Description

  • A. Umfeld der Erfindung
  • Die vorliegende Erfindung betrifft eine Vorrichtung zum Schutz des Oberschenkelhalses.
  • B. Beschreibung der herkömmlichen Ausführungsart
  • Während einem operativen Eingriff zum Implantieren eines künstlichen Hüftgelenkes wird in Krankenhäusern üblicherweise zuerst der Oberschenkelhals gesägt, wonach ein Oberschenkelstamm aus Metall in eine intrameduliäre Höhle eingeschoben und diese intrameduliäre Höhle zum Befestigen mit Knochenzement aufgefüllt wird. Ein solcher Aufbau führt jedoch zu einer Veränderung der Verteilung der Last auf den Oberschenkelknochen und hat eine unstabile Befestigung zur Folge. Außerdem können die neu gebildeten Blutgefäße und die schwielige Bildung das Metall der Oberschenkelstange aus Metall nicht effizient abdecken. Dies hat zur Folge, dass die Oberschenkelstange aus Metall nicht mit dem Oberschenkelknochen zum Schutz des Oberschenkelknochens integral verbunden werden kann, so dass der gebrochene Oberschenkelknochen und das abgedeckte Metall wegen der eingeschränkten Tragekraft lose werden. Eine solche Anordnung verursacht dem Patienten heftige Schmerzen, so dass er sich erneut einem unnötigen operativen Eingriff zur Reparatur, Justierung oder Korrektur unterziehen muss. Die oben genannten Nachteile stellten für Fachkräfte im medizinischen Fachbereich sowie für Patienten seit geraumer Zeit ein großes Problem dar.
  • Zur Behebung der oben genannten Nachteile wurde in der R. O. C.-Patentschrift Nr. 2318995 am 19. Mai 1999 eine stammlose Hüftprothese veröffentlicht, die aus einem modularen Kopf, einer Vorrichtung zum Schutz des Oberschenkelhalses und einer mittleren Oberschenkelschraube aufgebaut ist. Dabei ist der modulare Hals hauptsächlich in einer halbrunden Form ausgeführt, während an einem Ende des Querprofils der Außenfläche der Kugel ein konkaves Frustumloch gebildet, die Vorrichtung zum Schutz des Oberschenkelhalses hohl und ein zylindrischer Oberschenkelhals mit einem anatomischen Bund ausgeführt ist. Oben ist der anatomische Bund mit einem Loch versehen, in das der Einsatz der entsprechenden mittleren Oberschenkelschraube eingeführt wird. Diese mittlere Oberschenkelschraube ist als eine Gewindestange ausgeführt, die zum Schutz des Oberschenkelhalses in das Loch oben in der Vorrichtung eingesetzt wird. Die Nagelkappe und die Vorrichtung zum Schutz des Oberschenkelhalses befinden sich auf der gleichen Höhe und sind unbeweglich befestigt. Während dem Vorgang von klinischen Tests wurden zahlreiche übermäßige Punkte festgestellt, wie beispielsweise, dass die Vorrichtung zum Schutz des Oberschenkelhalses in einer hohlen Glockenform ausgeführt ist, wobei deren Form unregelmäßig ist, so dass es sich als ein Zusatzinstrument zum Ausführen eines bestimmten operativen Eingriffes für den unregelmäßigen Oberschenkelhals nicht eignet. Aus diesem Grund muss wegen dem Scheitern des vorherigen operativen Eingriffes am Patienten ein weiterer operativer Eingriff vorgenommen werden. In der US-Patentschrift 5.376.126 wird außerdem eine Vorrichtung zum Ersetzen eines künstlichen azetabularen Gelenks, das aus einem Körper zum Ersetzen des azetabularen Gelenks, einer azetabularen Kappe, einem Befestigungselement, einer oder mehreren Befestigungsschrauben und aus einer oder mehreren Befestigungsschnüren aufgebaut ist, offengelegt. Die Größe des das azetabulare Gelenk ersetzenden Körpers ist so ausgeführt, dass dieser Körper über ein vorverheiltes Teil eines Oberschenkelknochens angepaßt werden kann und am unteren Teil mit mindestens einem Schnurloch sowie mit einem oder mehreren Befestigungslöchern versehen ist. Das Befestigungselement weist ein oder mehrere Befestigungslöcher und Schnurlöcher auf. Mit den Befestigungsschrauben werden der das azetabulare Gelenk ersetzende Körper, der Oberschenkelknochen und das Befestigungselement durch Einrücken in die Befestigungslöcher des das azetabulare Gelenk ersetzenden Körpers und des Befestigungselements sowie in die vorgebohrten Löcher des Oberschenkelknochens miteinander befestigt. Mit den Befestigungsschnüren werden der das azetabulare Gelenk ersetzende Körper, der Oberschenkelknochen und das Befestigungselement stärker befestigt, indem diese Befestigungsschnüre durch die Schnurlöcher des das azetabulare Gelenk ersetzenden Körpers und des Befestigungselements hindurchgeführt werden.
