Description
- Gebiet der Erfindung
- Die vorliegende Erfindung betrifft allgemein das Gebiet der Gewebereparatur und insbesondere zusammengesetzte Gerüstimplantate und Gerüstfixierungsvorrichtungen mit stiftartigen Ankern, die in einem in darunterliegendem Gewebe ausgebildeten Loch aufgenommen werden.
- Hintergrund der Erfindung
- Poröse Keramikmaterialien, wie beispielsweise Hydroxyapatit, lösliche Gläser und keramische Formen sind als Gerüste für das Einwachsen von Gewebe infolge der zusammensetzungsmäßigen und morphologischen Biokompatibilität verwendet worden. Die Porosität derartiger Materialien fördert beispielsweise die Zellinfiltration. Eine Vielzahl von Verfahren ist verwendet worden, um poröse keramische Gerüste (Prothesen) herzustellen, wie beispielsweise hydrothermales Behandeln von Tierknochen oder Korallen, Abbrennen von Polymerköpfen, die in einem Keramikkörper gemischt sind, Dampfablagerung auf Schaum, Infiltration von Polymerschaum mit einem Keramikschlicker und Ausbilden eines Keramikschlickers.
- Eine Beschränkung, die von porösen Keramikmaterialien gezeigt wird, ist ihre inhärente Sprödigkeit. Bemühungen, diese Beschränkung anzugehen, haben ein Zurückfüllen eines keramischen Schaums mit Monomerlösungen aus PMMA oder PLA umfasst, Abziehen von überschüssiger Lösung aus dem keramischen Schaum, dann Polymerisieren durch Vulkanisieren und/oder Trocknen, um dem keramischen Schaum etwas Zähigkeit zu verleihen, umfasst. Andere haben vorgeschlagen, massive oder poröse Polymerschichten an keramische Schaumstrukturen zu laminieren.
- Unabhängig von den vorgeschlagenen Verwendungen zusammen mit Keramiken sind Polymerschäume bei der Reparatur und Regeneration von Gewebe nützlich. Beispielsweise sind amorphe Polymerschäume verwendet worden, um Lücken in Knochen auszufüllen. Verschiedene Verfahren sind für das Herstellen von Polymerschäumen untersucht worden, einschließlich, z. B. auslaugungsfähige Stoffe, Vakuumschaumbildungstechniken, präzipitierte Polymergelmassen; und Polymerschmelzen mit flüchtigen Verbindungen, die bei Temperaturen oberhalb Raumtemperatur sublimieren. Die Bildung von biokompatiblen absorbierbaren Schäumen durch Lyophilisierung ist in EP-A-1 064 958 diskutiert.
- Hinsch et al. (
EP 0274898 ) beschreiben einen porösen Schaum mit offenen Zellen aus Polyhydroxysäuren für das Einwachsen von Blutgefäßen und Zellen. Der Schaum kann mit Fasern, Garnen, Litzen, gestrickten Fasern, Scrim und dergleichen verstärkt sein. - Athanasiou et al. (US Patent 5,607,474) haben die Verwendung einer zweischichtigen Polymerschaumvorrichtung zur Reparatur von osteochondralen Defekten an einer Stelle mit zwei unterschiedlichen Arten von vorhandenem Gewebe vorgeschlagen. Die zwei Polymerschichten werden getrennt hergestellt und in einem nachfolgenden Schritt miteinander verbunden. Eine jede der Schichten ist so konstruiert, dass sie Steifheits- und Komprimierbarkeitswerte aufweist, die jeweils Knorpel- und Knochengewebe entsprechen, was die Schnittstelle Knorpel/Knochen nachahmt. Die Vorrichtung von Athanasiou weist jedoch eine abrupte Änderung der Eigenschaften von einer Schicht zur nächsten auf, wohingegen die Verbindung von Knorpel und Knochen einen graduellen Übergang zeigt, wobei die Knorpelzellen graduell die Zellmorphologie und -orientierung ändern, abhängig von der Stelle relativ zur darunterliegenden Knochenstruktur. Darüber hinaus ändert sich die Kollagenfaserorientierung innerhalb der Matrix ebenfalls relativ zu ihrer Stelle in der Struktur.
- H. Levene et al., US Patent 6,103,255, beschreiben ein Verfahren zum Herstellen eines Gerüsts mit einer im Wesentlichen kontinuierlichen Polymerfaser mit einer Verteilung von großen und kleinen Porengrößen, wobei die kleinen Poren in den der Wänden der großen Poren enthalten sind.
- In einer von G. Niederauer et al. durchgeführten und in Biomaterials 21 (2000) 2561 berichteten Studie wurden Gerüste für die Reparatur des Längsknorpels aus Schichten aus Polymilchsäure/Polyglycolsäure (PLG) und Polymilchsäure/Polyglycolsäure hergestellt, die mit Fasern aus den gleichen Materialien, Bioglas oder Kalziumsulfat verstärkt waren. Die PLG-Schicht wurde in allen Fällen dadurch porös gemacht, dass eine präzipitierte Gelmasse aus Polymer unter Vakuum und bei erhöhten Temperaturen expandiert wurde. Die verstärkten Schichten wurden in ähnlicher Art und Weise nach Einbau der Verstärkung in die Polymerlösung und vor der Präzipitation der Polymergelmasse porös gemacht. Wenn einmal die zwei Schichten hergestellt waren, wurden sie miteinander verbunden unter Verwendung einer geringen Menge an Lösungsmittel, um die zwei Schichten zusammenzukleben.
- Die Verwendung eines porösen Polymers für die Zwecke der Konstruktion von Knorpel ist in dem Patent von T. Mahood et al. (
EP 1027897 A1 ) beschrieben, das ein mehrlagiges Polymergerüst offenbart, bei dem die Schichten durch sukzessives Tauchbeschichten oder durch die Anbringung der zwei Schichten an eine dritte angebracht sind. Die dritte Schicht wird als eine Barriere für die Zelldiffusion beschrieben, wodurch die Chondrozyten auf die Polymerschicht und die Osteoblasten auf die Keramikschicht beschränkt sind. - Eine Knorpelreparatureinheit gemäß dem Oberbegriff von Anspruch 1 ist in US-B-6251143 beschrieben.
- Ein Verfahren zum Herstellen eines Gerüstes zur Regeneration von Knorpel umfassend die Schritte des Formens einer Polymerlösung, ist in US-A-5723508 beschrieben.
- Kreklau et al. haben in Biomaterials 20 (1999) 1743 ein fibröses Polymervlies, das an einem porösen Keramikmaterial angebracht war, für die Zwecke der Kultivierung von Chrondrozyten in dem Polymergerüst bewertet, während gleichzeitig ein Knochenbildungsinduzierendes absorbierbares Material bereitgestellt wurde, um Gelenksknorpel zu simulieren. Bei dieser Studie wurde ein Fibrin-Zell-Lösung verwendet, um die Keramik- und Polymerschichten vermittels Verkapselung zu befestigen mit der Absicht, dass die Fasen in vitro miteinander wechselwirken würden, um eine mechanisch belastbare Verbindung zu erzeugen. Die Autoren diskutieren die Möglichkeit, die Oberflächen der Schichten mit Zähnen zu versehen, um die Scherstärke zu erhöhen. Es gibt jedoch keinen Mechanismus, durch den die zwei unterschiedlichen Schichten miteinander verblockt werden, um Delaminationskräften in Richtungen senkrecht zu der Laminatfunktion zu widerstehen und es gibt einen abrupten Übergang zwischen den beiden Schichten.
- Zusätzlich zu den Beschränkungen des Standes der Technik hinsichtlich der Zusammensetzung und Morphologie von Gewebegerüsten bleibt die Fixierung des Gerüstes an der Verletzungsstelle eine Herausforderung. Verschiedene Fixierverfahren sind untersucht worden, einschließlich Druckpassung des Gerüstes in den Defekt (was zum Verschieben oder zur Zerstörung des implantierten Gerüstes führen kann) oder Annähen des Gerüstes an die periostealen Flügel. Der letztere Ansatz ist nicht immer ideal, da die Geometrie des Gerüstes nicht mit der der periostealen Flügel übereinstimmen muss, oder die Flügel zerstört worden sind nicht lokalisiert werden können.
- Es wäre deshalb vorteilhaft, die oben erwähnten Beschränkungen durch ein Gerüst zu überwinden, das eine sichere Befestigung an einer Defektstelle liefert.
- Zusammenfassung der Erfindung
- Die Beschränkungen des Standes der Technik werden durch die vorliegende Erfindung überwunden, die ein prosthetisches Implantat wie in Anspruch 1 definiert mit einem Gewebegerüst und einer Fixierungseinrichtung mit einem Gerüstträger und einem verankernden Stift umfasst. Der verankernde Stift erstreckt sich von einer Oberfläche des Gerüstträgers in einem ausgewählten Winkel zu dem Gerüstträger, wobei der Gerüstträger innerhalb des Gerüstes eingebettet ist. Die vorliegende Erfindung stellt auch ein Verfahren zum Herstellen eines prosthetischen Implantates bereit, wie in Anspruch 22 definiert.

