- Diese Erfindung bezieht sich allgemein auf biomedizinische Produkte zur Behandlung von Gefäßzuständen. Im spezielleren Sinn bezieht sich die Erfindung auf eine Stenttransplantat-Anordnung mit Abgabeöffnungen zur Eluierung eines therapeutischen Mittels, und zwar vorzugsweise in die Gefäßwand und nicht in das Gefäßlumen.
- HINTERGRUND DER ERFINDUNG
- Stents sind allgemein zylinderförmige Vorrichtungen, die radial ausdehnbar sind, um einen Gefäßabschnitt oder ein anderes anatomisches Lumen nach Implantation in das Körperlumen offen zu halten.
- Es werden derzeit verschiedene Stentarten verwendet, einschließlich ausdehnbare und selbstexpandierende Stents. Ausdehnbare Stents werden im Allgemeinen mit Ballonkathetern oder anderen ausdehnbaren Vorrichtungen in denjenigen Bereich eingebracht, der behandelt werden soll. Um den Stent einzuführen, wird er in komprimierter Form entlang des Einführinstruments positioniert, zum Beispiel auf einem Ballon befestigt, der um einen Führungsdraht als Teil des Einführinstruments gefaltet oder auf andere Weise gewickelt ist. Nachdem der Stent entlang der Läsion positioniert wurde, wird er mit dem Einführinstrument ausgedehnt, wodurch sich der Durchmesser vergrößert. Bei selbstexpandierenden Stents wird normalerweise eine Hülse zurückgezogen, um die Ausdehnung des Stents zuzulassen.
- Stents werden in Verbindung mit Ballonkathetern bei einer Vielzahl medizinisch-therapeutischer Anwendungen verwendet, darunter auch die intravaskuläre Angioplastie. Zum Beispiel wird bei einer perkutanen transluminalen Koronarangioplastie (PTCA) ein Ballonkatheter inflatiert, um ein stenotisches Blutgefäß zu dilatieren. Die Stenose kann die Folge einer Läsion, wie z. B. eines Plaques oder Thrombus sein. Wenn der Ballon inflatiert ist, übt er eine Kompressionskraft auf die Läsion aus, wodurch der Innendurchmesser des betroffenen Gefäßes vergrößert wird. Der vergrößerte Gefäß-Innendurchmesser erleichtert den Blutfluss. Einige Zeit nach dem Verfahren tritt jedoch bei einem großen Anteil der behandelten Gefäße eine Restenose auf.
- Um Restenosen zu vermeiden werden Stents, die aus Metall oder Polymer gefertigt sind, zur Erhaltung der Lumengröße in das Gefäß implantiert. Der Stent verhält sich wie ein Gerüst, welches das Lumen offen hält. Der Stentaufbau weist eine zylindrische Hülse, die durch ein Maschenwerk begrenzt ist, miteinander verbundene Stents oder ähnliche Abschnitte auf. Beispielstents sind im
US-amerikanischen Patent Nr. 5,292,331 von Boneau,US-Patent-Nr. 6,090,127 von Globerman,US-Patent-Nr. 5,133,732 von Wiktor,US-Patent-Nr. 4,739,762 von Palmaz undU.-Patent-Nr. 5,421,955 von Lau undU.-Patent-Nr. 2003/065379 - Die Einführung eines Stents kann zu unerwünschten Reaktionen führen, wie zum Beispiel Entzündung, Infektion, Thrombose und Proliferation des Zellwachstums, das den Durchgangsweg verschließt. Stents werden mit Beschichtungen zur Abgabe von Arzneimitteln oder anderen therapeutischen Mitteln am Ort des Stents verwendet, welche der Prävention dieser Zustände dienen. Bei einigen Methoden zur Herstellung eines Stents, der zur Abgabe eines Arzneimittels konzipiert wurde, wird ein Stent-Rahmenwerk mit einem Arzneimittel beschichtet. Dies kann dazu führen, dass das Arzneimittel nur zu denjenigen Abschnitten des Gefäßes gelangt, die sich in direktem Kontakt mit dem Stent befinden, wodurch nicht mehr als 20% abgedeckt werden.
