- Technisches Gebiet
- Die vorliegende Offenbarung betrifft allgemein ein elektrochirurgisches Instrument und insbesondere einen elektrochirurgischen Stift mit einem Bewegungsdetektor zur Steuerung seiner elektrochirurgischen Ausgabe.
- Stand der Technik
- Elektrochirurgische Instrumente wurden in den letzten Jahren von Chirurgen umfassend eingesetzt. Dementsprechend hat sich ein Bedarf nach Geräten entwickelt, die leicht zu handhaben, leicht zu bedienen sowie zuverlässig und sicher sind. Im Großen und Ganzen umfassen die meisten chirurgischen Instrumente typischerweise eine Vielzahl von in der Hand gehaltenen Stiften, z. B. elektrochirurgische Stifte, Zangen, Scheren und ähnliches, sowie elektrochirurgische Stifte, welche Energie auf eine Gewebestelle übertragen. Die elektrochirurgische Energie wird anfänglich von einem elektrochirurgischen Generator zu einer aktiven Elektrode übertragen, welche wiederum die elektrochirurgische Energie auf das Gewebe überträgt. In einem monopolaren System ist ein Rückführelektrodenpad unter dem Patienten positioniert, um den elektrischen Pfad zum elektroelektrochirurgischen Generator zu vervollständigen. In einer bipolaren Systemkonfiguration ist eine kleinere Rückführelektrode in Körperkontakt mit der chirurgischen Stelle oder unmittelbar daneben positioniert.
- Für die vorliegenden Zwecke umfasst der Begriff elektrochirurgische Fulguration das Anwenden eines elektrischen Funkens auf biologisches Gewebe, beispielsweise menschliches Fleisch oder das Gewebe der inneren Organe, ohne wesentliches Schneiden. Der Funke wird durch Stöße von elektrischer Radiofrequenzenergie erzeugt, welche von einem geeigneten elektrochirurgischen Generator erzeugt wurde. Im Allgemeinen wird die elektrochirurgische Fulguration verwendet, um Gewebe zu dehydrieren, zu schrumpfen, zu nekrotisieren (abzutöten) oder zu verkohlen. Als Ergebnis werden elektrochirurgische Fulgurationsinstrumente hauptsächlich dazu verwendet, das Bluten oder Nässen verschiedener chirurgischer Flüssigkeiten zu stoppen. Diese Vorgänge werden allgemein unter dem Begriff „Koagulation" zusammengefasst. Indes umfasst das elektrochirurgische „Schneiden" die eine Schneidwirkung erzeugende Verwendung des auf Gewebe angewandtenangewandten elektrischen Funkens. Im Gegensatz dazu umfasst das elektrochirurgische „Abdichten" die Verwendung einer einzigartigen Kombination von elektrochirurgischer Energie, Druck und einem Spaltabstand zwischen den Elektroden, um das Kollagen des Gewebes zu einer geschmolzenen Masse zu schmelzen.
- Es ist bekannt, dass gewisse elektrochirurgische Wellenformen für unterschiedliche Wirkungen bevorzugt sind. Z. B. ist eine durchgehende (d. h. stetige) sinusoidale Wellenform bevorzugt, um die Schneidwirkung der elektrochirurgischen Klinge in einem elektrochirurgischen Stift zu verstärken oder den operativen Effekt der beiden gegenüberliegenden Backenelemente zu verstärken. Eine Reihe von nicht zusammenhängenden elektrochirurgischen Hochenergieimpulsen sind bevorzugt, um die Koagulation des biologischen Gewebes zu verstärken. Andere Arten von elektrochirurgischen Wellenformen sind für das elektrochirurgische „Blending", „Verkürzen" oder Verschmelzen von Gewebe bevorzugt. Wie zu verstehen ist, werden diese Wellenformen typischerweise vom Generator geregelt und sind im Allgemeinen von der gewünschten Betriebsart, die vom Chirurgen zu Beginn (oder während) der Operation von Hand gewählt wurde, abhängig. Die Verwendung des Begriffs „elektrochirurgischer Griff" soll hier Instrumente umfassen, die ein Handstück umfassen, welches an einer aktiven Elektrode angebracht ist, und die verwendet werden, um Gewebe zu koagulieren, zu schneiden und abzudichten. Der Stift kann durch einen Handschalter (in Form eines herabdrückbaren Knopfes, der auf dem Handstück selbst vorgesehen ist) oder einen Fußschalter (in Form eines herabdrückbaren Pedals, das mit dem Handstück in Wirkverbindung steht) betätigt werden. Die aktive Elektrode ist ein elektrisch leitfähiges Element, welches gewöhnlich länglich ist und die Form einer dünnen flachen Klinge mit einem spitzen oder abgerundeten distalen Ende besitzen kann. Typischerweise sind Elektroden dieser Art im Stand der Technik als „Klingen"-Elektroden bekannt. Alternativ kann die aktive Elektrode eine längliche enge zylindrische Nadel umfassen, welche massiv oder hohl ist und ein flaches, abgerundetes, spitzes oder abgeschrägtes distales Ende besitzt. Typischerweise sind Elektroden dieser Art im Stand der Technik als „Schleifen"- bzw.. „Schlingen"-,"-, „Nadel"-"- oder „Kugel"-Elektroden bekannt.
