Material, das neues Knochenwachstum zwischen den einander gegenüberliegenden Wirbelkörpern erleichtert. Geeignete Knochenunterstützungsmatrizen können resorbierbar oder nicht-resorbierbar sein und osteokonduktiv oder osteoinduktiv. Beispiele von geeigneten Matrizen entsprechend der Erfindung beinhalten synthetische Materialien, wie zum Beispiel HealousTM, erhältlich von Orquest, Mountain View, California; NeOsteoTM, erhältlich von Sulzer Orthopedic Biologics, Denver, Colorado; oder jedes von einer Vielzahl knochenmorphogenetischer Proteine (BMPs). Geeignete Knochenunterstützungsmatrizen beinhalten auch heterologe (Xenotransplantat), homologe (Allotransplantat) oder autologe (Autotransplantat) Knochen und Derivate davon. Die Knochenunterstützungsmatrix ist vorzugsweise für Röntgenstrahlen strahlendurchlässig.
- Die Fusionsvorrichtungen der Erfindung können in einem Kit beinhaltet sein, der eine Vielzahl von Implantaten mit anwachsender Größe umfaßt, die von dem Kliniker, basierend auf der Größe, die für einen bestimmten Patienten benötigt wird, ausgewählt werden kann. In anderen Ausführungsformen können die Kits Instrumentierung zur Durchführung eines Im plantatverfahrens mit oder ohne einer Vielzahl von Implantaten mit anwachsender Größe beinhalten.
- Die Fusionsvorrichtungen der Erfindung werden nun durch Bezugnahme auf die folgenden dargestellten Ausführungsformen weiter beschrieben. Die dargestellten Ausführungsformen sollen nicht den Umfang der Erfindung begrenzen, sondern eher zur Erleichterung des Verständnisses der Vorrichtungen innerhalb der Prinzipen der Erfindung dargestellt werden.
- Detaillierte Beschreibung der illustrierten Ausführungsformen
- Unter Bezugnahme auf die verschiedenen gezeichneten Abbildungen wird eine Beschreibung der Ausführungsformen einer ausdehnbaren Fusionsvorrichtung gemäß der vorliegenden Erfindung nun bereitgestellt werden. Die folgende Diskussion beschreibt eine Fusionsvorrichtung der Erfindung unter Bezugnahme auf das Implantieren der Vorrichtung zwischen benachbarte Lendenwirbel. Diese Beschreibung dient jedoch nur zu Erklärungszwecken. Wie oben gesagt, können die hier offenbarten Vorrichtungen bei anderen Gelenken oder anderen Stellen des Rückgrats verwendet werden, um die Knochenfusion zu erleichtern.

1 ist eine perspektivische Sicht einer Ausführungsform einer Fusionsvorrichtung10 entsprechend der Erfindung in einer ausgedehnten Konfiguration. Die Fusionsvorrichtung10 beinhaltet ein erstes Element1 und ein zweites Element2 . Wie illustriert, beinhaltet das erste Element1 eine erste Seitenwand3 und eine zweite Seitenwand4 , beabstandet durch einen Boden5 . Das zweite Element2 beinhaltet auch eine erste Seitenwand6 und eine zweite Seitenwand7 , beabstandet durch den Boden8 . In der dargestellten Ausführungsform passen die äußere Oberfläche9a der ersten Seitenwand3 und die äußere Oberfläche9b der zweiten Seitenwand4 des ersten Elementes1 jeweils in die inneren Oberflächen11a und11b der ersten Seitenwand6 und zweiten Seitenwand7 des zweiten Elementes2 . Da die erste Seitenwand6 und zweite Seitenwand7 des zweiten Elementes2 "äußerlich" zu der ersten Seitenwand3 und der zweiten Seitenwand4 des ersten Elementes1 sind, kann daher das zweite Element2 auch als äußeres Element14 und das erste Element1 als inneres Element13 der Fusionsvorrichtung10 bezeichnet werden. In dieser Ausführungsform ist das vordere Ende30 der Fusionsvorrichtung10 identisch mit dem hinteren Ende31 . -