- Wenn ein Transplantat in Verbindung mit einem Stent verwendet wird, ist eine Abdeckung von 100% des Gefäßabschnitts möglich, der sich in direktem Kontakt mit dem Transplantat befindet. Mit Hilfe der Transplantatkomponente eines Stenttransplantats können auch Thrombosen minimiert, embolischen Ereignissen vorgebeugt sowie der Kontakt zwischen fissuriertem Plaque und hämatologischen Elementen im Blutstrom minimiert werden.
- Weiterhin kann die Vorrichtung aufgrund der Transplantatkomponente bei der Behandlung von Aneurysmen verwendet werden. Ein Aneurysma ist eine Ausbuchtung oder ein Sack, der sich in der Blutgefäßwand bildet. Die Kraft des normalen Blutdrucks im Aneurysma kann zum Zerreißen des Blutgefäßes führen. Aneurysmen sind in den meisten Fällen die Folge von Fettablagerungen an der Gefäßwand, können aber auch durch andere Ursachen wie Vererbung, Trauma oder Krankheiten bedingt sein, welche die Gefäßwand schwächen.
- Zur Behandlung von Aneurysmen sind eine Reihe von Methoden und Vorrichtungen entwickelt worden. Eine Standardbehandlung ist die Operation, bei der derjenige Gefäßabschnitt, in dem sich das Aneurysma gebildet hat, ersetzt wird. Einige Patienten sind für eine solche offene Operation keine geeigneten Kandidaten, und andere Patienten lehnen eine solche Behandlung aufgrund der stark invasiven Natur eines offenen Verfahrens ab.
- Eine alternative Behandlung ist eine Technik, die als endovaskuläres Stent-Grafting bekannt ist. Bei diesem Verfahren wird ein Stenttransplantat in das vom Aneurysma betroffene Gefäß platziert, um die geschwächte Gefäßwand abzustützen und dadurch eine Aneurysmaruptur zu verhindern.
- Stenttransplantate werden, wie auch Stents, mit Hilfe von Ballonkathetern oder anderen ausdehnbaren Vorrichtungen in den zu behandelnden Bereich gebracht. Bestimmte therapeutische Mittel können gegen die Bildung oder Progression von Aneurysmen wirksam sein. Daher kann es wünschenswert sein, zur Anlieferung dieser therapeutischen Mittel zum Ort des Aneurysmas ein Stenttransplantat zu verwenden.
- Es wurden verschiedene Methoden zur Herstellung von Stenttransplantaten entwickelt. Einige dieser Methoden, zum Beispiel solche, bei denen das Transplantatmaterial an den Stent angenäht wird, können sehr arbeitsintensiv sein oder nur eine ungenügende Adhärenz des Transplantatmaterials an den Stent gewährleisten. Bei anderen Methoden werden unter Umständen Vorrichtungen produziert, die das therapeutische Mittel nicht effizient anliefern können. Es wäre daher wünschenswert, eine Stenttransplantat-Anordnung zur Verfügung zu haben, welche die zuvor erwähnten und andere Nachteile überwinden kann.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Ein Aspekt der vorliegenden Erfindung ist eine Stenttransplantat-Anordnung, mit: mindestens einem Stent-Rahmenwerk; einem therapeutischen Mittel; und einem Transplantatelement, wobei das Transplantatelement das therapeutische Mittel und mindestens einen Abschnitt des Stent-Rahmenwerks umschließt; gekennzeichnet dadurch, dass das Transplantatelement über eine Vielzahl von Abgabeöffnungen verfügt, die über die Außenoberfläche des Stent-Rahmenwerks angeordnet sind.
- Ein anderer Aspekt der vorliegenden Erfindung ist ein System zur Behandlung von Gefäßzuständen, das einen Katheter und eine Stenttransplantation-Anordnung aufweist, wie sie oben definiert ist, sowie funktional mit dem Katheter gekoppelt ist.