- Wie oben erwähnt, ist das Handstück des Stiftes mit einer geeigneten elektrochirurgischen Quelle (z. B. einem Generator) verbunden, welcher die elektrochirurgische EnergieEnergie zuführt, die das leitende Element des elektrochirurgischen Stifts benötigt. Wenn an einem Patienten mit einem elektrochirurgischen Stift eine Operation durchgeführt wird, wird im Allgemeinen Energie vom elektrochirurgischen Generator durch die aktive Elektrode zum Gewebe an der Operationsstelle und dann durch den Patienten zu einer Rückführelektrode geführt. Die Rückführelektrode liegt typischerweise an einer geeigneten Stelle auf dem Körper des Patienten und ist durch ein Rückführkabel am Generator angeschlossen.
- Während der Operation drückt der Chirurg den Handschalter oder den Fußschalter, um den elektrochirurgischen Stift zu aktivieren. Dann stellt der Chirurg, abhängig von dem für die spezielle chirurgische Wirkung erwünschten Pegel der elektrochirurgischen Radiofrequenzenergie, den Leistungspegel des elektrochirurgischen Generators von Hand ein, indem er z. B. einen Knopf auf dem elektrochirurgischen Instrument dreht. Jüngst wurden elektrochirurgische Stifte entwickelt, die den Pegel der elektrochirurgischen Energie in Abhängigkeit von der Menge desdes von der aktiven Elektrode gefühlten WiderstandsWiderstands oder dem Ausmaß, um den der Handschalter vom Chirurgen gedrückt wurde, variieren. Beispiele einiger dieser Instrumente sind in den demselben Anmelder übertragenen vorläufigen US-Anmeldungen Nr. 60/398,620, eingereicht am 25. Juli 2002, nun
US-Veröffentlichungsnummer 2006/0058783 60/424,352 US 2004/0092927 - DARSTELLUNG DER ERFINDUNG
- Die vorliegende Erfindung betrifft einen elektrochirurgischen Stift gemäß Anspruch 1. BevorzugteBevorzugte Ausführungsformen sind in den abhängigen Ansprüchen definiert.
- Es wird in Betracht gezogen, dass der Sensor zum Erfassen der Bewegung des elektrisch leitfähigen Elements ein Kraft messender Wandler, ein Beschleunigungsmesser, ein optisches Positionierungssystem, ein Radiofrequenz-Positionierungssystem, ein Ultraschall-Positionierungssystem und/oder ein Magnetfeld-Positionierungssystem ist.
- Bevorzugt umfasst das elektrochirurgisch leitfähige Instrument eine durch es hindurch definierte Längsachse, und der Sensor misst eine axiale Bewegung des elektrisch leitfähigen Elements entlang der Längsachse, eine transversale Bewegung quer zur Längsachse des elektrisch leitfähigen Elements und/oder eine Drehbewegung um die Längsachse des elektrisch leitfähigen Elements. In einer Ausführungsform ist vorgesehen, dass die Quelle elektrochirurgischer Energie als Reaktion auf das Erfassen einer axialenaxialen Bewegung des elektrisch leitfähigen Elements entlang der Längsachse eine schneidende RF-Energieausgabe überträgt. In einer anderen Ausführungsform ist vorgesehen, dass die Quelle elektrochirurgischer Energie als Reaktion auf das Erfassen einer transversalen Bewegung des elektrisch leitfähigen Elements quer zur Längsachse überträgt.
- Es ist vorgesehen, dass der Sensor ein differenzieller Parallelplatten-Beschleunigungsmesser, ein Beschleunigungsmesser mit ausgeglichener, ineinander geschobener Kammstruktur, ein Beschleunigungsmesser mit versetzter, ineinander geschobener Kammstruktur und/oder ein filmartiger Beschleunigungsmesser ist. Bevorzugt umfasst der Sensor einen ersten Beschleunigungsmesser zum Erfassen einer Bewegung des elektrisch leitfähigen Elements in einer axialen Richtung entlang der Längsachse und einen zweiten Beschleunigungsmesser zum Erfassen der Bewegung des elektrisch leitfähigen Elements in einer transversalen Richtung quer zur Längsachse. Es wird auch in Betracht gezogen, dass der Sensor mindestens einen piezoelektrischen Film umfassen kann.
- In einer Ausführungsform ist vorgesehen, dass der erste Beschleunigungsmesser so gestaltet und geeignet ist, um ein Ausgabesignal an die Quelle elektrochirurgischer Energie zu übermitteln, das der axialen Bewegung des elektrochirurgischen Stifts entspricht, und der zweite Beschleunigungsmesser gestaltet und geeignet ist, um ein Ausgabesignal an die Quelle elektrochirurgischer Energie zu übermitteln, das der transversalen Bewegung des elektrochirurgischen Stifts entspricht. Bevorzugt ist der erste und zweite Beschleunigungsmesser jeweils ein differenzieller Parallelplatten-Beschleunigungsmesser, ein Beschleunigungsmesser mit ausgeglichener ineinander geschobener Kammstruktur, ein Beschleunigungsmesser mit versetzter ineinander geschobener Kammstruktur und/oder ein filmartiger Beschleunigungsmesser.
- In gewissen Ausführungsformen ist vorgesehen, dass die Quelle elektrochirurgischer Energie aufhört, die elektrochirurgische Energie zuzuführen, wenn der Sensor für eine vorbestimmte Zeitdauer keine Bewegung des elektrochirurgischen Stifts erfasst und/oder keine Bewegung des elektrochirurgischen Stifts oberhalb eines vorbestimmten Schellenwerts der Bewegung erfasst.