2 ist eine transversale Querschnittsansicht durch die Linie 2-2 der1 . Wie dargestellt, hat der Querschnitt des inneren Elementes13 und des äußeren Elementes14 ein "U"-förmiges Aussehen. Die U-Form der Fusionsvorrichtung10 in1 ist im wesentlichen rechteckig. Das bedeutet, dass zum Beispiel die Seitenwände3 und4 jeweils einen rechten Winkel mit dem Boden5 bilden. Die Form einer Fusionsvorrichtung10 jedoch ist nicht begrenzt auf einen rechteckigen Querschnitt, andere Formen von Fusionsvorrichtungen sind in dem Umfang der Erfindung miteinbezogen. Zum Beispiel hat die Fusionsvorrichtung100 , dargestellt in3 , eine im wesentlichen kreisförmig transversale Querschnittserscheinung. Entsprechend der Ausführungsform von3 beinhaltet das erste Element101 Seitenwände103 und104 , die sich im Scheitelbereich105 treffen. Das zweite Element102 beinhaltet die Seitenwände106 und107 , die sich im Scheitelbereich108 treffen. Die Fusionsvorrichtung100 kann innerhalb einer zylindrischen Bohrung implantiert werden, die zwischen benachbarten Wirbeln vorbereitet worden ist, wie unten beschrieben werden wird. Sobald die Fusionsvorrichtung100 implantiert wurde, wird sie mit den Endplatten der gegenüberliegenden Wirbel an den Scheitelbereichen (105 ,108 ) und entlang eines Teiles von jeder der Seitenwände (103 ,104 ,106 und107 ) in Kontakt kommen. -
4 ist eine Endansicht einer anderen Ausführungsform einer Fusionsvorrichtung150 , die ein inneres Element151 und ein äußeres Element152 umfaßt. In dieser Ausführungsform ist das innere Element151 geschlossen oder hat eine tubuläre Konfiguration153 . -
5 ist eine Seitenansicht einer Fusionsvorrichtung10 , die zwischen einander gegenüberliegenden Wirbeln eingefügt wurde. Bei der Verwendung wird die Fusionsvorrichtung10 in den Bandscheibenzwischenraum20 zwischen die Endplatten21 und22 der einander gegenüberliegenden Wirbeln23 und24 eingefügt. In5 wird die Fusionsvorrichtung10 in den Bandscheibenzwischenraum20 in der Orientierung, dargestellt in1 , eingefügt. Daher ist der Boden5 des inneren Elementes13 positioniert, um die Endplatte21 zu kontaktieren, und der Boden8 des äußeren Elementes14 ist positioniert, um Endplatte22 zu kontaktieren. Es wird jedoch klar sein, dass die Fusionsvorrichtung10 auch in einen Bandscheibenzwischenraum20 inseriert werden könnte, der 180° um die lange Achse AL rotiert wurde, unabhängig von dem Ansatz, der für die Insertion verwendet wurde. - Wie in
1 dargestellt, können die Böden5 und6 , die die Endplatten der benachbarten Wirbel kontaktieren, die Öffnungen15 beinhalten, wie für Boden5 dargestellt, oder keine Öffnungen beinhalten, wie für Boden8 dargestellt. Das Bereitstellen der Öffnungen15 in Böden5 und8 begünstigt vorteilhaft eine größere Kontinuität des neuen Knochenwachstums innerhalb der Fusionsvorrichtung10 . - Unter erneuter Bezugnahme auf