- Ein weiterer Aspekt der vorliegenden Erfindung ist eine Methode zur Herstellung einer Stenttransplantat-Anordnung, mit: Anordnen eines Transplantatelements in einem Stent-Rahmenwerk; Stülpen des ersten und zweiten Endabschnitts (
122 ,124 ) des Transplantatelements über die äußere Oberfläche des Stent-Rahmenwerks; Befestigung des ersten Endabschnitts an den zweiten Endabschnitt; die Methode beinhaltet ferner das Eingeben eines therapeutischen Mittels wenigstens in einen Abschnitt des Transplantatelements und des Stent-Rahmenwerks vor dem Stülpen der ersten und zweiten Endabschnitte des Transplantatelements über die äußere Oberfläche des Stent-Rahmenwerks; gekennzeichnet insoweit, als dass die Methode ferner die Anordnung von Abgabeöffnungen (150 ) in den ersten und zweiten Endabschnitten des Transplantatelements vor dem Stülpen der Endabschnitte über die äußere Oberfläche des Stent-Rahmenwerks beinhaltet. - Die zuvor erwähnten und andere Eigenschaften und Vorteile der Erfindung werden noch klarer durch die folgende, genaue Beschreibung der zurzeit bevorzugten Ausführungsformen, die zusammen mit den begleitenden Zeichnungen gelesen werden muss. Die genaue Beschreibung und die Zeichnungen dienen rein illustrativen Zwecken der Erfindung und sollen diese nicht etwa einschränkten, wobei der Umfang der Erfindung durch die angehängten Ansprüche definiert ist.

- Ein Aspekt der vorliegenden Erfindung ist eine Stenttransplantat-Anordnung. Eine Ausführungsform der Anordnung, in Übereinstimmung mit der Erfindung, ist in
1 dargestellt. Die Anordnung besteht aus einen Stent-Rahmenwerk110 , einem Transplantatelement120 und einem therapeutischen Mittel130 . In der Abbildung fehlt ein Stück des Transplantatelements120 , um das Stent-Rahmenwerk110 sichtbar zu machen. Das Transplantatelement120 besteht aus einem ersten Endabschnitt122 und einem zweiten Endabschnitt124 . - Das Stent-Rahmenwerk
110 kann aus einer Vielzahl von medizinischen Implantatmaterialien hergestellt werden, wie zum Beispiel rostfreiem Stahl, Nitinol, Tantal, Keramik, Nickel, Titan, Aluminium, Polymermaterialien, MP35N, rostfreiem Stahl, Titan ASTM F63-83 (Grad 1), Niobium, hochkarätigem Gold K 19-22, aus einer Kombination der obigen und ähnlichen Materialien. Die Anordnung kann ein Stent-Rahmenwerk, wie abgebildet, oder viele Stent-Rahmenwerke aufweisen. - Transplantatelement
120 befindet sich benachbart zu der inneren Oberfläche des Stent-Rahmenwerks und ist über die äußere Oberfläche des Stent-Rahmenwerks gestülpt, wobei es mindestens einen Abschnitt des Stent-Rahmenwerks umschließt. Transplantatelement120 ist aus einem ausdehnbaren, biokompatiblen Material hergestellt, wie zum Beispiel Polytetrafluorethylen (PTFE), ausgedehntem Polytetrafluorethylen (ePTFE), gesponnenem Polyester oder Dacron. Das Material des Transplantatelements kann entweder porös oder nichtporös sein. Das Transplantatelement kann in Form eines Schlauchs hergestellt werden, oder eine flache Platte aus dem Material des Transplantatelements kann vor dem Anordnen des Transplantatelements in einem Stent-Rahmenwerk in eine schlauchähnliche Form gebracht werden. Das therapeutische Mittel130 befindet sich als Beschichtung entweder auf dem Transplantatelement oder dem Stent-Rahmenwerk oder auf beiden und wird von Transplantatelement120 umschlossen. Zum Beispiel kann das therapeutische Mittel130 sowohl auf der äußeren Oberfläche des Transplantatelements als auch des Stent-Rahmenwerks aufgebracht sein. Wenn das Transplantatelement über das Stent-Rahmenwerk gestülpt wird, ist das therapeutische Mittel von der Hülle umschlossen, die sich aus dem Abschnitt des Transplantatelements innerhalb des Stent-Rahmenwerks sowie dem Abschnitt bildet, der über die äußere Oberfläche des Stent-Rahmenwerks gestülpt ist. Das therapeutische Mittel130 kann zum Beispiel ein antineoplastisches Mittel, ein antiproliferatives Mittel, ein Antibiotikum, ein entzündungshemmendes Mittel, eine Kombination der obigen und ähnlicher Mittel sein. - Transplantatelement
