Description
- Die Erfindung betrifft ein Werkzeug zum Einsetzen von Prothesen in den Körper und genauer Werkzeuge zum Einsetzen von Prothesen, wie etwa künstlichen Bandscheiben, in einen Zwischenwirbelraum.
- Die Wirbelsäulenchirurgie ist mit vielen Herausforderungen verbunden, da die Langzeitgesundheit und die Mobilität des Patienten häufig von der Technik und der Genauigkeit des Chirurgen abhängt. Eine Art der Wirbelsäulenchirurgie umfaßt die Entfernung des natürlichen Bandscheibengewebes, das zwischen benachbarten Wirbelkörpern gelegen ist. Es sind Vorgangsweisen bekannt, bei denen das natürliche, beschädigte Bandscheibengewebe durch einen Zwischenkörperkäfig oder eine Verschmelzungsvorrichtung, oder durch eine Bandscheibenprothese ersetzt wird. Das Einsetzen eines Gegenstands, wie etwa einer künstlichen Bandscheibenprothese, bietet dem Chirurgen mehrere Herausforderungen. Die benachbarten Wirbelkörper fallen aufeinander, sobald das natürliche Bandscheibengewebe entfernt ist. Diese Körper müssen bis zu einem Ausmaß separiert werden, das ausreicht, um die Anordnung der Prothese zu ermöglichen. Doch wenn die Wirbelkörper über einen bestimmten Grad hinaus separiert oder gespreizt werden, kann eine weitere Verletzung auftreten. Die Bandscheibenprothese muß auch richtig zwischen den benachbarten Wirbelkörpern angeordnet werden. Ein übermäßiges Einsetzen oder ein ungenügendes Einsetzen der Prothese kann zu Schmerzen, Haltungsproblemen und/oder begrenzter Mobilität oder Freiheit der Bewegung führen.
- Spezialisierte Werkzeuge wurden entwickelt, um die Anordnung von Vorrichtungen, wie etwa einer Bandscheibenprothese, zwischen benachbarten Wirbelkörpern der Wirbelsäule eines Patienten zu erleichtern. Unter den bekannten Werkzeugen zum Durchführen derartiger Vorgänge finden sich gesonderte Wirbelsäulendistraktoren und Einsetzvorrichtungen. Die Verwendung gesonderter Werkzeuge, um die Wirbelsäule abzulenken und eine Bandscheibenprothese oder ein Transplantat einzusetzen, kann sich als beschwerlich erweisen. Ferner kann die Verwendung mancher Distraktoren eine übermäßige Spreizung der Wirbelkörper verursachen.
- Beispielhafte Vorrichtungen zum Anbringen von Prothesen und/oder Transplantaten zwischen Wirbelkörpern sind in US-3,486,505, US-5,431,658 und US-5,505,732 offenbart. US-3,486,505 offenbart eine Vorrichtung, die verwendet werden kann, um ein Knochentransplantat zwischen zwei Wirbel einzusetzen, und zwei Arme umfaßt, die sich von einem senkrechten Griff erstrecken. US-5,431,658 offenbart eine Vorrichtung zur Erleichterung des Einsetzens von Knochentransplantaten zwischen zwei benachbarte Wirbel. Das offenbarte Werkzeug weist zwei flache, zangenartige Führungen auf, die die Wirbel spreizen, während ein Schraub-Inserter das Transplantat zwischen die gespreizten Wirbel zwängt. US-5,505,732 offenbart ein Instrument und ein Verfahren zum Einsetzen von Wirbelsäulenimplantaten. Der Zwischenwirbelraum wird zuerst gespreizt und dann eine hohle Manschette, die an einem Ende Zähne aufweist, in die Wirbel getrieben, die sich neben dem Bandscheibenraum befinden. Dann wird ein Bohrer durch die hohle Manschette geführt, der als Vorbereitung für das Aufnehmen des Wirbelsäulenimplantats die Bandscheibe und den Knochen entfernt, welches Implantat dann durch die Manschette eingesetzt wird.
- Trotz bestehender Werkzeuge und Technologien verbleibt ein Bedarf, eine Vorrichtung bereitzustellen, um das richtige und bequeme Einsetzen eines Gegenstands, wie etwa einer Bandscheibenprothese, zwischen benachbarte Wirbelkörper zu erleichtern, während die Gefahr einer weiteren Verletzung des Patienten auf ein Mindestmaß verringert wird.
- Die vorliegende Erfindung stellt eine Vorrichtung bereit, die zum Implantieren von Prothesen, wie etwa künstlichen Wirbelsäulenbandscheiben, in einer sicheren und leistungsfähigen Weise in einen Patienten nützlich ist. Das Einsetzwerkzeug der Erfindung umfaßt ein Paar von gegenüberliegenden Hebeln, von denen jeder einen proximalen Griffabschnitt und einen distalen Abschnitt aufweist. Ein Hebelpunkt ist zwischen den beiden Hebeln angeordnet, um den richtigen Betrieb des Werkzeugs zu ermöglichen. Das Werkzeug umfaßt ferner einen Schubblock, der zwischen den beiden Hebeln angeordnet ist. Der Schubblock ist wahlweise zwischen einer ursprünglichen Stellung distal von dem Hebelpunkt und einer endgültigen Stellung, die dem distalen Ende der Hebel benachbart ist, beweglich. Eine Schubstange, die die Bewegung eines Schubblocks erleichtert, weist ein mit dem Schubblock verbundenes distales Ende und ein proximales Griffende auf.