- Diese Erfindung umfasst eine implantierbare Vorrichtung mit einem Gerüstbestandteil und einem Fixierungsbestandteil zum mechanischen Halten des Gerüstbestandteils in Position relativ zu einem zu reparierenden Gewebedefekt. Der Gerüstbestandteil wird um eine Gerüstträgerplattform des Fixierungsbestandteiles herum ausgebildet und weist eine poröse biokompatible Polymerschicht auf, die an eine poröse Keramikschicht vermittels einer porösen Übergangsphasengrenzfläche befestigt ist. Die Gerüste sind besonders nützlich bei der Reparatur/Regeneration von Defekten, die an Verbindungen von Gewebetypen vorhanden sind, die eine Übergangs- oder Gradientenmorphologie/-physiologie aufweisen, wie beispielsweise die Verbindung von Knorpel und Knochen. Die vorliegende Erfindung kann verwendet werden, um eine Gewebeverbindung zu reparieren oder zu regenerieren, indem ein Zelltyp veranlasst wird, in der Polymerphase des Gerüstes zu proliferieren, und ein zweiter Zelltyp veranlasst wird, in der keramischen Phase des Gerüstes zu wachsen. Beispiele für derartige Verbindungsregenerationsstellen sind (a) Wirbelsäulenscheiben (nukleäre und Gelenkszellen kultiviert auf der Polymerphase und Osteoblasten kultiviert in der keramischen Phase); (b) Gelenks- oder Meniskusknorpel (Chondrozyten bzw. Fibrochondrozyten kultiviert auf der Polymerphase und Osteoblasten kultiviert in der Keramik). Die vorliegende Erfindung kann auch verwendet werden, um den Meniskus, Faserknorpel, Sehnen und Ligamente zu reparieren. Die Merkmale der porösen Polymerphase können so gesteuert werden, dass sie für eine gewünschte Anwendung passen, indem die gleichen Bedingungen während des Prozesses der Lyophilisierung oder Gefriertrocknung ausgewählt werden. Der poröse Polymerschaum kann direkt in die keramische Struktur lyophilisiert werden, wodurch ein mehrfasisches Material erzeugt wird, das aus einem Polymerschaum mit oder ohne Verstärkungsstrukturen, einer Phasengrenzflächenzone aus Polymerschaum, der innerhalb der porösen Keramik verblockt und darin verteilt ist, und der porösen Keramik zusammengesetzt ist. Ein Teil des Fixierungsbestandteiles kann zwischen der Polymer- und keramischen Schicht angeordnet sein, die strukturell integriert sind, um einer Ablösung des Gerüstbestandteils von dem Fixierungsbestandteils und/oder Delamination des zusammengesetzten Gerüstes unter in vivo Bedingungen zu widerstehen. Das Implantat kann teilweise oder vollständig absorbierbar sein.
- Der Phasengrenzflächenbereich weist einen mikroporösen Polymerschaum auf, der innerhalb der Makroporen einer porösen Keramik angeordnet ist. Die wechselseitige Durchdringung der zwei porösen Schichten erzeugt eine starke mechanische Verbindung, während gleichzeitig ein gradueller Übergang der Materialeigenschaften erzeugt wird für die Zwecke der Erzeugung unterschiedlicher Gewebe oder Co-Kultivieren verschiedener Arten von Zellen in engem Kontakt miteinander. Die verbindenden Poren und Kanäle erleichtern den Transport von Nährstoffen und/oder das Eindringen von Zellen in das Gerüst, wodurch das Einwachsen von Gewebe erleichtert wird und natürlicherweise auftretende Gewebeverbindungen stärker nachgeahmt werden. Die vorliegende Erfindung erleichtert somit die zelluläre Organisation und die Regeneration von Gewebeverbindungen mit normaler Morphologie und Physiologie. Die Zusammensetzungen und Merkmale des Gerüsts
20 sind in der parallel anhängigen Anmeldung, die gleichzeitig hiermit eingereicht wurde, beschrieben, die den Titel trägt „Porous Ceramic/Porous Polymer Layered Scaffolds For The Repair and Regeneration of Tissue",EP 1270025 A . - Die Merkmale eines Gerüstes gemäß der vorliegenden Erfindung können maßgeschneidert sein, um für eine spezielle Anwendung zu passen, indem die geeignete Keramik, das geeignete Polymer und geeignete Bedingungen zum Lyophilisieren des Polymers ausgewählt werden, um eine oder mehrere der folgenden Eigenschaften zu erhalten: (1) zwischenverbindende Polymerschäume, die an die poröse Keramik gebunden sind, (2) eine Vielzahl von Porositäten, die von 20 % bis etwa 98 % für den Polymerschaum reichen; (3) einen Gradienten hinsichtlich der Porengröße zwischen dem Polymer und der Keramik; (4) Kanäle, die durch den porösen Polymerschaum verlaufen für eine verbesserte Zellinvasion, Vaskularisierung und Nährstoffdiffusion; und (5) Mikromusterbildung aus Poren oder das Hinzufügen von anderen Polymerstrukturen auf der Oberfläche des Polymers für die zelluläre Organisation, oder um die Zellinvasion zu beschränken.
- Zusätzlich kann das Gerüst umfassen (1) poröse Verbundwerkstoffe mit einem Zusammensetzungsgradienten, um verschiedene Zellantwort auf unterschiedliche Materialien zu erzeugen oder daraus Nutzen zu ziehen; (2) Verstärkung mit gestrickten, geflochtenen, gewobenen oder nicht gewobenen Fasern oder Gittern, oder Fachwerkstrukturen, um die erwünschten mechanischen Eigenschaften zu verleihen; (3) Mischungen aus verschiedenen Polymerzusammensetzungen, um eine Polymerphase zu erzeugen, die Abschnitte aufweist, die mit unterschiedlichen Geschwindigkeiten zusammenbrechen werden; (4) mehrlagige zusammengesetzte Strukturen mit Schichten aus abwechselnden porösen Keramiken und Polymeren; (5) eine Polymerphase, die mit pharmazeutisch aktiven Verbindungen co lyophilisiert oder beschichtet ist; (6) eine keramische Phase, die mit pharmazeutisch aktiven Verbindungen, wie beispielsweise Wachstumsfaktoren, beschichtet ist, und/oder (7) Zellen, die vor oder zum Zeitpunkt der Implantation angezogen werden können.