1 und2 sind das vordere Ende30 und das hintere Ende31 der Fusionsvorrichtung10 identisch. Daher ist die Höhe (H1) der Seitenwände6 und7 des äußeren Elementes14 , gemessen von dem äußeren Boden32 des Bodens8 , zu einer der äußeren Kanten33a oder33b , identisch an dem vorderen Ende und hinteren Ende31 der Vorrichtung. Ebenso ist die Höhe (H2) der Seitenwände des inneren Elementes13 , gemessen von dem äußeren Boden35 des Bodens5 , zu einer der inneren Kanten36a oder36b , die gleiche an dem vorderen und hinteren Ende. In einigen Ausführungsformen kann die Gesamthöhe (H0), gemessen von dem Äußeren35 des Bodens5 zu dem Äußeren32 des Bodens8 , unterschiedlich sein aufgrund von: einem Unterschied in der Höhe (H1) der inneren Seitenwand (3 ,4 ) an dem vorderen Ende und hinteren Ende, einem Unterschied in der Höhe (H2) der äußeren Seitenwand (6 ,7 ) an dem vorderen und hinteren Ende; oder einem Unterschied in der Position der Komponenten der Arretierungsanordnung (oben diskutiert). -
6 stellt eine alternative Ausführungsform einer Fusionsvorrichtung200 vor, einschließlich einem inneren Element201 und einem äußeren Element202 . In dieser Ausführungsform beinhaltet der Boden203 des inneren Elementes201 eine Mehrzahl von Öffnungen204 . Zusätzlich beinhaltet der Boden205 des äußeren Elementes202 auch Öffnungen206 . In alternativen Ausführungsformen können die Böden keine, eine, zwei, drei oder mehrere Öffnungen beinhalten. Zusätzlich beinhalten die äußeren Oberflächen210 und211 der Böden203 bzw.205 eine Eingriffsvorrichtung212 , um das Eingreifen der Fusionsvorrichtung200 in die Endplatten der Wirbel zu erleichtern. Die Verwendung einer Eingriffsvorrichtung212 reduziert die Wahrscheinlichkeit der Wanderung oder des Ausstoßens der Vorrichtung nach Implantation. In6 beinhaltet die Eingriffsvorrichtung212 eine Mehrzahl von Spitzen215 (spikes) entlang der äußeren Bodenoberflächen210 und211 . Alternative Eingriffsvorrichtungen212 können auch verwendet werden, einschließlich zum Beispiel poröser beschichteter äußerer Wachstum-Oberflächen, Knoten, Kanten, Gewinde, usw. - Die Verwendung der Fusionsvorrichtung entsprechend der Erfindung wird nun beschrieben.
7 ist eine transversale Querschnittsansicht durch die Fusionsvorrichtung10 an der Linie 7-7 der1 in einer nicht-ausgedehnten Konfiguration. Wie dargestellt, erstrecken sich die Öffnungen50a und50b durch die Seitenwände6 und7 , nahe den äußeren Kanten33a und33b des äußeren Elementes14 . Öffnungen50a und50b haben Größen zur Aufnahme der Stifte51a und51b , die in der Nähe der inneren Enden36a bzw.36b der Seitenwände3 und4 des inneren Elementes13 positioniert sind. Daher sind die inneren Enden36a und36b , wenn in der nicht-ausgedehnten Position, in der Nähe oder an dem Boden8 positioniert. Auch wenn die Fusionsvorrichtung10 in der nicht-entfalteten Konfiguration ist, sind die Seitenwände3 und4 des inneren Elements13 gegeneinander gebeugt, innerhalb der elastischen Grenzen des Materials des inneren Elementes13 . - Nach der Insertion in den intervertebralen Zwischenraum kann die Fusionsvorrichtung
10 in die ausgedehnte Position entfaltet werden, unter Verwendung einer reversen Zange (z.B. Inge Retractor, KMedic), Spreizer oder Wundhaken, um die Böden5 und6 voneinander weg in die entfaltete Konfiguration in8 zu forcieren. Wie dargestellt, greifen die Stifte51a und51b mit den Öffnungen50a bzw.50b ineinander, um die Fusionsvorrichtung10 in der ausgedehnten Position zu arretieren. - Unter Bezugnahme auf
6 ,9 und10 beinhaltet die Fusionsvorrichtung200 eine alternative Ausführungsform einer Arretierungsanordnung220 zur Arretierung der Fusionsvorrichtung200 in der entfalteten Konfiguration. Entsprechend dieser Ausführungsform beinhaltet das äußere Element202 die Fugen221a (nicht sichtbar) und221b entlang der inneren Oberfläche der Seitenwände208 bzw.209 zur Führung der Stifte222a (nicht sichtbar) und222b in die Öffnungen223a bzw.223b . -