- Jeder Hebel ist im Wesentlichen langgestreckt, und der distale Abschnitt jedes Hebels umfaßt eine auswärts gerichtete Oberfläche, die distal in einer Anschlagoberfläche endet, und eine einwärts gerichtete Oberfläche, die sich distal über die Anschlagoberfläche hinaus erstreckt. Vorzugsweise ist die auswärts gerichtete Oberfläche jedes Hebels im Wesentlichen glatt. In einer Ausführungsform legt die Anschlagoberfläche jedes Hebels eine distal ausgerichtete Wand fest, die dazu geeignet ist, gegen einen Wirbelkörper zu stoßen, um das richtige Einsetzen einer künstlichen Bandscheibe zu erleichtern.
- Jeder Hebel weist gegenüberliegende Seitenwände auf, und jede Seitenwand weist eine darin festgelegte Bahn auf. Der Querschnitt jedes Hebels kann I-förmig oder T-förmig sein, und jede Bahn in den Hebelseitenwänden weist gegenüberliegende Schienen auf, die durch eine zentral angeordnete Rille getrennt sind. Der Schubblock kann mit einer oberen und einer unteren Vertiefung gestaltet sein, die in seiner oberen bzw. unteren Oberfläche gebildet ist, wobei jede Vertiefung dazu geeignet ist, einen der Hebel zu lagern. In dieser Ausführungsform weist jede der oberen und der unteren Vertiefung gegenüberliegende, im Wesentlichen vertikale Wände auf, die durch eine im Wesentlichen horizontale Basiswand getrennt sind. Jede der vertikalen Seitenwände, die die Vertiefung des Schubblocks festlegen, weist vorzugsweise an einer inneren Oberfläche davon eine vorspringende Führungssäule auf, die zur Anbringung in einer der Bahnen der Hebel geeignet ist.
- In einer Ausführungsform kann der Griffabschnitt der Hebel Vorsprünge enthalten, die eine Blattextraktion unter Verwendung eines Werkzeugs wie etwa eines Schlaghammers oder einer Schlaggabel erleichtern. In einer anderen Ausführungsform kann der distale Abschnitt der Hebel dazu gestaltet sein, eine Regulierung der Blattspitzenlänge zu gestatten.
- Das Einsetzwerkzeug der Erfindung kann in der folgenden Weise verwendet werden. Sobald das natürliche, beschädigte Bandscheibengewebe aus einem Patienten entfernt ist und der Bereich zum Aufnehmen einer künstlichen Bandscheibe vorbereitet ist, wird die künstliche Bandscheibe so zwischen die Hebel des Einsetzwerkzeugs geladen, daß eine Hinterseite der Scheibe gegen ein distales Ende des Schubblocks stößt. Die distale Spitze der Hebel wird dann so zwischen den Wirbelkörpern angeordnet, daß die auswärts gerichteten Oberflächen jedes Hebels mit gegenüberliegenden Wirbelkörpern in Kontakt treten. Sobald diese Stellung erreicht ist, wird die Schubstange distal vorgeschoben, wodurch verursacht wird, daß sich der Schubblock und die künstliche Bandscheibe in der gleichen Weise entlang der inneren Oberflächen der Hebel distal bewegen. Während sich die künstliche Bandscheibe und die Schubstange distal oder vorwärts bewegen, separieren sich die Hebel und verursachen sie auch eine vertikale Separation der benachbarten Wirbelkörper. In der richtigen Stellung sollten die distal gerichteten Oberflächen der beiden Hebel mit den Wirbelkörpern in Kontakt treten. Sobald ein derartiger Kontakt zwischen den distal gerichteten Oberflächen der Hebel und den Wirbelkörpern erreicht ist und die künstliche Bandscheibe damit beginnt, zwischen den beiden Wirbelkörpern positioniert zu werden, verursacht eine weitere Vorwärtsbewegung des Schubblocks, daß sich die Blattspitzen der Hebel aus dem Bereich zwischen den Wirbelkörpern zurückziehen und die künstliche Bandscheibe ausstoßen. Dieses Werkzeug ermöglicht somit die richtige Anordnung der künstlichen Bandscheibe zwischen den Wirbelkörpern ohne ein übermäßiges Einsetzen oder ein ungenügendes Einsetzen, während das Ausmaß der Spreizung der Wirbel auf ein Mindestmaß verringert wird.