- Unter Bezugnahme auf die
1 bis4 umfasst das Implantat10 einen Gerüstbestandteil20 und einen Fixierungsbestandteil30 (siehe3 ). Der Gerüstbestandteil20 weist eine Polymerphase22 und eine Keramikphase24 auf, die mechanisch an der Phasengrenzflächenregion26 mechanisch miteinander verblockt sind. Die Polymerphase22 , die Keramikphase24 und die Phasengrenzflächenregion26 weisen bevorzugte Poren23 ,25 ,27 mit einer offenen Zellstruktur auf. Wie in3 gezeigt, umfasst der Fixierungsbestandteil30 einen Gerüstträger32 und einen verankernden Stift34 . Obwohl nicht in den Figuren dargestellt, kann der verankernde Stift34 Rippen, Kerbverzahnungen oder andere Oberflächenrauheits- oder Oberflächeneingriffsmittel aufweisen, die die Befestigung des verankernden Stiftes an der Implantationsstelle, z. B. einem Loch in Knochengewebe, verbessern. Ein bevorzugter Fixierungsbestandteil zur Verwendung in der vorliegenden Erfindung ist in der europäischen Patentanmeldung 02251289, veröffentlicht alsEP 1234552 , beschrieben. - Das Implantat
10 muss eine strukturelle Integrität aufweisen, um eine leichte Handhabung in einer Operationsraumumgebung zu erleichtern, d. h. der Gerüstbestandteil20 und der verankernde Bestandteil30 dürfen sich nicht vor, während oder nach dem chirurgischen Eingriff trennen. Keine angemessene Stärke und angemessene physikalische Eigenschaften werden in dem Implantat durch die Auswahl von Materialien, die verwendet werden, um den Gerüst-20 und Fixierungs-30 Bestandteil auszubilden, und das Herstellungsverfahren entwickelt. - Wie in den
3 und4 gezeigt, verkapselt der Gerüstbestandteil20 den Gerüstträger32 des Fixierungsbestandteils30 vollständig. Diese Verkapselung dient als Befestigungsmittel für den Gerüstbestandteil20 an dem Fixierungsbestandteil30 .3 zeigt den Gerüstträger32 des Fixierungsbestandteils30 eingebettet in die Polymerphase22 , wobei die untere Oberfläche31 an die Phasengrenzflächenregion26 stößt. In einer in4 gezeigten bevorzugten Ausführungsform ist der Gerüstträger32 des Fixierungsbestandteiles30 vollständig in allen drei Bestandteilen22 ,24 ,26 des Gerüstbestandteiles20 eingekapselt. - Dies wird erreicht durch vollständiges oder teilweises Versenken des Gerüstträgers
32 in der keramischen Phase24 . - Das Eindringen der Polymerphase
22 in die Keramikphase24 befestigt die zwei Phasen22 ,24 sicher und stützt die spröde Struktur der porösen Keramikphase24 . Das Polymer22 fungiert als Kissen, um Einschlagsenergie aufzunehmen und die brüchige Keramik24 gegen katastrophal beschädigenden Belastungen abzuschirmen. Zusätzlich fördern die verbindenden Poren23 ,25 ,27 das Wachstum verschiedener Zelltypen, was die Regeneration unterschiedlicher benachbarter Gewebeschichten an einer verletzten Gewebeverbindung fördert. - Die Poren
25 in der Keramikphase24 sind untereinander verbunden und können so ausgewählt sein, dass sie eine Porengröße von 25 – 600 μm aufweisen, bevorzugterweise von 100 – 250 μm. Die Poren23 in der Polymerphase22 sind auch miteinander verbunden und reichen hinsichtlich ihrer Größe von 10 – 250 μm, bevorzugterweise 30 – 150 μm. Die Begriffe „Mikropore" und „Makropore" können verwendet werden, um zwei Größenskalen von Poren23 ,25 zu bezeichnen, die in dem Gerüst10 gefunden werden. Wenn die brüchige Keramikphase24 bricht, hält die Polymerphase22 in der Phasengrenzflächenregion26 den Gerüstbestandteil20 zusammen. Der zusammengesetzte Gerüstbestandteil20 erleichtert das Erzeugen einer starken Bindung zwischen unterschiedlichen Gewebearten, wie beispielsweise bei der Reparatur und Regeneration von Gelenksknorpel, Meniskus und Wirbelsäulenscheiben. - Das Einbetten des Fixierungsbestandteiles
30 innerhalb des Gerüstbestandteiles20 minimiert ihre kombinierte Stärke, wodurch die Tiefe des Lochs in dem Gewebe, das gemacht wird, um das Implantat10 aufzunehmen, und der damit verbundenen Schaden an dem Gewebe in der Nähe zu dem Defekt minimiert wird. Zusätzlich liefert die Keramikphase24 des Gerüstbestandteiles20 (zusammen mit der Polymerphase22 ) eine Stütze für das Hartgewebe, das das Implantat umgibt, wodurch die Wahrscheinlichkeit minimiert wird, dass das Hartgewebe in dem Bereich der Implantatvorrichtung10 zusammenbricht, und die Regeneration von mineralisiertem harten Gewebe (Knochen) erleichtert wird. - Die Implantatvorrichtung
10 kann hergestellt werden, indem ein verankernder Stift34 des Fixierungsbestandteiles30 durch ein Loch in der Keramikphase28 geführt wird, so dass der Gerüstträger32 auf der Spitze sitzt, oder in einer versenkten Region der Keramikphase24 . Diese Anordnung wird dann teilweise in ein Polymer-Lösungsmittel-System eingeführt, was dem Polymer-Lösungsmittelsystem erlaubt, in die poröse Keramikphase zu infiltrieren. Die Polymerphase22 wird dann geschäumt. Die erwünschten Polymere können durch Lyophilisierung, Schäumen mit superkritischem Lösungsmittel (d. h. wie inEP 464163 B1 32 Öffnungen darin aufweisen kann, durch den das Polymer22 die Keramik14 kontaktieren kann. - Es ist bevorzugt, das Polymer
22 durch Lyophilisierung oder Gefriertrocknung zu schäumen. Geeignete Verfahren zum Lyophilisieren von elastomeren Polymeren, um Schäume auszubilden, sind in dem folgenden Beispiel und in EP-A-1064598 beschrieben. -
5 veranschaulicht eine Gussvorrichtung50 mit der Form52 und der Tragpratze54 . Die Tragpratze54 umfasst ein Durchgangsloch53 , das über der Bohrung55 der Form52 ausgerichtet ist. Ein Halter40 mit dem Kopf42 und dem Stift44 kann verwendet werden, um den Fixierungsbestandteil30 zu halten. Der Stift44 wird durch das Loch53 geführt, wobei der Kopf42 gegen die Tragpratze54 stößt. Der Stift44 ist in die Bohrung46 eingeführt, wodurch der Fixierungsbestandteil30 über der Bohrung55 durch Presspassung pendelnd gehalten wird. - Das Polymer-Lösungsmittel-System
28 wird in die Bohrung55 der Form52 auf ein Niveau infundiert, so dass das Polymer-Lösungsmittel-System28 die Keramikphase24 kontaktiert. Das Polymer-Lösungsmittel-System28 weist eine niedrige Viskosität auf und dochtet vermittels Kapillarwirkung in die Poren25 der keramischen Phase. Andere Verfahren zum Infiltrieren umfassen, sind aber nicht darauf beschränkt, Injizieren des Polymer-Lösungsmittelsystems in die Keramik24 unter Druck und Vakuum-unterstützte Infiltration. Die Ausrichtung des Fixierungsbestandteiles30 innerhalb des Polymer-Lösungsmittel-Systems28 bestimmt die Orientierung des Fixierungsbestandteiles30 innerhalb des Implantates10 . Obwohl die Mittel zum Ausrichten des Fixierungsbestandteiles30 in der Bohrung55 der Form52 die Tragpratze54 und eine Reibungspassung zwischen dem Verbindungsstift44 des Halters40 und dem Fixierungsbestandteil30 umfassen, sollten andere Mittel, um das gleiche Ziel zu erreichen, für die Fachleute auf dem Gebiet offenkundig sein. Die Form52 kann aus irgendeinem Material hergestellt sein, die mit dem Polymer-Lösungsmittel-System nicht28 chemisch reagiert und ist bevorzugterweise aus einem wärmeleitfähigen Material hergestellt. - Die Formvorrichtung
50 ist in einem Lyophilisator (Gefriertrockner) angeordnet, um einem gerichteten Kühlen durch die Wand der Form52 unterzogen zu werden, die mit dem Gestell des Lyophilisators in Kontakt ist, das einem Thermozyklus unterzogen wird. Die Wärmeübertragungsfront bewegt sich von dem Lyophilisatorgestell durch die Formwand in das Polymer-Lösungsmittelsystem28 nach oben. Wenn die Temperatur der Polymerlösung unterhalb des Gelierungs- und/oder Gefrierpunktes fällt, wird sie sich in Polymer- und Lösungsmittelphase auftrennen und somit die Zell-/Schaumstruktur ergeben. - Die Porenmorphologie, die sich aus dem Gefrierschritt ergibt, ist eine Funktion von Lösungsthermodynamiken, Gefriergeschwindigkeit, Temperatur, auf die abgekühlt wird, Konzentration der Lösung, der Anwesenheit von Verstärkungselementen, der Anwesenheit einer verbindenden Schicht, das Auftreten von homogener oder heterogener Nukleation etc. Detaillierte Beschreibungen dieser Phasentrennungsphänomene sind in der Technik bekannt und können in den Literaturstellen „Microcellular foams via phase separation" von A. T. Young, J. Vac. Sci. Technol., A 4(3), Mai/Juni 1986, und „Thermodynamics of Formation of Porous Polymeric Membrane From Solutions" von S. Mazuda, Polymer J. 23(5), (1991) 435 gefunden werden. Der Lyophilisierungsprozess kann deshalb dazu verwendet werden, um die Polymer- und Keramikschichten
22 ,24 miteinander zu verbinden, während gleichzeitig ein Schichtmaterial mit der richtigen Porenstruktur erzeugt wird, um Gewebe zu regenerieren. - Die poröse keramische Phase
24 des Gerüstes kann aus Mono-, Di-, Tri-, α-tri-, β-tri- und tetra-Kalziumphosphat, Hydroxyapatit, Fluorapatiten, Kalziumsulphaten Kalziumfluoriden, Kalziumoxiden, Kalziumcarbonaten, Magnesiumkalziumphosphaten, Biogläsern und Mischungen davon zusammengesetzt sein. Es gibt eine Anzahl geeigneter poröser biokompatibler keramischer Materialien, die derzeit auf dem kommerziellen Markt erhältlich sind, wie beispielsweise Surgibone (Unilab Surgibone Inc., Kanada), Endobon (Merck Biomaterial France, Frankreich), Ceros (Mathies A. G., Bettlach, Schweiz) und Interpore (Interpore Irvin CA, Vereinigte Staaten). - Alternativ kann die Keramikphase
14 in der Form einer porösen Polymermatrix mit Einschlüssen aus kurzen Keramikfasern oder -partikeln vorliegen. Diese alternative Keramikphase14 kann durch herkömmliche Verfahren für Arbeitskunststoffe hergestellt werden, wie beispielsweise Spritzguss, wobei ihre Porosität durch auslaugbare Einschlüsse, Formen mit porenbildenden Stiften oder Bohren bereitgestellt wird. - Die Polymerphase
22 kann entweder ein natürliches oder ein synthetisches Polymer oder eine Kombination aus beiden sein. Natürliche Biopolymer umfassen Kollagen, Elastin, Alginat, Chitin, Hyaluronsäure und andere. Beispiele für geeignete synthetische biokompatible, bioabsorbierbare Polymere, die verwendet werden könnten, umfassen aliphatische Polyester, Polyaminosäuren, Copoly(ether-ester), Polyalkylenoxalate, Polyamide, Poly(iminocarbonate), Polyorthoester, Polyoxaester, Polyamidoester, Polyoxaester, die Amingruppen enthalten, Polyanhydride, Polyphosphazene, Biomoleküle und Mischungen davon. - Für die Zwecke dieser Erfindung umfassen aliphatische Polyester, sind aber nicht darauf beschränkt, Homopolymere und Copolymere von Lactid (was Milchsäure, D-, L- und meso-Lactid umfasst), Glycolid (einschließlich Glycolsäure), ε-Caprolacton, p-Dioxanon(1,4-Dioxan-2-on), Trimethylencarbonat(1,3-Dioxan-2-on), Alkylderivate von Trimethylcarbonat, δ-Valerolacton, β-Butyrolacton, γ-Butyrolacton, ε-Decalacton, Hydroxybutyrat, Hydroxyvalerat, 1,4-Dioxepan-2-on (einschließlich seines Dimers 1,5,8,12-Tetraoxacyclotetradecan-7,14-dion), 1,5-Dioxepan-2-on, 6,6-Dimethyl-1,4-dioxan-2-on, 2,5-Diketomorpholin, Pivalolacton, α,α-Diethylpropiolacton, Ethylencarbonat, Ethylenoxalat, 3-Methyl-1,4-dioxan-2,5-dion, 3,3-Diethyl-1,4-dioxan-2,5-dion, 6,8-Dioxabicycloctan-7-on und Polymermischungen davon.