9 ist eine Längsschnittansicht durch die Linie 9-9 des äußeren Elementes202 . Wie dargestellt, ist eine Arretierungsanordnung220 an dem vorderen Ende230 und hinteren Ende231 der Fusionsvorrichtung100 vorhanden. Bei Betrachtung des vorderen Endes230 ist die Öffnung223b in der Nähe der äußeren Kante232b des äußeren Elementes202 positioniert. Die Nut221b erstreckt sich von der Nähe des Bodens205 zur Öffnung223b .10 ist die Ansicht eines Schnittes durch die Linie 10-10 der9 , die zeigt, dass die Nut221b sich von der Nähe des Bodens205 zur Öffnung223b erstreckt. In dem nicht-entfalteten Stadium sind in dieser Ausführungsform die Seitenwände233 und234 des inneren Elementes201 gegen die Längsachse AL der Fusionsvorrichtung200 gebeugt. - Es wird klar sein, daß, während die Ausführungsform der Fusionsvorrichtung
200 der6 ,9 und10 verdeutlicht, dass die Öffnung223b die Wandstärke209 vom Inneren209a zum Äußeren209b durchläuft, die Tiefe der Öffnungen, wie zum Beispiel223b , nicht die gesamte Wandstärke durchlaufen muss, aber nur genügend tief sein muss, um die Arretierungsfunktion der Arretierungsanordnung bereitzustellen. - Unter Bezugnahme auf
11 wird eine alternative Ausführungsform einer Arretierungsanordnung320 für die Fusionsvorrichtung300 dargestellt.11 ist eine Ansicht von der gleichen Stelle einer Fusionsvorrichtung wie der von10 . In dieser Ausführungsform beinhaltet die Seitenwand301 eines inneren Elementes302 der Fusionsvorrichtung300 einen Stift304 nahe der inneren Kante305 der Seitenwand301 . Seitenwand310 des äußeren Elementes311 beinhaltet eine Nut312 , die zur Öffnung313 nahe der äußeren Kante314 führt. Jedoch hat entsprechend dieser Ausführungsform die Nut312 einen rampenförmigen Verlauf315 . Das bedeutet, dass die Nut312 sich tiefer in die Wandstärke310 erstreckt (von der inneren Oberfläche310a zu der äußeren Oberfläche310b ) nahe des Bodens314 , als sie das in der Nähe der Öffnung313 tut. Daher wird in einer bevorzugten Form dieser Ausführungsform die Seitenwand301 nur nach innen auf die gegenüberliegende innere Wand während der Entfaltung gebeugt, sobald das äußere Element311 und das innere Element302 getrennt werden und der Stift304 sich von einer Position nahe des Bodens311 zur Öffnung313 bewegt. - Während die vorhergehenden Ausführungsformen eine Fusionsvorrichtung beschreiben, die Stifte hat, die von der inneren Wand und den Öffnungen (und den Nuten) in der äußeren Wand herausragen, können die Stifte auch nach innen von den äußeren Wänden hineinragen, um mit den Öffnungen in den inneren Wänden ineinanderzugreifen. Während die vorhergehende Diskussion der Fusionsvorrichtungen die Beugung von inneren Wänden, abhängig von dem Material der Vorrichtung, beschreibt, kann eine gewisse elastische Beugung der Wände von sowohl den inneren als auch den äußeren Elementen auftreten, ohne wesentlich die Funktion der Vorrichtungen der Erfindung zu beeinflussen.
- In einer anderen Ausführungsform kann eine Fusionsvorrichtung entsprechend der Erfindung zur Schaffung und Beibehaltung eines erwünschten Grades von Lordose zwischen den benachbarten Wirbeln konfiguriert sein.