15B ist eine seitliche Aufrißansicht eines Abschnitts des distalen Endes des Einsetzwerkzeugs von13 in einer zweiten Stellung. - Die vorliegende Erfindung umfaßt ein Einsetzwerkzeug, das für die leistungsfähige und wirksame Anordnung eines Gegenstands, wie etwa einer künstlichen Bandscheibe, zwischen benachbarte Wirbelkörper nützlich ist. Obwohl die Erfindung in erster Linie unter Bezugnahme auf die Verwendung zum Einsetzen einer künstlichen Bandscheibe zwischen benachbarte Wirbelkörper beschrieben wird, versteht sich, daß das Einsetzwerkzeug der Erfindung verwendet werden kann, um andere Elemente zwischen Wirbelkörpern oder an anderen Stellen im Körper eines Patienten anzuordnen. Beispielhafte Elemente, die zwischen Wirbelkörpern angeordnet werden können, enthalten, jedoch ohne Beschränkung darauf, Zwischenkörperkäfige, Verschmelzungsvorrichtungen, Distanzstücke, Transplantate, und dergleichen.
- Wie in
1 bis3 gezeigt umfaßt das Einsetzwerkzeug10 der Erfindung gegenüberliegende Hebel12 ,14 , die jeweils einen distalen Abschnitt12A ,14A und einen proximalen Griffabschnitt12B ,14B aufweisen. Zwischen den Hebeln12 ,14 sind ein Hebelpunkt16 und ein Schubblock18 angeordnet, der distal von dem Hebelpunkt16 angeordnet ist. Der Schubblock18 ist wahlweise von einer ursprünglichen Stellung distal von dem Hebelpunkt16 zu einer endgültigen Stellung einem distalen Ende der Hebel benachbart beweglich. Eine Schubstange20 , die die wahlweise Bewegung des Schubblocks erleichtert, weist ein mit dem Schubblock verbundenes distales Ende20A und ein proximales Griffende20B auf. Obwohl dies nicht Teil der Erfindung ist, befindet sich eine Prothese, wie etwa eine künstliche Bandscheibe24 , distal von dem Schubblock18 zwischen den Hebeln in Stellung. - Vorzugsweise sind die Hebel
12 und14 langgestreckte Elemente, die zueinander spiegelbildlich sind. Es gibt keine absolute Oberseite oder Unterseite des Werkzeugs10 , da es möglich ist, jede der Oberflächen als eine "obere" Oberfläche zu verwenden. Zur Erleichterung der Bezugnahme werden jedoch Hebel hierin manchmal unter Bezugnahme auf eine veranschaulichte Ausrichtung beschrieben werden. Zum Beispiel können der Hebel12 und Bestandteile davon manchmal als der obengelegene, obere oder höherliegende Hebel bezeichnet werden, während der Hebel14 manchmal als der untengelegene, untere oder innenliegende Hebel bezeichnet werden kann. - Unter weiterer Bezugnahme auf
1 bis7 enthalten die Hebel12 ,14 distale Enden12A ,14A und proximale Enden12B ,14B . Das proximale Ende12B ,14B jedes Hebels kann eine Form aufweisen, die das bequeme Ergreifen und Handhaben des Werkzeugs erleichtert. Das proximale Ende jedes Hebels kann ungefähr eine Hälfte der Länge jedes Hebels umfassen. In einer Ausführungsform kennzeichnet eine Schulter26 das Ende des proximalen Abschnitts des Werkzeugs und den Beginn des distalen Abschnitts des Werkzeugs. Wie in1 und3 gezeigt weist der proximale Abschnitt jedes Hebels vorzugsweise eine Breite (Wp) auf, die größer als die Breite (Wd) des distalen Abschnitts jedes Hebels ist. - Der proximale Abschnitt
12B jedes Hebels kann einen vertieften Bereich28 zum Aufnehmen des Hebelpunkts16 enthalten. Wie in6 gezeigt weist der vertiefte Bereich die Form eines im Wesentlichen rechteckigen Ausschnitts auf, der an beiden Seiten der Hebel12B ,14B gebildet ist. Dieser vertiefte Bereich28 ermöglicht, daß die Hebel den Hebelpunkt16 , wie nachstehend beschrieben, bequem lagern können. Der vertiefte Bereich28 ist vorzugsweise geringfügig proximal der Schulter26 gebildet. In einer Ausführungsform beginnt ein distaler Abschnitt26A des vertieften Bereichs28 ungefähr 10 bis 100 mm proximal der Schulter26 . Der vertiefte Bereich26 weist im Allgemeinen eine Länge im Bereich von etwa 10 bis 40 mm auf und ist etwa 1 bis 40 mm vertieft. Man wird verstehen, daß die optionalen Grenzen bei Bereichen, die in dieser Beschreibung als gepaarte obere und untere Grenzen geboten werden, getrennt werden können, um Bereiche mit offenem Ende bereitzustellen, und neukombiniert werden können, um unterschiedliche geschlossene Bereiche bereitzustellen. - Der proximale Bereich jedes Hebels
12B ,14B kann auch eine Bohrung30A ,30B enthalten, die dazu geeignet ist, einen Schraubenbolzen32 zu lagern, der eine Steuerung der Beabstandung zwischen den Hebeln ermöglicht, damit der Schubblock genau mit dem Metallabschnitt der künstlichen Bandscheibe eingreift. Wie in6 gezeigt enthält der Bolzen bzw. Schraubenbolzen32 einen Kopf34 und einen Schaft35 , der einen Gewindebereich36 aufweist. Vorzugsweise ist im Inneren der Bohrung30A ein Innengewinde36 ausgebildet. - Der distale Abschnitt jedes Hebels