- Es wird weiter in Betracht gezogen, dass in gewissen Ausführungsformen die Quelle elektrochirurgischer Energie wieder anfängt, elektrochirurgische Energie zuzuführen, wenn der Sensor nach einer vorbestimmten Zeitdauer eine Bewegung des elektrochirurgischen Stifts erfasst und/oder eine Bewegung des elektrochirurgischen Stifts oberhalb eines vorbestimmten Schwellenwerts der Bewegung erfasst.
- Diese und andere Aufgaben werden im Folgenden durch die Beschreibung der Zeichnungen und die detaillierte Beschreibung der bevorzugten Ausführungsformen deutlicher veranschaulicht.
- KURZE BESCHREIBUNG DER ZEICHNUNGEN
- Die beigefügten Zeichnungen, welche in diese Beschreibung eingegliedert und einen Teil von ihr darstellen, veranschaulichen Ausführungsformen der Offenbarung und dienen, zusammen mit der oben gegebenen allgemeinen Beschreibung der Offenbarung und der im Folgenden gegebenen detaillierten Beschreibung der Ausführungsformen dazu, die Prinzipien der Offenbarung zu erläutern.

- Ausführungsformen des gegenwärtig offenbarten elektrochirurgischen Stifts werden nun im Detail mit Bezug auf die Zeichnungsfiguren beschrieben, wobei gleiche Bezugsziffern ähnliche oder identische Elemente identifizieren. In den Zeichnungen und der folgenden Beschreibung wird sich der Begriff „proximal", wie herkömmlich, auf das Ende des chirurgischen Stifts beziehen, welches dem Operateur am nächsten liegt, während sich der Begriff „distal" auf das Ende des elektrochirurgischen Stifts beziehen wird, welches am weitesten vom Operateur entfernt ist.
- Die Beschleunigung ist eine physikalische Eigenschaft, welche häufig erfasst oder gemessen werden muss. Die Beschleunigung ist als die Änderungsrate der Geschwindigkeit in Bezug auf die Zeit definiert. Beispielsweise wird die Beschleunigung häufig erfasst, um die Kraft oder Masse zu messen, oder um irgendeine Art von Steuerungssystem zu betätigen. Der Kern jeder Beschleunigungsmessung ist ein Beschleunigungsmesselement oder ein Kraftmesswandler. Der Wandler ist häufig ein mechanisches oder elektromechanisches Element (z. B. ein piezoelektrischer Wandler, ein piezoresistiver Wandler oder ein Dehnungsmesser), welches typischerweise mit elektrischen Signal- oder elektrischen Stromkreisen verbunden ist, um ein nützliches Ausgabesignal für einen Generator, Computer oder eine andere chirurgische Konsole bereitzustellen. Beispielhafte Wandler sind in den
US-Patenten Nr. 5,367,217 ,5,339,698 , und5,331,242 beschrieben. - Ein Beschleunigungsmesser ist als ein Instrument definiert, das die Beschleunigung oder die Schwerkraft, die in der Lage ist, eine Beschleunigung zu verleihen, misst. Eine andere Art von Kraft messendem Wandler ist ein Beschleunigungsmesser. Beispielhafte Beschleunigungsmesser sind in den
US-Patenten Nr. 5,594,170 ,5,501,192 ,5,370,639 ,5,377,545 ,5,456,111 ,5,456,110 und5,005,413 beschrieben. Andere Beispiele von elektrochirurgischen Instrumenten sind in denUS-Patenten Nr. 4,625,723 und6,649,882 beschrieben. - Verschiedene Arten von Beschleunigungsmessern sind bekannt. Eine erste Art von Beschleunigungsmesser enthält eine in großen Mengen mikrobearbeitete Silikonmasse, die an Silikonbalken aufgehängt ist, wobei Ionen-implantierte Piezowiderstände auf den Aufhängebalken die Bewegung der Masse fühlen. Eine zweite Art von Beschleunigungsmesser verwendet eine Kapazitätsänderung, um die Bewegung der Masse zu erfassen. Eine dritte Art von Beschleunigungsmesser erfasst die Beschleunigung, indem eine Änderung in der Resonanzfrequenz der Struktur als Ergebnis einer Verschiebung der physikalischen Belastung der Struktur gemessen wird. Es wird in Betracht gezogen, dass die Beschleunigungsmesser einen piezoelektrischen Film umfassen können, der in einen gewichtsbelasteten bedruckten flexiblen Schaltkreis eingelegt ist. Es istist auch vorgesehen, dass mindestens ein flexibler Widerstandsschaltkreis verwendet werden könnte, um statt der Beschleunigung die Position und/oder Orientierung des chirurgischen Instruments zu erfassen.