- Polyanhydride für die Zwecke dieser Erfindung umfassen jene aus Disäuren der Form HOOC-C6H4-O-(CH2)m-O-C6H4-COOH, wobei m eine ganze Zahl im Bereich von 2 bis 8 ist und Copolymere davon mit aliphatischen α-ω-Disäuren mit bis zu 12 Kohlenstoffen.
- Polyoxaester, Polyoxamide und Polyoxaester umfassend Amine und/oder Aminogruppen für die Zwecke dieser Erfindung umfassen jene, die in einem oder mehreren der folgenden US-Patente beschrieben sind: 5,464,929; 5,595,751; 5,597,579; 5,607,687; 5,618,552; 5,620,698; 5,645,850; 5,648,088; 5,698,213; 5,700,583; und 5,859,150. - Aliphatische Polyester sind bevorzugt für das Herstellen der Polymerphase
22 . Aliphatische Polyester können Homopolymere, Copolymere (Zufalls-, Block-, Kettensegment-, abgestufte Blöcke, Pfropf-, Dreifachblock- etc.) mit einer linearen, verzweigten oder Stern-Struktur sein. Die bevorzugte Morphologie der Copolymerketten ist linear. Geeignete Monomere zum Herstellen von aliphatischen Homopolymeren und Copolymeren können ausgewählt sein aus der Gruppe bestehend aus, aber nicht darauf beschränkt, Milchsäure, Lactid (einschließlich L-L-,D-, meso- und D,L-Mischungen), Glycolsäure, Glycolid, ε-Caprolacton, p-Dioxanon (1,4-Dioxan-2-on), Trimethylencarbonat (1,3-Dioxan-2-on), δ-Valerolacton, β-Butyrolacton, ε-Decalacton, 2,5-Diketomorpholin, Pivalolacton, α,α-Diethylpropiolacton, Ethylencarbonat, Ethylenoxalat, 3-Methyl-1,4-dioxan-2,5-dion, 3,3-Diethyl-1,4-dioxan-2,5-dion, γ-Butyrolacton, 1,4-Dioxepan-2-on, 1,5-Dioxepan-2-on, 6,6-Dimethyl-dioxepan-2-on, 6,8-Dioxabicycloctan-7-on und Kombinationen davon. - Elastomere Copolymere sind auch besonders nützlich bei der vorliegenden Erfindung. Geeignete bioabsorbierbare, biokompatible Elastomere umfassen, sind aber nicht darauf beschränkt, jene, die aus der Gruppe ausgewählt sind, die aus Elastomercopolymeren aus ε-Caprolacton und Glycolid (bevorzugterweise mit einem Molverhältnis von ε-Caprolacton zu Glycolid von etwa 35:65 bis etwa 65:35, bevorzugtererweise von 45:45 bis 35:65); Elastomercopolymere aus ε-Caprolacton und Lactid, einschließlich L-Lactid, D-Lactid-Mischungen davon oder Milchsäure-Copolymere (bevorzugterweise mit einem Molverhältnis von ε-Caprolacton zu Lactid von etwa 35:65 bis etwa 65:35 und bevorzugtererweise von 45:55 bis 30:70 oder von etwa 95:5 bis etwa 85:15); Elastomercopolymere aus p-Dioxanon (1,4-Dioxan-2-on) und Lactid einschließlich L-Lactid, D-Lactid und Milchsäure (bevorzugterweise mit einem Molverhältnis von p-Dioxanon zu Lactid von etwa 40:60 bis etwa 60:40), Elastomercopolymere aus ε-Caprolacton und p-Dioxanon (bevorzugterweise mit einem Molverhältnis von ε-Caprolacton zu p-Dioxanon von etwa von 30:70 bis etwa 70:30); Elastomercopolymere aus p-Dioxanon und Trimethylcarbonat (bevorzugterweise mit einem Molverhältnis von p-Dioxanon zu Trimethylencarbonat von etwa 30:70 bis etwa 70:30); Elastomercopolymere aus Trimethylencarbonat und Glycolid (bevorzugterweise mit einem Molverhältnis von Trimethylencarbonat und Glycolid von etwa 30:70 bis etwa 70:30); elastomere Copolymere aus Trimethylencarbonat und Lactid umfassend L-Lactid, D-Lactid, Mischungen davon oder Milchsäurecopolymere (bevorzugterweise mit einem Molverhältnis von Trimethylencarbonat zu Lactid von etwa 30:70 bis etwa 70:30); und Mischungen davon besteht. Beispiele für geeignete bioabsorbierbare Elastomere sind auch in den US-Patenten 4,045,418, 4,057,537 und 5,468,253 beschrieben.
- In den bevorzugten Ausführungsformen dieser Erfindung wird das Elastomer, aus dem die Schäume hergestellt sind, eine prozentuale Verlängerung aufweisen, die größer ist als etwa 200 % und bevorzugterweise größer als etwa 500 % ist. Die Eigenschaften, die den Elastizitätsgrad des bioabsorbierbaren Elastomers bestimmen, werden erreicht, während eine Zugfestigkeit von mehr als etwa 500 psi, bevorzugterweise mehr als etwa 1000 psi, und eine Reißfestigkeit von mehr als etwa 50 Pfund/Zoll, bevorzugterweise mehr als etwa 80 Pfund/Zoll erreicht wird.
- Das Polymer oder Copolymer, das für die Ausbildung der Polymerphase
22 für eine spezielle Anwendung geeignet ist, hängt von verschiedenen Faktoren ab. Die chemische Zusammensetzung, räumliche Verteilung der Phasen, das Molekulargewicht des Polymers und das Ausmaß an Kristallinität diktieren alle zusammen das Ausmaß des in vitro und in vivo Verhaltens des Polymers. Die Auswahl des Polymers, um Schäume für die Geweberegeneration herzustellen, hängt jedoch weitgehend ab von den folgenden Faktoren (ist aber nicht darauf beschränkt): (a) Bioabsorptions- (oder Bioabbau-) Kinetiken; (b) in vivo mechanische Leistung; (c) Zellantwort auf das Material im Sinne von Zellanheftung, Proliferation, Migration und Differenzierung und (d) Biokompatibilität. - Die Fähigkeit der Polymerphase, zeitig in vivo zu resorbieren, ist kritisch. Die Unterschiede in der Absorptionszeit unter in vivo Bedingungen kann auch die Grundlage für das Kombinieren zweier unterschiedlicher Copolymere sein. Beispielsweise wird ein Copolymer aus 35:65 ε-Caprolacton und Glycolid (ein vergleichsweise schnell absorbierendes Polymer) mit 40:60 ε-Caprolacton und (L)-Lactidcopolymer (einem vergleichsweise langsam absorbierenden Polymer) gemischt, um einen Schaum auszubilden. Ein derartiger Schaum könnte verarbeitet werden, um mehrere unterschiedliche physikalische Strukturen zu ergeben, abhängig von der verwendeten Technik. Die zwei Phasen können entweder zufällig miteinander verbundene bikontinuierliche Phasen sein, oder einen Gradienten oder eine Schichtzusammensetzung mit einer integrierten Phasengrenzfläche zwischen den Phasenschichten aufweisen. Die Mikrostruktur dieser Schäume kann optimiert sein, um die erwünschten anatomischen Merkmale des Gewebes, das regeneriert werden soll, zu regenerieren oder zu reparieren.