12A ,14A zeigt auswärts gerichtete Oberflächen38 ,40 (die als obere bzw. untere Oberfläche veranschaulicht sind) und einwärts gerichtete Oberflächen42 ,44 , auf denen während eines Einsetzvorgangs die künstliche Bandscheibe24 reitet. Die auswärts und einwärts gerichteten Oberflächen des Hebels sind vorzugsweise im Wesentlichen glatt. Jeder Hebel enthält ferner Seitenflächen46 ,48 . - Distale Abschnitte
12A ,14A der Hebel12 ,14 weisen auch Blattspitzen50A ,50B auf, die an den distalen Enden der Hebel gebildet sind. Die Blattspitzen sind in der Größe bemessen und gestaltet, um ihre Anordnung zwischen Wirbelkörpern201 ,202 zu erleichtern. Die auswärts gerichteten Oberflächen52A ,52B der Blattspitzen können so gestaltet sein, daß sie Oberflächen aufweisen, die abgeschrägt oder gerundet sind. - Die bei geschlossenem Werkzeug, wie in
1 gezeigt, an den Blattspitzen gemessene Dicke der Hebel ist beträchtlich geringer als die zwischen den auswärts gerichteten Oberflächen38 ,40 gemessene Dicke der Hebel, wenn sich das Werkzeug in der geschlossenen Stellung befindet. Vorzugsweise liegt die an den Blattspitzen gemessene Dicke im Bereich von etwa 0,5 bis 2 mm. - Ein Abschnitt jedes Hebels
12A ,12B , der einem proximalen Ende der Blattspitzen50A ,50B benachbart angeordnet ist, enthält eine Anschlagoberfläche54 . Die Anschlagoberfläche54 , die im Wesentlichen vertikal ausgerichtet und distal gerichtet ist, ist dazu geeignet, während eines chirurgischen Vorgangs zum Einsetzen eines Elements, wie etwa einer künstlichen Bandscheibe, zwischen benachbarte Wirbelkörper gegen einen Wirbelkörper zu stoßen. Die Anschlagoberfläche54 kann eine Vielfalt von Gestaltungen annehmen. In einer in2 und8A bis9 gezeigten beispielhaften Ausführungsform weist die Anschlagoberfläche ein im Wesentlichen konkaves Profil auf, wenn sie in der vertikalen Ebene betrachtet wird. Wie in1 bis7 gezeigt können die Seitenflächen der distalen Abschnitte12A ,14A der Hebel12 ,14 gegenüberliegende Schienen56 ,58 mit einer dazwischen angeordneten vertieften Bahn60 aufweisen. Die Bildung derartiger vertiefter Bahnen60 in den distalen Seitenwänden der Hebel12 ,14 verursacht, daß diese Elemente ein Profil aufweisen, welches wie in4 und7 gezeigt im Wesentlichen I-förmig oder T-förmig ist. Wie nachstehend besprochen können die Schienen56 ,58 und die Bahn60 wirksam sein, um eine glatte und leistungsfähige Bewegung des Schubblocks zu erleichtern. - Ein Durchschnittsfachmann wird erkennen, daß die Größe und die Form der Hebel unterschiedlich sein kann. Im Allgemeinen beträgt jedoch die Gesamtlänge der Hebel etwa 200 bis 400 mm, wobei der proximate Abschnitt
12B ,14B (von dem proximalen Ende bis zur Schulter26 ) eine Länge von etwa 100 bis 300 mm und der distale Abschnitt12A ,14A (von der Schulter26 bis zu den Blattspitzen) eine Länge von etwa 100 bis 300 mm aufweisen kann. - In einer in
1 bis3 und7 veranschaulichten Ausführungsform ist der Hebelpunkt16 im Wesentlichen ein blockartiger Gegenstand, der im Wesentlichen vertikale Seitenwände62 aufweist. Die obere und die untere Oberfläche64 ,66 des Hebelpunkts16 enthalten Vertiefungen oder Rillen68 , von denen jede durch eine Basiswand70 ,72 und gegenüberliegende innere Seitenwände74 festgelegt ist. Ein Kernabschnitt78 liegt zwischen den Basiswänden70 und72 . Der Kernabschnitt78 enthält vorzugsweise eine zentrale Bohrung80 , um einen Durchgang der Schubstange durch den Abschnitt zu erlauben. In einer beispielhaften Ausführungsform enthält jede Basiswand70 ,72 eine Anbringungssäule62 . Wie in6 gezeigt kann ein vorspannendes Element84 , wie etwa eine Spiralfeder, auf jeder der Anbringungssäulen62 angeordnet werden, und können die Anbringungssäulen wiederum in den Bohrungen31A ,31B der Hebel12 ,14 angeordnet werden. - Der Hebelpunkt
16 kann praktisch jede beliebige Größe und Form annehmen, die fähig ist, ihn wirksam zu machen, einen im Wesentlichen dazwischenliegenden Abschnitt der Hebel zu separieren, während den proximalen Griffabschnitten12B ,14B gestattet wird, zueinander geschlossen zu werden, was zu einer Öffnung oder Separation des distalen Abschnitts12A ,14A führt. Im Allgemeinen liegt die Höhe der vertikalen Seitenwände62 im Bereich von etwa 20 bis 70 mm, während die Höhe der Kernabschnitts78 im Bereich von etwa 5 bis 30 mm liegt. Die Länge des Kernabschnitts kann etwa 5 bis 40 mm betragen. - Ein Durchschnittsfachmann wird ferner erkennen, daß der Hebelpunkt eine Vielfalt von anderen Formen, Größen und Anbringungsgestaltungen annehmen kann. Die oben beschriebene Ausführungsform soll eine beispielhafte Hebelpunktausführung und Anbringungsgestaltung darstellen.