- Sich nun
1 zuwendend, ist eine teilweise gebrochene seitliche Aufrissansicht eines elektrochirurgischen Stifts dargestellt, der gemäß einer Ausführungsform der vorliegenden Offenbarung aufgebaut ist und allgemein mit der Bezugsziffer100 bezeichnet wird. Während die vorliegende Beschreibung auf elektrochirurgische Stifte gerichtet sein wird, wird in Betracht gezogen, dass die Merkmale und Konzepte der vorliegenden Offenbarung auch auf andere elektrochirurgische Instrumente, z. B. Dissektoren, Ablationsinstrumente, Sonden usw., angewendet werden können. Der elektrochirurgische Stift100 umfasst ein längliches Gehäuse102 , das gestaltet und geeignet ist, um eine Klingenaufnahme104 an seinem distalen Ende103 zu tragen, welche wiederum eine Elektrokauterisierungsklinge106 darin aufnimmt. Ein distales Ende108 der Klinge106 erstreckt sich distal von der Aufnahme104 , während ein proximales Ende110 der Klinge106 im distalen Ende103 des Gehäuses102 gehalten wird. Bevorzugt ist die ElektrokauterisierungsklingeElektrokauterisierungsklinge106 aus einem leitenden Material hergestellt, z. B. Edelstahl oder Aluminium, oder ist mit einem elektrisch leitfähigen Material beschichtet. - Wie gezeigt, ist der elektrochirurgische Stift
100 über ein Kabel112 mit einem herkömmlichen elektrochirurgischen Generator „G" verbunden. Das Kabel112 umfasst einen Übertragungsdraht114 , welcher den elektrochirurgischen Generator „G" mit dem proximalen Ende110 der ElektrokauterisierungsklingeElektrokauterisierungsklinge106 elektrisch verbindet. Das Kabel112 umfasst eine Steuerungsschleife116 , welche eine im Gehäuse102 gelagerte Bewegungserfassungsvorrichtung1124 (z. B. einen Beschleunigungsmesser) elektrisch mit dem elektrochirurgischen Generator „G" verbindet. - Lediglich im Zuge eines Beispiels kann der elektrochirurgische Generator „G" einer der folgenden Generatoren oder ihrer Äquivalente sein: der „FORCE FX"-,"-, „FORCE 2"-, „FORCE 4"-Generator, hergestellt von Valleylab, Inc., einem GeschäftszweigGeschäftszweig der Tyco Healthcare, LP, Boulder, Colorado. Bevorzugt kann die Energieausgabe des elektrochirurgischen Generators „G" variabel sein, um geeignete elektrochirurgische Signale zum Gewebeschneiden (z. B. 1 bis 300 W) und geeignete elektrochirurgische Signale für die Gewebekoagulierung (z. B. 1 bis 120 W) bereitzustellen. Ein Beispiel eines elektrochirurgischen Generators „G" ist im demselben Anmelder übertragenen
US-Patent Nr. 6,068,627 an OrszulakOrszulak et al. offenbart. Der in dem '627 Patent offenbarte elektrochirurgische Generator umfasst unter anderem einen Identifizierungsschaltkreis und einen Schalter darin. Im Allgemeinen reagiert der Identifizierungsschaltkreis auf Informationen, die von einem Generator empfangen werden, und überträgt ein Bestätigungssignal zurück zum Generator. Der Schalter währenddessen ist mit dem Identifizierungsschaltkreis verbunden und reagiert auf vom Identifizierungsschaltkreis empfangene Signale. - Der elektrochirurgische Stift
100 umfasst weiter einen Betätigungsknopf126 , der auf einer äußeren Oberfläche102 gelagert ist. Der Aktivierungsknopf126 ist betätigbar, um einen herabdrückbaren Schalter128 zu steuern, welcher verwendet wird, um die Zufuhr der zur ElektrokauterisierungsklingeElektrokauterisierungsklinge106 übertragenen elektrischen Energie zu steuern. - Zurück in
1 umfasst der elektrochirurgische Stift100 , wie oben erwähnt, einen Beschleunigungsmesser120 , welcher im Gehäuse102 gelagert ist. Der Beschleunigungsmesser124 ist operativ mit dem Generator „G" verbunden, welcher wiederum eine geeignete Menge an elektrochirurgischer Energie regelt und an die ElektrokauterisierungsklingeElektrokauterisierungsklinge106 überträgt und/oder die Wellenformausgabe aus dem elektrochirurgischen Generator „G" steuert. - Im Gebrauch betätigt der Chirurg den elektrochirurgischen Stift
100 , indem er den Aktivierungsknopf126 herabdrückt und es dadurch der elektrischen Energie erlaubt, zur ElektrokauterisierungsklingeElektrokauterisierungsklinge übertragen zu werden. Während der Chirurg bei herabgedrücktem Aktivierungsknopf126 den elektrochirurgischen Stift100 wiederholt entlang der X-Achse (d. h. auf stichartige Weise) bewegt, wie durch den zweispitzigen Pfeil „X" in1 gezeigt, überträgt der Beschleunigungsmesser124 ein entsprechendes Signal durch die Steuerschleife116 an den Generator „G". Der Generator „G" interpretiert dann das vom Beschleunigungsmesser124 empfangene Signal und überträgt wiederum eine entsprechende zerschneidende elektrochirurgische Energieausgabe (d. h. eine spezifische, mit dem Zerschneiden assoziierte Leistung und Wellenform) über den Übertragungsdraht114 zur Elektrokauterisierungsklinge106 . Elektrokauterisierungsklinge - Wenn andererseits der Chirurg den elektrochirurgischen Stift