- Geeignete Lösungsmittel für die bevorzugten absorbierbaren aliphatischen Polyester, die die keramischen Schäume nicht beeinflussen werden, umfassen, sind aber nicht darauf beschränkt, Lösungsmittel, die aus der Gruppe ausgewählt sind bestehend aus Ameisensäure, Ethylformat, Essigsäure, Hexafluorisopropanol (HFIP), zyklische Ether (d.h. THF, DMF und PDO), Aceton, Acetate mit C2-C5 Alkohol (wie beispielsweise Ethylacetat und t-Butylacetat), Glyme (d. h. Monoglyme, Ethylglyme, Diglyme, Ethyldiglyme, Triglyme, Butyldiglyme und Tetraglyme), Methylethylketon, Dipropylenglycolmethylether, Lactone (wie beispielsweise γ-Valerolacton, δ-Valerolacton, β-Butyrolacton, γ-Butyrolacton), 1,4-Dioxan -1,3-Dioxolan, 1,3-Dioxolan-2-on (Ethylencarbonat), Dimethylencarbonat, Diethylcarbonat, Benzol, Toluol, Benzylalkohol, p-Xylol, Naphthalen, Tetrahydrofuran, N-Methyl-pyrrolidon, Dimethylformamid, Chloroform, 1,2-Dichlormethan, Morpholin, Dimethylsulfoxid, Hexafluoracetonsesquihydrat (HFAS), Anisol und Mischungen davon. Unter diesen Lösungsmitlln ist das bevorzugte Lösungsmittel 1,4-Dioxan. Eine homogene Lösung aus dem Polymer in dem Lösungsmittel wird unter Verwendung von Standardtechniken hergestellt.
- Zusätzlich kann das Polymer-Lösungsmittel-System
28 durch verschiedene Verstärkungen, wie beispielsweise Folien, Scrims, gewobene, nicht-gewobene, gestrickte oder geflochtene Textilstrukturen verstärkt sein, die darin aufgenommen sind. Zusätzlich zum Verändern der mechanischen Eigenschaften des Polymers22 können Verstärkungen verwendet werden, (i) um das in vitro Verhalten des Polymers22 zu modifizieren, beispielsweise durch Einführen eines unterschiedlichen in vitro Profils; (ii) als ein Träger für die gesteuerte Freisetzung eines Arzneimittels und (iii) als ein Träger für mikroelektromechanische Systeme (MEMS). - Feststoffe können zu dem Polymer-Lösungsmittel-System
28 während des Verarbeitens des Implantats10 hinzugegeben werden, um als Puffer, Verstärkungsmaterialien, Porositätsmodifizierer und/oder strahlenundurchlässige Marker zu wirken, um eine Bildgebung nach Implantation zu erlauben. Geeignete Feststoffe umfassen, sind aber nicht darauf beschränkt, Partikel aus demineralisiertem Knochen, Kalziumphosphatpartikel, Kalziumcarbonatpartikel für die Knochenreparatur, auslaugbare Feststoffe für Porenerzeugung und Partikel aus bioabsorbierbaren Polymeren, die in den Lösungsmitteln nicht löslich sind, als Verstärkungsagenzien oder für die Ausbildung von Poren, während sie absorbiert werden. - Geeignete auslaugbare Feststoffe umfassen, sind aber nicht darauf beschränkt, nicht toxische auslaugbare Materialien, wie beispielsweise Salze (d. h. Natriumchlorid, Kaliumchlorid, Kalziumchlorid, Natriumtartrat, Natriumcitrat und dergleichen), biokompatible Mono- und Disaccharide (d. h. Glucose, Fructose, Dextrose, Maltose, Lactose und Saccharose), Polysaccharide, z. B. Stärke, Alginat, wasserlösliche Proteine (d. h. Gelatine und Agarose) und Paraffin. Allgemein werden alle diese Materialien einen durchschnittlichen Durchmesser von weniger als etwa 1 mm aufweisen und bevorzugterweise einen durchschnittlichen Durchmesser von etwa 50 bis etwa 500 μm. Die Partikel werden im Allgemeinen von etwa 1 bis etwa 50 Vol.% des Gesamtvolumens der Mischung aus Partikel und Polymer-Lösungsmittel ausmachen (wobei die Gesamtvolumenprozente 100 Volumenprozent eergeben). Die auslaugbaren Materialien können entfernt werden durch Eintauchen des Schaums mit dem auslaugbaren Material in ein Lösungsmittel, in dem das Partikel löslich ist, für eine ausreichende Zeitspanne, um ein Auslaugen von im Wesentlichen allen Partikeln zu erlauben, was aber den Schaum nicht auflöst oder in zerstörerischer Weise ändert. Das bevorzugte Extraktionslösungsmittel ist Wasser, am bevorzugtesten destilliertes dionisiertes Wasser. Das Verfahren ist in US-Patent 5,514,378 beschrieben. Bevorzugterweise wird der Schaum nach dem Auslaugprozess getrocknet werden, wird bei niedriger Temperatur vervollständigt und/oder vakuumgetrocknet werden, um die Hydrolyse des Schaums zu minimieren, sofern keine beschleunigte Absorption des Schaums erwünscht ist.
- Verschiedene Proteine (einschließlich kurzkettiger Peptide), Wachstumsagenzien, chemotaktische Agenzien und therapeutische Agenzien): (Antibiotika, Analgetika, antiinflammatorische Mittel, abstoßungsverhindernde (z. B. Immunsuppressiva und Krebsarzneimittel), oder keramische Partikel können zu dem zusammengesetzten Gerüst
20 während des Verarbeitens hinzugegeben oder auf der Oberfläche adsorbiert oder nach der Herstellung in das Gerüst20 zurückgefüllt werden. Die Poren25 der Keramikphase und/oder die Poren23 des Polymers22 können teilweise oder vollständig mit biokompatiblen resorbierbaren synthetischen Polymeren oder Polymeren (wie beispielsweise Kollagen oder Elastin) oder biokompatiblen Keramikmaterialien (wie beispielsweise Hydroxyapatit) und Kombinationen davon (die Materialien, die das Gewebewachstum fördern, enthalten, oder auch nicht enthalten) aufgefüllt werden. Geeignete Materialien umfassen, sind aber nicht darauf beschränkt, Autoimplantat-, Alloimplantat- oder Xenoimplantatknochen, Knochenmark, morphogene Proteine (BMPs), epidermalen Wachstumsfaktor (EGF), Fibroblasten-Wachstumsfaktor (FGF), von Plättchen abgeleiteten Wachstumsfaktor (PDGF), Insulin-abgeleiteten Wachstumsfaktor (IGF-I und IGF-II), transformierenden Wachstumsfaktoren (TGF-β), Gefäßendothelwachstumsfaktor (VEGF), plättchenreiches Plasma (PRP) und andere osteoinduktive oder osteokonduktive Materialien, die in der Technik bekannt sind. Die Polymerfüllmaterialien könnten auch leitfähige oder chemotaktische Materialien, oder Abgabevehikel für Wachstumsfaktoren sein. Beispiele wären rekombinantes oder von Tieren abgeleitetes Kollagen oder Elastin oder Hyaluronsäure. - Bioaktive Beschichtungen oder Oberflächenbehandlungen könnten auch auf die Oberfläche des Implantates
10 angewandt werden. Beispielsweise könnten bioaktive Peptidsequenzen (RGDs) aufgetragen werden, um die Proteinadsorption und nachfolgende Zellgewebeanheftung zu erleichtern. - Therapeutische Agenzien können auch vermittels des Implantates