  • ZUSAMMENFASSUNG DER ERFINDUNG
  • Das Hauptziel der vorliegenden Erfindung besteht in der Schaffung einer Schutzvorrichtung in einer regelmäßigen geometrischen Form, welche direkt an einen getrimmten Oberschenkelhals aufgebracht werden kann, um die vorhandenen Nachteile zu vermeiden.
  • Die vorliegende Erfindung ist in Anspruch 1 dargelegt und besteht aus einem Hauptkörper mit einer inwendig hohlen Manschette. Die Manschette weist nach dem Trimmen eine regelmäßige geometrische Form auf und wird an einen Oberschenkelhals in einer regelmäßigen geometrischen Form befestigt; ein Flansch erstreckt sich von einem peripheralen Rand an einem Ende der Manschette nach außen; ein am anderen Ende der Manschette befindliches hervorstehendes Halsteil ist nach innen gerichtet; ein Bolzen am Halsteil ragt durch den Hals des Oberschenkelknochens in der Manschette; der Hauptkörper ist mit mindestens einem Loch versehen, um darin ein am Oberschenkelhals befestigtes Verbindungselement aufzunehmen; und ein modularer Kopf einer Vorrichtung zum Schutz des Oberschenkelhalses ist an der Menschette gekoppelt, und zwar an dem Ende, welches vom Halsteil wegschaut, damit das Hüftgelenk frei bewegt werden kann.
  • Da mit dieser Erfindung eine regelmäßige geometrische Manschette angewendet wird kann der Flansch fest auf die Oberfläche des Oberschenkelknochens befestigt werden.
  • KURZBESCHREIBUNG DER ZEICHNUNGEN

  • DETAILLIERTE BESCHREIBUNG DER BEVORZUGTEN AUSFÜHRUNGSFORMEN
  • Wie in der 1, 2 und 3 gezeigt, ermöglicht die vorliegende Erfindung der Vorrichtung zum Schutz des Oberschenkelhalses nach dem Trimmen sowie nach dem integralen Verbinden mit den neu gebildeten Blutgefäßen und der schwieligen Bildung ein stabiles Sichern und Abdecken in einer geometrischen Form. An der Vorrichtung ist ein Hauptkörper (10) mit einer hohlen Manschette (11) angeordnet; ein Flansch (12) in einer geometrischen Form, der leicht gebogen ist (in dieser Ausführungsform wird ein dreieckiger Flansch (12) verwendet), erstreckt sich von einem peripheralen Rand eines Endes der Manschette (11) nach außen. Ein Halsteil (13) erstreckt sich vom anderen Ende der Manschette (11) nach außen, während ein Bolzen (14) – wie in der 6 gezeigt – in das Halsteil (13) eingeführt ist. An einem Ende des Bolzens (14) befindet sich ein Gewindekopf, während das andere Ende des Bolzens (14) mit einem Gewinde versehen ist. Wie in der 4 gezeigt, weist der Hauptkörper (10) weiter mindestens eine Öffnung (15) auf, in der sich ein Befestigungselement (40) befindet. Als Befestigungselement (40) wird in dieser Ausführungsform ein Knochennagel verwendet.
  • Wie dies in der 4, 5, 6 und 7 gezeigt ist, wird der Oberschenkelhals (20) bei einem operativen Eingriff so getrimmt, um diesen der geometrischen Form der Manschette (11) anzupassen, wonach die Manschette (11) des Hauptkörpers (10) mit dem Oberschenkelhals (20) befestigt wird, wobei dessen Flansch (12) unbeweglich an der Stelle auf die Oberfläche befestigt wird, wo der Oberschenkelknochen (30) und der Oberschenkelhals (20) miteinander verbunden sind; der Bolzen (14) ist durch den getrimmten Oberschenkelhals (20) hindurchgeführt und ist mit der Manschette (11) verbunden, um diesen – wie in der 5 und 6 gezeigt – auf dem Oberschenkelknochen (30) zu befestigen. Daher wird der Hauptkörper (10) am getrimmten Oberschenkelhals (20) sicher befestigt und der Oberschenkelknochen (20) kann dank dem Schutz des Hauptkörpers (10) ebenfalls wieder wachsen. Da die Manschette (11) eine geometrische zylindrische Form aufweist und als ein Operationsinstrument für Spezialzwecke ausgeführt ist, wird mit der vorliegenden Erfindung ein solcher operativer Eingriff ermöglicht.