12 ist eine Seitenansicht einer Ausführungsform von solch einer lordotischen Fusionsvorrichtung. Wie dargestellt, beinhaltet die Vorrichtung400 einen divergenten Winkel α, der sich von dem vorderen Ende401 zu dem hinteren Ende402 zuspitzt. In dieser Ausführungsform ist das äußere Element403 oberhalb des inneren Elementes404 positioniert. Die Seitenwand405 des äußeren Elementes403 beinhaltet parallele Oberflächen entlang der äußeren Kante406 und des Bodens407 . Im Gegensatz dazu ist der Boden408 des inneren Elementes404 nicht parallel mit der inneren Kante409 . Arretierungsanordnungen410 können in der Gestalt von jeder der Ausführungsformen sein, die oben beschrieben wurden. -
13 stellt eine Seitenansicht einer alternativen Ausführungsform einer lordotischen Fusionsvorrichtung500 dar. Entsprechend dieser Ausführungsform gibt es keine Verjüngung in der nicht-ausgedehnten Konfiguration, vom vorderen Ende501 zum hinteren Ende502 , wenn die Fusionsvorrichtung500 in der nicht-ausgedehnten Konfiguration ist. Jedoch, wie in13 gezeigt, sind die Öffnungen503 und504 des äußeren Elementes505 nicht in einem gleichen Abstand vom Boden506 positioniert. Statt dessen ist die Öffnung503 näher zum Boden506 positioniert als die Öffnung504 . Stifte507 und508 (Schraffur) des inneren Elementes510 sind im gleichen Abstand vom Boden511 lokalisiert. Daher kann die Fusionsvorrichtung500 , wenn sie entfaltet ist, wie in14 dargestellt, in einer Position zu einer Konfiguration arretiert werden, die eine lordotische Verjüngung eines Winkels α vorsieht. Verschiedene relative Positionen der Öffnungen503 und504 können verwendet werden, um unterschiedliche Vorrichtungen zu schaffen, die verschiedene Winkelgrade haben. Alternativ können die Öffnungspositionen fixiert bleiben, und die Position der Stifte507 und508 kann variiert werden, um eine lordotische Fusionsvorrichtung bereitzustellen, ähnlich der, die für die Fusionsvorrichtung500 beschrieben wurde. - Bei der Verwendung kann eine einzelne Fusionsvorrichtung in den Bandscheibenzwischenraum zwischen benachbarten Wirbeln inseriert werden. Alternativ können zwei Fusionsvorrichtungen parallel zueinander implantiert werden, wie es in US Patent Nr. 5,489,307, zum Beispiel für nicht-ausdehnbare Fusionsvorrichtungen, offenbart worden ist.
- Der Bandscheibenzwischenraum, der versteift werden soll, kann identifiziert werden, und ein chirurgischer Ansatz kann zu der Bandscheibe unter Verwendung von Methoden, die im Stand der Technik bekannt sind, unternommen werden. Sobald der Bandscheibenzwischenraum freigelegt worden ist, kann eine Diskektomie (Bandscheibenentfernung) durchgeführt werden. Ein Teil der Endplatten kann entfernt werden, um blutende Kanten zu schaffen, um die Fusion zu erleichtern. Wenn ein Ansatz von vorne für die Implantierung zweier Fusions vorrichtungen verwendet wird, wird eine erste Fusionsvorrichtung zwischen die benachbarten Wirbel auf einer ersten Seite der Mittellinie des Bandscheibenzwischenraumes inseriert. Die erste Fusionsvorrichtung kann, sobald sie in Position ist, entfaltet werden. Die zweite Fusionsvorrichtung kann dann auf die zweite Seite der Mittellinie platziert werden und nachfolgend entfaltet werden. Alternativ können die erste und zweite Fusionsvorrichtung zuerst in den intervertebralen Bandscheibenzwischenraum platziert werden und nachfolgend entfaltet werden. Wenn die Gewebe, die das Gelenk umgeben, die volle Entfaltung einer ausgewählten Fusionsvorrichtung zum Arretieren der Arretierungsanordnung verhindern, kann es notwendig sein, eine alternative Fusionsvorrichtung, die eine kürzere Gesamthöhe H0 hat, zu verwenden. Alternativ kann eine größere Tiefe der Knochenenden abgetragen werden, um die Gesamthöhe (H0) der ursprünglich ausgewählten Fusionsvorrichtung anzupassen.