10 verabreicht werden. Die Polymere und Mischungen, die verwendet werden, um das Gerüst20 auszubilden, können therapeutische Agenzien enthalten. Beispielsweise würde das Polymer22 mit einem therapeutischen Agens vor dem Ausbilden des zusammengesetzten Gerüstes20 gemischt werden oder in das Gerüst beschickt werden, nachdem dieses ausgebildet ist. Die Vielzahl von verschiedenen therapeutischen Agenzien, die im Zusammenhang mit dem Implantat10 der vorliegenden Erfindung verwendet werden können, ist enorm. Im Allgemeinen umfassen therapeutische Agenzien, die vermittels des Implantates10 verabreicht werden können, ohne Beschränkung: Antiinfektiva wie beispielsweise Antibiotika und antivirale Agenzien, chemotherapeutische Agenzien (d. h. Krebsmittel); abstoßungsverhindernde Agenzien; Analgetika und analgetische Kombinationen; antientzündliche Agenzien; Hormone wie beispielsweise Steroide; Wachstumsfaktoren (knochenmorphogene Proteine (d. h. BMPs 1–7), knochenmorphogenähnliche Proteine (d. h. GFD-5, GFD-7 und GFD-8), epidermaler Wachstumsfaktor (EGF), Fibroblastenwachstumsfaktor (z. B. FGF 1–9), von Plättchen abgeleiteter Wachstumsfaktor (PDGF), insulinähnlicher Wachstumsfaktor (IGF-I und IGF-II), transformierende Wachstumsfaktoren (z. B. TGF-beta I – III), Gefäßendothelwachstumsfaktor (VEGF)), und andere natürlich abgeleitete oder gentechnologisch hergestellte Proteine, Polysaccharide, Glycoproteine oder Lipoproteine. Diese Wachstumsfaktoren sind beschrieben in The Cellular and Molecular Basis of Bone Formation and Repair von Vicki Rosen und R. Scott Thies, herausgegeben von R. G. Landes Company, was hierin durch Bezugnahme aufgenommen wird. - Die zusammengesetzten Gerüste
20 , die bioaktive Materialien enthalten, können formuliert werden durch Mischen von einem oder mehreren therapeutischen Agenzien mit dem Polymer, das für die Herstellung der Polymerphase22 verwendet wird, mit dem Lösungsmittel oder mit der Polymer-Lösungsmittel-Mischung, die dann durch Lyophilisierung geschäumt wird. Alternativ kann ein therapeutisches Agens auf das zusammengesetzte Gerüst20 mit einem pharmazeutisch akzeptablen Träger, der sich nicht in dem Gerüst20 löst, beschichtet werden. Die therapeutischen Agenzien können eine Flüssigkeit, ein fein verteilter Feststoff oder irgendeine andere geeignete physikalische Form sein. Typischerweise, aber optional, wird die Matrix einen oder mehrere Zusätze enthalten, wie beispielsweise Verdünnungsmittel, Träger, Bindemittel, Stabilisatoren oder dergleichen. Die Art des Polymers und die Arzneimittelkonzentration können variiert werden, um das Freisetzungsprofil und die Menge des dispensierten Arzneimittels zu steuern. Nach Kontakt mit Körperflüssigkeiten wird das Arzneimittel freigegeben werden. Wenn das Arzneimittel in das Gerüst20 eingebaut ist, dann wird das Arzneimittel freigesetzt, während es einen graduellen Abbau (überwiegend durch Hydrolyse) erfährt. Dies kann zu einer verlängerten Abgabe (über, sagen wir, 1 – 5000 Stunden, bevorzugterweise 2 – 800 Stunden) von wirksamen Mengen (sagen wir 0,0001 mg/kg/Std. bis 10 mg/kg/Std.) des Arzneimittels führen. - Wie in Vacanti, US Patent 5,770,417 dargelegt, können Zellen von einem Patienten (vor oder während des chirurgischen Eingriffes, um das Gewebe zu reparieren) geerntet und die Zellen unter sterilen Bedingungen verarbeitet werden, um einen spezifischen Zelltyp (d. h. pluripotente Zellen, Stammzellen, Knochenmarkszellen, Vorläuferzellen von menschlichem autologen Fettgewebe (PHAAT)-Zellen oder Vorläuferzellen wie beispielsweise die in Kaplan, US-Patent 5,486,359 beschriebenen mesenchymalen Stammzellen) bereitzustellen. Diese Zellen, z. B. Myozyten, Adipozyten, Fibromyoblasten, ectodermale Zellen, Muskelzellen, Osteoblasten (d. h. Knochenzellen), Chondrozyten (d. h. Knorpelzellen), Endothelzellen, Fibroblasten, Pankreaszellen, Hepatozyten, Gallengangzellen, Knochenmarkszellen, neurale Zellen, Urogenitalzellen, (einschließlich nephritischer Zellen und Kombinationen davon können auf das poröse zusammengesetzte Gerüst
20 aufgetragen oder darin eingesät werden. Autogene, allogene, xenogene Zellen können verwendet werden. Die Zellen können ex vivo kultiviert und dann reimplantiert werden. Gewebe kann aus einem Patienten gewonnen, verarbeitet werden, um bestimmte Zellen und/oder Wachstumsfaktoren auszuwählen, wie beispielsweise PRP (plättchenreiches Plasma), und dann mit dem Implantat zurück in den Patienten implantiert werden. Die implantierten Zellen könnten auch eingefügte DNA enthalten, die ein Protein codiert, das die Anheftung, Proliferation oder Differenzierung von Gewebe codiert. - Zellen können in das Gerüst
20 implantiert werden durch Platzieren des Gerüstes20 in eine Zellkultur, so dass die Zellen in die Mikroporen23 und Makroporen25 eindringen. Das Gerüst20 kann dann in den Patienten implantiert werden. Das in vitro Aussäen von Zellen könnte einen schnelleren Entwicklungs- und Differenzierungsvorgang für das Gewebe bereitstellen. Es ist klar, dass die zelluläre Differenzierung und die Erzeugung von Gewebespezifischer extrazellulärer Matrix für das Gewebe-Rekonstruieren eines funktionalen Implantates entscheidend ist. Es ist bekannt, dass verschiedene Zellarten (Stromazellen und Chondrozyten) auf verschiedenen Strukturen kultiviert werden können. Eine Gradientenstruktur erlaubt auch, dass co-kultivierte Gewebegerüste20 erzeugt werden können. - Eine Verwendung des hierin beschriebenen Konstruktes besteht in der Reparatur und Regeneration von Gelenksknorpel. Gelenksknorpel ist ein Beispiel für eine natürlicherweise auftretende Struktur, die aus vier unterschiedlichen Zonen zusammengesetzt ist, die die oberflächliche oder tangentiale Zone innerhalb der ersten 10 – 20 % der Struktur (dies umfasst die Gelenksoberfläche), die mittlere Zone, die 40 – 60 % der mittleren Struktur ausmacht, die tiefe Zone, die der Tidennarbe benachbart ist, und eine Übergangszone zwischen dem Knochen und dem Knorpel umfaßt, die aus kalzifiziertem Knorpel besteht. Subchondraler Knochen ist benachbart der Tidennarbe angeordnet und dieser geht in spongiösem Knochen über. Wie oben beschrieben erlaubt die vorliegende Erfindung das Herstellen eines Gerüstes, z.B.