  • Weiter wird mit dem Befestigungselement (40), das durch die Öffnung (15) hindurchgeführt und am Oberschenkelknochen (30) befestigt ist, die Verbindung zwischen dem Oberschenkelknochen (30) und der Oberschenkeldiaphyse (in den Zeichnungen nicht abgebildet) verstärkt, um einerseits Drehungen zu vermeiden und um andererseits zu verhindern, dass der Flansch (12) des Hauptkörpers (10) lose und abgetrennt werden kann.
  • Durch die Öffnung (15) im Hauptkörper (10) können die neugebildeten Blutgefäße und die schwielige Bildung wachsen und den Hauptkörper (10) bedecken sowie den Oberschenkelhals (20) und den Oberschenkelknochen (30) in ein einziges Teil miteinander verbinden. Außerdem befindet sich am Halsteil (13) ein künstlicher Kugelkopf des Oberschenkelknochens (30), und zwar an dem Ende, welches von der Manschette (11) wegschaut, um ihn mit seinem Innengewinde mit dem Bolzen (14) zu befestigen, damit dank diesem Kugelkopf (50) des Oberschenkelknochens (30) das am Oberschenkelknochen (30) befestigte Hüftgelenk frei und ungehindert bewegt werden kann.
  • Anstelle des oben genannten Befestigungselements kann selbstverständlich eine andere beliebige Komponente als den Knochennagel verwendet werden, während die Manschette (11) in einer anderen beliebigen geometrischen Form anstelle der geometrischen zylindrischen Form ausgeführt sein kann. Der Flansch (12) kann eine beliebige Form aufweisen, die an den Oberschenkelhals (20) angepaßt werden kann. Der Bolzenkörper kann ebenfalls ohne Gewinde ausgeführt sein, während der künstliche Kugelkopf des Oberschenkelknochens (30) mit Bolzen befestigt werden kann. Es ist selbstverständlich, dass die hier beschriebene Ausführungsform lediglich die Prinzipien der vorliegenden Erfindung darstellt und dass von Fachleuten auf diesem Gebiet ein weiter Bereich von Modifizierungen und Abänderungen vorgenommen werden können, ohne dabei vom Umfang der Erfindung wie in den nachstehenden Schutzansprüchen dargelegt abzuweichen.

Claims (5)

  1. Vorrichtung zum Schutz eines Oberschenkelhalses umfassend: einen Hauptkörper (10) mit mindestens einer Öffnung (15) und eine Schraube (14), wobei der Hauptkörper eine hohle Manschette (11) entsprechend einem behandelten Oberschenkelhals (20) aufweist, wobei sich von einem Ende der Manschette (11) ein Flansch (12), der mit dem Oberschenkelknochen (30) verbindbar ist, und von dem anderen Ende der Manschette (11) ein Hals (13) erstreckt, wobei auf der Oberseite des Halses (13) ein Durchgang für die Schraube (14) vorgesehen ist, wobei die Schraube (14) den Oberschenkelhals (20) in der Manschette (11) durchdringend fixiert ist, dadurch gekennzeichnet, dass die Manschette (11) mindestens eine Öffnung (15) und der Flansch (12) mindestens eine Öffnung (15) aufweist, wobei diese Öffnungen (15) derart ausgebildet sind, dass diese den sich neu bildenden Blutgefäßen und der Gallus-Bildung gestatten sich durch diese hindurch zu erstrecken sowie sowohl die Manschette (11) als auch den Flansch (12) zu bedecken, wobei die Öffnung (15) zur Aufnahme von wenigstens einem Verbinder (40) zum Befestigen im Oberschenkelknochen (30) ausgebildet ist, wobei diese Vorrichtung ferner den wenigstens einen Verbinder (40) aufweist, wobei der Verbinder (40) ein Knochennagel ist.
  2. Vorrichtung gemäß Anspruch 1, dadurch gekennzeichnet, dass der Flansch (12) sich auf einer im Wesentlichen gekrümmten Oberfläche befindet, welche den Oberschenkelknochen (30) mit dem Oberschenkelhals (20) verbindet.
  3. Vorrichtung gemäß Anspruch 2, dadurch gekennzeichnet, dass der Flansch (12) auf eine im Wesentlichen dreieckförmigen Oberfläche angeordnet ist, die den Oberschenkelknochen (30) mit dem Oberschenkelhals (20) verbindet.
  4. Vorrichtung gemäß Anspruch 1, dadurch gekennzeichnet, dass die Schraube (14) an einem Ende einen Gewindekopf sowie einen Gewindeschraubenabschnitt trägt.
  5. Vorrichtung gemäß einem der Ansprüche 1 oder 4, dadurch gekennzeichnet, dass der Durchgang in dem Halsabschnitt (13) ein Innengewinde aufweist, wobei die Schraube die Manschette durchdringt und mittels des Gewindekopfes mit einem künstlichen Kugelkopf (50) verbunden ist