15 ist eine Diagrammdarstellung einer Vorderansicht von zwei Fusionsvorrichtungen10 entsprechend der Erfindung in einer ausgedehnten Konfiguration zwischen benachbarten Wirbeln23 und24 . Gemäß dieser Ausführungsform wird vorzugsweise die Breite (W) jeder Fusionsvorrichtung10 so ausgewählt, dass die Fusionsvorrichtungen in dem Bandschreibenzwischenraum komplett innerhalb der Begrenzungen der Wirbel23 und24 sind. -
16 ist eine Diagrammdarstellung einer Vorderansicht einer einzelnen Fusionsvorrichtung10 in dem Bandscheibenzwischenraum zwischen benachbarten Wirbeln23 und24 . Entsprechend dieser Ausführungsform ist die Breite (W) der Fusionsvorrichtung10 von ausreichender Größe, um Stabilität über einen Hauptanteil der Gelenkoberflächen der Wirbel23 und24 zur Verfügung zu stellen, vorzugsweise ohne über die Begrenzungen der Wirbel hinauszugehen. - Wie vorher angegeben, können die hier offenbarten Fusionsvorrichtungen mit Ansätzen, die im Stand der Technik bekannt sind, einschließlich anterior, posterior, lateral, usw., implantiert werden. Implantate verschiedener Größen werden erhältlich sein, die in einem Größenbereich von 5 bis 50 mm Breite liegen. Da eine Fusionsvorrichtung(en) der Erfindung eine enge gepackte Konfiguration annehmen kann (können), erfordern die Vorrichtungen der Erfindung einen kleineren Hauteinschnitt und verringern die Wahrscheinlichkeit eines Traumas neuraler und/oder vaskulärer Strukturen während einer chirurgischen Implantation. Zusätzlich kann die Zeit, die zur Durchführung eines Implantatverfahrens erforderlich ist, reduziert werden, da Aufweitungsprozeduren vor der Implementierung optional sind.
- Wenn eine Fusionsvorrichtung verwendet wird, die einen zirkulären Querschnitt hat, wie in
3 gezeigt, wird es voraussichtlich nötig sein, eine zylindrische Bohrung zwischen benachbarten Wirbeln bei dem betroffenen Bandscheibenzwischenraum zu schaffen. Verfahren zur Herstellung zylindrischer Bohrungen sind bekannt. Jedoch wird im Gegensatz zu einigen Verfahren zur Herstellung einer zylindrischen Bohrung zwischen einander gegenüberliegenden Wirbeln, wie zum Beispiel im US Patent Nr. 5,489,307 offenbart, keine Aufweitung (distraction) vor dem Herstellen der Bohrung benötigt. - Nachdem Fusionsvorrichtungen eingeführt worden sind, können der Intervertebralzwischenraum, der nicht von den Fusionsvorrichtungen besetzt ist, und das Innere des Körpers der Fusionsvorrichtungen mit einer Knochenunterstützungsmatrix, wie oben beschrieben, gefüllt werden.
- Aus der vorangegangenen detaillierten Beschreibung und den Beispielen wird deutlich sein, dass Veränderungen und Variationen in den Vorrichtungen der Erfindung gemacht werden können, ohne vom Umfang der Erfindung abzuweichen. Daher ist es beabsichtigt, dass alle Veränderungen und Verifizierungen in den Umfang der Ansprüche fallen.