20 , mit einer Vielzahl von Schichten, wobei eine jede ihre eigenen Merkmale hinsichtlich Zusammensetzung, Porosität, Stärke etc. aufweist. Entsprechend kann das Gerüst, z. B.20 , als eine Matrix für mehrere unterschiedliche Gewebezonen dienen, wie sie in Gelenkknorpel vorhanden sind. - Die Oberflächenporosität der Polymerphase
22 kann durch verschiedene Verfahren gesteuert werden, einschließlich Bereitstellen einer Form52 dafür mit einer Vielzahl von abstehenden Stiften zum Durchdringen der Oberfläche während des Gießens oder nachfolgendes Durchstechen der Oberfläche mit Nadeln, Laserbehandlung, chemische Behandlung etc., was zur Oberflächenporosität führt, die von undurchdringlich bis porös reicht und dadurch die Fluidpermeabilität bestimmt. Hinsichtlich der Herstellung eines Gerüstes20 zum Herstellen von Gelenksknorpel muss das Gerüst20 drei Zonen aufweisen, nämlich eine poröse Polymerphase20 , die benachbart zum Knorpelgewebe liegt, eine poröse Keramikphase24 , die benachbart dem Knochengewebe liegt, und eine Phasengrenzflächenregion26 . Die Polymerphase22 würde eine obere Oberfläche (Haut) aufweisen, die mit einer Porosität versehen wird von, beispielsweise, 75 – 150 μm, um den Durchtritt von Zellen zu erlauben, um das Einwachsen zu fördern. Für Gelenksknorpel wird die Polymerphase22 und die Keramikphase24 zusammen mit dem Fixierungsbestandteil30 das mechanische Beladen unterstützen müssen und dadurch die eindringenden Zellen schützen, bis sie sich differenziert und in dem Gewebe konsolidiert haben, das in der Lage ist, eine Last zu stützen. Die Polymerphase22 kann eine Porosität von etwa 80 bis etwa 95 % aufweisen mit Poren, die im Bereich von 100 μm sind (etwa 80 μm bis etwa 120 μm). Man erwartet, dass die Chondrozyten in diese Zone eindringen werden. Die Keramikphase24 kann größere Poren aufweisen (etwa 250 μm bis etwa 400 μm) und eine Porosität im Bereich von etwa 50 bis etwa 95 %, die strukturell kompatibel mit spongiösem Knochen ist. Die Phasengrenzflächenregion26 ähnelt dem Strukturübergang zwischen Knorpel und Knochen. Mehrere Patente haben Systeme zum Reparieren von Knorpel vorgeschlagen, die mit porösen Gerüsten der vorliegenden Formel verwendet werden könnten. Beispielsweise beschreibt das US-Patent 5,769,899 eine Vorrichtung zum Reparieren von Knorpeldefekten und das US-Patent 5,713,374 das Sichern von Knorpelreparaturvorrichtungen durch Knochenanker. - Das hierin beschriebene Implantat
10 kann für die Reparatur und Regeneration des Meniskus verwendet werden, weist eine Biokompatibilität auf, Widerstand gegenüber zerbröckeln zum Zeitpunkt des chirurgischen Eingriffes, weist einen ausreichenden Widerstand gegen Zusammendrücken auf, um das Eindringen von Zellen unter Belastung zu erlauben und zeigt eine hohe Porosität. Das Implantat wird leicht sterilisiert, durch eindringendes Gewebe umgeformt und abgebaut während neues Gewebe ausgebildet wird. Darüber hinaus kann der Gerüstbestandteil20 fest an die Verletzungsstelle vermittels des Fixierungsbestandteiles30 befestigt werden. - Das Implantat
10 kann bi-, tri- oder mehrlagige Gerüste20 aufweisen. Diese Schichten können in regelmäßigen oder unregelmäßigen Intervallen räumlich voneinander getrennt sein und die Polymerphasen können mit einer Anzahl von Verstärkungen verstärkt sein, wobei sich das Fixierungsbestandteil30 auf irgendeiner erwünschten Höhe innerhalb des Gerüstes20 befindet. Die Verstärkungen können als Faser, Gebinde oder in partikulärer Form vorliegen und können in unterschiedlichen Höhen, Winkeln, Konformationen oder Gradienten im Schaum angeordnet sein. Sowohl die Polymer-22 als auch die Keramikphase24 können abhängig von der Anwendung unterschiedliche Porositäten und eine Struktur mit offenen Zellen oder geschlossenen Zellen aufweisen. - Das Implantat
10 kann an dem zu reparierenden Gewebe festgemacht sein durch Einführen des Stifts34 in ein in geeigneter Weise dimensioniertes Loch in dem Gewebe, z. B. Knochen. Ein Fixiermittel wie beispielsweise Kalziumphosphat oder Kalziumsulfatzemente, PMMA, Fibrinklebstoff Klebstoffe (z. B. Cyanoacrylate, Butylacrylate, etc.) können ebenfalls verwendet werden, um das Implantat10 zu sichern. - Das folgende Beispiel veranschaulicht die Prinzipien und die Durchführung dieser Erfindung, obwohl sie nicht darauf beschränkt ist. Verschiedene zusätzliche Ausführungsformen innerhalb des Umfangs der Erfindung werden für die Fachleute auf dem Gebiet offensichtlich sein.
- In dem folgenden Beispiel bezeichnen die Abkürzungen PCL polymerisiertes ε-Caprolacton, PGA polymerisiertes Glycolid und PLA polymerisiertes (L)-Lactid. Zusätzlich gibt der Prozentsatz vor dem Copolymer den entsprechenden Molprozentsatz einer jeden Phase an.
- Beispiel 1
- Dieses Beispiel beschreibt die Herstellung eines zusammengesetzten Gerüstes mit einer integral ausgebildeten Fixierungsvorrichtung.
- Eine Lösung des zu einem Schaum zu lyophilisierenden Polymers wurde hergestellt, das aus einem Gewichtsverhältnis von 95/5 1,4-Dioxan zu35/65 PCL/PGA bestand. Das Polymer und das Lösungsmittel wurden in einen Kolben gegeben, der in ein Wasserbad gegeben und auf 70 °C erhitzt wurde. Die Lösung wurde erhitzt und für 5 Stunden gerührt. Danach wurde die Lösung unter Verwendung einer Extraktionshülse (extra grobe Porosität, Art ASTM 170–220 (EC)) filtriert und in dem Kolben gelagert.
- Eine Keramiktablette aus porösem Hydroxyapatit (CERAbio Prescott, WI wurde mit den folgenden Größen hergestellt: Außendurchmesser 7 mm, Innendurchmesser 2 mm und Stärke 2 mm.
- Ein bioabsorbierbarer Fixierungsbestandteil wurde hergestellt unter Verwendung eines Spritzgussverfahrens. Die Konstruktion des verwendeten Fixierungsbestandteiles ist in
EP 1234557 beschrieben, das den Titel „Scaffold Fixation Device for Use in Articular Cartridge Repair" trägt. Das Polymer, das verwendet wird, um die Fixierungsbestandteile herzustellen, war ein Copolymer aus 85 % PLA und 15 % PGA (85/15 PLA/PGA), das von Purac (Gorinchem, Holland) hergestellt wird mit einer I. V. von 1,79 dl/g, wie in Chloroform gemessen. Der Spritzgießer (Niigata NN35MI) wies einen Spritzgehäusedurchmesser von 18 mm auf. Der Bunker wurde mit einer Stickstoffspülung verbunden, um das Polymer trocken zu halten. Die Zufuhr,-Übergangs- und Kompressionszonentemperatur betrugen 185 °C, 185 °C bzw. 191°C. Die Düsen- und Formtemperaturen betrugen 191 °C bzw. 24 °C. Die maximale Injektionsgeschwindigkeit betrug 80 mm/Sek. Unter dem Zylinder Nr. 2 betrug der maximale Injektionsdruck 85 Kg f/cm2. Der Haltedruck betrug 75 Kgf/cm2. Die Gesamtzeit für die Injektion und das Halten betrug 3 Sekunden und die Abkühlzeit am Ende des Haltezyklus betrug 20 Sekunden. - Der Fixierungsbestandteil, der durch den vorstehenden Prozess vorgeschlagen wurde, wurde durch 2 mm Löcher, die in der Keramiktablette zuvor hergestellt waren, gedreht und etwa 1,0 – 1,5 mm oberhalb der Bodenoberfläche einer Form, wie unter Bezugnahme auf
5 beschrieben, aufgehängt. - Die zuvor hergestellte Polymerlösung wurde in die Form gegossen, bis diese gefüllt war. Die Gerüststütze der Fixierungsvorrichtung und die Seite der Keramikscheibe, die die Gerüststütze kontaktierte, wurde in die Polymerlösung mit wenigstens der Hälfte der Stärke der eingetauchten Keramikscheibe eingetaucht.
- Ein Lyophilisator im Labormaßstab (Model Freeze Mobile G von Virtis Company, Gardiner, N.Y.) wurde in diesem Beispiel verwendet. Die Formanordnung wurde auf dem Gerüst des Lyophilisators (oder Gefriertrockners) angeordnet und die Gefriertrocknungssequenz wurde durchgeführt. Die in diesem Beispiel verwendete Gefriertrocknungssequenz war die folgende: 1) 20 °C für 15 Min., 2) –5 °C für 180 Min., 3) –5 °C für 180 Min. unter Vakuum 100 mT, 4) 5 °C für 120 Min. unter Vakuum 100 mT und 5) 20 °C für 120 Min. unter Vakuum 100 mT. Während die Lösung in der Form gefriertrocknete, wich die Polymerphase zurück, was eine Grenzflächenregion mit weniger als 200 μl Polymerschaum zurückließ, der die poröse Keramik infiltrierte. Der Gerüstträger der Fixierungsvorrichtung war in der Polymerphase eingebettet, die fest an der Keramikphase gesichert war.