120 ist in1 dargestellt, wobei der erste Endabschnitt122 und der zweite Endabschnitt124 über die äußere Oberfläche des Stent-Rahmenwerks gestülpt und aneinander befestigt sind, um das gesamte Stent-Rahmenwerk zu umschließen. Wie in der Abbildung dargestellt, sind die Endabschnitte überlappt und mit, zum Beispiel, Klebenieten oder durch Ultraschallbindung an die Befestigungspunkte140 gebunden. - Ein Experte auf diesem Gebiet wird erkennen, dass das Transplantatelement über eine Vielzahl von Anordnungen gestülpt werden kann, wobei alle oder nur einige Abschnitte des Stent-Rahmenwerks umschlossen werden. Anordnungsbeispiele, außer dem in
1 gezeigten, sind insbesondere die Folgenden:
Ein erster Endabschnitt kann über die äußere Oberfläche des Stent-Rahmenwerks gestülpt und am zweiten Endabschnitt an jedem Punkt entlang der gesamten Länge des Stent-Rahmenwerks befestigt werden, direkt jenseits des Stent-Rahmenwerk-Randes, durch das Stent-Rahmenwerk hindurch oder durch Rückfaltung des ersten Abschnittes innerhalb des Stent-Rahmenwerks und durch Befestigung des ersten Abschnitts am zweiten Abschnitt im Stent-Rahmenwerk. Alternativ können ein oder beide Endabschnitte über das Stent-Rahmenwerk gestülpt und an einen Mittelteil des Transplantatelements befestigt werden. - Um zu gewährleisten, dass der innere Abschnitt des Transplantatelements (der Abschnitt benachbart zu der inneren Oberfläche des Stent-Rahmenwerks) den Blutfluss durch die Stenttransplantat-Anordnung hindurch nicht staut oder behindert, ist der innere Abschnitt des Transplantatelements
130 am äußeren Abschnitt (der Abschnitt, der über die äußere Oberfläche des Stent- Rahmenwerks gestülpt ist) durch Öffnungen im Stent-Rahmenwerk befestigt. Wie in1 gezeigt, sind die Befestigungspunkte140 an verschiedenen Stellen quer über der Stenttransplantat-Anordnung angeordnet. - Die Stenttransplantat-Anordnung weist außerdem Abgabeöffnungen zur Eluierung eines therapeutischen Mittels auf. Die Öffnungen können zum Beispiel durch das Stanzen von Löchern in das Transplantatelement mit Hilfe einer Stanze, eines Lasers oder eines anderen geeigneten, in diesem Bereich anerkannten Werkzeug gebildet werden. Die Abgabeöffnungen
150 sind in2 dargestellt, die das Transplantatelement120 vor dem Stülpen des ersten Endabschnitts122 und des zweiten Endabschnitts124 über die äußere Oberfläche des Stent-Rahmenwerks110 zeigt. Sobald die Endabschnitte übergestülpt wurden, werden die Abgabeöffnungen über die äußere Oberfläche des Stent-Rahmenwerks angelegt, wobei diese das therapeutische Mittel vorzugsweise in die Gefäßwand und nicht in das Gefäßlumen abgeben. Die Elutionsrate des therapeutischen Mittels kann durch die Größe und Anzahl der Abgabeöffnungen für das therapeutische Mittel reguliert werden. Dort, wo die Anordnung keine Abgabeöffnungen für das therapeutische Mittel aufweist, kann die Elutionsrate durch die Porosität des Transplantatelements reguliert werden. -
3 zeigt eine Querschnittszeichnung einer Stenttransplantat-Anordnung, einschließlich eines therapeutischen Mittels, in Übereinstimmung mit der vorliegenden Erfindung. Die Anordnung besteht aus einem Stent-Rahmenwerk310 , einem Transplantatelement320 und einem therapeutischen Mittel330 . Das Transplantatelement320 umschließt ein therapeutisches Mittel330 und mindestens einen Abschnitt des Stent-Rahmenwerks310 . Die Anordnung kann Abgabeöffnungen aufweisen, die im Transplantatelement320 über die äußere Oberfläche des Stent-Rahmenwerks310 angeordnet sind. - Eine Stenttransplantat-Anordnung kann natürlich auch weder ein therapeutisches Mittel noch Abgabeöffnungen für ein therapeutisches Mittel aufweisen. Eine solche Anordnung kann als Abstützung einer schwachen oder erkrankten Gefäßwand oder als Gerüst (in offener oder ausgedehnter Form) zur Unterstützung des Gefäßlumens dienen, ohne ein therapeutisches Mittel abzugeben.