Claims (15)
- Intervertebral-Implantat, das eine ausgedehnte und nicht-ausgedehnte Konfiguration aufweist, wobei das Implantat eine längsverlaufende Achse aufweist, die sich von einem vorderen Ende (
30 ) des Implantats zu einem hinteren Ende (31 ) des Implantats erstreckt, wobei das Implantat umfasst: – ein äußeres Element (14 ), das eine erste äußere Wand (6 ), beabstandet von einer zweiten äußeren Wand (7 ) einschließt; – ein inneres Element (13 ), das einschließt: – eine erste innere Wand (3 ), beabstandet von einer zweiten inneren Wand (4 ), so dass die ersten und zweiten inneren Wände (3 ,4 ) innerhalb der ersten und zweiten äußeren Wände (6 ,7 ) angeordnet sind; – eine Arretierungsanordnung zum Arretieren des Implantats in der ausgedehnten Konfiguration, umfassend: – einen ersten Stift (51a ), der aus einer ausgewählten jeder der Wände (6 ,7 ) des äußeren Elements (14 ) und jeder der Wände (3 ,4 ) des inneren Elements vorsteht; – eine erste Öffnung (50a ), positioniert in einer ausgewählten jeder der Wände der äußeren Elemente und jeder der Wände der inneren Elemente; – so dass die ersten Stifte von den ersten Öffnungen aufnehmbar sind; dadurch gekennzeichnet, dass – ein zweiter Stift (51b ) zur Verfügung gestellt wird, der von einer ausgewählten jeder der Wände (6 ,7 ) des äußeren Elementes und jeder der Wände (3 ,4 ) des inneren Elements vorsteht; – eine zweite Öffnung (50b ), positioniert in einer ausgewählten jeder der Wände der äußeren Elemente und jeder der Wände der inneren Elemente, so dass die zweiten Stifte von den zweiten Öffnungen aufnehmbar sind; – wobei die ersten Stifte und die ersten Öffnungen benachbart zu dem vorderen Ende (30 ) des Implantats positioniert sind und die zweiten Stifte und die zweiten Öffnungen benachbart zu dem hinteren Ende (31 ) sind; und – wobei, wenn in der nicht-ausgedehnten Konfiguration, die ersten und zweiten inneren Wände (3 ,4 ) gegeneinander innerhalb der elastischen Begrenzungen des Materials des inneren Elementes ausgelenkt sind. - Intervertebral-Implantat nach Anspruch 1, wobei: – das äußere Element (
14 ) ein vorderes Ende (30 ), ein hinteres Ende (31 ) und eine äußere Basis einschließt; und die erste eine äußere Wand (6 ) und die zweite eine äußere Wand (7 ) durch eine Breite der eine äußeren Basis beabstandet sind, wobei jede der ersten und zweiten äußeren Wand (6 ,7 ) ein äußeres Basisende am nächsten zu der äußeren Basis und eine äußere Kante (33 ), entfernt von der äußeren Basis, aufweist, und – das innere Element (13 ) ein vorderes Ende (30 ), ein hinteres Ende (31 ) und eine innere Basis einschließt; und die erste innere Wand (3 ) und zweite innere Wand (4 ) einen Abstand dazwischen aufweisen, damit die ersten und zweiten inneren Wände (3 ,4 ) innerhalb der ersten und zweiten äußeren Wände (6 ,7 ) hineinpassen, wobei jede der ersten und zweiten inneren Wände (3 ,4 ) ein inneres Basisende am nächsten zu der inneren Basis und eine innere Kante (36 ), entfernt von der inneren Basis, aufweist. - Intervertebral-Implantat nach Anspruch 2, wobei eine hintere Höhe der ersten und zweiten inneren Wände, gemessen von der inneren Basis zur inneren Kante (
36 ) am hinteren Ende der ersten und zweiten inneren Wände größer ist als eine vordere Höhe der ersten und zweiten inneren Wände, gemessen von der inneren Basis zu der inneren Kante (36 ) am vorderen Ende der ersten und zweiten inneren Wände. - Intervertebral-Implantat nach einem der Ansprüche 1–3, wobei – der erste Stift aus der ersten äußeren Wand vorsteht; – die erste Öffnung in der ersten inneren Wand so konfiguriert ist, um den ersten Stift darin aufzunehmen, wenn das Implantat in der ausgedehnten Konfiguration vorliegt; – der zweite Stift aus der zweiten äußeren Wand vorsteht; und – die zweite Öffnung in der zweiten inneren Wand so konfiguriert ist, um den zweiten Stift aufzunehmen, wenn das Implantat in der ausgedehnten Konfiguration vorliegt.