Claims (25)
- Prosthetisches Implantat (
10 ) umfassend: eine Fixierungseinrichtung (30 ) mit einem Gerüstträger (32 ) und einem verankernden Stift (34 ), wobei sich der verankernde Stift (34 ) von einer Oberfläche des Gerüstträgers (32 ) mit einem ausgewählten Winkel erstreckt, der verankernde Stift (34 ) in einen im Gewebe ausgebildeten Sitz einführbar ist, der Gerüstträger (32 ) innerhalb eines zusammengesetzten Gewebegerüstes (20 ) eingebettet ist, dadurch gekennzeichnet, dass das Gerüst eine Keramikschicht (24 ) mit einer ersten Vielzahl von Poren (27 ) und eine Polymerschicht (22 ) mit einer zweiten Vielzahl von Poren (23 ) umfasst, das Polymer (22 ) an der Keramik (24 ) in einer Phasengrenzflächenregion (26 ) angebracht ist, das Polymer (22 ) wenigstens teilweise in der ersten Vielzahl von Poren (27 ) in der Phasengrenzflächenregion (26 ) versenkt ist, und wenigstens entweder die Polymerschicht (22 ) oder die Keramikschicht (24 ) biologisch abbaubar ist. - Implantat (
10 ) nach Anspruch 1, wobei ein Teil der zweiten Vielzahl von Poren (23 ) wenigstens teilweise mit der ersten Vielzahl von Poren (27 ) in der Phasengrenzflächenregion (26 ) in Verbindung steht. - Implantat (
10 ) nach Anspruch 2, wobei die Keramik (24 ) ein Loch darin aufweist, sich der verankernde Stift (34 ) durch das Loch erstreckt, der Gerüstträger (32 ) gegen die Keramik (24 ) unmittelbar anschließend an das Loch anstößt, der Gerüstträger (32 ) größer als das Loch ist, was den Gerüstträger (32 ) daran hindert, sich durch das Loch zu bewegen, sich die Phasengrenzflächenregion (26 ) einem Umfang des Gerüstträgers (32 ) unmittelbar anschließend erstreckt. - Implantat (
10 ) nach Anspruch 3, wobei die Keramik (24 ) einen versenkten Bereich aufweist, der um das Loch angeordnet ist, wobei der Gerüstträger (32 ) wenigstens teilweise innerhalb des versenkten Bereiches enthalten ist. - Implantat (
10 ) nach Anspruch 4, wobei der Gerüstträger (32 ) wenigstens eine Öffnung aufweist, die sich dadurch hindurch erstreckt, wobei wenigstens eine Öffnung erlaubt, dass sich das Polymer (22 ) dadurch hindurch erstreckt. - Implantat (
10 ) nach Anspruch 1, weiter umfassend eine mechanische Verstärkung, die in dem Polymer (22 ) enthalten ist, wobei die mechanische Verstärkung ausgewählt ist aus der Gruppe, die aus Folien, Mullen, gewobenen Geweben, nicht-gewobenen Geweben, gestrickten Geweben, geflochtenen Geweben und Streben besteht. - Implantat (
10 ) nach Anspruch 1, weiter umfassend Füllstoffe innerhalb des Polymers (22 ), ausgewählt aus der Gruppe bestehend aus Wachstumsfaktoren und therapeutischen Materialien. - Implantat (
10 ) nach Anspruch 1, weiter umfassend lebende Zellen, die auf einer Oberfläche des Gerüstes (32 ) sitzen. - Implantat (
10 ) nach Anspruch 1, wobei die Keramik (24 ) ausgewählt ist aus der Gruppe bestehend aus Hydroxyapatit, Trikalziumphosphat, Tetrakalziumphosphat, Fluoroapatit, Magnesiumkalziumphosphat, Kalziumsulfat, Kalziumfluorid, Kalziumoxid und Kalziumkarbonat. - Implantat (
10 ) nach Anspruch 1, wobei das Polymer (22 ) ausgewählt ist aus der Gruppe bestehend aus Collagen, Elastin, Hyaluronsäure, Chitin und Alginat. - Implantat (
10 ) nach Anspruch 1, wobei das Polymer (22 ) ausgewählt ist aus der Gruppe bestehend aus aliphatischen Polyesterhomopolymeren und Polyestercopolymeren. - Implantat (
10 ) nach Anspruch 11, wobei das Polymer (22 ) ausgewählt ist aus der Gruppe bestehend aus Milchsäure, Lactidmischungen aus L-, D-, meso- und D,L-Lactiden, Glycolsäure, Glycolid, epsilon-Caprolacton, p-Dioxanon (1,4-Dioxan-2-on) und Trimethylencarbonat (1,3-Dioxan-2-on). - Implantat (
10 ) nach Anspruch 1, wobei das Polymer (22 ) ein aliphatisches Polyesterelastomercopolymer ist. - Implantat (
10 ) nach Anspruch 13, wobei das Copolymer ausgebildet ist aus epsilon-Caprolacton und Glycolid in einem Molverhältnis von etwa 35:65 bis etwa 65:35. - Implantat (
10 ) nach Anspruch 13, wobei das Copolymer ausgebildet ist aus epsilon-Caprolacton und Glycolid in einem Molverhältnis von etwa 45:55 bis etwa 35:65. - Implantat (
10 ) nach Anspruch 13, wobei das Copolymer ausgebildet ist aus epsilon-Caprolacton und Lactid, das ausgewählt ist aus der Gruppe umfassend L-Lactid, D-Lactid und Milchsäurecopolymeren, in einem Molverhältnis von epsilon-Caprolacton zu Lactid von etwa 35:65 bis etwa 65:35. - Implantat (
10 ) nach Anspruch 13, wobei das Copolymer ausgebildet ist aus epsilon-Caprolacton und Lactid, das ausgewählt ist aus der Gruppe umfassend L-Lactid, D-Lactid und Milchsäurecopolymeren, in einem Molverhältnis von epsilon-Caprolacton zu Lactid von etwa 45:55 bis etwa 30:70. - Implantat (
10 ) nach Anspruch 13, wobei das Copolymer ausgebildet ist aus epsilon-Caprolacton und Lactid, das ausgewählt ist aus der Gruppe umfassend L-Lactid, D-Lactid und Milchsäurecopolymeren, in einem Molverhältnis von Epsilon-Caprolacton zu Lactid von etwa 95:5 bis etwa 85:15. - Implantat (
10 ) nach Anspruch 1, wobei der Gerüstträger (32 ) eine im allgemeinen flache erste Oberfläche und eine zweite Oberfläche aufweist, die im wesentlichen parallel zur ersten Oberfläche ist, sich der verankernde Stift (34 ) von der zweiten Oberfläche erstreckt, die Keramikschicht (24 ) als nächstes an die Polymerschicht (22 ) anschließt, wobei der Gerüstträger (32 ) dazwischen gefasst ist, die Keramikschicht (24 ) unmittelbar anschließend an die zweite Oberfläche angeordnet ist und die Polymerschicht (22 ) unmittelbar benachbart der ersten Oberfläche angeordnet ist. - Implantat (
10 ) nach Anspruch 19, wobei wenigstens entweder die Keramikschicht (24 ) oder die Polymerschicht (22 ) einen versenkten Bereich darin aufweist, um wenigstens teilweise den Gerüstträger (32 ) aufzunehmen. - Implantat (
10 ) nach Anspruch 19, wobei die Keramikschicht (24 ) und die Polymerschicht (22 ) jeweils einen versenkten Bereich darin aufweisen, um wenigstens teilweise den Gerüstträger (32 ) aufzunehmen. - Verfahren zum Herstellen eines prosthetischen Implantates (
10 ) mit einem Gewebegerüst (20 ) und einer eingebetteten Fixierungseinrichtung (30 ) mit einem Gerüstträger (32 ) und einem verankernden Stift (34 ), umfassend die Schritte: a) Bereitstellen eines porösen Keramikkörpers (24 ) mit einem ersten Satz aus Poren (27 ) und einem Loch, das sich dadurch hindurch erstreckt; b) Bereitstellen einer Polymerlösung (22 ); c) Einführen des Stiftes (34 ) der Fixierungseinrichtung (30 ) durch das Loch in dem Keramikkörper (24 ), so dass der Gerüstträger (32 ) den Keramikkörper (24 ) kontaktiert, um eine erste Unteranordnung auszubilden; d) Kontaktieren des Gerüstträgers (32 ) und des Keramikkörpers (24 ) der ersten Unteranordnung mit der Polymerlösung (22 ); e) Erlauben, dass die Polymerlösung (22 ) wenigstens teilweise in die Poren (27 ) in dem Keramikkörper (24 ) eindringt; f) Aufschäumen der Polymerlösung (22 ), um einen Polymerschaum (22 ) mit einem zweiten Satz von Poren (23 ) zu produzieren, wobei der Polymerschaum (22 ) und der Keramikkörper (24 ) sich gegenseitig durchdringen, wo man der Polymerlösung (22 ) erlaubt, dass sie in den Keramikkörper (24 ) eindringt, und dieser den Gerüstträger (32 ) in dem sich ergebenden Verbundstoff (20 ) einbettet, wobei wenigstens ein Teil des ersten Satzes von Poren (27 ) mit wenigstens einem Teil des zweiten Satzes von Poren (23 ) in Verbindung steht. - Verfahren nach Anspruch 22, wobei der Schritt des Aufschäumens durch Lyophilisierung erfolgt.
- Verfahren nach Anspruch 23, wobei die Polymerlösung (
22 ) in eine Form (52 ) mit einem hohlen Napf gegossen wird und ein Implantatträger den Napf überspannt, und weiter den Schritt umfassend, dass das Implantat (10 ) an dem Implantatträger angehängt wird, so dass die Unteranordnung in der Polymerlösung in einem ausgewählten Ausmaß eingetaucht ist. - Verfahren nach Anspruch 22, Anspruch 23 oder Anspruch 24, wobei das prosthetische Implantat (
10 ) ein solches gemäß einem der Ansprüchen 1 bis 21 ist.