- Der wie oben erwähnte Schraubenbolzen
32 kann verwendet werden, um die Höhe/Beabstandung der Hebel zu regulieren. Eine der Bohrungen30A ,30B weist ein Innengewinde37 auf, das mit dem Gewindeabschnitt36 des Schraubenbolzens32 eingreift. Das Festziehen oder Lockern des Schaubenbolzens wird zu einem Vergrößern oder Verringern der Beabstandung/Entfernung zwischen den Hebeln führen. - Wie am besten in
7 gezeigt kann der Schubblock18 die Form eines im Wesentlichen blockartigen Gegenstands aufweisen, der vertikale Seitenwände84 aufweist, die eine obere und eine untere Vertiefung86 ,88 festlegen, wobei jede Vertiefung durch eine Basiswand90 und gegenüberliegende einwärts gerichtete Seitenwände92 festgelegt ist. Jede einwärts gerichtete Seitenwand92 enthält vorzugsweise eine Führungssäule94 , die in vertieften Bahnen60 , welche in den distalen Abschnitt der Hebel12A ,14A gebildet sind, eingreifen kann. - Der Kernabschnitt
91 des Schubblocks18 ist zwischen der oberen und der unteren Basiswand90 angeordnet. Der Kernabschnitt91 weist eine proximal ausgerichtete Wand96 und eine distal ausgerichtete Wand97 auf. Wie in7 gezeigt kann die proximal ausgerichtete Wand vorzugsweise eine Blindbohrung98 enthalten, die ein Innengewinde100 aufweist. Das Gewinde ist dazu geeignet, mit einem komplementären Gewinde einzugreifen, das an einem distalen Ende20A der Schubstange20 gebildet ist, wodurch der Schubstange20 erlaubt wird, fest mit dem Schubblock18 einzugreifen. - Die distal ausgerichtete Wand
97 des Schubblocks kann einen vertieften Bereich104 enthalten, der dazu geeignet ist, eine künstliche Bandscheibe24 unterzubringen. Wie veranschaulicht weist der vertiefte Bereich104 Abmessungen auf, die der künstlichen Bandscheibe ermöglichen, locker darin zu sitzen. Ein Durchschnittsfachmann wird erkennen, daß der vertiefte Bereich Abmessungen aufweisen sollte, die geringfügig größer als die Abmessungen der Scheibe sind, um einen reibenden Sitz, der ein freies Lösen der Scheibe von dem Werkzeug verhindern kann, zu vermeiden. - In einer in
12 veranschaulichten Ausführungsform weist der Schubblock18 einen vertieften Abschnitt104 mit einer Kontaktwand106 auf, die eine im Allgemeinen bogenförmige Form aufweist. Vorzugsweise ist die Kontaktwand im Wesentlichen konkav und gut geeignet, um einen Kontakt über einen wesentlichen Oberflächenbereich einer im Allgemeinen gekümmten (konvexen) Hinterfläche einer künstlichen Bandscheibe24 aufrechtzuerhalten. - Wie oben erwähnt kann eine Schubstange
20 benutzt werden, um den Schubblock18 zu betätigen. Die Schubstange20 ist vorzugsweise ein langgestrecktes zylinderförmiges Element, das ein proximales Ende20B und ein distales Ende20A aufweist. Vorzugsweise enthält das äußerste distale Ende der Schubstange einen Gewindebereich102 . Die Stange ist dazu geeignet, zwischen den proximalen Enden12B ,14B der Hebel12 ,14 angeordnet zu werden, so daß sie sich durch die Bohrung80 im Hebelpunkt16 erstreckt. Der Gewindebereich102 der Stange20 greift mit dem in der Bohrung98 des Schubblocks18 gebildeten Gewinde100 ein. Auf diese Weise greift die Stange fest mit dem Schubblock ein, so daß eine Vorwärts- und Rückwärts-Bewegung der Schubstange direkt den Schubblock bewegen wird. -
9 und10 veranschaulichen eine alternative Ausführungsform, wobei ein Einsetzwerkzeug10' im proximalen Griffabschnitt12B' ,14B' von Hebeln12' ,14' einen Arretiermechanismus108 enthält. Der Arretiermechanismus108 , der eine Sperrklinke und eine Nut umfassen kann, befestigt einfach den proximalen Griffabschnitt der Hebel in einer gewünschten Stellung und verhindert eine vollständige Trennung dieser Elemente.9 veranschaulicht das Werkzeug10' in einer Stellung, in der der proximale Abschnitt12B' ,14B' der Hebel offen oder unbetätigt ist. Der Arretiermechanismus108 verhindert eine weitere Öffnung oder Trennung der proximalen Abschnitte12B' ,14B' der Hebel. Doch wie in10 gezeigt erlaubt der Arretiermechanismus108 nach wie vor, daß die Griffe12B' ,14B' betätigt oder zueinander geschlossen werden, um das Werkzeug zu betätigen und eine Prothese, wie etwa eine künstliche Bandscheibe24 , einzusetzen. Ein Vorteil des Arretiermechanismus108 ist, daß es weniger wahrscheinlich ist, daß die Scheibe während der Handhabung des Werkzeugs vor dem chirurgische Einsetzen unabsichtlich von dem Werkzeug entfernt wird. -