- Intervertebral-Implantat nach Anspruch 2, wobei die Arretierungsanordnung weiterhin umfasst: – eine erste Nut (
221b ), die sich von der ersten Öffnung in Richtung des Basisende der ersten inneren Wand oder der ersten äußeren Wand erstreckt; – eine zweite Nut, die sich von der zweiten Öffnung in Richtung des Basisende der zweiten inneren Wand oder der zweiten äußeren Wand erstreckt. - Intervertebral-Implantat nach einem der Ansprüche 1–2, wobei die Arretierungsanordnung weiterhin umfasst: – einen dritten Stift, der aus einer der ersten inneren Wände vorsteht und eine dritte Öffnung in einer gegenüberliegenden Wand, die konfiguriert ist, um den dritten Stift aufzunehmen, wenn das Implantat in der ausgedehnten Konfiguration vorliegt; und – einen vierten Stift, der aus einer der zweiten inneren Wand oder zweiten äußeren Wand vorsteht und eine vierte Öffnung in einer gegenüberliegenden Wand, die konfiguriert ist, um den vierten Stift aufzunehmen, wenn in der ausgedehnten Konfiguration; wobei der dritte und vierte Stift von dem ersten und zweiten Stift beabstandet sind und der Abstand zwischen der äußeren Basis und der ersten Öffnung geringer ist als ein Abstand zwischen der äußeren Basis und der dritten Öffnung.
- Intervertebral-Implantat nach einem der Ansprüche 1–6, wobei die inneren und äußeren Elemente jeweils einen rechteckigen Querschnitt aufweisen.
- Intervertebral-Implantat nach einem der Ansprüche 1–6, wobei die inneren und äußeren Elemente jeweils einen kreisförmigen Querschnitt aufweisen.
- Intervertebral-Implantat nach einem der Ansprüche 1–8, wobei die innere Basis und die äußere Basis jeweils mindestens eine Öffnung einschließen.
- Intervertebral-Implantat nach einem der Ansprüche 1–9, wobei eine äußere Oberfläche des äußeren Elementes oder eine äußere Oberfläche des inneren Elementes eine Verbindungsanordnung (
212 ) zum Verbinden von Endplatten benachbarter Wirbel umfasst. - Intervertebral-Implantat nach Anspruch 10, wobei die Verbindungsanordnung einen Dorn (
215 ) umfasst. - Intervertebral-Implantat nach Anspruch 1–6, weiterhin umfassend eine erste Nut in der ersten gegenüberliegenden Wand, die sich von der ersten Öffnung in Richtung auf ein Basisende der ersten gegenüberliegenden Wand erstreckt; eine zweite Nut in der zweiten gegenüberliegenden Wand, die sich von der zweiten Öffnung in Richtung auf ein Basisende der zweiten gegenüberliegenden Wand erstreckt; eine dritte Nut in der dritten gegenüberliegenden Wand, die sich von der dritten Öffnung in Richtung auf eine Basis der dritten gegenüberliegenden Wand erstreckt; und eine vierte Nut in der vierten gegenüberliegenden Wand, die sich von der vierten Öffnung in Richtung auf eine Basis der vierten gegenüberliegenden Wand erstreckt.
- Intervertebral-Implantat nach Anspruch 5, wobei die ersten und zweiten Nuten (
312 ) von den ersten und zweiten Basisenden zu den ersten und zweiten Öffnungen jeweils ansteigend vorliegen. - Intervertebral-Implantat nach einem der Ansprüche 1–13, wobei das innere Element (
101 ) röhrenförmig ist. - Intervertebral-Implantat nach einem der Ansprüche 1–14, wobei eine Höhe des vorderen Endes des Implantat geringer ist als eine Höhe des hinteren Ende des Implantats.