100 in einer Richtung senkrecht zur X-Achse bewegt, wie z. B. durch den zweispitzigen Pfeil „Z" in1 gezeigt ist, überträgt der Beschleunigungsmesser124 ein entsprechendes Signal durch die Steuerungsschleife116 an den Generator „G". Der Generator „G" interpretiert dann das vom Beschleunigungsmesser124 empfangene orthogonale Signal und überträgt wiederum eine hämostatische elektrochirurgische Energieausgabe (d. h. eine spezifische, mit der Hämostase assoziierte Leistung und Wellenform) über den Übertragungsdraht114 zur ElektrokauterisierungsklingeElektrokauterisierungsklinge106 . - Dementsprechend wird der elektrochirurgische Stift der vorliegenden Offenbarung einen Chirurgen in die Lage versetzen, die Art der Ausgabe und/oder die Menge der zur ElektrokauterisierungsklingeElektrokauterisierungsklinge
106 geführten Energie dadurch zu regeln, dass er einfach den elektrochirurgischen Stift in einem speziellen Muster oder in einer speziellen Richtung bewegt. Auf diese Weise muss der Chirurg keine Knöpfe oder Schalter herabdrücken, welche auf dem elektrochirurgischen Stift100 angeordnet sind, um in der Elektrokauterisierungsklinge106 entweder eine zerschneidende oder eine hämostatische Energieausgabe zu erzeugen. Wie zu verstehen ist, muss der Chirurg keine Knöpfe oder Schalter auf dem Generator „G" einstellen, um entweder die zerschneidende oder die hämostatische Energieausgabe in der ElektrokauterisierungsklingeElektrokauterisierungsklinge106 zu erzeugen. - Beschleunigungsmesser, die zur Erfassung der Position oder zum elektrostatischen Antreiben („electrostatic forcing") geeignet sind, können in verschiedenen Gestaltungen mit festen und beweglichen Elektroden ausgebildet sein. Mehrere Ausführungsformen von Beschleunigungsmessern, die auf in der Ebene liegende Bewegungen empfindlich sind, sind in
2 gezeigt, zusammen mit einem orthogonalen Koordinatensystem. Wie insbesondere aus2A bis2C ersichtlich ist, ist ein differenzieller Parallelplatten-Beschleunigungsmesser allgemein mit150 bezeichnet. Der differenzielleDer differenzielle Parallelplatten-Beschleunigungsmesser150 umfasst eine Elektrode152 , die an einer Prüfmasse154 befestigt ist, welche entlang der Y-Achse beweglich ist und dadurch den Abstand zwischen der beweglichen Elektrode152 und den festen Elektroden156 und158 ändert. Die Bewegung der beweglichen Elektrode152 entlang der Y-Achse bewirkt entgegengesetzte Änderungen in der Kapazität des Kondensators, der durch die Elektrodenpaare152 ,156 und152 ,158 gebildet wird. In2B wird mit160 allgemein ein Beschleunigungsmesser mit ausgeglichener ineinander geschobener Kammstruktur bezeichnet. - Der Beschleunigungsmesser mit ausgeglichener ineinander geschobener Kammstruktur umfasst eine Elektrode
162 , die an einer Prüfmasse164 befestigt ist, welche entlang der Y-Achse beweglich ist und dadurch den Bereich der Überlappung zwischen der beweglichen Elektrode162 und einer festen herumgewickelten (umgebenden) Elektrode166 ändert. In2C ist mit170 allgemein ein Beschleunigungsmesser mit versetzter ineinander geschobener Kammstruktur bezeichnet. Der Bewegungsmesser mit versetzter ineinandergeschobener Kammstruktur170 umfasst eine Elektrode172 , die an einer Prüfmasse174 angebracht ist, welche entlang der Y-Achse beweglich ist und dadurch den Abstand zwischen der beweglichen Elektrode172 und einer festen herumgewickelten (umgebenden) Elektrode176 ändert. - Während ein einzigereinziger Beschleunigungsmesser
124 , der Änderungen in der Beschleunigung des elektrochirurgischen Stifts100 in der axialen (d. h. X-Richtung), der lateralen (d. h. der Y-Richtung) und der vertikalen Richtung (d. h. der Z-Richtung) messen kann, bevorzugt ist, wird in Betracht gezogen, dass ein Paar von identischen Beschleunigungsmessern oder unterschiedlichen Beschleunigungsmessern (d. h. Beschleunigungsmessern150 ,160 und170 ), wie in2A bis2C gezeigt ist, verwendet werden kann. Z. B. kann ein erster Beschleunigungsmesser, wie z. B. ein Beschleunigungsmesser mit versetzter ineinander geschobener KammstrukturKammstruktur170 , im elektrochirurgischen Stift100 angebracht sein, sodass eine Verlagerung der beweglichen Elektrode172 in der Y-Richtung zur Übertragung von zerschneidender elektrochirurgischer Energie mittels des Generators „G" an die Elektrokauterisierungsklinge106 führt, während ein zweiter Beschleunigungsmesser, wie z. B. ein weiterer Beschleunigungsmesser mit versetzter ineinandergeschobenerineinandergeschobener Kammstruktur170 , im elektrochirurgischen Stift100 senkrecht zum ersten Beschleunigungsmesser angebracht sein kann, sodass eine Verlagerung der beweglichen Elektrode172 in der X-Richtung zur Übertragung von hämostatischer elektrochirurgischer Energie durch den Generator „G" an die Elektrokauterisierungsklinge106 führt. - Es ist vorgesehen, dass jede Kombination von Beschleunigungsmessern im elektrochirurgischen Stift