13 veranschaulicht eine Ausführungsform der Erfindung, wobei ein Einsetzwerkzeug300 mehrere Vorsprünge302 ,304 enthält, die an proximalen oder Griffabschnitten312B ,314B von Hebeln312 ,314 ausgebildet sind. Vorzugsweise sind die Vorsprünge302 ,304 an lateralen Oberflächen der Griffabschnitte ausgebildet. In einer beispielhaften Ausführungsform sind sowohl am oberen als auch am unteren Hebel312 ,314 zwei Vorsprünge ausgebildet. Obwohl sie als im Wesentlichen zylinderförmig veranschaulicht sind, versteht sich, daß die Vorsprünge302 ,304 praktisch jede beliebige Form annehmen können. Die Vorsprünge können nützlich sein, um die Extraktion der Blattspitzen350A ,350B unter Verwendung eines Werkzeugs wie etwa eines Schlaghammers oder einer Schlaggabel (nicht gezeigt) in einer einem Durchschnittsfachmann bekannten Weise zu erleichtern. Die Abmessungen der Vorsprünge302 ,304 können innerhalb eines weiten Bereichs schwanken. Im Allgemeinen erstrecken sich die Vorsprünge302 ,304 jedoch um eine Entfernung von etwa 3 mm bis 50 mm von den lateralen Oberflächen der Hebel312 ,314 . - Obwohl nur bei der in
13 gezeigten Ausführungsform des Einsetzwerkzeugs Vorsprünge302 ,304 veranschaulicht sind, wird ein Durchschnittsfachmann leicht erkennen, daß die Vorsprünge an den Griffabschnitten von Einsetzwerkzeugen, die gemäß anderen Ausführungsformen der Erfindung aufgebaut sind, vorhanden sein können. - Unter Bezugnahme auf
13 bis15B kann das Einsetzwerkzeug300 auch ein Merkmal enthalten, das eine selektive Regulierung der Blattspitzenlänge (d.h., der Entfernung zwischen dem distalen Ende306 der Blattspitzen350A , B und der Anschlagoberfläche) erlaubt. - In einer derartigen Ausführungsform weist der distale Abschnitt eines jeden der Hebel
312A ,314A eine Anschlagelementkomponente307 und eine Blattelementkomponente308 auf, die in der Längsrichtung in bezug zueinander trennbar sind. Die Trennbarkeit der Komponenten307 und308 erlaubt, daß die Blattspitzenlänge durch Verändern der relativen Positionen der Komponenten307 ,308 reguliert wird. - Wie in
13 und14 gezeigt, enthält das Werkzeug300 einen Verbindungskasten309 , der die Komponenten307 ,308 unterbringt und befestigt. Ein Abschnitt307A der Anschlagelementkomponente307 , der mit der inneren Oberfläche der Abdeckung320 des Verbindungskastens309 eingreift, enthält eine Reihe von Rillen310 , die durch erhöhte Wülste311 getrennt sind. In der gleichen Weise enthält die auftreffende innere Oberfläche der Abdeckung320 des Verbindungskastens309 ebenfalls Rillen und Wülste310 ,311 . Wie ferner veranschaulicht ist die Abdeckung320 durch passende Befestigungsmittel, wie etwa Schrauben322 , an den Hebeln312 ,314 befestigt. Vorspannende Elemente, wie etwa Druckfedern (nicht gezeigt), werden vorzugsweise verwendet, um den Verbindungskasten in eine solche Stellung vorzuspannen, daß die Rillen und Wülste310 ,311 der Abdeckung320 und der Anschlagelementkomponente307 miteinander eingreifen und fest miteinander verbunden bleiben. Wie in14 gezeigt können an der Anschlagelementkomponente307 passende Anzeiger325 vorhanden sein, um die Position der Anschlagelementkomponente307 in bezug auf die Blattelementkomponente anzuzeigen. Obwohl14 nur die Gestaltung des Hebels12 ausführlich veranschaulicht, versteht sich, daß für den Hebel14 der gleiche Aufbau verwendet wird. - Das Ergebnis dieser selektiven Regulierbarkeit ist in