- Ein anderer Aspekt der vorliegenden Erfindung ist ein System zur Behandlung eines Gefäßzustandes. Eine Ausführungsform des Systems, in Übereinstimmung mit der vorliegenden Erfindung, ist in
4 dargestellt. Das System weist einen Katheter410 und eine Stenttransplantat-Anordnung420 auf, das mit dem Katheter funktional gekoppelt ist. Die Stenttransplantat-Anordnung420 weist ein Stent-Rahmenwerk430 und ein Transplantatelement440 auf. Das Transplantatelement440 besteht aus einem ersten Endabschnitt442 und einem zweiten Endabschnitt444 . Das Transplantatelement440 ist so dargestellt, dass ein Stück fehlt, um das Stent-Rahmenwerk430 sichtbar zu machen. - Der Katheter
410 kann einen Ballon zur Ausdehnung des Stents aufweisen oder eine Hülse, die sich zurückzieht, um die Ausdehnung eines selbst-ausdehnbaren Stents zuzulassen. Beide Katheterarten sind in der Fachwelt gut bekannt. Die Stenttransplantat-Anordnung420 ist in der Abbildung am Katheter410 befestigt dargestellt, der zur Einführung in ein Gefäß dient. - Das Stent-Rahmenwerk
430 kann aus einer Vielzahl von medizinischen Implantatmaterialien hergestellt werden, wie zum Beispiel rostfreiem Stahl, Nitinol, Tantal, Keramik, Nickel, Titan, Aluminium, Polymermaterialien, MP35N, rostfreiem Stahl, Titan ASTM F63-83 (Grad 1), Niobium, hochkarätigem Gold K 19-22, aus einer Kombination der obigen und ähnlichen Materialien. Die Anordnung kann ein Stent-Rahmenwerk aufweisen, wie dargestellt, oder viele Stent-Rahmenwerke. - Transplantatelement
440 ist aus einem ausdehnbaren, biokompatiblen Material hergestellt, wie zum Beispiel Polytetrafluorethylen (PTFE), ausgedehntem Polytetrafluorethylen (ePTFE), gesponnenem Polyester oder Dacron. Das Material des Transplantatelements kann entweder porös oder nichtporös sein. - Transplantatelement
440 ist in der Abbildung benachbart zu der inneren Oberfläche des Stent-Rahmenwerks dargestellt, wobei der erste Endabschnitt442 und der zweite Endabschnitt444 über die äußere Oberfläche des Stent-Rahmenwerks gestülpt, überlappt, und aneinander befestigt sind, um das gesamte Stent-Rahmenwerk zu umschließen. Ein Experte auf diesem Gebiet wird erkennen, dass das Transplantatelement noch über eine Vielzahl anderer Anordnungen als in4 . gestülpt werden kann, und das gesamte Stent-Rahmenwerk oder einen Teil des Stent-Rahmenwerks umschließen kann. - Das System weist ein therapeutisches Mittel auf, wie zum Beispiel ein antineoplastisches Mittel, ein antiproliferatives Mittel, ein Antibiotikum, ein entzündungshemmendes Mittel, Kombinationen der obigen und ähnliche Mittel. Wenn das therapeutische Mittel Teil des Systems ist, wird es vom Transplantatelement
440 umschlossen. Das therapeutische Mittel kann durch Poren im Transplantatelement abgegeben werden, wobei die Elutionsrate von der Porengröße reguliert wird. Das Transplantatelement440 enthält auch Abgabeöffnungen, die zum Beispiel durch mechanisches Durchbohren des Transplantatelements gebildet werden können. Die Abgabeöffnungen sind über der äußeren Oberfläche des Stent-Rahmenwerks angeordnet, wobei sie ein therapeutisches Mittel vorzugsweise in die Gefäßwand abgeben und nicht in das Gefäßlumen. Die Elutionsrate des therapeutischen Mittels kann durch die Größe und Anzahl der Öffnungen reguliert werden. - Ein weiterer Aspekt der vorliegenden Erfindung ist die Methode der Herstellung einer Stenttransplantat-Anordnung.