100 in jeder Anzahl von Orientierungen vorgesehen sein kann, um Änderungen der Beschleunigung in einer beliebigen Anzahl von Richtungen zu messen, inklusive der Drehbeschleunigung (Y-Richtung und Z-Richtung). Es ist auch vorgesehen, dass jede beliebige Kombination von Beschleunigungen in der X-Richtung, der Y-Richtung und der Z-Richtung ebenfalls erfasst, gemessen und berechnet werden kann, um die elektrochirurgische Ausgabe aus dem Generator „G" zu bewirken. - Zusätzlich zu Beschleunigungsmessern wird in Betracht gezogen, dass viele andere Arten von Sensoren zu Erfassung der Bewegung der ElektrokauterisierungsklingeElektrokauterisierungsklinge
106 vorgesehen sein können. Andere Arten von Kraft messenden Wandlern können verwendet werden. Andere Arten, inklusive ohne Einschränkung optische Positionierungssysteme, Radiofrequenz-Positionierungssysteme, Ultraschall-Positionierungssysteme und Magnetfeld-Positionierungssysteme können verwendet werden. - Während eine aktive Elektrode in Form einer Klinge gezeigt und beschrieben wurde, wird in Betracht gezogen, dass jede Art von Spitze als aktive Elektrode des elektrochirurgischen Stifts
100 verwendet werden kann. Z. B.. kann die aktive Elektrode eine längliche zylindrische Nadel sein, welche massiv oder hohl ist und ein flaches, abgerundetes, spitzes oder abgeschrägtes distales Ende aufweist. - Es wird weiter in Betracht gezogen, dass die Menge an Zeit, die für die Übertragung der elektrochirurgischen Energie vom Generator „G" zur ElektrokauterisierungsklingeElektrokauterisierungsklinge
106 als Reaktion auf ein vom Beschleunigungsmesser124 empfangenes Ausgabesignal erforderlich ist, basierend auf dem vom Chirurgen gewünschten Ansprechverhalten eingestellt werden kann. Z. B. würde eine kurze Reaktionszeit als schneller ansprechend angesehen als eine vergleichsweisevergleichsweise längere Reaktionszeit. - Zusätzlich ist vorgesehen, dass der Beschleunigungsmesser
124 mit BewegungserfassungsalgorithmenBewegungserfassungsalgorithmen versehen wird, welche Energieabschaltsignale an den Generator „G" übertragen, wenn der elektrochirurgische Stift100 für eine längere Zeit bewegungslos gehalten wird oder abgelegt wird. Es wird in Betracht gezogen, dass mit dem Lauf der Zeit seit dem letzten Mal, an dem der elektrochirurgische Stift100 verwendet wurde, die Aktivierungsempfindlichkeit des elektrochirurgischen Stifts100 als Reaktion auf eine axiale, vertikale oder transversale Bewegung verringert werden kann. Somit würde der elektrochirurgische Stift100 mit geringerer Wahrscheinlichkeit unbeabsichtigt aktiviert werden, je mehr Zeit vergeht. Zusätzlich verbessert die Fähigkeit, den elektrochirurgischen Stift abzuschalten, wenn er nicht verwendet wird, die klinische Sicherheit der Vorrichtung. Der Bewegungserfassungsalgorithmuserzeugt effektiv einen „virtuellen Halfter", welcher den elektrochirurgischen Stift100 davor bewahrt, unbeabsichtigt betätigt zu werden. - Sich nun
3 und4 zuwendend, ist eine teilweise aufgebrochene perspektivische Ansicht eines elektrochirurgischen Stifts dargestellt, der gemäß einer anderen Ausführungsform der vorliegenden Offenbarung aufgebaut ist und allgemein mit der Bezugsziffer200 bezeichnet ist. Der elektrochirurgische Stift200 ist dem elektrochirurgischen Stift100 ähnlich und wird lediglich in dem Ausmaß im Detail besprochen, das notwendig ist, um Unterschiede im Aufbau und im Betrieb zu identifizieren. - Wie aus
3 und4 ersichtlich ist, umfasst der elektrochirurgische Stift200 einen Filmbeschleunigungsmesser oder -sensor224 , der im Gehäuse102 gelagert ist. Der Sensor224 umfasst bevorzugt ein Substrat226 , das aus einem elastomerischen Material hergestellt ist. Der Sensor224 umfasst weiter eine Reihe von Elektroden228 (im Interesse der Klarheit wurden lediglich vier Elektroden228a bis228d gezeigt), die um den Rand des Substrats226 herum positioniert sind. Der Sensor224 umfasst weiter eine Prüfmasse230 , die über elektrische Leitungen232 mit jeder Elektrode228 elektrisch verbunden ist. Die Prüfmasse230 ist in jeder Richtung entlang der Achsen X, Y und Z beweglich und ändert dadurch den Spaltabstand zwischen sich selbst und den Elektroden228 sowie den Widerstand durch die Leitungen232 . - Dementsprechend führt die Bewegung der Prüfmasse