15A und15B gezeigt, wobei die Blattspitzenlänge in15A größer als in15B ist. -
1 und8A bis8D veranschaulichen aufeinanderfolgend die Verwendung des Werkzeugs10 zum Einsetzen einer künstlichen Bandscheibe24 . Das Werkzeug wird zuerst wie in1 gezeigt zusammengesetzt, und die hintere Oberfläche27 der Scheibe wird gegen die distal ausgerichtete Wand97 des Schubblocks angeordnet, während der Schubblock18 in seiner ursprünglichen Stellung angeordnet ist, so daß seine proximal ausgerichtete Wand96 gegen die Schulter26 stößt. Mit den Griffen12B ,14B in der offenen Stellung und den Blattspitzen50A , B geschlossen werden die Blattspitzen zwischen benachbarten Wirbelkörpern201 ,202 eingekeilt, um eine geringfügige Separation zwischen zumindest den hinteren Abschnitten der Wirbelkörper auszuführen. Die Blattspitzen50A , B sollten wie in8A gezeigt vollständig zwischen die Wirbelkörper eingesetzt sein, um der Wirbelanschlagoberfläche54 zu ermöglichen, gegen die Hinterseite204 der Wirbelkörper201 ,202 zu stoßen. Dann wird die Schubstange20 vorwärts geschoben, wodurch eine distale Bewegung des Schubblocks18 und der künstlichen Bandscheibe24 verursacht wird. Die Vorwärts- oder distale Bewegung des Schubblocks18 und der künstlichen Bandscheibe24 verursacht wie in8B gezeigt auch eine weitere Separation der Blattspitzen50A , B und somit eine weitere Separation der Wirbelkörper201 ,202 . Das Vorschieben des Schubblocks18 und der künstlichen Bandscheibe24 dauert an, bis, wie in8C gezeigt, die Scheibe richtig zwischen benachbarte Wirbelkörper eingesetzt ist.8B und8C veranschaulichen, daß die Separation der Wirbelkörper stets nur in dem Ausmaß ausgeführt wird, das nötig ist, um die Scheibe einzusetzen. Eine übermäßige Spreizung oder Separation der Wirbelkörper tritt nicht auf, da die Separation der Wirbelkörper durch die künstliche Bandscheibe verursacht wird und durch die Dickenabmessungen der künstlichen Bandscheibe gesteuert wird. Während die Schubstange20 nach dem Einsetzen der Scheibe weiter vorwärts geschoben wird, wird verursacht, daß die Blattspitzen50A , B wie in8D gezeigt aus dem Bereich zwischen den Wirbelkörpern zurückgezogen werden.
Claims (17)
- Einsetzwerkzeug (
10 ) für medizinische Vorrichtungen, umfassend ein Paar von gegenüberliegenden Hebeln (12 ,14 ), die jeweils einen proximalen Griffabschnitt (12B ,14B ) und einen distalen Abschnitt (12A ,14A ) aufweisen; einen Hebelpunkt (16 ), der zwischen den beiden Hebeln an einem Punkt angeordnet ist, der zwischen den Griffabschnitten und den distalen Abschnitten liegt; einen Schubblock (18 ), der zwischen den beiden Hebeln angeordnet ist und wahlweise zwischen einer ursprünglichen Stellung distal von dem Hebelpunkt und einer endgültigen Stellung, die sich distal von dem Hebelpunkt und zu einem distalen Ende der Hebel benachbart, weiter von dem Hebelpunkt entfernt als die ursprüngliche Stellung, befindet, beweglich ist; und eine Schubstange (20 ), die ein mit dem Schubblock verbundenes distales Ende (20A ) und ein proximales Griffende (20B ) aufweist; dadurch gekennzeichnet, daß der Hebelpunkt es den Griffabschnitten der Hebel erlaubt, geschlossen zu werden, so daß sich die distalen Abschnitte der Hebel separieren, wodurch die Bewegung des Schubblocks aus der ursprünglichen Stellung in die endgültige Stellung erleichtert wird. - Vorrichtung (
10 ) nach Anspruch 1, dadurch gekennzeichnet, daß jeder Hebel im wesentlichen langgestreckt ist und der distale Abschnitt (12A ,14A ) jedes Hebels eine auswärts gerichtete Oberfläche umfaßt, die distal in einer Anschlagoberfläche (54 ) endet, und eine einwärts gerichtete Oberfläche umfaßt, die sich distal über die Anschlagoberfläche hinaus erstreckt und eine Blattspitze (50A ,50B ) bildet. - Vorrichtung (