5 zeigt ein Flussdiagramm von einer Ausführungsform einer Methode in Übereinstimmung mit der vorliegenden Erfindung. - In dieser Ausführungsform ist ein Transplantatelement, zum Beispiel ein ePTFE-Schlauch, in dem Stent-Rahmenwerks angeordnet (Kasten
510 ). Dies kann zum Beispiel dadurch erreicht werden, dass ein Transplantatelement in das Stent-Rahmenwerk eingeführt wird oder das Transplantatelement auf eine Spindel gebracht wird und dann das Stent-Rahmenwerk über das Transplantatelement platziert wird. - Ein therapeutisches Mittel wird wenigstens in einen Abschnitt entweder des Stent-Rahmenwerks oder des Transplantatelements (Kasten
520 ) oder in beide eingegeben. Geeignete Methoden zum Eingeben des therapeutischen Mittels sind insbesondere: Eintauchverfahren, Aufsprühen, Tampondruck, Tintenstrahldruck, Einrollen, Anstreichen, Mikrosprühen, Wischen, elektrostatische Auftragung, Dampfauftragung, epitaktische Abscheidung und Kombinationen der erwähnten Verfahren. Nur als ein Beispiel: Das therapeutische Mittel kann in einem Gel enthalten sein, das auf die äußere Oberfläche des Stent-Rahmenwerks und des Transplantatelements aufgesprüht wird. - Die Abgabeöffnungen für das therapeutische Mittel werden im Transplantatelement gebildet (Kasten
530 ). Die Öffnungen können, zum Beispiel, durch Stanzen von Löchern in das Transplantatelement mit einem Ausstanzer, Laser oder einem anderen geeigneten Werkzeug gebildet werden. - In dieser Ausführungsform werden die Abgabeöffnungen sowohl im ersten als auch zweiten Endabschnitt des Transplantatelements gebildet, die dann über die äußere Oberfläche des Stent-Rahmenwerks gestülpt werden (Kasten
540 ). Die Abgabeöffnungen befinden sich daher über der äußeren Oberfläche des Stent-Rahmenwerks angeordnet und ermöglichen die Abgabe des therapeutischen Mittels vorzugsweise in die Gefäßwand und nicht in das Gefäßlumen. - Die Endabschnitte des Transplantatelements überlappen sich (Kasten
550 ) und sind aneinander befestigt (Kasten560 ). Die Endabschnitte können, zum Beispiel, durch Bekleben der Abschnitte mit Klebenieten befestigt werden. Eine Klebeniete kann durch Stanzen eines Lochs durch zwei oder mehrere Schichten des Transplantatelement-Materials gebildet werden, wobei ein Nietenende auf der distalen Oberfläche der inneren Schicht des Transplantatelements gebildet wird, wodurch die Klebeniete zurück durch alle Schichten des Transplantatelements gezogen und ein weiteres Nietenende auf der proximalen Oberfläche der äußeren Schicht des Transplantatelements gebildet wird. Andere geeignete Methoden, wie zum Beispiel Ultraschallbindung, können angewendet werden. Die beiden Endabschnitte sollten sicher befestigt werden um zu gewährleisten, dass das therapeutische Mittel vollständig eingeschlossen ist und nicht dort, wo die Endabschnitte befestigt sind, austreten kann. - Der innere Transplantatabschnitt (der Abschnitt benachbart zu der inneren Oberfläche des Stent-Rahmenwerks) ist durch das Stent-Rahmenwerk an mehreren Befestigungspunkten (Kasten
570 ) am äußeren Transplantatabschnitt befestigt (der Abschnitt, der über die äußere Oberfläche des Stent-Rahmenwerks gestülpt ist). Eine Befestigung der inneren und äußeren Abschnitte des Transplantatelements aneinander gewährleistet, dass der innere Abschnitt des Transplantatelements gegen die innere Oberfläche des Stent-Rahmenwerks positioniert bleibt und der Blutfluss durch die Stenttransplantat-Anordnung nicht gestaut und behindert wird.