230 entlang der X-, Y- und/oder Z-Achse zu einer Übertragung eines speziellen Signals durch die Steuerschleife116 an den Generator „G" (siehe1 ). Der Generator „G" interpretiert das vom Sensor224 empfangene spezielle Signal und überträgt wiederum eine entsprechende eindeutige elektrochirurgische Energieausgabe (z. B. eine spezifische Leistung und/oder Wellenform) über den ÜbertragungsdrahtÜbertragungsdraht114 an die Elektrokauterisierungsklinge106 . - Bei herabgedrücktem Betätigungsknopf
126 bewirkt z. B. eine Bewegung des elektrochirurgischen Stifts200 seitens des Chirurgen in Richtungen entlang der X-Achse (d. h. mitmit einer stichartigen Bewegung), dass der Sensor224 ein erstes charakteristisches Signal an den Generator „G" überträgt. Der Generator „G" interpretiert das erste charakteristische Signal und überträgt wiederum eine entsprechende zerschneidende elektrochirurgische Energieausgabe (d. h. eine spezifische Leistung und eine spezifische Wellenform, mit dem Zerschneiden assoziiert sind) an die Elektrokauterisierungsklinge106 . - In einem weiteren Beispiel bewirkt bei herabgedrücktem Betätigungsknopf
126 die Bewegung des elektrochirurgischen Stifts200 seitens des Chirurgen in Richtungen quer zur X-Achse, beispielsweise entlang der Y- und/oder der Z-Achse, dass der Sensor224 ein zweites charakteristisches Signal an den Generator „G" überträgt. Der Generator „G" interpretiert das zweite charakteristische Signal und überträgt wiederum eine entsprechende hämostatische elektrochirurgische Energieausgabe (d. h. eine spezifische Leistung und eine spezifische Wellenform, die mit der Hämostase assoziiert sind) an die Elektrokauterisierungsklinge106 . - Es ist vorgesehen, dass das Substrat
226 eine etwa konkave Gestalt hat. Auf diese Weise wird die Prüfmasse230 eine Neigung haben, zum Boden des Substrats226 zurückzukehren und sich effektiv automatisch zurückzusetzen, wenn der Chirurg den elektrochirurgischen Stift200 stillhält. In anderen Worten kann ein etwa konkaves Substrat226 selbstzentrierend sein und somit einen elektrochirurgischen Stift200 mit einer Selbstzurücksetzungsfähigkeit bereitstellen. Es wird auch in Betracht gezogen, dass andere Formen verwendet werden können. - Dementsprechend wird die elektrochirurgische Energieausgabe der elektrochirurgischen Stifte
100 ,200 durch die natürlichen Bewegungen der Hand des Chirurgen gesteuert und man muss nicht speziell daran denken, die entsprechende Energieausgabe von einer „zerschneidenden" Einstellung auf eine „hämostatische" Einstellung umzuändern und umgekehrt. - Es wird in Betracht gezogen, dass wenn der elektrochirurgische Stift
100 ,200 für eine vorbestimmte Zeitdauer bewegungslos und/oder unterhalb einem vorbestimmten Grenzwert der Bewegung gehalten wird (d. h. der Beschleunigungsmesser124 und/oder der Sensor224 erfassen für eine vorbestimmte Zeitdauer keine Bewegung des elektrochirurgischen Stifts100 oder200 und/oder erfassen eine Bewegung, welche sich unterhalb eines vorbestimmten Grenzwerts befindet), der elektrochirurgische Generator „G" keine elektrochirurgische Energie an die Elektrokauterisierungsklinge überträgt. Es wird weiter in Betracht gezogen, dass die Empfindlichkeit des elektrochirurgischen Stifts100 erhöht und/oder verringert werden kann, indem die Grenzwerte der Zeit und der Bewegung entsprechend eingestellt werden. - Es wird weiter in Betracht gezogen, dass der elektrochirurgische Generator „G" mit der Zufuhr elektrochirurgischer Energie an die Elektrokauterisierungsklinge beginnt oder sie wieder aufnimmt, wenn der Beschleunigungsmesser
124 und/oder der Sensor224 eine Bewegung des elektrochirurgischen Stifts100 oder200 erfasst, nachdem die vorbestimmte Zeitdauer verstrichen ist und/oder nachdem der vorbestimmte Grenzwert überschritten wurde. - Dem Vorangegangenen und mit Bezug auf die verschiedenen Zeichnungsfiguren wird der Fachmann auf dem Gebiet entnehmen, dass gewisse Abwandlungen an der vorliegenden Offenbarung vorgenommen werden können, ohne vom Schutzbereich der vorliegenden Offenbarung abzuweichen. Z. B. umfassen Ausführungsformen der vorliegenden Offenbarung einen elektrochirurgischen Stift, der zusätzlich zu dem oben besprochenen Sensor oder den Sensoren einen Knopf aufweist, um die elektrochirurgische Energieausgabe zu steuern. Während Ausführungsformen der elektrochirurgischen Instrumente gemäß der vorliegenden Offenbarung hier beschrieben wurden, ist nicht beabsichtigt, dass die Offenbarung hierauf beschränkt ist, und die obige Beschreibung sollte lediglich als beispielhafte Veranschaulichung bevorzugter Ausführungsformen ausgelegt werden.