10 ) nach Anspruch 2, dadurch gekennzeichnet, daß jede Blattspitze (50A ,50B ) eine sich von der Anschlagoberfläche (54 ) zu einem distalen Ende der Blattspitze erstreckende Länge im Bereich von etwa 5 bis 50 mm und eine Dicke im Bereich von etwa 0,5 bis 10 mm aufweist. - Vorrichtung (
10 ) nach Anspruch 3, dadurch gekennzeichnet, daß eine auswärts gerichtete Oberfläche jeder Blattspitze (50A ,50B ) eine Gestaltung aufweist, die aus der Gruppe gewählt wird, welche aus abgeschrägten und gerundeten Gestaltungen besteht. - Vorrichtung (
10 ) nach Anspruch 2, dadurch gekennzeichnet, daß die Anschlagoberfläche (54 ) jedes Hebels eine distal ausgerichtete Anschlagwand festlegt. - Vorrichtung (
10 ) nach Anspruch 2, dadurch gekennzeichnet, daß die nach innen gerichtete Wand jedes Hebels, die sich zu einem abschließenden Ende jeder Blattspitze (50A ,50B ) erstreckt, im wesentlichen glatt ist. - Vorrichtung (
10 ) nach Anspruch 2, dadurch gekennzeichnet, daß der Schubblock (18 ) eine obere und eine untere Vertiefung (86 ,88 ) aufweist, die jeweils dazu geeignet ist, einen der Hebel zu lagern, wobei jede der oberen und der unteren Vertiefung gegenüberliegende, im wesentlichen vertikale Wände aufweist, die durch eine im wesentlichen horizontale Basiswand getrennt sind. - Vorrichtung (
10 ) nach Anspruch 7, dadurch gekennzeichnet, daß die Tiefe jeder Vertiefung (86 ,88 ) des Schubblocks (18 ) im Bereich von etwa 0,5 bis 10 mm liegt, und die Höhe des Schubblocks zwischen der oberen und der unteren Vertiefung im Bereich von etwa 2 bis 25 mm liegt. - Vorrichtung (
10 ) nach Anspruch 8, dadurch gekennzeichnet, daß eine distal gerichtete Oberfläche des Schubblocks (18 ) einen Implantatsitzbereich (104 ) enthält. - Vorrichtung (
10 ) nach Anspruch 8, dadurch gekennzeichnet, daß der Schubblock (18 ) wirksam ist, um die Hebel und die Blattspitzen (50A ,50B ) zu separieren, während er aus der ursprünglichen Position in die endgültige Position vorgeschoben wird. - Vorrichtung (
10 ) nach Anspruch 1, dadurch gekennzeichnet, daß der Griffabschnitt (12B ,14B ) der gegenüberliegenden Hebel (12 ,14 ) einen Arretiermechanismus (108 ) beinhaltet, der wirksam ist, um eine unbeabsichtigte vollständige Trennung der Hebel zu verhindern. - Vorrichtung (
10 ) nach Anspruch 2, dadurch gekennzeichnet, daß der Hebelpunkt (16 ) eine obere Vertiefung (68 ) und eine untere Vertiefung (68 ) aufweist, die jeweils durch einen Zwischenabschnitt getrennt sind, welcher eine obere und eine untere Basiswand aufweist, wobei der Zwischenabschnitt eine Bohrung aufweist, die sich von einer proximalen Oberfläche zu einer distalen Oberfläche der Zwischenwand hindurch erstreckt. - Vorrichtung (
10 ) nach Anspruch 12, dadurch gekennzeichnet, daß die obere und die untere Wand jeweils einen vorspringenden Stift umfaßt. - Vorrichtung (
10 ) nach Anspruch 13, dadurch gekennzeichnet, daß jeder Hebel eine Bohrung enthält, die in der einwärts gerichteten Oberfläche seines proximalen Abschnitts gebildet ist, wobei jede Bohrung dazu geeignet ist, einen der vorspringenden Stifte des Hebelpunkts (16 ) aufzunehmen. - Vorrichtung (
10 ) nach Anspruch 14, dadurch gekennzeichnet, daß die Vorrichtung (10 ) ferner ein Paar von Spiralfedern enthält, von denen jede dazu geeignet ist, an einem der vorspringenden Stifte des Hebelpunkts (16 ) gelagert zu werden. - Vorrichtung (
10 ) nach Anspruch 12, dadurch gekennzeichnet, daß der Zwischenabschnitt des Hebelpunkts (16 ) eine Höhe aufweist, die die Höhe eines Bereichs des Schubblocks (18 ) zwischen der oberen und der unteren Vertiefung des Schubblocks um eine Entfernung im Bereich von etwa 1 bis 20 mm übersteigt. - Vorrichtung (
10 ) nach Anspruch 1, dadurch gekennzeichnet, daß an einer lateralen Oberfläche des proximalen Griffabschnitts (12B ,14B ) jedes Hebels ein oder mehrere Vorsprünge ausgebildet ist bzw. sind.