110 ); einem therapeutischen Mittel (130 ); und einem Transplantatelement (120 ), wobei das Transplantatelement das therapeutische Mittel und wenigstens einen Abschnitt des Stent-Rahmenwerks umgibt; dadurch gekennzeichnet, dass das Transplantatelement mehrere Abgabeöffnungen (150 ) aufweist, die über einer äußeren Oberfläche des Stent-Rahmenwerks angeordnet sind. - Anordnung nach Anspruch 1, wobei das Transplantatelement das gesamte Stent-Rahmenwerk umschließt.
- Anordnung nach Anspruch 1 oder 2, wobei der erste und der zweite Endabschnitt des Transplantatelements umgestülpt sind, um das Stent-Rahmenwerk zu umschließen.
- Anordnung nach Anspruch 1, wobei der erste und der zweite Endabschnitt (
122 ,124 ) des Transplantatelements aneinander befestigt sind. - Anordnung nach Anspruch 1, wobei ein Abschnitt des Transplantatelements, das benachbart zu der inneren Oberfläche des Stent-Rahmenwerks angeordnet ist, durch Öffnungen in dem Stent-Rahmenwerk an einem Abschnitt des Transplantatelements, der über die äußere Oberfläche des Stent-Rahmenwerks gestülpt ist, befestigt ist.
- System zum Behandeln eines Gefäßzustandes bzw. -leidens, mit: einem Katheter (
410 ); und einer Stenttransplantat-Anordnung (420 ) nach einem vorhergehenden Anspruch, die mit dem Katheter funktional gekoppelt ist. - System nach Anspruch 6, wobei der Katheter einen Ballon aufweist, der verwendet wird, um den Stent auszudehnen.
- System nach Anspruch 6, wobei der Katheter eine Hülle aufweist, die sich zurückzieht, um die Ausdehnung des Stents zuzulassen.
- Verfahren für die Herstellung einer Stenttransplantat-Anordnung, das umfasst: Anordnen eines Transplantatelements (
120 ) in einem Stent-Rahmenwerk (110 ); Stülpen eines ersten und eines zweiten Endabschnitts (122 ,124 ) des Transplantatelements über eine äußere Oberfläche des Stent-Rahmenwerks; und Befestigen des ersten Endabschnitts an dem zweiten Endabschnitt; wobei das Verfahren ferner das Eingeben eines therapeutischen Mittels (130 ) wenigstens in einen Abschnitt einer äußeren Oberfläche des Transplantatelements oder des Stent-Rahmenwerks vor dem Stülpen des ersten und des zweiten Endabschnitts des Transplantatelements über eine äußere Oberfläche des Stent-Rahmenwerks umfasst; dadurch gekennzeichnet, dass das Verfahren ferner das Bilden von Abgabeöffnungen (150 ) in dem ersten und in dem zweiten Endabschnitt des Transplantatelements vor dem Stülpen der Endabschnitte über eine äußere Oberfläche des Stent-Rahmenwerks umfasst. - Verfahren nach Anspruch 9, wobei das Befestigen des ersten Endabschnitts an dem zweiten Endabschnitt das Überlappen der Endabschnitte und ihre Befestigung aneinander umfasst.
- Verfahren nach Anspruch 5, das ferner umfasst: Befestigen eines inneren Abschnitts des Transplantatelements an einem äußeren Abschnitt des Transplantatelements durch Öffnungen in dem Stent-Rahmenwerk.