Claims (14)
- Elektrochirurgischer Stift (
100 ), umfassend: ein längliches Gehäuse (102 ); und ein elektrisch leitendes Element (106 ), das in dem Gehäuse (102 ) gelagert ist und sich distal aus dem Gehäuse (102 ) erstreckt, wobei das elektrisch leitende Element (106 ) mit einer Quelle elektrochirurgischer Energie (G) verbindbar ist; und gekennzeichnet durch einen Bewegungssensor (224 ), der in dem Gehäuse (102 ) angeordnet ist, und mit der Quelle elektrochirurgischer Energie (G) in elektrischer Verbindung steht, wobei der Sensor (224 ) in der Lage ist, die Bewegung des elektrochirurgischen Stifts (100 ) zu erfassen und ein Signal an die Quelle elektrochirurgischer Energie (G) zu übermitteln, das sich auf die Bewegung des elektrochirurgischen Stifts (100 ) bezieht, wobei die Quelle elektrochirurgischer Energie (G) als Reaktion auf das von dem Sensor (224 ) übermittelte Signal elektrochirurgischer Energie zuführt. - Elektrochirurgischer Stift (
100 ) nach Anspruch 1, wobei der Sensor (224 ) zur Erfassung der Bewegung des elektrisch leitfähigen Elements (106 ) einen Beschleunigungsmesser, eine optisches Positionierungssystem, ein Radiofrequenz-Positionierungssystem und/oder ein Ultraschall-Positionierungssystem ist. - Elektrochirurgischer Stift (
100 ) nach Anspruch 1, wobei das elektrisch leitende Element (106 ) eine durch es definierte Längsachse aufweist und der Sensor (224 ) eine axiale Bewegung des elektrochirurgischen Stifts (100 ) entlang der Längsachse, eine Transversalbewegung quer zur Längsachse und/oder eine Drehbewegung um die Längsachse erfasst. - Elektrochirurgischer Stift (
100 ) nach Anspruch 3, wobei die Quelle elektrochirurgischer Energie (G) als Reaktion auf die Erfassung einer Axialbewegung des elektrochirurgischen Stifts (100 ) entlang der Längsachse eine sezierende RF-Energieausgabe übermittelt. - Elektrochirurgischer Stift (
100 ) nach Anspruch 3, wobei die Quelle elektrochirurgische Energie (G) als Reaktion auf die Erfassung einer transversalen Bewegung des elektrochirurgischen Stifts (100 ) quer zur Längsachse eine hämostatische RF-Energieausgabe übermittelt. - Elektrochirurgischer Stift (
100 ) nach Anspruch 1, wobei der Sensor (224 ) ein differenzieller Parallelplatten-Beschleunigungsmesser, ein Beschleunigungsmesser mit ausgeglichener ineinander geschobener Kammstruktur, ein Beschleunigungsmesser mit versetzter ineinander geschobener Kammstruktur und/oder ein filmartiger Beschleunigungsmesser ist. - Elektrochirurgischer Stift (
100 ) nach Anspruch 6, wobei der Sensor (224 ) umfasst: einen ersten Beschleunigungsmesser zur Erfassung einer Bewegung des elektrochirurgischen Stifts (100 ) in einer axialen Richtung entlang der Längsachse; und einen zweiten Beschleunigungsmesser zur Erfassung der Bewegung des elektrochirurgischen Stifts (100 ) in einer transversalen Richtung quer zur Längsachse. - Elektrochirurgischer Stift (

100 ) nach Anspruch 7, wobei der erste und zweite Beschleunigungsmesser jeweils ein differenzieller Parallelplattenbeschleunigungsmesser, ein Beschleunigungsmesser mit ausgeglichener ineinander geschobener Kammstruktur, ein Beschleunigungsmesser mit versetzter ineinander geschobener Kammstruktur und/oder ein filmartiger Beschleunigungsmesser ist. - Elektrochirurgischer Stift (
100 ) nach Anspruch 7, wobei der erste und zweite Beschleunigungsmesser jeweils mindestens einen Bewegungsmesser mit einem piezoelektrischen Film umfasst. - Elektrochirurgischer Stift (
100 ) nach Anspruch 1, wobei die Quelle elektrochirurgischer Energie (G) im Wesentlichen die Zufuhr der elektrochirurgischen Energie verringert, wenn der Sensor (224 ): keine Bewegung des elektrochirurgischen Stifts (100 ) für eine vorbestimmte Zeitdauer; und/oder keine Bewegung des elektrochirurgischen Stifts (100 ) oberhalb eines vorbestimmten Schwellenwerts der Bewegung erfasst. - Elektrochirurgischer Stift (
100 ) nach Anspruch 11, wobei die Quelle elektrochirurgischer Energie (G) im Wesentlichen die Zufuhr der elektrochirurgischen Energie verringert, wenn der Sensor (224 ): eine Bewegung des elektrochirurgischen Stifts (100 ) nach einer vorbestimmten Zeitdauer; und/oder eine Bewegung des elektrochirurgischen Stifts (100 ) oberhalb eines vorbestimmten Schwellenwerts der Bewegung erfasst. - Elektrochirurgischer Stift (
100 ) nach Anspruch 3, wobei die Quelle elektrochirurgischer Energie (G) im Wesentlichen die Zufuhr elektrochirurgischer Energie verringert, wenn der Sensor (224 ): keine Bewegung des elektrochirurgischen Stifts (100 ) für eine vorbestimmten Zeitdauer; und/oder keine Bewegung des elektrochirurgischen Stifts (100 ) oberhalb eines vorbestimmten Schwellenwerts der Bewegung erfasst. - Elektrochirurgischer Stift (
100 ) nach Anspruch 13, wobei die Quelle elektrochirurgischer Energie (G) im Wesentlichen die Zufuhr der elektrochirurgischen Energie erhöht, wenn der Sensor (224 ): eine Bewegung des elektrochirurgischen Stifts (100 ) nach der vorbestimmten Zeitdauer; und/oder eine Bewegung des elektrochirurgischen Stifts (100 ) oberhalb des vorbestimmten Schwellenwerts der Bewegung erfasst.
